Abstract:Over the course of the twentieth century, human life expectancy at birth rose in high-income nations by approximately 30 years, largely driven by advances in public health and medicine. Mortality reduction was observed initially at an early age and continued into middle and older ages. However, it was unclear whether this phenomenon and the resulting accelerated rise in life expectancy would continue into the twenty-first century. Here using demographic survivorship metrics from national vital statistics in the eight countries with the longest-lived populations (Australia, France, Italy, Japan, South Korea, Spain, Sweden and Switzerland) and in Hong Kong and the United States from 1990 to 2019, we explored recent trends in death rates and life expectancy. We found that, since 1990, improvements overall in life expectancy have decelerated. Our analysis also revealed that resistance to improvements in life expectancy increased while lifespan inequality declined and mortality compression occurred. Our analysis suggests that survival to age 100 years is unlikely to exceed 15% for females and 5% for males, altogether suggesting that, unless the processes of biological aging can be markedly slowed, radical human life extension is implausible in this century.

Abstract:OBJECTIVES: To determine the contribution of progress in averting premature deaths to the increase in life expectancy and the decline in lifespan variation.DESIGN: International comparison of national life table data from the Human Mortality Database.SETTING: 40 developed countries and regions, 1840-2009.POPULATION: Men and women of all ages.MAIN OUTCOME MEASURE: We use two summary measures of mortality: life expectancy and life disparity. Life disparity is a measure of how much lifespans differ among individuals. We define a death as premature if postponing it to a later age would decrease life disparity.RESULTS: In 89 of the 170 years from 1840 to 2009, the country with the highest male life expectancy also had the lowest male life disparity. This was true in 86 years for female life expectancy and disparity. In all years, the top several life expectancy leaders were also the top life disparity leaders. Although only 38% of deaths were premature, fully 84% of the increase in life expectancy resulted from averting premature deaths. The reduction in life disparity resulted from reductions in early-life disparity, that is, disparity caused by premature deaths; late-life disparity levels remained roughly constant.CONCLUSIONS: The countries that have been the most successful in averting premature deaths have consistently been the life expectancy leaders. Greater longevity and greater equality of individuals' lifespans are not incompatible goals. Countries can achieve both by reducing premature deaths.

Abstract:The past century was a period of increasing life expectancy throughout the age range. This resulted in more people living to old age and to spending more years at the older ages. It is likely that increases in life expectancy at older ages will continue, but life expectancy at birth is unlikely to reach levels above 95 unless there is a fundamental change in our ability to delay the aging process. We have yet to experience much compression of morbidity as the age of onset of most health problems has not increased markedly. In recent decades, there have been some reductions in the prevalence of physical disability and dementia. At the same time, the prevalence of disease has increased markedly, in large part due to treatment which extends life for those with disease. Compressing morbidity or increasing the relative healthspan will require "delaying aging" or delaying the physiological change that results in disease and disability. While moving to life expectancies above age 95 and compressing morbidity substantially may require significant scientific breakthroughs; significant improvement in health and increases in life expectancy in the United States could be achieved with behavioral, life style, and policy changes that reduce socioeconomic disparities and allow us to reach the levels of health and life expectancy achieved in peer societies.

Abstract:Does the human lifespan have an impenetrable biological upper limit which ultimately will stop further increase in life lengths? This question is important for understanding aging, and for society, and has led to intense controversies. Demographic data for humans has been interpreted as showing existence of a limit, or even as an indication of a decreasing limit, but also as evidence that a limit does not exist. This paper studies what can be inferred from data about human mortality at extreme age. We show that in western countries and Japan and after age 110 the probability of dying is about 47% per year. Hence there is no finite upper limit to the human lifespan. Still, given the present stage of biotechnology, it is unlikely that during the next 25 years anyone will live longer than 128 years in these countries. Data, remarkably, shows no difference in mortality after age 110 between sexes, between ages, or between different lifestyles or genetic backgrounds. These results, and the analysis methods developed in this paper, can help testing biological theories of ageing and aid confirmation of success of efforts to find a cure for ageing.

Abstract:Background: Before the COVID-19 pandemic, stagnating life expectancy trends were reported in some high-income countries (HICs). Despite previous evidence from country-specific studies, there is a lack of comparative research that provides a broader perspective and challenges existing assumptions. This study aims to examine longevity trends and patterns in six English-speaking countries (Australia, Canada, Ireland, New Zealand, UK, USA) by combining period and cohort perspectives and to compare them with other HICs. Methods: Using data from the Human Mortality and WHO Mortality Databases, we estimated partial life expectancy, lifespan inequality and cohort survival differences for 1970-2021, as well as the contribution of causes of death to the gap in life expectancy between English-speaking countries and the average for other HICs in 2017-19. Results: In the pre-pandemic period, the increase in life expectancy slowed in all English-speaking countries, except Ireland, mainly due to stagnating or rising mortality at young-middle ages. Relative to other HICs, those born in Anglophone countries since the 1970s experienced relative survival disadvantage, largely attributable to injuries (mainly suicides) and substance-related mortality (mainly poisonings). In contrast, older cohorts enjoyed advantages for females in Australia and Canada and for males in all English-speaking countries except the USA. Conclusions: Although future gains in life expectancy in wealthy societies will increasingly depend on reducing mortality at older ages, adverse health trends at younger ages are a cause for concern. This emerging and avoidable threat to health equity in English-speaking countries should be the focus of further research and policy action.

Abstract:Humans age at different rates and families with exceptional survival provide the opportunity to understand why some people age slower than others. Unique features exhibited by centenarians include a family history of longevity, compression of morbidity with resultant extension of health span, and biomarkers such as low-circulating insulin-like growth factor 1 (IGF-1) and elevated high-density lipoprotein (HDL) cholesterol levels. Given the rarity of the centenarian phenotype, it has not been surprising that the use of discovery methods that relied on common population single nucleotide polymorphisms (SNPs) to unlock the genetic determinants of exceptional longevity have not yielded significant results. Conversely, gene sequencing has resulted in discoveries of functional gene variants that support several of the centenarian phenotypes. These discoveries have led to the strategic developments of drugs that may delay aging and prolong health span.

Abstract:Objective: To compare life expectancy levels and within-country geographic variation in life expectancy across six high-income Anglophone countries between 1990 and 2018. Design: Demographic analysis using aggregated mortality data. Setting: Six high-income Anglophone countries (USA, UK, Canada, Australia, Ireland and New Zealand), by sex, including an analysis of subnational geographic inequality in mortality within each country. Population: Data come from the Human Mortality Database, the WHO Mortality Database and the vital statistics agencies of six high-income Anglophone countries. Main outcome measures: Life expectancy at birth and age 65; age and cause of death contributions to life expectancy differences between countries; index of dissimilarity for within-country geographic variation in mortality. Results: Among six high-income Anglophone countries, Australia is the clear best performer in life expectancy at birth, leading its peer countries by 1.26-3.95 years for women and by 0.97-4.88 years for men in 2018. While Australians experience lower mortality across the age range, most of their life expectancy advantage accrues between ages 45 and 84. Australia performs particularly well in terms of mortality from external causes (including drug- and alcohol-related deaths), screenable/treatable cancers, cardiovascular disease and influenza/pneumonia and other respiratory diseases compared with other countries. Considering life expectancy differences across geographic regions within each country, Australia tends to experience the lowest levels of inequality, while Ireland, New Zealand and the USA tend to experience the highest levels. Conclusions: Australia has achieved the highest life expectancy among Anglophone countries and tends to rank well in international comparisons of life expectancy overall. It serves as a potential model for lower-performing countries to follow to reduce premature mortality and inequalities in life expectancy.

Abstract:Using segmented linear regression to reanalyze the "best-guess" maximum reported age at death data supplied in Aubrey de Grey's editorial, we find compelling evidence for a breakpoint in the mid-1990s, with a positive slope before the breakpoint and a flat or slightly negative slope after it. This confirmation of our earlier results was also modeled using exponential regression. Both the segmented and exponential models were superior to a simple linear regression, providing a better fit for the data even after taking into account their greater number of parameters. These findings are highly robust to the removal of several points from the data and bolster the existing evidence for a limit to human lifespan. Taken in light of both our original analysis and its confirmation by several independent groups, this latest result provides yet more evidence that human lifespan has reached its limit under the current technological paradigm. However, we cannot discount the possibility that novel innovations could propel human lifespan beyond the limit we have identified, if they can overcome the considerable challenges facing them.

Abstract:The analysis of the demographic transition of the past century and a half, using both empirical data and mathematical models, has rendered a wealth of well-established facts, including the dramatic increases in life expectancy. Despite these insights, such analyses have also occasionally triggered debates which spill over many disciplines, from genetics, to biology, or demography. Perhaps the hottest discussion is happening around the question of maximum human lifespan, which --besides its fascinating historical and philosophical interest-- poses urgent pragmatic warnings on a number of issues in public and private decision-making. In this paper, we add to the controversy some results which, based on purely statistical grounds, suggest that the maximum human lifespan is not fixed, or has not reached yet a plateau. Quite the contrary, analysis on reliable data for over 150 years in more than 20 industrialized countries point at a sustained increase in the maximum age at death. Furthermore, were this trend to continue, a limitless lifespan could be achieved by 2102. Finally, we quantify the dependence of increases in the maximum lifespan on socio-economic factors. Our analysis indicates that in some countries the observed rising patterns can only be sustained by progressively larger increases in GDP, setting the problem of longevity in a context of diminishing returns.

Abstract:No single cause can be found for the slowdown in life expectancy improvement seen in the UK, a report from Public Health England (PHE) has concluded.1It does not rule out the belt tightening in health and social care that began in 2011 and has coincided with higher mortality and a life expectancy slowdown, but neither does it give this the prominence accorded by some commentators.The report concludes that slowing improvements in heart disease and stroke, high winter mortality from flu in several recent years, the growth of dementia as a recorded cause of death, and more deaths from drug misuse among younger people have all played a part.Similar trends in mortality and life expectancy have been seen throughout Europe and the US over the same period, so a UK specific explanation is unlikely to be correct. But the trends visible everywhere are more pronounced in the UK, which showed the lowest rate of improvement among all large EU countries …

In fact, we acknowledge this in our paper, and conclude that “further studies need to include chronic diseases”. Li and Harmer also point out that we did not assess the magnitude of rural–urban differences, and Tomohiro Morita and colleagues describe our rural–urban dichotomy as “too simplistic”. We agree that an investigation of rural–urban differences is important, which is why we adjusted for rural–urban residence as a covariate in our study. We would like to point out that it might not be appropriate to divide the data into rural and urban subsamples, because it would cut the sample size by about half, and make the subsample data not fully compatible between 1998 and 2008 because of rural–urban migration. Morita and colleagues correctly point out that the Global Burden of Disease Study showed China can be divided into five areas, and suggest we consider this. However, besides the fact that the division of data would largely reduce the statistical power because of a much smaller sample size, there are wide disparities within each of the five areas, so we believe such analysis would be unhelpful. Adjustment for rural and urban residence plus education, although imperfect, captures most socioeconomic disparities. We disagree with Li and Harmer's comment that our ascribing of greater longevity or lower mortality among the oldest-old to benefits of success is “problematic because no valid measure of health status exists for this population”. The standard activities of daily living and objective tests of physical performance and cognitive function are valid measures of health status strongly associated with survival. We believe Li and Harmer's comment that “the costs of success hypothesis might not be tenable because of the lack of key measures, such as non-communicable disease” is invalid, because: the costs of success is an interpretation of the decline in average scores of physical performance and cognitive function as the survival of more frail, elderly individuals with health problems, rather than a hypothesis to be tested; and, a lack of non-communicable disease data does not prevent analysis of changes in the health status of physical and cognitive abilities. We agree with Morita and colleagues that “the information about family members who support an oldest-old individual is of concern”; however, we did adjust for marital status (ie, living with or without spouse), and thus the effects of family member support are controlled for, to some extent. The global pattern of centenarians highlights deep problems in demography

Abstract:Accurate age data is fundamental to medicine, social sciences, epidemiology, and good government. However, recent and heavily disputed debates on data quality have raised questions on the accuracy of demographic data at older ages. Here, we catalogue late-life survival patterns of every country in the world from 1970-2021 using comprehensive estimates of old-age populations provided by global governments and curated by the United Nations. Analysis of 236 nations or states across 51 years reveals that late-life survival data is dominated by anomalies at all scales and in all time periods. Life expectancy at age 100 and late-life survival from ages 80 to 100+, which we term centenarian attainment rate, is highest in a seemingly random assortment of states. The top 10 "blue zone" regions with the best survival to ages 100+ routinely includes Thailand, Kenya and Malawi - respectively now 212th and 202nd in the world for life expectancy, the non-self-governing territory of Western Sahara, and Puerto Rico where birth certificates are so unreliable they were recently declared invalid as a legal document. These anomalous rankings are conserved across long time periods and multiple non-overlapping cohorts, and do not seem to be sampling effects. Instead these patterns suggest a persistent inability, even for nation-states or global organisations, to detect or measure error rates in human age data, with troubling implications for epidemiology, demography, and medicine.

Abstract:We respond to claims by Dong et al . that human lifespan is limited below 125 years. Using the log-linear increase in mortality rates with age to predict the upper limits of human survival we find, in contrast to Dong et al ., that the limit to human lifespan is historically flexible and increasing. This discrepancy can be explained by Dong et al .’s use of data with variable sample sizes, age-biased rounding errors, and log(0) instead of log(1) values in linear regressions. Addressing these issues eliminates the proposed 125-year upper limit to human lifespan.

Abstract:Background: Similar to the study of the distribution of income within countries, population-level health disparities can be examined by analyzing the distribution of age at death. Methods: We sourced period-specific death counts for 18 OECD countries over 1900-2020 from the Human Mortality Database. We studied the evolution of country-year-specific distributions of age at death, with an examination of the lower and upper tails of these distributions. For each country-year, we extracted the 1st, 5th, 10th, 90th, 95th and 99th percentiles of the age-at-death distribution. We then computed the corresponding shares of longevity-the sum of the ages weighted by the age-at-death distribution as a fraction of the sum of the ages weighted by the distribution-for each percentile. For example, for the 10th percentile, this would correspond to how much longevity accrues to the bottom 10% of the age-at-death distribution in a given country-year. Results: We expose a characterization of the age-at-death distribution across populations with a focus on the lower and upper tails of the distribution. Our metrics, specifically the gap measures in age and share across the 10th and 90th percentiles of the distribution, enable a systematic comparison of national performances, which yields information supplementary to the cross-country differences commonly pointed by traditional indicators of life expectancy and coefficient of variation. Conclusions: Examining the tails of age-at-death distributions can help characterize the comparative situations of the better- and worse-off individuals across nations, similarly to depictions of income distributions in economics.

Abstract:There is sustained and widespread interest in understanding the limit, if there is any, to the human life span. Apart from its intrinsic and biological interest, changes in survival in old age have implications for the sustainability of social security systems. A central question is whether the endpoint of the underlying lifetime distribution is finite. Recent analyses of data on the oldest human lifetimes have led to competing claims about survival and to some controversy, due in part to incorrect statistical analysis. This article discusses the particularities of such data, outlines correct ways of handling them, and presents suitable models and methods for their analysis. We provide a critical assessment of some earlier work and illustrate the ideas through reanalysis of semisupercentenarian lifetime data. Our analysis suggests that remaining life length after age 109 is exponentially distributed and that any upper limit lies well beyond the highest lifetime yet reliably recorded. Lower limits to 95% confidence intervals for the human life span are about 130 years, and point estimates typically indicate no upper limit at all.

Abstract:Although average human life expectancy is rising, the maximum lifespan is not increasing. Leading demographers claim that human lifespan is fixed at a natural limit around 122 years. However, there is no fixed limit in animals. In animals, anti-aging interventions (dietary restrictions, rapamycin, genetic manipulations) postpone age-related diseases and thus automatically extend maximum lifespan. In humans, anti-aging interventions have not been yet implemented. Instead, by treating individual diseases, medical interventions allow a patient to live longer (despite morbidity), expanding morbidity span. In contrast, slowly aging individuals (centenarians) enter very old age in good health, but, when diseases finally develop, they do not receive thorough medical care and die fast. Although the oldest old die from age-related diseases, death certificates often list "old age", meaning that diseases were not even diagnosed and even less treated. The concept of absolute compression of morbidity is misleading in humans (in truth, there is no other way to compress morbidity as by denying thorough medical care) and false in animals (in truth, anti-aging interventions do not condense morbidity, they postpone it). Anti-aging interventions such as rapamycin may potentially extend both healthspan and maximal lifespan in humans. Combining anti-aging medicine with cutting-edge medical care, regardless of chronological age, will extend maximal lifespan further.
The relationship between increasing life expectancy and healthy life expectancy

Abstract:The continued increases in life expectancy with no obvious deceleration, the proliferation of centenarians and appearance of supercentarians (those aged 110 years and over) leave us in no doubt that the belief that life expectancy was limited to 85 years is now untenable. Although we may ask how long the limits to life can be pushed, the crucial question is whether the extra years gained year on year in life expectancy are healthy years. This paper begins by reviewing what was historically believed to be the theoretical relationships between life expectancy and healthy life expectancy. We debate how current knowledge of mortality rates in the old and oldest old, the trends in healthy life expectancy, and the gap between the genders shed light upon these theoretical models, discussing the fact that different models may exist in different cohorts of the same population. The paper closes with some speculations on how we might monitor the evolution of healthy life expectancy more closely, particularly in those countries still early on in the ageing transition. Background The debate about the relationship between life expectancy and healthy life expectancy and the move from simply considering the length of life to the quality of remaining life began over two decades ago, though the debate at that time was predominantly theoretical with little data to prove or disprove theories. As mortality rates at older ages were observed to be falling, three main theories emerged. Gruenberg (1977) predicted a pandemic of chronic diseases, or expansion of morbidity, through the progress in medical care extending the life of those with disease and disability as well as life expectancy increasing to the ages at which disease and disability are more likely. Fries (1980) on the other hand gave a more optimistic outlook, that through behaviour change and prevention, we could postpone the onset of disease and disability closer to the end of life: the compression of morbidity scenario. Central to this argument was the existence of the limit to life expectancy, then thought to be around 85 years. Manton (1982) proposed an intermediate view between these extremes of a dynamic equilibrium, where the prevalence of disability may increase as mortality falls, but the severity of disability is reduced. The dynamic equilibrium scenario points much more clearly to the crucial question of whether the extra years of life are good quality years or not and for the need to introduce a notion of severity. Disability is, at the same time, an indicator of the severity of morbid states and an indicator of the quality of years lived. The introduction of disability into models of health significantly clarified the relationships between disease and mortality leading to the partitioning of years of life into those with and without disability and to the development of health expectancy as a population health indicator to elucidate the three scenarios. This can be seen from Figure 1 (WHO, 2004) which distinguishes three elements: total survival, disability-free survival and survival without disabling chronic disease, leading to life expectancy (LE: area under the ‘mortality’ curve), disability-free life expectancy (DFLE: area under the ‘disability’ curve) and life expectancy without chronic disease (DisFLE: area under the ‘morbidity’ curve). The difference between LE and DFLE measures life expectancy with a disability, the difference between LE and DisFLE measures life expectancy with at least one chronic disease and the difference between DisFLE and DFLE measures life expectancy with at least one chronic disease but without disability.
