Disseminated Abscesses in a Child

Abstract:A 20-month-old girl diagnosed with pre-B acute lymphoblastic leukemia developed a prolonged episode of fever and neutropenia during her induction regimen of vincristine, daunorubicin, L-asparaginase, and oral dexamethasone. She has no clear focus of infection, but her blood culture grew Candida tropicalis susceptible to echinocandins. Her central line was removed, and she was started on caspofungin (70 mg/m 2 loading dose followed by 50 mg/m 2 daily). All initial and repeat blood cultures remained negative. Screenings for disseminated fungal infection including physical and funduscopic examination, computed tomography (CT) of the chest, and abdominal ultrasound (US) were normal. Despite antifungal coverage, she continued to have high-grade fever, with high inflammatory markers (C-reactive protein, 150–300 mg/L). In view of her risk, we opted to resume chemotherapy while she was on caspofungin. Four weeks later, her fever became more persistent, and her C-reactive protein increased to 350 mg/L. Repeat blood cultures were negative. In addition, she developed a generalized maculopapular rash that started initially on the scalp. CT of the abdomen showed multiple new hypodense lesions in both liver and spleen and 2 subphrenic perisplenic abscesses measuring 9 × 17.5 mm and 28 × 15 mm (Fig. 1). A liver biopsy was obtained, and histopathological examination showed features suggestive of healed inflammatory lesions and no fungal elements. Repeated bacterial and fungal cultures from blood and liver biopsy remained negative. During this time she received about 3 months of caspofungin and received multiple courses of antipseudomonal antibiotics. She continued to have recurrent fevers for the following 6 months. Repeat CT of the abdomen after 3 months showed persistent abscesses. After 6 months of continuous high fevers, a positron emission tomography scan was performed, unveiling widespread abnormal fluorodeoxyglucose uptake involving the left lung, spleen, and left cardiophrenic area. (Fig. 1). A new treatment was introduced, which led to the resolution of fevers and overall improvement in her condition.
