Abstract:Abstract Pancreatic β cell damage is the primary contributor to type 2 diabetes mellitus (T2DM); however, the underlying mechanism remains nebulous. This study explored the role of ferroptosis in pancreatic β cell damage and the protective effects of grape seed proanthocyanidin extract (GSPE). In T2DM model rats, the blood glucose, water intake, urine volume, HbA1c, and homeostasis model assessment-insulin resistance were significantly increased, while the body weight and the insulin level were significantly decreased, indicating the successful establishment of the T2DM model. MIN6 mouse insulinoma β cells were cultured in high glucose and sodium palmitate conditions to obtain a glycolipid damage model, which was administered with GSPE, ferrostatin-1 (Fer-1), or nuclear factor erythroid 2-related factor 2 (Nrf2) small interfering (si) RNA. GSPE and Fer-1 treatment significantly improved pancreatic β-cell dysfunction and protected against cell death. Both treatments increased the superoxide dismutase and glutathione activity, reduced the malondialdehyde and reactive oxygen species levels, and improved iron metabolism. Furthermore, the treatments reversed the expression of ferroptosis markers cysteine/glutamate transporter (XCT) and glutathione peroxidase 4 (GPX4) caused by glycolipid toxicity. GSPE treatments activated the expression of Nrf2 and related proteins. These effects were reversed when co-transfected with si-Nrf2. GSPE inhibits ferroptosis by activating the Nrf2 signaling pathway, thus reducing β-cell damage and dysfunction in T2DM. Therefore, GSPE is a potential treatment strategy against T2DM. What problem does this paper attempt to address?
The main issue this paper attempts to address is the role of ferroptosis in the damage of islet β-cells in type 2 diabetes mellitus (T2DM) and the study of how grape seed proanthocyanidin extract (GSPE) protects islet β-cells from ferroptosis by activating the Nrf2 signaling pathway. Specifically, the research background and objectives of the paper include: 1. **Pathological Mechanisms of Type 2 Diabetes Mellitus**: - Type 2 diabetes mellitus (T2DM) is a global health issue characterized mainly by hyperglycemia, islet β-cell damage, and dysfunction. - Although mechanisms such as oxidative stress, apoptosis, endothelial cell damage, inflammation, and autophagy are known to be related to the development of T2DM, its exact pathogenesis remains unclear. 2. **Relationship Between Ferroptosis and Type 2 Diabetes Mellitus**: - Ferroptosis is an iron-dependent form of cell death accompanied by the accumulation of lipid peroxides. - Studies have shown that T2DM is associated with ferroptosis, manifested by the production and accumulation of iron-dependent harmful lipid peroxidation products in the cell membrane. - Persistent hyperglycemia not only further damages β-cells but also leads to excessive iron accumulation in pancreatic β-cells, causing insulin resistance, loss of islet cell mass, and destruction of islet structure, ultimately leading to ferroptosis. 3. **Role of the Nrf2 Signaling Pathway in Ferroptosis**: - Nuclear factor erythroid 2-related factor 2 (Nrf2) plays a key role in antioxidation and is a major regulator in inhibiting ferroptosis. - Nrf2 regulates multiple genes involved in ferroptosis, such as glutathione peroxidase 4 (GPX4) and cystine/glutamate transporter (XCT), which can inhibit iron overload and lipid peroxidation. 4. **Potential Therapeutic Role of Grape Seed Proanthocyanidin Extract (GSPE)**: - Grape seed proanthocyanidin extract (GSPE) has free radical scavenging, antioxidant, and anti-inflammatory properties and has been proven effective in treating iron overload diseases. - Previous studies have shown that GSPE can improve hyperglycemia, insulin resistance, and the progression of diabetic nephropathy in STZ-induced diabetic rats by activating the Nrf2 signaling pathway. Therefore, this paper explores the role of ferroptosis in T2DM and the specific mechanisms by which GSPE protects islet β-cells from ferroptosis by activating the Nrf2 signaling pathway through the establishment of a T2DM rat model and a high glucose and high fat-induced MIN6 cell model. These research findings may provide new insights and potential therapeutic strategies for the treatment of T2DM.
