Abstract:Background: Hemodialysis (HD) impacts glucose and insulin metabolism and increases hypoglycaemia rates. Glucose-containing dialysate decreases the risk, but studies using CGM metrics are lacking. Methods: Prospective observational study of insulin-treated adults with T2D, receiving hemodialysis (dialysate glucose 100mg/dl), at least thrice weekly. Subjects were instructed to wear a Dexcom G6-Pro for 10 days. We assessed CGM metrics (mean glucose, %TIR, %TAR, %TBR, and hypoglycemic and hyperglycemic rates) during three time periods: 20-hours before (PreHD), during (HD, ~4 hours), and 20-hours after (PostHD) hemodialysis sessions. Results: Among 56 subjects (mean age 57.3±9, HbA1c 7.2±1.4), mean glucose, %TAR, %TIR and rates of hypoglycaemia <70mg/dl were significantly better during HD. %TAR >180mg/dl and > 250mg/dl were significantly higher after HD, see Table 1. Other hypoglycaemia metrics were overall low, and not different in relation to HD timing. Conclusion: CGM patterns demonstrated improved glycemic control during dialysis, with tendency for higher glucose metrics before HD. Hypoglycaemia metrics were low, while severe hyperglycemia excursions are common across all HD-related periods. Future studies using newer CGM technology and providing longer duration of CGM monitoring are needed for subjects with T2D treated by HD. Abstract:Abstract Background and Aims End-stage kidney disease (ESKD) is a global health problem, with a steep increase in its prevalence. Diabetes is a major cause and common comorbidity in patients with ESKD. As already known, glycemic control is an important factor in preventing micro- and macrovascular complications in diabetes. Besides, glycemic variability has recently been considered an important factor in the treatment of diabetes. However, both hypoglycemia and hyperglycemia can frequently occur in patients with diabetes undergoing hemodialysis. This study aimed to determine the role of continuous glucose monitoring (CGM) for glycemic control and glycemic variability stabilization in patients with diabetes undergoing hemodialysis. Method Eighteen patients aged ≥18 years with type 1 or 2 diabetes and ≥3 months on hemodialysis at the Eulji Medical Center, Daejeon, Republic of Korea between November 2021 and May 2022 were included. Patients underwent 7-day CGM twice: the baseline study period (T0) and the follow-up study period (T1), at a 12-week interval. Physicians modified the treatment strategy according to the T0 results, and then patients conducted T1. iPro2 CGM (MMT-7745) and Enlite glucose sensors MMT-7008A (Medtronic Minimed, Northridge, CA, USA) were used to assess glycemic control. As indicators of glycemic control, the mean glucose levels and glycated hemoglobin A1c (HbA1c) were measured. As indicators of glycemic variability, standard deviation (SD) and %coefficient variation (%CV) were measured. Results Data from 18 patients were analyzed. The mean age was 62.0±11.2 years, male/female ratio was 13 (72.2%)/5 (27.8%). The mean dialysis duration was 5.2±3.5 years. Among the 18 patients, only one had type 1 diabetes, and the mean diabetes duration was 22.9±7.0 years. The mean glucose levels decreased from 179.1±42.3 mg/dL during T0 to 153.2±25.6 mg/dL during T1 (P = 0.001). HbA1c decreased from 7.4±1.3% during T0 to 6.9±1.2% during T1 (P = 0.023). SD improved from 55.7±19.8 mg/dL during T0 to 42.6±15.3 mg/dL during T1 (P = 0.001). %CV improved from 30.5±7.3% during T0 to 25.5±5.5% during T1 (P < 0.001). During T0, the mean glucose level was significantly lower on a day with hemodialysis than on a day without (P≪0.05), and SD and %CV were significantly higher on a day with hemodialysis than on a day without (P≪0.05). After the physicians modified the treatment according to the T0 results, the mean blood glucose levels decreased on both the hemodialysis-on and hemodialysis-off days, as compared to those values during T0 (from 174.7±46.5 to 154.6±29.7 mg/dL, and from 184.7±50.5 to 156.3±28.7 mg/dL, respectively), and consequently there were no statistically significant differences between the hemodialysis-on and hemodialysis-off days during T1 (P = 0.638). SD improved on both the hemodialysis-on and hemodialysis-off days compared to those values during T0 (from 46.9±21.5 to 33.2±14.6, and from 39.5±20.6 to 32.3±13.3, respectively), and consequently, there were no statistically significant differences between the hemodialysis-on and hemodialysis-off days during T1 (P = 0.384). %CV improved from 27.8±10.9 on the hemodialysis-on day during T0 to 21.4±9.2 on that during T1, and there was no statistically significant difference between the hemodialysis-on and hemodialysis-off days during T1 (P = 0.166). Conclusion CGM could be a promising tool for individualizing treatment strategies in patients with diabetes undergoing hemodialysis.

Abstract:Continuous glucose monitoring (CGM) facilitates the assessment of short-term glucose variability and identification of acute excursions of hyper- and hypo-glycemia. Among 37 diabetic hemodialysis patients who underwent 7-day CGM with the iPRO2 device (Medtronic Diabetes, Northridge, CA, USA), we explored the accuracy of glycated albumin (GA) and hemoglobin A1c (HbA1c) in assessing glycemic control, using CGM-derived metrics as the reference standard. In receiver operating characteristic (ROC) analysis, the area under the curve (AUC) in diagnosing a time in the target glucose range of 70–180 mg/dL (TIR70–180) in <50% of readings was higher for GA (AUC: 0.878; 95% confidence interval (CI): 0.728–0.962) as compared to HbA1c (AUC: 0.682; 95% CI: 0.508–0.825) (p 250 mg/dL (TAR>250) in >10% of readings did not differ from that of HbA1c (AUC: 0.854; 95% CI: 0.699–0.948) (p = 0.16). GA (AUC: 0.712; 95% CI: 0.539–0.848) and HbA1c (AUC: 0.740; 95% CI: 0.570–0.870) had a similarly lower efficiency in detecting a time below target glucose range < 70 mg/dL (TBR 1% of readings (p = 0.71). Although the mean glucose levels were similar, the coefficient of variation of glucose recordings (39.2 ± 17.3% vs. 32.0 ± 7.8%, p < 0.001) and TBR<70 (median (range): 5.6% (0, 25.8) vs. 2.8% (0, 17.9)) were higher during the dialysis-on than during the dialysis-off day. In conclusion, the present study shows that among diabetic hemodialysis patients, GA had higher accuracy than HbA1c in detecting a 7-day CGM-derived TIR70–180 < 50%. However, both biomarkers provided an imprecise reflection of acute excursions of hypoglycemia and inter-day glucose variability.

Abstract:Objective: In the general population, continuous glucose monitoring (CGM) provides convenient and less-invasive glucose measurements than conventional self-monitored blood glucose and results in reduced hypo-/hyperglycemia and increased time-in-target glucose range. However, accuracy of CGM versus blood glucose is not well established in hemodialysis patients. Research design and methods: Among 31 maintenance hemodialysis patients with diabetes hospitalized from October 2020-May 2021, we conducted protocolized glucose measurements using Dexcom G6 CGM versus blood glucose, with the latter measured before each meal and at night, plus every 30-min during hemodialysis. We examined CGM-blood glucose correlations and agreement between CGM versus blood glucose using Bland-Altman plots, percentage of agreement, mean and median absolute relative differences (ARDs), and consensus error grids. Results: Pearson and Spearman correlations for averaged CGM versus blood glucose levels were 0.84 and 0.79, respectively; Bland-Altman showed the mean difference between CGM and blood glucose was ∼+15 mg/dL. Agreement rates using %20/20 criteria were 48.7%, 47.2%, and 50.2% during the overall, hemodialysis, and nonhemodialysis periods, respectively. Mean ARD (MARD) was ∼20% across all time periods; median ARD was 19.4% during the overall period and was slightly lower during nonhemodialysis (18.2%) versus hemodialysis periods (22.0%). Consensus error grids showed nearly all CGM values were in clinically acceptable zones A (no harm) and B (unlikely to cause significant harm). Conclusions: In hemodialysis patients with diabetes, although MARD values were higher than traditional optimal analytic performance thresholds, error grids showed nearly all CGM values were in clinically acceptable zones. Further studies are needed to determine whether CGM improves outcomes in hemodialysis patients.
Continuous glucose monitoring and use of alternative markers to assess glycemia in chronic kidney disease

Abstract:Objective: In chronic kidney disease (CKD), glycated albumin and fructosamine have been postulated to be better biomarkers of glycemic control than HbA1c. We evaluated accuracy, variability, and covariate bias of three biomarkers (HbA1c, glycated albumin, and fructosamine) compared with continuous glucose monitoring (CGM)-derived measurement of glycemia across estimated glomerular filtration rate (eGFR) in type 2 diabetes. Research Design and Methods: Prospective cohort study of 104 participants with type 2 diabetes, 80 with eGFR <60 mL/min/1.73m 2 (not treated with dialysis) and 24 frequency-matched controls with eGFR ≥60 mL/min/1.73m 2 . Participants wore a blinded CGM for 2 six-day periods separated by two weeks, with blood and urine collected at the end of each CGM period. HbA1c, glycated albumin and fructosamine were measured by HPLC, enzymatic, and colorimetric nitroblue tetrozolium methods, respectively. Results: Within-person biomarker values were strongly correlated between the two CGM periods (r=0.92-0.95), although no marker fully captured the within-person variability of mean CGM glucose. All markers were similarly correlated with mean CGM glucose (r = 0.71-77). Compared with mean CGM glucose, glycated albumin and fructosamine were significantly biased by age, BMI, serum iron concentration, transferrin saturation, and albuminuria; HbA1c was under-estimated in those with albuminuria. Conclusions: Glycated albumin and fructosamine were not less variable than HbA1c at a given mean CGM glucose level, with several additional sources of bias. These results support measuring HbA1c to monitor trends in glycemia among patients with eGFR <60 mL/min/1.73m 2 . Direct measurements of glucose are necessary to capture short-term variability.

Glucose variability in maintenance hemodialysis patients with type 2 diabetes: Comparison of dialysis and nondialysis days

Abstract:Introduction: Hemodialysis (HD) induces several physiological changes that can affect plasma glucose levels in patients with diabetes and in turn their glycemic control. Studies using continuous glucose monitoring (CGM) to assess glucose variations on dialysis days compared with nondialysis days report conflicting results. Here, we used CGM to examine glucose variations induced by HD in patients with type 2 diabetes. Methods: Patients with type 2 diabetes undergoing maintenance HD were included. CGM (Ipro2®, Medtronic) was performed at baseline and Week 4, 8, 12, and 16 for up to 7 days at each visit. CGM profiles on days where participants received HD were compared with days without HD using a linear mixed model. Findings: Twenty-seven patients were included. The median number of CGM days performed was 8 (interquartile range [IQR] 6-10) for dialysis days and 16 (IQR 12-17) for nondialysis days. The median sensor glucose was 9.4 (95% confidence interval [CI] 8.8-10.2) mmol/L on dialysis days compared with 9.5 (95% CI 8.9-10.2) mmol/L on nondialysis days (p = 0.58). Nocturnal mean sensor glucose was higher on dialysis days compared with nondialysis days: 8.8 (95% CI 8.0-9.6) mmol/L versus 8.4 (95% CI 7.7-9.2) mmol/L (p = 0.029). Discussion: Similar median sensor glucose values were found for days on and off HD. Nocturnal glucose levels were modestly increased on dialysis days. Our findings indicate that antidiabetic treatment does not need to be differentiated on dialysis versus nondialysis days in patients with type 2 diabetes undergoing maintenance HD.

Abstract:Diabetic kidney disease (DKD) is a common complication of diabetes mellitus that contributes to the risk of end-stage kidney disease (ESKD). Wide glycemic var-iations, such as hypoglycemia and hyperglycemia, are broadly found in diabetic patients with DKD and especially ESKD, as a result of impaired renal metabolism. It is essential to monitor glycemia for effective management of DKD. Hemoglobin A1c (HbA1c) has long been considered as the gold standard for monitoring glycemia for > 3 months. However, assessment of HbA1c has some bias as it is susceptible to factors such as anemia and liver or kidney dysfunction. Continuous glucose monitoring (CGM) has provided new insights on glycemic assessment and management. CGM directly measures glucose level in interstitial fluid, reports real-time or retrospective glucose concentration, and provides multiple glycemic metrics. It avoids the pitfalls of HbA1c in some contexts, and may serve as a precise alternative to estimation of mean glucose and glycemic variability. Emerging studies have demonstrated the merits of CGM for precise monitoring, which allows fine-tuning of glycemic management in diabetic patients. Therefore, CGM technology has the potential for better glycemic monitoring in DKD patients. More research is needed to explore its application and management in different stages of DKD, including hemodialysis, peritoneal dialysis and kidney transplantation.

Abstract:Background: Glycated haemoglobin A 1c (HbA 1c ) has limitations as a glycemic marker for patients with diabetes and CKD and for those receiving dialysis. Glycated albumin is an alternative glycemic marker, and some studies have found that glycated albumin more accurately reflects glycemic control than HbA 1c in these groups. However, several factors are known to influence the value of glycated albumin including proteinuria. Continuous glucose monitoring (CGM) is another alternative to HbA 1c . CGM allows one to assess mean glucose, glucose variability, and the time spent in hypo-, normo-, and hyperglycemia. Currently, several different CGM models are approved for use in patients receiving dialysis; CKD (not on dialysis) is not a contraindication in any of these models. Some devices are for blind recording, while others provide real-time data to patients. Small studies suggest that CGM could improve glycemic control in hemodialysis patients, but this has not been studied for individual CKD stages. Summary: Glycated albumin and CGM avoid the pitfalls of HbA 1c in CKD and dialysis populations. However, the value of glycated albumin may be affected by several factors. CGM provides a precise estimation of the mean glucose. Here, we discuss the strengths and limitations for using HbA1c, glycated albumin, or CGM in CKD and dialysis population. Key Messages: Glycated albumin is an alternative glycemic marker but is affected by proteinuria. CGM provides a precise estimation of mean glucose and glucose variability. It remains unclear if CGM improves glycemic control in the CKD and dialysis populations.

Abstract:Abstract Diabetic nephropathy is currently the leading cause of end-stage kidney disease. The present methods of assessing diabetes control, such as glycated hemoglobin or self-monitoring of blood glucose, have limitations. Over the past decade, the field of continuous glucose monitoring has been greatly improved and expanded. This review examines the use of continuous glucose monitoring in people with end-stage kidney disease treated with hemodialysis (HD), peritoneal dialysis (PD), or kidney transplantation. We assessed the use of both real-time continuous glucose monitoring and flash glucose monitoring technology in terms of hypoglycemia detection, glycemic variability, and efficacy, defined as an improvement in clinical outcomes and diabetes control. Overall, the use of continuous glucose monitoring in individuals with end-stage kidney disease may improve glycemic control and detection of hypoglycemia. However, most of the published studies were observational with no control group. Moreover, not all studies used the same assessment parameters. There are very few studies involving subjects on peritoneal dialysis. The small number of studies with limited numbers of participants, short follow-up period, and small number of manufacturers of continuous glucose monitoring systems are limitations of the review. More studies need to be performed to obtain more reliable results. Graphical abstract

Abstract:In developed countries, diabetes is the leading cause of chronic kidney disease (CKD) and accounts for 50% of incidence of end stage kidney disease. Despite declining prevalence of micro- and macrovascular complications, there are rising trends in renal replacement therapy in diabetes. Optimal glycemic control may reduce risk of progression of CKD and related death. However, assessing glycemic control in patients with advanced CKD and on dialysis (G4-5) can be challenging. Laboratory biomarkers, such as glycated haemoglobin (HbA 1c ), may be biased by abnormalities in blood haemoglobin, use of iron therapy and erythropoiesis-stimulating agents and chronic inflammation due to uraemia. Similarly, glycated albumin and fructosamine may be biased by abnormal protein turnover. Patients with advanced CKD exhibited heterogeneity in glycemic control ranging from severe insulin resistance to ‘burnt-out’ beta-cell function. They also had high risk of hypoglycaemia due to reduced renal gluconeogenesis, frequent use of insulin and dysregulation of counterregulatory hormones. Continuous glucose monitoring (CGM) systems measure glucose in interstitial fluid every few minutes and provide an alternative and more reliable method of glycemic assessment, including asymptomatic hypoglycaemia and hyperglycaemic excursions. Recent international guidelines recommended use of CGM-derived Glucose Management Index (GMI) in patients with advanced CKD although data are scarce in this population. Using CGM, patients with CKD were found to experience marked glycemic fluctuations with hypoglycemia due to loss of glucose and insulin during haemodialysis (HD) followed by hyperglycemia in the post-HD period. On the other hand, during peritoneal dialysis, patients may experience glycemic excursions with influx of glucose from dialysate solutions. These undesirable glucose exposure and variability may accelerate decline of residual renal function. Although CGM may improve the quality of glycemic monitoring and control in populations with CKD, further studies are needed to confirm the accuracy, optimal mode and frequency of CGM as well as their cost-effectiveness and user-acceptability in patients with advanced CKD and dialysis.

Hemodialysis-Related Glycemic Disarray Proven by Continuous Glucose Monitoring; Glycemic Markers and Hypoglycemia

Abstract:Objective: There is a high risk of asymptomatic hypoglycemia associated with hemodialysis (HD) using glucose-free dialysate; therefore, the inclusion of glucose in the dialysate is believed to prevent intradialytic hypoglycemia. However, the exact glycemic fluctuation profiles and frequency of asymptomatic hypoglycemia using dialysates containing >100 mg/dL glucose have not been determined. Research design and methods: We evaluated the glycemic profiles of 98 patients, 68 of whom were men, with type 2 diabetes undergoing HD (HbA1c 6.4 ± 1.2%; glycated albumin 20.8 ± 6.8%) with a dialysate containing 100, 125, or 150 mg/dL glucose using continuous glucose monitoring. Results: Sensor glucose level (SGL) showed a sustained decrease during HD, irrespective of the dialysate glucose concentration, and reached a nadir that was lower than the dialysate glucose concentration in 49 participants (50%). Twenty-one participants (21%) presented with HD-related hypoglycemia, defined by an SGL <70 mg/dL during HD and/or between the end of HD and their next meal. All these hypoglycemic episodes were asymptomatic. Measures of glycemic variability calculated using the SGL data (SD, coefficient of variation, and range of SGL) were higher and time below range (<70 mg/dL) was lower in participants who experienced HD-related hypoglycemia than in those who did not, whereas time in range between 70 and 180 mg/dL, time above range (>180 mg/dL), HbA1c, and glycated albumin of the two groups were similar. Conclusions: Despite the use of dialysate containing 100-150 mg/dL glucose, patients with diabetes undergoing HD experienced HD-related hypoglycemia unawareness frequently. SGL may fall well below the dialysate glucose concentration toward the end of HD.
Continuous Glucose Monitoring in the Patients with Diabetic Nephropathy

Abstract:The aim of this study was to compare the difference of blood glucose (BG) fluctuation in the patients of type-2 diabetes mellitus (DM-2) with and without clinical diagnosed diabetic nephropathy (DN) by the continuous glucose monitoring system (CGMS). Thirty DM-2 patients with clinical diagnosed DN and fifteen DM-2 patients without complication underwent continuous glucose monitoring for 3 days (72 h) by CGMS. The difference of daily glucose fluctuation in both groups was compared by the parameter of CGMS. The 24-h mean blood glucose (MBG), minimal BG (MIN-BG), area under curve of BG over 7.8 (AUC7.8), percentage of time of BG over 7.8 (PT7.8), area under curve of BG over 11.1 (AUC11.1), percentage of time of BG over 11.1 (PT11.1), as well as mean of daily difference (MODD) were significantly increased in the group of DN, compared with those in the group of DM-2 without complication (all statistic probability P <0.05). No statistical significance of mean amplitude of glycaemic excursion (MAGE) was found. In the group of DN, MBG, standard deviation of blood glucose (SDBG), large amplitude of glycaemic excursion (LAGE), AUC7.8, PT7.8, AUC11.1, PT11.1, MAGE and MODD were (10.7±1.9)mmol/L, (2.5±1.3)mmol/L, (9.2±3.9)mmol/L, 3.2±1.7, (81±18)%, 1.2±1.0, (42±24)%, (5.8±2.5)mmol/L and (2.6±1.5)mmol/L, respectively. The study showed that the BG level of the patients with DN fluctuated throughout the day. MBG of the patients with DN was higher than that of the patients of DM-2 without complications, with the characteristics of long-lasting high BG period, dramatic instability during the day and especially high postprandial blood glucose. CGMS is a useful tool for physicians to know the details of the change of BG in the patients with DN.

Abstract:It is recognized that traditional measures of glucose control (such as hemoglobin A1c [A1C]) provide little information regarding the need for day to day changes in therapies. While intermittent self-monitored blood glucose (SMBG) provides additional information with which to make treatment decisions, significant barriers to its use exist, such as inconvenience and lack of timely and regular feedback. Furthermore, important information regarding glucose trends may be missed. Continuous glucose monitoring (CGM) has become increasingly reliable and has demonstrated efficacy in terms of improving A1C, reducing hypoglycemia, and improving the time in target glucose range. Incremental progress continues to be made toward a fully functional artificial pancreas, of which CGM will play a vital role. As more and more data are presented to patients and providers, it has become increasingly paramount that the data are organized in a standardized way and that communication of data is streamlined using patients’ mobile devices where available and within the existing clinic infrastructure. Systems that provide immediate feedback to patients and decision support tools for patients and providers have demonstrated superior outcomes compared to routine SMBG alone. Alternate markers of glucose control may provide complementary information about glucose control and long-term prognosis. This chapter will review the latest evidence for use of professional and personal CGM, mobile glucose monitoring approaches, and biomarkers of glycemic control. Accuracy Assessment of Continuous Glucose Monitoring by Frequent Venous Blood Collection in Patients with Type 2 Diabetes Mellitus

Abstract:Objective To assess the accuracy of continuous glucose monitoring system (CGMS) in type 2 diabetic patients by frequent venous blood collection.Methods A total of 11 patients with type 2 diabetes treated from August to October 2010 were enrolled in this study.The Medtronic GOLDTM CGMS was used to monitor the continuous changes of the blood glucose levels for 3 successive days in all subjects,meanwhile the fingertip blood glucose was tested at least 4 times each day for calibration.The venous blood of the subjects was randomly collected every 15 min per hour for 7 successive hours on one day when the subjects wore the sensor,and was tested using the YSI 2300 STAT PlusTM glucose&lactate analyzer.The accuracy was comprehensively evaluated by using statistical methods including the rate of agreement between the sensor readings and the paired YSI values within ± 20％ and ± 30％ of deviations,the error grid analysis,absolute relative difference (ARD), Bland-Altman analysis and trend analysis. Correlation analysis of two variables was done with Pearson correlation analysis. Results A total of 319 paired YSI-sensor values were collected from the 11 patients.Total 88.4％ of the sensor readings (95％ CI:0.84-0.92) were within ±20％ of the YSI values,and 96.9％ within ±30％ of the YSI values.Clarke error grid analyses showed that the ratios of the YSI-sensor values in region A to region B were 88％ and 12％ ;Consensus error grid analyses showed that the ratios of the YSI-sensor values in region A to region B were 96.2％ and 3.8％ ; Continuous error grid analyses showed that the ratios of the YSI-sensor values in region of accurate reading,benign errors,erroneous reading were 94.4％,2.8％,2.8％,respectively.The mean ARD for all subjects was 10.5％ and the median ARD was 8.4％. The Bland-Altman analysis detected the mean blood glucose level of 0.47 mmol/L (95％ CI:- 1.90-3.01 mmol/L).Trend analysis revealed that 82.5％ of the difference of the rates of change between the YSI values and sensor readings occurred within the range of 0.06 mmol · L-1 · min -1,and only 1.7％ of the absolute differences of the paired values were more than 0.17 mmol · L-1 · min-1.Conclusions The CGMS has high accuracy both in monitoring the blood glucose at a specific time and reflecting the trend of changes of the blood glucose level. However,actual clinical manifestations should be taken into account for diagnosis of hypoglycemia.

Performance of an interstitial glucose monitoring in patients with type 1 diabetes during haemodialysis

Abstract:Abstract Background The use of interstitial glucose monitoring devices such as Flash Glucose Monitoring has been shown to be beneficial in patients with type 1 diabetes (T1DM). However, these devices have been little studied in patients with diabetes treated by chronic haemodialysis (HD). Methods The goal of this prospective, observational, multicentric, study was to evaluate the analytical performance of the FreeStyle Libre® 2 (FSL2) sensor in T1DM patients during HD sessions. During three HD sessions, interstitial glucose concentrations (ISFG) given by the FSL2 were compared, every 15 minutes, with blood glucose concentration (BG) obtained simultaneously. BG concentrations were measured by two different glucometers: the Accu-Chek Guide® and the StatStrip® meter. Results Twelve hemodialysis patients were included. Mean age was 54 ± 11 years. Mean diabetes duration was 36.5 ± 11.6 years and dialysis vintage was 35 ± 22 months. A total of 565 pairs of ISFG/BG values were available for analysis. The mean absolute relative difference, defined as the mean of the absolute relative differences between the ISFG and BG measurements, was 17.4% and 20.9% when the ISFG were compared to the StatStrip glucometer or Accu-Chek Guide, respectively. Interstitial results tend to underestimate blood results but all values were classified as having clinically acceptable error. The differences observed remained stable during the dialysis session and were not associated with the ultrafiltration rate. Conclusion The use of the FSL2 interstitial glucose monitoring device in HD patients with T1DM is clinically acceptable, even though the accuracy of the device is generally poorer than in studies including non-dialysis patients.

Abstract:In this editorial, we comment on the article by Zhang et al . Chronic kidney disease (CKD) presents a significant challenge in managing glycemic control, especially in diabetic patients with diabetic kidney disease undergoing dialysis or kidney transplantation. Conventional markers like glycated haemoglobin (HbA1c) may not accurately reflect glycemic fluctuations in these populations due to factors such as anaemia and kidney dysfunction. This comprehensive review discusses the limitations of HbA1c and explores alternative methods, such as continuous glucose monitoring (CGM) in CKD patients. CGM emerges as a promising technology offering real-time or retrospective glucose concentration measure-ments and overcoming the limitations of HbA1c. Key studies demonstrate the utility of CGM in different CKD settings, including hemodialysis and peritoneal dialysis patients, as well as kidney transplant recipients. Despite challenges like sensor accuracy fluctuation, CGM proves valuable in monitoring glycemic trends and mitigating the risk of hypo- and hyperglycemia, to which CKD patients are prone. The review also addresses the limitations of CGM in CKD patients, emphasizing the need for further research to optimize its utilization in clinical practice. Altogether, this review advocates for integrating CGM into managing glycemia in CKD patients, highlighting its superiority over traditional markers and urging clinicians to consider CGM a valuable tool in their armamentarium.
