Abstract:Objective To investigate the clinical significance of mild fetal ventriculomegaly by prenatal ultrasonography examination.Methods Prenatal ultrasonography examination was performed on 18200 singleton pregnancy women from 20 weeks gestation.172 women with mild fetal ventriculomegaly(transverse diameter of the atrium of the lateral ventricle measuring between 10 and 15mm) were enrolled in this study.The changes of ventriculomegaly and the associated intracranial and extracranial anomalies were observed regularly every 2 or 4 weeks until delivery.The infants were also followed up.Results The incidence of mild fetal ventriculomegaly was 0.095%(172/18200).Most of them were found between 28-32 weeks(59 cases,34.3%).24 were associated with structural anomalies(13.95%).Isolated mild fetal ventriculomegaly resolved throughout pregnancy in 69 of 139(49.6%),remained stable in 51 of 139(36.7%),progressed in 19 of 139(13.7%).The rate of neurodevelopmental delay was 5.4%(6/111).The rate of structural anomalies,progressing in utero and neurodevelopmental delay of ventriculomegaly group(12-15mm) were higher than those of ventriculomegaly group(10.0-11.9mm)(20.6% vs 10.0%,29.8% vs 6.5% and 12.1% vs 2.6%,respectively) which reached significant difference(P<0.05).Conclusion Most of cases could be diagnosed between 28-32 weeks gestation.Mild fetal ventriculomegaly was often associated with structural anomalies.We recommend to perform follow-up ultrasound examinations and detailed ultrasound examination depending on the gestational age at diagnosis.Isolated mild fetal ventriculomegaly with a transverse atrial size ≥12mm and progression in utero is usually associated with a poor prognosis,which should be observed carefully.
Quantification of the distance of pulmonary valve to pulmonary artery bifurecation of normal fetuses in second and late trimester

Abstract:Objective To quantitatively study the correlation of the distance of pulmonary valve to pulmonary artery bifurcation (DPVB) of normal fetuses in second and late trimester with the fetal biometric parameters using fetal echocardiography.Methods The distances of pulmonary valve to pulmonary artery bifurcation in end-systole (DPVBs) and in end-diastole (DPVBd) were measured in a standard short axis view or right ventricular outflow tract view using fetal echocardiography on 419 normal fetuses.The gestation ages ranged from 20 to 35+ weeks.Fetal non-cardiac biometric parameters including biparietal diameter (BPD),femoral length (FL) and gestation age (GA) based on menstrual age,pulmonary valve annulus diameters (PA) at end-systole were measured.The correlation between DPVBs,DPVBd and non-cardiac biometric parameters were analyzed.Results In normal fetuses,the DPVBs was (1.14±0.24)cm with a range of 0.67 to 1.61 cm,the DPVBd was (0.93±0.20)cm with a range of 0.53 to 1.33 cm.The DPVBs and DPVBd were increased with the growth of GA,and were positively correlated with GA,BPD,FL and PA (r=0.827,0.798,0.793,0.769;0.802,0.764,0.773 and 0.771,respectively,all P<0.001).The linear regression equations between DPVBs,DPVBd and GA,BPD,FL,PA were:Y=0.054×GA-0.369,Y=0.184×BPD-0.179,Y=0.217×FL+0.011,Y=1.602×PA+0.156,Y=0.045×GA-0.330,Y=0.152×BPD-0.157,Y=0.182×FL-0.018,Y=1.380×PA+0.080.Conclusions In normal fetuses,the DPVBs and DPVBd increase with the growth of GA,and have good correlation with GA,BPD,FL and PA,respectively.Normal reference ranges of DPVBs and DPVBd have been provided.These normative data may become a new tool for assessment of fetal heart,especially has potential applications in screening of complex congenital heart defects.

Abstract:Objective:To explore the application value of fetal heart quantification (fetal HQ) in evaluation of fetal cardiac function and morphology in fetuses with left ventricular outflow tract obstruction (LVOTO).Methods:Twenty-five fetuses with LVOTO diagnosed by fetal echocardiography in Sir Run Run Shaw Hospital, Zhejiang University Medical College from April to July 2020 were enrolled. The end-diastolic basal apical length(BAL), transverse length (TL), global spherical index (GSI) of fetal four-chamber view, and the left and right ventricular end-diastolic area, long diameter, 24-segment end-diastolic transverse length (ED) and its Z-scores adjusted by gestational age and 24-segment spherical index(SI) were calculated by using fetal HQ. Subsequently the Z-scores of left and right ventricle fractional shortening (FS) in 24 segments were obtained by gestational age.Results:SI of 24-segments of left ventricles in fetuses with LVOTO were significantly different from that of right ventricles (all P<0.05). There was significant difference between left and right ventricular areas ( P<0.05). There was no significant difference between left and right ventricular lengths ( P>0.05). There were significant differences between the 24-segment Z-scores of left and right ventricular ED(all P<0.05). The ratios of RVED to LVED of 24 segments were analyzed by box diagram. The results showed that the transverse length of right ventricle was significantly higher than that of left ventricle. The highest value was 1.49(1.26-1.86), and the lowest value was 1.40(1.26-1.86), both significantly higher than the normal value of 1.19. There were no significant differences between the 1-19-segment Z-scores of left and right ventricular FS( P>0.05). There were significant differences between the 20-24-segment Z-scores of left and right ventricular FS( P<0.05). Conclusions:Fetal HQ can be used to quantitatively analyze left ventricular shape, size and function of fetuses with LVOTO, which provides a new method for quantitative analysis of fetal heart function.

Abstract:Background: Beyond 18 weeks of gestation an increased size of the fetal lateral ventricles is reported in the majority of fetuses with open spina bifida. At 1st trimester, the definition of ventriculomegaly is based on the ratio between the size of the choroid plexus and that of the ventricular space or of the entire fetal head. However, contrary to what is seen from midtrimester, in most fetuses with open spina bifida at 11-13 weeks the amount of fluid in the ventricular system seems to be reduced rather than increased. Objective: To compare the biometry of the lateral ventricles at 11+0-13+6 weeks between normal fetuses and those with a confirmed open spina bifida. Study design: Retrospective cohort study including all cases of isolated open spina bifida detected at 11+0-13+6 weeks of gestation over a period of 5 years and a group of structurally normal fetuses attending at our Centre over a period of 1 year for the aneuploidies screening as controls. The transventricular axial views of the fetal brain obtained among cases and controls were extracted from the archive for post-hoc measurement of the cerebral ventricles. The ratios between choroid plexus and lateral ventricle lenghts (CPVLr) and areas (CPVAr), the ratio between the sum of both choroid plexus areas and fetal head area (CPHAr) and the ratio between the mean of choroid plexus lengths and occipito-frontal diameter (CPHLr) were calculated for both groups. Measurements obtained from the two groups were compared, and the association of each parameter with open spina bifida was investigated. Results: Ten fetuses with open spina bifida were compared with 358 controls, showing significantly smaller size of the cerebral ventricles measurements as expressed by larger values of CPVAr (0.72 vs 0.49, p<0.001), CPVLr (0.79 vs 0.70, p<0.001), CPHAr (0.33 vs 0.28, p=0.006) and CPHLr (0.60 vs 0.52, p<0.001) compared with controls. CPVAr was found to be the most accurate predictor of OSB showing an area under the curve of 0.88 with a sensitivity of 90% and a specificity of 82%. Conclusions: At 11+0-13+6 weeks' gestation open spina bifida is consistently associated with a reduced amount of fluid in the lateral cerebral ventricles of the fetus, as witnessed by significantly increased CPVLr, CPVAr, CPHAr and CPHLr.

Study about Expansion of Fetal Carum Septum Pellucidum and Carum Vergae

Abstract:Objective To discuss the clinical significance of fetal large cavum septum pellucidum and cavum vergae by prenatal ultrasonography. Methods Prenatal ultrasonography was performed on women at our hospital for prenatal diagnosis during January 2006 to December 2008. The changes of fetal large cavum septum pellucidum and cavum vergae were observed regularly until delivery.The live infants were followed up regularly in one year. Results Totally 58 women with fetal large cavum septum pellucidum and cavum vergae were included in the study,with the incidence of 0.19%.But 7 cases losed in the observating progress. They were diagnosed between 24~37 gestational weeks,with a median of (30±4) weeks. In the pregnancy period,24 cases (24/51,47.0%) of the large cavum septum pellucidum and cavum vergae were decreased,and 19 cases were seen decreased to nomal degree in gestation. There was significant difference between the two groups,which the degree of cavum septum pellucidum and cavum vergae expansion decreased and the other didn’t,also with statistical significance (P<0.05). And between cases which were combined with other abnormalities with large cavum septum pellucidum and cavum vergae and the others not,perinatal outcomes also have significant difference (P<0.05).Conclusion Fetus with large cavum septum pellucidum and cavum vergae occur mainly in the second and third trimesters of pregnancy. When we found that the degree of the expansion degree of fetal large cavum septum pellucidum and cavum vergae decreased,it may be possible good signs of perinatal outcomes. Either it is combined with other intracranial and extracranial structures anomalies can affect the perinatal outcomes. So when the ultrasound examination diagnosis of large cavum septum pellucidum and cavum vergae,we should be close ultrasound follow-up of the size of cavum septum pellucidum and cavum vergae and other intracranial,extracranial structures anomalies.

Clinical significance and prognosis of asymmetric ventricles in fetuses

Abstract:Objective To investigate the clinical significance and prognosis of ultrasound-identified asymmetric ventricles in fetuses,and to provide evidences for clinical consultation and management.Methods From January 2014 to May 2017,142 singleton fetuses were enrolled who were diagnosed with asymmetric ventricles through prenatal ultrasound in Peking University First Hospital and successfully followed up after birth.Asymmetric ventricles included simple lateral ventricular asymmetry [lateral ventricular widths were normal (＜10 mm) but with a difference of ≥ 2 mm between the two lateral ventricles and unilateral ventricular widening (only one lateral ventricle was ≥ 10 mm in width and the other was ＜10 mm).All 142 fetuses were divided into two groups according to whether they had other abnormalities (including abnormal ultrasound soft markers and structural abnormalities) or not.Clinical and imaging data of them were analyzed retrospectively.They were followed up for at least six months after birth.Neurological development regarding language,behavior and motor was evaluated using Denver Developmental Screening Test.Results There were 109 cases without other abnormalities and among them,38 (35％) spontaneously recovered in utero,two (2％) became worse,and 69 (63％) remained unchanged.Thirty-one of the 109 cases received fetal cranial MRI and abnormal brain development was found in only one case.No abnormal findings were reported in karyotyping in 38 cases and viral detection in 31 cases.One hundred and five fetuses were successfully delivered and four were induced including two cases for wider in unilateral lateral ventricle,one of continued widening,and one with abnormal brain development suggested by fetal cranial MRI.Among the 33 cases with other abnormalities,two were spontaneously recovered in utero,one became worse,15 unchanged and 15 were induced.Of the 20 cases underwent chromosomal examination,one unbalanced chromosome translocation was identified.Twelve fetuses underwent cranial MRI among which two were found abnormal (periventricular cyst and brain asymmetry).Sixteen cases were aborted including 15 with major abnormalities (major deformity or absence of extremities) and one with mild abnormality (abnormal ultrasound soft markers with progression in lateral ventricular width ≥15 mm in in utero follow-up).Seventeen cases were born including 16 with mild abnormalities and one with severe abnormality (unilateral renal cystic disease without chromosome abnormality).No significant developmental abnormality in language,behavior and motor was reported during follow-up.Conclusions Asymmetric ventricles have no significant impact on fetal growth and postnatal development.The prognosis of asymmetric ventricles mainly depends on the width of lateral ventricles in utero and concomitant abnormalities.Therefore,close attention should be paid on monitoring and follow-up.
P07.12: Atrial Volume Tracking (AVT): a Novel Echocardiographic Technique to Evaluate Fetal Atrial Volume and Function

Abstract:The objectives of this study were to use atrial volume tracking (AVT) to evaluate fetal atrial volume and function in normal fetuses and that of fetuses with congenital heart diseases. One hundred thirty-two fetuses were normal, 5 case of VSD, 4 cases of TGA, 4 cases of TOF. According to gestational age, normal fetus were divided into three groups: 23 ∼ 28 weeks (group 1), 29 ∼ 34 weeks (group 2) and 35 ∼ 40weeks (group 3). Using AVT, the maximal and minimal volumes of RA (and LA were measured from time-atrial volume curves (TAVCs). For normal fetuses, RAmax ranged from 1.21 ± 0.51 ml (group 1), 2.35 ± 0.86 ml (group 2), to 4.28 ± 1.5 ml (group 3). RAmin ranged 0.8 ± 0.35 ml (group 1), 1.41 ± 0.58 ml (group 2) to 2.54 ± 0.96 ml (group 3). LAmax ranged 0.87 ± 0.41 ml (group 1), 1.56 ± 0.62 ml (group 2) to 2.4 ± 1.35 ml (group 3). LAmin in ranged 0.56 ± 0.41 ml (group1), 0.97 ± 0.39 ml (group 2) to 1.45 ± 0.88 ml (group 3). All above results correlated with gestational ages. The volumes of LA were significantly smaller than that of RA (P < 0.01). RAEF were 37.78 ± 10.74% (group 1), 39.74 ± 9.9% (group2), 39.57 ± 11.56% (group 3) respectively, while LAEF were 36.82 ± 9.0%(group 1), 37.22 ± 8.7%(group 2), 39.8 ± 7.8%(group 3) respectively. The atrial EF remained stable with advancing gestational age and there is no significantly difference between RAEF and LAEF (P = 0.461). In our study, there are 18 fetuses with heart abnormalities, no difference of aerial volumes were noted in fetuses with VSD, TGA and TOF compared with normal fetuses. Bland-Altman analysis shows that there was a strong agreement on the RA and LA volumes data between two observers and single observer. This study establishes reference values for fetal RA and LA volumes during the second and third trimesters of pregnancy. The volumes of RA and LA are linearly correlated with gestational weeks, and the volume of RA is bigger than that of LA. RAEF and LAEF are uncorrelated with gestational weeks, and there's no difference between RVEF and LAEF.
Antenatal Diagnosis and Prognosis of Fetal Lateral Ventriculomegaly in Different Degrees by Ultrasound

Abstract:Objective:To study the value of ultrasound in antenatal diagnostic and prognostic evaluation of fetal lateral ventriculomegaly.Methods:The pregnant women with fetal lateralventriculomegaly undertook ultrasonic examination and were followed up for six months after birth.The degrees of fetal lateral ventriculomegaly were measured and recorded.Fifty-three cases underwent amnio-centesis for chromosome karyotype analysis.Results:There were 116 cases with different degrees of lateral ventriculomegaly,of which,51 cases underwent artificial labor in gestation stage,including 17 cases in mild degree and 34 cases in severe degree.Of the other 65cases without artificial labor,61 cases were in mild group,including 31 cases(50.82%)whose venticulomegaly gradually disappeared,6 cases(9.83%)became more serious,and 24 cases(39.34%)kept existence.The other 4cases without artifcial labor were in severe grade,including one was dead-birth and three died in six months after birth.Twelve(23.53%) of the 53cases underwent chromosome karyotype analysis showed chromosome abnormality.Conclusion:About 50% of simplex mild lateral ventriculomegaly vanished before birth,40%kept existence,and only very few became more serious.The lateral ventriculomegaly would have a poor prognosis when complicates with other deformations or in severe degree,unless the pregnant women undertake artificial labor.
Chromosome karyotype analysis of 147 fetuses with ventriculomegaly

Abstract:Objective To investigate the relationship between fetal lateral ventriculomegaly and abnormal karyotype. Methods The karyotype analysis was performed among 147 fetuses with lateral ventriculomegaly from December 2015 to November 2017 in the First Affiliated Hospital of Fourth Military Medical University, and the relationship between lateral ventriculomegaly and abnormal karyotype was retrospectively analyzed. Results Among the 147 cases of fetal lateral ventriculomegaly revealed by ultrasonic screening, there were 123 cases of mild ventriculomegaly, 14 cases of moderate ventriculomegaly, 10 cases of severe ventriculomegaly, 94 cases of isolated ventriculomegaly, and 53 cases of non isolated ventriculomegaly. In 53 cases of non isolated lateral ventriculomegaly, 20 cases had abnormal cardiovascular system. Karyotype analysis showed that there were 126 normal karyotypes and 21 abnormal karyotypes in 147 cases of fetal lateral ventriculomegaly, including 9 cases of trisomy 21. Conclusion The incidence of chromosomal abnormalities in fetuses with lateral ventriculomegaly is high. The abnormal karyotypes include trisomy 21, trisomy 18, chromosome deletion, and translocation. Therefore, when the ultrasonic screening suggests the broadening of the fetal lateral ventricle, it is necessary to perform karyotype analysis.
Fetal Ventriculomegaly: Pregnancy Outcomes and Follow-Ups in Ten Years

Abstract:The aim of this study is to evaluate the pregnancy outcomes and prognoses for fetuses with ventriculomegaly. Two hundred and forty-one cases of fetuses with ventriculomegaly were included in this study. The subjects were divided into three groups according to their lateral ventricular width: "Mild Ventriculomegaly" (10 ‒ < 12 mm), "Moderate Ventriculomegaly" (12 ‒ < 15 mm) and "Severe Ventriculomegaly" (≥ 15 mm). Pediatric examination records and telephone interviews were conducted to track the outcomes of children until the age of 9 years. Eight-two cases were Isolated Ventriculomegaly (34.0%), while Non-Isolated Ventriculomegaly was found in 159 cases (66.0%). The pregnancy was terminated in 91 cases, and a higher abortion ratio was found in the NIVM (Non-Isolated Ventriculomegaly) group compared with the IVM (Isolated Ventriculomegaly) group. The fetuses were delivered in 150 cases, and four infants suffered deaths with NIVM. Of the surviving fetuses, 7 with IVM and 9 with NIVM showed significant abnormalities. The Mild and Moderate VM groups had more favorable prognoses compared with the Severe VM group. Regarding the outcomes and progression of lateral ventricular width, 1 out of 42 cases in the regressed group and 19 out of 108 cases in the stable group showed significant abnormalities. This study suggests that the degree and the progression of ventricular dilatation are main factors that affect pregnancy outcomes and prognoses.
Cortical Overgrowth in Fetuses with Isolated Ventriculomegaly.

Abstract:Mild cerebral ventricular enlargement is associated with schizophrenia, autism, epilepsy, and attention-deficit/hyperactivity disorder. Fetal ventriculomegaly is the most common central nervous system (CNS) abnormality affecting 1% of fetuses and is associated with cognitive, language, and behavioral impairments in childhood. Neurodevelopmental outcome is partially predictable by the 2-dimensional size of the ventricles in the absence of other abnormalities. We hypothesized that isolated fetal ventriculomegaly is a marker of altered brain development characterized by relative overgrowth and aimed to quantify brain growth using volumetric magnetic resonance imaging (MRI) in fetuses with isolated ventriculomegaly. Fetal brain MRI (1.5 T) was performed in 60 normal fetuses and 65 with isolated ventriculomegaly, across a gestational age range of 22-38 weeks. Volumetric analysis of the ventricles and supratentorial brain structures was performed on 3-dimensional reconstructed datasets. Fetuses with isolated ventriculomegaly had increased brain parenchyma volumes when compared with the control cohort (9.6%, P < 0.0001) with enlargement restricted to the cortical gray matter (17.2%, P = 0.002). The extracerebral cerebrospinal fluid and third and fourth ventricles were also enlarged. White matter, basal ganglia, and thalamic volumes were not significantly different between cohorts. The presence of relative cortical overgrowth in fetuses with ventriculomegaly may represent the neurobiological substrate for cognitive, language, and behavioral deficits in these children.
A Preliminary Study on the Postnatal Neurodevelopment of Fetus with Ventriculomegaly

Abstract:Objective To study the neurological prognosis of neonates with ventriculomegaly and its influencing factors.Method A retrospective study was conducted among neonates with ventriculomegaly in Peking University First Hospital from January 2013 to December 2015.A series of cranial ultrasonography were performed after birth and the Gesell development scale was conpleted after six months.x2 test and two-independent-sample t test were used for statistical analysis.Result Among 103 cases of ventriculomegaly,95 cases (92.2％) had mildly enlarged lateral ventricles and 8 cases (7.8％) significantly enlarged.83 cases received serial cranial ultrasound examinations after birth.The lateral ventricles of 9 patients (10.8％) bacame wider and 74 (89.2％) not.The Gesell development scales were completed in 65 cases 6 months after birth.Among them,8 patients with widening lateral ventricles got poor prognosis (100％).Among 57 patients without progressively widening lateral ventricles,6 (10.5％) had poor prognosis.The difference was statistically significant (P ＜ 0.001).No correlation was found between the severity of the lateral ventricle widening and the neurological outcome (P =1.000).There were 2 cases with other abnormalities,and 1 case got poor prognosis on follow-up.Conclusion Most neonatal ventriculomegaly patients have mild and isolated lateral ventricle enlargement.Most of them remain stable or gradually return to normal.The patients with progressively widening lateral ventricles are likely to have adverse neurological prognosis.

Abstract:Background Tools in image reconstruction, motion correction, and segmentation have enabled the accurate volumetric characterization of fetal brain growth at MRI. Purpose To evaluate the volumetric growth of intracranial structures in healthy fetuses, accounting for gestational age (GA), sex, and laterality with use of a spatiotemporal MRI atlas of fetal brain development. Materials and Methods T2-weighted 3.0-T half-Fourier acquired single-shot turbo spin-echo sequence MRI was performed in healthy fetuses from prospectively recruited pregnant volunteers from March 2013 to May 2019. A previously validated section-to-volume reconstruction algorithm was used to generate intensity-normalized superresolution three-dimensional volumes that were registered to a fetal brain MRI atlas with 28 anatomic regions of interest. Atlas-based segmentation was performed and manually refined. Labels included the bilateral hippocampus, amygdala, caudate nucleus, lentiform nucleus, thalamus, lateral ventricle, cerebellum, cortical plate, hemispheric white matter, internal capsule, ganglionic eminence, ventricular zone, corpus callosum, brainstem, hippocampal commissure, and extra-axial cerebrospinal fluid. For fetuses younger than 31 weeks of GA, the subplate and intermediate zones were delineated. A linear regression analysis was used to determine weekly age-related change adjusted for sex and laterality. Results The final analytic sample consisted of 122 MRI scans in 98 fetuses (mean GA, 29 weeks ± 5 [range, 20-38 weeks]). All structures had significant volume growth with increasing GA (P < .001). Weekly age-related change for individual structures in the brain parenchyma ranged from 2.0% (95% CI: 0.9, 3.1; P < .001) in the hippocampal commissure to 19.4% (95% CI: 18.7, 20.1; P < .001) in the cerebellum. The largest sex-related differences were 22.1% higher volume in male fetuses for the lateral ventricles (95% CI: 10.9, 34.4; P < .001). There was rightward volumetric asymmetry of 15.6% for the hippocampus (95% CI: 14.2, 17.2; P < .001) and leftward volumetric asymmetry of 8.1% for the lateral ventricles (95% CI: 3.7, 12.2; P < .001). Conclusion With use of a spatiotemporal MRI atlas, volumetric growth of the fetal brain showed complex trajectories dependent on structure, gestational age, sex, and laterality. © RSNA, 2021 Online supplemental material is available for this article.
