Abstract:Background: Cancer not only affects patients but also their caregivers. The objective of the current study was to assess the unmet needs of cancer caregivers and to identify possible predictors of their supportive care needs. Methods: In a cross-sectional survey, 188 dyads of patients diagnosed with lung, urological, or gastrointestinal cancer and their primary caregivers were recruited. Caregivers were asked to complete the Supportive Care Needs Survey self-report questionnaire (for partners and caregivers); patients completed the corresponding questionnaire. Both groups provided information regarding their distress (National Comprehensive Cancer Network Distress Thermometer), anxiety, and depression (Patient Health Questionnaire-4). Clinical characteristics were obtained from medical records. Results: The mean age of the caregivers was 57.8 years. Approximately 72.3% were female. Patients had an average age of 62.5 years, with 33.0% being male. Caregivers were more distressed (P<.01) and exhibited higher anxiety scores (P<.01) compared with patients. Approximately 14.4% of caregivers reported no unmet need and 43.6% had at least 10 needs that were unmet. Main caregiver concerns were regarding health care service and information needs followed by emotional and psychological needs. To some degree, unmet needs in patients and caregivers' anxiety predicted unmet caregiver needs. Sociodemographic and clinical variables were not found to be significant predictors. Conclusions: A substantial percentage of caregivers have unmet needs for support, mainly with regard to fears concerning the patient's condition, receiving disease-related information, and emotional support for themselves. Prediction of unmet needs in caregivers from other clinical and psychological variables was rather poor. Therefore, by means of the frequency and disparity of caregivers unmet needs, they should be systematically assessed to direct specific offers.

Abstract:Abstract The affect of Covid -19 and changes in healthcare has shifted most of cancer care of elderly to home settings. Calcutta Metropolitan Institute of Gerontology (CMIG), Regional Resource and Training Centre in Ageing studied the daily tasks, requirements and emotional burden of caregivers in West Bengal, India. Caregivers of lung cancer, liver cancer and colorectal cancer patients (n=500) completed questionnaires provided by CMIG. The mean age of cancer patients was 72.5 years. CMIG found that 47% of the caregivers were the spouses of elderly patients, 33% were their adult children and 20% were other family members. Forty six percent of the caregivers were found to care for patients with metastatic disease, 33% cared for co morbidities, 63% cared for those undergoing chemotherapy or radiotherapy. Ninety eight percent of the caregivers provided assistance with Activities of Daily Living, 75% in administering medicine, 47% helped in pain management, 39% helped in hospital visits or deciding to call doctor. Twenty one percent of the caregivers reported poor health and 16% were not confident of the quality of care they provided. Sixty six percent reported suffering from stress and perceived cancer as a family disease, 45% thought they lacked social support. Of the adult children who provided care, 78% resported they needed more training to provide quality cancer care and transitioning to formal care would improve results.This study will help influence State policies for allocation of resources for training for cancer caregivers, build support groups for stress management and create cancer care helplines.
A Way Forward for Comprehensive Cancer Caregiver Support

Abstract:Family caregivers are integral to patient care. However, a combination of systemic forces places enormous pressure on family caregivers, while simultaneously devaluing them. Recently, more public attention has been paid to caregivers' importance, prevalence, and needs, generating supportive responses by government, employers, and the media. As of yet, there has not been a commensurate response by health care institutions. We identify four key challenges to building comprehensive cancer caregiver support and propose five necessary components for future programs that cancer centers and organizations can adopt. Comprehensive cancer caregiver support is attainable but national organizations need to lead the effort through standardization of guidelines and metrics for cancer centers.

Abstract:As the number of people with cancer increases, so does the number of informal caregivers. These caregivers frequently have multiple unmet needs and experience numerous burdens. Here we explore the crucial roles of these caregivers and categorize their unmet needs into four areas: information, relationship and communication, emotional support, and practical or financial needs. We provide evidence on emerging interventions aimed at supporting caregivers, including patient/caregiver assessments, education, collaborative care, financial assistance, wellness, informational programs, and an integrated caregiver clinic. Finally, we delve into the vital role that patient advocacy groups play in addressing the unmet needs of cancer patients and their caregivers by providing comprehensive support, including education, resources, counseling, guidance, and financial aid.

Abstract:Internationally, family members or spouse caregivers are critical in maintaining and improving the quality of life of individuals living with cancer. By providing about 70%–80% of patients’ cancer care, informal caregivers reduce the demands on the health‐care system, with significant economic benefits. Despite their value to society and economy, caregivers remain largely a hidden workforce, taking on their important roles and responsibilities with little to no formal training. This lack of formal training leads to high levels of burden and lower quality of life for not only the caregiver but also the person they are caring for. Cancer caregivers have been found to report higher burden than caregivers for individuals with diabetes or frail elders, highlighting their particular vulnerability. Caregivers’ Unmet Needs The responsibility that caregivers take on has, in turn, prompted in‐depth documentation into the kind of support they need, which is also referred to as unmet supportive care needs. Unmet needs assessments can inform intervention development and the prioritization of resources to address key deficiencies in services. A recent systematic review we completed found that 16%–68% of caregivers report unmet supportive care needs, with an average of 1.3–16 unmet needs (possible maximum range = 17–67) identified. Such rates of unmet supportive care needs exceed those reported by the patients. Caregivers in the acute postdiagnosis phase or advanced phase of the illness

Cancer Caregiver Anxiety Over Time: The Influence of Palliative Care

Abstract:Objectives: To explore anxiety experienced by caregivers providing home-based, end-of-life care to patients with cancer. We examined the relationship between caregiver anxiety and receipt of palliative care by the patient. Methods: A case series of terminal cancer patient-caregiver dyads (n = 223) were recruited from oncology clinics in Virginia and Pennsylvania and followed for 12 months or until patient death. Data collected included qualitative, quantitative, and observational data; this analysis utilizes the quantitative data. Longitudinal Latent Growth Models were used to characterize the heterogeneity of primary caregiver anxiety over time. The influence of palliative care on caregiver anxiety over time was assessed. Characteristics associated with membership in the trajectory groups rendered from those models are presented. Results: The majority of caregivers were female (73.9%), white (54.9%), and patient spouses (45.3%). Three classes of caregivers were identified based on their anxiety scores over time (low, elevated, and high). The 2 groups who had elevated and high anxiety had significant increases in anxiety over time. Controlling for patient receipt of palliative care attenuated those increases. Caregivers with the lowest level of anxiety were more likely to be Black, report fewer symptoms of depression or caregiver burden and higher self-rated physical health. Caregivers who were younger reported higher anxiety. Conclusions: Our analysis detected 3 distinct cancer caregiver groups reporting low, elevated, and very high levels of anxiety. Caregivers with elevated or high anxiety also demonstrated increasing anxiety overtime; however increases were attenuated with patient receipt of palliative care. For cancer caregivers with elevated and high levels of anxiety, palliative care buffers further deterioration of their mental health.

Abstract:Cancer caregiving, a critical component in the cancer-care model, has deleterious effects on the caregiver's physical and mental health. The degree to which these negative effects are uniformly experienced by caregivers is unclear; effects may be exacerbated at the end of life when caregiving is intensified. Not all caregivers have the support of an additional involved support person (secondary caregiver). The impact of the secondary caregiver's absence on the primary caregiver's well-being is understudied.

Impact of informal cancer caregiving across the cancer experience: A systematic literature review of quality of life

Abstract:Objective: Informal caregiving may likely increase as the number of cancer survivors grows. Caregiving responsibilities can impact caregivers' quality of life (QOL). Understanding the current state of the science regarding caregiving QOL could help inform future research and intervention development. Methods: A systematic literature review in PubMed/Medline examined research on QOL among informal cancer caregivers and related psychosocial health outcomes. Original research articles in English, published between 2007 and 2017 about caregivers (aged >18 years) of adult cancer patients in the United States were included. Abstracted articles were categorized according to caregiving recipient's phase of survivorship (acute, middle to long-term, end of life/bereavement). Results: Of 920 articles abstracted, 60 met inclusion criteria. Mean caregiver age ranged from 37 to 68 with the majority being female, non-Hispanic white, with at least a high school degree, and middle income. Almost half of the studies focused on caregivers who provided care for survivors from diagnosis through the end of active treatment. Studies examined physical health, spirituality, psychological distress, and social support. Differences in QOL were noted by caregiver age, sex, and employment status. Significance of results: Additional research includes the examination of the needs of diverse cancer caregivers and determines how additional caregiver characteristics (e.g., physical functioning, financial burden, etc.) affect QOL. This includes studies examining caregiver QOL in the phases following the cessation of active treatment and assessments of health systems, support services, and insurance to determine barriers and facilitators needed to meet the immediate and long-term needs of cancer caregivers.

Abstract:82 Background: Caregivers are essential members of the health team, often assuming diverse primary care roles for the cancer patient. Caregiver needs vary across the trajectory of illness and range from informational needs to emotional support to assistance with accessing resources. The needs of caregivers of brain tumor patients are complex not only because of the life threatening nature of the illness but also because of the wide range of neurological and cognitive deficits patients experience, a known risk factor for added stress to the caregiver. To address these challenges and improve caregiver outcomes, the Neuro Oncology Division at UCSF developed a program to provide an additional layer of care that includes information and support to caregivers of brain tumor patients across the trajectory of illness, from diagnosis to death. Methods: The program consists of a medical director and a dedicated nurse practitioner, social worker and coordinator, all focused on the caregiver of the brain tumor patient. The goals of the program are to provide information needed to assist with disease and symptom management, effective communication with the health care team, maximization of use of available resources, and to provide emotional support. Three groups of caregivers were targeted: caregivers of all new patients with a focus on newly diagnosed glioblastoma and caregivers of patients deemed at “high risk,” either identified by the provider, or because of the phase in the illness trajectory. Results: Since June 2013, 13 caregivers reported no needs for the first 4 months from diagnosis. 13 caregivers received care, with over half reporting concerns regarding financial issues and disability as well as need for emotional support around adjusting to the diagnosis, role changes, and care demands. In addition, varying levels of care have been provided to 70 caregivers of patients identified at “high risk” for distress due to disease progression or when transitioning to hospice. Conclusions: This program provides care to caregivers of brain tumor patients at specific time points to improve caregiver outcomes. Challenges include tailoring the information at the right time as well as measuring outcomes with minimal extra burden to the caregiver.

Abstract:BackgroundSeriously ill patients often depend on their informal caregivers to help and support them through the disease course. This study investigated informal cancer caregivers’ experiences of caregiving tasks and consequences and how caregiver status (primary vs. non-primary caregiver) and the caregiver’s relationship to the patient (spouse/partner, etc.) are related to these experiences.MethodsIn a cross-sectional questionnaire study, randomly selected cancer patients with a range of diagnoses and disease stages were invited to pass on the ‘Cancer Caregiving Tasks, Consequences and Needs Questionnaire’ (CaTCoN) to 1–3 of their caregivers.ResultsA total of 590 caregivers related to 415 (55% of 752 eligible) cancer patients participated. Large proportions of caregivers experienced substantial caregiving workload, e.g., provision of psychological support (74%), as well as a range of negative consequences, most commonly stress (59%). Some caregivers experienced personal growth, but relatively large proportions did not. Caregiver status and the caregiver’s relationship to the patient were associated with some caregiving aspects. Primary caregivers experienced the highest caregiving workload, and non-primary caregivers experienced most problems with getting time off from work. Spouses/partners and/or parents experienced the highest workload, most lack of time for social relations, most financial difficulties, and had the greatest need for seeing a psychologist. They furthermore experienced the highest degree of personal growth and had the smallest need for living a normal life while being a caregiver. Yet, regarding the majority of caregiving aspects, no associations with caregiver status or the caregiver’s relationship to the patient were found.ConclusionsOverall, the findings confirm that cancer caregiving is burdensome. The primary and the closest caregivers seemed to take on most caregiving tasks, but, contrary to expectations, regarding the majority of caregiving consequences non-primary and more distant caregivers were affected to the same degree as the primary and closest caregivers. Initiatives and interventions to support not only the primary caregivers are therefore warranted.

Abstract:Background/Purpose: Caregivers often face physical, psychological, and financial burdens when caring for people with cancer. Caregivers of Hispanic/Latino (HL) patients with cancer are often informal caregivers, such as families and friends, reflecting the traditional cultural value of familismo. While many studies acknowledge the impact of cultural differences on caregiving and caregiver burden, our understanding of the specific barriers HL patients with rectal cancer and their caregivers encounter is limited. The aim of this study was to explore the unique challenges, experiences, and perspectives of individuals caring for HL patients with rectal cancer in ways that could inform future interventions. Methods/Study population: We conducted semi-structured interviews of Bay Area and San Joaquin Valley HL patients with past or current rectal cancer in English and/or Spanish based on participant preference. We partnered with Visión y Compromiso, a promotores and community health worker organization, and developed a community advisory board to help refine our interview guide and interpret our results. We encouraged patients to identify and include someone who helped care for them during their treatment, but intentionally did not define caregiver in any other way. Over six months, we interviewed a total of 17 patients and eight caregivers and used reflexive thematic analysis to identify themes from their shared experiences using interview transcripts in NVivo 14. Results: Sixteen of seventeen interviews identified family members including spouses, sons, brothers, and friends as primary caregivers. Identified themes include (1) roles informal caregivers undertake for transportation, daily activities, and healthcare management; (2) limitations in utilizing social workers as a resource due to a knowledge gap and language barriers; (3) personal financial burden from supporting patients and navigating through available resources, and (4) psychosocial impacts resulting from role overload as an advocate, a translator, and a caregiver. Conclusion: In supporting cancer patients, caregivers wear many hats. While many caregivers share similar burdens, informal caregivers experience added emotional stress from unexpectedly assuming new responsibilities. Specific to rectal cancer care, caregivers are often tasked with aiding in ostomy bag changes and navigating the multidisciplinary treatment teams. Furthermore, HL patients and their caregivers face unique challenges due to language barriers, such as the burden of translation and delivering news. Understanding the caregiver burden among HL patients with rectal cancer is crucial in enhancing cancer treatment and care, creating culturally relevant resources, and reducing cancer outcome disparities faced by HL communities. Financial stress and burden among caregivers of cancer survivors in the United States.

Abstract:12027 Background: United States caregivers perform annual services equating to nearly $500 billion, with three-quarters of family caregivers utilizing personal funds. Nearly a third of all caregivers of cancer survivors report using all or most of their savings to contribute to the cost of cancer care. Prior studies indicate the duration of caregiving is associated with an increase in caregiving burden. This study aims to further describe the financial stress and burden in relation to caregiving duration for people who provide care to cancer survivors. Methods: A retrospective cross-sectional study was conducted using the 2015 and 2020 National Alliance for Caregiving and American Association of Retired Persons survey data. Multivariable logistic regressions were used to examine differences in reporting financial stress between various racial/ethnic groups while adjusting for education, income, and employment. Results: A total of 2,998 caregivers were included, where 208 of them were caring for cancer patients. Caregivers of cancer patients who provided care > 21 hours/week were statistically more likely to be less educated, lower income, single, and unemployed compared to those who provided 21 hours/week compared to 21 hours/week were more likely to advocate for recipients (68.7% vs. 56.8%; p = 0.086) and monitor condition severity to adjust care needs (84.9% vs. 70.7%; p = 0.100). After controlling for confounders (education, income, and employment), Non-Hispanic Black (odds ratio (OR) = 1.30; 95% CI: 1.02-1.65), Hispanic (OR = 1.39; 95% CI: 1.02-1.89), and Asian (OR = 1.51; 95% CI: 1.02-2.24) cancer caregivers had increased odds of reporting increased financial stress compared to Non-Hispanic White caregivers. Conclusions: Longer duration of caregiving to cancer survivors is associated with a greater burden for caregivers. Financial stress disparities related to caregiving exist between racial/ethnic groups, with non-white caregivers being more likely to report increased financial strain. Special financial programs are needed to assist caregivers of cancer survivors, a growing but understudied population.

Abstract:BACKGROUND: The supporting role of caregivers is crucial to cancer patients' care and well-being. Periods of inpatient hospital treatment are common in the cancer trajectory. There is insufficient systematic knowledge of caregivers' experiences and information needs in hospital context.AIM: Aim of this study is to identify information needs and factors contributing to unmet information needs of caregivers in hospital.DESIGN: A qualitative approach was used to identify major themes and specific types of information needs. Interviews with caregivers were conducted and analysed using a qualitative three-step process.SETTING/PARTICIPANTS: The study was conducted in a hospital oncology department. Seventeen caregivers of patients with advanced cancer were interviewed.RESULTS: Caregivers' needs during inpatient treatment vary and are largely unmet. Four major themes emerged from the analysis, revealing information needs of caregivers related to the cancer disease, patient, caregivers themselves and hospital context. The most mentioned issues were appropriate treatment, treatment outcomes and the related consequences for caregivers' life, hospital processes and transition back home.CONCLUSION: Caregivers have a variety of specific information needs which often remain unmet. Based on our findings, we provide recommendations for integrating caregivers into oncology care, which should be implemented in clinical practice, policy decisions and research.

Abstract:Abstract Background ‏ : Cancer is one of the most critical health problems worldwide today. Studies by different researchers have shown that disease can reduce people's ability to self-care. Therefore, this study was conducted to investigate Self-care needs in patients with cancer.Methods ‏ : ‏ This study is a conventional, qualitative content. Data saturation achieved after interviewing 13 participants in 15 interviews. With the in-depth semi-structured approach, the participants were asked to narrate their experiences of Self-care needs in the process of disease. The analysis was done according to Graneheim and Lundman's method, the following considered: the unit of analysis, meaning unit, condensation, code, sub-category ‏,‏ category, and main category.Results ‏ : Data analysis leads to two main categories of the two main categories of "Deficiency in management of disease". Including three sub-categories: "the need to get rid of annoying physical symptoms"," Need for a caregiver in daily routine," and" Feeling frustrated and giving up treatment". The second main category of "the need for sympathetic and knowledgeable caregivers" with the following five sub-categories: "Need for sympathy and interaction with nurse", "the need to hear the truth about illness", " Need for appropriate therapeutic interaction from physicians", "Inadequate qualification of physicians' secretary about the patient", and" need for a psychologist for psychological adaptation. "Conclusion ‏ : ‏ The most critical needs are compassionate and informed caregivers. So nursing management and care with kindness, competence, and conscience is the fundamental right of patients with cancer. Identifying the needs and problems of patients can be used as a background for increasing the knowledge and experience of nurses and providing care for patients with cancer.
Family Support in Advanced Cancer

Abstract:Changes in the health care system have resulted in a shift of cancer care from the in-patient arena to ambulatory and home settings. This shift has likewise translated into increased family involvement in the day-to-day care of the person with cancer. Cancer patients have multifaceted needs, including disease and treatment monitoring, symptom management, medication administration, emotional support, assistance with personal care, and assistance with instrument care. Family caregivers may be ill prepared to assume these tasks, requiring information on the disease and treatment, as well as instruction in technical and care skills. Moreover, caregiving must be balanced against already established roles and role responsibilities. In addition, family caregivers have their own emotional responses to the patients' diagnosis and prognosis, and may require coaching and emotional support themselves. The health care system can facilitate positive outcomes by embracing the family caregiver as a partner in the health care team, providing instruction and guidance to the caregiver as he/she assumes this role, and evaluating the home care situation. Research to date has only scratched the surface of testing interventions that meet the needs of the cancer caregiver. A research agenda is proposed to more fully elucidate the cancer caregiver's experience throughout the illness and treatment trajectory, and identify the means to effecting positive outcomes for the person with cancer, their family caregiver, and the health care system.

Caregiver burden in caregivers of patients with advanced stage cancer: A concept analysis

Abstract:Purpose: Caregiver burden is a term used to describe the strain endured by the family member who is providing care for a patient and does not have formal training to do so. Caregiver burden is associated with a lack of self-efficacy, a decrease in the caregiver's quality of life, and high levels of anxiety and depression, that can worsen over time, impacting the well-being of the dyad. Due to the dynamic nature of caregiver burden, the precise understanding of this condition in relation to advanced stage cancer patients has not been fully conceptualized. Clarity of the nature of caregiver burden guides researchers, policy makers, and providers with a better understanding of what is needed to detect who the caregivers are and who is at most risk for severe caregiver burden. Method: An electronic search of the literature using PubMed, CINAHL, PsycINFO, and Cochrane Library databases informed the analysis. Quantitative and qualitative studies related to caregiver burden of the cancer patient, published in English between 2000 and 2021 were used. Results: Of the over 20,000 articles initially identified, 19 ultimately met the inclusion criteria. Analysis identified the defining attributes of caregiver burden of the advanced stage cancer patient as: (1) decreased self-efficacy, (2) decreased quality of life, (3) increased anxiety, (4) increased depression, (5) and time sensitivity. Antecedents were identified as predictors of negative outcomes without intervention. Consequences were highlighted for those who are not supported. Conclusions: Caregiver burden impacts more than 2.8 million informal caregivers in the United States, who are providing complex care to patients with cancer without proper preparation. The conceptual understanding of caregiver burden guides the design of specific interventions to identify and prepare the caregiver for their critical role.

Abstract:The COVID-19 pandemic likely exacerbated caregiving challenges for caregivers of parents diagnosed with a blood cancer. Providing care during a public health crisis presents a complex web of uncertainties regarding cancer care, personal health, and COVID-19 risk. Identifying caregivers' uncertainty experiences during the COVID-19 pandemic can be a first step in learning where to direct resources or alter policies to ensure that they can not only perform their caregiver role but also cope in health-promoting ways. Using uncertainty management theory, this study explored how the pandemic has impacted adult child caregivers' experiences caring for a parent diagnosed with a blood cancer, as well as their experiences of uncertainty and uncertainty management. As part of a larger study on blood cancer caregivers' needs, a survey was administered from March 30 to June 1, 2020, to recruit caregivers through the Leukemia and Lymphoma Society. A qualitative and quantitative content analysis was conducted on open-ended responses from 84 caregivers. Caregivers described changes illustrating the complexity of providing care during a pandemic: (a) increased fears and uncertainty-related distress, b) reduced in-person care opportunities, (c) increased isolation, and (d) enhanced family communication. Caregivers with parents diagnosed with acute blood cancers used significantly more uncertainty management strategies and had more sources of uncertainty than caregivers with parents living with chronic blood cancer types. Findings highlight the need for supportive services to help caregivers manage uncertainty and improve their capacity to provide care in an unpredictable global health crisis. Such support may reduce poor psychosocial outcomes.
