Abstract:Objective To evaluate the therapeutic effect of comprehensive treatments for radiation-induced lower cranial neuropathy in patients with nasopharyngeal carcinoma (NPC). Meathods Fifty NPC patients with radiation-induced lower cranial neuropathy were randomly divided into comprehensive treatment group and control group. In the former group, the 25 patients received 10 sessions of hyperbaric oxygen treatment in addition to intravenous infusion of methyprednisolone (60-80 mg, once daily for 10 consecutive days) and aceglutamide (500 mg, once daily for 28 days), with also patients in the control group were given 2 compound vitamine B tablets and 3 compound Denshen tablets three times a day for 45 days. Results The excellent effect, improvement, non-response, and exacerbation rates in the comprehensive treatment group were 24%, 72%, 4%, and 0%, as compared to the rates of 0%, 12%, 80%, and 8% in the control group, respectively. The overall response rate in the comprehensive treatment group was 96%, significantly higher than that in the control group (12%, P<0.05). In the patients receiving comprehensive treatment, exclusion of chemotherapy and shorter duration of symptoms of nerve damage were associated with better therapeutic effect (P<0.05). Conclusion Methyprednisolone, mouse nerve growth factor and hyperbaric oxygen treatment can relieve the symptoms of radiation-induced lower cranial neuropathy in NPC patients, and the therapeutic effect of the treatments is associated with chemotherapy and sustaining time of the symptoms of nerve damage, suggesting the necessity of early treatment of neurological impairment in these patinets.

Abstract:The aim of this study is to investigate the status and dose-volume relationship of hypothalamic-pituitary-thyroid dysfunction induced by intensity-modulated radiotherapy (IMRT) for adult patients with nasopharyngeal carcinoma (NPC). The hormone levels of 98 patients treated with IMRT were retrospectively reviewed. Hormone values including prolactin (PRL), follicle-stimulating hormone (FSH), luteinizing hormone, adrenocorticotropic hormone, thyroid-stimulating hormone (TSH) and free thyroxine (FT4) were tested. The pituitary and thyroid glands were re-contoured to obtain dose-volume histograms. The relationship between the dose distributions and hormone values was analyzed, as well as combined chemotherapy, age, gender, stages and interval time. The median follow-up period was 17 months. Median values of mean radiation dose to the pituitary and thyroid were 51.2 and 49.72 Gy. Evidences of hormone disorder were found in 54.1 % patients. About 33.7 % patients experienced dysfunction of TSH, 20.4 % of sexual axis and 11.2 % of PRL. Twenty-nine patients presented central hypothyroidism, while 4 patients developed primary hypothyroidism. Dosimetric analysis showed that thyroid axis was significantly associated with Vt30-50 and Dmean of thyroid, V40 (p = 0.000) was the most significant parameters. In addition, TSH and FT4 were correlated with volume of pituitary receiving above 55 Gy (Vp55; p = 0.014, 0.035). Vp55 was most significantly influencing factor of PRL axis (p = 0.044) and gonad axis (p = 0.047). TSH dysfunction was more common with interval time longer than 10 months and age older than 45 years in both univariate and multivariate analysis. FSH and PRL were affected by both gender (p = 0.009, 0.001) and age (p = 0.004, 0.012). Hormone changes were not affected by either clinical stages or combined chemotherapy. At the era of IMRT, a high rate of endocrine deficiency was still seen. Hypothalamic-pituitary-thyroid dysfunction is more common in NPC patients. Regular monitoring is needed.

Abstract:[Abstra ct] Objective To investigate the long-term efficacy and adverse effects of intensity-modulated radiotherapy (IMRT) for nasopharyngeal carcinoma (NPC).Methods A total of 869 patients with biopsy-proven NPC without distant metastasis who underwent the whole course of IMRT from 2009 to 2010 were enrolled.Of all the patients, 84.8%received cisplatin-based chemotherapy.The prescribed dose to the primary lesion in the nasopharynx was 66-70Gy in 30-32 fractions, and the dose to the positive lymph nodes in the neck was 66 Gy in 30-32 fractions.The Kaplan-Meier method was used to calculate survival rates, the log-rank test was used for difference analysis and univariate prognostic analysis , and the Cox proportional hazards model was used for multivariate prognostic analysis .Rseu lts The 5-year overall survival( OS ) , local recurrence-free survival, regional recurrence-free survival, distant metastasis-free survival, and disease-free survival ( DFS ) were 84.0%, 89.7%, 94.5%, 85.6%, and 76.3%, respectively.In the patients with locally advanced NPC,concurrent chemotherapy tended to reduce distant metastasis (83.6%vs.75.7%, P=0.050) and improve OS (82.6%vs.77.0 %, P=0.082).Induction chemotherapy tended to improve OS ( 80.7% vs.71.4%, P=0.057 ) , and the induction chemotherapy containing docetaxel or gemcitabine tended to improve OS (83.3%vs.72.2%, P=0.058).The patients who received a boost after the initial radiotherapy had a significantly lower DFS rate than those who did not (52.2%vs.71.1%, P=0.004).The concurrent chemotherapy increased the incidence rates of long-term xerostomia and trismus, while a high dose of cisplatin increased the incidence rates of xerostomia and hearing impairment.Conclusions IMRT for NPC provides satisfactory long-term efficacy.Concurrent chemotherapy combined with IMRT tends to reduce the incidence of distant metastasis, and other values need further investigation.The boost therapy after radiotherapy may be associated with poor prognosis.Chemotherapy increases the incidence of long-term toxicities.
Prognostic factors of 305 nasopharyngeal carcinoma patients treated with intensity-modulated radiotherapy.

Abstract:Radiotherapy is effective in treating nasopharyngeal carcinoma (NPC). This study evaluated the treatment efficacy, toxicity, and prognostic factors of intensity-modulated radiotherapy (IMRT) in the treatment NPC.Between September 2003 and September 2006, 305 patients with NPC were treated with IMRT in Fujian Provincial Cancer Hospital. IMRT was delivered as follows: gross tumor volume (GTV) received 66.0-69.8 Gy in 30-33 fractions, high-risk clinical target volume (CTV-1) received 60.0-66.65 Gy, low-risk clinical target volume (CTV-2) and clinical target volume of cervical lymph node regions (CTV-N) received 54.0-55.8 Gy. Patients with stages III or IV disease also received cisplatin-based chemotherapy. All patients were assessed for local-regional control, survival, and toxicity.With a median follow-up of 35 months (range, 5-61 months), there were 16, 8, and 39 patients who had developed local, regional, and distant recurrence, respectively. The 3-year rates of local control, regional control, metastasis-free survival, disease-free survival, and overall survival were 94.3%, 97.7%, 86.1%, 80.3%, and 89.1%, respectively. Multivariate analyses revealed that T-classification had no predictive value for local control and survival, whereas N-classification was a significant prognostic factor for overall survival (P < 0.001), metastasis-free survival (P < 0.001), and disease-free survival (P = 0.003). For stages III-IV disease, concurrent and adjuvant chemotherapy did not influence prognosis. The most severe acute toxicities included Grade III mucositis in 14 patients (4.6%), Grade III skin desquamation in 90 (29.5%), and Grades III-IV leucocytopenia in 20 (6.5%). There were 7% patients with Grade II xerostomia after 2 years of IMRT, no Grades 3 or 4 xerostomia was detected.IMRT provided favorable locoregional control and survival rates for patients with NPC, even in those with locally advanced disease. The acute and late toxicities were acceptable. N-classification was the main factor of prognosis. Further study is needed on chemotherapy for patients with NPC.
Predictors of Radiation-induced Hypothyroidism in Nasopharyngeal Carcinoma Survivors after Intensity-modulated Radiotherapy

Abstract:Abstract Background The aim of the study is to identify clinical and dosimetric factors that could predict the risk of hypothyroidism in nasopharyngeal carcinoma (NPC) patients following intensity-modulated radiotherapy (IMRT). Methods A total of 404 non-metastatic NPC patients were included in our study. All patients were treated with IMRT. The thyroid function were performed for all patients before and after radiation at regular intervals. The time onset for developing hypothyroidism was defined as the time interval between the completion of RT and the first recorded abnormal thyroid hormone test. The cumulative incidence rates of hypothyroidism were estimated using Kaplan–Meier method. Univariate and multivariate Cox regression analyses were performed to detect the most promising factors that were associated with hypothyroidism. Results Median follow up was 60.6 months. The 3-, 5- and 7- year cumulative incidence rate of hypothyroidism was 39.4%, 49.1% and 54.7%, respectively. The median time to primary hypothyroidism and central hypothyroidism were 15.4 months (range 2.9–83.8 months) and 29.9 months (range 19.8–93.6 months), respectively. Univariate and multivariate analyses revealed that younger age, female gender and small thyroid volume were the most important factors in predicting the risk of hypothyroidism. Dtmean (mean dose of thyroid), V30-V50 (percentage of thyroid volume receiving a certain dose level) and VS45-VS60 (the absolute volumes of thyroid spared from various dose levels) remained statistically significant in multivariate analyses. Cutoff points of 45 Gy (Dtmean), 80% (Vt40) and 5 cm3 (VS45Gy) were identified to classify patients as high-risk or low-risk group. Conclusion Thyroid Vt40 highly predicted the risk of hypothyroidism after IMRT for NPC patients. We recommended plan optimization objectives to reduce thyroid Vt40 to 80%. Intracavity Hyperthermia In Nasopharyngeal Cancer: A Phase Iii Clinical Study

Abstract:Methods and materials: Previously untreated NPC patients were assigned randomly into the conventional RT group and the hyperthermia group. In addition to curative RT, hyperthermia group patients received intracavity hyperthermia before or after RT; T90 was 42.5 degrees A degrees--43 degrees A degrees C for 50 min twice a week for 7 weeks.Results: From August 2001 to July 2006, 180 eligible patients with NPC were enrolled in this study. The complete response (CR) rate in the two arms (RT plus hyperthermia versus conventional RT) was 95.6%% and 81.1%%, respectively (p == 0.003, chi chi<SU2</SU test). CR rates for T2 and T3 patients in the hyperthermia group were 97.1%% and 96.9%%, respectively, while in the conventional RT group they were 79.5%% and 76.7%%, respectively. The difference between the two groups was statistically significant (p == 0.03 and p == 0.024, respectively). The 5-year local control rate was 91.1%% and 78.9%% for the two arms, respectively (p == 0.022). Oral mucous toxicity in both arms was comparable. The 5-year PFS and 5-year OS rate for the hyperthermia arm vs. the conventional arm were 72.7%% versus 63.1%% (p == 0.039) and 78.2%% versus 70.3%% (p == 0.14), respectively.Conclusions: Conventional RT treatment followed by intracavity hyperthermia was well tolerated by the NPC patients. The addition of hyperthermia improved the local tumour control, and our results indicated a positive impact on PFS of NPC patients.
Long-term efficacy of hyperfractionated intracavitary brachytherapy on 69 patients with locally recurrent nasopharyngeal carcinoma

Abstract:Background and purpose:Local recurrence is one of treatment failure cause for nasopharyngeal carcinoma(NPC).Further treatments for locally recurrent NPC are available,such as surgery,chemotherapy,targeted therapy and radiotherapy is the most important of all.There are several radiotherapy methods,including conventional external beam,brachytherapy,three-dimensional conformal and intensity modulated radiotherapy.This study retrospectively analyzed the long-term outcomes and late radiation injuries of locally recurrent NPC that were treated with hyperfractionated intracavitary brachytherapy or combined with external beam radiotherapy for the purpose of assessing the value of hyperfractionated intracavitary brachytherapy.Methods:Sixty-nine patients were enrolled to restaged T1(51 cases) and T2(18 cases) of locally recurrent NPC.Twenty patients received hyperfractionated intracavitary brachytherapy with a median dosage of 50 Gy(ranged 40-50 Gy).The remaining 49 patients received external beam radiotherapy with a median dosage of 50 Gy(ranged 30-65 Gy) plus hyperfractionated intracavitary brachytherapy with the median dose of 20 Gy(ranged 8-48 Gy) boosted.Two courses of brachytherapy were delivered,each consisting of 2.5-3.0 Gy per fraction,each of the 2 fractions were spaced 6 hours apart and lasting for 5 days.The second course of brachytherapy was delivered after one week of rest.Results:The 3 and 5-year overall survival rates were 59.66% and 39.66%.The 3 and 5-year local tumor control rates were 86.3% and 77.2%,respectively.After the reirradiation,the incidence rates for nasopharyngeal ulceration,trismus,lower cranial nerve palsy and radiation encephalopathy were 26.1%,39.1%,30.4% and 13.0%,respectively.Cases of trismus and nasopharyngeal ulceration in the brachytherapy group versus in the combined group were 4 versus 23(P=0.038) and 6 versus 12(P=0.636),respectively.Conclusion:Hyperfractionated intracavitary brachytherapy is feasible for locally recurrent nasopharyngeal carcinoma that is restaged early.The 5-year overall survival and local tumor control rates were 39.66% and 77.2%.Hyperfractionated intracavitary brachytherapy can be recommended for rT1 and rT2 locally recurrent nasopharyngeal carcinoma.

Abstract:BACKGROUND & OBJECTIVE:Intracavitary hyperfractionated brachytherapy is one of the main methods in brachytherapy for early stage nasopharyngeal carcinoma (NPC). We developed the parapharyngeal space interstitial brachytherapy for the NPC patients with residual tumor in the parapharyngeal space after external radiotherapy, and used hyperfractionated brachytherapy as a boosting radiation for the locally advanced patients who have no tumor invasion in the base of skull. This study was to analyze the long-term efficacy of external radiotherapy plus intracavitary hyperfractionated brachytherapy on NPC, and explore the clinical value and suitable dose.METHODS:From Jan. 1998 to Dec. 2002, 352 NPC patients were treated by external radiotherapy plus intracavitary hyperfractionated brachytherapy in Fujian Provincial Tumor Hospital. The parapharyngeal space interstitial brachytherapy was used in the patients with residual tumor in the parapharyngeal space after external radiotherapy. The external irradiation dose varied from 50 to 70 Gy. Brachytherapy was delivered by 2.5-3.0 Gy per fraction, and 2 fractions per day with an interval of 6 h; the total dose ranged from 5 to 32 Gy with a median of 17 Gy.RESULTS:The overall 1-, 2-, 3-, and 5-year disease-free survival rates were 97.0%, 91.3%, 87.6%, and 84.7%, respectively. The 5-year survival rate was significantly higher in stage I-II patients than in stage III-IV patients (88.2% vs. 79.2%, P=0.016)û but the local control rate was similar in the 2 groups (94.1% vs. 91.7%). Thirty-two (9.4%) patients had radiation injury at the lower cranial nerve.CONCLUSION:External radiotherapy plus intracavitary and interstitial hyperfractionated brachytherapy can improve local control rate and survival rate of NPC, but the occurrence rate of radiation injury at the lower cranial nerve is higher in the patients with parapharyngeal space involvement than in other patients.

Abstract:Purpose: Radiotherapy is the most effective treatment for nasopharyngeal carcinoma (NPC). The aim of this study is to evaluate the efficacy and toxicity of fractionated stereotactic body radiation therapy (SBRT) boost for NPC. Methods and materials: Sixty-four patients with newly diagnosed, nonmetastatic NPC were treated with conventional radiotherapy 64.8-68.4 Gy followed by fractionated SBRT boost 12-15 Gy between January 2002 and July 2004. Most patients (72%) presented with Stage III-IV disease. Fifty-two patients also received cisplatin-based concurrent (38) or neoadjuvant (14) chemotherapy. The major endpoints were local control, overall survival, and complications. Results: All patients finished the planned dose of radiotherapy. After a median follow-up of 31 months (range, 22-54), 15 patients developed tumor recurrences--3 in the nasopharynx, 4 in the neck, 5 in distant sites, 1 in both nasopharynx and neck, 2 in the neck and a distant site. The 3-year actuarial rate of local control was 93.1%, regional control 91.4%, freedom from distant metastasis 90.3%, and overall survival 84.9%, respectively. There were no Grade 4 acute or chronic radiation-related complications. Conclusions: Fractionated SBRT boost for NPC is technically feasible and provides good local control without any severe complications.
Analysis of intensity-modulated radiotherapy for untreated patients with nasopharyngeal carcinoma

Abstract:Objective To investigate the clinical effects and toxicity of intensity-modulated radiotherapy(IMRT) for untreated nasopharyngeal carcinoma(NPC) patients.Methods Seventy-four NPC patients receiving IMRT were enrolled from January 2005 to April 2011.The prescription dose was given 69.9 Gy /30f to the nasophaxynx gross tumor volume(GTVnx),69.9Gy /30f to positive neck lymph nodes(GTVnd),60Gy /30f to the first clinical target volume(CTV1) and 54Gy /30f to the second clinical target volume(CTV2).The acute and late radiation injury were evaluated according to RTOG/EORTC criteria.Kaplan-Meier method was used to calculate the overall survival rate(OS),distant metastasis-free survival rates(DMFS) and loco-regional control rates from the last day of therapy.Results The median follow-up time was 39 months(range 13 to 80).The 1-,2-,3-and 5-year survival rates were 97.3%,93.8%,88.2% and 77.2% respectively;1-,2-,3-and 5-year DMFS were 93.2%,91.1%,91.1% and 84.1% and the 1-,2-,3-and 5-year loco-regional control rates were 98.6%,91.0%,86.8% and 80.6%,respectively.The statistically significant difference in OS was found in different N stage(P < 0.05),rather than for both clinical stage and T stage(P > 0.05).Out of 12 failure patients,there were 6 with recurrence at the primary site(50.0%),2 patients in the neck(16.7%),and 7 patients(58.3%) by distant metastases.Among the 74 cases,3 cases had grade 3 xerostomia,one case had radio encephalopathy,four cases had grade 3 trismus.No patients suffered cranial nerves palsy.Conclusion The IMRT technique with the prescription dose of 69.9 Gy /30f to GTV provides favorable long-term effects with acceptable toxicity in NPC patients without distant metastasis.N stage is a prognostic factor for NPC patients.
Therapeutic effect of radiotherapy and chemotherapy in 202 elderly patients with nasopharyngeal carcinoma

Abstract:Objective To retrospectively analyze the outcomes,adverse events,and prognostic factors in elderly patients initially diagnosed with nasopharyngeal carcinoma (NPC) who receive radiotherapy and chemotherapy.Methods From January 2004 to April 2007,202 patients (over 60 years of age) initially diagnosed with NPC received radical radiotherapy or chemotherapy.These patients included 165 males (81.7％) and 37 females (18.3％),with a median age of 65 years (range 60-86 years).Of all patients,1 had stage Ⅰ NPC,25 had stage Ⅱ NPC,120 had stage Ⅲ NPC,and 56 had stage Ⅳa/Ⅳb NPC,according to the AJCC staging system (6th edition).All patients received a radiation dose of ≥66 Gy,and 146 patients (72.3％) with locally advanced NPC also received chemotherapy.The Cox model was used for multivariate analysis.Results The follow-up rate was 100％.The 5-year local control rate,regional lymph node control rate,distant metastasis-free survival (DMFS) rate,disease-free survival rate,and overall survival (OS) rate were 93％,94％,82％,74％,and 67％,respectively.The multivariate analysis revealed that T stage,N stage,and age were prognostic factors for OS and DMFS (P =0.006,0.002,and 0.008 ;P =0.020,0.002,and 0.022).The severe acute toxicities included grade 4 radiation dermatitis (1 patient,0.5％),grade 4 radiation mucositis (2 patients,1.0％),and grade 4 bone marrow suppression (5 patients,2.5％).Conclusions The elderly NPC patients can tolerate radiotherapy and chemotherapy well.Multimodality therapy should be adopted for the patients aged 60-74 years who have advanced-stage NPC and good general conditions.
Thyroid V40 is a good predictor for subclinical hypothyroidism in patients with nasopharyngeal carcinoma after intensity modulated radiation therapy: a randomized clinical trial

Abstract:Hypothyroidism (HT) and subclinical HT after radiotherapy is frequent in nasopharyngeal carcinoma (NPC) patients, results in negative impact on patients' quality of life. The percentage of thyroid volume receiving more than 40 Gy (V40) ≤ 85% was reported to be a useful dose constraint to adopt during intensity-modulated radiation therapy (IMRT) planning. This study aims to verify whether V40 ≤ 85% can be used as an effective dose constraint in IMRT planning in a randomized clinical trial.

Triptolide Combined with Radiotherapy for the Treatment of Nasopharyngeal Carcinoma via NF-κB-Related Mechanism

Abstract:Advanced nasopharyngeal carcinoma (NPC) has a poor prognosis because of the lack of an effective treatment. Here we explored the efficiency and the molecular mechanisms of combined treatment with triptolide and ionizing radiation for treating NPC. Human nasopharyngeal carcinoma (CNE) cells were treated with triptolide, ionizing radiation, or triptolide plus ionizing radiation in vitro. Tumor potency was examined in an in vivo CNE cell xenograft mouse model, which was treated as above. Our results demonstrated that triptolide caused a significant reduction in cell growth and colony number, and induced a marked apoptosis that was further enhanced with increasing doses of ionizing radiation. Combination treatment synergistically reduced tumor weight and volume without obvious toxicity. Western blot analysis in vitro and in vivo showed that triptolide induced apoptotic protein Bax expression and inhibited phosph-NF-κB p65, Bcl-2 and VEGF proteins without affecting other NF-κB related protein expression. In conclusion, our findings revealed that triptolide plus ionizing radiation had synergistic anti-tumor and anti-angiogenesis effects in NPC via down-regulating NF-κB p65 phosphorylation. The combination therapy may provide novel mechanism insights into inhibit NPC.
Study on the radiotherapy effect and serum neutral granulocyte lymphocyte ratio and inflammatory factor expression of nasopharyngeal carcinoma

Abstract:Purpose To compare target area delineation schemes in intensity-modulated radiotherapy (IMRT) effect on patients with locally advanced nasopharyngeal carcinoma (NPC). Methods A total of 88 NPC patients received IMRT and were assigned into control group (n = 44) and observation group (n = 44) based on MRI and CT imaging. In the control group, the treatment range was determined as the clinical target volume (CTV) as the gross tumor volume (GTV) + 5 mm. In the observation group, high-risk target areas CTVp1 was GTVp + 5 mm, lymphatic drainage area CTVn1 was GTVn + 5 mm, medium-risk CTVp2 was CTVp1 + 5 mm margin + the whole nasopharyngeal area, CTVn2 was CTVn1 + 5 mm. Radiotherapy treatment course was 6–8 weeks, four times a week. Results The observation group had higher total effective rate, with fewer complications. Neutrophil lymphocyte ratio (NLR), interleukin (IL)-6, and tumor necrosis factor (TNF)-α levels were lower after radiotherapy in both groups compared to before radiotherapy, with the observation group demonstrating lower levels than the control group. The effective group exhibited lower serum NLR, IL-6, and TNF-α compared to the non-effective group. T stage, target location, serum NLR, IL-6, and TNF-α were risk factors for the effect of radiotherapy. Conclusions Serum NLR, IL-6, and TNF-α have predictive significance for radiotherapy effect.

Re-irradiation with Moderate Hypo-fractionation Using Intensity Modulated Photon or Proton Radiation Therapy in Locally Recurrent Squamous Cell Carcinoma of Nasopharynx

Abstract:Purpose: This study aimed to analyze the treatment outcomes of locally recurrent nasopharyngeal cancer (NPC) patients following moderate hypo-fractionation re-irradiation (re-RT). Materials and methods: Sixty locally recurrent NPC patients underwent hypo-fractionation re-RT. Forty-eight point three percentage had rT3-4, and 30.0% did keratinizing squamous cell carcinoma. Intensity-modulated radiation therapy (IMRT), with or without intensity-modulated proton therapy (IMPT), was used in 66.7% of patients. Results: With the median follow-up of 22 months (range, 2 to 254 months), 31 patients (51.7%) died, 38 (63.3%) developed further treatment failure, and 30 (50.0%) developed ≥ grade 3 toxicity (including seven grade 5) at time of analysis. The 2- and 5-year rates of overall survival, local failure-free survival, and ≥ grade 3 toxicity-free survival were 57.9% and 45.8%, 64.1% and 52.5%, and 54.8% and 44.9%, respectively. In multivariate analyses, worse factors for overall survival (OS) were iT3-4 (p=0.010) and age at re-RT ≥ 53 years (p=0.003), those for local failure-free survival (LFFS) were rT3-4 (p=0.022) and rN0-1 (p=0.035), and those for toxicity-free survival (TFS) were iT3-4 (p=0.020) and re-IMRT/IMPT (p=0.030), respectively. Cumulative dose or fraction size ≥ 3 Gy at re-RT, however, showed no significance for OS, LFFS and TFS. Conclusion: Current re-RT with modern RT techniques by moderate hypo-fractionation scheme seemed feasible in treating locally recurrent NPC patients.
Nasopharyngeal Carcinoma Treated with Intensity Modulated Radiation Therapy Using A Reduced Clinical Target Volume for the Neck Nodal Regions

Abstract:To evaluate the efficacy of intensity-modulated radiation therapy (IMRT) using reduced clinical target volumes for regional lymph nodes (CTV-N) in the treatment of nasopharyngeal carcinoma (NPC). From January 2006 to December 2010, a total of 1,208 patients with NPC treated with IMRT according to our institutional protocol were collected in the current study. The CTV-N was contoured according to DAHANCA, EORTC, GORTEC, NCIC, and RTOG consensus guidelines but the level I, the retrostyloid space, and the medial retropharyngeal nodes below the cranial edge of the second cervical vertebra (C2) were spared. Patients with locoregionally advanced disease were also prescribed cisplatin-based chemotherapy. With a median follow-up of 57 months (range, 2-102 months), 33 patients had developed regional failure, of which, 22 patients encountered recurrence in the irradiation field and only 11 patients (1%) experienced relapse outside the irradiation field. The 5-year overall survival, distant metastasis-free survival, regional relapse-free survival, and local relapse-free survival were 80.8%, 82.6%, 96.8%, and 93.2% respectively. Multivariate analyses revealed that the N-classification was the only significant prognostic factor for predicting regional relapse-free survival. The IMRT approach using reduced CTV-N sparing the level I, the retrostyloid space, and the medial retropharyngeal nodes below the cranial edge of C2 for NPC is suitable.
Effectiveness And Toxicities Of Intensity-Modulated Radiotherapy For Patients With Locally Recurrent Nasopharyngeal Carcinoma

Abstract:Purpose: To analyze the effectiveness and toxicities in the re-irradiation of locally recurrent nasopharyngeal carcinoma (NPC) using intensity-modulated radiotherapy (IMRT).Methods: This is a retrospective analysis of 54 NPC patients with local recurrence re-irradiated with IMRT. The re-staging for rT1, rT2, rT3, rT4 were 3 (5.6%), 8 (14.8%), 9 (16.7%), 34 (63%) respectively. The average dose to GTV was 69.95 Gy (49.8-76.58 Gy), the average BED3Gy was 116.8 Gy (83.5-127.9 Gy). V-95 was 96%, and D-95 was 65.75Gy. 33.3% of them received concurrent chemoradiotherapy.Results: Median overall survival (OS) was 21 months (1-93 mon). The 1-, 2-year local progression free survival (LPFS) rate was 84.5%, 64% and OS rate was 71.7%, 44.3%. Severe late adverse events (SLAE) occurred in 48.1% of patients, including 31.5% with ulcer or necrosis of the nasopharyngeal mucosa, 20.4% with difficulty in feeding, 18.5% with temporal lobe necrosis, 11.1% with massive hemorrhage. 15.4% died of local regional progression, 5.8% died of distant metastasis, 25% died of SLAE, 9.6% died of both local regional progression and SLAE that could not be differentiated, 5.8% died of other medical complications. Concurrent chemoradiotherapy was the independent negative prognostic factors for LPFS; PTV>100 ml was a predictive factor of poor OS; patients with invasion of post-styloid space were at higher risk of SLAE.Conclusions: The present study demonstrated that IMRT with 70Gy was efficient for local tumor control. However, we observed a high frequency of serious late complications. More optimized combination treatment and patient selection are required to achieve excellent local control without significant late morbidities in locally recurrent NPC.

Abstract:Objective To evaluate the primary results with using intensity modulated radiation therapy(IMRT) in the primary treatment of nasopharyngeal carcinoma(NPC) and to observe the normal tissue toxicities.Methods From August 2003 to March 2006,230 newly diagnosed NPC patients were treated with IMRT in Fujian tumor hospital.According to the Fuzhou Staging Classification,the disease was staged Ⅰ in 1(0.4%),stage Ⅱ in 36(15.7%),stage Ⅲ in 113(49.1%) and stage Ⅳa in 80(34.8%).The target volumes of nasopharyngeal and upper neck node area were treated with IMRT.The mean dose delivered to target area was 67.5 Gy(65.1~74 Gy),each time was 2.10～2.25 Gy.The lower neck node area and supraclavicular fossae were irradiated with the conventional technique by anterior field.Results With a median follow up of 12 months(range 2~32 months),6 patients died(4 died of distant metastasis),5 patients relapse in the primary site,2 patients relapse in the neck and 16 patients showed distant metastasis.The 1-,2-year overall survival rates,local relapse-free survival,regional relapse-free survival and distant metastasis-free survival were 100%,94.6%,98.4%,91.4%,99.1%,(97.9%) and 91.9%,85%,respectively.Univariate analysis showed that the N classification was a signicant factor for distant metastasis-free survival(P=0.04).The worst acute side effect was mucositis,grade 1 in(22.6%),graed 2 in 44.3%,grade 3 in 28.7%,grade 4 in 0.9%.Rates of long-term xerostomia were(33.7%) in grade 1 and 7.3% in grade 2.Conclusion(IMRT) is a effective methods for NPC,which achive higher rate of local regional control and less toxicity.Distant metastases is the predominant cause of failure.
Long-term survival and late complications of intensity-modulated radiotherapy for recurrent nasopharyngeal carcinoma

Abstract:Background: To evaluate the effectiveness and toxicities of intensity-modulated radiotherapy (IMRT) for locally recurrent nasopharyngeal carcinoma (NPC). Methods: One hundred and eighty-four previously irradiated NPC patients with recurrent disease and re-irradiated by IMRT between February 2005 to May 2013 had been reviewed. The disease was re-staged I in 33, II in 27, III in 70 and IV in 54 patients. Seventy-five percent of the patients received cisplatin-based chemotherapy. Results: The median survival time was 33 months. The 3-year actuarial rates of local recurrence-free survival (LRFS), distant metastases-free survival (DMFS), and overall survival (OS) rates were 85.1, 91.1, and 46.0%, respectively. About 53% of the patients experienced Grade 3-4 late toxicities. Forty-four patients died of massive hemorrhage of the nasopharynx caused by radiation induced mucosal necrosis. Multivariate analysis indicated that chemotherapy and time interval between initial radiotherapy and re-irradiation were independent predictors for DMFS. Conclusion: IMRT is an effective method for patients with locally recurrent NPC. Massive hemorrhage of the nasopharynx is the major sever late complication and also the leading cause of death. Early recurrence is negative factor for DMFS. Combination of chemotherapy can improve DMFS, but not for OS. Optimal salvage treatment strategies focusing on improvement of survival and minimization of late toxicities are warranted.
