Abstract:Postpartum sexual health has historically been viewed and discussed in specific ways, often dominated by biomedical discourse. There is a need to expand understandings of sexual health for postpartum women in the context of interdisciplinary health care. Research surrounding postpartum sexual health is largely focused on physical measures, such as vaginal lubrication or initiation of intercourse, without accounting for the diverse and subjective ways that sexuality and sexual health are experienced during the postpartum period. This critical analysis uses feminist post-structuralism to critique and analyze current health research and practice surrounding postpartum sexual health. Agency, subjectivity, gender and sex considerations, relations of power, and discourse are essential to understanding postpartum sexual health in a more holistic, woman-centered way. This includes awareness of dominant discourses that have shaped how health researchers, practitioners, postpartum women, and health institutions care for, support, and promote postpartum sexual health. There is a need to move beyond physically focused, reductionist, heteronormative understandings of sexual health to better promote overall postpartum health and wellbeing.

Abstract:Background: The reduced number of postpartum hospitalization days and decrease in providing intensive parenting guidance experienced in Taiwan in recent years may impact negatively on postpartum mothers. Moreover, these changes have reduced the opportunities available to professional caregivers to learn about the physical impacts of the postpartum period on new mothers. Purpose: This study was designed to explore how postpartum women perceive their feelings and experience their bodies and roles after childbirth. Methods: Based on Merleau-Ponty's theory, this qualitative study included purposive sampling at two medical centers in northern Taiwan. Information on bodily changes and self-perceptions during postpartum care was collected using in-depth, open-ended interviews with women between two and six months postpartum and was analyzed using interpretative phenomenological analysis. Results: Two major themes were identified. The first theme, "My postpartum body is not my own", indicates that postpartum women perceive their bodies as no longer belonging to themselves. The second theme, "Refamiliarization with the postpartum body", describes how the participants adapted, accepted, recognized, and gave new meaning to the changes in their bodies during the postpartum period. Conclusions / implications for practice: Although women may feel that they lose their identity and become "objects" after childbirth, this experience may trigger an opportunity for transformation that allows them to re-identify as "subjects. " By listening with respect, clinicians can help postpartum women understand they are in control of their lives. This may help postpartum women identify their capabilities and embrace postpartum life with greater optimism.
A systematic review of influences and outcomes of body image in postpartum via a socioecological framework

Abstract:Background: Women in the postpartum period come under multi-level pressure to return to pre-pregnancy body shape and size. This pressure can lead to reduced body image satisfaction, self-esteem, and mood. In this systematic review we explored the influences and outcomes of body dissatisfaction during the postpartum period. Methods: Four databases were searched using keywords: postpart* OR postnatal OR peripart* OR 'new mother' OR 'breast feed*' AND 'body image' OR 'body dissatisf*' OR 'body satisf*' OR 'body attitude' OR 'body shape'. Results: The influences and outcomes of body image dis/satisfaction in the postpartum period from 55 international studies were found to align within four of the five factors of the socioecological model. Intrapersonal factors: weight and body shape concerns, mental health and stressors, attitudes and behaviours, and protective coping skills and interventions. Interpersonal factors: social support (partner, family, and friends) and sexual functioning. Institutional factors: experiences with the healthcare system and returning to work. Societal factors: culture and ethnicity, media influences and social norms such as the thin ideal. Conclusions: A focus on intrapersonal factors alone is insufficient to understand women's experiences of body dis/satisfaction during the postpartum period. This suggests a need for better education and policy practices in pre-natal and postpartum care directed at body image and education to dispel societal norms such as the thin ideal. Doing so shifts the focus to include interpersonal, institutional, and societal influences alongside intrapersonal experiences. Research is needed to explore the utility and efficacy of broader approaches for women during this vulnerable life period.

Abstract:Pregnancy represents a unique time during which women's bodies undergo significant physical changes (e.g., expanding belly, larger breasts, weight gain) that can elicit increased objectification. Experiences of objectification set the stage for women to view themselves as sexual objects (i.e., self-objectification) and are associated with adverse mental health outcomes. Although women may experience heightened self-objectification and behavioral consequences (such as body surveillance) due to the objectification of pregnant bodies in Western cultures, there are remarkably few studies examining objectification theory among women during the perinatal period. The present study investigated the impact of body surveillance, a consequence of self-objectification, on maternal mental health, mother-infant bonding, and infant socioemotional outcomes in a sample of 159 women navigating pregnancy and postpartum. Utilizing a serial mediation model, we found that mothers who endorsed higher levels of body surveillance during pregnancy reported more depressive symptoms and body dissatisfaction, which were associated with greater impairments in mother-infant bonding following childbirth and more infant socioemotional dysfunction at 1-year postpartum. Maternal prenatal depressive symptoms emerged as a unique mechanism through which body surveillance predicted bonding impairments and subsequent infant outcomes. Results highlight the critical need for early intervention efforts that not only target general depression, but also promote body functionality and acceptance over the Western "thin ideal" of attractiveness among expecting mothers.

Abstract:The present inquiry examines the psychosocial meanings and processes that shape consumers' sense of body image and the consumption behaviors motivated by those perceptions. Poststructuralist interpretive procedures were used to analyze interviews with 30 male and female consumers, aged 6–54. This discourse analysis led to the development of three process-orientated themes: (1) the ideology of self-control, (2) the social processes of normalization and problematization, and (3) the operation of the disciplinary gaze. The systematic manifestations of these themes are illustrated across a range of consumer experiences and body-focused perceptions. Implications of these themes for the theoretical conceptualizations of body image and the nature of self-concept in contemporary consumer culture are discussed.

Abstract:Objective: To analyze puerperal women's experiences of sexual health after childbirth from the perspective of symbolic interactionism. Method: Descriptive, qualitative study. Twenty semi-structured interviews were conducted by videoconference with women in the remote puerperium, captured by snowball technique and searched for "seeds" on Instagram®. Bardin's content analysis and Symbolic Interactionism were used as references. Results: The puerperal women signify sexual health from a perspective of comprehensive healthcare. However, due to the duality between "being a woman" and "being a mother", they recognize fear, bodily transformations and changes in focus from the love relationship to caring for the baby as factors that interfere with sexual health. And they choose to put themselves aside, prioritizing caring for others. They re-signify sexual health by recognizing the importance of taking care of themselves in biopsychosocial aspects and try to recover self-care for a healthy sexual experience. Conclusion: Despite the meanings attributed, women's social interactions with the puerperium interfere negatively with sexual health. Professionals should be sensitized to the inclusion of actions that promote changes in the social action of these women towards self-care.

Abstract:Introduction Childbirth is a period of imbalance responsible for multidimensional changes that can influence sexual life. The aim of the study is to study the sexuality of women after childbirth, to specify the factors that influence it and to analyze the aspects of sexual dysfunction that accompany this period. Objective Evaluate women's postpartum experience and sexuality, while identifying difficulties that may arise during this high-risk period. Assess women's satisfaction with the information they receive on sexuality during perinatal and post-partum consultations. Methods This was a descriptive study that was spread over a period of 6 months from July 1, 2021 to December 31, 2021.200 women who gave birth in the six months preceding the study were included in the study. Two self-questionnaires were sent by internet through a social network. Results Our population of 200 women had a mean age of 34.3 years and the majority of women were urban, and university-educated. fourty-five per cent were second gestations and 58,5% of them had given birth by cesarean section. The average FSFI score in our sample was 21,99/36 postpartum, which implied sexual dysfunction in 72% of the women in our population. The delay in resumption of sexual activity was later in primiparous women, women with vaginal delivery and breastfeeding women. There was an association between instrumental delivery and the degree of dyspareunia. No difference was found in sexuality between vaginal delivery and caesarean section, nor for scheduled or emergency caesarean section. All areas of female sexual function were affected with mainly pain associated with sexuality. Some factors influencing the resumption of sexual activity could be identified such as breastfeeding, the presence of perineal tears, lack of lubrication and fear of becoming pregnant. Conclusions The majority of women in our population had sexual dysfunction in the postpartum period. The different areas of female sexual function were affected and correlated with the different parameters related to childbirth and the postpartum period. Disclosure No.

Abstract:Introduction Pregnancy and postpartum is an important life event associated with profound physiognomic and psychosocial changes affecting the female body in all its physiological, psychic and affective reality. It might influence the sexual function in expectant mothers. Objectives To investigate the relationship between the body satisfaction and perception and the sexual function among pregnant and postpartum women. Methods It was a cross-sectional study established over a period of 3 months from the June 1 st , 2023 to August 31, 2023. This study focused on a population of pregnant and postpartum women recruited from outpatient consultations and inpatient of the obstetric gynecology department at the university hospital of Gabes. We used a pre-established sheet exploring socio-demographic data, medical and gyneco-obstetric history, the body mass index (BMI) and informations concerning the marital relationship and the woman’s sexual activity. We administered the validated Arabic version of the Arizona Sexual Experiences Scale (ASEX) to assess sexual functioning and we used the body satisfaction and global self-perception questionnaire (QSCPGS) to explore the body satisfaction and perception. Results Fifty-eight women were included. The average age was 35.6±5.5 years; they were from an urban origin in 75%. They were pregnant in the first, second and third trimester in (15.6%, 15.6% and 25% respectively). They were in postpartum in 43.8% of cases with a cesarean delivery in 73.3% and breastfeeding in 56%. All women reported being on good terms with their spouses and satisfied with their sexuality. The usual frequency of sexual relations was (1/day: 22.6%, 1/week: 74.2%, 1/month: 3.2%) and 25% reported wanting to reduce the frequency. The mean ASEX score was 13 ± 4.3 and 47% of the sample had sexual dysfunction. For the total score of the QSCPGS, we observe a mean value of 33 ±28.3, which means that our sample has a good level of positive body satisfaction and self-perception. The mean value of the “body satisfaction” factor is higher (23.7 ± 10.4) than the mean value of the “self-perception” factor (11.4 ± 14.3). The mean value of BMI was 28.74 ± 4.4 wich means an overweigh. We found a significant association between the “body satisfaction” factor and the sexual dysfunction (p=0.03), insufficient lubrication (p=0.01) and difficulty reaching orgasm (p=0.001). Conclusions We found that body and physical changes among pregnant and postpartum women can negatively affects their body perception and it might deteriorate its global sexual function. Body after baby: a pilot survey of genital body image and sexual esteem following vaginal birth

Abstract:Objective: The aim of this study was to determine acceptability of the Vaginal Changes Sexual and Body Esteem (VSBE) scale for women post childbirth and explore the association between childbirth events and sexual/body esteem. Design: This is a cross-sectional study within the Evaluating Maternal Recovery from Labor and Delivery study. Setting: This study was conducted in a community setting. Population: The study was conducted in women post first vaginal birth with birth events that posed risk factors for levator ani muscle tears. Methods: Survey, magnetic resonance images of levator ani, and physical examination were the data collected 8 months postpartum. Birth variables were collected by hospital chart review. Descriptive analysis of VSBE response rates and distribution of responses was conducted. An exploratory analysis of the potential association of demographic, birth, clinical, and magnetic resonance image characteristics with VSBE scores was conducted. Main outcome measures: The outcome measure used in this study is VSBE scale. Results: The majority of participants (97%) completed the scale, with responses to most questions skewed toward positive sexual/body esteem, with the exception of sexual enjoyment, where 38% indicated some interference due to genital changes. The scale showed high internal consistency (alpha =0.93). In the exploratory analysis of potential characteristics associated with VSBE, women with episiotomies had lower sexual/body esteem compared to those who did not (median VSBE scores 35 vs 42.5, P=0.01). Anal sphincter tear was not associated with sexual/body esteem (P=0.78). Additional study is indicated to further explore observed trends toward the association of severe levator ani tear, maternal age at childbirth, and forceps with VSBE scores. Conclusion: The VSBE is suitable for use to assess sexual/body esteem in women post childbirth. Most women in this sample did not indicate negative genital body image/sexual esteem. However, some indicated that the changes post birth negatively affected their sexual/body esteem, particularly those who had episiotomies.

Abstract:The objective of this paper is to explore mothers' psychological adaptation related to separation from their newborn children immediately after birth. Mother and child separation has traditionally received attention from the child's perspective, but given that the bond is dyadic, the mother's wellbeing should also be considered. This qualitative study is based on interview data with mothers of premature infants. From the analysis, three themes emerged: concerns during pregnancy about premature birth; emotional strain caused by separation; and the need to protect and to be close to the newborn. Drawing on the perspectives of dialogical self theory and semiotic regulation model, the paper will focus on intra-psychological dynamics and will analyze the adaptation process in terms of I-positions. Based on the analysis, the maternal bond represents the integration of the I-position I-as-mother and My child, which is integrated during the pregnancy into the core of the self. It can be assumed that rupture of the self-continuity is activated by the birth of the child, which is then compounded if the child leaves. The maternal superordinate viewpoint (meta-I-position) directs a woman's behavior and allows for a sense of coherence under dynamic organization related to the birth of the child. The potential for psychological adaptation is presented as an ability to establish self-continuity.

Abstract:Childbearing/rearing families in Canada face a variety of conflicting discourses related to infant feeding, entrenched in a complex web of gendered, social, institutional and political discourses. For parents of preterm and/or critically ill infants, this area remains largely under-explored through a feminist lens. We offer a critical examination of the applicability of feminist poststructuralism (FPS) as a theory to explore infant feeding interactions in the neonatal intensive care unit (NICU). Scholarly literature from diverse perspectives, including nursing, healthcare, gender studies, and social sciences is critiqued and the use of FPS as a guiding framework for nursing research and praxis is discussed. We discuss FPS and the relevance of various discourses to explore the phenomenon of infant feeding interactions in the NICU. Ultimately, we propose that FPS does offer a relevant lens through which to critically examine infant feeding interactions and bring voice to the complex processes embedded in the NICU.
Two parts of an indivisible whole - Midwifery education and feminism: An exploratory study of 1st year students' immersion into midwifery

Abstract:Background: Emancipation and self-determination are critical elements of midwifery care and therefore should be explicit in midwifery education. If not, the woman-centred midwife with her trust in women and birth may become a thing of the past, and the patriarchal, technocratic medical model of maternity care, with its trust in machines and misguided interventions will continue to dominate childbearing practices. The optimal time for exposure to feminist principles within the midwifery educational journey, however, is unknown, despite recognition that teaching feminist theory and related concepts positively impacts the way students value women-centredness in midwifery practice. Objective: To understand midwifery student's perspectives of assimilating feminist theory and midwifery philosophy. Design: A qualitative approach using reflective journals was used to explore student midwives understanding of midwifery as a feminist profession. Setting: One regional Australian University that teaches midwifery at two campuses, one of which straddles a state border. Participants: First session, first-year midwifery students undertaking a unit of study focusing on "what is midwifery" and how at its core, midwifery is a feminist emancipatory political discipline. Methods: Data from weekly reflective journals were analysed to produce themes. Findings: Three themes were identified 'Midwifery: Past and Present', 'What is this 'F' word? - feminism revisited', and 'Midwifery-feminism dyads'. These themes demonstrated transformative learning had occurred as participants appeared to value understanding feminism as the essence of midwifery philosophy early in their midwifery studies. Conclusion: Students embarking on their journey appear to value assimilation of feminist theory as a core tenet of midwifery philosophy. Feminist principles, particularly the protection of women's rights to informed choice, trusted relationships, dignity, and control throughout their childbearing journey appear to illustrate the emancipatory nature, and importance of, truly 'woman-centred, partnership-based midwifery care'. Recommendations for practice and further research: Whilst the findings of this study relate to midwifery students, the findings point to a need to explore ways to strengthen midwives' assimilation with feminist theory, and their ability to promote feminism and provide woman-centred, partnership-based approaches.

Abstract:Drawing on examples from in-depth interviews with 49 women, in this article we aim to open up a discursive space for women and health professionals to begin to explore the phenomenon of the interembodied experience of breastfeeding. Although acknowledging that social dimensions partially constitute the lived body, we further the view that the lived body’s understanding is embedded in contexts far more complex than those that can be represented by language. We argue that women’s narratives of their breastfeeding experience contained instances of the body “understanding” its emotional task at a prelogical, preverbal level. We identified three central, iterative dimensions of the phenomenon— calling, permission, and fulfillment—that occurred prereflexively in the protected space provided by the mother, a space that was easily disrupted by unsupportive postnatal practices. We offer this eidetic understanding and conceptual framework and suggest that it provides new (less damaging) subject positions and ways of behaving.

Abstract:The current study investigated how foundational conversations about the body and sexuality begin, how they develop longitudinally, and whether parental body talk varies as a function of characteristics of both the parent and child. Participants included 442 mothers ( M age = 32.50, SD = 5.61, 93% heterosexual, 85% White) and 337 available father-figures ( M age = 33.99, SD = 5.68, 98% heterosexual, 80% White), who were the parents of infants (age at Time 1 = 28.13 months, SD = 6.85, 52% male). Factor analyses suggested that parents engaged in positive, avoidant, and scolding body talk with their young children, and growth curve analyses suggested that frequency of positive body talk increased from when the child was age two to when they were age four for both mother- and father-figures. Results also suggested that the most consistent predictors of body talk included parents' perceptions of whether their child was too young to discuss sexuality, and the child's own questions and behaviors about their bodies and sexuality. The discussion focuses on the frequency of parental body talk and the active role of the child in the early socialization of healthy sexuality.

Abstract:Drawing on Black feminist theories of intersectionality and concepts of temporality and embodiment, this study centers Black girlhood to understand Black girls embodied journeys of "becoming" Black women. Findings revealed that: (a) Black girls often have complicated histories that lead to constrained and liberatory choices in adulthood and motherhood; (b) becoming a Black woman involves continual adaptations to temporal and historical contexts; and (c) Black motherhood is a contested space, deeply shaped by remembering Black girlhood and navigating maternal responses to legacies of harm. This study highlights the challenges and strengths in the transitory states of Black womanhood.

Abstract:Postpartum support for new parents can normalize experiences, increase confidence, and lead to positive health outcomes. While in-person gatherings may be the preferred choice, not all parents can or want to join parenting groups in person. Online asynchronous chat spaces for parents have increased over the past 10 years, especially during the COVID pandemic, when "online" became the norm. However, synchronous postpartum support groups have not been as accessible. The purpose of our study was to examine how parents experienced postpartum videoconferencing support sessions. Seven one-hour videoconferencing sessions were conducted with 4-8 parents in each group (n = 37). Nineteen parents from these groups then participated in semi-structured interviews. Feminist poststructuralism and sociomaterialism were used to guide the research process and analysis. Parents used their agency to actively think about and interact using visual (camera) and audio (microphone) technologies to navigate socially constructed online discourses. Although videoconferencing fostered supportive connections and parents felt less alone and more confident, the participants also expressed a lack of opportunities for individual conversations. Nurses should be aware of the emerging opportunities that connecting online may present. This study was not registered.

Abstract:Guided by the developmental theory of embodiment, the current study explored feminist embodiment as a protective factor that may simultaneously promote more positive (and less negative) body talk and body image outcomes among mothers and their young adult daughters. Results from an expanded actor-partner interdependence model (APIM; N = 169 dyads) revealed that our construct of feminist embodiment, which was composed of feminist attitudes, mind-body connection, and psychological empowerment, has potential benefits for both mothers and daughters. Specifically, actor effects indicated that feminist embodiment was positively associated with both mothers' and daughters' body image, and it was indirectly associated with both women's own body image through more positive (and less negative) body talk. Additionally, partner effects revealed a number of direct effects between one's own body talk and the other's body image, and also that daughters' feminist embodiment was indirectly associated mothers' body image through daughters' positive body talk. This study highlights the empowering and supportive nature of mother-daughter relationships and suggests the potential body image benefits of women experiencing feminist embodiment.

Abstract:There is a well-established body of literature on parent-child sexual communication (PCSC) among Black families. Still, most of this work focuses on reducing sexual risk behaviors among Black girls and focuses less on the multidimensionality of PCSC among Black parents and their daughters. In this qualitative study, we explored Black mother-daughter PCSC by analyzing interviews with 29 Black mothers in the U.S. with daughters ages 6–11. We used consensual qualitative research methods to analyze mother's responses to questions about the messages and strategies they used when educating their daughters about sex-related topics, such as pleasure and sexual identity. We identified the following themes related to mothers' goals with their messages: (1) increasing awareness through neutral information, (2) building daughter's sense of self, (3) challenging societal norms and White heteropatriarchy, and (4) upholding societal norms and White heteropatriarchy. We also developed five themes for mother's strategies for PCSC: (5) direct and open PCSC, (6) modeling, (7) integrating real-life examples, (8) centering the child, and (9) monitoring and tailoring daughters' experiences with others. Our findings add to existing research on sex-positive practices among Black families, highlight the need to use critical lenses when exploring PCSC, and have important implications for parent education.
