Abstract:A 59-year-old man was admitted to our hospital for further examination of an abnormal shadow which appeared on his chest X-ray. A postero-anterior view of his chest X-ray demonstrated a large, sharply defined mass. The mass measured about 10 cm in diameter with a smooth outline (Figure 1A). The patient was asymptomatic, without abnormal physical or laboratory findings. The contrast enhanced CT examination showed that a mass in the inferior posterior mediastinum was located between the heart and thoracic vertebra. No herniation of the stomach or intestines into the thorax was found (Figure 1C). Fine branching linear strands were evident in the mass which was considered to be the branches of the gastroepiploic artery. During the enhanced phase, we could also see gastroepiploic artery passing though the esophageal hiatus (Figure 1D). The density of the mass was about -115 to -120 Hounsfield units, which was in the range of fatty tissue. Our primary diagnosis of the mass was intrathoracic omental herniation through the esophageal hiatus.Figure 1. Gastroepiploic artery can be seen during the enhanced phase (arrows).We performed an operation through a seventh left posterolateral thoracotomy. A 10 cm × 10 cm encapsulated fatty mass was noted in the posterior mediastinum arising from esophageal hiatus (Figure 2A). When the sac was opened, the omental contents emerged (Figure 2B). We returned the omentum back into the abdominal cavity. We resected the redundant part of the sac and repaired the hiatus (Figure 2C). The patient had an uneventful postoperative recovery without any complications (Figure 1B).Figure 2. A:: Intraoperative findings through thoracotomy. C: The hernia is repaired.Intrathoracic omental herniation through the esophageal hiatus is rare.1-10 Our patient is the eleventh case in both English and Japanese literatures. Omental herniation without abdominal viscera has been misdiagnosed as lipomatous tumor or resected without conclusive diagnosis.1,2,4,6,7,9,10 Kubota et al8 summarized the CT, MRI, and angiographic findings of seven previous reports to determine an adequate method of diagnosis, suggesting that angiography provided decisive evidence of a mass containing omental vessels passing through the esophageal hiatus, which led to the final diagnosis. We diagnosed paraesophageal omental herniation based on the findings of the contrast enhanced CT scan. Omental vessels passing through the esophageal hiatus and the branching linear strands can be seen clearly during the enhanced phase. The contrast enhanced CT scan, which is a non-invasive angiographic procedure, might act as an alternative to angiography. Moreover, angiography is not essential because the omental vessels are absent on angiography in one case reported by Tamura et al.2 The treatment for omental herniation through the esophageal hiatus is controversial. Only three patients underwent observation after a diagnosis of omental herniation from imaging findings.3,5,8 Kubota et al 8 recommends long-term observation to avoid invasive treatment. However, its safety has not been confirmed. Although our patient was asymptomatic, we performed surgery finally due to the following reasons: (1) An omental hernia may induce paraesophageal hernia with an intrathoracic stomach, which is potentially life-threatening due to obstruction, bleeding and perforation. (2) This 10 cm × 10 cm membrane sac may rupture and cause bleeding when the bulge gradually enlarges. (3) Symptoms may gradually increase with time as the hernia gradually enlarges, stretching surrounding tissue. (4) The patient had experienced increasing anxiety since he was admitted to our hospital. In summary, rare paraesophageal omental herniation should be differentiated from lipomatous tumors, including well-differentiated liposarcoma. Contrast enhanced CT may actively assist in diagnosing paraesophageal omental herniation. The treatment for omental herniation through the esophageal hiatus is controversial which needs more clinical investigations.
