A Clinical Analysis of 52 Cases Beyond 40-Week Gestation

Abstract:Objectives To determine the timing of delivery beneficial to mothers and neonates in pregnant women beyond 40 weeks gestation. Methods A randomized trial was carried on fifty-two normal pregnant women.Three groups were classified according to their gestational age.Fetal movement and non-stress test(NST) were monitored.Amniotic fluid index(AFI),maternal serum level of free E 3 and human placental lactogen(HPL) were measured.Histopathology exam was performed in 8 placentas. Results (1) AFI gradually decreased as the increase of gestational week,which was (8.2±3.5)cm, (7.3±4.2)cm and (6.6±1.9)cm in 40 weeks,41 weeks and 42 weeks respectively (P0.05).There was 32.1 percent(9/28) with AFI≤5 cm in week which was higher than that in the other groups,which was 23.1 %(3/13) in 40 weeks and 27.3 %(3/11) in 42 weeks.There were no significant differences on fetal distress and neonatal asphyxi among three groups(P0.05).(2)The serum level of E 3 decreased as gestational age increased and were significant difference among three groups (243.2±138.1) nmol/L,(166.7±82.2) nmol/L and (118.2±70.2) nmol/L(P0.01).There was no significant changes on HPL(P0.05).But an intimate relationship between AFI and perinatal outcome was observed.While AFI below 5 cm,more cases had fetal distress and neonatal asphyxia(4/15)compared with AFI in normal group (P0.05),but no significant change with E.3 and HPL. Conclusion AFI can be used as a reliable predicting for determineing pregnancy in cases of 40-week gestation.
