Abstract:Introduction: Qualitative approaches have flourished in medical education research. Many research articles use the term 'lived experience' to describe the purpose of their study, yet we have noticed contradictory uses and misrepresentations of this term. In this conceptual paper, we consider three sources of these contradictions and misrepresentations: (1) the conflation of perspectives with experiences; (2) the conflation of experience with lived experience; and (3) the conflation of researching lived experience with phenomenology. We offer suggestions to facilitate more precise use of terminology. Argument: Our starting point is to free researchers from unnecessary shackles: Not every problem in medical education should be studied through experience, nor should every study of experience be phenomenological. Data based on participants' perceptions, beliefs, opinions and thoughts, while based on reflections of experiences, are not in and of themselves accounts of experience. Lived experiences are situated, primal and pre-reflective; perspectives are more abstract. Lived experience-as opposed to experiences as such-deeply attune to bodies, relationality, space and time. There is also a difference between experiences as lived, how a person makes sense of these and what the researcher interprets and represents. Phenomenology is a meaningful approach to the study of lived experience, but other approaches, such as narrative inquiry and self-study, can also offer useful avenues for undertaking this type of research. Discussion: We aim to broaden researchers' scope with this paper and equip researchers with the information they need to be clear about the meaning and use of the terms experience and lived experience. We also hope to open new methodological possibilities for researching experiences as lived and, through highlighting tensions, to prompt researchers of lived experience to strive for ontological closeness and resonance.
'Keeping it real': A qualitative exploration of preferences of people with lived experience for participation and active involvement in mental health research in Australia

Abstract:Background: Historically, researchers have been apt at conducting research on, rather than with, the people who are the focus of their efforts. Such approaches often fail to effectively support and benefit the populations they are intended to. This study aimed to explore the preferences of people with lived experience for engagement with research either as research participants within studies, or through active involvement in mental health research. Methods: Data for this paper were collected in three separate lived experience agenda-setting studies conducted over a 9-year period from 2013 to 2022; two group discussions and an open-ended online survey. Data were combined and thematic analysis undertaken. Results: Participants described the inclusion of lived experience as a critical ingredient and the highest level of knowledge and expertise in mental health research that should lead to knowledge generation and research agendas. Participants discussed the importance and value of research that enables sharing experiences and stories, expressed a need for flexibility in research methods for choice and agency, and support for greater active involvement of people with lived experience across all stages of research. Participants also spoke to the need for perspective and knowledge generated from people with lived experience to have equal power in research, making space for lived experience voices across multiple aspects of research, and greater respect and recognition of the value of lived experience. Conclusion: Lived experience in mental health research is coming of age, but dedicated, cocreated development is needed to get it right. People with lived experience increasingly understand the value their experiential knowledge brings to the mental health research effort, and describe a wide range of ways that researchers can support them to be research participants, and to get actively involved. Power-sharing, respect and recognition of lived experience as central to effective mental health research are the keys to 'keeping it real'. Patient or public contribution: People with lived experience of mental health problems or distress either personally, and/or as carers, family and kinship group members, were involved in the coideation and codesign of this research. Abstract:Feminism &Psychology, Ahead of Print. Using an autoethnographic approach, I reflect on and unpack my journey towards my doctoral research on queer South Africans of Indian descent. I demonstrate how my decision to become an insider-researcher forced me to confront personal resistances towards turning the academic gaze upon myself. Although my journey towards intersectional LGBTQ+ research began with a yearning for epistemic visibility, prompted by a curious search for psychological literature about people like me, I now grapple with a flip side—doing the emotional labour of visibility and managing my vulnerabilities. Drawing on Gloria Anzaldúa's feminist theory of Borderlands, I (re)conceptualize these vulnerabilities as vulnerable advantages, that is, liminal spaces of heightened reflexivity that can ignite psychopolitical potential in scholar-activism. I discuss and theorize the productive possibilities of these emotionally complex spaces by drawing on four interconnected life experiences related to the epistemic, ethical, artistic, and activist dimensions of vulnerabilities. I consider the emotional labour embedded in researching insider communities, and my anxieties about traversing through these psychosocial conflicts. I argue that reflecting on one's own vulnerabilities can provide researchers with unique perspectives as scholar-activists. These vulnerable advantages exist because of—not in spite of —one's initial trepidations as valuable psychopolitical and decolonial resources.

Abstract:BackgroundThe concept of patient engagement in health research has received growing international recognition over recent years. Yet despite some critical advancements, we argue that the concept remains problematic as it negates the very real complexities and context of people’s lives. Though patient engagement conceptually begins to disrupt the identity of “researcher,” and complicate our assumptions and understandings around expertise and knowledge, it continues to essentialize the identity of “patient” as a homogenous group, denying the reality that individuals’ economic, political, cultural, subjective and experiential lives intersect in intricate and multifarious ways.DiscussionPatient engagement approaches that do not consider the simultaneous interactions between different social categories (e.g. race, ethnicity, Indigeneity, gender, class, sexuality, geography, age, ability, immigration status, religion) that make up social identity, as well as the impact of systems and processes of oppression and domination (e.g. racism, colonialism, classism, sexism, ableism, homophobia) exclude the involvement of individuals who often carry the greatest burden of illness — the very voices traditionally less heard in health research. We contend that in order to be a more inclusive and meaningful approach that does not simply reiterate existing health inequities, it is important to reconceptualize patient engagement through a health equity and social justice lens by incorporating a trauma-informed intersectional analysis.SummaryThis article provides key concepts to the incorporation of a trauma-informed intersectional analysis and important questions to consider when developing a patient engagement strategy in health research training, practice and evaluation. In redefining the identity of both “patient” and “researcher,” spaces and opportunities to resist and renegotiate power within the intersubjective relations can be recognized and addressed, in turn helping to build trust, transparency and resiliency — integral to the advancement of the science of patient engagement in health research.

Abstract:Background: Studies have highlighted common challenges and barriers to patient engagement in research, but most were based on patient partners' or academic researchers' experiences. A better understanding of how both groups differentially experience their partnership could help identify strategies to improve collaboration in patient engagement research. Aim: This study aimed to describe and compare patient partners' and academic researchers' experiences in patient engagement research. Methods: Based on a participatory approach, a descriptive qualitative study was conducted with patient partners and academic researchers who are involved in the PriCARE research programme in primary health care to examine their experience of patient engagement. Individual semi-structured interviews with patient partners (n = 7) and academic researchers (n = 15) were conducted. Academic researchers' interview verbatims, deidentified patient partners' summaries of their interviews and summaries of meetings with patient partners were analysed using inductive thematic analysis in collaboration with patient partners. Results: Patient partners and academic researchers' experiences with patient engagement are captured within four themes: (1) evolving relationships; (2) creating an environment that fosters patient engagement; (3) striking a balance; and (4) impact and value of patient engagement. Evolving relationships refers to how partnerships grew and improved over time with an acceptance of tensions and willingness to move beyond them, two-way communication and leadership of key team members. Creating an environment that fosters patient engagement requires appropriate structural support, such as clear descriptions of patient partner roles; adequate training for all team members; institutional guidance on patient engagement; regular and appropriate translation services; and financial assistance. For patient partners and academic researchers, striking a balance referred to the challenge of reconciling patient partners' interests and established research practices. Finally, both groups recognized the value and positive impact of patient engagement in the programme in terms of improving the relevance of research and the applicability of results. While patient partners and academic researchers identified similar challenges and strategies, their experiences of patient engagement differed according to their own backgrounds, motives and expectations. Conclusion: Both patient partners and academic researchers highlighted the importance of finding a balance between providing structure or guidelines for patient engagement, while allowing for flexibility along the way. Patient or public contribution: Patient partners from the PriCARE research programme were involved in the following aspects of the current study: (1) development of the research objectives; (2) planning of the research design; (3) development and validation of data collection tools (i.e., interview guides); (4) production of data (i.e., acted as interviewees); (5) validation of data analysis tools (code book); (6) analysis of qualitative data; and (7) drafting of the manuscript and contributing to other knowledge translation activities, such as conference presentations and the creation of a short animated video.

Abstract:In this article, I illustrate lived experiences within my roles as caregiver, clinical social worker, and researcher. Empirical attention has focused on the emotional toll of caregiving and management of emotions as clinical social workers, however, little attention has been given to the impact emotions have on the researcher and how emotional awareness can be useful in the creation of knowledge. Using a personal reflexive account, I “out” myself as an emotionally aware caregiver, clinical social worker, and researcher. Finally, I provide examples of how to incorporate emotion into the research process.

Abstract:Research ethics is often understood by researchers primarily through the regulatory framework reflected in the research ethics review process. This regulatory understanding does not encompass the range of ethical considerations in research, notably those associated with the relational and everyday aspects of human subject research. In order to support researchers in their effort to adopt a broader lens, this article presents a "person-oriented research ethics" approach. Five practical guideposts of person-oriented research ethics are identified, as follows: (1) respect for holistic personhood; (2) acknowledgement of lived world; (3) individualization; (4) focus on researcher-participant relationships; and (5) empowerment in decision-making. These guideposts are defined and illustrated with respect to different aspects of the research process (e.g., research design, recruitment, data collection). The person-oriented research ethics approach provides a toolkit to individual researchers, research groups, and research institutions in both biomedical and social science research wishing to expand their commitment to ethics in research.

Abstract:Introduction: Many institutions seek to engage postsecondary students in research to grow future researchers. Despite this common goal, the means to achieve that end is often unknown creating difficulties for students as they seek out research opportunities. Objective: This article will share first-hand reports on the experiences of graduate students, undergraduate students, and family partners following their engagement as an interdisciplinary research team. Methods: This is a qualitative study using individual open-ended interviews with undergraduate students, graduate students, and patient family partners. The research was conducted using a Patient-Oriented Research approach. The transcripts were analyzed inductively by using Braun and Clarke's six phases of reflexive thematic analysis. Results: Two themes were developed from the analysis. The theme of Benefits & Facilitators was developed based on the many positive thoughts, feelings, resiliency, and learning expressed by the students and family partners. The undergraduate students found the experience of "doing" research fulfilling and a great tool for learning how to better use research in practice. Within the second theme Perceived Hierarchy, undergraduate students described themselves as helpers, not responsible, and they did not have the power to make decisions. Conclusion: Given the challenges identified in this study, future efforts in this approach should carefully consider the culture and how to best engage graduate students, undergraduate students, and family partners in research teams.

Abstract:Patient and public involvement and engagement (PPIE) is an increasingly important component of research conduct to enhance processes and potential for impact, yet is rarely critically interrogated. This paper draws on Foucauldian analysis to highlight the disciplinary powers and tensions arising in PPIE. The paper draws on a nested evaluation interview study with three PPIE members and eight academics, who had been involved in an implementation science study focused on palliative care. PPIE members were involved in the whole study and are co‐authors of this article. Through shared values and commitments to the study, a team culture of equality was developed. Yet while power was dispersed and taken‐up by all team members, in so doing a self‐governance approach within the team was developed. The pace and focus of discussions was at times more subjugating than co‐production. Identities and positions were porous; the simplistic division of 'academic' and 'PPIE' did not stand up to scrutiny, with an increasing blurring of boundaries as people's experiences and insights changed over time. Continual, subtle, negotiations of roles, inputs and identities were manifest throughout the project. PPIE in research involves subtle, complex and ongoing disciplinary practices enacted by all members of the team.

Abstract:Background: There is a need to unpack the empirical, practical, and personal challenges within participatory approaches advocated to optimize implementation. The unpredictable, chaotic nature of participatory approaches complicates application of implementation theories, methods, and strategies which do not address researchers' situatedness within participatory processes. As an implementation scientist, addressing one's own situatedness through critical reflection is important to unearth how conscious and unconscious approaches, including ontological and epistemological underpinnings, influence the participatory context, process, and outcomes. Therefore, the aim of this exploratory work is to investigate the heretofore blind spot toward the lived experience of implementation researchers within the participatory process. Methods: We developed an integrated research-practice partnership (IRPP) to inform the implementation of a gestational weight gain (GWG) control program. Within this IRPP, one investigator conducted a 12-month autoethnography. Data collection and triangulation included field notes, cultural artifacts, and systematic timeline tracking. Data analysis included ethnographic-theoretical dialogue and restorying to synthesize key events and epiphanies into a narrative. Results: Analysis revealed the unpredicted evolution of the GWG program into a maternal health fair and three themes within the researchers' lived experience: (1) permeable work boundaries, (2) individual and collective blind spots toward the ontological and epistemological underpinnings of implementation paradigms, and (3) maladaptive behaviors seemingly reinforced by the research culture. These themes contributed to the chaos of implementation and to researchers' experience of inadequate recovery from cognitive, emotional, and practical demands. These themes also demonstrated the importance of contextual factors, subjectivity, and value-based judgments within implementation research. Conclusion: Building on extant qualitative research guidelines, we suggest that researchers anchor their approach to implementation in reflexivity, intentionally and iteratively reflecting on their own situatedness. Through this autoethnography, we have elucidated several strategies based on critical reflection including examining philosophical underpinnings of research, adopting restorative practices that align with one's values, and embracing personal presence as a foundation of scientific productivity. Within the predominant (post-) positivism paradigms, autoethnography may be criticized as unscientifically subjective or self-indulgent. However, this work demonstrates that autoethnography is a vehicle for third-person observation and first-person critical reflection that is transformative in understanding and optimizing implementation contexts, processes, and outcomes.
Negotiating access to community-based participatory research

Abstract:Purpose: Community-based participatory research (CBPR) that improves social capital can be a powerful tool for promoting mental health and well-being. This work explores what gaining, maintaining, and losing access to this type of CBPR looks like from a reflexive research perspective. Method: I describe and reflect on my experiences conducting a mixed-methods study of an existing CBPR to increase social capital in Switzerland. I draw on ethnographic observations, field notes, and reflexive memos collected during fieldwork between 2016 and 2020. Results: I negotiated access to the CBPR across three levels: (1) formal organizational with intervention leaders, (2) implementational with facilitators, and (3) the community/group level with participants. Intervention leaders let me conduct research if they benefitted from my work in a timely and reinforcing way, facilitators granted access if I made myself helpful and supported their work, and community members accepted me if I participated in their community meaningfully. I lost access when my findings posed a potential risk to the intervention funding. Conclusion: I highlight how access is a fluid and complex process that can change throughout CBPR. I show the importance of reflexive analysis to understand how access is negotiated in diverse settings, what sources of social capital are needed to engage in these negotiations, and how positionality and power play a role in this process.
Ethnographic closeness: methodological reflections on the interplay of engagement and detachment in immersive ethnographic research

Abstract:With the reflexive turn in the social sciences, emotional engagement is an inevitable and crucial part of data-gathering and analysis. However, there is a glaring gap in methodological discussions to this end. Presenting ethnographic research into end of life with people living at home in England with heart failure, we argue for a methodological blend of engagement and detachment that shifts throughout the research process, and that sensory experience is a core part of engagement. We offer ethnographic examples which present and explore some alternatives to emotional engagement and objective detachment: (1) moving with participants to facilitate engagement during fieldwork through shared sensory experience; (2) detachment as a different way of relating when exiting the field and drawing participant relationships to a close; and (3) ethnographic closeness as the interplay of engagement and detachment in participant debriefing and data analysis. Based on well-established anthropological concepts, and taking both engagement and detachment as embodied and relational, we develop a notion of ethnographic closeness in which detachment is a necessary part. Our detailed methodological discussion thus offers theoretically grounded possibilities and alternatives for approaching and managing the core tension of 'how close is too close?' in ethnographic practice. Further contributions supporting researchers in navigating ethnographic research are needed.
Exploring patient and caregiver perceptions of the meaning of the patient partner role: a qualitative study

Abstract:Background: The re-conceptualization of patients' and caregivers' roles in research from study participants to co-researchers ("patient partners") has led to growing pains within and outside the research community, such as how to effectively engage patients in research and as part of interdisciplinary teams. To support the growth of more successful research partnerships by developing a shared understanding of how patient partners conceptualize and contribute to their role, this study aimed to explore patient partners' motivations for engagement and understanding of their role. Methods: We conducted semi-structured interviews with participants (n = 13) of an online survey of activities and impacts of patient engagement in Strategy for Patient-Oriented Research projects. Eligibility criteria included being a patient partner that indicated interest in interview participation upon survey completion, the ability to read/write in English and provide informed consent. Data were analyzed thematically using an inductive, codebook thematic analysis. Results: Illuminating the lived/living patient and caregiver experience was central to how most patient partners conceptualized the role in terms of its definition, purpose, value, and responsibilities. Participants also identified four additional categories of motivations for becoming a patient partner and contributions that patient partners make to research that build upon and are in addition to sharing their lived/living experiences. Lastly, participants highlighted important connotations of the term patient partner, including temporal and context-specific considerations for the term "patient" and what "partner" may imply about the nature of the research relationship. Conclusions: At the onset of partnership, academic researchers and patient partners must create the space necessary to discuss and understand each other's underlying motivations for partnering and their perspectives on the purpose, value, and responsibilities of the patient partner role. These early conversations should help unearth what research partners hope to get out of and feel that they are able to contribute to engaging, and in such contribute to the development of reciprocal relationships that work towards shared and valued goals. Bridging the divide: supporting and mentoring trainees to conceptualize, plan, and integrate engagement of people with lived experience in health research

Abstract:Health researchers are encouraged by governments, funders, and journals to conduct research in partnership with people with lived experience. However, conducting research with authentic engagement and partnership with those who are experts by experience, but may not have research methods training, requires resources and specialized skills. The McMaster Collaborative for Health and Aging developed a fellowship program for trainees that builds their capacity to conduct research in partnership with older adults with relevant lived experience. We share this case example, with its successes and challenges, to encourage creative reformation of traditional research training.The Collaborative used an iterative design process, involving researchers, trainees and older adult and caregiver partners, who, together, developed a fellowship program for trainees that provides support and mentorship to plan and conduct health research in partnership with people with lived experience.Since 2022, the Partnership in Research Fellowship has been offered biannually. The application process was purposefully designed to be both constructive and supportive. Opportunities for one-on-one consultations; key resources, including a guide for developing a plan to involve people with relevant lived experience; and feedback from older adult and researcher reviewers are provided to all applicants. Successful trainees engage with older adult and caregiver partners from the Collaborative to advance and enhance a range of skills from facilitating partner meetings to forming advisory committees. Trainees are awarded $1500 CAD to foster reciprocal partnerships. Ten graduate students from various disciplines have participated. Trainees reported positive impacts on their knowledge, comfort, and approach to partnered research. However, the time required for undertaking partnered research activities and involving diverse partners remain obstacles to meaningful engagement.Partnering with people with lived experience in the design of educational programs embeds the principles of partnership and can increase the value and reward for all involved. We share the Partnership in Research Fellowship as a case example to inspire new and transformative approaches in research training and mentorship that will move the field forward from engagement theory to meaningful enactment.

Abstract:<span>Despite the common use of qualitative in-depth interviews in healthcare research, little empirical research exists on how dialogues about personal and sensitive topics affect the interviewees. This study aims to describe the meaning of participating in lifeworld-based research interviews conducted in a healthcare context experienced from the patients' perspective. The study has a hermeneutic-phenomenological approach. Eleven persons with severe obesity were purposefully recruited for repeated individual interviews about physical activity and lifestyle changes and asked about their experiences of being interviewed. The data underwent a hermeneutic text interpretation and a data-driven coding focusing on meaning. Ethical approvals were received. One main theme emerged: 'Lifeworld reflections are facilitated through the relationship with the researcher based on the sub-themes: 'Being interviewed is an occasion to stop, think and unfold one's narrative', 'Gaining insight can facilitate action and emotions' and 'It requires willingness to leave one's comfort zone'. Our conclusions are that a person's lifeworld experiences are important for being reflective, which can be both rewarding and hurtful for the interviewee. The research interview holds numerous ethical aspects, which must be considered to ensure the wellbeing of the interviewee.</span>

Abstract:It has been said that governmental bureaucracies lack the animating life force that is normally provided by the human conscience. Research efforts that include patients and their representatives in the planning and regulatory process can add back this animating life force, a force Friedrich Wilhelm Nietzsche describes "... that your self be in your deed as the mother is in her child-let that be your word concerning virtue." This paper comprises our invited introductory remarks as patient activists at this symposium, entitled "The Menstrual Cycle and Adolescent Health" and held in Potomac, Maryland in mid October 2007. Attendees included patients, patient advocates, and experts from a variety of fields and disciplines. While our stories have their share of pain, that pain developed into a passion to help others in similar circumstances. A consortium of passionate community activists interested in the menstrual cycle could play the role as a "governmental conscience" around this issue. Developing a community consortium initiated via partnerships between patient advocates and investigators could direct more attention and funding toward menstrual cycle research.

Abstract:Imagine being given the immense responsibility and gift as a researcher to deliver a person's final nature experience. People living with severe chronic obstructive pulmonary disease (COPD) are often sheltered from nature experiences due to their disease status; however, they have rich memories of past nature experiences. The goals of this overall journey of co-creation were two-fold: (1) co-create personalized nature-based utilizing immersive 360-degree virtual reality (VR) experiences based on participants' experiences in outdoor spaces; (2) utilize a narrative approach to explore the lived realities of people living with severe COPD. Throughout this collaborative research process, I made home visits, phone calls, and many trips to four people's personalized outdoor places in the Upstate of South Carolina. In doing so, I lived the experience of person-centered research. This essay focuses specifically on one participant, David, his wife and caregiver, Anne, and the co-creation of David's last virtual trip to his most cherished places in nature. Re-gifting David's last nature experience in his favorite outdoor places profoundly changed my research approach. This essay considers how I reconciled the disconnect between the "scientific" language used in health research and the lived experience of being a researcher during end-of-life processes, specifically attending to the terms: attrition , participant , and researcher .

Abstract:Neuropalliative care developed to address the needs of patients living with life‐limiting neurologic disease. One critical consideration is that disease‐related changes to cognition, communication, and function challenge illness experiences and care practices. We conducted an ethnography to understand neuropalliative care as a phenomenon; how it was experienced, provided, conceptualized. Personhood served as our conceptual framework; with its long philosophical history and important place in nursing theory, we examined the extent to which it captured neuropalliative experiences and concerns. Personhood contextualized complex losses, aligning the impact of functional and relational changes. Cognition, communication, and functional alterations stretched conceptions of personhood, insinuating it can be relational, fluid, adaptive. Although normative conceptions of personhood guided research and decision‐making, ethical considerations suggested personhood could be transformed, remade. We consider the implications of our findings through three themes. First, we examine how literature on illness experience fails to integrate the realities of people living with and dying from neurologic disease; we counter this by interrogating the concept of experience. Second, we turn to Ricoeur's work on recognition to illuminate relational conceptions of personhood to inform care practices. Finally, we reflect on how personhood can bridge the gap left by functional changes, enhance relational engagement, and promote dignity at the end of life.

Abstract:A virtues-based approach to research ethics offers an alternative perspective to the rule-based approaches that dominate many current ethical regulations. For researchers engaging in insider research where ethical dilemmas can be complex and unpredictable, ethical decisions often need to be made on a case-by-case basis. This requires researchers to have greater awareness of the dispositions of their character to ensure appropriate and just decisions are made. In this paper, a virtues-based approach to research ethics is used as framework for reflecting on research conduct. Past experiences as a teacher-researcher engaging in an insider research project are drawn on and used to demonstrate the usefulness of this framework for reflection. It is argued that by engaging in such reflection, a researcher may be able to strengthen their awareness of the excellences of character required to navigate ethical dilemmas confidently and safely in future research endeavours.

Abstract:Background: Community-academic partnerships are increasingly used in interventions to address health care disparities. Little is known about motivations and perceptions of participating community members. Objectives: To elicit community members' perspectives of involvement in a community-academic partnership to address implicit bias in health care. Methods: With our partnering community organizer, we conducted one-on-one semistructured interviews and a follow-up group interview with participating community members to solicit experiences about involvement in an National Institutes of Health-funded clinician training; responses were organized using content analysis. Results: Community members revealed that their participation was motivated by trust in our community organizer; they derived personal pride from participation in clinician training; the power differential between community members and clinicians in the training environment needed to be levelled. Our community organizer noted that the benefits of community-academic partnerships propagate to the larger community via community members' experiences. Conclusions: Community members note trust, pride, and power as important elements in community-academic partnership.
