Abstract:Although investigations on the glycemic control of inpatients with diabetes show that hypoglycemia was ranked highly by clinicians as a barrier to achieving glycemic targets, there are few representative data regarding episodes of hypoglycemia in Chinese hospitals. The aim of the present study was to ascertain the prevalence and patterns of hypoglycemia in hospitalized patients, as identified by intensive bedside glucose monitoring. In all, 552 hospitalized patients with diabetes were recruited from endocrinology departments in six tertiary teaching hospitals in China between March and November 2009. All participants were aged 18 years and had been diagnosed with type 1 or type 2 diabetes (T1D and T2D, respectively) according to World Health Organization (WHO) criteria. Blood glucose values were obtained eight times a day: at fasting, after breakfast, before lunch, after lunch, before dinner, after dinner, at bedtime, at night, and at random, if necessary. Hypoglycemia was defined as a blood glucose level 3.9 mmol/L. Severe hypoglycemia was defined as a hypoglycemic event during which assistance from another person was required to actively administer resuscitative actions, including carbohydrate or glucagons. The characteristics of patients enrolled in the study are given in Table 1. Altogether, information was available for 5592 hospital days, with the median length of stay being 12 days per person. Overall, 262 patients (47.5%) developed 675 hypoglycemic episodes; of these episodes, 22 (3.3%) were severe hypoglycemia. The mean ( SD) blood glucose level during severe events was 2.86 0.63 mmol/L, significantly lower than blood glucose levels during episodes of mild hypoglycemia (3.37 0.50 mmol/L). When analyzing the prevalence of hypoglycemia according to the type of diabetes (i.e. T1D or T2D), 66% (21/32) of patients with T1D and 46% (241/520) of patients with T2D were found to be hypoglycemic. Of these, 21 patients with T1D recorded 119 episodes of hypoglycemia (5.7 episodes per person) and 241 patients with T2D recorded 556 episodes of hypoglycemia (2.3 episodes per person). Most of the hypoglycemic episodes occurred before lunch (28.6%), followed by night-time (14.9%), bedtime (13.4%), fasting (13.1%), after lunch (8.7%), before dinner (7.8%), after dinner (7.6%), and after breakfast (5.9%). Most of the hypoglycemic symptoms (92.1%) were resolved within 10 min, with low glucose levels returning to the safe range within 15 min in 86.8% of patients and returning to normal levels within 30 min in 9.5% of patients. In conclusion, the per person prevalence of hypoglycemia in six tertiary teaching hospitals in China was found to be 3.3, compared with a per person prevalence of 2 in a US study using the same definition of hypoglycemia. The differences in the values may be due to differences in monitoring of blood glucose levels. Herein, we used intensive bedside glucose testing, whereas the US study used retrospective chart review. Furthermore, our results indicate that hypoglycemic events are more likely to occur before lunch and at night. Similar findings have been reported in a study from the Mayo Clinic.
