Abstract:Background:Salivary gland cancer (SGC) is relatively rare and constitutes a variety of histological subtypes. Previously published studies of SGC patients suggest that postoperative radiation using conventional radiotherapy (RT) or 3-dimensional (3D) conformal radiotherapy may have led to suboptimal oncological outcomes.Methods:We identified 60 patients with major SGC treated with surgery followed by postoperative intensity-modulated radiotherapy (IMRT). Data for overall survival (OS), progression-free survival (PFS), locoregional relapse-free survival (LRRFS), distant metastasis-free survival (DMFS), prognostic factors, and treatment-related toxicities were analyzed. Survival was analyzed using the Kaplan-Meier method and compared using the log-rank test.Results:With a median follow-up of 55.5 months, based on Kaplan-Meier analyses, the OS and PFS rates for SGC patients at 3, 5, and 10 years were 90.7%, 85.1%, and 85.1% and 80.1%, 72.7%, and 63.1%, respectively. The LRRFS and DMFS rates at 3, 5, and 10 years were 87.4%, 82.1%, and 82.1% and 85.3%, 78.4%, and 66.1%, respectively. In multivariable analysis (MVA), the node stage (N stage) was an independent predictor of PFS [P=0.047; hazard ratio (HR) =0.089]. A positive margin was a significant prognostic factor for PFS (P=0.036; HR =4.086), LRRFS (P=0.026; HR =5.064), and DMFS (P=0.011; HR =6.367). Major nerve involvement was significantly correlated with PFS (P=0.034; HR =2.394) and DMFS (P=0.008; HR =2.115). The interval from surgery to radiotherapy predicted PFS (P=0.036; HR =3.934) and DMFS (P=0.012; HR =6.231). Adenoid cystic carcinoma (ACC) was the most common histology (n=21; 35%). For ACC, the 5-year OS, PFS, LRRFS, and DMFS were 100%, 67.7%, 76.2%, and 90.2%, respectively. The most common acute toxicities were mucositis and dermatitis, and xerostomia was the most common late adverse event. Lung metastasis was the most common pattern of distant failure.Conclusions:N stage, positive margin, major nerve involvement, and interval from surgery to radiotherapy were important factors associated with PFS, LRRFS, and DMFS. Postoperative IMRT leads to improved survival for SGC patients, with acceptable toxicities.

Abstract:PurposeSpinal metastasis of malignant adrenal tumor (SMMAT) is an extremely rare and poorly understood malignant tumor originating from the adrenal gland. The objective of this study is to elucidate the clinical characteristics and discuss surgical management and outcomes of SMMAT. MethodsIncluded in this study were six SMMAT patients who received surgical treatment in our center between February 2013 and May 2022. Their clinical data and outcomes were retrospectively analyzed to gain a better understanding of SMMAT. In addition, ten cases from the literature focusing on SMMAT were also reviewed. ResultsSurgery was performed successfully, and the associated symptoms were relieved significantly in all patients postoperatively. The mean follow-up duration was 26.2 (range 3-55) months. Two patients died of tumor recurrence 12 and 48 months after operation respectively. The other four patients were alive at the last follow-up. ConclusionsThe prognosis of SMMAT is usually poor. Preoperative embolization and early surgical radical resection can offer satisfactory clinical outcomes. The patient's health status, preoperative neurological function, tumor location and the resection mode are potential prognostic factors of SMMAT.

Abstract:The most common of age distribution ranges from second to fourth decade of life and patients with giant cell tumors (GCT) aged less than 18 years is more uncommon. We are aiming to reveal what the conservative nerve-sparing surgery values for adolescent patients with sacral GCT. We retrospectively reviewed 15 adolescent patients with sacral GCT aged <eighteen years, who received the conservative nerve- sparing surgery at our center from 2007 to 2018. Four patients presented with tumor of Campanacci grade II and 11 patients with grade III. According to the location of tumor at the sacrum, we classified the surgical resection of sacral giant cell tumor into three types. In our adolescent cohort, nine had the aortic balloon occlusion and five received the selective arterial embolization. All patients were free of disease and no one had the pulmonary metastasis or died of disease. All five wound complications required the debridement. 6 out of 15 patients had the local recurrence that required surgery treatment. The average time to recurrence after initial surgery in the present adolescent cohort was 22.4 months. Two patients had the surgical site infection and three had the wound dehiscence. One had the femoral artery thrombosis due to the aortic balloon occlusion and received the surgical removal of thrombosis. With the help of preoperative selective embolization and intraoperative aortic balloon occlusion, adolescent patients undergoing conservative nerve-sparing surgery for giant cell tumor of sacrum based on the proposed surgical resection classification have an acceptable clinical outcome and neurologic function. Clinical feature and surgical management of pediatric sacral tumors

Abstract:Objective Sacral tumors in children are very rare.The purpose of this research is to investigate and discuss the clinical characteristics, pathological types, therapeutic process and prognosis of such lesions.Methods From Jan.2000 to Dec.2013, the study retrospected 20 patients (≤14 years) who were diagnosed as sacral tumors.Each patient's hospital records, radiographic reports and pathological results were reviewed.The pathological types of these lesions were Ewing sarcoma/primitive neuroectodermal tumor (PNET, n=7), osteosarcoma (n=3), osteoblastoma (n=2), hemangioma (n=2), teratoma (n=1), chondroblastoma (n=1), schwannoma (n=1), aneurysmal bone cyst (n=1), eosinophilic granuloma (n=1) and hemangioendothelioma (n=1).Seven patients had sacral nerve impairment as initial symptoms and the rest 13 complained with sacrococcygeal pain.Eleven patients presented with presacral mass during rectal examination.All 20 patients received surgical treatment.The survival rate was described with Kaplan-Meier survival curve.Results Thirteen patients with sacrococcygeal pain had their symptoms all relieved.Three of the 7 patients with sacral nerve impairment experienced improvement of the symptoms while 3 patients found no much change.One patient found the symptom deteriorated and suffered ambulation dysfunction postoperatively.The mean 2-year survival rate was 70.6％, 5-year survival rate 64.7％.Conclusions Sacral tumors in children are very rare, and most of them are primary tumors like Ewing sarcoma/PNET or osteosarcoma, which are usually highly malignant.Once a child is suspected with sacral tumors, prompt radiological and pathological examinations should be performed and therapeutic strategy should be made in order to improve the survival and preserve the sacral nerve function as much as possible.

Abstract:Background: Primary benign osseous tumors and tumor-like lesions at the sacrum are rare in the pediatric population and exact surgical strategy is still unclear. In this study, we evaluate the outcome for pediatric patients with benign tumors and tumor-like lesions at the sacrum who were receiving surgical treatment according to our proposed surgical strategy and classification. Methods: We analyzed 49 pediatric patients with sacral benign tumors or tumor-like lesions aged 18 years and below from 2005 to 2018. There were 23 men and 26 women with a mean age of 14.0 +/- 3.8 years. Nineteen patients had giant cell tumors (GCTs), 9 aneurysmal bone cysts, 5 osteoblastomas, 5 neurogenic tumors, 3 hemangiomas, 3 teratomas, 2 Langerhans cell histiocytosis, 1 chondroblastoma, 1 fibrous dysplasia, and 1 GCT of tendon sheath. We proposed our surgical plan and surgical classification for pediatric patients with sacral benign tumors or tumor-like lesions. Results: The mean follow-up duration was 6.2 years (range, 1.0 to 18.9 y). GCTs (39%, 19/49) and primary aneurysmal bone cysts (18%, 9/49) are the top 2 common histologic types. Preoperative selective arterial embolization (SAE) was performed in 12 cases and 24 patients received intraoperative aortic balloon occlusion (ABO) as the preoperative surgical plan. Furthermore, according to tumor location at the sacrum, we classified surgical excision of sacral benign tumors and tumor-like lesions into 3 types. Fourteen cases were classified as type I, 27 as type II, 3 as type III, and 5 patients with neurogenic tumors cannot be classified into this surgical classification. Ten patients had wound complications. Two had femoral artery thrombosis because of ABO application. One had mechanical failure. Rate of local recurrence was 16%. Seven patients with GCTs and 1 with neurogenic tumor had local recurrence. No patient died of disease at the last follow-up. For the assessment of neurological function, the rate of neurological dysfunction was 12% (6/49). Four cases had urinary incontinence, 3 fecal incontinence, and 3 had bowel obstruction. Next, univariate analysis for influence of preoperative SAE and intraoperative ABO on complications demonstrated that both of them exerted no significant influence on the occurrence of oncological and nononcological complications. Conclusions: The proposed surgical strategy can provide an excellent therapeutic effect for pediatric benign tumors and tumor-like lesions at the sacrum. Preoperative SAE and intraoperative ABO can safeguard pediatric patients with high vascularity of benign tumor at the sacrum during the operation.
Surgical treatment of primary sacral tumor

Abstract:Objective:To summarize the surgical outcome for primary sacral tumors.Method:26 cases(including 17 males and 9 females) with sacral tumor from January 1991 to August 2009,underwent surgical protocol.The mean age at admission was 50 years(range,17-71 years).The clinical presentation,imaging and needle biopsy were used to determine the diagnoses.Combined anterior-posterior approach(APA) was used in 15 patients with tumors involving S2 and above.Among them,balloon dilation catheter(BDC) was used in 9 cases with huge malignant tumors.Single posterior approach(PA) was used in 11 patients with tumors involving vertebrae below S2.Bilateral S1 and S2 nerve roots and at least unilateral S3 nerve root were preserved in the surgery.Lumboiliac fixation was performed in 9 patients due to the removal of S1.Result:All patients underwent the operations successfully with no perioperative death or serious complications.The blood loss in PA group ranged from 200ml to 5000ml(mean,1995±2076ml),whereas that in APA group ranged from 400ml to 6300ml(mean,2400±1678ml),and the mean blood loss for BDC was 2333ml.Skin incision complications were noted in 11 cases including non-healing in 6 cases,infection in 4 cases,skin necrosis in 2 cases and sinus formation in 1 case.Two of them healed after skin flap transplantation and the others healed through debridement and dressing change.11 cases presented with neurological deterioration at the early following up,including weakness and numbness of lower extremity,saddle numbness and bladder and bowel dysfunction.Of them,8 cases recovered completely and 3 cases recovered incompletely after an average of 27 months follow up.One patient with malignant peripheral nerve sheath tumors died of multiple organ failure 11 months later.9 cases were noted to have local recurrence including 5 chordoma,3 malignant peripheral nerve sheath tumors and 1 nerve sheath tumors.The primary local recurrence was dated at 12.2 months on average.At final followup,17 patients recovered routine life and normal cauda equina function.Conclusion:CT-guided biopsy is valuable.We recommend single posterior approach for tumors involving vertebrae below S2 and combined anterior-posterior approach for tumors involving S2 and above,BDC is helpful to blood control.Bilateral S1 and S2 nerve roots and at least unilateral S3 nerve root should be preserved to improve life quality.
Treatment outcome for patients with primary NSCLC and synchronous solitary metastasis

Abstract:Introduction Non-small cell lung cancer (NSCLC) patients with synchronous solitary metastasis were generally considered as stage IV and believed to be incurable. Recently, growing evidence has indicated that surgical treatment may provide these patients with a survival benefit. The aim of this study was to retrospectively analyze the effectiveness of different treatments for primary tumors and solitary metastases. Materials and methods Patients older than 18 years with histologically confirmed stage IV NSCLC and a confirmed synchronous solitary metastasis that diagnosed within 2 months of primary NSCLC. Patients with uncontrolled massive pleural effusion were excluded. Between February 2002 and October 2010, 213 patients were considered eligible and enrolled in this cohort. Results The median survival time (MST) for the 213 patients was 12.6 months. Forty-five patients received primary pulmonary tumor surgery in the entire cohort. The MSTs of patients who received primary tumor resection and those who did not were 31.8 and 11.4 months ( p < 0.01). The MST of the patients with solitary brain metastasis was 12.3 months. Forty-one patients who received brain surgical treatment or SRS had a MST of 15.4 months and others who only received WBRT had a MST of 11.5 months ( p = 0.002). Gender, the stage of the primary tumor, PS and whether the primary tumor was removed all affected prognosis independently. Conclusions Aggressive local and metastasis treatments could lead to better clinical outcomes and thus provide an option for clinicians in the future management of patients with NSCLC and synchronous solitary metastasis.

Abstract:Study Design. Twenty-six patients were treated surgically for sacral osteosarcomas in our center. The oncological and functional results are analyzed.Objective. To describe the general characteristics and evaluate the outcome of surgical treatment of primary osteosarcoma of the sacrum.Summary of Background Data. A very few reports have addressed the technique and outcome of surgical treatment of primary sacral osteosarcoma. The survival rate of such cases is low.Methods. The clinical data of 26 patients treated surgically for sacral osteosarcomas from June 2000 to December 2013 at our hospital were retrospectively reviewed.Results. There were 15 males and 11 females with a median age of 28 years (range, 12-68 years). Distal metastasis occurred in 13 patients (50%), and local recurrence occurred in 10 patients (38.5%, including 6 patients with additional distal metastasis). The 1-year and 5-year survival rates were 92.3% and 38.7%, respectively. Adequate and inadequate surgical margins were obtained in 16 and 10 cases, respectively. Patients with adequate margins had a significantly lower recurrence rate (P = 0.015) and higher event-free survival rate (P = 0.04) than those with inadequate margins. However, the overall survival rate did not differ significantly between patients with adequate and inadequate margins (P = 0.22). Only seven cases showed a good response based on necrosis rate (>90%). Patients with a better response to chemotherapy had better event-free and overall survival rates.Conclusion. Adequate surgical margins are achievable in most cases of sacral osteosarcomas. Adequate margins can significantly improve the recurrence rate and event-free survival rate compared with inadequate margins. However, sacrectomy is still a challenging surgical technique. Adequate margins are also difficult to obtain in sacrectomy. In addition, only a few patients had a good response to chemotherapy. These reasons lead to a low 5-year survival rate in primary sacral osteosarcoma patients.

Of malignant tumors, chordoma was the most common malignant tumor (316 cases, 22.8% of all tumors), followed by chondrosarcoma, myeloma and other histologic types. The most common histological type of benign tumors was giant cell tumor accounting for 14.8% (205 cases) of all tumors, followed by neurofibroma, schwannoma and other types. The most common age group affected by malignant bone tumors was the 51- to 60-year-old group, followed by the 41- to 50-year-old group. The most commonly affected age group for benign tumors and tumor-like lesions was the 31- to 50-year-old group, followed by the 21- to 30-year old group. Furthermore, the following histologic types had the gender predilection. Chordoma, chondrosarcoma, myeloma and osteosarcoma affected more frequently males than females. Malignant peripheral nerve sheath tumor, lymphoma, giant cell tumor, neurofibroma, tuberculosis, teratoma and epidermoid cyst more frequently affected females than males.Conclusions: The large cohort of sacral tumors and tumor-like lesions in our database may reveal their clinical characteristics of age, gender, histologic type and anatomic site in China and features of sacral tumors and tumor-like lesions is fairly distinct from the mobile spine and extremities.

The Outcome of En Bloc Resection of Primary Sacral Tumors.

Abstract:Objective To investigate the outcome of En bloc resection of primary sacral tumors.Methods 27 cases of primary sacral tumors patients had been treated by En bloc resection between January 1998 to June 2004,the records of them were retrospectively reviewed.Results In this group of 27 patients,16 patients were free from disease with an average follow-up time of 49 months.The other 11 cases was observed local recurrence or widespread metastasis by radiograph or MRI,and the median follow-up period of no evidence of recurrence or metastasis was 36.7 months.No patient died on the complication around the surgery.23 patients whose sacral nerve roots had been reserved at and above S2 level,the sphincter muscle function of bladder and bowl were good or recovered by 3~6 months' rehabilitation exercise.4 patients experienced severe bladder dysfunction and moderate or severe bowel dysfunction.Conclusion En bloc resection of tumor is an effective way to manage primary sacral tumors.It could minimize the recurrence rate of sacral tumors and increase the patients' life span of free from disease.

Abstract:OBJECTIVE:To investigate the way of sacral tumors surgical treatment.METHODS:This retrospective study included 119 cases of sacral tumors surgically treated from July, 1996 to December, 2001. The age of patients ranged from 18 to 80 years (mean 57 years), including of 72 male and 47 female. Out of the patients, there were 52 chordomas, 16 giant cell tumor, 5 neurofibroma, 23 metastases tumors, 9 myeloma, 2 osteoblastomas, 5 aneurysmal bone cysts, 3 osteosarcomas, 4 chondrosarcomas. Posterior approach and combined anterior-posterior approach were used in 83 and 36 cases respectively. Twenty-nine patients had received surgical management at least once and 16 of them had received radiation therapy before came to our department.RESULTS:Three patients died on the complication around the surgery. Most of the patients with metastases tumor or multiple myeloma died 1 to 3 years after the surgery. Out of three osteosarcoma patients, 2 died and one alive with tumor. Three chondrosarcoma patients died, and one alive with tumor. Out of 52 chordoma patients, 3 patients had died of metastatic chordoma, 3 patients died of many times recurrence. Among the other 46 patients who were stay alive, 31 were free from disease with average follow-up time of 42 months. In the patients whose sacral nerve roots had been reserved bilaterally at and above S(3) level, the sphincter muscle function of bladder and bowl was good. While the function of sphincter muscle impaired in 2 patients with nerve roots reserved only at and above S(1) level. To manage these 2 patients, indwelling bladder catheters were used, but colostomy had not been performed.CONCLUSIONS:Complete resection of tumor (radical surgery when possible) is the most effective way to manage sacral tumors. Postoperative adjuvant radiation therapy can reduce the tumor recurrence rate, but it also can cause troubles that would hinder further surgical managements. Even if the tumor is relatively huge and the upper resection margin is as high as at S(1) or S(2) level, the tumor can be removed successfully by posterior approach and the postoperative complications could be accepted. To the patients with aneurysmal cyst or giant cell tumor on sacrum, for control bleeding purpose, anterior approach should be performed to ligate the bilateral internal iliac artery.
Surgical and Oncologic Outcome following Sacrectomy for Primary Malignant Bone Tumors and Locally Recurrent Rectal Cancer

Abstract:Introduction: Bone sarcoma or direct pelvic carcinoma invasion of the sacrum represent indications for partial or total sacrectomy. The aim was to describe the oncosurgical management and complication profile and to analyze our own outcome results following sacrectomy. Methods: In a retrospective analysis, 27 patients (n = 8/10/9 sarcoma/chordoma/locally recurrent rectal cancer (LRRC)) were included. There was total sacrectomy in 9 (incl. combined L5 en bloc spondylectomy in 2), partial in 10 and hemisacrectomy in 8 patients. In 12 patients, resection was navigation-assisted. For reconstruction, an omentoplasty, VRAM-flap or spinopelvic fixation was performed in 20, 10 and 13 patients, respectively. Results: With a median follow-up (FU) of 15 months, the FU rate was 93%. R0-resection was seen in 81.5% (no significant difference using navigation), and 81.5% of patients suffered from one or more minor-to-moderate complications (especially wound-healing disorders/infection). The median overall survival was 70 months. Local recurrence occurred in 20%, while 44% developed metastases and five patients died of disease. Conclusion: Resection of sacral tumors is challenging and associated with a high complication profile. Interdisciplinary cooperation with visceral/vascular and plastic surgery is essential. In chordoma patients, systemic tumor control is favorable compared to LRRC and sarcomas. Navigation offers gain in intraoperative orientation, even if there currently seems to be no oncological benefit. Complete surgical resection offers long-term survival to patients undergoing sacrectomy for a variety of complex diseases.

Abstract:Purpose: To investigate the indications, approaches, resection methods, and complications of total sacrectomy with a combined antero-posterior approach for malignant sacral tumours. Methods: Fourteen cases of primary malignant sacral tumours treated with total sacrectomy between January 2012 and 2018 were retrospectively analysed. All patients presented with pre-operative lumbosacral pain or constipation. A combined antero-posterior approach was used for tumour resection, and the spinal pedicle screw rod system was used to achieve ilio-lumbar stability. The visual analogue scale (VAS) and Musculoskeletal Tumor Society (MSTS) scores were used to assess pain and lower limb function, respectively. The mean operative time and intra-operative blood loss were 6.54 hours and 2935 mL, respectively. The mean follow-up period was 62 months. Results: None of the patients died peri-operatively. At the last follow-up, ten patients were continuously disease-free, three were alive with disease, and one died of disease from lung metastasis. Tumour recurrence occurred in three patients. The MSTS scores ranged from 6 to 28 (20.00-93.33%, 6/30-28/30) with an average of 20 (66.67%, 20/30). Seven patients could walk independently in public, five could only walk at home using a walking aid, and two could only lie down and stand for a short time. Thirteen patients developed post-operative complications such as skin necrosis, screw loosening, connecting rod fracture, neuropathic pain, sciatic nerve injury, dysuria, and urinary incontinence. Conclusion: Total sacrectomy can effectively treat malignant sacral tumours with good resection boundaries and prognosis. However, the high incidence of post-operative complications may impact post-operative neurological function.
Adult Sacrococcygeal Teratoma: a Retrospective Study over Eight Years at a Single Institution

Abstract:ObjectiveTo determine the clinical, imaging, and histological features, and surgical resection modalities and outcomes of adult sacrococcygeal teratoma (SCT).MethodsAdult patients with histopathologically diagnosed SCT were enrolled in our hospital between August 2010 and August 2018. Each patient's characteristics and clinical information were reviewed.ResultsThere were 8 patients in the study (2 males, 6 females) with a median age of 34 years (range, 18-67 years). The time to clinical symptoms was 14 d to 35 years, with a median time of 4 years. Six patients presented with symptoms of sacrococcygeal pain, and four with signs of sacrococcygeal mass and ulceration in the sacrococcygeal region. Six patients were evaluated using a combination of computed tomography (CT) and magnetic resonance imaging (MRI). All patients showed a presacral tumor with heterogeneous intensity on CT images. All patients underwent surgical treatment, including 6 parasacral, 1 transabdominal, and 1 combined anterior-posterior surgery cases. Seven patients were histopathologically diagnosed with benign mature SCT, and have shown no recurrence. One patient had malignant SCT, with recurrence at 84 months after surgery. After a second surgery, the patient had no recurrence within 6 months follow-up after re-resection.ConclusionsOur retrospective study demonstrated: (1) adult SCT is difficult to diagnose because of a lack of typical clinical symptoms and signs; (2) a combination of CT and MRI examination is beneficial for preoperative diagnosis; (3) the choice of surgical approach and surgical resection modality depends on the size, location, and components of the tumor, which can be defined from preoperative CT and MRI evaluation; (4) most adult SCTs are benign; the surgical outcome for the malignant SCT patient was good after complete resection. Even for the patient with recurrent malignant SCT, the surgical outcome was good after re-resection.

Abstract:Objective:To evaluate the results and complications of surgical treatments of metastatic sacral tumor.Method:Between December 2000 and June 2006,sixty-one patients with metastatic sacral tumors treated in our department were studied retrospectively.Fifty-seven patients had pain at sacral region and/or lower limbs,of which thirty-two patients felt pain aggravated while ambulating.There were seventeen patients with muscle weakness of lower limbs,and twelve patients with abnormality of urination or defecation.All patients underwent surgical treatment.Marginal resection was performed in two patients with tumors below the third sacrum.Thirty-five patients with sacral-iliac joint instability received the reconstruction of pelvic ring integrity by rod and screw system,and twenty-four patients without sacral-iliac joint involvement received cytoreductive surgery and nerve root release.The status of pain of patient was recorded according to numeric rating scale.The main evaluation points included neural function,urination or defecation function status and surgical complication.Result:One patient with severe bleeding during operation died after surgery because of multiple organs failure.Six patients received débridement of wound incision because of non-healing of the wound,of which two patients received the débridement for second time,and all the six patients got wound healing when they were discharged.Neural injury happened in three cases which were not recovered during follow-up,and these patients can only walk with crutch.Cerebral-spinal fluid leakage happened in four cases,the drainage decreased gradually and patients recovered after they stayed in bed strictly for a few days.Follow-up period ranged from 6 months to 72 months with a mean follow-up time of 35 months.According to numeric rating scale,the average score of pain before and after surgery was 7.4 and 2.4 respectively.There was significant difference between the two scores(P<0.05).Eight patients recovered normal urination or defecation function three months after surgery,three patients got partial recovery and one patient became worse.Strength of lower limb was significantly recovered in thirteen patients.Twenty-seven patients did not experience the pain while walking after the stability of sacro-illic joint was established.Conclusion:For the patients with metastatic sacral tumor,surgical treatments can improve the life quality of the patients by relieving the pain,restoring the neural function and regaining the stability of sacral-iliac joint.

Abstract:BACKGROUND AND OBJECTIVES:Few reports have investigated patient experiences following total en bloc sacrectomy. The aims of this study were to obtain a deeper understanding of patients' personal experiences, needs, and satisfaction with the treatment to reveal areas in which perioperative and long-term patient care can be improved.METHODS:A qualitative design was applied to examine patient experiences and supportive care needs. Patients treated between 2007 and 2017 were identified from our institutional database.RESULTS:A total of 28 survivors were interviewed (13 females, age 13-75 years). Eight themes were identified: the effect of surgery on patients' (a) daily lives, (b) social activities, (c) work or school activities, (d) and family lives; (e) acceptance of ostomy surgery; (f) need for guidance regarding long-term rehabilitation; (g) satisfaction with the medical services provided in the hospital; and (h) satisfaction with the treatment outcomes.CONCLUSION:Total en bloc sacrectomy can yield satisfactory oncological outcomes; however, the procedure is a life-changing event for patients and their families. Physicians must provide long-term support and guidance after surgery to enable patients to fully understand and cope with the changes in their lives.
Clinical Analysis of Primary Presacral Tumors: a Report of 147 Cases

Abstract:The objective of this study was to analyse retrospectively the clinical and pathologival data of 147 patients with primary presacral tumor from 1954 to 2004.The results showed: that the main diagnosis approaches of the primary presacral tumors were anoscopy, ultrasonography (USG) and computed tomographic(CT). The pathological types of primary presacral tumors were complex. As to the data, there were 30 different pathological types revolved, and among them, 49.7% were benign and 50.3% malignant respectively.The main therapy was surgical resection. In the data, the tumor was completely resected in 77 cases;partially resected in 32 cases; secondary operation was completed in 18 cases. A total of 78 cases were treated by surgery combined with radiotherapy, chemotherapy or immunotherapy.The effect of integrative therapy was depended on the histological type, the volume of the tumor and so on. The life span of the patients were from 3 months to 14 years and the median survival term was 2 years. The main influencing factors were the histological type, the resected rate of the tumor, the application of integrative therapy.
Total sacrectomy for primary multilevel malignant sacral tumors

Abstract:Objective:To investigate the surgical approach and indication of total sacrectomy for primary multilevel malignant sacral tumors.Method:Between July 2007 and July 2009,6 patients with malignant sacral tumors received one-stage total sacrectomy by a combined anterior-posterior approach and rod-screw reconstruction in our center.There were 4 males and 2 females,with the average age of 38.8 years(range,20-59 years).S1-S4,S1-S3 and L5-S4 were involved in 4,1 and 1 patient respectively.Histological diagnoses were chordoma(2),chondrosarcoma(2),malignant peripheral nerve sheath tumor(1)and Ewing sarcoma(1).Result:The mean operation time was 12 hours(range,11-13 hours),with the mean intraoperative blood loss of 4500ml(range,3800-4500ml).There was no perioperative death and severe complications such as visceral leakage in all 6 patients.Bilateral S1 nerve roots were sacrificed in 6 patients and plantar flexion was not noted sequentially.Lumbosacral stability was reconstructed using rod-screw system in all cases.All patients were allowed to ambulate with the aid of crutches 6 weeks after operation.The mean follow-up was 10.3 months(range,3-25 months).No local recurrence or metastasis was found at final follow up.Conclusion:One-stage total sacrectomy with combined anterior-posterior approach can ensure satisfied oncological margin,however the indication should be strictly limited due to its great surgical challenge.
