Abstract:Objective To study the impacts of different tube voltage and different noise index(NI) guided automatic tube current modulation on the image quality and radiation dose in cerebrovascular imaging and determine the optimal scanning condition. Methods PH3 angiographic CT head phantom was used for head CTA examination. Scanning protocols: all the scanning objectives were divided into three groups according to the different tube voltages of 80, 100 and 120 kV. Each group applied certain tube current(300,400 mA)and automatic tube current modulation technique with NI from 3 to 10 to perform head CTA. There were 30 scanning proposals with different parameter combinations of tube voltage and tube current. The radiation dose [ CT dose index volume (CTDIvol), dose length product (DLP) and effective dose (ED)], objective indicators of images(CT value of the blood vessels and its noise, CT value of brain tissue and its noise, signal-to-noise ratio , contrast-to-noise ratio) and the subjective scores of the five cerebrovascular segments were recorded. Differences of CT value of the blood vessels and its noise, CT value of brain tissue and its noise, signal-to-noise ratio and contrast-to-noise ratio between different tube voltages and tube currents were analyzed by One-Way ANOVA. Results When the tube voltage was certain, the CTDIvol, DLP and ED were all dropped while NI was increased from 3 to 10. Compared with group(120 kV, 300 mA), CTDIvol of group with 100 kV, 300 mA decreased 35.32%(12.22/34.59), CTDIvol of group(100 kV, NI=6) decreased 46.72%(16.16/34.59). Compared with group(100 kV, 300 mA), CTDIvol of group (100 kV, NI=6) decreased 17.61%(3.94/22.37). When the tube voltage was certain and the tube current and NI were not certain, there is no statistical difference (P>0.05) between CT values of blood vessel and brain, while blood vessel noise, noise of brain, SNR and CNR showed statistical difference (P<0.05). When tube current and NI were certain while tube voltage was varied, all objective indicators discussed above all exhibited statistical difference (P<0.05). SNR and CNR of group(100 kV, NI=6) were higher than group(120 kV, 300 mA) with 6.31%(2.69/42.66)and 7.18%(2.64/36.78), respectively. The tube voltage, NI and tube current had no effect on the subjective scores of first and second grade vessel but greater impact on the fourth and fifth grade vessel. Conclusion In the head CTA scanning, combined the use of NI 6 guided automatic adjustment tube current and low tube voltage(100 kV)technique not only can get better image quality but also significantly decreased the radiation dose.

A multi-modality medical imaging head and neck phantom: Part 2. Medical imaging

Abstract:The head and neck phantom discussed in an accompanying paper (part 1), is imaged with MRI, X-ray CT, PET and ultrasound. MRI scans show a distinct image contrast between the brain compartment and other anatomical regions of the head. The silicone matrix that was used to create a porous brain compartment has a relatively high proton density and a spin-spin relaxation time (T2) that is long enough to provide an MRI signal. While the longitudinal magnetization was found to recover according to a mono-exponential, a bi-exponential decay was observed for the transverse relaxation with a slow T2 relaxation component corresponding to the perfusate and a fast T2 relaxation component corresponding to the silicone. The fraction of the slow T2 relaxation component increases upon perfusion. A dynamic contrast enhanced (DCE) MRI experiment is conducted in which the injection rate of the contrast agent is varied. Parametric DCE maps are created and reveal regional differences in contrast agent kinetics as a result of differences in porosity. The skull, vertebra and the brain compartment are clearly visible on X-ray CT. Dynamic PET scanning has been performed while the carotic arterial input function is monitored by use of a Geiger-Müller counter. Similar regions of perfusion are found in the PET study as in the DCE MRI study. By doping the perfusate with a lipid micelle emulsion, the phantom is applicable for carotic Doppler ultrasound demonstration and validation.
Validation of CT brain perfusion methods using a realistic dynamic head phantom.

Abstract:PURPOSE Development and evaluation of a realistic hybrid head phantom for the validation of quantitative CT brain perfusion methods. METHODS A combination, or hybrid, of CT images of an anthropomorphic head phantom together with clinically acquired MRI brain images was used to construct a dynamic hybrid head phantom. Essential CT imaging parameters such as spatially dependent noise, effects of resolution, tube settings, and reconstruction parameters were intrinsically included by scanning a skull phantom using CT perfusion (CTP) protocols with varying mAs. These data were combined with processed high resolution 7T clinical MRI images to include healthy and diseased brain parenchyma, as well as the cerebral vascular system. Time attenuation curves emulating contrast bolus passage based on perfusion as observed in clinical studies were added. Using the phantom, CTP images were generated using three brain perfusion calculation methods: bcSVD, sSVD, and fit-based deconvolution, and the linearity and accuracy of the three calculation methods was assessed. Dependency of perfusion outcome on calculation method was compared to clinical data. Furthermore, the potential of the phantom to optimize brain perfusion packages was investigated. RESULTS All perfusion calculation methods showed overestimation of low perfusion values and underestimation of high perfusion values. Good correlation in behavior between phantom and clinical data was found (R2 = 0.84). CONCLUSIONS A dynamic hybrid head phantom constructed from CT and MRI data was demonstrated to realistically represent clinical CTP studies, which is useful for assessing CT brain perfusion acquisition, reconstruction, and analysis.

Advancing preclinical micro-photon counting CT perfusion imaging: from phantom experiments to in vivo applications

Abstract:In preclinical studies, micro-CT is frequently employed to yield valuable anatomical insights. However, there has been an increasing need for micro-CT in extracting functional measurements such as with perfusion imaging. Perfusion imaging plays a crucial role in understanding and quantifying tissue vascular properties. This paper focuses on our development of preclinical micro-photon counting (PC)CT perfusion imaging and demonstrates quantification of perfusion metrics in a controlled fluid flow phantom experiment. For this study, we utilized a novel bioprinted perfusion phantom and a dedicated preclinical photon-counting CT (PCCT) system to estimate perfusion maps at different flow rates. A continuous water flow through the phantom was maintained by a peristaltic pump. PCCT imaging was performed during a delayed contrast injection of clinical iodinated contrast agent. Imaging was repeated under 3 different flow rates: 4, 6, and 8 mL/min. Our results demonstrate successful visualization and quantification of flow parameters by employing gamma variate curves fit to voxel measurements of temporal PCCT reconstructions as well as decomposed iodine material maps, enabling the calculation of volumetric maps for mean transit time, blood volume index, and blood flow index. Furthermore, we showcase the application of this technique in quantifying in vivo perfusion characteristics in a head and neck cancer model in a mouse. Through our research, we aim to highlight the potential of preclinical micro-PCCT perfusion imaging in advancing our understanding of tissue perfusion dynamics and its potential applications in studying various pathologies and cardiovascular conditions.

Image quality of lung perfusion with photon-counting-detector CT: comparison with dual-source, dual-energy CT

Abstract:Purpose: To evaluate the quality of lung perfusion imaging obtained with photon-counting-detector CT (PCD-CT) in comparison with dual-source, dual-energy CT (DECT). Methods: Seventy-one consecutive patients scanned with PCD-CT were compared to a paired population scanned with dual-energy on a 3rd-generation DS-CT scanner using (a) for DS-CT (Group 1): collimation: 64 × 0.6 × 2 mm; pitch: 0.55; (b) for PCD-CT (Group 2): collimation: 144 × 0.4 mm; pitch: 1.5; single-source acquisition. The injection protocol was similar in both groups with the reconstruction of perfusion images by subtraction of high- and low-energy virtual monoenergetic images. Results: Compared to Group 1, Group 2 examinations showed: (a) a shorter duration of data acquisition (0.93 ± 0.1 s vs 3.98 ± 0.35 s; p < 0.0001); (b) a significantly lower dose-length-product (172.6 ± 55.14 vs 339.4 ± 75.64 mGy·cm; p < 0.0001); and (c) a higher level of objective noise (p < 0.0001) on mediastinal images. On perfusion images: (a) the mean level of attenuation did not differ (p = 0.05) with less subjective image noise in Group 2 (p = 0.049); (b) the distribution of scores of fissure visualization differed between the 2 groups (p < 0.0001) with a higher proportion of fissures sharply delineated in Group 2 (n = 60; 84.5% vs n = 26; 26.6%); (c) the rating of cardiac motion artifacts differed between the 2 groups (p < 0.0001) with a predominance of examinations rated with mild artifacts in Group 2 (n = 69; 97.2%) while the most Group 1 examinations showed moderate artifacts (n = 52; 73.2%). Conclusion: PCD-CT acquisitions provided similar morphologic image quality and superior perfusion imaging at lower radiation doses. Clinical relevance statement: The improvement in the overall quality of perfusion images at lower radiation doses opens the door for wider applications of lung perfusion imaging in clinical practice. Key points: The speed of data acquisition with PCD-CT accounts for mild motion artifacts. Sharply delineated fissures are depicted on PCD-CT perfusion images. High-quality perfusion imaging was obtained with a 52% dose reduction.

Abstract:PURPOSE Optimizing CT brain perfusion protocols is a challenge because of the complex interaction between image acquisition, calculation of perfusion data, and patient hemodynamics. Several digital phantoms have been developed to avoid unnecessary patient exposure or suboptimum choice of parameters. The authors expand this idea by using realistic noise patterns and measured tissue attenuation curves representing patient-specific hemodynamics. The purpose of this work is to validate that this approach can realistically simulate mean perfusion values and noise on perfusion data for individual patients. METHODS The proposed 4D digital phantom consists of three major components: (1) a definition of the spatial structure of various brain tissues within the phantom, (2) measured tissue attenuation curves, and (3) measured noise patterns. Tissue attenuation curves were measured in patient data using regions of interest in gray matter and white matter. By assigning the tissue attenuation curves to the corresponding tissue curves within the phantom, patient-specific CTP acquisitions were retrospectively simulated. Noise patterns were acquired by repeatedly scanning an anthropomorphic skull phantom at various exposure settings. The authors selected 20 consecutive patients that were scanned for suspected ischemic stroke and constructed patient-specific 4D digital phantoms using the individual patients' hemodynamics. The perfusion maps of the patient data were compared with the digital phantom data. Agreement between phantom- and patient-derived data was determined for mean perfusion values and for standard deviation in de perfusion data using intraclass correlation coefficients (ICCs) and a linear fit. RESULTS ICCs ranged between 0.92 and 0.99 for mean perfusion values. ICCs for the standard deviation in perfusion maps were between 0.86 and 0.93. Linear fitting yielded slope values between 0.90 and 1.06. CONCLUSIONS A patient-specific 4D digital phantom allows for realistic simulation of mean values and standard deviation in perfusion data and makes it possible to retrospectively study how the interaction of patient hemodynamics and scan parameters affects CT perfusion values.

Abstract:While drastic improvements in CT technology have occurred in the past 25 years, spatial resolution is one area where progress has been limited until recently. New photon counting CT systems, are capable of much better spatial resolution than their (energy integrating) predecessors. These improvements have the potential to improve the evaluation obstructive coronary artery disease by enabling more accurate delineation between calcified plaque and coronary vessel lumen. A new set of vessel phantoms has been designed and manufactured for quantifying this improvement. Comparisons are made between an existing clinical CT system, a prototype photon counting system, with images from a micro CT system being used as the gold standard. Scans were made of the same objects on all three systems. The resulting images were registered and the luminal cross section areas were compared. Luminal cross-sections near calcified plaques were reduced due to blooming, but this effect was much less pronounced in images from the prototype photon counting system as compared to the images from the clinical CT system.

Abstract:Abstract Background We assessed and compared image quality obtained with clinical 18 F-FDG whole-body oncologic PET protocols used in three different, state-of-the-art digital PET/CT and two conventional PMT-based PET/CT devices. Our goal was to evaluate an improved trade-off between administered activity (patient dose exposure/signal-to-noise ratio) and acquisition time (patient comfort) while preserving diagnostic information achievable with the recently introduced digital detector technology compared to previous analogue PET technology. Methods We performed list-mode (LM) PET acquisitions using a NEMA/IEC NU2 phantom, with activity concentrations of 5 kBq/mL and 25 kBq/mL for the background (9.5 L) and sphere inserts, respectively. For each device, reconstructions were obtained varying the image statistics (10, 30, 60, 90, 120, 180, and 300 s from LM data) and the number of iterations (range 1 to 10) in addition to the employed local clinical protocol setup. We measured for each reconstructed dataset: the quantitative cross-calibration, the image noise on the uniform background assessed by the coefficient of variation (COV), and the recovery coefficients (RCs) evaluated in the hot spheres. Additionally, we compared the characteristic time-activity-product (TAP) that is the product of scan time per bed position × mass-activity administered (in min·MBq/kg) across datasets. Results Good system cross-calibration was obtained for all tested datasets with < 6% deviation from the expected value was observed. For all clinical protocol settings, image noise was compatible with clinical interpretation (COV < 15%). Digital PET showed an improved background signal-to-noise ratio as compared to conventional PMT-based PET. RCs were comparable between digital and PMT-based PET datasets. Compared to PMT-based PET, digital systems provided comparable image quality with lower TAP (from ~ 40% less and up to 70% less). Conclusions This study compared the achievable clinical image quality in three state-of-the-art digital PET/CT devices (from different vendors) as well as in two conventional PMT-based PET. Reported results show that a comparable image quality is achievable with a TAP reduction of ~ 40% in digital PET. This could lead to a significant reduction of the administered mass-activity and/or scan time with direct benefits in terms of dose exposure and patient comfort.

Abstract:Purpose: Existing CT perfusion (CTP) techniques are limited in accurate assessment of stroke. The purpose of this study is to validate a novel low-dose whole-brain first-pass analysis (FPA) technique as compared with gold standard microsphere measurement in a swine model. Materials and Methods: Contrast-enhanced CTP of 5 Yorkshire swine (55 ± 24 kg) was performed over 30 seconds at baseline and following incremental brain embolization. For each acquisition, gold standard fluorescent microspheres, IV contrast, and saline were injected (5 ml/s), followed by dynamic CTP imaging in a 320-slice CT scanner. Scan parameters were 320 x 0.5 mm collimation, 100 kVp, 200 mA, and 350 ms gantry rotation time. On average, 47 contrast-enhanced volume scans were acquired per acquisition to capture the entire time attenuation curve (TAC) of the carotid artery. For each acquisition, only two retrospectively selected volume scans were used to quantify brain perfusion with the low-dose FPA technique. Specifically, the first volume scan was selected at the base, while the second volume scan was selected at the peak of the TAC. Regional low-dose FPA CTP measurements from different lobes were then quantitatively compared to gold standard microsphere perfusion of corresponding tissue samples by using linear regression and Bland-Altman analysis. The CT dose index (CTDI) and dose-length product (DLP) of two-volume FPA technique were also determined. Results: Low-dose FPA perfusion measurements (P FPA ) were related to gold standard microsphere perfusion measurements (P MICRO ) by P FPA = 1.14 P MICRO - 0.01 (r = 0.93, p < 0.01). The CTDI and DLP per two-volume FPA CTP were estimated to be 25.6 mGy and 409.6 mGy, respectively. Conclusion: Low dose FPA CTP technique was retrospectively validated in a swine model and has the potential to be used for accurate diagnostic assessment and follow up of patients with stroke and vasospasm.

Abstract:The use of accurate system response modeling has been proven to be an essential key of SPECT image reconstruction, with its usage leading to overall improvement of image quality. The aim of this work was to investigate the imaging performance using an XCAT brain perfusion phantom of two modeling strategies, one based on analytic techniques and the other one based on GATE Monte-Carlo simulation. In addition, an efficient forced detection approach to improve the overall simulation efficiency was implemented and its performance was evaluated. We demonstrated that accurate modeling of the system matrix generated by Monte-Carlo simulation for iterative reconstruction leads to superior performance compared to analytic modeling in the case of clinical 123I brain imaging. It was also shown that the use of the forced detection approach provided a quantitative and qualitative enhancement of the reconstruction.

Abstract:Purpose: Cerebral CT perfusion (CTP) summary maps classify the ischemic core, penumbra, and normal tissue from traditional parametric maps, which is a criterion for indicating thrombectomy. Since perfusion maps change when the CTP radiation dose is reduced, summary maps also might change. This study aimed to assess the noise characteristics of a summary map in simulation experiments. Methods: We added various amounts of noise to a digital phantom and clinical CTPs, used Vitrea (Canon Medical Systems, Tochigi, Japan) to perform perfusion analysis, and assessed the relationship between the noise and cerebral blood volume (CBV), time to maximum (Tmax), and ischemic core and penumbra volumes. Results: As the noise increased, the obtained CBV increased, the obtained Tmax shortened, and the obtained ischemic core and penumbra volumes decreased, which depended on the tissue's CBV and Tmax. Conclusion: Under low-dose conditions, the ischemic core and penumbra volumes decreased, so the criteria for thrombectomy may differ from those for standard doses.

Abstract:Purpose: To develop a software package Virtual examination of brain perfusion by the method of SPECT/CT with 99mTc-HMPAO (Teoxime) and its practical application to study the conditions for achieving the best image quality in clinical studies of patients. Material and methods: The studies were performed using clinical data and the method of computer simulation. Clinical data of single-photon emission computed tomography combined with X-ray computed tomography (SPECT/CT) with 99mTc-hexamethylpropyleneamine oxime (99mTc-Teoxime, produced by DIAMED LLC) of a patient with an ischemic stroke of the right frontal cortex were obtained on a two-detector gamma-camera NM/CT 670 DR GE Discovery (USA) using high-resolution low-energy collimators (LEHR). The measured data were processed using specialized software Q.Brain and Q.Volumetrix MI on a Xeleris 4.0 DR workstation from GE Healthcare (USA) to obtain reconstructed axial tomographic slices. To carry out simulation computer simulation of the procedure of examination of perfusion of GM by the method of SPECT/CT has developed a software package that includes a mathematical Hoffman phantom with the ability to simulate clinical cases of hypoperfusion of different localization and size (Virtual Patient), modeling the collection of “raw” projection data and an image reconstruction program based on the OSEM algorithm (Ordered Subset Expectation Maximization). An important advantage of the mathematical modeling method is the ability to assess the quality of the reconstructed image by calculating the root-mean-square error when compared with a given phantom. In numerical experiments, the dependence of the reconstruction error on the parameters of the OSEM algorithm (on the number of subgroups – subsets, and on the number of iterations) was investigated in order to determine the conditions for achieving the best image quality. A statistical stop criterion was developed and tested. Results: For the first time, a software package was developed and tested that allows us to investigate errors in the reconstruction algorithm, which is a great difficulty when using clinical research methods. A criterion for stopping iterations is proposed when using the OSEM reconstruction algorithm – minimizing the functional deviation of the chi-square function from the target value, while the detector pixels with non-zero values are combined into blocks according to the 2×2 scheme. There is a reliable good correlation between the proposed stop criterion and the minimum of the root-mean-square error of image reconstruction. This makes it possible to introduce this criterion into the clinical practice of using computational tools for reconstructing sections of the SPECT to obtain the best image. The simulation results demonstrated the possibility of reducing the time of data recording, during which the patient must remain motionless, at least twice. Conclusion: The method of computer simulation developed in this work is a practically useful technology that helps optimize the use of SPECT to achieve the best possible results of brain imaging in patients.
Image quality, radiation dose and diagnostic accuracy of 70 kVp whole brain volumetric CT perfusion imaging: a preliminary study

Abstract:Purpose To evaluate image quality and diagnostic accuracy for acute infarct detection and radiation dose of 70 kVp whole brain CT perfusion (CTP) and CT angiography (CTA) reconstructed from CTP source data. Methods Patients were divided into three groups ( n = 50 each): group A, 80 kVp, 21 scanning time points; groups B, 70 kVp, 21 scanning time points; group C, 70 kVp, 17 scanning time points. Objective and subjective image quality of CTP and CTA were compared. Diagnostic accuracy for detecting acute infarct and cerebral artery stenosis ≥ 50 % was calculated for CTP and CTA with diffusion weighted imaging and digital subtraction angiography as reference standards. Effective radiation dose was compared. Results There were no differences in any perfusion parameter value between three groups ( P > 0.05). No difference was found in subjective image quality between three groups ( P > 0.05). Diagnostic accuracy for detecting acute infarct and vascular stenosis showed no difference between three groups ( P > 0.05). Compared with group A, radiation doses of groups B and C were decreased by 28 % and 37 % (both P < 0.001), respectively. Conclusion Compared with 80 kVp protocol, 70 kVp brain CTP allows comparable vascular and perfusion assessment and lower radiation dose while maintaining high diagnostic accuracy in detecting acute infarct. Abstract:BACKGROUND AND PURPOSE: Parenchymal blood volume measurement by C-arm CT facilitates in-room peritherapeutic perfusion evaluation. However, the radiation dose remains a major concern. This study aimed to compare the radiation dose of parenchymal blood volume measurement using C-arm CT with that of conventional CTP using multidetector CT.MATERIALS AND METHODS: A biplane DSA equipped with C-arm CT and a Rando-Alderson phantom were used. Slab parenchymal blood volume (8-cm scanning range in a craniocaudal direction) and whole-brain parenchymal blood volume with identical scanning parameters, except for scanning ranges, were undertaken on DSA. Eighty thermoluminescent dosimeters were embedded into 22 organ sites of the phantom. We followed the guidelines of the International Commission on Radiation Protection number 103 to calculate the effective doses. For comparison, 8-cm CTP with the same phantom and thermoluminescent dosimeter distribution was performed on a multidetector CT. Two repeat dose experiments with the same scanning parameters and phantom and thermoluminescent dosimeter settings were conducted.RESULTS: Brain-equivalent dose in slab parenchymal blood volume, whole-brain parenchymal blood volume, and CTP were 52.29 ± 35.31, 107.51 ± 31.20, and 163.55 ± 89.45 mSv, respectively. Variations in the measurement of an equivalent dose for the lens were highest in slab parenchymal blood volume (64.5%), followed by CTP (54.6%) and whole-brain parenchymal blood volume (29.0%). The effective doses of slab parenchymal blood volume, whole-brain parenchymal blood volume, and CTP were 0.87 ± 0.55, 3.91 ± 0.78, and 2.77 ± 1.59 mSv, respectively.CONCLUSIONS: The dose measurement conducted in the current study was reliable and reproducible. The effective dose of slab parenchymal blood volume is about one-third that of CTP. With the advantages of on-site and immediate imaging availability and saving procedural time and patient transportation, slab parenchymal blood volume measurement using C-arm CT can be recommended for clinical application.

Abstract:BACKGROUND:Computed tomography perfusion imaging is commonly used for the rapid assessment of patients presenting with symptoms of acute stroke. Maps of perfusion parameters, such as cerebral blood volume (CBV), cerebral blood flow (CBF), and mean transit time (MTT) derived from the perfusion scan data, provide crucial information for stroke diagnosis and treatment decisions. Most CT scanners use singular value decomposition (SVD)-based methods to calculate these parameters. However, some known problems are associated with conventional methods.METHODS:In this work, we propose a Bayesian inference algorithm, which can derive both the perfusion parameters and their uncertainties. We apply the variational technique to the inference, which then becomes an expectation-maximization problem. The probability distribution (with Gaussian mean and variance) of each estimated parameter can be obtained, and the coefficient of variation is used to indicate the uncertainty. We perform evaluations using both simulations and patient studies.RESULTS:In a simulation, we show that the proposed method has much less bias than conventional methods. Then, in separate simulations, we apply the proposed method to evaluate the impacts of various scan conditions, i.e., with different frame intervals, truncated measurement, or motion, on the parameter estimate. In one patient study, the method produced CBF and MTT maps indicating an ischemic lesion consistent with the radiologist's report. In a second patient study affected by patient movement, we showed the feasibility of applying the proposed method to motion corrected data.CONCLUSIONS:The proposed method can be used to evaluate confidence in parameter estimation and the scan protocol design. More clinical evaluation is required to fully test the proposed method.
Multislice CT Brain Perfusion Imaging: Improved Techniques and Early Clinic Experiences

Abstract:Objective To explore the improved techniques of MSCT brain perfusion imaging and its clinical value. Methods Using self control study method,two healthy hybrid dogs were underwent routine and reformation 4-slice CT brain perfusion imaging.The delayed time and scanning time of routine and improved technique were 5s,50s and 8s,40s respectively. Six of 14 patients with acute ischemic stroke were underwent improved brain CT perfusion, and two also brain CT angiography. The mapping and value of cerebral blood flow (BF), cerebral blood volume (BV) and mean transit time (MTT) of ROI were analyzed and computed at workstation related the parameters of cerebral h ino-dynamics. T-test was used in statistical analysis. Results Diagnostic sources data were successfully acquired in all improved CTP scanrings. The mapping of BF、BV and MTT of internal capsule(posterior limb), lentiform nucleus and gray matter (superior temporal gyrus) in dog was similar in both usual and improved CTP. The mean value of BF、BV and MTT in these areas had no significant difference(p0.05) in the two CTP methods,and the exposure time of CT tube was decreased 10 seconds every CTP scan in the improved technique.The improved CT brain perfusion imagings accurately detected the site, region and degree of the acute cerebral ischemic nidi in all 6 cases. Conclusion The improved MSCT perfusion imaging has similar diagnostic effects with that of usual CTP, and the radiation-induced injuring will he deereas(?) as well as the tube lifetime will be prolonged.
Radiation dose and image quality assessment on Body Tom CT

Abstract:Objective To evaluate and compare the radiation dose and image quality of the new generation of whole body mobile CT (BodyTom CT) with commonly used fixed CT.Methods The image quality was evaluated with CATPHAN 500 performance test body model.The radiation dose was measured by conventional 100 mm pen ionization chamber and PMMA body phantom and head phantom (head diameter 160 mm,phantom diameter 320 rm and width 140 mm).Results The spatial and contrast resolution of BodyTom CT images were similar to two kinds of fixed CT(P ＞ 0.05).The CNR of image with BodyTom CT decreased by about 20％:In head scan mode,significantly lower than that with Philps 64 slice CT and Toshiba 320 slice CT (with soft,t =-4.82,-6.98,P ＜ 0.05;with standard,t =-20.60,-20.09,P ＜0.05);in body scan mode,significantly lower than that with Philps 64 slice CT and Toshiba 320 slice CT (with soft,t =-5.67,-12.82,P ＜ 0.05;with standard,t =-3.39,-9.18,P ＜ 0.05;with sharp,t =-3.88,-3.21,P ＜0.05).The radiation dose with BodyTom CT was significantly higher than that with fixed CT:in body model,22.97％ than that with Philps 64(t=9.48,P＜0.05),29.6％ than that with Toshiba 320 slice CT(t =11.66,P ＜0.05);in head model,29.76％ than that with Philps 64 slice CT(t=23.44,P＜0.05),33.22％ than that with Toshiba 320 slice CT(t=23.11,P＜0.05).Conclusions The radiation dose with mobile CT was over 20％ higher than that with routine multi-row CT while with the similar image quality.
Benefit of Improved Performance with State-of-the Art Digital PET/CT for Lesion Detection in Oncology

Abstract:The latest digital whole-body PET scanners provide a combination of higher sensitivity and improved spatial and timing resolution. We performed a lesion detectability study on two generations of Biograph PET/CT scanners, the mCT Flow and the Vision, to study the impact of improved physical performance on clinical performance. Our hypothesis was that the improved performance of the Vision would result in improved lesion detectability, allowing shorter imaging times or, equivalently, a lower injected dose. Methods: Data were acquired with the Society of Nuclear Medicine and Molecular Imaging Clinical Trials Network torso phantom combined with a 20-cm-diameter cylindrical phantom. Spherical lesions were emulated by acquiring sphere-in-air data and combining them with the phantom data to generate combined datasets with embedded lesions of known contrast. Two sphere sizes and uptakes were used: 9.89-mm-diameter spheres with 6:1 (lung) and 3:1 (cylinder) local activity concentration uptakes and 4.95-mm-diameter spheres with 9.6:1 (lung) and 4.5:1 (cylinder) local activity concentration uptakes. Standard image reconstruction was performed: an ordinary Poisson ordered-subsets expectation maximization algorithm with point-spread function and time-of-flight modeling and postreconstruction smoothing with a 5-mm gaussian filter. The Vision images were also generated without any postreconstruction smoothing. Generalized scan statistics methodology was used to estimate the area under the localized receiver-operating-characteristic curve (ALROC). Results: The higher sensitivity and improved time-of-flight performance of the Vision leads to reduced contrast in the background noise nodule distribution. Measured lesion contrast is also higher on the Vision because of its improved spatial resolution. Hence, the ALROC is noticeably higher for the Vision than for the mCT Flow. Conclusion: Improved overall performance of the Vision provides a factor of 4-6 reduction in imaging time (or injected dose) over the mCT Flow when using the ALROC metric for lesions at least 9.89 mm in diameter. Smaller lesions are barely detected in the mCT Flow, leading to even higher ALROC gains with the Vision. The improved spatial resolution of the Vision also leads to a higher measured contrast that is closer to the real uptake, implying improved quantification. Postreconstruction smoothing, however, reduces this improvement in measured contrast, thereby reducing the ALROC for small, high-uptake lesions.
