Abstract:Objective To analyze the research hotspot of critical care nursing abroad in recent 5 years,in order to provide reference for critical care nursing research in China. Methods PubMed database was searched for articles related to critical care nursing from 2017 to 2022. Bicomb software was used for word frequency analysis,and gCLUTO software was used for cluster analysis. Results A total of 1 263 articles related to critical care nursing were included,the number of publications reached the highest level from 2018 to 2020,and showed a downward trend in the past two years. Forty-four high-frequency keywords were extracted,and the cumulative percentage of occurrence frequency was 55.50%. Four research hotspots of critical care were summarized by cluster analysis,which were humanistic care of critical care nursing,working environment of critical care,clinical nursing practice of critical care and nursing of special critical diseases. Conclusion In recent years,the research objects and contents extended and deepened. The research objects included ICU nurses,patients and their families. The research contents involved humanistic care,clinical practice and working environment,focused on clinical difficulties such as surrogate decision-making,auxiliary communication and hospice care of ICU patients. In the future,we still need to summarize the hotspots of critical care nursing research in order to provide ideas for further exploring the depth and breadth of critical care nursing research.

Abstract:Physiological parameters monitoring is essential to direct medical staff to evaluate, diagnose and treat critical patients quantitatively. ECG, blood pressure, SpO2, respiratory rate and body temperature are the basic vital signs of patients in the ICU. The measuring methods are relatively mature at present, and the trend is to be wireless and more accurate and comfortable. Hemodynamics, oxygen metabolism and microcirculation should be taken seriously during the treatment of acute critical patients. The related monitoring technology has made significant progress in recent years, the trend is to reduce the trauma and improve the accuracy and usability. With the development of machine vision and data fusion technology, the identification of patient behavior and deterioration has become hot topics. This review is focused on current parameters monitoring technologies, aims to provide reference for future related research.
The Top 2,000 Cited Articles in Critical Care Medicine: a Bibliometric Analysis.

Abstract:BACKGROUND:The bibliometric analysis has been performed on several topics in critical care medicine (CCM) focusing on top 100 cited articles, but the analysis on CCM literature as a whole is missing. The present study aimed to perform a complete bibliometric analysis in the field of CCM.METHODS:An electronic search of the Scopus database was performed on Feb 13, 2018. The search strategy involved core terms related to CCM. The top 2,000 most cited articles in the field of CCM were included in the analysis. Descriptive statistics on these top-cited articles, country distributions, and journals are reported. Individual author's productivity was assessed with the Lotka's law. Co-occurrence of keywords was visualized with the Fruchterman-Reingold layout. The Walktrap algorithm was employed for clustering analysis.RESULTS:A total of 2,000 documents were included in the analysis with median citations of 386 times [interquartile range (IQR): 308-562 times]. The most cited article was the original paper that described the Acute Physiology and Chronic Health Evaluation (APACHE) II score. The included articles were published in 411 journals. The median number of documents published in one journal was 1, and the mean number was 4.9, indicating a skewed distribution. The maximum number of publications was 217 in CCM. Author's productivity profile was significantly different from the Lotka's law (P=0.001), with n and C values of 2.8 and 0.52, respectively. Fruchterman-Reingold network plot showed that studies involving human subject were the most common literature type. Sepsis was a major research topic that co-occurred with keywords such as disease severity, nonhuman, risk assessment and practice guideline.CONCLUSIONS:The study performed bibliometric analyses of 2,000 top-cited articles in CCM. The most cited article was the one which developed the APACHE II score. Author's productivity was significantly different from the Lotka's law.

Abstract:Together with the development of the focused assessment with sonography for trauma protocol for ultrasound in trauma patients, point-of-care ultrasound is widely used in the field of emergency and critical care medicine. Researchers have established several classic operating protocols for different diseases, such as acute respiratory failure, shock, and cardiac arrest, in the emergency department and intensive care unit. With further development in the portability and popularity of ultrasonic equipment, it is expected to be used as a new type of technology for physical examination, including inspection, palpation, percussion, auscultation, and insonation, which will improve the clinical medicine practice in the future.

Abstract:Lung ultrasound has become a part of the daily examination of physicians working in intensive, sub-intensive, and general medical wards. The easy access to hand-held ultrasound machines in wards where they were not available in the past facilitated the widespread use of ultrasound, both for clinical examination and as a guide to procedures; among point-of-care ultrasound techniques, the lung ultrasound saw the greatest spread in the last decade. The COVID-19 pandemic has given a boost to the use of ultrasound since it allows to obtain a wide range of clinical information with a bedside, not harmful, repeatable examination that is reliable. This led to the remarkable growth of publications on lung ultrasounds. The first part of this narrative review aims to discuss basic aspects of lung ultrasounds, from the machine setting, probe choice, and standard examination to signs and semiotics for qualitative and quantitative lung ultrasound interpretation. The second part focuses on how to use lung ultrasound to answer specific clinical questions in critical care units and in emergency departments.

Abstract:Critical care ultrasonography (CCUS) is essential in the evaluation of shock, respiratory failure, and to guide central venous access. [1] Nurses also use ultrasound for nursing practice such as thrombosis screening, ultrasound-guided vascular puncture, feeding tube placement, and lung physical therapy. [2] CCUS strengths are lack of radiation, portability, and low costs. Therefore, CCUS has considerable clinical use in critical care. However, the acquisition of ultrasound images is the basis of accurate evaluation, and the accuracy of ultrasound is related to operator skill level. Additionally, the application of CCUS in nursing care depends on nurses' knowledge, and training. Although many clinical studies have been conducted on CCUS, there is little evidence on ultrasound training programs for intensive care unit (ICU) nurses. [3] In this study, we designed and implemented a novel CCUS training program for ICU nurses. Our objective was to evaluate the effect of a short didactic training session on trainees' CCUS knowledge and skills.

Abstract:PurposeCritical care ultrasonography has utility for the diagnosis and management of critical illness and is in widespread use by frontline intensivists. As there is a need for research to validate and extend its utility, the Editor of Intensive Care Medicine included critical care ultrasonography as a topic in the ICM Research Agenda issue.MethodsEleven international experts in the field of critical care ultrasonography contributed to the writing project. With the intention of developing a research agenda for the field, they reviewed best standards of care, new advances in the field, common beliefs that have been contradicted by recent trials, and unanswered questions related to critical care ultrasonography.ResultsThe writing group focused on the provision of training in critical care ultrasonography, technological advances, and some specific clinical applications.ConclusionsThe writing group identified several fields of interest for research and proposed ten research studies that would address important aspects of critical care ultrasonography.

Abstract:The evolution of critical care medicine is inextricably linked to the development of critical care procedures. These procedures not only facilitate diagnosis and treatment of critically ill patients, but also provide valuable insights into disease pathophysiology. While critical care interventions offer undeniable benefits, the potential for iatrogenic complications necessitates careful consideration. The recent surge in critical care ultrasound (US) utilization is a testament to its unique advantages: non-invasiveness, real-time bedside availability, direct visualization of internal structures, elimination of ionizing radiation exposure, repeatability, and relative ease of learning. Recognizing the need to optimize procedures and minimize complications, critical care utrasound study group of Beijing critical care ultrasound research assocition convened a panel of critical care experts to generate this consensus statement. This document serves as a guide for healthcare providers, aiming to ensure patient safety and best practices in critical care.

Abstract:In critically ill patients with acute respiratory failure, thoracic images are essential for evaluating the nature, extent and progression of the disease, and for clinical management decisions. For this purpose, computed tomography (CT) is the gold standard. However, transporting patients to the radiology suite and exposure to ionized radiation limit its use. Furthermore, a CT scan is a static diagnostic exam for the thorax, not allowing, for example, appreciation of "lung sliding". Its use is also unsuitable when it is necessary to adapt or decide to modify mechanical ventilation parameters at the bedside in real-time. Therefore, chest X-ray and lung ultrasound are today's contenders for shared second place on the podium to acquire a thoracic image, with their specific strengths and limitations. Finally, electrical impedance tomography (EIT) could soon have a role, however, its assessment is outside the scope of this review. Thus, we aim to carry out the following points: (1) analyze the advancement in knowledge of lung ultrasound use and the related main protocols adopted in intensive care units (ICUs) over the latest 30 years, reporting the principal publications along the way, (2) discuss how and when lung ultrasound should be used in a modern ICU and (3) illustrate the possible future development of LUS.

Abstract:The conventional ultrasound and color doppler ultrasound have been a focus in the field of emergency and critical care medicine in recent years,but cannot evaluate the microcirculation perfusion quantitatively.Thus,their diagnostic accuracy in predicting clinical outcomes for critically ill patients is suboptimal.Contrast-enhanced ultrasound (CEUS) is based on conventional ultrasound in which ultrasound contrast agent is used to enhance echo and signal intensity,and helps to characterize tissue perfusion more clearly and dynamically.It has been widely used in the field of emergency and critical care medicine,such as the location and dynamic evaluation of traumatic lesions,renal microcirculation monitoring for the acute kidney injury (AKI) and accuracy evaluation of central venous catheterization.CEUS provides a simple,non-invasive and effective way for clinical diagnosis and treatment of critically ill patients.

Abstract:Objective: Diagnostic tools in emergency medicine have been widely studied. As a non-invasive and quick tool, ultrasound plays a role in the field of emergency medicine. Thus, it is significant to understand the global scientific output of this topic. An analysis of publications on the use of ultrasound in emergency medicine over the past decade was performed and summarized to track the current hotspots and highlight future directions. Methods: Globally relevant publications on ultrasound in emergency medicine from 2009 to 2020 were extracted from the Web of Science collection database. VOSviewer software and CiteSpace were employed to visualize and predict the trends in the research on the topic. Results: The overall volume of global publications is on the rise ; furthermore, the United States published the most publications in this field and had the most citations and H-index. University of California at San Francisco in the United States has most publications in terms of institutions. The American Journal of Emergency Medicine published the most papers related to ultrasound in emergency medicine in terms of journals. Pulmonary embolism was once the main research direction, and importantly, “point-of-care ultrasound” was determined to be a new research hotspot. Conclusion: Altogether, the number of publications on ultrasound in emergency medicine will rise in the future. In addition, the findings reported here shed new light on the major progress on ultrasound in emergency medicine, which may be mutually cooperative in various fields. Moreover, this bibliometric study provides further indications for the topic of “point-of-care ultrasound”.

Use of Point-of-Care Ultrasound by Intensive Care Unit Triage Teams in Evaluating Unstable Patients Outside Intensive Care Units

Abstract:Introduction Point-of-care ultrasound (POCUS) has become an integral asset in intensive care units (ICUs). However, there is limited literature on the value of POCUS in evaluating deteriorating patients outside the ICU. In this study, we sought to investigate the use and impact of POCUS by ICU triage teams in hospitals outside of the ICU setting. Methods ICU triage fellows were provided a portable ultrasound to use as part of their evaluations during consultations and hospital code activations. Fellows were asked to fill out a survey on how ultrasound was used and its impact on patient management. Free-text data such as reason for ultrasound use, views obtained, clinical impressions before and after ultrasound, and clinical actions were recorded. These data were transcribed and categorized electronically. Results A total of 51 total resuscitations were documented. The most common reason for ICU triage team evaluation was hypotension (53%, N=27). The most common clinical focus for ultrasound use was cardiac assessment (53%, N=27), followed by volume status assessment (35%, N=18). The most common ultrasound views per encounter obtained were parasternal long (82%, N=42), followed by apical four-chamber view (76%, N=39) and subcostal view (75%, N=38). Out of 38 encounters with clinical impressions documented, 79% (N=30) of pre-ultrasound clinical impressions were confirmed by ultrasound use. Of total encounters, 35% (N=18) had a significant clinical action taken based on ultrasound findings (fluid resuscitation, vasopressor initiation, etc.). Conclusions Ultrasound is a valuable tool for patient evaluation in non-ICU wards, especially in confirming clinical impressions and guiding therapeutic actions. Some limitations of this study include reporting bias and incomplete capture of ultrasound use in non-ICU wards.

Abstract:Objective To explore the clinical application value of bedeside-ultrasound in acute and critical illness.Methods A retrospective analysis was made in 496 cases which received the bedside-ultrasound examination,including 172 patients with cardiovascular acute and critical illness and 324 patients with abdominal acute and critical illness.Results In 470 of 496 cases,the ultrasonic diagnosis were consistent with clinical diagnosis,including 76 cases of acute myocardial infarction,7 cases of aortic dissection,5 cases of pulmonary embolism,82 cases of pericardial effusion,162 cases of visceral injury,30 cases of acute pancreatitis,54 cases of ectopic pregnancy,38 cases of ruptured corpus luteum,and 16 cases of placenta previa.The compliance rate was 94.8%.26 cases were missed,accounting for 5.2% among all the patients.Conclusion Bedside-ultrasound can provide valuable information quickly and easily for the diagnosis of acute and critical patients,and it has important clinical application value.

Abstract:Imaging has made significant contributions to the understanding of lung diseases in critically ill patients in recent times. Thoracic ultrasonography is a real-time imaging technique that is easy to use, non-invasive, potentially widespread, and radiation-free. Its rising use in critical care and accompanying study findings, support its status as a new approach for bedside chest imaging. Imaging is now used to diagnose lung pathology, track its progression, and direct therapeutic strategy. It can be used repeatedly over time as a part of a dynamic clinical evaluation and is a useful tool for determining the etiology of a patient's shock or respiratory failure. The purpose of this research is to review the available information about utilization and clinical significance of thoracic ultrasonography. When it comes to diagnosing pneumothorax, pneumonia, pleural effusion, and pulmonary edema, thoracic ultrasonography is more reliable than a chest radiograph. The placement and confirmation of subclavian central lines is one of the crucial functions of thoracic ultrasonography. Following the insertion of a chest tube, lung ultrasonography may be beneficial for monitoring and assessing the pneumothorax's resolution. To determine the size and location of a mass on the chest wall and to assist with the mass lesion's biopsy, a thoracic ultrasonography may be efficient, practical, and affordable. In cases of pulmonary effusion, ultrasonography can be used to estimate the quantity of effusion. Additionally, thoracic ultrasonography was efficiently used in coronavirus disease- 2019 infection patients since it posed less of a threat to break airborne isolation and spread infection to others also showed significant findings.
Research on ultrasound-based radiomics: a bibliometric analysis

Abstract:Background: A large number of studies related to ultrasound-based radiomics have been published in recent years; however, a systematic bibliometric analysis of this topic has not yet been conducted. In this study, we attempted to identify the hotspots and frontiers in ultrasound-based radiomics through bibliometrics and to systematically characterize the overall framework and characteristics of studies through mapping and visualization. Methods: A literature search was carried out in Web of Science Core Collection (WoSCC) database from January 2016 to December 2023 according to a predetermined search formula. Bibliometric analysis and visualization of the results were performed using CiteSpace, VOSviewer, R, and other platforms. Results: Ultimately, 466 eligible papers were included in the study. Publication trend analysis showed that the annual publication trend of journals in ultrasound-based radiomics could be divided into three phases: there were no more than five documents published in this field in any year before 2018, a small yearly increase in the number of annual publications occurred between 2018 and 2022, and a high, stable number of publications appeared after 2022. In the analysis of publication sources, China was found to be the main contributor, with a much higher number of publications than other countries, and was followed by the United States and Italy. Frontiers in Oncology was the journal with the highest number of papers in this field, publishing 60 articles. Among the academic institutions, Fudan University, Sun Yat-sen University, and the Chinese Academy of Sciences ranked as the top three in terms of the number of documents. In the analysis of authors and cocited authors, the author with the most publications was Yuanyuan Wang, who has published 19 articles in 8 years, while Philippe Lambin was the most cited author, with 233 citations. Visualization of the results from the cocitation analysis of the literature revealed a strong centrality of the subject terms papillary thyroid cancer, biological behavior, potential biomarkers, and comparative assessment, which may be the main focal points of research in this subject. Based on the findings of the keyword analysis and cluster analysis, the keywords can be categorized into two major groups: (I) technological innovations that enable the construction of radiomics models such as machine learning and deep learning and (II) applications of predictive models to support clinical decision-making in certain diseases, such as papillary thyroid cancer, hepatocellular carcinoma (HCC), and breast cancer. Conclusions: Ultrasound-based radiomics has received widespread attention in the medical field and has been gradually been applied in clinical research. Radiomics, a relatively late development in medical technology, has made substantial contributions to the diagnosis, prediction, and prognostic evaluation of diseases. Additionally, the coupling of artificial intelligence techniques with ultrasound imaging has yielded a number of promising tools that facilitate clinical decision-making and enable the practice of precision medicine. Finally, the development of ultrasound-based radiomics requires multidisciplinary cooperation and joint efforts from the field biomedicine, information technology, statistics, and clinical medicine.
Current status and trends in researches based on public intensive care databases: A scientometric investigation

Abstract:Objective: Public intensive care databases cover a wide range of data that are produced in intensive care units (ICUs). Public intensive care databases draw great attention from researchers since they were time-saving and money-saving in obtaining data. This study aimed to explore the current status and trends of publications based on public intensive care databases. Methods: Articles and reviews based on public intensive care databases, published from 2001 to 2021, were retrieved from the Web of Science Core Collection (WoSCC) for investigation. Scientometric software (CiteSpace and VOSviewer) were used to generate network maps and reveal hot spots of studies based on public intensive care databases. Results: A total of 456 studies were collected. Zhang Zhongheng from Zhejiang University (China) and Leo Anthony Celi from Massachusetts Institute of Technology (MIT, USA) occupied important positions in studies based on public intensive care databases. Closer cooperation was observed between institutions in the same country. Six Research Topics were concluded through keyword analysis. Result of citation burst indicated that this field was in the stage of rapid development, with more diseases and clinical problems being investigated. Machine learning is still the hot research method in this field. Conclusions: This is the first time that scientometrics has been used in the investigation of studies based on public intensive databases. Although more and more studies based on public intensive care databases were published, public intensive care databases may not be fully explored. Moreover, it could also help researchers directly perceive the current status and trends in this field. Public intensive care databases could be fully explored with more researchers' knowledge of this field.

Ultrasound use in the ICU for interventional pulmonology procedures

Abstract:Critical care ultrasound has shifted the paradigm of thoracic imaging by enabling the treating physician to acquire and interpret images essential for clinical decision-making, at the bedside, in real-time. Once considered impossible, lung ultrasound based on interpretation of artifacts along with true images, has gained momentum during the last decade, as an integral part of rapid evaluation algorithms for acute respiratory failure, shock and cardiac arrest. Procedural ultrasound image guidance is a standard of care for both common bedside procedures, and advanced procedures within interventional pulmonologist's (IP's) scope of practice. From IP's perspective, the lung, pleural, and chest wall ultrasound expertise is a prerequisite for mastery in pleural drainage techniques and transthoracic biopsies. Another ultrasound application of interest to the IP in the intensive care unit (ICU) setting is during percutaneous dilatational tracheostomy (PDT). As ICU demographics shift towards older and sicker patients, the indications for closed pleural drainage procedures, bedside transthoracic biopsies, and percutaneous dilatational tracheostomies have dramatically increased. Although ultrasound expertise is considered an essential IP operator skill there is no validated curriculum developed to address this component. Further, there is a need for developing an educational tool that matches up with the curriculum and could be integrated real-time with ultrasound-guided procedures.
Echocardiography in the intensive care unit: An essential tool for diagnosis, monitoring and guiding clinical decision-making

Abstract:In the last years, new trends on patient diagnosis for admission in cardiac intensive care unit (CICU) have been observed, shifting from acute myocardial infarction or acute heart failure to non-cardiac diseases such as sepsis, acute respiratory failure or acute kidney injury. Moreover, thanks to the advances in scientific knowledge and higher availability, there has been increasing use of positive pressure mechanical ventilation which has its implications on the heart. Therefore, there is a growing need for Cardiac intensivists to quickly, noninvasively and repeatedly evaluate various hemodynamic conditions and the response to therapy. Transthoracic critical care echocardiography (CCE) currently represents an essential tool in CICU, as it is used to evaluate biventricular function and complications following acute coronary syndromes, identify the mechanisms of circulatory failure, acute valvular pathologies, tailoring and titrating intravenous treatment or mechanical circulatory support. This could be completed with trans-esophageal echocardiography (TOE), advanced echocardiography and lung ultrasound to provide a thorough evaluation and monitoring of CICU patients. However, CCE could sometimes be challenging as the acquisition of good-quality images is limited by mechanical ventilation, suboptimal patient position or recent surgery with drains on the chest. Moreover, there are some technical caveats that one should bear in mind while performing CCE in order to optimize its use and avoid misleading findings. The aim of this review is to highlight the key role of CCE, providing an updated overview of its main applications and possible pitfalls in order to facilitate its use in CICU for clinical decision-making.

Abstract:Sepsis is a systemic inflammatory response syndrome (SIRS) caused by infection, which may lead to multiple organ dysfunction in susceptible patient. The most frequently involved organs/systems include the lung, kidney and circulation system. It is well established that sepsis is a risk factor for acute lung injury. While overt pulmonary infiltrates can be well captured by human operators, subtle structural changes of the lung might be ignored. Since the advantage of chest computed tomography (CT) is its capability of providing fine structural changes in high spatial resolution, the study of chest CT by means of computer science may provide further insights into the underlying pathophysiology. The integration of chest CT into the study of sepsis is limited partly due to the lack of well-curated database. The study aims to establish a database comprising detailed clinical tabular data, as well as the raw chest CT images. The database is intended to support a wide array of research studies involving radiomics in sepsis patients, helping to reduce barriers to the reproducibility of clinical research.

Abstract:Background: Interest in critical care–related artificial intelligence (AI) research is growing rapidly. However, the literature is still lacking in comprehensive bibliometric studies that measure and analyze scientific publications globally. Objective: The objective of this study was to assess the global research trends in AI in intensive care medicine based on publication outputs, citations, coauthorships between nations, and co-occurrences of author keywords. Methods: A total of 3619 documents published until March 2022 were retrieved from the Scopus database. After selecting the document type as articles, the titles and abstracts were checked for eligibility. In the final bibliometric study using VOSviewer, 1198 papers were included. The growth rate of publications, preferred journals, leading research countries, international collaborations, and top institutions were computed. Results: The number of publications increased steeply between 2018 and 2022, accounting for 72.53% (869/1198) of all the included papers. The United States and China contributed to approximately 55.17% (661/1198) of the total publications. Of the 15 most productive institutions, 9 were among the top 100 universities worldwide. Detecting clinical deterioration, monitoring, predicting disease progression, mortality, prognosis, and classifying disease phenotypes or subtypes were some of the research hot spots for AI in patients who are critically ill. Neural networks, decision support systems, machine learning, and deep learning were all commonly used AI technologies. Conclusions: This study highlights popular areas in AI research aimed at improving health care in intensive care units, offers a comprehensive look at the research trend in AI application in the intensive care unit, and provides an insight into potential collaboration and prospects for future research. The 30 articles that received the most citations were listed in detail. For AI-based clinical research to be sufficiently convincing for routine critical care practice, collaborative research efforts are needed to increase the maturity and robustness of AI-driven models.
