Abstract:Recent advances in neuroscience have paved the way to innovative applications that cognitively augment and enhance humans in a variety of contexts. This paper aims at providing a snapshot of the current state of the art and a motivated forecast of the most likely developments in the next two decades. Firstly, we survey the main neuroscience technologies for both observing and influencing brain activity, which are necessary ingredients for human cognitive augmentation. We also compare and contrast such technologies, as their individual characteristics (e.g., spatio-temporal resolution, invasiveness, portability, energy requirements, and cost) influence their current and future role in human cognitive augmentation. Secondly, we chart the state of the art on neurotechnologies for human cognitive augmentation, keeping an eye both on the applications that already exist and those that are emerging or are likely to emerge in the next two decades. Particularly, we consider applications in the areas of communication, cognitive enhancement, memory, attention monitoring/enhancement, situation awareness and complex problem solving, and we look at what fraction of the population might benefit from such technologies and at the demands they impose in terms of user training. Thirdly, we briefly review the ethical issues associated with current neuroscience technologies. These are important because they may differentially influence both present and future research on (and adoption of) neurotechnologies for human cognitive augmentation: an inferior technology with no significant ethical issues may thrive while a superior technology causing widespread ethical concerns may end up being outlawed. Finally, based on the lessons learned in our analysis, using past trends and considering other related forecasts, we attempt to forecast the most likely future developments of neuroscience technology for human cognitive augmentation and provide informed recommendations for promising future research and exploitation avenues.

Abstract:The neurosciences are not unusual in science in having periodically been transformed by the advent of new techniques. This chapter highlights three examples of technologies that offer considerable promise for the future. Firstly, the widespread use of non-invasive human brain imaging has transformed cognitive neuroscience. Instead of relying on techniques from experimental psychology in the analysis of neurological patients, human brain imaging allows one to derive a spatial picture of brain activation in normal subjects as they undertake different tasks. Secondly, single-cell and multiple single-neuron recording technologies are advancing to the point where one can simultaneously record from large ensembles of cells, sometimes in different brain areas. In this way data can be obtained about how the firing of one or more cells may influence or be influenced by that of other cells. Finally, optical imaging provides a new window on the brain, enabling microscopic techniques to reveal the molecular dynamics of activity within individual neurons. A wide variety of optical techniques are available, with the most recent using two-photon confocal microscopy. These three techniques alone cannot transform neuroscience. Many other techniques are also very important, ranging from novel behavioral techniques through to targeted manipulations of individual genes.

Abstract:The future of neuroprosthesis and brain-computer interface (BCI) technologies is poised to transform medical treatments, cognitive development, and human-machine interaction. Neuroprosthetics, which integrate artificial devices with the nervous system to restore or enhance sensory and motor functions, have already improved the lives of people with disabilities. Advances in materials science and neural interfacing are taking these technologies even further, enabling more seamless integration and better control of prosthetic limbs and sensory devices. As these technologies improve, they may soon be able to restore complex functions such as speech, vision, and memory. At the same time, BCIs go beyond traditional applications for neurological disorders, opening up possibilities for cognitive enhancement, neural data transfer, and immersive virtual experiences. With advances in machine learning and neuroimaging, BCIs can enhance cognitive abilities, treat neurological diseases, and redefine human-computer interaction. However, these technologies also raise ethical, social, and security concerns. The risk of misuse, such as unauthorized access to neural data, and the potential creation of cognitive hierarchies emphasize the need for robust regulation. Ensuring accessibility and equity is also essential to ensure that the benefits of these advances are shared equitably. The development of neuroprostheses and BCIs has had a positive impact.

By merging biology, psychology, physics, and computer science, researchers are gaining profound insights into cognition, behaviour, and the neurological underpinnings of diseases. Brain mapping is a key component of recent advancements. Techniques like fMRI, PET, and DTI offer unprecedented views of brain structure and function. The Human Connectome Project and similar initiatives have produced detailed maps of brain connections, revealing how different regions interact to support cognition and behaviour. These maps are crucial for identifying disease biomarkers, predicting treatment responses, and developing targeted therapies. Molecular biology and genetics are also driving progress. Researchers are uncovering the genetic basis of neurological disorders, providing clues about disease susceptibility and progression. Molecular imaging techniques visualise neurotransmitter systems and cellular processes, shedding light on disease mechanisms. The integration of neuroscience with computer modelling and AI is revolutionising research. AI algorithms analyse vast datasets, simulate neural networks, and even decode neural signals for brain-machine interfaces. This has the potential for personalised medicine and ground-breaking treatments. The future holds immense promise. Techniques like optogenetics and single-cell imaging will offer even greater precision in studying brain circuits. However, we must address ethical considerations around data privacy, cognitive enhancement, and brain-altering interventions. Neuroscience is not just about understanding the brain; it's about improving lives. Researchers are striving to conquer neurological disorders and maximize human potential by pushing the boundaries of knowledge and technology while upholding ethical principles.

Abstract:Background: The rapid advancement of technology-focused strategies in neurorehabilitation has brought optimism to individuals with neurological disorders, caregivers, and physicians while reshaping medical practice and training. Objectives: We critically examine the implications of technology in neurorehabilitation, drawing on discussions from the 2021 and 2024 World Congress for NeuroRehabilitation. While acknowledging the value of technology, it highlights inherent limitations and ethical concerns, particularly regarding the potential overshadowing of humanistic approaches. The integration of technologies such as robotics, artificial intelligence, neuromodulation, and brain-computer interfaces enriches neurorehabilitation by offering interdisciplinary solutions. However, ethical considerations arise regarding the balance between compensation for deficits, accessibility of technologies, and their alignment with fundamental principles of care. Additionally, the pitfalls of relying solely on neuroimaging data are discussed, stressing the necessity for a more comprehensive understanding of individual variability and clinical skills in rehabilitation. Results: From a clinical perspective, the article advocates for realistic solutions that prioritize individual needs, quality of life, and social inclusion over technological allure. It underscores the importance of modesty and honesty in responding to expectations while emphasizing the uniqueness of each individual's experience. Moreover, it argues for the preservation of human-centric approaches alongside technological advancements, recognizing the invaluable role of clinical observation and human interaction in rehabilitation. Conclusion: Ultimately, the article calls for a balanced attitude that integrates both scientific and humanistic perspectives in neurorehabilitation. It highlights the symbiotic relationship between the sciences and humanities, advocating for philosophical questioning to guide the ethical implementation of new technologies and foster interdisciplinary dialogue.

Abstract:The complexity of the human brain creates a spectrum of sophisticated behavioral repertoires, such as language, tool use, self-awareness, symbolic thought, cultural learning, and consciousness. Understanding how the human brain achieves that has been a longstanding challenge for neuroscientists and may bring insights into the evolution of human cognition and disease states. Human pluripotent stem cells could differentiate into specialized cell types and tissues in vitro. From this pluripotent state, it is possible to generate models of the human brain, such as brain organoids. The recent observation that brain organoids can spontaneously develop complex neural network activity in a dish can help one understand how neural network oscillations evolve and vary between normal and disease states. Moreover, this finding can be leveraged to other applications outside medicine, including engineering and artificial intelligence. However, as the brain model technology becomes more complex, it raises a series of ethical and moral dilemmas. This article discusses the status of this technology, some of its current limitations, and a vision of the future.

Abstract:Today applications of neural prostheses that successfully help patients to increase their activities of daily living and participate in social life again are quite simple implants that yield definite tissue response and are well recognized as foreign body. Latest developments in genetic engineering, nanotechnologies and materials sciences have paved the way to new scenarios towards highly complex systems to interface the human nervous system. Combinations of neural cells with microimplants promise stable biohybrid interfaces. Nanotechnology opens the door to macromolecular landscapes on implants that mimic the biologic topology and surface interaction of biologic cells. Computer sciences dream of technical cognitive systems that act and react due to knowledge-based conclusion mechanisms to a changing or adaptive environment. Different sciences start to interact and discuss the synergies when methods and paradigms from biology, computer sciences and engineering, neurosciences, psychology will be combined. They envision the era of "converging technologies" to completely change the understanding of science and postulate a new vision of humans. In this chapter, these research lines will be discussed on some examples as well as the societal implications and ethical questions that arise from these new opportunities.

Abstract:Advanced brain machine interfaces provide potentially transformative approaches to treating neurological conditions and enhancing the performance of users. Yet, as technological capabilities continue to progress in leaps and bounds, there is a possibility that these capabilities outstrip our collective understanding of how to ensure brain machine interfaces are developed and used ethically and responsibly. In this case, there is an overt danger of rapid technological developments leading to unanticipated harm through a lack of foresight including threats to privacy, autonomy, self-identity, and other areas of personal and social value which, while hard to quantify, represent substantial risks. There is also a very real likelihood of such risks undermining value creation around the technologies and the associated enterprises, as key stakeholders push back against perceived and actual threats to what they, in turn, hold to be of value. In order to successfully traverse the resulting risk landscape, researchers and developers will need to become increasingly adept at integrating a sophisticated understanding of ethical and socially responsible innovation into their enterprises. Here, we illustrate how a "risk innovation" approach may provide novel insights into mapping out this landscape and revealing potentially blindsiding risks. We show how this approach can be used to illuminate challenges and opportunities to the successful, ethical, and responsible development of advanced brain machine interfaces. In addition, we emphasize how success will ultimately depend on the willingness of innovators and others to take ethical and responsible innovation seriously and to draw on the interdisciplinary and transdisciplinary expertise that is necessary to translate good intentions into positive outcomes.

Abstract:The pandemic is transforming neurology. Long COVID will linger, neurologic diseases will increase, and technology, artificial intelligence, and new virtual worlds will usher a new age of the brain and new roles for neurologists. The pandemic has compelled international collaboration, greatly increased communications, and accelerated drug and vaccines approvals. It also dramatized the close interconnection of cognitive, mental, and social health and their relevance to building back better health, education, work, and leisure. Brain health is the key to health, productivity, and well-being. Neurologists are best placed to lead brain knowledge integration and application through the unifying theme of brain health by becoming advocates, healers, and guardians of the brain.

Abstract:Experts from academia and industry discuss how to create a new, more effective translational neuroscience drawing on novel technology and recent discoveries. Today, translational neuroscience faces significant challenges. Available therapies to treat brain and nervous system disorders are extremely limited and dated, and further development has effectively ceased. Disinvestment by the private sector occurred just as promising new technologies in genomics, stem cell biology, and neuroscience emerged to offer new possibilities. In this volume, experts from both academia and industry discuss how novel technologies and reworked translation concepts can create a more effective translational neuroscience. The contributors consider such topics as using genomics and neuroscience for better diagnostics and biomarker identification; new approaches to disease based on stem cell technology and more careful use of animal models; and greater attention to human biology and what it will take to make new therapies available for clinical use. Abstract:Advances in large-scale single-unit human neurophysiology, single-cell RNA sequencing, spatial transcriptomics and long-term ex vivo tissue culture of surgically resected human brain tissue have provided an unprecedented opportunity to study human neuroscience. In this Perspective, we describe the development of these paradigms, including Neuropixels and recent brain-cell atlas efforts, and discuss how their convergence will further investigations into the cellular underpinnings of network-level activity in the human brain. Specifically, we introduce a workflow in which functionally mapped samples of human brain tissue resected during awake brain surgery can be cultured ex vivo for multi-modal cellular and functional profiling. We then explore how advances in human neuroscience will affect clinical practice, and conclude by discussing societal and ethical implications to consider. Potential findings from the field of human neuroscience will be vast, ranging from insights into human neurodiversity and evolution to providing cell-type-specific access to study and manipulate diseased circuits in pathology. This Perspective aims to provide a unifying framework for the field of human neuroscience as we welcome an exciting era for understanding the functional cytoarchitecture of the human brain.

There is great scientific effort being committed to understanding cognitive and brain functioning, with the aim of helping to promote healthy ageing and independence, and improve quality of life. This Cognitive Ageing Collection brings together cutting-edge research using a variety of methods and from diverse disciplinary perspectives, with example topics including cognitive strategies, genetic risk factors, and emotion regulation. Articles in the Collection highlight advances in our understanding of cognitive and brain health, and outline important directions for future research.

Abstract:Abstract In recent years, brain research has indisputably entered a new epoch, driven by substantial methodological advances and digitally enabled data integration and modelling at multiple scales— from molecules to the whole brain. Major advances are emerging at the intersection of neuroscience with technology and computing. This new science of the brain combines high-quality research, data integration across multiple scales, a new culture of multidisciplinary large-scale collaboration and translation into applications. As pioneered in Europe’s Human Brain Project (HBP), a systematic approach will be essential for meeting the coming decade’s pressing medical and technological challenges. The aims of this paper are to: develop a concept for the coming decade of digital brain research, discuss this new concept with the research community at large, to identify points of convergence, and derive therefrom scientific common goals; provide a scientific framework for the current and future development of EBRAINS, a research infrastructure resulting from the HBP’s work; inform and engage stakeholders, funding organisations and research institutions regarding future digital brain research; identify and address the transformational potential of comprehensive brain models for artificial intelligence, including machine learning and deep learning; outline a collaborative approach that integrates reflection, dialogues and societal engagement on ethical and societal opportunities and challenges as part of future neuroscience research.

Abstract:The human brain is one of the most mystifying biological structures in nature. Overwhelming research, technology, and innovations in neuroscience have augmented clinical assessments, diagnosis, and treatment capabilities. Nonetheless, there is still much to be discovered about nervous system disorders and defects. Neuralink, a neurotechnology company, is advancing the field of neuroscience and neuroengineering. The company's initial aim is to develop an implantable brain-machine interface device that will enhance the lives of people with severe brain and spinal cord injuries. Here, we provide insight into Neuralink's design, early testing, and future applications in neurosurgery. While early testing with small and large animals show promising results, no clinical trials have been conducted to date. Additionally, a term search for "Neuralink" was performed in PubMed. The literature search yielded only 28 references, of which most indirectly mentioned the device but not in direct testing. In order to conclude the safety and viability of the Neuralink device, further research studies are needed to move forward beyond speculation.

Abstract:As population aging advances at an increasing rate, efforts to help people maintain or improve cognitive function late in life are critical. Although some studies have shown promise, the question of whether cognitive training is an effective tool for improving general cognitive ability remains incompletely explored, and study results to date have been inconsistent. Most approaches to cognitive enhancement in older adults have taken a 'one size fits all' tack, as opposed to tailoring interventions to the specific needs of individuals. In this Perspective, we argue that modern technology has the potential to enable large-scale trials of public health interventions to enhance cognition in older adults in a personalized manner. Technology-based cognitive interventions that rely on closed-loop systems can be tailored to individuals in real time and have the potential for global testing, extending their reach to large and diverse populations of older adults. We propose that the future of cognitive enhancement in older adults will rely on harnessing new technologies in scientifically informed ways.

Abstract:As the Brain Research through Advancing Neurotechnologyies, better known as the BRAIN Initiative moves forward at a rapid pace increasing attention has focused on neuroethics. The National Institutes of Health recently mandated a review of the progress of the BRAIN Initiative progress with the goal of fine-tuning its future directions. The BRAIN Working Group 2 focused its discussion on science while the BRAIN Neuroethics Subgroup focused on neuroethics. The Brain Neuroethics Subgroup deliberated for over a year collecting information and recommendations through in person and video meetings, a public one-day neuroethics symposium and soliciting input from neuroscientists and neuroethicist's. The resulting report entitled "The BRAIN Initiative and Neuroethics: Enabling and Enhancing Neuroscience Advances for Society" was accepted by Director of NIH in October of 2019. The recommendations span many BRAIN research neuroethics concerns including privacy considerations, the use of nonhuman primate model systems, neural modulation and enhancement, subject participation in BRAIN research and equity in neuroscience research. Further the group recommended a transformative project whose goal of detailing the scientific mechanisms and ethical underpinnings of consciousness is one of the most daunting issues that impact our perceptions of ourselves. It is anticipated that the report?s recommendations will provide a foundation or "roadmap" for ensuring that neuroethics and BRAIN research move forward as an integrated effort thereby insuring that BRAIN research is of the highest quality.

Exploration of Neuroprotective Therapy

Abstract:More than 600 different neurological diseases affect the human population. Some of these are genetic and can emerge even before birth, and some are caused by defects, infections, trauma, degeneration, inflammation, and cancer. However, they all share disabilities caused by damage to the nervous system. In the last decades, the burden of almost all neurological disorders has increased in terms of absolute incidence, prevalence, and mortality, largely due to the population’s growth and aging. This represents a dangerous trend and should become our priority for the future. But what new goals are we going to set and reach now, and how will we exploit thought-provoking technological skills for making these goals feasible? Machine learning can be at the root of the problem. Indeed, most recently, there has been a push towards medical data analysis by machine learning, and a great improvement in the training capabilities particularly of artificial deep neural networks (DNNs) inspired by the biological neural networks characterizing the human brain. This has generated competitive results for applications such as biomolecular target and protein structure prediction, structure-based rational drug design, and repurposing, all exerting a major impact on neuroscience and human well-being. By approaching early risks for diseases, non-invasive diagnosis, personalized treatment assessment, drug discovery, and automated science, the machine learning arena has thus the potential of becoming the new frontier for empowering neuroscience research and clinical practice in the years ahead.
