Abstract:Cardiovascular disorders cause nearly one in three deaths in the United States. Short- and long-term care for these disorders is often determined in short-term settings. However, these decisions are made with minimal longitudinal and long-term data. To overcome this bias towards data from acute care settings, improved longitudinal monitoring for cardiovascular patients is needed. Longitudinal monitoring provides a more comprehensive picture of patient health, allowing for informed decision making. This work surveys sensing and machine learning in the field of remote health monitoring for cardiovascular disorders. We highlight three needs in the design of new smart health technologies: (1) need for sensing technologies that track longitudinal trends of the cardiovascular disorder despite infrequent, noisy, or missing data measurements; (2) need for new analytic techniques designed in a longitudinal, continual fashion to aid in the development of new risk prediction techniques and in tracking disease progression; and (3) need for personalized and interpretable machine learning techniques, allowing for advancements in clinical decision making. We highlight these needs based upon the current state of the art in smart health technologies and analytics. We then discuss opportunities in addressing these needs for development of smart health technologies for the field of cardiovascular disorders and care.

Abstract:Cardiovascular health informatics is a rapidly evolving interdisciplinary field concerning the processing, integration/interpretation, storage, transmission, acquisition, and retrieval of information from cardiovascular systems for the early detection, early prediction, early prevention, early diagnosis, and early treatment of cardiovascular diseases (CVDs). Based on the first author's presentation at the first IEEE Life Sciences Grand Challenges Conference, held on October 4-5, 2012, at the National Academy of Sciences, Washington, DC, USA, this paper, focusing on coronary arteriosclerotic disease, will discuss three significant challenges of cardiovascular health informatics, including: 1) to invent unobtrusive and wearable multiparameter sensors with higher sensitivity for the real-time monitoring of physiological states; 2) to develop fast multimodal imaging technologies with higher resolution for the quantification and better understanding of structure, function, metabolism of cardiovascular systems at the different levels; and 3) to develop novel multiscale information fusion models and strategies with higher accuracy for the personalized predication of the CVDs. At the end of this paper, a summary is given to suggest open discussions on these three and more challenges that face the scientific community in this field in the future.

Abstract:Background: Cardiovascular disease and stroke are common and costly, and their prevalence is rising. Forecasts on the prevalence of risk factors and clinical events are crucial. Methods: Using the 2015 to March 2020 National Health and Nutrition Examination Survey and 2015 to 2019 Medical Expenditure Panel Survey, we estimated trends in prevalence for cardiovascular risk factors based on adverse levels of Life's Essential 8 and clinical cardiovascular disease and stroke. We projected through 2050, overall and by age and race and ethnicity, accounting for changes in disease prevalence and demographics. Results: We estimate that among adults, prevalence of hypertension will increase from 51.2% in 2020 to 61.0% in 2050. Diabetes (16.3% to 26.8%) and obesity (43.1% to 60.6%) will increase, whereas hypercholesterolemia will decline (45.8% to 24.0%). The prevalences of poor diet, inadequate physical activity, and smoking are estimated to improve over time, whereas inadequate sleep will worsen. Prevalences of coronary disease (7.8% to 9.2%), heart failure (2.7% to 3.8%), stroke (3.9% to 6.4%), atrial fibrillation (1.7% to 2.4%), and total cardiovascular disease (11.3% to 15.0%) will rise. Clinical CVD will affect 45 million adults, and CVD including hypertension will affect more than 184 million adults by 2050 (>61%). Similar trends are projected in children. Most adverse trends are projected to be worse among people identifying as American Indian/Alaska Native or multiracial, Black, or Hispanic. Conclusions: The prevalence of many cardiovascular risk factors and most established diseases will increase over the next 30 years. Clinical and public health interventions are needed to effectively manage, stem, and even reverse these adverse trends.

Abstract:In 1924, the founders of the American Heart Association (AHA) envisioned an international society focused on the heart and aimed at facilitating research, disseminating information, increasing public awareness, and developing public health policy related to heart disease. This presidential advisory provides a comprehensive review of the past century of cardiovascular and stroke science, with a focus on the AHA’s contributions, as well as informed speculation about the future of cardiovascular science into the next century of the organization’s history. The AHA is a leader in fundamental, translational, clinical, and population science, and it promotes the concept of the “learning health system,” in which a continuous cycle of evidence-based practice leads to practice-based evidence, permitting an iterative refinement in clinical evidence and care. This advisory presents the AHA’s journey over the past century from instituting professional membership to establishing extraordinary research funding programs; translating evidence to practice through clinical practice guidelines; affecting systems of care through quality programs, certification, and implementation; leading important advocacy efforts at the federal, state and local levels; and building global coalitions around cardiovascular and stroke science and public health. Recognizing an exciting potential future for science and medicine, the advisory offers a vision for even greater impact for the AHA’s second century in its continued mission to be a relentless force for longer, healthier lives.

Abstract:Cardiovascular disease (CVD) remains the leading cause of morbidity and mortality worldwide and challenges the capacity of health care systems globally. Atherosclerosis is the underlying pathophysiological entity in two-thirds of patients with CVD. When considering that atherosclerosis develops over decades, there is potentially great opportunity for prevention of associated events such as myocardial infarction and stroke. Subclinical atherosclerosis has been identified in its early stages in young individuals; however, there is no consensus on how to prevent progression to symptomatic disease. Given the growing burden of CVD, a paradigm shift is required—moving from late management of atherosclerotic CVD to earlier detection during the subclinical phase with the goal of potential cure or prevention of events. Studies must focus on how precision medicine using imaging and circulating biomarkers may identify atherosclerosis earlier and determine whether such a paradigm shift would lead to overall cost savings for global health.

Abstract:Early-career academic cardiologists, who many believe are an important component of the future of cardiovascular care, face myriad challenges. The Early Career Section Academic Working Group of the American College of Cardiology, with senior leadership support, assessed the progress of this cohort from 2013 to 2016 with a global perspective. Data consisted of accessing National Heart, Lung, and Blood Institute public information, data from the American Heart Association and international organizations, and a membership-wide survey. Although the National Heart, Lung, and Blood Institute increased funding of career development grants, only a small number of early-career American College of Cardiology members have benefited as funding of the entire cohort has decreased. Personal motivation, institutional support, and collaborators continued to be positive influential factors. Surprisingly, mentoring ceased to correlate positively with obtaining external grants. The totality of findings suggests that the status of early-career academic cardiologists remains challenging; therefore, the authors recommend a set of attainable solutions.

Abstract:Cardiovascular disease (CVD) is the leading cause of death worldwide. Population aging is becoming the most important driver of the CVD epidemic. With the rapid increase in an aging population, the burden of CVD will continuously increase. Most old people also suffer multimorbidity, which is strongly associated with impaired quality of life, disability, dependence, and mortality. However, few reviews evaluated the CVD burden accompanied by population aging and the challenges of CVD care in elderly individuals with multimorbidity. This review identified and summarized the current status of the CVD epidemic associated with aging and highlighted the challenges and needs of CVD care for the elderly.

Abstract:The American Heart Association (AHA), founded in 1924, is anchored in the core belief that scientific research can lead the way to better prevention, treatment, recovery, and ultimately a cure for cardiovascular disease. Historically, the association's involvement in international efforts centered on scientific cooperation. Activities mostly involved AHA leadership presenting at international scientific meetings and leaders from other countries sharing scientific and medical information at AHA meetings. Although the AHA's and American Stroke Association's international efforts have expanded substantially since those early days, global knowledge exchange remains the bedrock of its international endeavors. As the AHA turns 100, we reflect on the successful global efforts in prevention, resuscitation, global advocacy, quality improvement, and health equity that have guided the organization to a place of readiness for "advancing health and hope, for everyone, everywhere." Motivated by the enormous potential for population health gains in an aging world, the AHA is entering its second century with redoubled commitment to improving global cardiovascular and cerebrovascular health for all.

Abstract:Our knowledge of the epidemiology of coronary heart disease (CHD) is extensive. Although CHD mortality peaked in the 1960s and then declined dramatically,1,2 CHD remains the leading cause of death in both men and women in the United States. The aging of the United States population contributes to the high prevalence of CHD. By the year 2020, it is estimated that ischemic heart disease will be the number one cause of disability and death worldwide.3 Data on noncoronary atherosclerotic vascular disease (AVD) are far less available than data on CHD. Studies of the atherosclerotic process underlying ischemic stroke are hampered by the differing vascular mechanisms that lead to stroke, including large-artery atherosclerosis with occlusion and distal embolization and small penetrating arterial disease with lacunar infarction. Also, there are a considerable number of ischemic stroke events for which the mechanism is unclear and the infarct is of undetermined cause. What is known about the epidemiology of atherosclerotic disease in different territories tends to come from snapshots based on several prevalence studies, which often reflect a medical milieu very different from the one today in which many preventive interventions, including lipid-lowering therapy, hypertension control, and antiplatelet therapies, are more widely used. Epidemiologic, pathophysiological, and therapeutic studies of noncoronary atherosclerotic disease have tended to focus on individual territories rather than on the vascular tree in its totality. Although it is still important to understand critical differences among the different territories, it is also important that basic, epidemiologic, and clinical investigators recognize the links among vascular territories (namely, that atherosclerosis is a systemic disease) and that collection of current prevalence and incidence data for both clinical and subclinical vascular disease allows for robust comparisons and improved understanding of both the similarities and differences in atherosclerosis in different vascular territories.\n\nThe barriers that …
