Abstract:Objective To improve understanding of pleural involvement in rheumatoid arthritis(RA).Methods A retrospective analysis was performed in patients with pleural involvement.Results The 29 cases presented with pleural disease in 248 Rheumatoid arthritis(RA) patients,5 of them with pleural thickening,24 patients with pleural effusion(PE).18 patients with PE were small and asymptomatic and did not require intervention.Middle amounts of pleural fluid compromise respiratory function and require aspiration to allow underlying lung expansion in other 6 patients.The fluid was exudative.Rheumatoid pleural effusion was considered in 4 patients,responding to corticosteroid.Tuberculous pleurisy was conformed in 1 patient.Another patient was diagnosed as parapneumonic effusion.Both received antibiotic therapy with a good response.Conclusion Pleural effusion or pleural thickening is a common feature of RA.In most cases,PE is small and asymptomatic,and there is no need for intervention.Middle amounts of pleural fluid compromise respiratory function and require aspiration and corticosteroid.Exclusion of other causes of exudates is important,such as bacterial infection,tuberculosis and malignancy.Secondary infection should receive antibiotic therapy instead of corticosteroid.
A Case of Rheumatoid Arthritis-Associated Interstitial Lung Disease - An Unfortunate Complication of Untreated Rheumatoid Arthritis!

Abstract:Rheumatoid arthritis-associated interstitial lung disease (RA-ILD) is a rare extraarticular manifestation of the systemic autoimmune disease rheumatoid arthritis. RA-ILD is one of the leading causes of morbidity and mortality in patients with rheumatoid arthritis. It is more commonly seen in patients with risk factors, which include male sex, severe rheumatoid arthritis, and smoking. The presentation depends on the extent of the underlying pathology. Patients can remain asymptomatic for extended periods of time and progressively develop symptoms such as shortness of breath on exertion and a nonproductive cough. The diagnosis is made using a combination of clinical presentation, pulmonary function testing, and imaging. It is difficult for a practitioner to distinguish the presentation of interstitial lung disease from other causes and RA-ILD. However, it is extremely important to differentiate them due to the differences in management. The treatment usually involves a multidisciplinary team guided approach towards treating the underlying rheumatoid arthritis. Asymptomatic patients are managed by observation, while symptomatic patients are managed with glucocorticoids and immunosuppressants. Accurate and early treatment can lead to an improvement in a patient's symptoms and significantly improve their quality of life. We present an interesting case of a female with a long-standing history of rheumatoid arthritis not on any treatment presenting to the ED with exertional shortness of breath, dry cough, and abdominal distension.
Primary membranous nephrotic syndrome with chylothorax as first presentation: A case report and literature review

Abstract:BACKGROUND: Primary membranous nephrotic syndrome with chylothorax as the first manifestation is an unusual condition. To date, only a few cases have been reported in clinical practice.CASE SUMMARY: The clinical data of a 48-year-old man with primary nephrotic syndrome combined with chylothorax admitted to the Department of Respiratory and Critical Care Medicine of Shaanxi Provincial People's Hospital were retrospectively analysed. The patient was admitted to the hospital for 12 d due to shortness of breath. Imaging showed pleural effusion, laboratory tests confirmed true chylothorax, and renal biopsy revealed membranous nephropathy. After primary disease treatment and early active symptom treatment, the prognosis of the patient was good. This case suggests that chylothorax is a rare complication of primary membranous nephrotic syndrome in adults, and early lymphangiography and renal biopsy can assist in the diagnosis when there are no contraindications.CONCLUSION: Primary membranous nephrotic syndrome combined with chylothorax is rare in clinical practice. We report a relevant case to provide case information for clinicians and to improve diagnosis and treatment.

Constrictive pericarditis requiring pericardiectomy: an unusual first presentation of extra-articular rheumatoid arthritis-a case report

Abstract:Background: We report an unusual case of rheumatoid arthritis presenting for the first time with pericardial constriction and bilateral pleural calcification, in the absence of prior articular disease. Case summary: A 46-year-old Caucasian male, who initially presented with shortness of breath, intermittent chest tightness and general malaise, underwent extensive diagnostic workup over a period of six months involving multiple hospital admissions. He was found to have pericardial constriction on echocardiogram and ultimately required surgical pericardiectomy due to decompensation. After multiple diagnostic tests and specialist opinion, the aetiology of pericardial disease was ultimately confirmed to be extra-articular rheumatoid disease without synovitis. Discussion: Significant pericardial constriction can occur as the initial presentation of rheumatoid disease and anti-CCP is a highly specific confirmatory test. Pericardial pathological specimen can be unhelpful in determining this aetiology, and constrictive physiology can occur due to chronic inflammation/fibrosis in the absence of significant calcification.
Interstitial Lung Disease as the Initial Manifestation of Rheumatoid Arthritis: A Case Report and Review of the Literature

Abstract:Background: Rheumatoid arthritis (RA) is a chronic, systemic, inflammatory disorder that primarily affects synovial joints. Approximately 18–41% of patients with RA develop extra-articular manifestations [1]. However, extra-articular manifestations preceding or occurring without articular symptoms in RA have rarely been reported. Such atypical presentations of RA pose a diagnostic challenge to the clinician and may delay treatment. Case presentation: A 57-year-old female with long standing diabetes, hypertension, hyperlipidemia and Raynaud’s phenomenon presented shortness of breath, cough and new subcutaneous nodules. Four years before, she had been diagnosed with non specific interstitial pneumonia but had declined treatment. The physical exam did not reveal any signs suggestive of RA however, she was seropositive for rheumatoid factor (RF) and anti-citrullinated peptide antibody (ACPA). Treatment for RA-associated interstitial lung disease was discussed. Conclusion: Extra-articular involvement of RA can be observed as initial presentation of the disease in a handful of cases. However, RA diagnosis must be achieved to correctly manage these patients which can at that time receive targeted therapeutic interventions. From our literature review, pulmonary involvement was seen in over half of the cases in seropositive RA patients who lacked articular involvement at initial presentation.
Rheumatoid arthritis complicated with cervical actinomycosis and ureteral obstruction: A case report and literature review

Abstract:Actinomycosis is a rare chronic granulomatous disease characterized by granuloma formation and tissue fibrosis with sinus tracts, often misdiagnosed due to its similarity to many infectious and non-infectious diseases. This report presents a case of a 60-year-old female with more than 10 years history of rheumatoid arthritis who developed actinomycosis infection after long-term treatment with immunosuppressants and biologics, including methotrexate, leflunomide, and infliximab, leading to recurrent joint pain, poorly controlled rheumatoid arthritis activity, and persistent elevation of white blood cell counts. Abdominal CT revealed a pelvic mass and right ureteral dilation. Pathological examination of cervical tissue showed significant neutrophil infiltration and sulfur granules, indicating actinomycosis. The patient received 18 months of doxycycline treatment for the infection and continued rheumatoid arthritis therapy with leflunomide, hydroxychloroquine sulfate, and tofacitinib, resulting in improved joint symptoms and normalized white blood cell counts. After 2 years of follow-up, the patient remained stable with no recurrence. This case highlights the importance of clinicians being vigilant for infections, particularly chronic, occult infections from rare pathogens, in rheumatoid arthritis patients on potent immunosuppressants and biologics, advocating for early screening and diagnosis.
Diffuse large B-cell lymphoma involving the left sternoclavicular joint mimicking rheumatoid arthritis flare: a case-based review

Abstract:Rheumatoid arthritis (RA) is a chronic autoimmune disease affecting primarily joints and an increased risk of developing malignant lymphomas in RA has been well reported. However, primary lymphoma in a joint in RA patient is rare. We report the case of a 65-year-old man with RA suffering from pain and swelling of left sternoclavicular (SC) joint, which was not relieved by adding low-dose glucocorticoid. Magnetic resonance imaging (MRI) showed a para-osseous soft tissue swelling around the SC joint and a fracture of proximal clavicle. Histology of the soft tissue demonstrated diffuse large B-cell lymphoma and the patient subsequently underwent R-CHOP (rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisolone) chemotherapy. He was successfully treated with six cycles of R-CHOP chemotherapy, with discontinuation of MTX, resulting in a complete response. We performed a literature review and identified nine cases of lymphoma which involved joints in patients with rheumatoid arthritis. This is the first described case of a primary large B-cell lymphoma involving the unilateral SC joint in a patient with RA, which was initially confused with aggravation of RA. Therefore, malignant lymphoma should be considered in the differential diagnosis when a RA patient develops monoarthritis with spontaneous fracture, even without B symptoms.

Possible Environmental Exposure-Associated Pulmonary Cryptococcosis in a Patient with Rheumatoid Arthritis: a Case Report and Literature Review

Abstract:Patients with rheumatoid arthritis (RA) taking long-term immunosuppressive drugs are more susceptible to opportunistic infections, such as cryptococcosis. A 65-year-old woman was transferred to our hospital for rapidly progressing pulmonary lesions identified by lung computed tomography. She had a 7-year history of RA and had been prescribed methotrexate and glucocorticoids for 10 months. Additionally, our patient had a history of environmental exposure to house renovation lasting approximately 1 week before onset. Her serological test results and histopathological examination confirmed the diagnosis of pulmonary cryptococcosis (PC). The patient recovered well after 6 months of fluconazole treatment. In addition, we summarized 28 reported cases of RA patients with PC and found that older age might be a risk factor for cryptococcal infection in RA patients. The most common location for pulmonary lesions was the lower lobe, and the most common radiologic manifestations were nodules. Detection of cryptococcal capsular polysaccharide antigen was important for diagnosis. Patients undergoing antirheumatic therapy should avoid exposure to Cryptococcus.
Pulmonary hypertension secondary to seronegative rheumatoid arthritis overlapping antisynthetase syndrome: A case report

Abstract:Background: Polyarthritis is the most frequent clinical manifestation in antisynthetase syndrome (ASS) forms of idiopathic inflammatory myositis and may be misdiagnosed as rheumatoid arthritis (RA), particularly in patients with seronegative RA (SNRA). It is unclear whether there is an overlap between ASS and RA, or if ASS sometimes mimics RA. Pulmonary hypertension (PAH) is common in connective tissue diseases (CTDs). However, published reports on CTD-PAH do not include overlapping CTDs, and its incidence and impact on patient prognosis are unclear. Case summary: We report the case of a 63-year-old woman who presented with a 3-mo history of symptom aggravation of recurrent symmetrical joint swelling and pain that had persisted for over 10 years. The patient was diagnosed with RA and interstitial lung disease. The patient repeatedly presented to the hospital's respiratory and rheumatology departments with arthralgia, plus shortness of breath after activity. Relevant tests indicated that anti-CCP and RF remained negative, while anti-J0-1 and anti-Ro-52 were strongly positive. It was not until recently that we recognized that this could be an unusual case of SNRA with concurrent ASS. Joint pain was relieved after regular anti-rheumatic treatment. Chest computed tomography scans showed that pulmonary interstitial changes did not progress significantly over several years; however, they showed gradual widening of the pulmonary artery, and cardiac ultrasound indicated elevated pulmonary artery systolic pressure. The prescribed treatment of PAH was not effective in improving shortness of breath. Conclusion: Overlap of RA and ASS may be missed. Further research is necessary to facilitate early diagnosis, effective evaluation, and prognosis.
Rare but characteristic MRI Shoulder findings of chlamydia trachomatis-associated reactive arthritis

Abstract:Spondyloarthritis is an inflammatory disease characterised by tendon adhesions and sacroiliitis. Herein, we present a case of reactive arthritis (ReA) after Chlamydia infection. The condition was characterised by rotator cuff enthesitis accompanied by inflammation of the rotator cuff muscles and presence of contrast-enhanced lesions at several tendon attachments without pelvic sacroiliitis. Some studies have reported about shoulder joint involvement observed on magnetic resonance imaging (MRI) in ankylosing spondylitis and psoriatic arthritis. However, there are no reports on shoulder lesions detected on MRI in C. trachomatis infection-associated ReA. The patient presented with hip, lower back and right shoulder pain. MRI of the pelvis revealed inflammation of the tendon attachments such as the spinous process, sciatic tuberosity and greater and lesser trochanter. However, sacroiliitis was not observed. These imaging findings indicated enthesitis. The patient tested positive for C. trachomatis immunoglobulin but negative for HLA-B27 antigen. Hence, he was diagnosed with Chlamydia-related ReA. Antibiotic treatment combined with sulfasalazine was initiated. This resulted in an evident clinical improvement without remission. To the best of our knowledge, this is the first case report showing the presence of shoulder lesions on MRI in C. trachomatis infection-associated ReA. Further, this study showed that shoulder lesions in spondyloarthritis, including ReA, are characterised by not only adhesive inflammation but also bone marrow oedema in the tendon attachments and rotator cuff inflammation.

Abstract:Chylothorax, which accounts for 1% to 3% of pleural effusions, typically results from either surgery (traumatic) or underlying malignancy (nontraumatic). Less common causes of nontraumatic chylothorax are numerous and include congenital lymphatic abnormalities, connective tissue diseases, cirrhosis, and infection, among others.1 We describe what appears to be the first reported case of chylothorax caused by chylous ascites in Crohn disease. This case highlights the importance of using diagnostic evidence to link new symptoms to preexisting diseases whenever possible, as well as the systemic nature of Crohn disease.

Abstract:Rheumatoid arthritis (RA) is a common autoimmune disease. Tuberous sclerosis complex(TSC) is a rare autosomal dominant disorder. We report a case of RA with TSC. The patient was a 46-year-old woman with polyarthritis and cough symptoms, rheumatoid arthritis associated interstitial lung disease (RA-ILD) was initially considered, and after more than 3 months of anti-rheumatic treatment, the patient still had cough, and further examination revealed that the patient had lymphangioleiomyomatosis in the lungs, hepatic and renal angiomyolipomas, multiple subependymal nodules, Vertebral osteosclerotic nodules, as well as facial angiofibromas and periungual fibroma, RA was finally diagnosed with TSC, and everolimus 10mg qd was added to anti-rheumatic therapy for 1 month, and the patient's cough symptoms were relieved.

Abstract:Rheumatic heart disease (RHD) is a widespread illness in developing countries. RHD causes 99% of mitral stenoses in adults and 25% of aortic regurgitation. However, it only causes 10% of stenoses of the tricuspid valve, and is almost always associated with left-side valvular lesions. Isolated right-side valves are rarely affected, but may result in severe rheumatic pulmonary regurgitation. Herein, we present a case of rheumatic right-sided valve disease with severe pulmonary valve contracture and regurgitation in a symptomatic patient, successfully managed by surgical valvular reconstruction with a tailored bileaflet bovine pericardial patch. The options for surgical approach are also discussed. To our knowledge, the presented rheumatic right-sided valve disease with severe pulmonary regurgitation is the first to be reported in the literature.

Abstract:Abstract Background/Aims A 65-year-old man with a 20-year history of rheumatoid arthritis (RA) presented with dysphagia, odynophagia, polyarthralgia and significant weight loss. Methods - Results In addition to raised markers of inflammation, he was anti-nuclear antibody, anti-dsDNA antibody, anti-chromatin antibody and rheumatoid factor positive. He was initiated on 20mg once daily prednisolone but reported continued dysphagia and odynophagia. Computerised tomography (CT) of the neck revealed sclerosis at both cricoarytenoid joints with positron emission tomography (PET-CT) imaging identifying focal FDG uptake in the right hemiglottis projected over the posterior right true cord. Direct visualisation showed reduced mobility of the arytenoids with incomplete glottic closure in the absence of other abnormalities. He was diagnosed with cricoarytenoid (CA) joint arthritis on a background of RA and systemic lupus erythematosus overlap, with the resolution of his symptoms following intravenous methylprednisolone. Conclusion CA arthritis is an increasingly rare manifestation of autoimmune disease, and severe ongoing odynophagia an atypical presentation. This case highlights the need for continued awareness of laryngeal manifestations of autoimmune disease and demonstrates the benefit of PET-CT in cases where the diagnosis is uncertain or delayed. [Rheumatoid Leptomeningitis: a Case Report and Literature Review].

Abstract:To report the clinical, radiological and neuropathological findings of a patient with rheumatoid meningitis. The patient was a 71-year-old Chinese man with a two-year history of rheumatoid arthritis and no other significant medical history, who presented to our hospital recurrent weakness of his left extremities, dysarthria and a continuous bilateral hand tremor. Cerebrospinal fluid (CSF) and serum examinations were normal apart from a mildly raised serum perinuclear antineutrophil cytoplasmic autoantibody (p-ANCA). Brain magnetic resonance imaging (MRI) showed leptomeningeal enhancement in both frontal and parietal lobes, in addition to several old white matter infarcts. Meningeal biopsy showed numerous infiltrating macrophages and lymphocytes within the leptomeninges. The patient responded clinically and radiologically to corticosteroid and cyclophosphamide therapy. The patient subsequently developed herpes zoster over his left chest as a complication of his immunosuppressive treatment. His cyclophosphamide was ceased and intravenous immunoglobulin (IVIG) therapy was commenced, with good clinical response to both the herpes zoster and meningitis. According to the result of the biopsy, aseptic meningitis was considered the MRI results and the patient's clinical history were given, and a diagnosis of rheumatoid meningitis was made. The patient was p-ANCA positive. Although there was no evidence for cerebral vasculitis on biopsy, it remains a possibility that the patient's recurrent minor cerebral infarcts visible on MRI were vasculitic in nature.
[Coexisting Rheumatoid Arthritis and Ankylosing Spondylitis: 3 Cases Report].

Abstract:OBJECTIVE To understand the clinical profiles of coexisting rheumatoid arthritis (RA) and ankylosing spondylitis (AS). METHODS Coexisting RA and AS was discussed with 3 cases, 108 similar cases were found in the literature and reviewed here. RESULTS Coexisting RA and AS was rarely reported. The patients might have family histories of its related diseases, and initially present with diverse symptoms mainly in their youth. All the patients displayed severe symmetrical axial and peripheral arthritis. Joint destruction observed by imaging and the loss of functions were obvious. The prevalence of autoantibodies and HLA alleles was low. Most of the patients hardly demonstrated a good response to ordinary therapies. CONCLUSION Patients with coexisting RA and AS present with various symptoms initially and have considerable joint dysfunction as well as unfavorable outcomes. They should receive aggressive therapeutic strategies.
Bacteremia Possibly Caused by Helicobacter cinaedi and Associated with Painful Erythema in Rheumatoid Arthritis with Malignant Lymphoma

Abstract:We herein report the case of a 69-year-old woman with rheumatoid arthritis (RA) and malignant lymphoma who developed Helicobacter cinaedi bacteremia after starting rituximab plus cyclophosphamide, doxorubicin, vincristine, and prednisone (R-CHOP) chemotherapy. She had a recurrent fever and painful erythema for 13 months before the diagnosis was made. This delayed diagnosis was attributable to the underlying RA, which typically presents with various cutaneous manifestations and elevated C-reactive protein levels. The erythema on the thighs, abdomen, and left forearm improved following treatment with intravenous aminobenzyl penicillin; she received antibiotics for six weeks. This case emphasizes the importance of recognizing this opportunistic infection in immunocompromised patients.

Abstract:SESSION TITLE: Pulmonary Manifestations of Systemic Disease SESSION TYPE: Case Report Slide PRESENTED ON: Saturday, April 16, 2016 at 09:45 AM - 11:15 AM INTRODUCTION: Ankylosing spondylitis is a chronic inflammatory condition that usually affects young men. It is common to see series of related pulmonary manifestationseven in early stage. But the diagnosis of ankylosing spondylitis is often delayed for asymptomatic features. CASE PRESENTATION: A 34 years old male patient was admitted to our hospitial with chief complaint of "cough for 2 years, recurrent purulent sputum for 1 year, hemoptysis for 4 months, recurred for 1week". He denied lower back pain. 16 years ago he was diagnosed as lung tuberculosis and received anti-tuberculosis drugs for 8 months. his right hip ached intermittently, the symptom alleviated gradually without any treatment several months later. The patient experienced 3 episodes of acute pneumothorax. Physical examination showed extremly emaciation. multiple surgical scars on chest wall. Double lung percussion showed hyperresonance, auscultation showed roughbreath sounds without wheezes or crackles. Spinal forward bend, extend, rotation seemed slightly stiff, lateral normal. The thoracic mobility test: (2 cm); Other examiniations almost normal. Auxiliary examination:T - SPOT. TB negative; (bronchial secretions) tests: tuberculosis bacili, common bacteria cultivation, fungi, mycobacterium tuberculosis PCR negative, no fungus spore and mycelium; G test 31.18 pg/ml (negative); GM test 1.85 (positive) rheumatoid factor (RF): 20.7 IU/ml write, c-reactive protein (CRP): 102.00 mg/L write; The HLA-B27: +. Chest CT:double lung multiple pulmonary emphysema, bronchiectasis, multiple infection, left upper lobe cystic shadow Pericardial a small amount of effusion, bilateral partial pleura hypertrophy. Sacroiliac joint CT: bilateral sacroiliac arthritis and osteoarthritis stiffness. Lumbar positive side: lumbar bone not seen obvious abnormity. Diagnosis:Ankylosing spondylitis associated pulmonary damage mixed with infection (aspergillus and bacteria). DISCUSSION: This case is an asymptomatic ankylosing arthritis patient long affected by pulmonary damage, including "tuberculosis" diagnosing and treatment, pneumothorax, bronchiectasis, cavity, ground glass shadow and succeeded mixed pulmonary infection. Early diagnosis and intervention is really valuable, this case is an example of delayed diagnosis and treatment, which developed to damaged lung structure and repeated lung infections which led to treatment contradictions. CONCLUSIONS: Ankylosing spondylitis associated pulmonary damage maybe the only obvious and most important clinical clue for AS diagnosis and should be learned and carefully disdinguished especially in young men with upper lung lesions. Relapsing polychondritis causing breathlessness: Two case reports

Abstract:Background: Relapsing polychondritis is a rare multisystem autoimmune disease that mainly involves systemic cartilage and proteoglycan-rich tissues. If the larynx and trachea are involved, the patient's condition deteriorates rapidly. When relapsing polychondritis becomes more advanced, the airways collapse and treatment is difficult, rendering a poor prognosis. Therefore, the diagnosis method, treatment strategy and prognosis of relapsing polychondritis with larynx and trachea involvement need to be elucidated to improve clinicians' awareness of the disease. Case summary: A man and a woman were admitted because of breathlessness. Relapsing polychondritis was diagnosed after a series of accessory examinations. They were both treated with glucocorticoids and immunosuppressants, and underwent tracheotomy as their breathing difficulties could not be relieved by the medication. Conclusion: The two cases highlight the importance of the timely diagnosis, full evaluation and initiating individualized treatment of relapsing polychondritis with larynx and trachea involvement. Laryngoscopy, bronchoscopy and pathological examination are helpful in diagnosis of this disease.
