Abstract:Background and aim: Traditionally, the principle of the treatment for anal fistula is "open fistula, fistulotomy ".Fistulotomy is the standard treatment for simple and low fistulas.However for the high fistula, the anorectal ring will be disconnected by fistulotomy.A better treatment should damage the anorectal ring less and protect the anal function more.The aim of the present study is to explore a new method-bidirectional isobaric drainage radical resection to treat simple high anal fistula.Materials and methods: One hundred and twenty patients with simple high anal fistula were randomly divided into Experimental group and Control group, 60 patients in each group.The patients of Experimental group were treated using bidirectional isobaric drainage radical resection, i.e., moving internal orifice of the fistula up and making another opening at top end of fistula penetrating to the rectal cavity, then put the drainage seton to form a two-way isobaric drainage with the low incision.The patients in Control group were treated using a high cutting seton therapy with the low incision.The changes of average wound healing time, the cure rate, the recurrence rate, the postoperative pain (four-category verbal rating scale, VRS), and the score of anal function (Wexner Incontinence Score) were quantitatively evaluated.Results: Comparison of the efficacy between two groups indicated that the complete cure rate differed significantly between two groups (P< 0.05).The healing time was significantly shorter in the Experimental group than in the Control group (P<0.01).VRS and Wexner scores were significantly smaller in Experimental group than in Control group (P<0.01).These indicated that the treatment for the simple high anal fistula had obvious advantages in the Experimental group than in the Control group to reduce postoperative pain and protect anal function.In the patients of Experimental group, the appearance was kept well and had good anal closure (Figure 1 A-D).However, in the patients of Control group, the appearance was changed and the anal closure was not well (Figure 1 E-H).Conclusions and perspectives: In this study we first time report to use the bidirectional isobaric drainage method to treat the simple high anal fistula.This method has high cure rate, less recurrence rate, low expenses, less wound, less pain and short course.In the future, we will use this method to treat complicated high anal fistula.The mechanism of the relation between anal pressure and anal fistula will be further investigated.The long-term follow-up and a large sample, multi-center clinical studies should also be considered in the future.Furthermore, the anal and rectal pressures are not monitored in this study and definitely should be considered in the future studies.
