Abstract:Journal of Black Psychology, Ahead of Print. Black women may adhere to the Superwoman Schema (SWS), which may be associated with psychological distress in particular contexts. Emotion regulation may be an important factor in understanding the relationship between SWS and psychological distress. In this cross-sectional study of 167 college Black women, we examined the relationship between dimensions of SWS and psychological distress. We also investigated if two aspects of emotion regulation, expressive suppression, and cognitive reappraisal, mediated these relationships. Findings indicated that all dimensions of SWS were positively associated with psychological distress. Expressive suppression mediated these relationships for all dimensions except for an obligation to suppress emotions and an obligation to help others. However, the intense motivation to succeed, despite limited resources, was positively associated with cognitive reappraisal, which was inversely associated with psychological distress. Findings underscore the complex nature of the Superwoman Schema and highlight the importance of understanding and addressing emotional regulation strategies in clinical settings to support Black women's mental well-being effectively.

Central Hemodynamics in African American Women: Examining the Role of Superwoman Schema Endorsement

Abstract:Background: African American women bear a disproportionate burden of cardiovascular diseases, potentially due to altered central hemodynamics. Racism and sexism often lead to African American women taking on numerous caretaking roles and overall increases their use of the Strong Black Woman (ie, Superwoman) mindset, which may have negative health consequences. We hypothesized that endorsing the Superwoman role and its Obligation to Help Others dimension would be associated with a deleterious central hemodynamics profile in African American women. Methods and results: Using cross-sectional data, we examined central systolic blood pressure (mm Hg; n=408), augmentation index (percentage, adjusted for height and heart rate; n=408), and pulse wave velocity (m/s; n=368) in African American women aged 30 to 46 years. The Giscombe Superwoman Schema (SWS) questionnaire assessed endorsement of Overall SWS (range, 0-105) and SWS-Obligation to Help Others (range, 0-3). Multiple linear regression modeled associations between Overall SWS (10-unit increments) and SWS-Obligation to Help Others (1-unit increments) and central hemodynamics while adjusting for pertinent sociodemographic, clinical, and psychosocial factors. In fully adjusted models, central systolic blood pressure was significantly associated with Overall SWS (β=0.83 [95% CI, 0.19-1.47]) and SWS-Obligation to Help Others (β=2.03 [95% CI, 0.39-3.67]). Augmentation index was associated with Overall SWS (β=0.66 [95% CI, 0.02-1.30]) and SWS-Obligation to Help Others (β=2.21 [95% CI, 0.58-3.84]). Significant associations were not observed between pulse wave velocity and SWS. Conclusions: Greater endorsement of the Superwoman role and prioritizing caregiving over self-care were associated with higher central systolic blood pressure and augmentation index, which may contribute to adverse cardiovascular health among African American women.

Abstract:Clinical Nursing Research, Ahead of Print. The Strong Black Woman (SBW) schema is described as a statue of unrelenting strength, resilience, and self-sufficiency, serving as a shield of protection and cultural adaptation to suppress and control manifestations of racial and gender oppression. Stemming from superwoman syndrome, a conceptual model exploring the multifactorial roles women hold and their impact, the SBW extends beyond gender roles to the sociopolitical context of the Black woman's lived experience. Endorsement of the SBW posits risk for health disparities including stress, anxiety, depression, and obesity. This review was conducted to explore the SBW schema and experiences of Black women who endorse it, to delineate how Black women describe themselves in relation to the SBW persona, and to inform further inquiry, nursing practice, and clinical approaches to improving health outcomes of this population. A systematic review of qualitative studies was conducted with a literature search from CINAHL, APA PsycINFO, MEDLINE, PubMed, and SocINDEX databases yielding seven relevant papers for this analysis. Studies using the superwoman schema and the SBW schema with participants who identified as Black women were included in the review. Consistent with the SBW phenomenon, many participants described examples and consequences of being an SBW. While most women identified with SBW, not all endorsed the persona entirely, challenging its ideal and reinforcing positive self-care. Themes include (a) Strength by nature, not choice, (b) Suppressed emotion, (c) Success over everything, and (d) Prioritizing others over self. Additional emerging themes are also included. Black women increasingly recognize the negative impacts of the SBW schema, pinpointing how their internal feelings manifest in their external world. The conceptual framework itself is an anomaly, incongruently impacting both the mental and physical health of Black women, further contributing to the long-term health and sociopolitical disparities that Black women experience. Simply acknowledging and understanding these experiences by healthcare practitioners are not enough to prevent or eliminate the risks involved with the endorsement of the SBW schema but rather intentionally addressing these as a contributing social determinant of health that predisposes them to long-term chronic conditions.

Abstract:The Superwomen Schema (SWS) describes a social framework that encompasses the role that many Black women adopt in response to chronic stress, financial pressures, and an intersection of oppression. Woods-Giscombé (Superwoman Schema: African American Women's Views on Stress, Strength, and Health. Qualitative Health Research 20 (5): 668–683, 2010) characterizes SWS using five tenets: obligation to manifest strength, obligation to suppress emotions, resistance to vulnerability or dependency, determination to succeed despite a lack of resources, and an obligation to help others. The goal of this study is to determine the connection between SWS among Black women and substance use as a means of maintaining mental health, garnering resilience, and coping with external pressures. We aimed to highlight systemic and infrastructural racism and prejudice and how they relate, not only to the adoption of SWS, but also how they may contribute to substance use. This study is a secondary analysis of a larger study on HIV prevention Black and Latine women at high risk for HIV. Only Black participants (n = 10) were included in this secondary analysis. The interviews were conducted 3 times across 3 months. Interviews were coded and analyzed using thematic content analysis in NVivo. Themes of undiagnosed mental health symptoms, medical mistrust, institutional distrust, and aversion to help-seeking were recurrent in our data. Our research confirmed and assessed dual repercussions of SWS among Black women both as a defense that granted resilience in the face of seemingly insurmountable odds and as a construct that encouraged substance use as a coping mechanism for compromised mental health. This study contextualized this subset of coping and substance use to address and dismantle systemic contributors.

The Strong Black Woman Concept: Associated Demographic Characteristics and Perceived Stress Among Black Women

Abstract:Journal of Black Psychology, Ahead of Print. Being a "Strong Black Woman" (SBW) is a frequent cultural demand for Black Women. The physical and emotional disadvantages of this persistent racial gender role expectation are notable. The purpose of the current exploratory study was to (1) confirm a link between endorsement of the SBW schema and self-reported perceived stress and (2) investigate the potential demographic features that predict the level of endorsement of the SBW schema. A community sample of n = 185 Black women completed an online survey containing the Giscombé Superwoman Schema Questionnaire (Woods-Giscombé et al., 2019), the Perceived Stress Scale-10 (Cohen & Williamson, 1988), and a demographic questionnaire. The results indicated that more endorsement of SBW ideals uniquely predicted greater perceived stress. Also, only the demographic variables of age and number of children were predictors of SBW endorsement. Endorsement of SBW expectations did not vary across the other demographic variables indicating the SBW schema was prominent for most participants in the sample.
Associations Between the Superwoman Schema, Stress, and Cardiovascular Health Among African-American Women

Abstract:Background: African-American (AA) women are less likely to achieve ideal cardiovascular (CV) health compared with women of other racial/ethnic subgroups, primarily due to structural and psychosocial barriers. A potential psychosocial construct relevant to ideal CV health is the superwoman schema (SWS). Purpose: We explored whether the SWS was associated with perceived stress, CV risk factors, and overall CV health among AA women. Methods: This cross-sectional analysis of the FAITH! Heart Health+ Study was conducted among AA women with high cardiometabolic risk. Pearson correlation evaluated associations between SWS and CV risk factors (e.g., stress, hypertension, diabetes, etc.). The 35-item SWS questionnaire includes five domains. Stress was measured by the 8-item Global Perceived Stress Scale (GPSS). CV health was assessed using the American Heart Association Life's Simple 7 (LS7) rubric of health behaviors/biometrics. Data acquisition spanned from February to August 2022. Results: The 38 women included in the analysis (mean age 54.3 [SD 11.5] years) had a high CV risk factor burden (71.1% hypertension, 76.3% overweight/obesity, 28.9% diabetes, 39.5% hyperlipidemia). Mean GPSS level was 7.7 (SD 5.2), CV health score 6.7 (SD 1.8), and SWS score 60.3 (SD 18.0). Feeling an "obligation to help others" and "obligation to present an image of strength" had strongest correlations with GPSS score among all SWS domains (r = 0.51; p = .002 and r = 0.39; p = .02, respectively). Correlation among the SWS domains and traditional CV risk factors was not statistically significant. Conclusion: Our findings suggest that an obligation to help others and to project an image of strength could be contributing to stress among AA women.

Regression results indicated the SBW schema was a statistically significant predictor of wellness. Grounded in critical Black feminist theory, an interpretative phenomenological analysis of focus group data identified five themes of wellness and coping for Black women counselors.

Abstract:The Strong Black Woman (SBW) race-gender schema prompts African American women to use self-reliance and self-silence as coping strategies in response to stressors. Utilizing the coping strategies associated with the SBW race-gender schema could trigger anxiety and depression symptoms that may intensify when coupled with negative attitudes toward professional psychological help. The present study investigated whether African American women's endorsement of the SBW race-gender schema predicted increased symptoms of anxiety and depression and whether attitudes toward professional psychological help-seeking intensified psychological distress. Data were collected from 95 participants ranging in age from 18 to 65. Hierarchical regression analysis demonstrated significant main effects for the SBW race-gender schema and greater anxiety and depression, respectively. Greater indifference to stigma, 1 dimension of help-seeking attitudes, predicted lower levels of anxiety. African American women's attitudes toward professional help-seeking did not moderate the associations between endorsement of the SBW race-gender schema and anxiety or depression, respectively. Finally, endorsement of the SBW race-gender schema was inversely and significantly associated with 2 facets of help-seeking attitudes: (a) psychological openness and (b) help-seeking propensity. Taken together, these findings provide empirical support for the role of cultural factors, like the SBW race-gender schema, in African American women's experience of psychological distress and potential underutilization of mental health services. Abstract:Abstract Background Black young adult women (ages 18–35) are at disproportionate risk for obesity and emotional eating. Emotional eating interventions target psychological flexibility, such as reducing experiential avoidance and increasing acceptance of food-related thoughts. Yet Black women face gendered racism, and some endorse roles that reduce psychological flexibility, such as the superwoman schema role. Culturally centered stress and coping has often been overlooked, leading to an incomplete understanding of processes that engender emotional eating and the implications for appropriate and effective interventions for Black young adult women. Purpose We investigated direct and indirect pathways of associations between stress from gendered racial microaggressions to emotional eating through the endorsement of superwoman schema and two aspects of psychological flexibility. Methods Black young adult women (N = 504; Mage = 24.72; 75.2% African American; 98.4% cisgender) participated in an online survey wherein they reported demographics, stress from gendered racial microaggressions, superwoman schema, experiential avoidance, acceptance of food-related thoughts, and emotional eating. Path analysis was conducted to examine direct and indirect effects. Results Results provided evidence for indirect associations between more stress from gendered racial microaggressions and more emotional eating. More stress was associated with greater endorsement of the superwoman schema which was associated with more experiential avoidance and less acceptance of food-related thoughts, which were each associated with more emotional eating. Conclusions Endorsement of superwoman schema and concomitant avoidance and less acceptance may be one way that gendered racial stress propels emotional eating. Future research could test intervention components that disrupt this path.

Abstract:Journal of Black Studies, Ahead of Print. This qualitative study examines how the "Strong Black Woman" (SBW) label can have potentially negative health effects for African American/Black women that contribute to eating disorders. This study addresses the gap in literature on racial disparities that are present in understanding eating disorders that contribute to obesity and obesityrelated issues. Through semi-structured individual interviews conducted with (N = 11) Black female higher education students, participants were able to discuss how disorganized overeating patterns were associated with the emotional stress of being labeled a Strong Black Woman. Findings provide implications to clinicians, educators, and researchers by identifying stress inducing factors heightened by racist and sexist microaggressions that contribute to the mental and physical health of Black women. This study also adds to the limited literature on the intersection of racism and sexism that contribute to poor health outcomes in Black women.

Abstract:There is growing recognition that race and socioeconomic variables in health research demand greater attention. The investigators compared racial differences in health definition, health value, and health-promoting behavior of 62 pairs (N = 124) of Black and White college women matched on age, body mass index, and socioeconomic status. Both groups of women had similar definitions of health, valued health to the same extent, and reported similar levels of self-actualization, health responsibility, exercise, and stress management. Black women, relative to White women, practiced fewer nutrition behaviors and had less interpersonal support. Interventions to reduce health risk associated with nutrition practices of Black women are warranted and further research is needed to explore the influence of the social structure of educational institutions on interpersonal relationships and other health behaviors. When socio-economic status is taken into consideration, Black and White college women demonstrated more commonalities in health behavior than differences.

We interviewed Black women with a personal or relation diagnosis of breast or ovarian cancer to understand their views and experiences, including how the perception of the SBW stereotype influenced their care.

Abstract:The purpose of this article is to provide theoretical and practical application for working with Black women in therapy, especially those who embody the Strong Black Woman schema. The authors will detail the oppressive systemic conditions which create and reinforce this schema, starting in infancy. A historical perspective is adopted to contextualize how the remnants of strength from chattel slavery complicate present day expressions of Black womanhood. The authors discuss the complexity of the Strong Black Woman schema as a way of being that developed out of Black women navigating systemic, generational, and situational violence. Elements of Womanist care and a Black Feminist ethic of care are applied to support Black women's mental health. The authors discuss how to resist the Strong Black Woman schema as well as affirmingly dismantle the deeply internalized messaging within therapy sessions to incorporate trauma healing and recovery beyond the confines of colonized and misogynistic frameworks. The article concludes with a case example and therapist reflexivity.

Abstract:Objective: The researchers applied Lindsay-Dennis' Black Feminist-Womanist research paradigm to Andersen's Behavioral Model for Health Service Use to guide initial research about Black American women's preventive health behaviors. Methods: This article highlights this application, using interpretive phenomenological analysis for qualitative questions assessing how 40 Black college women define health and their experiences in health care. This was part of a larger convergent parallel mixed-methods approach in a 2022 cross-sectional online survey. Results: Participants defined health as a concept involving health literacy, physical and mental health, and being free from health conditions or disease. Regarding health-related lived experiences, negative experiences were more frequently reported than positive experiences. However, many participants reported both positive and negative health care related experiences. Predisposing, enabling, and need factors were all present in qualitative responses. Conclusions: This article highlights the fit of a Black Feminist-Womanist research paradigm to Andersen's model to better understand Black women's health experiences and illustrates ways that medical mistrust, health literacy, and past experiences with health care can influence health service use. Areas for future research on barriers and facilitators to preventive care and implications for reducing health disparities are also discussed.

Abstract:The aim of this systematic mixed studies review was to examine how social networks influence physical activity and dietary patterns among African American women. We searched PUBMED, CINAHL, JSTOR, SocioIndex, Medline, Cochrane Library, sportDISCUS, and PsycINFO to review articles published between Jan 1, 2007, and July 1, 2021. Only 23 studies met the inclusion criteria. All studies were conducted in the US. Social support received from family members, friends, and churches was found to positively influence physical activity and/or dietary patterns among African American women. An inconsistent relationship was found between social network size, health behavior characteristics of social networks, and physical activity and/or dietary patterns among African American women. Future research should incorporate structural and functional social network strategies to promote physical activity and healthy eating behaviors effectively and sustainably among African American women.

Abstract:Background African American women have lower levels of leisure time physical activity compared to White American women. Interventions to improve physical activity have mixed benefits for African American women, even when guided by theory. Understanding how theoretical constructs used in physical activity interventions relate to changing behavior may provide direction for more successful interventions. Objective The study aimed to examine the relationships among social cognitive constructs (self-efficacy, social support from group behavioral meetings, outcome expectations/realizations), and change in physical activity from baseline to 48 weeks in African American women participating in a lifestyle physical activity program. Methods A secondary data analysis of longitudinal data using a correlational design was conducted using data from a 48-week physical activity randomized controlled trial (RCT). The RCT included a group behavioral meeting component with one of three telephone intervention conditions (no calls, personal motivation calls, or automated motivational calls) randomly assigned across six community healthcare sites. The participants were 260 sedentary, midlife African American women with no major signs or symptoms of cardiovascular disease who completed baseline and 48-week assessments of the RCT. Measures included self-efficacy for change in overcoming barriers to physical activity at 24 weeks, physical and psychological outcome realizations at 24 weeks, social support from group behavioral meetings at 24 weeks, and physical activity (self-report and device-measured) change from baseline to 48 weeks. Results In a hierarchical regression model predicting change in self-reported time spent in weekly moderate–vigorous physical activity at 48 weeks, psychological outcome realizations at 24 weeks were significant positive predictors. In a hierarchical regression model for change in device-measured daily steps at 48 weeks, a self-efficacy change at 24 weeks was a significant positive predictor. Discussion Attention should be given to increasing self-efficacy to overcome physical activity barriers and achieve self-identified physical and psychological outcomes in physical activity programs.

Following Their Lead? Connecting Mainstream Media Use to Black Women’s Gender Beliefs and Sexual Agency

Abstract:Although media exposure has emerged as a significant predictor of consumers' sexual decision making, less is known about the mechanisms involved and about the dynamics of these relations for adults, in general, and for African American adults, in particular. To address these gaps, we used structural equation modeling (SEM) to test whether heterosexual Black women's endorsement of traditional gender and sexual roles mediates connections between their consumption of four mainstream media (music videos, reality TV programming, movies, and women's magazines) and three dimensions of their sexual well-being (sexual assertiveness, sexual inhibition, and sexual deception). We surveyed 594 heterosexual Black women aged 17 to 55 who were undergraduate and graduate students at two universities (one historically Black university and one predominantly White institution). Results confirmed expectations, such that greater media consumption was associated with greater support of traditional gender and sexual roles; in turn, endorsing these roles predicted lower levels of sexual assertiveness, greater sexual inhibition, and more frequent use of sexual dishonesty to retain a partner. We discuss implications of these findings for psychology and sexuality research and also for Black women's sexual relationships.

Abstract:Black/African American women continue to be disproportionately affected by HIV, facing multiple intersecting challenges that influence how they age and effectively manage their health. Supportive social relationships have been shown to help mitigate challenges and improve health in women with HIV, but little is known about Black/African American women's perceptions of social relationships. Guided by Life Course Theory, in-depth life history interviews were conducted with 18 Black/African American women aged 50+ years. In older adulthood, most important relationships among Black/African American women were with their adult children and grandchildren, intimate partners, God, and friends from the community. Factors that influenced relationships over time included: (a) a desire to build a community; (b) a need to empower oneself and give back; (c) yearning to engage the younger generation; and (d) battling HIV stigma. Older Black/African American women with HIV played a critical role in the education of the younger generation.

Abstract:Chronic diseases continue to rise among African American women making lifestyle programs a critical aspect of risk reduction and disease prevention. Weight management programs often have a reduced impact among African American women compared to White women, in part due to interactions between individual, social, and environmental factors. A secondary analysis of focus group data evaluated how cultural elements and contextual factors identified by church-going African American women influence intervention design, approaches for cultural adaptation, and solutions to weight management. Using the Community Energy Balance framework and the Community-Based Participatory Research model, research questions were formed, and a thematic analysis was conducted using data from six focus groups held in predominately African American churches (n = 6). Four themes emerged that represent identity and body appearance perspectives inside African American cultural contexts and across social and environmental contexts for how they work as motivators and barriers to health behaviors. These themes provide guidance for intervention approaches that center the experiences and needs of church-going African American women and identify targets for future cultural adaptations. Further work is needed to measure how specific cultural adaptations connect to improving health outcomes and engagement among African American women.
