Abstract:Background: Low-dose UV treatment has been shown to be effective in mild psoriasis. However, the prolonged use of this treatment modality may raise concerns about its safety. These concerns are mainly focused on potential carcinogenic risks and overuse of this treatment modality. Objectives: This study was set out to evaluate possible carcinogenic risks of prolonged low-dose phototherapy. Methods: Three groups of psoriasis patients were evaluated: patients with local treatment only (n = 15); low-dose UV treatment at home for at least 18 months (n = 39); and patients with conventional NB-UVB (n = 8). Patients underwent visual inspection for signs of photoageing, and p53, CPDs and γH2AX were measured in skin biopsies. Patients undergoing low-dose phototherapy answered a survey about their recent patterns of use in a survey. Results: In the skin biopsies, low-dose UV treatment caused a lower amount of CPDs (p = .016) and p53 (p = .015) than NB-UVB. γH2AX did not show a significant difference. Self-report in patients undergoing low-dose phototherapy showed only one case of overuse (2.7%). Visual skin inspection showed no difference in signs of photoageing in the three groups. Conclusion: Prolonged treatment with low-dose UV for 18 months appears at least as safe as a course of conventional NB-UVB.

Abstract:Background: The risk of developing non-melanoma skin cancers (NMSCs) in patients with psoriasis is highly debated, and, to date, there is no unambiguous consensus opinion. Psoriasis is known to be related to an increased likelihood of other comorbidities such as psoriatic arthritis, obesity, metabolic syndrome, depression, and cardiovascular disease. Regarding cancer risk, previous studies have reported a greater tendency for the development of cutaneous T-lymphomas and colon, breast, kidney, and lung cancers. Furthermore, data from network meta-analyses have shown that patients with psoriasis have a higher risk of developing squamous cell carcinomas (SCCs) and/or basal cell carcinomas (BCCs). Multiple factors may contribute to the development of NMSCs in psoriatic patients, ranging from immunosuppression induced by biologic agents to previous phototherapy. However, the extent to which each factor may impact this risk has not been entirely assessed. The aim of this study was to evaluate the risk of developing NMSCs in patients with psoriasis observed for at least 5 years, by directly comparing patients only treated with phototherapy and patients treated with anti-tumor necrosis factor α (TNFα) agents, naive to other systemic treatments or phototherapy. Methods: We conducted a single-center retrospective study at Siena University Hospital, Italy, on 200 adult patients with psoriasis divided into two groups: (i) group 1, including 100 patients treated with narrow-band UVB phototherapy (nb-UVB), and (ii) group 2, including 100 patients treated with anti-TNFα. The patients included in group 2 had to be naive to cDMARDs and biologics and treated with anti-TNFα continuously for 5 years without loss of efficacy. All patients were observed for 5 years and underwent annual dermatologic examinations to assess for the occurrence of BCC or SCC. Results: A total of 34 out of 100 patients treated with phototherapy had one BCC or one SCC and 10 out of 34 developed two skin cancers. In particular, five had both types (one BCC and one SCC), and five had two BCCs. Conclusions: The results of our study highlight how the risk of developing NMSCs is greater in patients undergoing phototherapy compared to those treated with anti-TNFα. It also draws attention to the consideration that patients with scalp psoriasis might need closer follow-up as they could be more at risk of developing NMSCs.

Abstract:Background: A two-fold risk increase to develop basal cell carcinoma was seen in outdoor workers exposed to high solar UV radiation compared to controls. However, there is an ongoing discussion whether histopathological subtype, tumor localization and Fitzpatrick phototype may influence the risk estimates. Objectives: To evaluate the influence of histological subtype, tumor localization and Fitzpatrick phototype on the risk to develop basal cell carcinoma in highly UV-exposed cases and controls compared to those with moderate or low solar UV exposure. Methods: Six hundred forty-three participants suffering from incident basal cell carcinoma in commonly sun-exposed anatomic sites (capillitium, face, lip, neck, dorsum of the hands, forearms outside, décolleté) of a population-based, case-control, multicenter study performed from 2013 to 2015 in Germany were matched to controls without skin cancer. Multivariate logistic regression analysis was conducted stratified for histological subtype, phototype 1/2 and 3/4. Dose-response curves adjusted for age, age2, sex, phototype and non-occupational UV exposure were calculated. Results: Participants with high versus no (OR 2.08; 95% CI 1.24-3.50; p = 0.006) or versus moderate (OR 2.05; 95% CI 1.15-3.65; p = 0.015) occupational UV exposure showed a more than two-fold significantly increased risk to develop BCC in commonly UV-exposed body sites. Multivariate regression analysis did not show an influence of phototype or histological subtype on risk estimates. The restriction of the analysis to BCC cases in commonly sun-exposed body sites did not influence the risk estimates. The occupational UV dosage leading to a 2-fold increased basal cell carcinoma risk was 6126 standard erythema doses. Conclusion: The risk to develop basal cell carcinoma in highly occupationally UV-exposed skin was doubled consistently, independent of histological subtype, tumor localization and Fitzpatrick phototype.

Abstract:Actinic keratosis (AK) is considered a chronic skin disease mostly caused by long-term exposure to UV radiation and other risk factors such as immunosuppression, leading to an individual susceptibility for skin cancer manifestation. The treatment of AK is laborious and costly, and the incidence of skin cancer is forecasted to double until the year 2030 in an aging society.Risk factors in AK for malignant transformation in cutaneous squamous cell carcinoma (cSCC) are not fully understood, but studies suggest that histological features, such as atypia in the basal epidermal third and basal proliferation (PRO score) in AK play a pivotal role for development of malignancy. As the clinical appearance of AK does not correlate with the risk for malignancy, guidelines suggest treating every single AK lesion upon diagnosis. Skin imaging techniques, such as line-field confocal optical coherence tomography (LC-OCT) can help to provide an individual holistic follow-up for AK lesions by non-invasive visualization of atypia and basal proliferation. A follow-up for patients with AK may be critical for treatment success in terms of strengthening therapy adherence. When AK presents therapy refractory, cSCC manifests in nearly 30% of the cases after several years. Patients with AK suffering from field cancerization and immunosuppression are susceptible for a severe course of disease including metastasis and high mortality rates. Those vulnerable subgroups benefit from close skin cancer screening, early adequate treatment and chemoprevention, such as niacinamide or acitretin. Skin cancer prevention is substantial. Primary prevention should include chemical and physical UV-light protection and avoidance of indoor tanning. Secondary prevention is essential in high-risk populations, such as fair skin type elderly men and STORs. Tertiary prevention should comprise adequate treatment strategies to prevent therapy resistance, reoccurrence and cSCC, especially when field cancerization and immunosuppression are present.

Abstract:Objective: The aim of this study was to investigate the role of occupational and nonoccupational ultraviolet (UV)-exposure concerning the development of basal cell carcinoma (BCC). Methods: We undertook a population-based multicenter case-control study. Patients with first incident BCC (n = 836) were propensity score matched by age and sex to controls without skin cancer (n = 836). Sociodemographic characteristics, clinical characteristics, and lifetime UV-exposure were assessed by trained investigators. The differential estimation of occupational and nonoccupational UV-exposure dosages was based on validated instruments and established reference values. Associations were assessed using multivariable-adjusted conditional logistic regression models. Results: Individuals with high levels of occupational UV-exposure were at significantly increased BCC-risk compared with individuals with low [odds ratio (OR) 1.84; 95% confidence interval (95% CI) 1.19 to 2.83 and moderate (OR 1.97; 95% CI 1.20 to 3.22) occupational UV-exposure. Nonoccupational UV-exposure was not independently associated with BCC. Conclusion: Skin cancer prevention strategies should be expanded to the occupational setting.

Abstract:Several studies have described increased risk ratios of certain types of malignancies in patients with severe psoriasis. Among these, the lymphoproliferative disorders, including non-Hodgkin's lymphoma, cutaneous T-cell lymphoma and non-melanoma skin cancer, have been described most frequently. In addition to traditional cancer risk factors, some psoriasis treatments may also be implicated as potential carcinogens. The aim of this study was to perform a review of current literature on the association between psoriasis, the therapies against this disease and skin cancer, focusing on both epidemiology and the potential mechanism involved. Some psoriasis treatments, such as psoralen and ultraviolet A (PUVA) therapy and cyclosporine, have been associated with increased risk of skin cancer. Variable data have been reported for anti-tumour necrosis factor (TNF) drugs, whereas other class of biologics, like anti-IL17 and IL23, as well as ustekinumab, seem not to be related to skin cancer risk, such as the case of currently available small molecules.

Personal History of Psoriasis and Risk of Incident Cancer among Women: a Population-Based Cohort Study.

Abstract:BACKGROUND:To our knowledge, no prospective studies have examined the association between personal history of psoriasis and risk of nonmelanoma skin cancer.OBJECTIVE:We sought to examine this association based on 2 prospective cohorts, the Nurses' Health Study and Nurses' Health Study II.METHODS:Diagnoses of nonmelanoma skin cancer, including basal cell carcinoma and squamous cell carcinoma (SCC), were obtained by self-reported questionnaires. Information on clinician-diagnosed psoriasis and diagnosis year was collected and validated with a supplementary questionnaire.RESULTS:After 2,487,941 and 2,478,148 person-years of follow-up, we documented 1725 SCC cases and 16,075 basal cell carcinoma cases, respectively. For the combined cohorts, personal history of psoriasis was associated with an elevated risk of SCC, with a multivariate-adjusted relative risk (RR) of 1.51 (95% confidence interval [CI] 1.11-2.05). The associations appeared stronger with increasing psoriasis severity, with RR of 1.42 (95% CI 0.94-2.15) in the mild psoriasis group and RR of 1.99 (95% CI 0.74-5.32) in the moderate to severe psoriasis group (P trend = .03). There was no association between psoriasis and the risk of basal cell carcinoma (RR 0.95; 95% CI 0.75-1.18).LIMITATIONS:Lack of treatment data may bias the result.CONCLUSION:Personal history of psoriasis may be associated with an increased risk of SCC. Further investigations are warranted to understand the underlying mechanisms.

Abstract:Abstract Previous literature suggests an association between the use of biologics and the development of keratinocyte carcinoma in psoriasis. Our aim was to evaluate the risks of developing basal cell carcinoma (BCC) and cutaneous squamous cell carcinoma (SCC) in patients with moderate-to-severe psoriasis treated with biologic therapy compared with those receiving nonbiologic systemic therapy. We used data from BADBIR, a multicentre register evaluating the long-term safety and effectiveness of systemic therapy for patients with moderate-to-severe psoriasis, between September 2007 and November 2023. Data are linked with additional keratinocyte carcinoma events recorded in NHS Digital. Patients (Fitzpatrick skin types I–IV) with chronic plaque psoriasis, who were biologic naive at registration to BADBIR, with no history of any cancer, and who had completed at least one follow-up visit, were eligible for study inclusion. To balance between the compared groups in their baseline characteristics, we calculated probability-weighted regression adjustment estimators with a propensity score model including age, sex, previous acitretin, previous ciclosporin, and number of courses of psoralen and ultraviolet A or narrowband ultraviolet B. These weights were therefore used in a flexible parametric survival model to estimate hazard ratios (HRs) for developing incident BCC and SCC, using imputed data. In total, there were 9303 patients (67%) registered to the biologic cohort and 4622 patients (33%) in the nonbiologic systemic cohort. The biologic cohort was older (median age 44 years, interquartile range 35–53) vs. median 42 years, interquartile range 32–53), with more men (58% vs. 56%) and comorbidities (68% vs. 61%) compared with those in the nonbiologic systemic cohort. Patients in the biologic cohort contributed 62 141 and 62 433 person-years of follow-up for the BCC and SCC analyses, respectively, while patients in the nonbiologic systemic cohort contributed 19 565 and 19 592 person-years of follow-up. In total, 119 patients (1.3%) developed first BCC and 60 patients (0.6%) developed first SCC in the biologic cohort during follow-up. In the nonbiologic systemic cohort, 28 patients (0.6%) developed first BCC, with 17 patients (0.4%) developing first SCC. These differences in risk of developing first BCC or SCC were not statistically significant between the two cohorts: for BCC, adjusted hazard ratio (aHR) 1.18, 95% confidence interval (CI) 0.71–1.95; and for SCC, aHR 0.91, 95% CI 0.41–1.73. The findings from these real-world data showed no increased risk of BCC or SCC for biologic-treated patients compared with those receiving nonbiologic systemic therapy. However, caution is needed in interpreting the current findings due to the small number of first events. This study is funded by the Psoriasis Association.

Abstract:Abstract Photodermatoses comprise a heterogeneous group of disorders characterized by abnormal skin responses following physiological exposure to solar radiation. Previous investigations have established that photodermatoses are associated with significant psychological morbidity and poor health-related quality of life (HRQoL), although few studies have quantified this phenomenon in large patient cohorts. Therefore, the aim of this study was to perform a cross-sectional evaluation of patient-reported psychological morbidity and HRQoL in a large tertiary photobiology unit. Patients aged ≥ 16 years who attended outpatient appointments at a tertiary photobiology unit between October 2022 and August 2023 were issued a survey delivered directly to their smartphone using an online platform (DrDoctor). This survey used both free-text questions and established questionnaire tools to measure psychological morbidity and HRQoL. The questionnaires included General Anxiety Disorder-7 (GAD-7), Patient Health Questionnaire-9 (PHQ-9), Brief Illness Perception Questionnaire, Patient-Reported Outcome Measurement System-Profile 57, and Dermatology Life Quality Index (DLQI). Based on screening questions that identified the patient as new or follow-up and their diagnosis (if known), a tailored selection of questionnaires was administered. Formal thematic analysis was performed on free-text answers using NVivo (QSR International, Doncaster, Australia) and WordCloud. Descriptive statistical analyses were performed using SPSS (IBM, Armonk, NY, USA). During the study period, 1538 surveys were sent to patients, including those attending for repeat follow-ups. Data from 176 new patients were analysed and, following exclusion of surveys derived from repeat attendances, data from 414 follow-up patients were analysed. Among new patients, 86.9% were aware of their skin’s sensitivity to sunlight, with 61.3% reporting symptoms year round and 27.8% in summer only. Additional perceived triggers included stress, genetics, heat and medication, while patients’ aims for their appointments were to identify the cause of their condition, their diagnosis, a potential cure, and the best treatment options. Of 414 follow-up patients, 75% reported knowing their diagnosis. These were chronic actinic dermatitis (19%), erythropoietic protoporphyria (19%), polymorphic light eruption (15%), solar urticaria (12%), photoaggravated eczema (12%), lupus or other connective tissue disorders (7%), and other diagnoses (16%). In total, 32.6% of follow-up patients had a GAD-7 score ≥ 5 (consistent with clinically relevant anxiety) and 36.7% had a PHQ-9 score ≥ 5 (consistent with clinically relevant depression). Among conditions for which DLQI was measured, the highest median DLQI over the past year was 13, reported by patients with chronic actinic dermatitis. This study highlights the significant psychological impact of photodermatoses and their detrimental effects on HRQoL, which affect patients with chronic actinic dermatitis in particular.

Abstract:Abstract Background Psoriasis is a chronic systemic disorder with ocular involvement. Objectives The aim of the present study is to evaluate the risk of glaucoma among psoriatic patients. Methods Subjects of this cohort study were selected based on Chang Gung Research Database from January 1, 2003, to December 31, 2012. Follow-up ended on December 31, 2017. The participants of the control group were matched with the psoriatic group by gender, age, and index date at a 4:1 ratio. The hazard ratios of glaucoma were estimated using Cox regression analysis. We also evaluated the relationship between the risk of glaucoma and systemic therapies as well as phototherapy and topical corticosteroid in patients with psoriasis. Results A total of 6,682 patients with psoriasis and 26,728 matched controls were enrolled. The study population is composed of mainly Males accounting for 64.2% of the study population. The psoriatic group had higher incidence rates than the control group for glaucoma (adjusted hazard ratio 1.405 [95% confidence interval, 1.051-1.879]). Psoriatic patients receiving psoralen and ultraviolet-A (PUVA) therapy for more than 200 sessions had an increased risk of glaucoma. Conclusions Psoriatic patients had an increased risk of glaucoma. Long-term PUVA therapy raised the risk of glaucoma in psoriatic populations.

Abstract:Abstract Background: Psoriasis has long been hypothesized to pose an increased risk of cancer due to the chronic, inflammatory state induced by psoriasis. Most prior studies on psoriasis and cancer were based on clinical settings, and the interest has been remarkably focused on the association between systemic therapies, typically only used in moderate-to-severe psoriasis, and risk of psoriasis. The evidence linking psoriasis with cancer is still controversial and few data are available from prospective studies. We systemically examined the association between a personal history of psoriasis and risk of incident cancer (other than skin basal cell carcinoma and squamous cell carcinoma). Methods and Findings: A total of 64,990 participants were included from the Nurses’ Health Study (1996-2012). Diagnoses of cancers were obtained by biannual self-reported questionnaires and all cancers were confirmed. Clinician-diagnosed psoriasis and diagnosis year were collected in 2008 and validated with a supplementary questionnaire. We included 2180 cases with psoriasis and most had mild disease (92%). During 957,359 person-years’ follow-up, 8,348 incident cancers were documented. We did not find an altered risk of total cancer associated with personal history of psoriasis (multivariate-adjusted hazard ratio [HR] = 1.02; 95% CI: 0.89-1.17). However, for individual cancers, a personal history of psoriasis was associated with an increased risk of melanoma (HR = 1.94, 95% CI: 1.20-3.12) and kidney cancer (HR = 2.50, 95% CI: 1.27-4.92). The associations appeared stronger among psoriasis with 1 or more palm body surface area (BSA, P for trend 0.003 for melanoma and 0.0004 for kidney cancer), but remained significant even after exclusion of psoriasis with more than two palms’ BSA involvement. Conclusions: We suggest a positive association between a personal history of psoriasis and risk of melanoma and renal cancer, which appears not be entirely a result of greater medical scrutiny or use of systemic therapies. The Impact of Education Level on Basal Cell Carcinoma Development Risk

Abstract:Background: Basal cell carcinoma (BCC) stands as the most common skin malignancy, with its high incidence rate and associated costs rising annually. The origin of BCC is related to environmental, genetic, and phenotypic factors. Among these, the most important risk factor is exposure to UV light triggering keratinocyte carcinogenesis, causing cumulative cellular damage that leads to BCC development. Individuals' educational background and awareness of skin cancer risk factors may influence the development of BCC. Lack of knowledge about risk factors (like chronic UV exposure, sunburn, artificial solar beds, and fair skin color), prevention methods, and jobs involving outdoor activities may be associated with BCC formation. Aim: The aim of the study was to analyze recent trends and the risk factors associated with BCC, while also revealing any potential link between BCC and the patient's education level and awareness of skin cancer risk factors. Design and methods: A hospital-based case-control study was conducted, involving a total of 141 individuals. Among them, 47 were clinically and histologically confirmed BCC patients, while the remaining participants served as controls. The control group comprised 94 individuals matched for age and gender. Data on various factors including gender, age, residency, education level, Fitzpatrick skin type, outdoor activities, use of solariums, and UV therapy, as well as awareness of potential BCC triggers, were collected using an adapted questionnaire and subjected to analysis. The collected data underwent statistical evaluation. Results: Most of the BCCs (n = 52; 71.2%) were located in sun-exposed areas (p < 0.001), with a female/male ratio of 1.35 to 1. The nodular type of BCC was the most common form (n = 49; 67.2%). The percentage of patients in the study group with Fitzpatrick phototypes I and II (n = 38; 80.9%) was significantly higher than in controls (n = 50; 53.2%, p = 0.002). The percentage of persons with higher education levels (bachelor's degree, master's degree, and post-diploma) was significantly less prevalent among cases compared to controls (n = 20 (42.6%) vs. n = 58 (61.7%), respectively (p = 0.033)). Notably, BCC patients with low education levels exhibited significantly lesser awareness concerning genetic factors and chronic solar radiation. Conclusions: Coexistence of factors, such as a medical history of skin cancer, having Fitzpatrick skin types I and II, engaging in outdoor work exposed to the sun, knowledge that genetic factors are risk factors of skin cancer, and knowledge that stress is a risk factor of skin cancer, are significant predictors of the disease. A lower level of education and limited awareness about risk factors can also be a risk factor for BCC. It is essential to raise awareness about potential triggers and preventive measures within the population to reduce the incidence of the disease.

Abstract:Abstract There are long-standing concerns that the use of biologic therapies to treat patients with psoriasis could confer an increased risk of developing cancer due to their immunomodulatory effects. We aimed to assess the risk of any incident cancer (excluding keratinocyte carcinoma, KC), hereafter termed ‘incident cancer’, in patients with moderate-to-severe psoriasis treated with biologic therapy compared with nonbiologic systemic therapy. We used data from BADBIR, a multicentre register evaluating the long-term safety and effectiveness of systemic therapy for patients with moderate-to-severe psoriasis, between September 2007 and November 2023, with linkage to data from NHS Digital to ascertain incident cancers not captured in BADBIR. Eligible for inclusion were patients with chronic plaque psoriasis, who were biologic naive at registration to BADBIR, who had no personal history of cancer (excluding KC) and who had completed at least one follow-up visit. To balance between the compared groups in their baseline characteristics, we calculated probability-weighted regression adjustment estimators with a propensity score model including age, sex, body mass index, smoking, alcohol consumption, previous exposure to nonbiologic systemic therapy and number of comorbidities. Therefore, these weights were used in a flexible parametric survival model to estimate hazard ratios for developing incident cancers, using imputed data. There were 11 185 patients (66%) registered to the biologic cohort and 5673 patients (34%) to the nonbiologic systemic cohort. The biologic cohort was older (median age 44 years, interquartile range 34–54 vs. 42 years, interquartile range 32–53), with more men (59% vs. 57%) and comorbidities (68% vs. 61%) compared with the nonbiologic systemic cohort. Patients in the biologic cohort contributed 73 643 person-years of follow-up and patients in the nonbiologic systemic cohort contributed 23 694 person-years. During follow-up, 467 patients (4%) in the biologic cohort and 169 patients (3%) in the nonbiologic systemic cohort developed incident cancer. There was no significant difference in the risk of developing incident cancer between the biologic cohort and the nonbiologic systemic cohort (adjusted hazard ratio 0.88, 95% confidence interval 0.69–1.11). The results from these real-world data are reassuring for patients with psoriasis and clinicians, as it appears that treatment with biologic therapy does not confer an increased risk of developing overall cancer (excluding KC), in the short-to-medium term, compared with nonbiologic systemic therapy. However, given the heterogeneity of all cancers combined, risks of developing cancers from common subgroups or site-specific cancers (breast, prostate, lung, colorectal and melanoma) in biologic-treated patients need to be investigated. The Prevalence and Awareness of Regular Follow Up of Patients Treated with Phototherapy for Skin Cancer Surveillance

Abstract:Objective background: Regular full-body skin examination is commonly ignored in patients post-phototherapy, despite ultraviolet (UV) radiation being carcinogenic. Our objectives are to assess the prevalence of regular follow-up and full-body skin examination for patients treated with phototherapy, as well as the relationship between phototherapy exposure and the development of skin cancer in Saudi Arabia. Methods settings design: This was a cross-sectional retrospective study conducted from January 2022 to July 2022. The study included 99 patients, selected via simple random sampling, from King Saud University Medical City, Riyadh, Saudi Arabia, who underwent phototherapy for at least 8 weeks and were followed for a minimum of 18 months post-treatment. Patients who met the inclusion criteria were called and given a questionnaire. Results: Out of 99 patients, only 26 (26.26%) underwent full-body skin examinations by their physicians after phototherapy treatment. The average follow-up time after phototherapy was 3.2 years. Most study participants (85.85%) were unaware that cancer was a possible complication of phototherapy. Participants with knowledge about skin cancer complications were more likely to have undergone a full-body exam (P = 0.001). None of the participants developed any type of skin cancer after phototherapy. Conclusion: Patients treated with phototherapy had no adequate information about the risk of skin cancer. The incidence of skin cancer was zero in our small cohort. Dermatologists in Saudi Arabia do not have an evidence-based notion regarding the risk of skin cancer among the Arab population after phototherapy. Since There is a lack of data examining the relationship between phototherapy and skin cancer in the Arab region, this study should trigger future studies with large populations and longer follow up periods.
Ultraviolet B phototherapy does not increase the risk of skin cancer among patients with atopic dermatitis: A population-based retrospective cohort study

Abstract:Background: UV-B phototherapy is a common treatment modality for patients with atopic dermatitis (AD), but its long-term safety in terms of cutaneous carcinogenic risk has not been studied. Objective: To investigate the risk of skin cancer among patients with AD receiving UV-B phototherapy. Methods: We conducted a nationwide population-based cohort study from 2001 to 2018 to estimate the risk of UV-B phototherapy for skin cancer, nonmelanoma skin cancer, and cutaneous melanoma in patients with AD. Results: Among 6205 patients with AD, the risks of skin cancer (adjusted hazard ratio [HR], 0.91; 95% CI, 0.35-2.35), nonmelanoma skin cancer (adjusted HR, 0.80; 95% CI, 0.29-2.26), and cutaneous melanoma (adjusted HR, 0.80; 95% CI, 0.08-7.64) did not increase among patients with AD treated with UV-B phototherapy, compared with those who did not receive UV-B phototherapy. Additionally, the number of UV-B phototherapy sessions was not associated with an increased risk of skin cancer (adjusted HR, 0.99; 95% CI, 0.96-1.02), nonmelanoma skin cancer (adjusted HR, 0.99; 95% CI, 0.96-1.03), or cutaneous melanoma (adjusted HR, 0.94; 95% CI, 0.77-1.15). Limitations: Retrospective study. Conclusion: Neither UV-B phototherapy nor the number of UV-B phototherapy sessions was associated with an increased risk of skin cancers among patients with AD.

Abstract:Abstract Background Psoriasis is a chronic and non-transient disease with increased epidermal proliferation in the skin. Dysregulation of the immune system is an important factor in this pathology. Inflammation markers, pro-inflammatory cytokines, and immune cells are reported to be changed in psoriasis. Study design In the current cohort study, the possible changes in interleukin-6 (IL-6), neopterin levels, and kynurenine (Kyn) pathway in 42 psoriasis patients compared to 30 controls, and their change with narrow-band (NB) UVB treatment were investigated. Methodology IL-6 and serum neopterin levels were analyzed with ELISA kits. HPLC analyses were performed to detect urinary neopterin, serum Kyn, and tryptophan (Trp) levels. Results IL-6 levels were lower, while Kyn levels and the Kyn-to-Trp ratio were higher in psoriasis patients compared to control subjects ( p < 0.01, all). Conclusion Narrow-band ultraviolet B (NB-UVB) treatment decreased Psoriasis Area and Severity Index (PASI) scores and increased urinary neopterin levels of the patients (both, p < 0.01). Serum neopterin was correlated with Kyn and Kyn/Trp levels before and after NB-UVB treatment ( p < 0.05, all). These findings point out that the measured parameters might be considered to support the PASI score in both diagnosis and prognosis of psoriasis rather than evaluating the severity of the disease.

Abstract:Background and Objectives: The incidence of skin cancer is increasing worldwide and is causing a massive burden to the health-care system. It arises as a result of interplay between various predisposing factors. We intend to study the various clinical presentations of cutaneous malignancies and the demographic pattern and risk factors associated with them. Methodology: The study was done over a period of 7 years, 181 patients were diagnosed with cutaneous malignancy in the department of dermatology of our tertiary care hospital. A diagnosis was done based on history, clinical and histopathological examination. Results: Of 181 patients, majority of the patients belonged to 7 th decade ( n = 56, 31%) with the mean age being 55 years. The study group had 105 males (58%) and 76 (42%) females. The most common site was the head-and-neck region ( n = 105, 58%). The most important associated condition/predisposing factor was ultraviolet (UV) radiation exposure ( n = 79, 44%). Keratinocytic tumors formed the largest group ( n = 131, 72%) in which basal cell carcinoma (BCC) was the most common ( n = 75, 41%). Surgical excision was the main modality of treatment and was done in 111 (61%) patients. Conclusion: BCC was the most common skin cancer with head-and-neck region being the most common location in our study. Important risk factors were increasing age, male sex, and prolonged exposure to UV radiation. Early diagnosis helps in planning effective management strategies.
