Abstract:Hepatic endometriosis has an extremely rare occurrence characterized by the presence of ectopic endometrium in the liver. A diagnosis of hepatic endometriosis is established after surgery. A 51-year-old multiparous female was referred to our unit for investigation of a liver tumor. The patient reported a 6-month history of epigastric pain and vomiting. She had undergone conservative hysterectomy for uterine leiomyomas several years earlier. The results of liver function tests and the levels of tumor markers (CA 19.9, CEA, CA125, αFP) were normal. Radiological imaging (USS, CT and MRI) suggested the presence of liver cystadenoma, liver cystadenocarcinoma or cystic metastasis of the liver in the left liver lobe extending to the diaphragm with left hepatic vein compression. Laparotomy was performed. The intraoperative frozen sections suggested a diagnosis of endometriosis. Anatomical resection was performed, including left lobectomy with diaphragm resection. The final histology confirmed the presence of hepatic endometrioma without malignant transformation. Fourteen cases of hepatic endometrioma have been described in the medical literature. We herein report the 15th case. Making a preoperative diagnosis of hepatic endometriosis is very difficult, despite conducting a complete investigation, in the absence of clinical and radiological characteristics. The diagnosis is made according to a histological examination of the whole surgical sample.

Primary Extrapelvic Umbilical Endometriosis Presenting With Cyclical Umbilical Bleeding: A Case Report

Abstract:Primary extrapelvic endometriosis is the presence of endometrial tissue in sites outside the uterine cavity in an individual who has had no prior abdominal surgeries. Various theories have been postulated to describe the etiology of endometriosis. Our case study involves a multiparous patient in her late 40s with no prior abdominal surgeries who presented with bleeding from her umbilicus associated with swelling and pain corresponding to her menstrual cycle. A computed tomography scan of the abdomen detected a homogenous granuloma-like umbilical soft tissue mass. The umbilical nodule and the umbilicus were excised, and the specimen was sent for histopathological examination that validated the diagnosis of an umbilical endometrioma by revealing endometrial glands with stroma involving the dermis. Postoperatively, the patient was symptomatically better and was discharged. Primary umbilical endometriosis can mimic conditions like omphalitis, umbilical granuloma, and umbilical hernia; hence, it is important to understand how to differentiate this case from other diagnoses. This case contextualizes that the likelihood of primary umbilical endometriosis in such unusual presentations must always be considered.

Abstract:Endometriosis is characterized by the ectopic implantation of a functional uterine lining outside of the uterus. Commonly, it occurs in the fallopian tubes, ovaries, uterosacral ligaments, and gastrointestinal tract. Less commonly, it may occur in the pericardium, pleura, and central nervous system. Thoracic endometriosis syndrome includes multiple presentations, most commonly catamenial pneumothorax. We present a case of a catamenial pneumothorax that was incidentally found on imaging after the patient presented with complaints of abdominal pain and weight loss.

Abstract:Hepatobiliary cystadenomas are rare cystic neoplasms that often occur in middle aged women. The exact etiology of these tumors is unknown. Diagnosis is often delayed in these cases. However, misdiagnosis and inappropriate treatment may result in unfavorable outcome. We report a case of hepatobiliary cystadenoma with pleural effusion. We also review the literature and discuss the current diagnostic and treatment modalities.

Abstract:Multifocal endometriosis found outside of the pelvis is very rare. We present here a case of endometriosis found in the pelvis, appendix and umbilicus. A 52-year-old female had a previous umbilical hernia repair, and years later started to develop a recurrent umbilical mass. After a full work-up, it was decided the patient have a diagnostic laparoscopy with wide local excision of umbilical mass to rule out any underlying malignancy. Findings during the procedure included an umbilical mass, dilated appendix and ovoid mass abutting the appendix. Pathology of the umbilical mass was found to be consistent with endometriosis. Umbilical and pelvic endometriosis is a rare condition. Options for diagnosis prior to surgical interventions are limited in endometriosis. In this case, ruling out underlying malignancy took priority, and the mass was removed and she will have less chance of recurrence.

Abstract:Introduction and importance: Endometriosis is a prevalent condition within the female reproductive age group, but its presentation and diagnosis still pose a challenge as it mimics other diseases and affects multiple organ systems. Case presentation: A 24-year-old nulliparous female presented with complaints of menorrhagia, lower abdominal pain, post-coital bleeding, and significant weight loss for 7 months. Clinical discussion: The case highlights the challenge of diagnosing endometriosis due to its ability to mimic other conditions, such as carcinoma cervix and rectum. The presence of elevated Cancer Antigen-125 (CA-125), typically associated with malignancy, underscores the need for a comprehensive evaluation to differentiate between endometriosis and other pathologies. Furthermore, the involvement of multiple organ systems emphasizes the systemic nature of endometriosis and the importance of considering it in the differential diagnosis of pelvic masses and symptoms in young women. Conclusion: Endometriosis, although a common disease among females of reproductive age its variation in presentation causes significant misdiagnosis and undertreatment. Multi-modal diagnosis and early treatment are necessary for proper outcomes.

Abstract:Hepatoid adenocarcinoma is a rare and unusual tumor in the female genital tract. Hepatoid adenocarcinoma resembles hepatocellular carcinoma morphologically but develops in extrahepatic organs, and usually demonstrates foci of adenocarcinoma of the primary organ. Tumor cells often stain positive for anti--fetoprotein antibody, and may be associated with elevated serum -fetoprotein, which may be useful as a tumor marker to guide treatment. There is little reliable information to guide clinical management of these unusual tumors and prognosis is poor despite multi-modal treatment. This report describes the diagnosis and treatment of this tumor in a postmenopausal woman.

Abstract:A 25-year-old nulliparous woman presented with ascites and pleural effusion for 2 months detected by multiple imaging tests (Fig. 1). She reported progressive dysmenorrhea in the past 10 years and denied other obvious symptoms. Her medical history included a thoracoscopic bullae resection performed six years ago due to spontaneous recurrent pneumothorax, pathology of which demonstrated chronic inflammatory and fibrotic changes and no signs of malignancy. One month after this surgery, the patient suffered another pneumothorax and achieved remission with conservative treatment. Since then, the patient has not experienced any severe chest symptoms and has not undergone any further reviews. Through meticulous inquiry into the patient's menstrual cycle during each pneumothorax episode, our gynecologist ascertained that every attack coincided with the patient's menstrual period and diagnosed the patient with catamenial pneumothorax. For further diagnosis and treatment, a multi-team surgery involving the gynecologist and the thoracic surgeon was performed. On the day of the operation, right-sided thoracoscopic surgery revealed extensive pleural adhesions, a large amount of hemorrhagic pleural effusion, and numerous dark brown nodules densely distributed on the parietal pleura (Fig. 2). 800 mL pleural effusion was drained and nodules were biopsied. Subsequently, laparoscopy revealed a massive pool of bloody ascitic fluid in the pelvic cavity. After suction of about 400ml ascites, pelvic endometriosis lesions were more clearly observed, including blisters approximately 0.5-2 cm in size on the surface of the uterus and chocolate cysts in bilateral ovaries. The patient then underwent bilateral ovarian cystectomy and electrocautery of peritoneal endometriosis. Figure. 2Operative findings during thoracoscopy (A-D). Abstract:Primary endometrial hepatoid adenocarcinoma is a rare tumor. We present a case of a 64-year-old woman with vaginal bleeding for 2 months and a markedly elevated serum alpha-fetoprotein of 3931 ng/mL. Hysteroscopic segmental curettage revealed endometrial adenocarcinoma. A pelvic MRI was performed for local tumor invasion, which showed only endometrial mass. An abdominal enhanced CT showed no liver lesion, which excluded primary hepatic carcinoma. Staging F-18-FDG PET/CT demonstrated a hypermetabolic mass in the endometrium. A total hysterectomy with bilateral salpingo-oophorectomy and regional lymphadenectomy were performed. Postoperatively, the mass was diagnosed as primary endometrial hepatoid adenocarcinoma.
Extracapsular Hepatocellular Adenoma: A Diagnostic Dilemma

Abstract:Hepatocellular adenoma (HCA) is a benign neoplasm of the hepatic parenchyma. The use of oral contraceptives (OCP) in women is the most well-established risk for the development of HCA. HCA commonly presents as an intracapsular mass of the liver but there are very few cases of extracapsular HCA. This is a case of a middle-aged female who presented to the emergency department with left shoulder pain and epigastric tenderness on physical exam. Subsequent imaging of the abdomen revealed a mass arising from the anterior wall of the stomach, with evidence of surrounding hemorrhage. The patient underwent exploratory laparotomy that revealed free blood in the peritoneum and a hemorrhagic mass arising from the stomach wall. The mass was successfully removed with no postoperative complications. Histopathological examination of the mass was consistent with an infarcted inflammatory HCA. This case illustrates this unusual presentation of a rare diagnosis.

Abstract:Endometriosis refers to the extrauterine presence of hormonally active endometrial glands and stroma. This ectopic endometrial tissue exhibits cyclic bleeding, inflammation, fibrosis and leads to formation of adhesions. Endometriosis affects about 10% to 15% of women between 15 to 45 years of age. Clinical presentations vary from infertility, dysmenorrhoea, chronic pelvic pain, deep dyspareunia and even bleeding at external sites like the umbilicus. Besides involving fallopian tubes, bowel, liver, thorax, pericardium, pleura etc, the most commonly affected areas in the gastrointestinal tract are the descending colon, rectosigmoid, appendix, and ileo-caecum in descending order of frequency. This case series highlights some unusual presentations of endometriosis along with co-existence of other ovarian pathologies which are not frequently encountered in clinical practice. This series also highlights the role of preoperative radiology for an adequate clinical diagnosis and complete surgical excision.

Abstract:Endometriosis is defined as the presence of endometrial tissue outside the uterus, which induces a chronic inflammatory response. Its prevalence remains unknown, but it has been estimated to affect up to 10% of women of reproductive age. Although it is a benign oestrogen-dependent gynaecological condition, women may describe painful symptoms such as cyclical pelvic pain, dysmenorrhoea and dyschezia. Intestinal endometriosis may affect the ileum, appendix, sigmoid colon and rectum. It may present with a myriad of symptoms such as abdominal pain, vomiting, diarrhoea, constipation and haematochezia. Caecal endometriosis can present as an acute appendicitis, making the diagnosis challenging to establish in pregnancy. Transmural involvement and acute occlusion are very rare events. The gold standard for diagnosis remains laparoscopy with tissue sampling for histological confirmation. Although endometriosis improves during pregnancy under the effect of progesterone, the ectopic endometrium becomes decidualised with a progressive reduction in size. The authors present the case of a multiparous woman in her mid-30s with acute onset of right-sided abdominal pain at 35 weeks gestation. Physical examination was suggestive of an acute appendicitis and MRI showed an inflamed caecum. She became acutely unwell requiring an emergency caesarean section. A mass in the caecum was observed with impending perforation at the caecal pole. A right hemicolectomy was performed. Histopathological examination confirmed the diagnosis of endometriosis with decidualisation. Although endometriosis improves during pregnancy, this case shows the unexpected complications of the disease and demonstrates the importance of considering endometriosis in the differential diagnosis of an acute abdomen in women of childbearing age to prevent maternal morbidity and fetal loss.

Abstract:Endometriosis is the ectopic implantation of endometrial tissue outside of the uterine cavity. Involvement of the gastrointestinal tract is common, particularly the rectosigmoid colon; however, small bowel involvement is rare. The ileocecal junction and terminal ileum are affected in 4% of women with bowel endometriosis. Endosalpingiosis on the contrary, is marked by the ectopic presence of fallopian tube tissue and has prevalence of approximately 7% in premenopausal women. We present a unique case of endometriosis with proliferation to the terminal ileum with concurrent endosalpingiosis within pericolic lymph nodes mimicking carcinoid tumor.

Abstract:Endometriosis, characterized by the ectopic implantation of endometrial tissue, typically involves pelvic structures but infrequently extends to extrapelvic sites such as the gastrointestinal tract. In this report, we present a case of a 44-year-old woman with diffuse abdominal discomfort and constipation. Computed tomography imaging revealed a mass in the cecum, leading to diagnostic colonoscopy and subsequent magnetic resonance imaging. Ultimately, laparoscopy revealed a puckered cecal mass suspected to be invasive malignancy but pathologically confirmed as extensive endometriosis involving the colonic wall and pericolic lymph nodes. Cecal volvulus secondary to endometriosis is exceedingly rare, with surgical intervention being the primary treatment modality. Endometriosis-associated gastrointestinal involvement often masquerades as irritable bowel syndrome, highlighting the importance of considering atypical presentations in young women. This case underscores the need to maintain a high index of suspicion for unusual etiologies in patients with gastrointestinal symptoms and atypical imaging findings to facilitate timely diagnosis and management.

Abstract:Hydatid cyst disease, caused by the larval stage of Echinococcus granulosus, is a parasitic infection endemic in many regions, including South Asia. We present a case of a 36-year-old South Asian female with concurrent liver and peritoneal hydatid cysts, emphasizing the diagnostic challenges and management complexities associated with this condition. The patient presented with abdominal pain, nausea, and decreased appetite, and imaging studies revealed characteristic cystic lesions in the liver and peritoneum. Initial medical management with albendazole was followed by surgical excision due to inadequate response to therapy. Postoperative care included prophylactic albendazole to prevent recurrence. This case highlights the importance of a multidisciplinary approach involving medical therapy and surgical intervention tailored to the individual patient's needs and disease presentation.

Abstract:Epiploic appendagitis (EA) is an uncommon cause of acute abdominal pain that may mimic other causes of acute abdomen. Epiploic appendages are outpouching of fat tissue located on the external wall of the colon, being more numerous in the descending and sigmoid colon that account for up to 80% of EA cases. We present the case of a 59-year-old woman with right upper quadrant pain. Abdominal ultrasound and contrast-enhanced computed tomography suggested the diagnosis of epiploic appendagitis of the right colonic flexure. Our case highlights the fact that epiploic appendagitis may occur in unusual locations and must be included in the differential diagnosis of acute abdominal pain, in order to avoid unnecessary medical and surgical treatment.
Spontaneous umbilical endometriosis: Rare occurrence in nulliparous women - Case report & literature review

Abstract:Introduction and importance: Endometriosis is an inflammatory disease associated with pelvic pain and infertility that is characterized by lesions of endometrial-like tissue outside of the uterus. It affects roughly 10 % of reproductive age women and girls globally. Umbilical endometriosis is a rare pathology accounting for 0.5 to 1 % of all extra-pelvic endometriosis. Due to the varied presentations and rare incidence of endometriosis, it remains a diagnostic dilemma and challenge to treat it timely and properly. Case presentation: A 32-year-old nulliparous lady who presented with umbilical lump, cyclical pain, and bleeding during menstrual cycle without any prior history of abdominal surgery of a year duration. Ultrasound and magnetic resonance imaging support the diagnosis of umbilical endometriosis and ruled out multifocal involvement. She was managed with surgical excision with free margin followed by umbilical reconstruction and the diagnosis was confirmed by biopsy. Discussion: Umbilical endometriosis could be either secondary to endometrial tissue implantation during laparoscopic or open surgical procedures also called scar endometriosis or as a primary umbilical endometriosis with no previous surgery. The characteristic presentation for umbilical endometriosis is a brown to dark nodule in the umbilicus, which may be swollen, painful, and sometimes bleed during menstrual periods. Conclusion: Umbilical endometriosis is a rare condition that should be considered as a differential diagnosis in women with umbilical lump, cyclical pain, and bleeding. The diagnosis is clinical and confirmed by histopathology. Surgical excision is the treatment of choice with a low risk of recurrence or malignancy.
Endometrioid adenocarcinoma arising from abdominal wall endometriosis: A case report and literature review

Abstract:Endometriosis, affecting 6%-10% of women of reproductive age, can lead to severe symptoms such as chronic pelvic pain and infertility. Among its rarer manifestations is abdominal wall endometriosis (AWE), which has been increasingly reported following cesarean deliveries. This case discusses a 39-year-old woman who presented with a 13-year history of cyclical pain at her cesarean section scar, exacerbated over the last year by the development of a painful abdominal mass. Medical evaluations indicated endometriosis at the scar, with further investigations including ultrasound and magnetic resonance imaging showing involvement of the rectus abdominis muscle. Elevated tumor markers HE4 and CA-125, along with a biopsy, confirmed adenocarcinoma. The patient underwent extensive surgical treatment, including the resection of the mass, hysterectomy, bilateral salpingo-oophorectomy, and lymphadenectomy. Pathology confirmed moderately differentiated infiltrative adenocarcinoma originating from endometriosis. Despite the absence of postoperative chemotherapy, the patient showed no recurrence, emphasizing the effectiveness of comprehensive surgical management. This case highlights the critical importance of recognizing the potential for malignant transformation in AWE, particularly following cesarean deliveries, and underscores the necessity for vigilant monitoring and personalized treatment strategies. The management of AWE, especially when malignant transformation is suspected, necessitates a multidisciplinary approach similar to that used in ovarian cancer, focusing on rigorous surgical intervention and the potential for adjuvant therapies.
