Abstract:BackgroundDezocine is considered to be an alternative medication for managing postoperative pain. The aim of this study was to assess the efficacy and safety of this drug in this regard.MethodsMedline, EMBASE and the Cochrane Central Register of Control Trials (CENTRAL) were searched to identify all randomized controlled trials (RCTs) that compare dezocine with placebo or dezocine with morphine on postoperative pain. The data were extracted and pooled using Mantel-Haenszel random effects model. Heterogeneity was tested using the I-2 statistic with values >50% and Chi(2) test with P <= 0.05 indicating obvious heterogeneity between the studies.ResultsSeven trials evaluating 665 patients were included. The number of patients with at least 50% pain relief was increased (N = 234; RR 3.04, 95% CI 2.27 to 4.08) and physician (N = 465; RR 2.84, 95% CI 1.66 to 4.84) and patient satisfaction (N = 390; RR 2.81, 95% CI 1.85 to 4.26) were improved following the administration of dezocine compared with the placebo. The effects of dezocine were similar to those of morphine in terms of the number of patients reporting at least 50% pain relief within 2-6 h after surgery (N = 235; RR 1.29, 95% CI 1.15 to 1.46) and physician (N = 234; RR 1.18, 95% CI 0.93 to 1.49) and patient (N = 158; RR 1.33, 95% CI 0.93 to 1.92) satisfaction. While, the number of patients with at least 50% pain relief within 0-1 h after surgery increased following dezocine compared with morphine treatment (N = 79; RR 1.45, 95% CI 1.18 to 1.77). There was no difference in the incidence of postoperative nausea and vomiting (PONV) following dezocine treatment compared with the placebo (N = 391; RR 1.06, 95% CI 0.42 to 2.68) or morphine treatment (N = 235; RR 0.65, 95% CI 0.14 to 2.93).ConclusionDezocine is a promising analgesic for preventing postoperative pain, but further studies are required to evaluate its safety.

Abstract:Remifentanil, ultra-short-acting mu-opioid receptor agonist, has the greatest advantage in analgesia but could increase postoperative pain scores and induces postoperative hyperalgesia. Dezocine is a mixed opioid receptor partial agonist/antagonist and has been used for postoperative hyperalgesia management in clinical patients,but the potential molecular mechanism is still unclear. Ca2+ /calmodulin-dependent protein kinase II(CaMKII) has been reported involved in remifentanil-induced hyperalgesia (RIH) in previous studies, but the relationship between CaMKII and dezocine in RIH is still unclear. To investigate the mechanism of dezocine in RIH, we used a remifentanil induced postoperative hyperalgesia (RIPH) in incisional pain model of mouse. We subcutaneously infused remifentanil (40 mu g/kg) to induce postoperative hyperalgesia. Dezocine (1.5 mg/kg, 3.0 mg/kg, and 6.0 mg/kg) was infused subcutaneously with remifentanil using the apparatus pump for 30 min. Paw withdrawal thermal latency (PWTL) and paw withdrawal mechanical threshold (PWMT) were used to assess thermal hyperalgesia and mechanical allodynia. Western blotting analysis and immunohistochemistry analysis were used to assess the expression of phosphorylated CaMKII alpha (p-CaMKII alpha) in somatosensory cortex, hippocampus and spinal cord. Subcutaneous infusion of remifentanil enhanced postoperative pain induced by surgical incision and increased PWTL and PWMT. Dezocine dose-dependently decreased the PWTL and PWMT in RIPH model. Correlating with behavioral effects, dezocine inhibited remifentanil-induced up-regulation of p-CaMKII alpha expression in somatosensory cortex, hippocampus and spinal cord. Dezocine could attenuate RIPH by suppressing p-CaMKII alpha.
Effects of parecoxib sodium preemptive analgesia on perioperative cytokine responses and stress responses in patients undergoing ophthalmology surgery

Abstract:Objective To investigate the effects of parecoxib sodium preemptive analgesia on perioperative cytokine responses and stress respses and postoperative analgesia in patients undergoing ophthalmology surgery. Methods One hundred ASA Ⅰ- Ⅱ patients undergoing ophthalmology surgery were randomly divided into 2 groups of fifty: the parecoxib group received parecoxib 40 mg by muscle injection, the control group received an equal volume of 0. 9% normal saline two ml. Venous blood samples were obtained at 10 minutes before parecoxib or 0. 9% normal saline was injected (T1) , 30 minutes ( T2) and 60 minutes (T3) after surgery was initiated, the moment when surgery was finished ( T4), 6 hours (T5 ), 12 hours(T6)and 24 hours (T7 ) after surgery was finished for determination of the plasma levels of IL-6, IL-8,IL-1RA,TNF and IL-1β. At the same time, the plasma levels of epinephrine, norepinephrine, cortisol and adrenocorticotropic hormone at Tl - T6 were measured. VAS scores with patients were recorded at 1,2, 6,12,24 h after surgery. Result The change of the plasma levels of IL-6, IL-8, IL-1RA, TNF, IL-1β in the two groups:in the parecoxib group the plasma levels of TNF and IL-1β did not decreased significantly as compared with the control group ( P > 0. 05 ) . The plasma levels of IL-6, IL-8 at T4 - T6 decreased significantly as compared with the control group (P < 0. 01) . The plasma levels of IL-1RA at T3 - T6 decreased significantly as compared with the control group ( P < 0. 01 ). The change of the plasma levels of epinephrine, norepinephrine, cortisol and adrenocorticotropic hormone in the two groups: in the parecoxib group the plasma levels of epinephrine, norepinephrine at T2 ,T3 decreased significantly as compared with the control group ( P < 0. 01 ). In the parecoxib group the plasma levels of cortisol at T2 - T4 decreased significantly as compared with the control group(P <0.01 ). In the parecoxib group the plasma levels of adrenocorticotropie hormone at T2 - T5 decreased significantly as compared with the control group ( P <0. 01 ). Postoperation VAS scores:VAS scores of the parecoxib group were less than that of the control group (P < 0. 05). Conclusion Preoperatively-administered pareeoxib during ophthalmology surgery can produce better analgesia effect, reduce the production of cytokines, decrease central nervous system sensitization so as to improve the quality of postoperative pain relief. At the same time, it can reduce perioperative stress hormone release so as to have a positive effect on post operative recovery.
Analgesic and Sedative Effects of Dezocine and Midazolam During Vitrectomy.

Abstract:Purpose: To evaluate the analgesic and sedative effectiveness of dezocine and midazolam in vitrectomy.Methods: One hundred and sixty patients undergoing vitrectomy were randomized into four groups. The control group was given local anesthetic in retrobulbar nerve block only, while the other three groups received a single dose of 0.1 mg/kg dezocine or/and 0.05 mg/kg midazolam (groups D, M, DM), respectively, at 10 min before retrobulbar block. Mean arterial pressure (MAP), heart rate (HR), and oxygen saturation (SpO(2)) were recorded prior to local anesthesia (Tp), at the start of surgery (T0), 5, 15, 30 min during (T1-T3), and at the end of surgery (T4). Pain at T2 and T4 was assessed by Numeric Rating Scale (NRS). Level of sedation at T2 was assessed using the Ramsay scale. Postoperative nausea and vomiting (PONV) events were recorded for 24 h.Results: Group DM showed a decrease in HR during T0-T4 compared to Tp (p < 0.001). The MAP of all groups decreased significantly at T0 compared to Tp (p < 0.05), but remained unchanged at T1-T4. At T2, all groups have lower pain level than control (p < 0.001), while group DM showed the lowest pain level. At T4, groups D and DM showed a lower pain level than control (p < 0.01). Groups M and DM showed a higher Ramsay score than group C (p < 0.01). Compared with control, group D had a higher PONV rate (p < 0.1). Groups M and DM showed a lower PONV rate than group D (p < 0.0001), and group M had a lower PONV rate than group C (p < 0.01).Conclusion: Combinatory use of dezocine and midazolam in addition to local anesthetic in retrobulbar nerve block can help relief pain and anxiety during vitrectomy and reduce PONV.
Prospective investigation of intravenous patient-controlled analgesia with hydromorphone or sufentanil: impact on mood, opioid adverse effects, and recovery

Abstract:Background Radical surgery for colorectal cancer, associated with moderate to severe postoperative pain, needs multimodal analgesia with opioid for analgesia. Despite considerable advancements, the psychological implications and other side effects with opioid remain substantially unresolved. This study aimed to investigate the impact on mood, side effects relative to opioid, and recovery of the patients with hydromorphone or sufentanil intravenous patient-controlled analgesia (IV-PCA) in a multimodal perioperative analgesia regimen undergoing radical surgery for colorectal cancer. Methods Eighty patients undergoing elective laparoscopic or open radical surgery for colorectal cancer under general anesthesia were randomized to receive postoperative IV-PCA with either sufentanil (group S) or hydromorphone (group H). All patients received additionally flurbiprofen axetil 50 mg 30 min before the end of surgery and wound infiltration with 10 ml of 0.75% ropivacaine at the end of surgery. The primary endpoint was mood changes at 48 and 96 h after surgery. The secondary endpoints were the incidence of opioid-related adverse effects, recovery results and patient satisfaction after surgery. Results Seventy-two patients completed the study finally. There were no significant differences between the two groups with respect to preoperative parameters, surgical and anesthetic characteristics ( P > 0.05). No obvious significant differences were observed in VAS score (at rest and during mobilization) and rescue analgesics use ( P > 0.05). Compared with group S, the anger scores in the group H at 48 h and 96 h after surgery were significantly lower ( P = 0.012 and 0.005; respectively), but the incidences of pruritus and nausea were higher ( P = 0.028 and 0.008; respectively). There were no significant differences in the incidences of vomiting, respiratory depression, dizziness, Ramsay score, and hemodynamic changes between the two groups ( P > 0.05). Moreover, there were no significant differences in the time to gastrointestinal recovery, time to drainage tube removal, time to walk, hospital stay after surgery and patient satisfaction between the two groups ( P > 0.05). Conclusions Under the similar analgesia effect with different opoiods postoperatively, hydromorphone IV-PCA resulted in an improved mood, however, a higher occurrence of pruritus and nausea while compared to sufentanil IV-PCA in a multimodal perioperative analgesia regimen. Both regimens of opioid with IV-PCA may serve as promising candidates for good postoperative pain management, and provide with similar postoperative recovery for the patients undergoing radical surgery for colorectal cancer. Efficacy and safety of dezocine in postoperative pain relief: Systematic review and Meta-analysis

Abstract:Abstract Objective: To systematically evaluate the efficacy index and adverse reactions of dezocine in postoperative pain relief, provide statistical theoretical support for guiding clinical application. Methods: We extracted and analyzed multiple data of patients from the PubMed, Embase, The Cochrane Library and China National Knowledge Infrastructure (CNKI) for use in randomized controlled trials of various surgical postoperative pain relief. We used meta-analysis to study several measures of efficacy and safety of dezocine, including visual analogue score (VAS), Ramsay sedation score, mean arterial pressure (MAP), heart rate (HR), Pulse Oxygen Saturation (SpO2) and the incidence of adverse events(AEs). The material data were calculated and analyzed using Review Manager 5.3. Results: After exclusion of literature that did not meet the inclusion criteria, our analysis included 14 randomized controlled trials. The Mean Difference (MD) of VAS at 1 h/6 h/24h between the dezocine group and the placebo group was -1.37 (95% CI -2.07,-0.67, P=0.0001)，-0.52 (95% CI -1.04,0.01, P=0.05)，-0.10 (95% CI -0.39,0.20, P=0.52), respectively. The MD of Ramsay sedation score at 2h/8h was 1.21 (95% CI 0.67,1.75, P＜0.0001) and -0.17 (95% CI -0.59,0.26, P=0.44). The MD of MAP at T0/T1/T2 was -0.28 (95% CI -2.46,1.89, P=0.80)，-2.66 (95% CI -5.07,-0.25, P=0.03)，-4.53 (95% CI -6.17,-2.89, P＜0.00001). The MD of HR at T0/T1/T2 was -2.26(95% CI -4.32,-0.21, P=0.03)，-3.58(95% CI -5.21,-1.96, P＜0.0001),-3.75 (95% CI -11.55,4.04, P=0.35). The MD of SpO2 at T0/T1 was -0.90(95% CI -1.77,-0.03, P=0.04) and 0.36(95% CI 0.02,0.71, P=0.04).The odds ratio (OR) of AEs was 0.53(95% CI 0.39,0.71, P＜0.0001). Conclusion: Dezocine shows appropriate anesthetic efficacy and fewer adverse effects, which can reduce postoperative pain effectively.
Clinical study on dezocine and sufentanil for postoperative analgesia in upper-abdominal surgery or hip replacement surgery

Abstract:Objective To investigate the effect of dezocine combined with sufentanil on postop-erative analgesia and side effects in the upper-abdominal surgery and hip replacement surgery,and ex-plore the potential mechanisms.Methods One hundred patients scheduled for selective upper-abdomi-nal operation and hip replacement surgery were randomly divided into group dezocine (group D), dezocine 0.3 mg/kg combined with sufentanil 1 μg/kg group(group DS1),dezocine 0.3 mg/kg com-bined with sufentanil 1.5 μg/kg group(group DS2)and dezocine 0.3 mg/kg combined with sufentanil 2 μg/kg group(group DS3).Analgesia was maintained by remifentanil 6-8 μg·kg-1·h-1 under total intravenous anesthesia.Patients were administered 5 μg sufentanil during sewing the skin.Visual Analogue Score (VAS)of both silence and 90°turn over situation,Ramsay score,and adverse effects at 1 h (T1 ),4 h (T2 ),8 h (T3 ),12 h (T4 ),24 h (T5 ),36 h (T6 ),48 h (T7 )after the operation were recorded respectively.Results The total amount of sufentanil and dezocine of group DS1 group showed a significant higher than the other three groups (P <0.05).The VAS in silence of group DS1 were higher than group DS3 at T1-T3 (P <0.05).There was no significant difference in VAS under 90°turn over situation.The side effect of group DS3 were higher than the other three groups (P <0.05).Conclusion Dezocine combined with sufentanil for postoperative patient-controlled intravenous analgesia(PCIA)is effective and safe in patients undergoing upper-abdominal surgery and hip replace-ment surgery,and while dezocine 0.3 mg/kg combined with sufentanil 1.5 μg/kg,it has the best effect of postoperative analgesia and least side effects.
Clinical Observation of Dezocine Combined with Flurbiprofen for Postoperative Analgesia after Abdominal Surgeries: a Prospective Study

Abstract:Objective To investigate the effects of dezocine combined with flurbiprofen for postoperative analgesia after abdominal surgeries.Methods This was a multi-center,prospective, single-blind and controlled clinical study.Two hundred and sixteen patients scheduled for selective upper-abdominal operation were randomly divided into dezocine group(group D) and sufentanil group (group S).Thirty minutes before the end of operation,patients in group D were given 5 mg dezocine and 50 mg flurbiprofen,and then an intravenous pump containing 25 mg dezocine and 250 mg flurbiprofen in a volume of 100 ml was administered at the speed of 2 ml/h for 48 h after operation. For patients in group S,3μg sufentanil were administered before the end of operation and then 3μg/kg sufentanil for continuous infusion.Visual analogue score(VAS) during both silence and 90 turn over situation,Ramsay score,and adverse effects at 1,4,8,12,24,36,48 h after the operation were recorded respectively.Results Compared with group S,patients in group D showed no significant difference in the Ramsay score and VAS pain during silence,but a significant decrease in VAS pain under turn-over was observed(P0.05).Less perspiration,nausea and chill were observed in group D than group S(P0.05).Also,there was no shivering occurred in group D. Conclusion Administration of dezocine combined with flurbiprofen has a preferable therapeutic effect for post-operative movement pain and less adverse effect as compared to sufentanil alone.
Application of dezocine patient-controlled epidural analgesia in postoperative analgesia in patients with total myomectomy

Abstract:BACKGROUND Uterine fibroids are common benign gynecological conditions. Patients who experience excessive menstruation, anemia, and pressure symptoms should be administered medication, and severe cases require a total hysterectomy. This procedure is invasive and causes severe postoperative pain, which can affect the patient’s postoperative sleep quality and, thus, the recovery process. AIM To evaluate use of dezocine in patient-controlled epidural analgesia (PCEA) for postoperative pain management in patients undergoing total myomectomy. METHODS We selected 100 patients undergoing total abdominal hysterectomy for uterine fibroids and randomized them into two groups: a control group receiving 0.2% ropivacaine plus 0.06 mg/mL of morphine and an observation group receiving 0.2% ropivacaine plus 0.3 mg/mL of diazoxide in their PCEA. Outcomes assessed included pain levels, sedation, recovery indices, PCEA usage, stress factors, and sleep quality. RESULTS The observation group showed lower visual analog scale scores, shorter postoperative recovery indices, fewer mean PCEA compressions, lower cortisol and blood glucose levels, and better polysomnographic parameters compared to the control group (P < 0.05). The cumulative incidence of adverse reactions was lower in the observation group than in the control group (P < 0.05). CONCLUSION Dezocine PCEA can effectively control the pain associated with total myomectomy, reduce the negative impact of stress factors, and have less impact on patients’ sleep, consequently resulting in fewer adverse effects.

Effect of Dezocine on Postoperative Pain in Laparoscopic Patients

Abstract:Objective] To explore the effect of dezocine on postoperative pain in laparoscopic patients .[Methods] From January 2012 to January 2014 ,a total of 108 patients undergoing laparoscopic hysterectomy were randomly divided into observation and control groups ( n = 54 each) .At 30 ~ 40 min pre‐surgery ,an in‐travenous injection of 0 .1 mg/kg dezocine (diluted with saline to 10 ml) was administered for observation group while the same volume of saline for control group .At different timepoints ,visual analog scale (VAS) , comfort level (BCS score) ,sedation status (Ramsay score) and postoperative adverse events .[Results] At different timepoints ,the analgesia and comfort scores of observation group were significantly better than those of control group .And the inter‐group differences were statistically significant ( P < 0 .05) .And the postoper‐ative incidence of adverse reactions was significantly lower in observation group than that in control group . And the inter‐group differences were statistically significant ( P < 0 .05) .[Conclusion] For mini‐invasive total uterus removal ,preoperative use of dezocine may effectively relieve 24h postoperative pain better analgesic and sedative effects ,boost patient comfort and reduce the postoperative incidence of adverse reactions .Such a practice is worth wider popularization .
Research of the Treatment Effects on Patient's Visceral Pain by Dezocine in Laparoscopic Cholecystectomy

Abstract:Objective To observe the effectiveness and safety of dezocine in patients with visceral pain after laparoscopic cholecystectomy.Methods 60 patients with ASA Ⅰ ~ Ⅱ in laparoscopic cholecystectomy,randomly divided into 2 groups,30 cases each: group Ⅰ was the control group,and group Ⅱ as test group.2 μg /kg fentanyl was injected into the vein while the gallbladder was removed and then taken out from the cavity in group Ⅰ.And 0.1 mg /kg dezocine in group Ⅱ.The 2 groups were observed the recovery time,the analgesic effect of postoperative body surface pain and visceral pain,the degree of comfort,and the adverse reactions.Results The recovery times were no significant difference between the two groups( P 0.05).The patients of two groups could have good analgesic effect of postoperative body surface pain.There were no significant difference( P 0.05).The patients,visceral pain in group Ⅱ were significantly better than those in group Ⅰ( P 0.05).The patients,comfort scores in group Ⅱ were significantly better than those in group Ⅰ( P 0.05).The adverse reactions occurred in group Ⅱ was significantly lower than that in group Ⅰ( P 0.05).Conclusions With low incidence of adverse reactions,dezocine was used safely and effectively for laparoscopic cholecystectomy visceral pain,and it is an ideal analgesic drug.
Randomized and Double -Blind Controlled Clinical Trial of Dezocine Injection for Analgesia

Abstract:Objective To evaluate the efficacy and safety of dezocine injection for analgesia.Methods A multicenter , randomized , double blind, parallel group controlled study was performed with dezocine as the test drug , and pethidine injection and morphine injection as positive con-trol for the single treatment and 7 days treatment of moderate and severe pain , respectively.The test drug was dezocine injection.One hundred of forty patients with postoperative moderate and severe pain received single treatment with dezocine ( single injection of dezocine , n=70 ) or pethi-dine ( single injection of pethidine and placebo , n =70 ).Fifty -six cancer pain patients received 7 days treatment with dezocine or morphine as follows:the test group was intramuscularly injected two dezocines each time ( n=31 ) , and the control group was injected morphine and moe-phine simulation respectively ( n=25 , four times a day ) .Pain intensity difference ( PID) was evaluated as the primary analgesic effect criteria.Results There was no significant difference in PID between the test group and the control one both in the controlled trail of postoperative pain and that of chronic cancer pain.The most common adverse reactions of dezocine were nausea , vomiting and dizziness , et al.Conclusion The analgesic effect and adverse effects of dezocine were similar to those of morphine , which indicats dezocine is an effective and safe analgesic for postoperative pain and chronic cancer pain that are moderate and severe.

Abstract:Objective To evaluate the psychological states of the patients of hemifacial spasm(HFS) before and after operation. Methods The patients were assessed with Self rating Anxiety Scale(SAS) and Self rating Depression Scale(SDS) before and after operation. It showed that patients had anxious and depression psychological state universally. Result Surgical treatment had good result but the anxious and depression psychological state had no obvious improvement than that before operation. Whereas, the anxious and depression psychological state had obvious improved 6 month after surgery. Conclusion The anxious and depression psychological state can not be significantly improved by operation in a short time after procedure. But the psychological state can get very significant improvement at a specified future date.

Abstract:Objective To analyze subarachnoid block with dexmedetomidine compounding dezocine in total hip replacement surgery(THA) patients.Methods A total of THA patients graded ASAⅠ-Ⅱ were divided into group A (n=40),group B (n=40) and C group (n=40) from June 2015 to June 2016.Group A were implemented with endotracheal intubation operation.Group B were anaesthetized in the gap of line L2-3,and after a fixing,intermixture of 0.2 mg dezocine 3 mL was pumped.Group C were treated with 3 μg of mixed solution of miconazole 3.5 mL.The levels of mean arterial pressure (MAP),heart rate (HR),oxygen saturation (SPO2),end-tidal carbon dioxide (PETCO2) and other hemodynamic parameters and plasma epinephrine (E),norepinephrine (NE),malondialdehyde (MDA) of three groups at 5 mins (T0) before anesthesia,10 mins after anesthesia (T1),at the start of surgery (T2) and 1 h (T3) after completion of anesthesia,the end of the surgery (T4).The anesthetic complications occur of three groups were compared.Results The levels of MAP,HR,SPO2,PETCO2,E,NE and MDA in T1 to T4 time were significantly higher than T0 stage (P<0.05),which of the group A were significantly higher than that of group B and group C.The levels of MAP,HR,SPO2,PETCO2,E,NE and MDA in T1 to T4 time were not statistically significant between group B and group C (P>0.017).The rates of cognitive disorders,nausea,vomiting,restlessness,chills and respiratory depression of group A were significantly higher than group B and group C (P<0.017),and the incidences of chills and respiratory depression of group B were significantly higher than that of groups C (P<0.017).Conclusion Subarachnoid block dexmedetomidine compounding dezocine spinal anesthesia could effectively stabilize hemodynamics THA patients and reduce patient stress and blood vessels,it has lower incidence of postoperative complications,which is worthy of promotion application.

Abstract:BACKGROUND:The incidence of reproductive system tumors in Chinese females is increasing year by year, with the trend of younger onset ages. Laparoscopic surgery has been recognized by the majority of medical staff and patients, but the postoperative pain still exists. Therefore, it has become the focus of attention of medical workers to take effective analgesia measures to alleviate postoperative pain as well as to improve the degree of comfort and satisfaction for the patients. OBJECTIVES:The research objective was to study the effect of different doses of dezocine preemptive analgesia on the safety and the pain post laparoscopic surgeries, in order to explore the best dose of dezocine for postoperative analgesia in gynecological laparoscopic surgeries. METHODS:Gynecological laparoscopic surgery patients conformed to the criteria (n = 390) were randomly divided into three groups (group A, B and C) by the methods of randomized, double-blind studies. 0.1 mg/kg, 0.15 mg/kg or 0.2 mg/kg dezocine was intravenously injected 15 min before surgeries for preemptive analgesia. VAS score, Ramsay score and MMSE score were used to evaluate the efficacy and safety of dezocine in preemptive analgesia and sedation, and the use of adjuvant analgesic drugs and the incidence of adverse reactions were also observed. RESULTS:The VAS scores of the 0.15 mg/kg and 0.2 mg/kg dezocine groups were significantly lower than that of the 0.1 mg/kg group at 2h, 4h, 6h, 8h, 12h and 24h post-surgery, and the difference was statistically significant (p < 0.05). There was no statistically significant difference between the 0.15 mg/kg and 0.2 mg/kg groups (p > 0.05) except for the 12h time point. The MMSE scores 12h post-surgery of the three groups were compared with those 12h prior-to-surgery, and the differences were not statistically significant (p > 0.05) and no increase in the incidence of cognitive impairment was observed. The use rate of analgesic drugs in the 0.1 mg/kg group was significantly higher than those in the 0.15 mg/kg and 0.2 mg/kg groups, and the difference was statistically significant (p < 0.05). There were no significant differences in the incidence of adverse events between the three groups (P > 0.05). CONCLUSIONS:The analgesia and sedation effects of dezocine were enhanced with the increase of usage dose, which suggested that the effects of dezocine were dose-dependent. Intravenous injection of 0.15 mg/kg dezocine 15 min before gynecological laparoscopic surgery showed better analgesic and sedative effects as well as less adverse reactions, and should be the appropriate dose to be used in the preemptive analgesia in gynecological laparoscopic surgeries.
Dezocine as preemptive analgesia alleviates ultrapulse CO2 fractional laser treatment induced pain in patients with acne scars

Abstract:Background Ultrapluse CO2 fractional laser technology has emerged as an effective treatment for scar management. However, one drawback of this modality is the pain caused during the procedure. This study aims to explore the efficacy and safety of dezocine (DZC) as preemptive analgesia for reduction of pain induced by ultrapulse CO2 fractional laser treatment for acne scars. Methods The study cohort included 78 outpatients with acne scars between February and April 2023. Patients were randomly assigned into three groups with intravenous injection (iv) of DZC prior to laser treatment: (1) control, iv of saline; (2) DZC group 1 (DZC_1), iv of DZC at 0.15 mg/kg; and (3) DZC_2, iv of DZC at 0.20 mg/kg. After 30 min, one session of ultrapulse CO2 fractional laser treatment on acne scars was performed. Hemodynamics, visual analogue scale (VAS), and anxiety visual analog test (AVAT) were monitored prior to, during, and after the procedure. Results Operative success rates for patients in the control, DZC_1, and DZC_2 groups were 34.6%, 84.6%, and 100%, respectively. DZC administered with either dosage significantly reduced the VAS and AVAT scores of patients in treatment groups as compared with the subjects in the control group during the course of ultrapulse CO2 fractional laser treatment. Patients in DZC_1 and DZC_2 groups did not show any significant difference in hemodynamic parameters, VAS, and AVAT scores. Temporary adverse effects such as nausea and dizziness were observed in some subjects after treatment; the symptoms were quickly dissolved after a rest in supine position. Conclusions DZC as preemptive analgesia could effectively reduce pain and anxiety induced by ultrapulse CO2 fractional laser treatment in patients. This study provided an option of preemptive anesthesia to minimize the pain and discomforts associated with laser treatments in clinical practices.

[Application of Dezoxine Plus Dexmedetomidine in Strabismus Surgery].

Abstract:OBJECTIVE:To observe the effects of intra-operative combined dosing of dezoxine and dexmedetomidine on sedation, analgesia and the incidence of untoward events during and after surgery in teenagers undergoing horizontal concomitant strabismus surgery.METHODS:For this prospective and randomized trial, approval was obtained from the Ethical Committee and informed consent from their parents. A total of 60 patients aged 10-16 years undergoing strabismus surgery at Second Affiliated Hospital from September to December 2013 were collected. The treatment group received an intravenous dose of dezoxine 0.1 mg×kg(-1) at 15 minutes before surgery. And another intravenous injection of dexmedetomidine was administered at 0.4 µg×kg(-1)×h(-1) until the end of the first ocular muscle correcting. The control group received the same volume of normal saline. Observational parameters including visual analogue scale (VAS), Ramsay scores, self-rating anxiety scale (SAS), heart rate (HR), blood pressure and the incidence of untoward events such as nausea and vomiting or arrhythmia were recorded.RESULTS:The VAS pain scores in the treatment group at T2-T5 decreased significantly versus the control group (P < 0.05) . The Ramsay scores in the treatment group at T2-T3 increased and significantly differences existed with the control group (P < 0.05). SAS scores showed significant differences between pre-operative and post-operative periods in the treatment group (P < 0.05). The treatment group has shorter operative duration with 5 cases of dragging pain in the treatment group versus 25 in the control group (P < 0.05). During dragging reflex, HR was lower in the control group at T2 and T3. There were significant differences with the control group (P < 0.05).CONCLUSION:A combination of dezoxine and dexmedetomidine provide multiple benefits for patients undergoing strabismus surgery. Compared with simple local anesthesia, it effectively attenuates pain and anxiety and provides mild sedation and better cooperation during strabismus surgery.

Abstract:Objective:To observe the analgesic efficacy and safety of dezocine combined with fentanyl for patient-controlled intravenous analgesia(PCIA) for elderly patients with heart disease undergoing non-cardiac surgery operations.Methods: Sixty ASA II or III elderly patients with heart disease undergoing general anesthesia for non-cardiac surgeries were randomized into two groups: the fentanyl group(group F) and the dezocine combined with fentanyl group(group FD).The medicatiors for PCIA ware as follows: in group F,16μg/kg fentanyl with 10mg tropisetron in 100 mL 0.9%NaCl;the group FD,l8μg/kg fentany,0.4mg/kg dezocine and 10mg tropisetronin in 100 mL 0.9%NaCl;in both groups loading dose was 5mL,the infusion rate 2 mL/h,bolus dose was 0.5 mL,and lockout time was 15min.Visual analog score(VAS) of pain and Ramsay sedation score were measured at 4 h,8 h,12 h,24 h,36 and 48 h during the postoperative analgesia.The side effects were recorded for every enrolled patient.Results:In the two groups,the analgesia scorings were all <3 at each time points but without significant difference between the two groups.The Ramsay scores in the group F were significant higher than that of group FD at 8 h,12 h,24 h(P<0.05).Postoperatively the incidence of side effects were lower in the group FD than that of the Group.F(P<0.05).Conclusion: Dezocine combined with Fentanyl produces significant analgesia effection in elderly patients with heart disease after non-cardiac surgery,meanwhile less side effects were observed.

Abstract:Objective To study the effects of dezocine combined with flurbiprofen axetil multimodal analgesia preventing hyperalge-sia caused by using ultra short acting opioid.Methors:Petions of preserving cholelithotomy was administered.Fourty-five patients (Aged be-tween 35 and 45;ASA Ⅰ~Ⅱ)that didn t have obvious important viscera dysfunction,were randomly divided into three groups(D is defined dezocine group;F is defined flurbiprofen group,D+F is defined the combination group),and were given intravenous injection respectively fif-teen minutes before the operation.Patients in group D received 0.06mg/kg dezocine,group F 1mg/kg flurbiprofen axetil,and patiens in group D+F recieved 0.06 mg/kg dezocine and flurbiprofen axetil 1 mg/kg.Then dynamic observation of three groups was conducted in terms of gen-eral anesthesia to stir,pain,nausea or vomiting,and the time from completing surgery to pulling out the tube.Results The pain score of group D+F patients in different time of postoperative 4h significantly lower than in group D and group F(P<0.05);Pain score in 2hour after oper-ation of group D was lower than that in group F(P<0.05),but after 4 hours of scores between the two groups had no statistically significant (P>0.05);The incidence of nausea or vomiting in group D and D +F were more than in group F,which had statistically significant (P<0.05).Conclusion Using dezocine combined with flurbiprofen axetil before operation can effectively prevent the hyperalgesia caused by using remifentanil.

Abstract:Residual hemifacial spasm (HFS) after microvascular decompression (MVD) is common, and the factors associated with residual HFS are still controversial. In the present study, we analyzed the outcome of 212 patients with hemifacial spasm after a single microvascular decompression and evaluated the prognostic factors involved in residual hemifacial spasm. Based on our study, possible prognostic factors included indentation of the root exit zone (REZ), preoperative illness duration, and preoperative psychological state. We suggest that MVD should be performed as early as possible for it may decrease the rate of residual HFS. Preoperative assessment of psychological state in HFS patients is a timely intervention that should be implemented to minimize the residual HFS.
