Two patients with Knobloch syndrome due to mutation in COL8A1 gene: case report and review of the literature

Abstract:Knobloch syndrome (KNO, OMIM # 267,750) is a rare ciliopathy group sydrome characterized by a collagen synthesis disorder. It represents an uncommon cause of pediatric retinal detachment. This report presents two cases with different COL18A1 gene mutations, complicated by retinal detachment.

What problem does this paper attempt to address?
The problem that this paper attempts to solve is to describe and analyze two cases of Knobloch syndrome caused by different COL18A1 gene mutations, and to explore their clinical manifestations, genetic backgrounds and treatment strategies. Knobloch syndrome is a rare hereditary disease, and its main features include high myopia, retinal detachment and occipital area abnormalities. Through a detailed report of the clinical features, imaging examination results and gene test results of two patients, this study aims to improve the understanding of this disease and provide references for clinical diagnosis and treatment. Specifically, the first patient mentioned in the paper is a 6 - month - old baby girl, showing bilateral congenital retinal detachment, posterior embryotoxon and other ocular abnormalities. The second patient is a 5 - year - old boy, showing unilateral congenital retinal detachment, neuromotor development retardation, etc. Different mutations were found in the COL18A1 gene in both patients, which indicates the genetic heterogeneity of Knobloch syndrome. The paper also discusses the possible mechanisms of retinal detachment and the importance of early surgical intervention for improving the restoration of anatomical structures.
