Abstract:Objective:To investigate the value of Breast Ultrasound Report and Data System (BI-RADS) classification in diagnosis of special types of breast cancer.Methods:A total of 112 patients with special type of breast cancer (112 breast lesions) confirmed by pathology were analyzed by using BI-RADS ultrasound category in the Second Affiliated Hospital, Zhejiang University School of Medicine from August 2009 to August 2020. All patients underwent ultrasound before surgery. The breast lesions were evaluated by senior attending and junior resident according to BI-RADS ultrasound category respectively. Taking histopathological result as the gold standard, the sensitivity and accuracy of BI-RADS classification in the diagnosis of special types of breast cancer were calculated.The differences between different special types of breast cancer in terms of ultrasound characteristics and pathological features were analyzed. Kappa consistency test was used to evaluated the consistency of the results of two physicians.Results:In the 112 patients, pathological results showed that there were 20 cases of metaplastic carcinoma, 19 cases of invasive carcinoma with medullary features, 16 cases of differentiated carcinoma of apocrine gland, 12 cases of mucinous carcinoma, 12 cases of invasive micropapillary carcinoma, 10 cases of invasive papillary carcinoma, 6 cases of invasive lobular carcinoma and 17 cases of other special types of carcinoma. Among them, 4 cases (3.5%) were BI-RADS 3, 13 cases (11.6%) were BI-RADS 4a, 42 cases (37.5%) were BI-RADS 4b, 47 cases (42.0%) were BI-RADS 4c and 6 cases (5.4%) were BI-RADS 5. The accuracy and sensitivity of BI-RADS classification in diagnosis of special types of breast cancer was 96.43% and 96.43%, respectively. There was significant difference in BI-RADS grade among different special types of breast cancer ( P<0.05). Most lesions were characterized by hypoechoic with irregular shape and angular or microlobulated margin. The nodule size, boundary, echo and posterior echo in breast cancer with different special types showed significant differences (all P<0.05). There was a good consistency between the two physicians (Kappa=0.789). Conclusions:The ultrasonography features of different special types of breast cancer are different. BI-RADS classification has great value in diagnosis of special types of breast cancer.

Abstract:Secretory carcinoma of the breast (SCB) is a rare and specific type of breast cancer. Owing to its rarity, the number of SCB reports available is limited, with most of them focusing on clinical and pathological characteristics but no reports on its multimodal ultrasound (US) features. Thus, we present a rare case of SCB, retrospectively analyzing manifestations of US and contrast-enhanced US, as well as its pathological basis, aiming to enhance the understanding of US image features of SCB and provide more valuable information for clinical diagnosis. Moreover, the treatment strategy adopted for this patient may serve as a template for future management of SCB.

Atypical Appearance of Hepatic Hemangiomas with Contrast-Enhanced Ultrasound

Abstract:To describe the atypical enhancement pattern of hepatic hemangiomas on contrast-enhanced sonography. 22 patients with hepatic hemangiomas that were atypical on contrast-enhanced ultrasound were included in the study, all of them were confirmd by biopsy or surgery pathology. Atypical appearance of hepatic hemangiomas on contrast-enhanced ultrasound were divided into seven subtypes: (i) peripheral nodular enhancement in the arterial phase with centripedal filling, hypoechoic change in the delayed phase; (ii) peripheral circular enhancement; (iii) peripheral nodular enhancement; (iv) none enhancement; (v) septal enhancement; (vi) a central enhancing focus in the arterial phase, followed by a centrifugal enhancement in the portal venous phase, hypoechoic change in the delayed phase; (vii) slowly spoke wheel enhancement. Atypical appearance of hepatic hemangiomas were various. Radiologists should be aware. Establishing knowledge of the entire spectrum of atypical hepatic hemangiomas may benefit the rational approach to future cases.
Diagnostic value and characterization of contrast enhanced ultrasound for hepatic giant hemangionmas

Abstract:Objective To explore the enhancement pattern and characterization of hepatic hemangionmas with contrast enhanced ultrasound (CEUS).Methods A total of 44 patients with 49 nodules preliminary diagnosed of liver hemangioma were included in this study.For each nodule,the enhancement pattern,level,and dynamic change of CEUS were evaluated,and the features of hemangionmas were groups as echoic and compared with those on CEUS.Results All hemangiomas enhanced in arterial phase with centripetal progression in venous and late phase on CEUS,among which 41 lesions showed peripheral nodular enhancement while 8 showed peripheral rim-like enhancement.The whole-tumor enhancement pattern was seen in 13 lesions and part-tumor enhancement was shown in 36 lesions.The performance of part-tumor was independent of tumor echoic and hypoechoic tumors mostly presented to be whole tumor enhancement pattern.During portal venous and late phase,42 lesions showed hyperenhancement,while 7 lesions showed isoenhancement.Conclusions CEUS can suggest the enhancement dynamic characters of hepatic giant hemangionmas and reveals the relationship of grey-scale echoic and enhancement pattern of hemangiomas.It is important to diagnose the hepatic giant hemangionmas for CEUS.
An Evaluation of Ultrasound Features of Breast Fibroadenoma

Abstract:BACKGROUND: Breast cancer is among the most common cancers in the world. Ultrasound evaluations of breast have come into attention as an alternative route. Ultrasound features of benign lesions such as fibroadenoma can be overlapping with those in a malignant tumor. Here, we assessed the reports of breast ultrasound in patients with pathologic diagnosis of fibroadenoma.MATERIALS AND METHODS: We conducted a cross-sectional study and enrolled female patients with confirmed histologic diagnosis of fibroadenoma. Ultrasound studies were performed on the participants to see which sonographic patterns are more frequent in such lesions.RESULTS: In 92 patients with 40.4 ± 9.2 years of age, all participants were classified as stage 4 on Breast Imaging-Reporting and Data System scale. The mean ± standard deviation of size for the lesions was 167.4 ± 101.4 mm<sup>2</sup>. Upper outer quadrants in the breasts had the most number of lesions. Almost lesions were round with only 2.2% were oval. When assessed for the margin definition, 57.8% were circumscribed. Noncircumscribed masses were reported in 21.7%. About 91.3% of cases were hypoechoic in the ultrasound evaluation. Lobulated masses were in 28.3% of the cases. 8.7% of the masses were spongy whereas 9.8% and 2.2% of them had calcification and heterogenic appearance, respectively.CONCLUSION: The most frequent features include a hypoechoic mass with a circumscribed border; however, complex presentations that overlap malignant masses are also detectable including noncircumscribed margin, lobulation, presence of a posterior shadow, heterogenicity, and micro calcification.
Diagnostic Analysis of Hepatic Angiomyolipoma

Abstract:BACKGROUND:Hepatic angiomyolipoma (HAML) is a rare tumor containing a variable amount of fat, vessels and smooth muscle. We report the image findings on ultrasonography and computed tomography (CT) of huge HAML.METHOD:The clinical, imaging and pathological data of a case of HAML were retrospectively collected and analyzed.RESULTS:A huge heterogeneous hyperecho mass with anecho and hypoecho areas inside was found in the left hepatic lobe on ultrasonography. Color Doppler showed blood flow and arterial spectrum in it. CT scan showed a huge heterogeneous solid mass in the left lobe of the liver, with a low density and hypervascular area in arterial phase. The serum tumor marks were all negative. Ultrasound-guided biopsy was taken twice before resection and both showed necrosis tissue and reaction of inflammatory cells. Postoperative pathological results showed that the tumor was composed of epithelioid smooth muscle cells, thick-walled blood vessels and a few adipose cells with necrosis. The immunohistochemistry results showed appearance of typical HAML, with HMB-45 positive and alpha fetoprotein (AFP) negative.CONCLUSIONS:Preoperative diagnosis of HAML relies on combination of CT, MRI and ultrasonography. Our case of HAML showed heterogeneous hyperecho image on ultrasonography. Ultrasound-guided biopsy combined with morphological manifestation and specimen examination for HMB-45 may be helpful in the diagnosis of HAML.
Hyperechoic Breast Lesions on Ultrasound: Easily Misdiagnosed Conditions

Abstract:OBJECTIVE:To evaluate the incidences of hyperechoic breast lesions and hyperechoic breast cancers in lesions categorized as Breast Imaging Reporting and Data System (BI-RADS) 4 and 5 on ultrasound and investigate their sonographic features and the underlying histological causes.METHODS:The pathologic records for 848 sonographically guided core needle biopsies or surgical resection were retrospectively reviewed from June 2012 to March 2014. Hyperechoic lesions were identified and their sonographic features were evaluated. The incidence of hyperechoic breast lesions and the frequency of hyperechoic cancers among all hyperechoic breast lesions were calculated.RESULTS:Of all 848 lesions, 0.9% (8/848) were hyperechoic. Twenty-five percent (2/8) of the hyperechoic lesions were malignant and the remaining six were benign. Among the 280 malignant lesions, 0.7%(2/280) were hyperechoic. The pathological basis of breast lesions presenting as hyperechoic nodules included inflammatory edema, galactoceles, and grit calcifications. Little difference of sonographic feature was found between benign and malignant hyperechoic lesions in this study.CONCLUSIONS:Hyperechoic breast lesions are rare conditions but can be associated with a high ratio of breast cancer. History-taking and imaging techniques may help to avoid misdiagnosis.

Abstract:Objective:To explore the imaging characteristics of CT and ultrasound of angioleiomyolipoma.[WT5”HZ]Methods:16 cases of CT and ultrasound manifestations of angioleiomyolipoma proved by operation and pathology were analyzed retrospectively .Results:11 of 16 occurred in single side,5 of 16 occurred in both sides, 2 of 16 cases accompanied with nodular sclerosis.Conclusion:CT and ultrasound comprehensive imaging diagnosis can find small volume mass in early stage, CT can measure the density of masses more exactly than ultrasound.

Abstract:Hepatic hemangioma is the most common benign liver tumor, with cavernous hemangioma being the predominant clinical variant. Most hepatic hemangiomas exhibit typical features on imaging. Nevertheless, a minority of hepatic hemangiomas display atypical imaging features, which can lead to misdiagnosis, particularly when confused with certain liver malignancies. In such cases, surgical pathology is necessary to confirm diagnosis, potentially resulting in unnecessary surgical procedures. Among atypical hemangiomas, multilocular cystic hemangiomas are rare with limited reports of relevant imaging studies, primarily isolated case reports. In this report, we present 3 cases of multilocular cystic hemangiomas with pathologically confirmed diagnoses, showing compartmentalized imaging findings across ultrasound (US), computed tomography (CT), and magnetic resonance (MR) imaging. Additionally, we offer a comprehensive review and summary of all reported cases found in the relevant Chinese and English language literature.

Abstract:A hepatic hemangioma is a benign liver tumor made up of a number of blood-filled chambers surrounded by liver-supplied endothelial cells. Most liver hemangiomas are asymptomatic and are only discovered during imaging studies for other conditions. Ultrasound is used for initial screening followed by a computed tomography scan, which shows slow enhancement due to small vessels and can be used to diagnose the location, number, and size of a hepatic hemangioma. A large liver hemangioma can range in size from 10 centimeters to more than 20 centimeters and can cause symptoms and complications that require prompt intervention. Hepatic hemangiomas can co-occur with other localized hepatic lesions; a careful diagnosis is necessary to distinguish them. In this case study, a 48-year-old woman complained of a stomachache that had persisted for three months. Following an initial clinical evaluation, hepatomegaly was found, and contrast-enhanced computed tomography (CECT) abdomen and pelvis was performed, revealing numerous giant hepatic hemangiomas. Significant improvements were noted in the patient's condition with tumor embolization.
[Diagnosis and Treatment of Subcutaneous Cavernous Hemangioma with Color Doppler Ultrasonography].

Abstract:OBJECTIVE:To discuss clinical application of the color Doppler ultrasonography in diagnosis and treatment of cavernous hemangioma in deep subcutaneous tissue.METHODS:From 1996, 15 cases of cavernous hemangioma were diagnosed and located with color Doppler ultrasonography and were embolized under monitoring of the ultrasonography or resected by operation before re-examination of the hemangioma via the color Doppler ultrasonography after the intervention.RESULTS:Direct embolization was achieved in 10 cases after pinpoint location of the hemangioma by the ultrasonography, and guided embolization was performed successfully in 2 cases via the monitoring of ultrasonography, and operation had to be adopted to remove the focus. No reoccurrence of the hemangioma was observed in all the cases.CONCLUSION:Cavernous hemangioma in deep subcutaneous tissue could be easily diagnosed and located with color Doppler ultrasonography, and could be removed by embolization under monitoring of the ultrasonography successfully.

Abstract:Twenty-five pathologically proven hepatic angiomyolipomas (AMLs) were included in the study. Ultrasonic features of hepatic AMLs were reviewed. Three types of echogenicity were observed on ultrasound examination: (i) strong hyper-echogenicity, (ii) moderate hyper-echogenicity and (iii) hypo-echogenicity. Vascular signals within tumors could be detected in 22 (88.00%) tumors as multiple punctiform, filiform or dendriform signals by color Doppler flow imaging. Based on the enhancement patterns in the arterial, portal and late phases, the features of hepatic AMLs on contrast-enhanced ultrasound were divided into four subtypes: (i) “fast in slow out” (68.00%, n = 17); (ii) “fast in same out” (16%, n = 4); (iii) “fast in fast out” (12.00%, n = 3); and (iv) “fast in uneven out” (4.00%, n = 1). Contrast-enhanced ultrasound diagnosed 22 (88.00%) tumors as benign tumors and 13 (52.00%) as hepatic AMLs. Four cases were misdiagnosed as hepatic hemangioma, five cases as focal nodular hyperplasia (total = 36.00%). The rate of correct diagnosis of hepatic AMLs increased significantly from 24.00% for ultrasound alone to 52.00% for contrast-enhanced ultrasound. Therefore, information obtained from ultrasound, color Doppler flow imaging and contrast-enhanced ultrasound should be combined to improve diagnosis.

Abstract:Frequent advances in transducer design, electronics, computers, and signal processing have improved the quality of ultrasound images to the extent that sonography is now a major mode of imaging for the clinical diagnosis of breast cancer. Breast ultrasound is routinely used for differentiating cysts and solid nodules with high specificity. In combination with mammography, ultrasound is used to characterize solid masses as benign or malignant. There is growing interest in using Doppler ultrasound and contrast agents for measuring tumor blood flow and for imaging tumor vascularity. Ease of use and real-time imaging capability make breast ultrasound a method of choice for guiding breast biopsies and other interventional procedures. Breast ultrasound is used in many forms. B-mode is the most common form of imaging for the breast, although compound imaging and harmonic imaging are being increasingly applied to better visualize breast lesions and to reduce image artifacts. These developments, together with the formulation of a standardized lexicon of solid mass features, have improved the diagnostic performance of breast ultrasound. Several approaches that are currently being investigated to further improve performance include: (1) computer-aided-diagnosis; (2) the assessment of tumor vascularity and tumor blood flow with Doppler ultrasound and contrast agents; and (3) tissue elasticity imaging. In the future, ultrasound will play a greater role in differentiating benign from malignant masses and in the diagnosis of breast cancer.

Ultrasound performance in pediatric deep soft-tissue tumor characterization

Abstract:This study investigated the performance of ultrasonography in diagnosing deep soft-tissue tumors and tumor-like lesions in children with histological results. Demographic information and ultrasound characteristics of benign and malignant masses were statistically analyzed. Three radiologists (Radiologists 1, 2, and 3) independently reviewed the ultrasonography studies while being blinded to the medical history and other imaging findings. The 82 lesions included in the study were histopathologically classified as malignant (n = 25) or benign (n = 57). No statistically significant differences were observed between the benign and malignant subgroups regarding age (p = 0.059), sex (p = 1.0), disease course (p = 0.812), presence or absence of symptoms (p = 0.534), maximum diameter (p = 0.359), margin (p = 1.0), calcification (p = 0.057), or blood Adler type (p = 0.563). However, statistically significant differences were observed between the benign and malignant subgroups in terms of isolated or Multiple occurrences (p < 0.001), history of malignancy (p < 0.001), shape (p < 0.001), and echogenicity (p < 0.001). Parameters such as tumor shape (p = 0.042, OR = 6.222), single or multiple occurrences (p = 0.008, OR = 17.000), and history of malignancy (p = 0.038, OR = 13.962) were identified as independent predictors of benign and malignant tumors. The diagnostic sensitivities evaluated by the three radiologists were 68.0%, 72.0%, 96.0%, respectively, while the specificities were 77.2%, 82.5%, 77.2%, respectively. Ultrasound demonstrates good performance in the diagnosis of benign deep lesions such as hemangiomas/venous malformation and adipocytic tumors. Multiple irregular morphologies and a history of malignancy were identified as independent risk factors for malignant masses. The experience of radiologists in recognizing specific tumors is important. Careful attention should be paid to masses with ambiguous ultrasound features, as well as small lesions.

Bile Duct Hamartoma in Liver: a Disease Which is Easily Misdiagnosed by Ultrasound

Abstract:Objective To increase the diagnostic value of ultrasound in bile duct hamartomas (BDH) by discussing the ultrasonic characteristics of BDH.Methods This retrospective study was about 21 cases of BDH which were diagnosed by MRI and the ultrasonic images were analyzed.Results Of the 21 cases,only 5 were diagnosed as BDH by ultrasound (23.8％).In 17 cases,BDH manifested as multiple small cysts in liver with irregular shapes,which were accompanied by multiple hyperechoic foci with comet tails in 7 cases and multiple small hyperechoic nodules in 8 cases.In addition,in other 4 cases,BDH manifested as hepatic diffuse disease,which was also accompanied by multiple hyperechoic foci with comet tails and small hyperechoic nodules.Conclusions BDH is not rare and the recognition of its ultrasonic characteristics will improve the diagnostic value of ultrasound.

Abstract:Objective To investigate the diagnostic value of ultrasonography in spleen hemangiolyphangioma and the sonographic feature.Methods Sixteen patients with spleen hemangiolyphangioma were reviewed by routine ultrasound and color Doppler flow imaging(CDFI).The diagnosis of spleen hemangiolyphangioma was confirmed by pathology after operation.Contrast-enhanced ultrasonography(CEUS) was performed in five patients.Results There were 25 lesions totally.The lesions were cystic,honeycomb-shaped,well-demarcated(80.0%,20/25),clear margin(80.0%,20/25),and homogeneous(72.0%,18/25).CDFI showed blood flow in 56.0%(14/25) lesions.CEUS showed the lesions were mainly enhanced synchronously with the spleen parenchyma.The lesions were hypoechoic in comparison with the spleen parenchyma with branch-shaped diffuse enhancement pattern.The lesions also showed persistent enhancement.The detection rate of color flow before and after CEUS were 62.5%(5/8) and 100%(8/8),respectively.Conclusions Homogeneous hyperechoic leision with major solid portion is the characteristic sonographic feature of spleen hemangiolyphangioma.Ultrasound examination is helpful for the diagnosis of spleen hemangiolyphangioma.

Abstract:AIM:To investigate the specific findings and characteristics of real-time contrast-enhanced ultrasound (CEUS) in hypoechoic hepatic hemangioma.METHODS:A total of 101 lesions in 83 patients were included. Analysis was made of the relationship between tumor size and CEUS enhance patterns in arterial phase, portal phase and delayed phase, phase changes, echoic changes in perfusion regression stage and filling defect.RESULTS:CEUS showed a lesion detection rate of 92.7%. In regression stage, only 46.5% lesions were fully filled. Enhancement of the lesions was categorized into Pattern I, peripheral nodular enhancement and centripetal filling (68.3%); Pattern II, peripheral ring enhancement and centripetal filling (27.7%); and Pattern III, overall rapid enhancement (4.0%). The most common phase changes were "fast-in slow-out" (74.3%). More than half (61.4%) lesions were hyperechoic in regression stage. Among all the lesions, 4 (3.9%) lesions regressed to hypo-echo in portal phase and 2(2.0%) in delayed phase). The frequent enhancement in tumors >3.0 cm was of Pattern I or II, "fast-in slow-out" or "slow-in slow-out" phase change, and hyperechoic or isoechoic change in perfusion regression stage, but in those ≤3.0 cm, the enhancement was of Pattern III, "fast-in fast-out" phase change and hypoechoic change in perfusion regression stage. The differences were statistically significant (p < 0.05). The percentage of lesions with complete filling on CEUS in the larger hypoechoic hepatic hemangiomas was significantly lower than that of smaller ones (p < 0.01).CONCLUSIONS:CEUS has high diagnostic value in hypoechoic hepatic hemangiomas mainly characterized by peripheral nodular or ring enhancement, centripetal filling, and "fast-in slow-out" and "slow-in slow-out" phase changes.

Abstract:Objective To investigate the characteristics of parametric imaging of contrast-enhanced ultrasound (CEUS) in evaluating hepatic hemangioma. Methods Totally 42 cases of patients with hemangioma were selected from October 2013 to April 2015 in the Tenth People's Hospital of Tongji University. Thirty-eight cases were confirmed by CT and magnetic resonance imaging (MRI), and 4 cases were confirmed by surgery pathology for cavernous hemangioma. SonoLiver CAP software was used to quantitatively analyze the CEUS and calculate dynamic vascular imaging model parameters (DVP) based on the enhancement difference between lesions and surrounding normal liver parenchyma. The peak intensity (Imax), the rise time (RT), time to peak (TTP), mean transit time (mTT), perfusion index (PI) and DVP curve of hemangioma and surrounding parenchyma were analyzed. T test was used to compare the differences of Imax, RT, TTP, mTT and PI among the large liver hemangioma, small liver hemangioma and surrounding normal liver parenchyma, between large liver hemangioma and small liver hemangioma, and between the edge and central part of liver hemangioma, respectively. Results Imax and PI in both large hemangioma and small hemangioma were higher than that in normal liver parenchyma; RT, TTP and mTT were shorter in the surrounding normal liver parenchyma, and the differences were statistically significant. Imax in large liver hemangioma was higher than that in small hemangioma, and the difference was statistically significant. The difference of RT, TTP, mTT and PI between large hemangioma and small hemangioma were not statistically significant. Imax in central part was higher than that in the edge part of liver hemangioma. RT, TTP in central part were also longer than that in edge part, and the difference had statistical significance. mTT and PI in both large and small hemangioma had no significant difference. DVP curves and DVP parameter figures of liver hemangioma can be divided into three types: type Ⅰ, positive model with gradual washout; type Ⅱ, positive model with no washout; type Ⅲ, negative model. For DVP curves and DVP parameter , type Ⅰ, type Ⅱ and type Ⅲ respectively had 16 cases (16/42, 38.1%), 20 cases (20/42, 47.6%), 6 cases (6/42, 14.3%) and 15 cases (15/42, 35.7%), 10 cases (10/42, 23.8%), 17 cases (17/42, 40.5%). Conclusions TTP and mTT of hemangiomas was shorter than the surrounding liver parenchyma by using contrast-enhanced ultrasound imaging parameters. The Imax and PI in large hemangioma was higher than that in small hemangioma. The RT and TTP in hemangioma edges was faster than that in central parts. The difference of blood perfusion in the liver hemangioma and surrounding liver parenchyma could be shown by DVP curve. Key words: Ultrasonography; Contrast media; Liver; Hemangioma

Abstract:Objective To explore the image performance of mucinous breast carcinoma( MBC) on automated breast volume scanner ( ABVS ) and its pathologic features ,for the purpose of improving the recognition of this disease on ABVS ,as well as diagnostic ability . Methods Retrospectively analyzed the sonogram and pathological data of 17 patients with 18 MBC lesions that included 9 PMBC ( pure mucinous breast carcinoma) and 9 MMBC( mixed mucinous breast carcinoma) ,all patients were examined by ABVS before operation and confirmed by pathology . Results Luminal A was the main molecular subtype of MBC ,as well as rarely lymph node metastases;The image of the MBC on ABVS mainly showed a round like or irregular mass ,isoechoic or slightly lower with posterior acoustic enhancement ,and clearly edge with microlobulated or angular ;While on the coronal plane ,it might presented unsmoothed contour ,retraction phenomenon and expansive growth pattern like a spherical nodular;The image characteristics of the PMBC and MMBC lesions on ABVS had no statistical difference( P > 0 .05) ,but the PMBC often performed isoechoic mass with clearly marge ,in contrast , the MMBC usually performed hypoechoic mass with indistinct and angular edge ,and the malignant image features on the coronal plane . Conclusions The three vertical sections of ABVS are beneficial to observe the morphological features of MBC completely ,which can help to identify it from the benign lesions ,reduce misdiagnosis ,and provide some guidance for MBC surgical treatment in the early stage .
