Abstract:BACKGROUND:The aesthetic repair of complete traumatic cleft earlobe is a common procedure for the plastic surgeon. In this article, a new surgical technique for the repair of complete traumatic cleft earlobe using a 3-flap method was introduced.METHODS OR DESIGN:We shared the experience of treating patients with complete traumatic cleft earlobe by using the 3-flap method in this article. The patients were followed up for >3 months. The reconstructed earlobes were aesthetically pleasing and retained a smooth margin in all cases without prospective complications.CONCLUSIONS:This method not only increases the longitudinal length of the cleft in the earlobe, but also regulates the tissue quantity of the reconstructed earlobe and avoids extension of suture line.

Abstract:Introduction: Congenital cleft earlobe is a common congenital deformity of earlobe. Various techniques have been described for its repair, but cosmetic deformity usually persists. We present our result using triangular flap repair for simple longitudinal cleft of earlobe. Materials and Methods: The procedure was done in five cases who presented between 2017 and 2020. The technique uses the same principles as in Randall-Tennison triangular flap repair for cleft lip. Results: Earlobes were near-symmetrical, postoperative scar minimal and patient satisfaction was very good in all cases. Ear piercing was done without any complications at least 6 months after surgery. After a follow-up of 1 year, no complications were noted. Conclusions: Triangular flap repair technique for cleft earlobe follows the principles of Randall-Tennison triangular flap repair for cleft lip. It provides very good results with esthetic contour, minimal scar with no complications in case of simple longitudinal cleft.

Abstract:Objective: This article discusses a treatment technique for ectopic earlobe in microtia reconstruction using a delayed postauricular skin flap. Methods: From January 2015 to September 2018, microtia reconstruction using a delayed postauricular skin flap was performed on 10 patients with microtia. During the operation, the position of the affected-side earlobe was designed according to the position of the opposite side. The ectopic earlobe was corrected by performing asymmetric Z-plasty or using an irregular "Y-V" advancement skin flap. This allowed full use of the remnant tissue of the microtia-affected ear to form a soft, plump, and realistic earlobe. Results: After the earlobe displacement surgery, the blood supply of the earlobe was good, and the reconstructed earlobe survived well without ulceration in all 10 patients. All patients were followed up for 2 months to 2 years. During this time, the positions of the reconstructed ear and the healthy side were basically symmetrical, and the shape of the earlobe formed by the remnant ear was natural. Conclusion: The use of asymmetric Z-plasty or an irregular "Y-V" advancement skin flap for the treatment of ectopic earlobe in microtia reconstruction using a delayed postauricular skin flap is a safe and effective method.

Abstract:Ear, Nose &Throat Journal, Ahead of Print. Background: Question mark ear is a rare congenital deformity, mainly characterized the interruption of the natural continuity between the lower border of the helix and the earlobe. In severe cases, the earlobe may be absent. In addition, there may be protrusion and outward expansion of the upper part of the auricle, with partial or complete disappearance of the antihelix. This article aims to introduce a technique that combines autologous costal cartilage carving with auricular cartilage folding to achieve a stable and aesthetic auricle. Method: This study included 26 patients with sporadic question mark ear deformity who were treated at our clinical center from January 2020 to December 2022. Based on the different appearances of the lower part of the auricle, they were divided into 2 categories:11 cases showed a natural continuity interruption between the helix and the earlobe, while 15 cases showed the absence of the earlobe. All patients underwent corrective surgery using costal cartilage transplantation combined with the upper part of the auricular cartilage folding, performed by senior surgeons. Results: Question mark ear was effectively improved and with no significant rebound. The average follow-up period was 8.4 months (ranging from 6 to 12 months). A satisfaction survey showed that 23 patients (88%) were satisfied, 3 patients (12%) were partially satisfied, and no patients were dissatisfied. Most patients experienced temporary swelling after surgery, which resolved within 3 months to half a year. Conclusion: Autologous costal cartilage transplantation combined with folding of the auricular cartilage is an ideal surgical method to correct question mark ear.

Auricular reconstruction for microtia: personal 6-year experience based on 350 microtia ear reconstructions in China

Abstract:Background: Favorable results in auricular reconstruction are difficult to achieve. The authors report 350 patients with different types of microtia who underwent total ear reconstruction. The youngest was aged 5.5 years and the oldest was aged 50 years. Methods: The authors performed auricular reconstruction using autologous costal cartilage with the combination of the Brent and Nagata methods, with some modifications. At the first stage, an individualized framework fabrication based on different degrees of strength and thickness of the rib cartilages was performed. One piece of cartilage was added vertically under the reconstructed tragus to enhance conchal depth to provide a more prominent appearance. At the second stage, the elevated reconstructed ear was supported by bone cement with a special shape that was 2 mm wider at the end of two poles, giving the reconstructed ear more projection on the upper and lower poles. This external ear morphology occurs quite often in China. Results: In this series, the follow-up time in 322 patients ranged from 8 months to 6 years. Two hundred eighty-eight patients were satisfied with the results, including good shape, accurate size, right orientation, and duplication of more than 10 well-detailed structures. Surgery-related complications such as broken helix, skin necrosis, infection, blunted convolution, and extrusion of cartilage occurred in 13 cases and hypertrophic scars occurred in 21 patients. Conclusions: The authors' techniques produce acceptable results and fewer complications. This 6-year experience confirms the reliability, versatility, and reproducibility of this type of combined technique in auricular reconstruction, especially for Asian patients.

Abstract:Backgrounds "Flat ear" is a subtype of Grade II conchal-type microtia characterized by severely underdeveloped conchal bowl. Traditional surgical techniques often rely on autologous costal cartilage, which poses several risks and complications. This study aimed to introduce a novel surgical technique using autologous ear cartilage in auricular deformity correction surgery with simultaneous conchal bowl reconstruction for "flat ear". Methods A total of 19 patients were involved in this single-center, retrospective cohort study. All patients underwent the described surgical technique. The data before the surgery, immediately after the surgery, and at the last follow-up were collected, including length, width, perimeter of auricle and depth of conchal bowl. Patient satisfaction was assessed via the Visual Analog Scale (VAS). Results The study included 12 males and 7 females, with an average age of 10.67 ± 3.86 years. Postoperative results showed significant improvements in auricular measurements, with the affected ear achieving near symmetry with the normal ear. The mean conchal bowl depth was restored from a preoperative 4.33±1.78 mm to 17.32±1.28 mm postoperatively. VAS scores for patient satisfaction increased significantly from 1.44±0.92 preoperatively to 7.72±1.49 postoperatively, with stability observed at the last follow-up. Conclusion The novel technique offers a promising alternative to traditional costal cartilage-based auricular deformity correction surgery, providing excellent aesthetic outcomes and high patient satisfaction. This approach may expand treatment options for patients with "flat ear" and related auricular deformities, with reduced invasiveness and potential for future auditory rehabilitation.

Abstract:PurposeEar reconstruction is a challenging operation with a steep learning curve. In view of its rarity, attaining a high standard for new surgeons is extremely difficult. This study describes the author’s experience of 53 ear reconstructions using costal cartilage for congenital and post-traumatic ear deformity.MethodsThe author performed 53 autologous ear reconstructions for microtia and post-traumatic ear defect over a period of 5 years utilizing the two-stage technique popularized by Firmin in most of the cases. An assessment of complications, pattern of progress and aesthetic outcome of the reconstructed ears was carried out.ResultsThere were 4 cases of partial skin necrosis. In early cases, deficiencies were seen in the proportions of the reconstructed ear and the quality of definition. Better shape and definition were evident as more surgical experience was gained. This occurred as a result of increased appreciation of the ear proportions and improved framework carving.ConclusionsThe series demonstrates the early learning curve in microtia reconstruction and underlines the importance of appropriate training and case availability in achieving high-quality results in autologous ear reconstruction.
Ear Reconstruction for Auricle Defects Using an Expanded Neck Flap: A Case Report

Abstract:Ear, Nose &Throat Journal, Ahead of Print. This case report describes a patient with a left ear deformity resulting from a flame burn sustained 20 years ago. The patient underwent an ear reconstruction procedure utilizing an expanded neck flap. The autologous rib cartilage was used as the framework, while an expanded neck flap served as the covering for the framework. The surgery was completed in 3 stages. Initially, tissue expanders were implanted and gradually inflated with water. After sufficient expansion, the expanders were removed, and the scar tissue was excised. Subsequently, the expanded flap was used to cover the defects, and the expanded neck flap was rotated to cover the autogenous costal cartilage framework obtained intraoperatively. Finally, the reconstructed ear was repaired by constructing the cephaloauricular sulcus, removing postauricular scars, and trimming the neck-flap pedicle. After a 1-year follow-up, the wound had healed satisfactorily with only minor complications. The shape of the reconstructed ear appeared realistic, and its function was maintained. Most of the scars were repaired, and the scarred alopecia was significantly improved. In patients with limited availability of the postauricular flap, especially burn patients, using an expanded neck flap can lead to superior outcomes.

Abstract:Background Auricular reconstruction is one of the most challenging procedures in the field of plastic surgery. The aims of this study were to apply the modified 2-stage method to perform auricular reconstruction and to summarize the clinical experience in the past 10 years. Methods Auricular reconstruction was performed in 243 patients (total 254 ears) of congenital microtia using autologous costal cartilage. The whole procedure is divided into 2 stages. Stage I: the individualized framework fabricated with autologous costal cartilage is inserted into subcutaneous pocket in the mastoid region, and then the earlobe was transposed backward to connect with the lower part of the framework. The second-stage surgery mainly includes the following main contents: ear elevation, fixing the bracket behind the reconstructed ear framework, harvesting the retroauricular fascial flap to cover the bracket, and closing the defect with free skin grafts. Results A total of 243 patients (254 ears) of congenital microtia underwent ear reconstruction. The follow-up time ranged from 6 months to 4 years; 220 patients were satisfied with the results. Surgery-related complications such as infection, partial skin graft necrosis, flap necrosis, bad projection of the constructed auricle, and extrusion of cartilage occurred in 24 cases, and hypertrophic scars occurred in 16 patients. Conclusions This modified 2-stage method for auricle reconstruction can receive acceptable results and fewer complications; furthermore, it is relatively simple and easy to master. The 10-year experience validates that this modified method is an ideal method in auricular reconstruction.

Abstract:BACKGROUND: Reconstruction of Grade III microtia is a challenging entity in maxillofacial esthetic rehabilitation. Several advocacies and philosophies exist in this field. The aim of the manuscript is to present a single South Indian Experience with Ear reconstruction among South Indian Population.MATERIALS AND METHODS: Retrospective analysis of unilateral Grade III microtia reconstruction was performed. Using a set of predefined inclusion and exclusion criteria, the population was selected. Outcome measures in terms of the ear size, auriculocephalic angle, and the conchal depth were measured in the reconstructed and normal side. Descriptive statistics is presented.RESULTS: Twenty-four patients formed the study group and had undergone the classical two-stage reconstruction in a similar fashion. The mean ear size in normal side was 65.8 ± 2.8 mm whereas on the reconstructed side, it was 61.3 ± 5.8 mm. The center's technique achieved above 75% similarity as that of the other normal ear. The mean auriculocephalic angle was 44.6 ± 5.2° whereas for the surgically reconstructed ear, it was 41.9 ± 2.6°. Overall, in these patients, we achieved a 79.94% similarity of auriculocephalic angle in the reconstructed ear as compared to the normal auricle. The conchal depth was 19.2 ± 2.1 mm and 16.6 ± 1.9 mm for normal and reconstructed ear, respectively. In terms of conchal depth, the present study group showed an achievement of 82.88% of accuracy even after a prolonged follow-up.DISCUSSION AND CONCLUSION: The center employs a classic two stage reconstruction with a customized prosthesis that helps to avoid the loss of projection geometry and minimizes adhesion, infection, and early loss of structural stability.

Abstract:Auricular reconstruction is challenging for plastic surgeons because of its rarity and technical complications.<a class="ejp-citation-link js-ejp-citation-link">[1,2]</a> During development over 60 years, many surgical techniques have been explored and have produced good effects; these techniques include the Tanzer method, Brent method, Nagata's two-stage method, and tissue expansion method.<a class="ejp-citation-link js-ejp-citation-link">[3]</a> The key to successful auricular reconstruction is to replicate the elaborate ear framework and ensure sufficient soft tissues for coverage. However, auricular reconstruction is often associated with undesirable esthetic outcomes because of the postoperative complications, the patient's individual condition, and the surgeon's poor skills. In this report, we introduced several strategies in salvage procedures. Improvement of an unfavorably reconstructed ear usually involves correcting the position, increasing the height, remolding the contour, and blending the color tones. If the superior part of the reconstructed ear is pitched forward, the ear should be mobilized and rotated back into the correct position or a full-thickness skin graft should be added to the face anteriorly, if necessary.<a class="ejp-citation-link js-ejp-citation-link">[2]</a> If the position of the reconstructed lobule is higher or lower than that of the opposite ear, an incision can be made in the upper pole of the cranioauricular sulcus to adjust the framework and fix the scaffold on the cranial periosteum.<a class="ejp-citation-link js-ejp-citation-link">[4]</a> Patients who have previously undergone external auditory meatoplasty might exhibit an inferoposterior position deformity of the reconstructed ear with indirect separation of the auricle and canal orifice. Ji et al<a class="ejp-citation-link js-ejp-citation-link">[5]</a> and Zhang et al<a class="ejp-citation-link js-ejp-citation-link">[6]</a> described an ear-forward technique in which an inverted U-shaped purse was used to change the high anterior location of the external auditory canal orifice along with the use of rotation flaps to relocate the ear position.

New Strategies for Remnant Ear Treatment in Lobule-Type Microtia Reconstruction

Abstract:Background: Numerous modifications have been successfully applied in microtia reconstruction over the past several decades, and lobular transposition has become a routine technique in a two-stage operation. However, it is still a demanding challenge to manipulate the remnant ear with various quantities or positions in lobule-type microtia. The authors demonstrate the relevant details of treating remnant tissue in different conditions. Methods: A total of 390 lobule-type microtia patients underwent autogenous costal cartilage auricular reconstruction between 2010 and 2015. Because the quantity or position of the remnant ear varies from patient to patient, corresponding tactics need to be taken into consideration for appropriate treatment. Four possible scenarios of remnant ear shape have been described, and relevant strategies for each were introduced. Results: Favorable reconstructed auricles with aesthetic natural earlobes have been produced; the location of the reconstructed ear is symmetric to the contralateral ear. It provides a harmonized framework and periphery tissue for the integrity of the contour of the constructed auricle. Conclusions: Proper use of the remnant ear along with framework fabrication and skin flap dissection are critical factors for attaining a satisfactory contour of the auricle in microtia reconstruction. Modification of two-flap method and framework construction for reconstruction of atypical congenital auricular deformities

Abstract:A single-stage two-flap method utilizing an anteriorly based subcutaneous pedicled skin flap and a mastoid fascial flap has been applied successfully for reconstruction of lobule-type microtia. This paper presents three modifications of the two-flap method, with which atypical congenital auricular deformities necessitating both framework construction and coverage were reconstructed successfully. In middle and upper auricular deformities such as a concha-type or a scapha-type microtia, a cranially based postauricular skin flap and lower mastoid fascial flap were used. For upper auricular deformities such as microtia representing lop-ear deformity, a narrow cranially based postauricular skin flap (Grotting flap) and upper mastoid fascial flap were used. For lower auricular deformities such as total absence of the earlobe, a cranially based skin flap and lower mastoid fascial flap were utilized. In each deformity the framework was totally or partially constructed with autogenous costal, conchal, or septal cartilage or their combination according to the size or shape of the defect. Fifty-two atypical auricular deformities were corrected with the modified two-flap method and framework construction. The follow-up has ranged from 4 months to 3 years (average 18 months). No major surgical complications occurred in this series. Representative cases reconstructed with each modified method are shown. While the shape of the cartilage framework is sculptured according to the type and degree of deformity in each patient, the modified two-flap method not only adequately covers the cartilage but also preserves the fabricated framework in its natural contour and position in the diverse varieties of atypical congenital auricular deformities.

Abstract:Prominauris, often known as prominent ears, is one of the most frequent congenital deformities of the head and neck. Therefore, otoplasty has gained significant popularity and is now one of the most performed procedures in the field of facial plastic surgery. Reconstructing and correcting ear abnormalities remains a challenging procedure in facial plastic and reconstructive surgery. These deformities, whether congenital or acquired, have a substantial influence on patients' lives and inflict psychological harm. Addressing the deformity and tissue loss will improve the overall quality of life. Throughout history, various surgical techniques have been reported for treating external ear abnormalities. While most of these surgeries are considered safe, a comprehensive preoperative evaluation and understanding of the different techniques and risks can minimize potential complications. Abstract:Annals of Otology, Rhinology &Laryngology, Ahead of Print. Background:The defects of the upper third of the auricle are considered significant reconstructive challenges, as they require frequent operations with a high risk of morbidity at the donor site and result in unacceptable cosmetic abnormalities.Objective:Is to perform the reconstruction of a full-thickness auricular defect located in the upper third of the ear using a conchal cartilage graft with postauricular flap coverage, aiming to minimize both donor and recipient morbidity.Patients and Methods:The current study included 20 patients with unilateral upper-third auricular defects. The repair involved 2 components: a cartilage graft from the concha to provide structural support and a flap for coverage. Follow-up was conducted for 6 months after the operation.Results:Successful outcomes were achieved in both subjective and doctors' assessments. Regarding subjective outcomes, 85% of the patients reported high satisfaction (P < .001). In terms of doctors' subjective assessment, 90% of the patients had excellent results (P < .001). Mild early and postoperative complications, if encountered, resolved spontaneously.Conclusion:The use of a combined conchal cartilage graft and postauricular flap in treating a full-thickness upper third auricular defect is safe and effective, with no major complications. The technique preserves the cosmetic and functional outcomes of the auricle, providing an excellent color match and minimal donor-site morbidity.

Abstract:Abstract Introduction The ear is a protruding appendage with multiple functional and aesthetic implications. Literature indicates that up to 40-60% of facial burns involve the ear. Ear burns with considerable tissue loss and sensory deficits can negatively impact quality of life, psychosocial functioning, and physical health. Successful ear reconstruction mitigates these undesirable outcomes. The complex architecture of the external ear presents a formidable surgical challenge after burn injury, when scar tissue, impaired blood supply, and trauma to cartilage all influence reconstructive options. A lack of materials that truly replicate the characteristics of uninjured elastic cartilage also presents a longstanding surgical dilemma. In this retrospective study, the authors highlight the utility of reconstructive techniques to address significant cartilage deficits, including conchal transposition flap, composite graft, costal cartilage graft, and porous polyethylene implant. Methods A retrospective review was conducted on patients aged 0 to 21 years who underwent cartilage framework reconstruction between January 2004 to January 2021 at a specialized pediatric burn center. Medical records from the hospital’s patient database were screened, and 52 patients (60 ears) who met study criteria were identified. Patient demographics, procedural characteristics, and patient outcomes were analyzed. Results For helical rim cartilage defects, 20 patients (23 ears) with an average age of 15 ± 4 years underwent a conchal transposition flap, which was associated with no major complications. In cases involving repair of small to medium cartilage deficits, 9 patients (9 ears) with an average age of 13 ± 5 years underwent a composite graft, which was associated with one case of infection. A total of 20 patients (23 ears) with an average age of 13 ± 6 years underwent porous polyethylene implantation, which was associated with two cases of exposure and one case of infection. Of these porous polyethylene cases, 20 ears involved helical rim reconstruction and 3 involved total ear reconstruction. Costal cartilage grafting was performed in 4 patients (5 ears) with an average age of 13 ± 5 years and was associated with one case of infection. Costal cartilage grafting was utilized to reconstruct 2 helical rims and 3 total ears. Conclusions In cases of focal cartilage defects or medium-sized helical rim cartilage loss, highly aesthetic results and minimal complication rates can be achieved with composite graft or conchal transposition flap. When presented with large to total helical rim loss or total ear loss, either costal cartilage graft or porous polyethylene implantation is typically necessary.

Auricular reconstruction of congenital microtia: personal experience in 225 cases

Abstract:SUMMARY Microtia is a congenital disease with various degrees of severity, ranging from the presence of rudimentary and malformed vestigial structures to the total absence of the ear (anotia). The complex anatomy of the external ear and the necessity to provide good projection and symmetry make this reconstruction particularly difficult. The aim of this work is to report our surgical technique of microtic ear correction and to analyse the short and long term results. From 2000 to 2013, 210 patients affected by microtia were treated at the Maxillo-Facial Surgery Division, Head and Neck Department, University Hospital of Parma. The patient population consisted of 95 women and 115 men, aged from 7 to 49 years. A total of 225 reconstructions have been performed in two surgical stages basing of Firmin's technique with some modifications and refinements. The first stage consists in fabrication and grafting of a three-dimensional costal cartilage framework. The second stage is performed 5-6 months later: the reconstructed ear is raised up and an additional cartilaginous graft is used to increase its projection. A mastoid fascial flap together with a skin graft are then used to protect the cartilage graft. All reconstructions were performed without any major complication. The results have been considered satisfactory by all patients starting from the first surgical step. Low morbidity, the good results obtained and a high rate of patient satisfaction make our protocol an optimal choice for treatment of microtia. The surgeon's experience and postoperative patient care must be considered as essential aspects of treatment.

Abstract:Despite various surgical techniques for ear elevation in autogenous cartilage microtia reconstruction, it is still challenging for plastic surgeons to obtain a satisfactory depth of the cephaloauricular sulcus and stable projection of the reconstructed ear. Here, the authors demonstrate individualized options for surgical approaches and relevant details for complication management. Between January 2014 and June 2020, a series of 895 patients who underwent the second stage of microtia reconstruction were reviewed. Complications occurred in 103 patients aged between 8 and 34 years. Recommended surgical selections, as well as appropriate strategies for complication prophylaxis and treatment, were shown to minimize the negative influence on the contour of the cephaloauricular sulcus according to individual conditions. We found that 78% of the patients were satisfied with the auricle contour with harmonious integrity. Individualized strategies for ear elevation and complication treatment contribute to symmetry and satisfactory projection of the reconstructed auricle.

Abstract:Objectives: Complications, including framework exposure, infections, and reconstructed auricle deformation, may occur after auricular reconstruction. However, reports on surgical methods for cases with unsatisfactory outcomes after auricular reconstruction using an autologous costal cartilage are insufficient. Herein, we summarized retreatment casesfor poor ear morphology in patients who had undergone auricular reconstruction in our department for 5 years and discussed other techniques. Methods: Between September 2014 and September 2019, 24 ears of 24 patients with poor morphology, unsatisfactory macroscopic characteristics and anatomical structures, and unsatisfactory outcomes of local repair after auricular reconstructive surgery were treated. Patients were divided into the following three groups: type 1 (9 ears), with intact and sufficient hairless skin in the mastoid region behind the reconstructed ear; type 2 (7 ears), with intact, but insufficient, hairless skin in the mastoid region behind the reconstructed ear; and type 3 (8 ears), with hairless skin in the mastoid region behind the reconstructed ear with impaired skin integrity. Results: Twenty-two (91.6%) patients successfully completed the surgical treatment and recovered well; one experienced delayed wound healing and another developed hypertrophic scarring at the incision site at 3 months postoperatively. All patients were followed for 0.5-4 (mean, 2.8) years. The macrostructure of the reconstructed ear post-revision was stable and significantly improved in terms of morphology and structure. Conclusions: In patients with unsatisfactory outcomes after auricular reconstruction, the appropriate technique for the revision surgery should consider the local soft tissue conditions of the reconstructed ear to obtain satisfactory results.
