Abstract:ObjectiveWe aimed to describe the clinical picture, management and outcomes of Chinese patients with peptic ulcer bleeding (PUB), especially in those with high risks.MethodsA multicenter endoscopic survey was conducted. All consecutive patients with endoscopy confirmed PUB from October 2010 to June 2011 were enrolled. Data including patients' gender, age, symptoms and endoscopic findings, Forrest classification, and endoscopic and medical treatment were documented. High-risk ulcer was defined as Forrest grades Ia to IIb upon endoscopy. Rates of rebleeding, surgery and mortality were recorded.ResultsIn all, 1006 patients were included. Of these 437 (43.4%) were categorized with high-risk PUB, among whom 110 (25.2%) received endoscopic treatment, and the success rate was 99.1%. Rebleeding rates 1-3 days, 4-5 days and 6-30 days after treatment in high-risk patients who did and did not receive endoscopic treatment were 10.9% versus 10.4%, 3.6% versus 3.7% and 0.9% versus 1.5%, respectively. The surgery rates of high-risk patients with or without endoscopic treatment were 1.8% (2/110) versus 1.8% (6/327). During the 9-month study period, two patients with high-risk PUB died, therefore, the overall mortality rate of high-risk PUB was 0.5% (2/437).ConclusionThe study suggests that the proportions of high-risk PUB in China is 43.4%, while rebleeding and surgery rate after endoscopic treatment as well as the mortality rate of high-risk PUB in China are 15.6%, 1.8% and 0.5%, respectively.

Abstract:To study the relationship between ulcer recurrence and Helicobacter Pylori . 147 patients with peptic ulcer were divided into H. Pylori infected group and H. pylori uninfected group by rapid urease test serum antibody examination and histopathology of gastric biopsy specimens. We assigned all patients to receive dual oral therapy with omeprazole 40 mg/day for 4 weeks and amoxicillin 2000 mg/day for 2 weeks. To all patients, repeated gastroscope was performed at 8 weeks and one-year after beginning therapy to record whether the ulcer was cured. Endoscopic biopsy was also performed to determine H. pylori status. One year later, H. pylori positive patients had a higher ulcer recurrent rate than H. pylori negative patients (23. 33% vs 2. 70%; P=0.001). Ulcer recurrence was closely related to H. pylori infection. H. pylori was the risk factor for ulcer relapse.
Risk Factors for Recurrence of Peptic Ulcer Disease: A Retrospective Study in Tertiary Care Referral Center

Abstract:Backgrounds Peptic ulcer disease (PUD) is a common gastrointestinal tract disease characterized by mucosal damage secondary to pepsin and gastric acid secretion. This study evaluated the five-year recurrence rate for patients with PUD and risk factors contributing to PUD relapses. Methods From 2016 through 2021, all patients with endoscopy-proved PUD were identified by reviewing medical records (Best-Care system). Possible risk factors including smoking, nonsteroidal anti-inflammatory drugs (NSAIDs), aspirin, alcohol, caffeine, and steroids were analyzed by multivariate analysis. Treatment outcomes, 5-year recurrence rate, and mortality rate were assessed. Results Among 223 patients, there were 187 (83.8%) diagnosed with endoscopy-proved PUD and 36 (16.2%) diagnosed with clinical PUD. Among them, 126 (56.5%) patients were males, and the mean age was 62±2 years. The five years recurrence rate of PUD was 30.9%. There was no significant difference in the recurrence rate between the duodenal ulcer (33.3%) and the gastric ulcer (28.8%). By univariate analysis, the use of steroids and NSAID and Helicobacter pylori (H. pylori) infection were potential risk factors for PUD (P < 0.005). The common complication of PUD was gastrointestinal bleeding (34.1%). Patients who had a complicated PUD were associated with a higher rate of recurrence (45.9%) compared to the uncomplicated PUD (19.2%) (P > 0.05). Conclusion Our findings demonstrated that the five years recurrence rate of PUD was 30.9%. The use of steroids and NSAID and H. pylori infection were risk factors for recurrence of PUD. PUD places a significant burden on health care systems. Therefore, a multicenter prospective study is needed for effective management to prevent recurrence and complications of PUD.
Impact of peptic ulcer bleeding on the in-hospital outcomes of cirrhotic patients with acute gastrointestinal bleeding: an international multicenter study

Abstract:Objectives: Peptic ulcer is the most common source of non-variceal bleeding. However, it remains controversial whether the outcomes of cirrhotic patients with peptic ulcer bleeding differ from those with variceal bleeding. Methods: Cirrhotic patients with acute gastrointestinal bleeding (AGIB) who underwent endoscopy and had an identifiable source of bleeding were retrospectively screened from an international multicenter cohort. Logistic regression analyses were performed to explore the impact of peptic ulcer bleeding on in-hospital death and 5-day failure to control bleeding. Propensity score matching (PSM) analysis was performed by matching age, gender, Child-Pugh score, and model for end-stage liver disease score between the peptic ulcer bleeding and variceal bleeding groups. Results: Overall, 1535 patients were included, of whom 73 (4.7%) had peptic ulcer bleeding. Multivariate logistic regression analyses showed that peptic ulcer bleeding was not independently associated with in-hospital death (OR = 2.169, p = 0.126) or 5-day failure to control bleeding (OR = 1.230, p = 0.680). PSM analyses demonstrated that both in-hospital mortality (9.7% vs. 6.3%, p = 0.376) and rate of 5-day failure to control bleeding (6.9% vs. 5.4%, p = 0.787) were not significantly different between the two groups. Conclusions: The impact of peptic ulcer bleeding on the in-hospital outcomes of cirrhotic patients is similar to that of variceal bleeding.

Abstract:UNLABELLED Prevalence of peptic ulcer (PU) and risk factors associated with its recurrence are unknown among the Mexican population. OBJECTIVE To determine clinical features and risk factors associated with peptic ulcer recurrence. MATERIAL AND METHODS Five-year follow-up study that included 211 men and 140 women 20 years age or older. PU diagnosis was made by clinical and endoscopic criteria. Associated risk factors such as NSAIDs use, smoking, alcohol consumption, and H. pylori infection were assessed. Clinical recurrence was defined as the time between basal diagnosis and the first PU clinical drop. RESULTS Diagnosis of duodenal ulcer (DU) and gastric ulcer (GU) was documented in 41.9 and 58.1% of subjects. Predominant risk factor for DU was smoking, and NSAID intake for GU. H. pylori were documented in 39.3%. Sixty-five years of age or older (61 and 33% for GU and DU, respectively), and three or more associated risk factors (70%) were the main associated causes for PU recurrence. PU recurrence associated with Hp was 34%. CONCLUSIONS GU frequency was the highest and associated risk factors for its recurrence were NSAID intake, 65 years of age or older, and Hp infection.

Association Between Short-Term Exposure to Air Pollution and Peptic Ulcer Bleeding: A Case-Crossover Study in China

Abstract:Background: Air pollution has already become a serious public health concern worldwide, while evidence for the association between air pollution and digestive disease is not consistent. Objective: To examine the association between air pollution and peptic ulcer bleeding (PUB). Methods: We performed a time-stratified case-crossover study among 1,246,270 gastroscopy records in Zhejiang, China from January 1st, 2013 to June 30th, 2019. Individual exposure to air pollution for each subject was estimated by using data from 55 fixed-sites with an inverse distance weighted method. Conditional logistic regression models were used to estimate the odds ratios (ORs) and 95% confidence intervals (95% CIs) for PUB associated with an interquartile range (IQR) increase in air pollution on the entire population. Subgroup analyses were performed for season, age and sex. Results: Totally 3543 patients diagnosed with PUB were included in the analyses. Significant associations were observed between NO2, CO and PM2.5 and the risk of PUB. The effect was observed strongest measured as lag(06) and an IQR increase of 7-day moving average concentrations of NO2 resulted in a 24.0% increase of getting PUB (95% CI: 2.6%-50.0%). Significance: Our findings strengthen the hypothesis that short-term air pollution exposure has adverse health effects on digestive system.

Abstract:目的探讨胰十二指肠切除术(PD)后上消化道出血(UGIH)的原因及防治.方法对本院1998年1月至2004年6月行PD术的195例患者的临床资料进行回顾性分析.结果 18例患者发生UGIH,死亡4例.UGIH组术中低血压7例,行捆绑式胰肠吻合6例;而无UGIH组分别为26例和118例.术后UGIH组合并胰漏3例,合并肝功能不全6例,合并肾功能不全5例;而无UGIH组分别为6例、18例和12例.上述数据相比,两者差异有显著性(P＜0.05).PD术后UGIH早期(术后5 d以内)发生4例,晚期(术后5 d以上)发生14例.结论早期出血主要由胃肠吻合口或胰腺断端出血造成,晚期出血以吻合口溃疡或应激性溃疡为主.术中低血压,术后合并胰漏,肝肾功能不全是重要诱发因素.行捆绑式胰肠吻合能减少术后胰漏和出血的机会.早期出血以手术治疗为主,晚期出血以内科保守治疗为主。
The Prediction of Mortality in Bleeding Peptic Ulcers Treated with Therapeutic Endoscopy - Lesson Learnt from 3000 Patients

Abstract:Background: Bleeding peptic ulcers is among the leading cause of hospitalization. The reported mortality approaches 10%. We reviewed our prospective database to identify risk factors for in-hospital mortality in patients with bleeding peptic ulcers. Patients & Method: We prospectively collected data on patients with bleeding peptic ulcers since 1993. The parameters collected included the patient's demographics, the characteristics of the peptic ulcer, treatment received and the outcome. We used a univariate followed by multivariate analyses to identify factors for in-hospital deaths. Results: From 1993 to 2003, 9032 patients presented to our endoscopy center with bleeding peptic ulcers. The overall mortality is 4.6%, and the overall rate of rebleeding is 4.9%. 3220 patients had bleeding peptic ulcers with endoscopic stigmata of active bleeding, visible vessel or adherent clot. 229 of them died during hospitalization (7.1%). From multivariate analysis, age > 70, the presence of comorbidities, hematemesis upon presentation, initial systolic BP < 100 mm Hg, in-hospital ulcer bleeding, non-H pylori related ulcer, rebleeding and need of surgery were significant predictors for mortality. (Table 1) Conclusions: For high risk bleeding peptic ulcers, elderly, presence of comorbidities, hematemesis, shock, in-hospital bleeding and ulcer rebleeding are significant predictors for mortality. HP related ulcers are associated with a lower mortality. Peptic Ulcer Is the Most Common Cause of Non-Variceal Upper-Gastrointestinal Bleeding (NVUGIB) in China

Abstract:BACKGROUND This study aimed to discover the common cause of non-variceal upper-gastrointestinal bleeding (NVUGIB) by conducting a multi-center retrospective study from 2008 to 2012. MATERIAL AND METHODS Hospitalized patients ages ≥18 years old, from 8 hospitals in China, diagnosed with NVUGIB by endoscopy from 1 January 2008 to 31 December 2012 were enrolled. Questionnaires were developed and a data-entry graphical user interface was designed by using EpiData software. RESULTS Total of 2977 hospitalized patients from 8 medical centers were included. A total of 95.47% (2842/2977) of patients were admitted to a general ward, 3.53% (105/2977) were admitted to an emergency ward, and 1.00% (31/2977) were admitted to an intensive care unit. Peptic ulcer remained the most common cause of NVUGIB (73.26%), but there was a declining trend in its constituent ratio, from 2008 to 2012. A total of 14.41% (429/2977) of patients had co-morbid conditions, 92.85% (2764/2977) used proton-pump inhibitors (PPIs) prior to endoscopic treatment, 19.65% (585/2977) underwent emergency endoscopy, and 23.45% (698/2977) received a transfusion of red blood cell suspensions. A total of 5.34% (159/2977) underwent endoscopic therapy, with a treatment rate of 16.9% in high-risk peptic ulcer patients (96/568). A total of 7.69% (237/2977) were administered aspirin, of whom 32.50% (77/237) resumed aspirin intake after gastrointestinal bleeding was controlled. The median length of hospitalization was 8 days (IQR, 5-11) and the mortality rate was 1.71% (51/2977). CONCLUSIONS Peptic ulcer was still the most common cause of NVUGIB in China. The proportion of patients with high-risk peptic ulcer bleeding who received endoscopic therapy was 16.9%. Only 19.65% of NVUGIB patients underwent emergency endoscopy.
Characteristics of peptic ulcer bleeding in cirrhotic patients with esophageal and gastric varices

Abstract:Abstract Upper gastrointestinal bleeding (UGIB) is common in liver cirrhosis. Although esophageal and gastric varices (EGV) is the main bleeding source, there were still a proportion of patients with peptic ulcer bleeding. Thus, this study aimed to analyze the characteristic of variceal bleeding and peptic ulcer bleeding in liver cirrhosis. Cirrhotic patients with confirmed UGIB by urgent endoscopy from July 2012 to June 2018 were enrolled, and classified into peptic ulcer bleeding group (n = 248) and variceal bleeding group (n = 402). Clinical and endoscopic characteristics, therapeutic efficacy and prognosis were evaluated, and independent risk factors for 42-day morality were determined. The mean age and gender ratio of peptic ulcer bleeding group were higher than those in variceal bleeding group (55.58 ± 11.37 vs. 52.87 ± 11.57, P < 0.01; 4.51:1 vs. 2.87:1, P = 0.023). Variceal bleeding group most commonly presented as red blood emesis and coffee grounds (67.16%), while peptic ulcer group primarily manifested as melena (62.10%). Hepatocellular carcinoma was more prevalent in peptic ulcer group (141 vs. 119, P < 0.01). Albumin level in variceal bleeding group was lower higher ( P < 0.01), but serum bilirubin, creatinine and prothrombin time were significantly higher (all P < 0.01). Success rate of endoscopic hemostasis for variceal bleeding and peptic ulcer bleeding was 89.05% and 94.35% ( P = 0.021). Univariate and multivariate analysis identified prothrombin time ( P = 0.041, OR [95% CI] 0.884 [0.786–0.995]), MELD score ( P = 0.000, OR [95% CI] 1.153 [1.073–1.240]), emergency intervention ( P = 0.002, OR [95% CI] 8.656 [2.219–33.764]), hepatic encephalopathy before bleeding ( P = 0.003, OR [95% CI] 8.119 [2.084–31.637]) and hepatic renal syndrome before bleeding ( P = 0.029, OR [95% CI] 3.877 [1.152–13.045]) as the independent predictors for 42-day mortality. Peptic ulcer bleeding should be distinguished from variceal bleeding by clinical and endoscopic characteristics.

Abstract:Objective To explore the incidence and related risk factors for stress ulcer (SU) bleeding in neurocritical patients at multiple centers.Methods We retrospectively analyzed the data of neurocritical patients at Department of Neurosurgery of 12 hospitals in China,which consisted of the patient's age,diagnosis,past medical history and 17 potential associated risk factors including duration of mechanical ventilation,gastrointestinal (GI) ulcer or bleeding history,GCS (Glasgow coma scale),duration of neurosurgical operation,intracranial infection,stroke,traumatic brain injury,intracerebral hemorrhage,history of brain tumor operation,resuscitation of heart,lung and brain,shock,hepatic failure,renal failure,multiple organ failure,acid base imbalance,high dose of corticosteroids,administration of anticoagulant and antiplatelet drugs.Univariate analysis was conducted with Log-rank and multivariate analysis was performed using the Cox proportional hazard model.Results A total of 1 416 patients were enrolled into this study,among whom 182 cases (12.9％,95％ CI:11.2-14.7) developed SU.The incidence of GI bleeding (10 cases) was 0.7％ (95％ CI:0.3-1.3).Multivariate analysis showed that mechanical ventilation lasting over 48 hours (RR =0.526,95％ CI:0.381-0.726,P ＜ 0.0001) and use of anticoagulants (RR =0.458,95％ CI:0.327-0.643,P ＜ 0.0001) were independent risk factors for SU bleeding in neurocritical patients.The patients with history of GI ulcer or bleeding (RR =0.082,95％ CI:0.016-0.411,P =0.0023) had higher chance of developing SU bleeding.Conclusion SU bleeding is not uncommon in neurocritical patients and cautions should be warranted particularly for those with risk factors.

Methods: Using the National Health Insurance Research Database of Taiwan and matching age, gender, underlying co-morbidities and enrollment time by propensity score, 11105 aspirin users and 11105 controls were identified for comparison from a cohort dataset of 1,000,000 randomly sampled subjects. Cox proportional hazard regression models were used to identify independent risk factors for first-time occurrence of non-variceal UGIB in the study cohort and in the aspirin users after adjusting for age, gender, underlying co-morbidities, and medications (e.g., non-steroidal anti-inflammatory drugs [NSAIDs], cyclooxygenase-2 [COX-2] inhibitors, steroids, thienopyridines, selective serotonin reuptake inhibitors, warfarin, and dipyridamole). Results: By Cox proportional hazard regression analysis, aspirin use increased the risk of first-time occurrence of UGIB (hazard ratio [HR]: 1.48; 95% confidence interval [CI]: 1.28-1.72). Age, male gender, Helicobacter pylori (H. pylori)infection, diabetes, chronic kidney disease (CKD), cirrhosis, history of uncomplicated peptic ulcer disease (PUD), and use of NSAIDs, COX-2 inhibitors, steroids, and thienopyridines were independent risk factors for UGIB among aspirin users. Conclusion: In addition to age, male gender, H. pylori infection, and concomitant use of NSAIDs, COX-2 inhibitors, steroids, and thienopyridines, underlying co-morbidities including diabetes, CKD, cirrhosis, history of PUD are also important risk factors for first-time occurrence of non-variceal UGIB in aspirin users.
Risk Factors of Rebleeding in Patients with Obscure Gastrointestinal Bleeding

Abstract:Objective To investigate the long-term (＞1 year) rebleeding rate after capsule endoscopy (CE)-guided intervention in patients with obscure gastrointestinal bleeding (OGIB) and to identify the risk factors of rebleeding.Methods A total of 307 consecutive patients who underwent CE for OGIB in our hospital from June 2002 to October 2010 were enrolled.Follow-up data were obtained by reviewing medical records,CE database and contacting the patients or their relatives by telephone.We evaluated the rebleeding rates and analyzed risk factors predictive of rebleeding by means of COX ratio hazard model.Results The medium follow-up was 52 months (range13-112 months).Significant lesions were found in 202 patients (65.8％).The overall rebleeding rate after interventional therapy induced by CE findings was 28.0％ (86/307).CE positive patients had higher rebleeding rate than CE negative patients (37.6％ vs 9.5％,log-rank test,P=0.000),while specific therapy could prevent rebleeding,compared with nonspecific therapy (32.9％ vs 23.0％,P=0.042).95.3％ (82/86) rebleeding occurred within 24 months after CE.Multivariate analysis performed by using COX proportional hazards model showed that age over 50 years,CE positive findings,lowest hemoglobin (Hb) level 3 months before CE ≤7 g/dl,receiving nonspecific therapy after CE,hypertension,administration of anticoagulants,antiplatelet medicine or NSAIDs after CE were six risk factors associated with rebleeding.Conclusion Clinicians should be aware of these risk factors for OGIB rebleeding,which can reduce the occurrence of rebleeding and improve OGIB patients' prognosis.Those high risk OGIB patients should be followed up for at least 24 months after CE.

Abstract:OBJECTIVE:To investigate the clinical characteristic and prognosis of nonsteroidal anti-inflammatory drugs (NSAIDs) and aspirin associated peptic ulcer bleeding.METHOD:All patients with peptic ulcer bleeding were studied by the same researcher after admission and discharge.RESULTS:Fifty-one cases with NSAIDs and aspirin medication of the total peptic ulcer bleeding patients (147 cases) were included (34.7%). Compared with patients not associated with NSAIDs and aspirin medication, they are older [(41.2 ± 1.9) years vs (59.4 ± 2.2) years, P < 0.001], more commonly associated with hypertension (10.4% vs 39.2%, P < 0.001), coronary heart disease (10.4% vs 17.6%, P < 0.001), diabetes (4.2% vs 19.6%, P = 0.005); and had more severe anemia (7.3% vs 23.5%, P = 0.008). Fewer patients in NSAIDs group had epigastric pain (61.5% vs 27.5%, P < 0.001), while there was more re-bleeding (9.4% vs 15.7%, P = 0.034). In all bleeding patients, factors associated with re-bleeding, surgical intervention and death included NSAIDs and aspirin medication, and low platelet count. In patients with NSAIDs and aspirin medication, re-bleeding was associated with previous ulcer history (P < 0.05).CONCLUSION:Peptic ulcer bleeding patients with NSAIDs and aspirin medication were more severe ill, and less likely to present with epigastric pain.

Abstract:Acute upper gastrointestinal bleeding is a common medical emergency worldwide, a major cause of which are bleeding peptic ulcers. Endoscopic treatment and acid suppression with proton-pump inhibitors are cornerstones in the management of the disease, and both treatments have been shown to reduce mortality. The role of emergency surgery continues to diminish. In specialised centres, radiological intervention is increasingly used in patients with severe and recurrent bleeding who do not respond to endoscopic treatment. Despite these advances, mortality from the disorder has remained at around 10%. The disease often occurs in elderly patients with frequent comorbidities who use antiplatelet agents, non-steroidal anti-inflammatory drugs, and anticoagulants. The management of such patients, especially those at high cardiothrombotic risk who are on anticoagulants, is a challenge for clinicians. We summarise the published scientific literature about the management of patients with bleeding peptic ulcers, identify directions for future clinical research, and suggest how mortality can be reduced.

Abstract:Background Marginal ulcer (MU) is one of the postoperative complications of pancreaticoduodenectomy (PD), which needs particular attention in postoperative treatments. Methods The data of 190 patients who underwent PD and follow-up gastroscopic review due to upper GI symptoms within two years were retrospectively analyzed. The incidence of MU and risk factors were analyzed based on personal history, surgical procedure, past medical history, postoperative complications, and other relevant indicators. Results The proportion of MU in patients who underwent endoscopic follow-up for upper gastrointestinal symptoms in the postoperative period in this cohort was 10.5% (20/190). Advanced age (69y vs. 59y, P = 0.012), alcohol consumption (20% vs. 8.2%, P = 0.03), and cigarette smoking (35% vs. 14.7%, P = 0.022) were associated with an increased incidence of MU. Longer surgery time (276.5min vs. 240min, P = 0.049), postoperative bleeding (10% vs. 1.8%, P = 0.030), and failure to take antacid regularly postoperatively (75% vs. 97.1%, P = 0.000) would increase the risk of MU; taking antacid regularly was an independent protective factor for postoperative anastomotic ulceration (OR: 0.091, CI: 0.022–0.383, P = 0.001). Conclusion Advanced age, alcohol consumption, smoking, longer operation time, or postoperative extraluminal hemorrhage are associated with MU. Regular use of antacids is an independent protective factor against the development of MU.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background & Aims</h3><p>Little is known about risk of upper gastrointestinal bleeding (UGIB) in patients failed by <em>Helicobacter pylori</em> eradication therapy. We investigated the effects of different time until retreatment, after failure of initial <em>H pylori</em> eradication therapy, on subsequent risk of UGIB.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>We performed a territory-wide retrospective cohort study of 70,518 patients with <em>H pylori</em> infection who had received their first course of clarithromycin-based triple therapy from January 2003 through December 2012 in Hong Kong. Patients who required retreatment after failed initial therapy (n= 8330, 11.8%) were categorized based on time between initial and final <em>H pylori</em> eradication (3 months or less, 3–12 months, and more than 12 months). We collected clinical data from 30 days after prescription of the last course of <em>H pylori</em> therapy until hospitalization for non-variceal UGIB, death, or the end of the study (30 Jun 2016; median follow-up time, 7.65 years). The primary outcome was difference in development UGIB (determined from ICD-9 codes) between patients who required retreatment and those who did not (reference group).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>Compared with the reference group, patients who required retreatment had an overall higher risk of UGIB, even after last eradication therapy (adjusted hazard ratio (HR), 1.50, 95% CI, 1.34–1.69). There was a progressive increase in risk of UGIB with longer time from initial until final eradication therapy: hazard ratio for time less than 3 months, 1.16; 95% CI, 0.88–1.54, hazard ratio for time 3–12 months, 1.35; 95% CI, 1.07–1.69, and hazard ratio for time more than 12 months, 1.68; 95% CI, 1.46–1.94 (<em>P</em> for trend = .038).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>In a retrospective study of patients in Hong Kong, we found that those failed by initial <em>H pylori</em> eradication have an increased risk of UGIB, compared to patients who responded to the initial therapy. Risk increased progressively with longer time until retreatment. Early retreatment within 3 months should be considered to minimize subsequent UGIB risk.</p>

Abstract:Non-Helicobacter pylori, non-NSAID ulcer is relatively common in Western countries. Whether it is a significant problem in the Orient is unclear. The aim of this study was to investigate the incidence of non-H pylori, non-NSAID ulcers presenting with GI bleeding.A prospective study was done of 1675 consecutive patients presenting with upper GI bleeding over a period of 12 months. Upper endoscopy was performed with biopsy specimens taken from the antrum and body of the stomach for a biopsy urease test (BUT) and histology for detection of H pylori. Exposure to nonsteroidal anti-inflammatory drugs (NSAID) or aspirin within 4 weeks of hospitalization was carefully scrutinized. A 6-week course of treatment with an H2-receptor antagonist was prescribed for patients who did not use an NSAID and had a negative BUT result. Follow-up endoscopy was performed to confirm H pylori status with a BUT and histology. Positive histology at either initial or follow-up endoscopy was used as the standard for diagnosing H pylori infection.Among 977 patients who were found to have ulcer bleeding, 434 (44%) had exposure to aspirin or an NSAID. Of the 543 non-NSAID users, 431 (79.4%) had a positive BUT and 112 (20.6%) were BUT negative on initial endoscopy. Eighty-nine of 112 patients who were NSAID negative, BUT negative returned for follow-up endoscopy. Forty-nine of 89 (55.1%) were found to have a positive BUT and positive histology at follow-up endoscopy. Only 40 of 977 (4.1%) patients admitted with ulcer bleeding were confirmed to have non-H pylori, non-NSAID ulcers.Non-H pylori, non-NSAID bleeding ulcer is uncommon. A negative BUT is unreliable for exclusion of H pylori infection during the acute phase of ulcer bleeding.
Predicting Mortality in Patients with Bleeding Peptic Ulcers after Therapeutic Endoscopy

Abstract:Background & Aims: Despite advances in management of patients with bleeding peptic ulcers, mortality is still 10%. This study aimed to identify predictive factors and to develop a prediction model for mortality among patients with bleeding peptic ulcers. Methods: Consecutive patients with endoscopic stigmata of active bleeding, visible vessels, or adherent clots were recruited, and risk factors for mortality were identified in this deprivation cohort by using multiple stepwise logistic regression. A prediction model was then built on the basis of these factors and validated in the evaluation cohort. Results: From 1993 to 2003, 3220 patients with bleeding peptic ulcers were treated. Two hundred eighty-four of the patients developed rebleeding (8.8%); emergency surgery was performed on 47 of these patients, whereas others were managed with endoscopic retreatment. Two hundred twenty-nine of these sustained in-hospital death (7.1%). In patients older than 70 years, presence of comorbidity, more than 1 listed comorbidity, hematemesis on presentation, systolic blood pressure below 100 mm Hg, in-hospital bleeding, rebleeding, and need for surgery were significant predictors for mortality. Helicobacter pylori-related ulcers had lower risk of mortality. The receiver operating characteristic curve comparing the prediction of mortality with actual mortality showed an area tinder the curve of 0.842. From 2004 to 2006, data were collected prospectively from a second cohort of patients with bleeding peptic ulcers, and mortality was predicted by using the model developed. The receiver operating characteristic curve showed an area under the curve of 0.729. Conclusions: Among patients with bleeding peptic ulcers after endoscopic hemostasis, advanced age, presence of listed comorbidity, multiple comorbidities, hypovolemic shock, in-hospital bleeding, rebleeding, and need for surgery successfully predicted in-hospital mortality.
[Risk Factors Associated with Failure from Endoscopic Therapy in Acute Non-Variceal Upper Gastrointestinal Bleeding].

Abstract:OBJECTIVETo determine risk factors associated with failure of endoscopic therapy in acute non-variceal upper gastrointestinal bleeding (ANVUGIB ).METHODSThis was a retrospective cohort study of 223 patients admitted to Peking University Third Hospital between 1 January 2005 and 31 December 2009, with acute non-variceal upper gastrointestinal bleeding. Data on clinical presentation, laboratory test, endoscopic findings, and treatment outcomes were collected. Risk factors for treatment failure were identified using multivariable Logistic regression with backward selection.RESULTSTherapeutic failure rate was 19.3%(43/223). In univariate analysis, the two groups had significant difference in age, history of gastrointestinal bleeding, ASA, shock, haemoglobin level, Hct, PLT, time of endoscopic treatment, gastric ulcer, duodenal ulcer, lesion size and active spurting of blood. Multivariate Logistic regression analysis revealed that shock [odds ratio (OR) 3.058, 95% confidence interval (CI) 1.295-7.221], history of gastrointestinal bleeding (OR 2.809, 95% CI 1.207-6.539), PLT>100×10⁹/L (OR 0.067, 95% CI 0.009-0.497), active spurting of blood (OR 10.390, 95% CI 2.835-38.080) and lesion size≥2.0 cm (OR 7.111, 95% CI 1.628-31.069) were risk factors associated with failure of endoscopic therapy. The number of comorbidities>1 (OR 9.580,95%CI 1.383-66.390) and active spurting of blood (OR 9.971, 95% CI 1.820-54.621) were factors related with need for surgical intervention or death.CONCLUSIONPatients with shock, history of gastrointestinal bleeding, PLT<100×10⁹/L, active spurting of blood and large lesion size, have high risks for continued bleeding or rebleeding after endoscopic treatment. These patients may be more likely to benefit from aggressive post-hemostasis care.
