Abstract:Objective: To investigate the incidence of myocardial injury in children with critically ill children without primary cardiac disease and the association between elevated cardiac troponin I (cTnl) and creatine kinase MB (CK-MB) concentrations and disease progression and prognosis to guide early treatment. Methods: The serum cTnI and CK-MB concentrations of 292 children with critically ill children without primary cardiac disease in Yantai Yuhuangding Hospital between January 2021 and January 2024 were retrospectively analyzed within 24 h after entering the Pediatric Intensive Care Unit (PICU). The children were divided into normal and abnormal groups according to the myocardial marker results. The abnormal group was further divided into the cTnI-elevated, CK-MB-elevated, single-elevated (cTnI- or CK-MB-elevated) and double-elevated (cTnI- and CK-MB-elevated) groups. The differences in the clinical indicators and their relationships with prognosis for the groups were compared. Results: The incidence of myocardial injury among the critically ill children without primary cardiac disease was 55.1%. The incidence of myocardial injury in children with infectious diarrhea combined with moderate and severe dehydration reached 85.19%. The pediatric critical illness score; frequency of use of vasoactive drugs; hypotension, shock, heart failure, respiratory failure, and multiple organ dysfunction syndrome; and mortality indexes differed significantly for the normal and abnormal myocardial marker groups (P < 0.05). The single-elevated and normal groups only showed a difference in mortality (P < 0.017). The cTnI and CK-MB concentrations were negatively correlated with prognosis (P < 0.01). Conclusion: Myocardial injury, as evidenced by elevated cardiac biomarkers, is common in critically ill children without primary cardiac illness. cTnI and CK-MB are associated with outcomes. Shock, heart failure, and multiple organ dysfunction syndromes are independently associated with simultaneous elevations of CK-MB and cTnI concentrations. Further prospective studies are needed to elucidate the clinical utility of these biomarkers.

In addition, the '+' and '-' signs for the 'Cell lysis' experiments shown underneath the gels in Fig. 1B were incorporated the wrong way around. Abstract:Objectives Identifying high-risk paediatric patients with a poor prognosis and providing timely and adequate treatment are critical. This study aimed to evaluate the effects of different types of cardiac enzyme spectrum within 24 hours of admission on the short-term prognosis of patients in paediatric intensive care units. Design A retrospective study. Setting A single-centre, tertiary care hospital in China, with patient data from 2010 to 2018. Participants A total of 4343 critically ill children were enrolled. Intervention None. Primary and secondary outcome measures The main outcome measure was in-hospital mortality, which was defined as death from any cause during hospitalisation. The secondary outcome was 30-day mortality, intensive care unit (ICU) length of stay (LOS) and total LOS. Results Using the local polynomial regression fitting method, an approximately linear increase in in-hospital mortality was detected for creatine kinase (CK), creatine kinase MB (CK-MB), aspartate aminotransferase (AST) and lactate dehydrogenase (LDH). Among the different types of cardiac enzyme spectrum, LDH had the highest area under the curve value (0.729), followed by AST (0.701), CK-MB (0.613) and CK (0.557). The Kaplan-Meier analysis showed that the patients in the high LDH group had higher 30-day mortality. The multivariate logistic regression revealed that high LDH was independently associated with in-hospital mortality (OR 2.45, 95% CI 1.84 to 3.24). After propensity score matching (PSM) and sensitivity analysis, the results remained consistent. Conclusions LDH is a reliable outcome predictor in critically ill children, including those with various comorbidities.

Abstract:Objective To observe the changes of serum cardiac troponin I(cTnI) and the changes of creatin kinase MB(CK-MB) at perioperation for the children with congenital cardiac diseases,and to estimate the clinical value of cTnI in reflecting myocardium injury in open heart surgery with cardiac pulmonary bypass. Methods Forty child patients undergoing correction operation of congenital cardiac defects were randomly divided into two groups: group I(beating heart,n=20) and groupⅡ(cold crystalloid cardioplegia,n=20).Central venous blood samples were taken at different times at perioperation to measure serum cTnI and activities of CK-MB in the two groups.(Results)Serum cTnI and activities of CK-MB were increased in various degrees at postoperation in each group.CK-MB reached the peak value at 8 h to 12 h,while cTnI reached the peak value at 24 h postoperatively.Levels of cTnI were significantly higher in groupⅡthan those in groupⅠin all time spots at postoperation(P0.05),and the activities of CK-MB were significantly higher in groupⅡthan those in groupⅠfrom 1 h to 48 h on four spots postoperatively(P0.05). Conclusion The measurement of cTnI is more sensitive and specific than that of CK-MB in evaluating injury in open heart surgery.cTnI measurement can accurately reflect heart injury and evaluate curative effect.The level of cTnI in beating heart group is lower than that of cold crystalloid cardioplegia group,which indicates that the method of beating heart is better than that of cold crystalloid cardioplegia in protecting the heart in open heart(surgery injury.)

Abstract:This study aimed to evaluate the distribution of plasma troponin I concentration and establish the 99th percentile reference for hs-cTnI in a hospitalized population without a cardiovascular discharge diagnosis from the Shandong area. The hs-cTnI data of anonymous paediatric patients were collected from Qingdao University-Affiliated Yantai Yuhuangding Hospital from 2016 to 2020. Indirect methods were used to calculate the hs-cTnI 99th percentile reference of the whole population and different age groups. Fitting curves and corresponding equations were displayed to determine the relationship between age and hs-cTnI level using the analysis of covariate variance. Hs-cTnI plasma levels were highest in the first week of life and declined with age in days. This study found significant differences in the troponin reference intervals for children in different age stratification. The serum hs-cTnI concentration decreased with age in days. In some subgroups, hs-cTnI levels between genders showed a significant difference after the analysis of covariance showed that age was the only predictor of hs-cTnI plasma levels. A non-linear relationship was observed between age and hs-cTnI levels. Thus, curvilinear fitting curve equations for each group were constructed to evaluate the possible relationship between age and hs-cTnI concentration. During paediatric period, the highest hs-cTnI concentrations were observed in children aged <1 year, especially those under 7 days. This study presented the 99th percentile cut-offs for different age groups in children aged 0–14 years, which can provide a certain reference value for the clinical diagnosis and treatment of myocardial injury in children.

The Value of High-Sensitivity C-reactive Protein in Evaluating Myocardial Damage and the Prognosis in Children with Mycoplasmal Pneumonia

Abstract:Objective: To investigate the value of high-sensitivity C-reactive protein (hs-CRP) in evaluating the myocardial damage and prognosis in children with mycoplasmal pneumonia. Materials: A total of 150 children with mycoplasmal pneumonia were selected. According to their serum creatine kinase isoenzyme (CK-MB) level, they were divided into 72 cases of the myocardial damage group and 78 cases of the non-myocardial damage group. Eighty healthy children undergoing physical examination were selected as the control group. The electrocardiography results and serum CK-MB and hs-CRP levels were compared among the subjects. The correlations among the above indexes were analyzed. Results: The levels of hs-CRP and CK-MB in the myocardial damage group were significantly higher than those in the nonmyocardial damage group and control group, respectively (P<0.05). The rates of abnormal hs-CRP and abnormal electrocardiogram in the myocardial damage group were significantly higher than those in the non-myocardial damage group, respectively (P<0.05). In the 150 children with mycoplasmal pneumonia, the serum hs-CRP and CK-MB levels were positively correlated (P<0.001), and the abnormal hs-CRP rate was positively correlated with the abnormal electrocardiogram rate (P<0.001). In the myocardial damage group, the serum levels of hs-CRP and CK-MB after treatment were significantly lower than those before treatment, respectively (P<0.05). After treatment, each index in the myocardial damage group had no significant difference with those in the control group (P>0.05). Conclusion: hs-CRP may be an important index for evaluating the myocardial damage and prognosis in children with mycoplasmal pneumonia. The combination of hs-CRP and CK-MB detection has obvious guiding significance for the monitoring and treatment of mycoplasmal pneumonia complicated by myocardial damage.

Abstract:Background: The diagnosis of acute myocardial infarction (AMI) using high-sensitivity cardiac troponin T (hs-cTnT) remains challenging in patients with kidney dysfunction. Methods: In this large, multicenter cohort study, a total of 20 912 adults who underwent coronary angiography were included. Kidney function-specific cut-off values of hs-cTnT were determined to improve the specificity without sacrificing sensitivity, as compared with that using traditional cut-off value (14 ng/L) in the normal kidney function group. The diagnostic accuracy of the novel cut-off values was validated in an independent validation cohort. Results: In the derivation cohort (n = 12 900), 3247 patients had an estimated glomerular filtration rate (eGFR) <60 mL/min/1.73 m2. Even in the absence of AMI, 50.2% of participants with eGFR <60 mL/min/1.73 m2 had a hs-cTnT concentration ≥14 ng/L. Using 14 ng/L as the threshold of hs-cTnT for diagnosing AMI led to a significantly reduced specificity and positive predictive value in patients with kidney dysfunction, as compared with that in patients with normal kidney function. The kidney function-specific cut-off values were determined as 14, 18 and 48 ng/L for patients with eGFR >60, 60-30 and <30 mL/min/1.73 m2, respectively. Using the novel cut-off values, the specificities for diagnosing AMI in participants with different levels of kidney dysfunction were remarkably improved (from 9.1%-52.7% to 52.8-63.0%), without compromising sensitivity (96.6%-97.9%). Similar improvement of diagnostic accuracy was observed in the validation cohort (n = 8012). Conclusions: The kidney function-specific cut-off values of hs-cTnT may help clinicians to accurately diagnose AMI in patients with kidney dysfunction and avoid the potential overtreatment in practice.

Abstract:The authors of the above-mentioned paper noticed an error in Fig. 6C as reported. The loading sequence of LC3B and the corresponding loading control GAPDH did not follow the sequence of bar graph. The LC3B gel band was thus spliced together (although from the same membrane) to accommodate the loading sequence of bar graph. We failed to insert vertical lines between groups to indicate the non-continuous nature of these gels. The original GAPDH loading for LC3B was not used due to the complicated splicing procedure.
