Abstract:Background The purpose of this study was to evaluate changes in subfoveal choroidal thickness after photodynamic therapy in patients with acute idiopathic central serous chorioretinopathy (ICSCR). Methods This was a retrospective observational study conducted in 63 participants. The primary outcome measure was subfoveal choroidal thickness at baseline and 3 days, one week, 4 weeks, and 12 weeks after photodynamic therapy. The secondary outcome measure was indocyanine green angiography at baseline and 4 weeks and 12 weeks after photodynamic therapy. Results Four weeks after photodynamic therapy, 20 (64.51%) symptomatic eyes showed hypofluorescence corresponding to the area of photodynamic therapy irradiation at the posterior pole. The mean subfoveal choroidal thickness increased significantly from 422±132 μm at baseline to 478±163 μm at day 3 after treatment (P=0.022) and then decreased to 362±113 μm at week 4 (P<0.001) and 339±135 μm at week 12 (P<0.001). Conclusion The subfoveal choroid in patients with acute ICSCR is thicker than in the normal population, and in symptomatic eyes is significantly thicker than in fellow eyes. Photodynamic therapy using a one third dose of verteporfin may decrease choroidal vascular hyperpermeability and choroidal thickness in patients with acute ICSCR.

Abstract:Purpose: To utilize volumetric analysis to quantify volumetric changes in choroidal vessels and stroma after photodynamic therapy (PDT) and focal laser photocoagulation (PC) for central serous chorioretinopathy (CSCR). Methods: This retrospective, comparative study included 58 eyes (58 patients) with CSCR (PC, 33 eyes; PDT, 25 eyes) followed up with swept-source optical coherence tomography at 3 months after treatment. Three-dimensional (3D) choroidal vessel and stromal volumes in each area of the central 1.5-mm-diameter circle, the torus-shaped area with 6-mm-diameter circle excluding the area of the central 1.5-mm-diameter circle, and the treated area of the Early Treatment Diabetic Retinopathy Study (ETDRS) grid centered at the fovea were analyzed using a deep learning-based method. Changes in volume at baseline and 1 and 3 months after treatment were compared. Results: The mean patient age was 49.3 ± 10.5 years. In the central 1.5-mm-diameter circle, the mean vessel and stromal volume rates significantly decreased after the treatment in both the PDT and PC groups (P = 0.00029 and P = 0.0014, respectively), and significant differences between the PDT and PC groups of continuous variables within times were observed in both volumes (P = 0.024 and P = 0.037, respectively). In the torus-shaped area and treated area, the PDT and PC groups both showed similar decreases in vessel and stromal volume over time. Conclusions: In the 3D optical coherence tomography volumetric analysis, both PDT and focal PC reduced choroid vessel volume in eyes with CSCR. Translational relevance: This new finding is useful in elucidating the pathogenesis and healing mechanisms of CSCR.
Short-Term Retinal Sensitivity and Metamorphopsia Changes following Half-Fluence Photodynamic Therapy in Central Serous Chorioretinopathy

Abstract:PURPOSE: The aim of this study was to investigate the short-term retinal sensitivity and metamorphopsia changes after half-fluence photodynamic therapy (PDT) in patients with central serous chorioretinopathy (CSC).METHODS: Patients with acute CSC and evidence of angiographic leakage and subretinal fluid on optical coherence tomography (OCT) were enrolled. Before, and 1 week (W-1) and 2 months (M-2) after PDT, all patients underwent best-corrected visual acuity, enhanced depth imaging OCT, microperimetry (MP) and preferential hyperacuity perimetry (PHP). We considered 'affected' the Early Treatment of Diabetic Retinopathy Study (ETDRS) sectors showing a mean retinal thickness greater than 350 μm on the baseline OCT thickness map. MP retinal sensitivity, PHP 'score' and retinal thickness were measured in each ETDRS 'affected sector'. Functional correlation between MP and PHP was also investigated.RESULTS: Twelve eyes of 12 patients were included. MP and PHP correlated with each other at baseline (p = 0.04) but not at W-1 (p = 0.12) and M-2 (p = 0.05). Mean MP retinal sensitivity slightly improved from 12.9 ± 5.0 dB at baseline to 14.5 ± 4.7 dB at M-2 (p = 0.2). The PHP score changed from 0.33 at baseline to 0.32 at M-2 (p = 0.3). In the ETDRS 'affected sectors', MP retinal sensitivity improved significantly from 12.9 ± 5.0 dB at baseline to 14.5 ± 4.7 dB at M-2 (p = 0.03), whereas the PHP score did not show significant changes over time.CONCLUSION: Half-fluence PDT achieved short-term improvement in the sensitivity of the retinal 'affected sectors' but did not influence metamorphopsia as measured on PHP in patients with acute CSC.

Macular function after PDT in myopic maculopathy: psychophysical and electrophysiological evaluation

Abstract:Purpose: To evaluate the effects of photodynamic therapy (PDT) on macular function in myopic subfoveal choroidal neovascularization (CNV). Methods: Fourteen eyes of 14 patients (mean age, 48.1 +/- 13.3 years) with myopic CNV (myopia ranging from -6.50 to -20 D) were enrolled. In each eye, at baseline and at 15 and 90 days after PDT with verteporfin, logMAR visual acuity (logarithm of the minimum angle of resolution VA), macular sensitivity by scanning laser ophthalmoscope microperimetry, and focal (central 9 x 9 degrees ) ERGs (FERGs) and pattern ERGs (PERGs) were assessed. Results: At 15 days after PDT, myopic CNV eyes showed, in relation to baseline values, a significant (ANOVA, P < 0.01) reduction in the diameter of the lesion that correlated (Pearson test, P < 0.01) with the significant (ANOVA, P < 0.01) increase in FERG and PERG amplitudes, VA, and scanning laser ophthalmoscopy (SLO) microperimetry results obtained from the central 1 degrees to 2 degrees of the macular area (SLO-CM). At 90 days after PDT, myopic CNV eyes showed, in comparison with baseline values, a nonsignificant (ANOVA, P > 0.01) reduction in the diameter of the lesion, a nonsignificant increase in VA and SLO-CM, and a still significant increase in FERG and PERG amplitudes. Conclusions: In myopic CNV eyes, PDT induces an increase, though not significant, in VA and macular sensitivity. These changes may be related to a reduction in the diameter of the lesion, with an improvement in the function of both ganglionic and preganglionic elements of the macular region, as suggested by the improvement in FERG and PERG responses.

Abstract:European Journal of Ophthalmology, Ahead of Print. PurposeTo analyse the longitudinal changes in imaging parameters in eyes with acute or chronic central serous chorioretinopathy (CSCR).MethodsThis was a multicentric, retrospective, longitudinal, observational study in patients with a diagnosis of CSCR and having at least 4 years of follow-up. Trend in choroidal thickness (CT), area of double layer sign (DLS), area of retinal pigment epithelium (RPE) alterations, and area of hyper-autofluorescence were analysed.ResultsA total of 175 eyes of 146 patients with a mean age of 52.9 ± 12.5 years were included. Fifty-two eyes had acute and 123 had chronic CSCR at baseline. There was an overall decreasing trend of CT values during follow up (from 354.4 ± 74.8 at baseline to 343.2 ± 109.4 at final visit). There was a steady rise in DLS width and RPE alteration in the overall cohort (from 811.7 ± 760.6 microns and 3.5 ± 3.3 disc areas respectively at baseline to 1209.1 ± 1113.18 microns, 4.1 ± 3.6 disc areas at final visit). An initial rise in hyper-autofluorescent area followed by steady fall was seen in chronic CSCR. A significant positive correlation (r = 0.54, p = 0.004) was seen between change in DLS width and change in area of hyper-autofluorescence. On linear regression analysis, lower CMT and absence of intraretinal fluid at baseline were associated with greater increase in RPE abnormalities.ConclusionAcute and chronic CSCR had different pattern of change in imaging parameters with area of RPE alterations in acute CSCR not reaching values close to that of chronic CSCR eyes even after long follow up.

Microvascular Retinal and Choroidal Changes in Retinal Vein Occlusion Analyzed by Two Different Optical Coherence Tomography Angiography Devices

Abstract:Purpose: To investigate retinal and choroidal microvascular changes and structural choroidal involvement in retinal vein occlusion (RVO). Methods: Retrospective analysis of treatment-naïve macular edema secondary to RVO, studied by optical coherence tomography (OCT) and OCT angiography (OCTA), before and after the loading phase of intravitreal injections of ranibizumab (IVR-LP). OCTA was performed using two different devices: AngioVue RTVue XR Avanti (spectral-domain OCTA) and Zeiss PLEX® Elite 9000 (swept-source OCTA). Results: 30 eyes of 30 consecutive patients (17 branch and 13 central RVO) were included. Central macular thickness and subfoveal choroidal thickness (SCT) were significantly reduced after IVR-LP (p < 0.001 and p = 0.046, respectively). 23 eyes were eligible for OCTA analysis. Baseline vessel density (VD) in deep capillary plexus (DCP) was significantly reduced in RVO eyes compared with fellow eyes (p = 0.03 and p = 0.002 for PLEX® Elite and AngioVue, respectively). After IVR-LP, no significant VD changes in any vascular layer was found. PLEX® Elite VD analysis showed significant differences in DCP between ischemic versus non-is-chemic eyes (p = 0.011). Conclusion: OCTA suggests a retinal vascular impairment of DCP but no involvement of choroid in RVO eyes. A greater baseline SCT could be due to a choroidal exudation. OCTA imaged with PLEX® Elite allowed to differentiate ischemic and non-ischemic patients at baseline.
Decrease in choroidal blood flow after half and one-third dose verteporfin photodynamic therapy for chronic central serous chorioretinopathy

Abstract:Abstract Background The effect of various reduced doses of verteporfin photodynamic therapy (PDT) on choroidal blood flow in chronic central serous chorioretinopathy (CSC) remains unclear. Therefore, this study aimed to evaluate choroidal blood flow after half-dose PDT (1/2PDT) and one-third dose PDT (1/3PDT) with verteporfin for chronic CSC using laser speckle flowgraphy and spectral-domain optical coherence tomography. Methods Twenty-seven eyes of 27 patients with serous retinal detachment (SRD) caused by chronic CSC for more than 6 months were included in this study. Patients were divided into the 1/2PDT ( n = 12; January 2018 to July 2019) and 1/3PDT ( n = 15; August 2016 to December 2017) groups based on the treatment period. The best-corrected visual acuity (BCVA), central retinal thickness (CRT), central choroidal thickness (CCT), and mean blur rate in the macular area (m-MBR) and optic nerve head (ONH-MBR) were obtained using laser speckle flowgraphy and evaluated at baseline (pre-treatment), and 2 weeks, 1 month, 3 months, and 6 months after treatment. Results We found that SRD disappeared after 1 month in 92 and 93% of patients’ eyes in the 1/2PDT and 1/3PDT groups, respectively. Recurrence of SRD was observed in one eye at the 6-month follow-up after 1/2PDT and two eyes at the 3-month follow-up after 1/3PDT. No significant improvement was observed in baseline BCVA in the 1/3PDT and 1/2PDT groups. The average m-MBR against baseline significantly decreased at 2 weeks and 1, 3, and 6 months in the 1/2PDT group. The average m-MBR against baseline decreased significantly only at the 2 weeks follow-up in the 1/3PDT group. The average rate of change in the CCT against baseline decreased significantly throughout for up to 6 months in the 1/2PDT group and for up to 3 months in the 1/3PDT group. No significant fluctuation was observed in the ONH-MBR. Conclusions: Here, PDT significantly affected choroidal blood flow depending on the verteporfin dose in chronic CSC. Abstract:Purpose To test the prognostic role of anterior scleral substantia propria (ASSP) thickness in predicting the 3‐month response after half‐dose photodynamic therapy (PDT) in central serous chorioretinopathy (CSCR) and to assess its clinical relevance of ASSP in different CSCR phenotypes. Methods A prospective, exploratory, multi‐centre cohort study conducted at IRCCS San Martino Hospital (Genoa, Italy) and Jules‐Gonin Eye Hospital (Lausanne, Switzerland). Demographic and clinical data, and optical coherence tomography (OCT) were collected at baseline and 3 months after PDT. Based on OCT images, we categorized CSCR phenotypes and collected clinically relevant imaging metrics. ASSP thickness was obtained from four different measurements using anterior segment (AS) OCT. Multivariable regression models were performed to evaluate the distribution of ASSP thicknesses among different CSCR phenotypes and to test the prognostic role of ASSP thickness in discriminating between PDT responders (complete subretinal fluid reabsorption) and partial responders. Results The study cohort comprised 109 Caucasian patients (82 males, 75.2%) with a total of 142 eyes: 84 eyes simple (59.1%) versus 58 eyes complex (40.9%) CSCR. Abstract:Purpose: To evaluate the efficacy and safety of photodynamic therapy (PDT) with chlorin E6 in chronic central serous chorioretinopathy (CSCR). Methods: This single-center, retrospective cohort study. All patients received standard PDT with 6 mg of chlorin E6 (Fotoran E6) for each m 2 of body surface area with 50 J/cm 2 fluence and a treatment time of 83 seconds. The changes in central retinal thickness (CRT), subfoveal choroidal thickness (SCT), and best-corrected visual acuity (BCVA) were evaluated at the end of the follow-up. Main outcome measure was the presence of subretinal fluid (SRF) at three months after PDT. Results: Thirty-nine eyes (32 males and 7 females, 43.7 ± 10.0 years) were included. In total, 50 PDT sessions were performed. At three months of follow-up, 31 out of 39 (79.5%) study eyes showed complete resolution of SRF. Mean follow-up period after PDT was 15.3 ± 7.8 months. At the end of follow-up, a complete resolution of SRF was observed in 32 out of 39 (82.7%) eyes. Mean change in CRT, SCT, and BCVA at the end of the follow-up was -139.7 ± 136.4 μm, -66.8 ± 36.2 μm, and -0.05 ± 0.08 LogMAR, respectively (p < 0.001). Neither systemic nor ocular adverse events were observed, nor were local reactions seen. Conclusion: Standard PDT with chlorin E6 is an effective and safe option in the management of chronic CSCR and may be considered as an alternative to the standard PDT with Visudyne.

Abstract:Abstract Background Optical coherence tomography angiography (OCTA) enables detailed, non-invasive assessment of ocular vasculature. This study uses OCTA imaging to evaluate choriocapillaris and retinal capillary perfusion density (CPD) changes in diabetic retinopathy following anti-vascular endothelial growth factor (VEGF) treatment. Methods Records of 38 eyes at a single institution were reviewed, grouped as non-diabetic controls (19 eyes), diabetes mellitus patients with diabetic retinopathy (DR, 19 eyes) and macular edema (DME). DR eyes were imaged at baseline, 6-months and 12-months after anti-VEGF treatment. Quantitative analyses assessed CPD of the choriocapillaris and retinal plexus. Results DR eyes showed decreased choriocapillaris whole-image CPD (62.6 ± 6.1 vs. 68.4 ± 5.1, p < 0.003), foveal CPD (61.2 ± 7.4 vs. 66.3 ± 9.8, p < 0.014), and parafoveal CPD (61.9 ± 6.6 vs. 68.2 ± 4.8, p < 0.002) at baseline. DR eyes also showed decreased retinal density, including whole-image CPD (46.9 ± 5.1 vs. 50.7 ± 5.6, p < 0.04), foveal CPD (27.6 ± 5.9 vs. 34.1 ± 6.1, p < 0.002), and parafoveal CPD (49.0 ± 5.6 vs. 53.1 ± 6.0, p < 0.011). Following 12 months of anti-VEGF treatment, no changes to retinal or choriocapillaris or CPD were observed. Retinal central subfield thickness decreased (397.1 ± 93.2 μm vs. 294.2 ± 71.5 μm, p < 0.005). Lastly, FAZ area (0.307 ± 0.133 mm 2 vs. 0.184 ± 0.058 mm 2 , p = 0.008) and perimeter (2.415 ± 0.692 mm 2 vs. 1.753 ± 0.408 mm 2 , p = 0.002) were increased in DR eyes at baseline. No changes to FAZ area or perimeter were seen with anti-VEGF treatment in DR eyes. Conclusions Compared to control, choriocapillaris and retinal CPD are reduced in DR, while FAZ area and perimeter are increased. No retinal capillary or choriocapillaris CPD changes were observed in DR eyes following anti-VEGF treatment.
Characterization of Retinal Microvascular Abnormalities in Birdshot Chorioretinopathy Using OCT Angiography

Abstract:Objective: To characterize changes in the retinal microvasculature in eyes with birdshot chorioretinopathy (BCR) using OCT angiography (OCTA). Design: Retrospective, observational, single center. Subjects: Twenty-eight patients (53 eyes) with BCR and 59 age-matched controls (110 eyes). Methods: En face OCTA images of the superficial capillary plexus (SCP) and deep capillary plexus (DCP) of each eye were assessed for the presence of microvascular abnormalities and used to measure the vessel and foveal avascular zone (FAZ) areas. A longitudinal analysis was performed with a representative cohort of 23 BCR eyes (16 patients) at baseline and at a 2-year time point. Main outcome measures: Whole-image vessel density (VD, %), extrafoveal avascular zone (extra-FAZ) VD (%), and FAZ area (%) were calculated and compared between control and BCR eyes. The frequency of microvascular abnormalities in BCR eyes was recorded. Results: In the SCP, increased intercapillary space and capillary loops were common features present on OCTA images. Whole-image and extra-FAZ VD were lower in the BCR group compared with controls (P < 0.0001 [SCP and DCP]). Foveal avascular zone area was enlarged in BCR eyes (P = 0.0008 [DCP]). Worsening best-corrected visual acuity was associated with a decrease in whole-image and extra-FAZ VD in the SCP (P < 0.0001 for both) and the DCP (P < 0.005 for both). Multivariable analysis, with vessel analysis parameters as outcomes, demonstrated that increasing age, increasing disease duration, lower central subfield thickness, and treatment-naive eyes (compared with those on only biologics) were associated with a significant decrease in both DCP whole-image and extra-FAZ VD. Increasing disease duration was associated with a significant decrease in both SCP whole-image and extra-FAZ VD. Longitudinal analysis demonstrated no significant difference in any vessel analysis parameters except for an increase in DCP FAZ area. Conclusions: Our results demonstrate a significant a decrease in VD in BCR eyes and an association on multivariable analysis with disease duration. Quantifying VD in the retinal microvasculature may be a useful biomarker for monitoring disease severity and progression in patients with BCR. Further studies with extended longitudinal follow-up are needed to characterize its utility in monitoring disease progression and treatment response. Abstract:OBJECTIVE:To evaluate long-term efficacy of half-dose photodynamic therapy on chronic central serous chorioretinopathy using optical coherence tomography. METHODS:A retrospective, interventional case series. Thirty-five eyes of 32 patients (35 eyes) with chronic central serous chorioretinopathy were included, with CSC durations from 6 to 60 months. Patients treated with half-dose verteporfin (3 mg/m(2)) PDT were followed up for 24 months. Best-corrected visual acuity (BCVA), fluorescein angiography (FA), indocyanine green angiography (ICGA) and optical coherence tomography (OCT) pre and post-treatment were all examined to observe the anatomic and functional changes. OCT findings highlighted the alterations of the ellipsoid zone (EZ), the interdigitation zone (IZ) and the central neural retina thickness (CNT) from 3 month to 24 month post-treatment. Regression analysis was applied to determine the correlation between the visual function (BCVA) and structure status (EZ, IZ, CNT). RESULTS:Thirty-five eyes of 32 patients with chronic CSC were treated with half-dose PDT. The logMAR BCVA was 0.53±0.36 at baseline, 0.49±0.39 at 3 months and 0.15±0.38 at 24 months post-PDT. There was no statistical significance between baseline and 3 month BCVA (t=1.952, P>0.05). There existed statistical significance between baseline and 24 month BCVA(t=10.761, P<0.01) as well as 3month and 24 month BCVA(t=8.930, P<0.01). OCT demonstrated the recovery of EZ and IZ. Statistical differences of EZ and IZ integrity degree were found between the 3 month and 24 month (Z=-3.419, -4.082, P<0.01). The mean CNT was (126.8±34.9) μm at baseline. And it kept constant at 3 month [(125.6±34.3)μm] and the 24 month visit [(124.8±35.1)μm]. No statistical significance was found between any two values (t=-1.830, -1.569, 0.793, P>0.05). At baseline, 3 month and 24 month, there was negative correlation between logMAR BCVA and CNT (β=-0.604, -0.634, -0.685, P<0.01) . At 3 month and 24 month, negative correlation between logMAR BCVA and EZ as well as logMAR BCVA and IZ were also found(β=-0.376, -0.394, -0.628, -0.496, P<0.05). A complete resolution of subretinal fluid was noted in 94.3% eyes at 3 month, further increased to 100% eyes at 24 month. The mean session of PDT was 1.03±0.17. CONCLUSIONS:Half-dose PDT was safe and effective for chronic CSC both functionally and morphologically in the long term. Visual benefits may not appear in a short run, yet became remarkable in a long run. Visual benefits might be attributed to recovery of EZ and IZ.

Abstract:Purpose: To investigate the changes in choroidal thickness (CT) in the macular and photocoagulated areas of patients with diabetic retinopathy after panretinal photocoagulation (PRP) using enhanced depth imaging optical coherence tomography.Methods: Patients with severe nonproliferative diabetic retinopathy or early proliferative diabetic retinopathy who needed PRP were included in this study. Choroidal thickness in the macula and the photocoagulated area was measured with enhanced depth imaging optical coherence tomography at baseline, 1 month, and 3 months after PRP.Results: The mean subfoveal CT increased significantly at 1 month (318.0 +/- 76.4 mu m, P < 0.001) and 3 months (317.4 +/- 75.3 mu m, P < 0.001) after PRP when compared with baseline (307.2 +/- 70.7 mu m). The mean CT in the photocoagulated area decreased significantly from 227.5 +/- 45.0 mu m to 206.9 +/- 41.1 mu m (P < 0.001) at 1 month and 206.0 +/- 41.4 mu m (P < 0.001) at 3 months after PRP. The subgroup analysis showed similar trends of CT changes in patients with or without diabetic macular edema. The mean change in CT was not statistically significantly correlated with the mean change in best-corrected visual acuity after PRP.Conclusion: In patients with severe nonproliferative diabetic retinopathy or early proliferative diabetic retinopathy, the mean CT increased significantly in the macular area and decreased significantly in the photocoagulated area after PRP. The results might reflect a redistribution of choroidal blood flow, which may be critical for retinal metabolism.

Abstract:OBJECTIVE:To evaluate the effect of Spectral HRA + OCT-guided photodynamic therapy with half-dose verteporfin in the treatment of chronic or recurrent central serous chorioretinopathy. METHODS:It was a retrospective case series. Twenty eyes of 18 patients with chronic or recurrent central serous chorioretinopathy were included. PDT was applied with half-dose verteporfin (3 mg/m(2)) on the site of active area shown on Spectral HRA + OCT (defined as focal or diffuse retinal pigment epithelial leakage, choroidal hyperpermeability, or pigment epithelial detachment located within the neurosensory detachment), and patients were observed to determine the anatomic and functional outcomes. Statistical analysis was performed using SAS (version 9.2). A P value of 0.05 was considered statistically significant. Comparisons of pre- and post-treatment best-corrected visual acuity (BCVA) and central foveal thickness (CFT) were performed using a paired t test. The relationship between BCVA and CFT post-treatment was analyzed by linear correlation analysis. Comparisons of the BCVA of eyes with and without the integrity of photoreceptor IS-OS and/or external limiting membrane at the last follow-up visit were performed using two sample t test. RESULTS:The median CSC duration was 4.5 months (ranged 1 month to 2 years). The median follow-up period after PDT was 8 months (ranged 6 to 20 months). The mean BCVA before PDT was 0.35 ± 0.16 (ranged 0.05 to 0.6), at 3 months after PDT was 0.72 ± 0.32 (ranged 0.1 to 1.5) and at the last follow-up visit was 0.78 ± 0.29 (ranged 0.3 to 1.5) (t = 6.444, 6.883, P < 0.05). Fifteen eyes (75.0%) had improved vision, and 5 eyes (25.0%) had stable vision. The mean CFT was reduced from (369.0 ± 120.9) µm before PDT to (193.3 ± 30.6) µm 1 month after PDT, (194.9 ± 28.3) µm 3 month after PDT and (190.6 ± 33.7) µm at the last follow-up visit (t = -6.836, -6.826, -7.316; P < 0.05). At the last follow-up visit BCVA was not correlated with CFT (r = 0.166, P > 0.05), but BCVA of the eyes with the integrity of photoreceptor IS-OS and/or external limiting membrane was better than that of without (t = -3.53, P < 0.05). Subretinal fluid disappeared in all eyes 1 month after PDT and there was no recurrence during the follow-up. CONCLUSIONS:Spectral HRA + OCT-guided photodynamic therapy with half-dose of verteporfin seems effective and safe for the treatment of chronic or recurrent central serous chorioretinopathy.

Abstract:Purpose To evaluate different quantitative non-invasive retinal biomarkers of microvascular impairment and neurodegeneration in patients affected by mild and moderate non proliferative diabetic retinopathy (NPDR) with or without macular microaneurysms (MAs). Methods A cross-sectional case-control study. Ninety-seven eyes with NPDR, 49 with no central MAs and 48 with central MAs, underwent color fundus photography and optical coherence tomography (OCT)/OCT-angiography (OCT-A). Thickness of central macula, retinal nerve fiber layer (NFL), ganglion cell layer (GCL+) and NFL + GCL + was evaluated on OCT. FAZ metrics (ImageJ), perfusion and vessel density (PD/VD), and fractal dimension (FD) (MATLAB) were evaluated on 3 × 3 OCT-A slabs of both superficial and deep capillary plexuses (SCP/DCP). All evaluations were performed on the full image and after subdivision in 4 quadrants. Results In the MA group, 77 MAs were detected (45.5% in the DCP). The MA group showed: increased FAZ area and perimeter in the SCP ( p < 0.01) and DCP ( p = 0.02), and reduced circularity index in the SCP ( p = 0.03); reduced VD in the SCP ( p < 0.01) and reduced PD, VD ( p < 0.01) and FD ( p = 0.02) in the DCP; decreased VD and FD in the SCP ( p = 0.02 and p = 0.05), and in VD and FD in the DCP in the inferior quadrant ( p = 0.04 and p = 0.03); a decrease in VD in the SCP in the nasal quadrant ( p = 0.05). No differences have been detected in OCT parameters. Conclusions Our results suggest that the presence of central MAs in patients with NPDR may correlate with more pronounced macular microvascular impairment, particularly during the mild and moderate stages of the disease.

Abstract:Background: To evaluate the anatomical and functional outcomes of high-density subthreshold micropulse laser (HSML) treatment in a cohort of patients diagnosed with chronic central serous chorioretinopathy (CSCR) whose treatment with photodynamic therapy (PDT) was delayed due to the worldwide shortage of verteporfin. Methods: Prospective interventional study which included 42 eyes of 40 patients diagnosed with chronic CSCR and on the waiting list for PDT who received rescue therapy with HSML using the Navilas® System device (OD-OS GmBH, Teltwo, Germany). Best corrected visual acuity (BCVA), subretinal fluid (SRF), and subfoveal choroidal thickness (SFCT) were measured at inclusion and during the follow-up visits at 2, 4, and 6 months. Results: The mean waiting time from the indication of PDT until treatment with HSML was 14.6 ± 9.7 months (range 5-21). There were no differences in the pre-treatment BCVA compared with the 6-month follow-up visit (67 ± 16.7 letters and 67.5 ± 8.2 letters respectively, p = 0.136). However, there was a significant decrease in the mean SFCT of -39.6 ± 37.1 μm (p = 0.030). Additionally, there was a decrease in SRF height between the pre-treatment measure (123.0 ± 49.8 μm) and the 2, 4, and 6-month follow-up visits after HSML of -58.5 ± 68.2 μm, -53.2 ± 76.3 μm, and -65.4 ± 53.6 μm respectively (p < 0.001). A complete resolution of the SRF was observed in 16/42 eyes (38.1 %) and a reduction of the SRF height in 85.7 % of the overall cohort was observed after HSML treatment. Conclusion: A significant anatomical improvement in SRF and a decrease in SFCT were observed in patients with CSCR who were previously waiting for PDT and were rescued by HSML. However, the rate of complete SRF resolution was low.
Optical coherence tomography for evaluation of photodynamic therapy in symptomatic circumscribed choroidal hemangioma.

Abstract:Purpose: To evaluate the results of photodynamic therapy for circumscribed choroidal hemangioma by optical coherence tomography. Methods: A retrospective review of 14 eyes with systematic circumscribed choroidal hemangioma receiving photodynamic therapy was evaluated by time-domain optical coherence tomography. Central retinal thickness and foveal photoreceptor layer thickness were assessed, including the status of the junction between the inner and the outer segments of the photoreceptors (IS/OS) in the fovea at the final examination. Results: The optical coherence tomography findings included 9 eyes (64.3%) with foveal cystoid edema, 9 eyes (64.3%) with subretinal fluid, and 1 eye (7.1%) with foveal thinning. After photodynamic therapy, optical coherence tomography showed absorbed subretinal fluid in 9 eyes and resolved edema in 7 of 9 eyes, with significantly decreased central retinal thickness (440 +/- 214 vs. 175 +/- 66 mu m, P = 0.003). IS/OS lines were observed in 4 eyes, which had a better final visual acuity (VA) (20/40-20/17) and a mean foveal photoreceptor layer thickness of 127.3 +/- 5.6 mu m. The remaining 10 eyes with an invisible IS/OS line had a poorer final VA and an attenuated foveal photoreceptor layer. The final VA was significantly correlated with initial VA (r = 0.796, P = 0.001) and foveal photoreceptor layer thickness at the final examination (r = 20.832, P = 0.001). Conclusion: Optical coherence tomography was useful for evaluating VA loss and prognosis in circumscribed choroidal hemangioma. Resolved foveal edema, absorbed subretinal fluid, and preserved photoreceptors contributed to the prognosis of photodynamic therapy for circumscribed choroidal hemangioma. RETINA 31: 336-343, 2011
