Abstract:OBJECTIVE:To evaluate the clinical application of lung function analysis in diagnosis of children respiratory diseases.METHODS:Respiratory function was evaluated with portable lung function detector in 6 261 children with respiratory diseases. FEV1, FVC and PEFR were measured in 268 children with cough variant asthma (CVA) and 147 children with asthma (AS), and the results were self-compared during follow-up.RESULT:There were no significant differences in above parameters between children with cough variant asthma and those with asthma (P>0.05). Lung functions of asthma children were improved evidently by administration of glycocorticoid inhalator (P<0.001).CONCLUSION:Lung function detection is essential for diagnosis of asthma and it can be used as a reliable index for therapeutic effect of asthma children.
Tidal Breathing Analysis as a Prognostic Index for Airway Obstruction Trajectory and Asthma in Preterm Infants

Abstract:Introduction: An easy-to-implement and accurate lung function assessment tool for preterm infants is crucial to manage lifelong respiratory morbidities. We aimed to determine which pulmonary function parameters in preterm infants can predict the trajectory of airway obstruction and asthma development after 4 years of age. Methods: We evaluated 52 preterm infants who had undergone both tidal breathing flow-volume loop (TBFVL) and multiple-breath washout (MBW) analyses in infancy and spirometry after the age of 4 years. We evaluated the association between pulmonary function parameters in infancy and childhood and the pulmonary function trajectory until 13 years of age and compared the changes in this trajectory according to pulmonary function parameters in infancy. Results: Time to peak expiratory flow/expiratory time (TPEF/TE) in infancy was associated with FEV1, FEF25-75, and dysanapsis ratio in childhood and differed according to level of airway obstruction assessed by FEV1, FEV1/FVC, and FEF25-75, an asthma development. TPEF/TE was a significant predictive factor for airway obstruction and asthma after 4 years of age, after adjusting for sex, extreme prematurity, duration of supplementary oxygen and mechanical ventilation, and recurrent wheezing during infancy. In premature infants with lower TPEF/TE, subsequent pulmonary function parameters remained low until 13 years of age. Conclusion: In preterm infants, TPEF/TE could be useful to predict airway obstruction and asthma after 4 years of age and even a lower pulmonary function trajectory until 13 years of age. This information may help clinicians to provide lifelong care for pulmonary morbidity in children and adolescents born preterm.

Abstract:Introduction: The tidal breathing flow-volume loop (TBFVL) is a safe, quick, and noninvasive pulmonary function test; in the newborn it provides useful information on the patient's pulmonary characteristics. Bronchiolitis is a potentially serious lower respiratory tract disease even in children without comorbidities, however there are no tools capable of predicting which patients will develop a more severe form of the disease. Objectives. Evaluate the early use of TBFVL as a predictive tool to identify subjects at risk of developing bronchiolitis in the first six months of life. Methods. Term newborns who did not require respiratory assistance at birth were consecutively enrolled at the Policlinico Umberto I Hospital in Rome, Italy. TBFVL test was performed on the 5th day of life, in the sleeping state. The patients were followed for one year with quarterly telephone follow-up of the families, and the possible onset of bronchiolitis was monitored. Results. We recruited 41 infants (20 M, 21 F) between November 2021 and November 2022; 3 subjects (7.3%) developed bronchiolitis, of which 2 (4.8%) required hospitalization. The mean value of the ratio tPTEF/Te recorded at birth in subjects who developed bronchiolitis is significantly lower than the mean value observed in the population who did not experience bronchiolitis (28.4% vs 40.6% p=0.04). Conclusions. Our results suggest that a lower tPTEF/Te ratio, indicative of smaller airways, may be used to early identify subjects at risk of having more serious clinical manifestations of bronchiolitis, and who could benefit from more targeted prevention measures.

Clinical significance of FEV0.5 and FEV0.75 in the determination of pulmonary function in children with bronchial asthma

Abstract:Objective·To evaluate the abnormal rates of measured/predicted values of forced expiratory volume in 0.5 second (FEV0.5) and FEV0.75 in lung ventilation function testing, as well as the clinical significance of different cut-off points for improvement rates before and after bronchodilation testing in the diagnosis of children with bronchial asthma (asthma).Methods·From January 2020 to February 2021, school-age children diagnosed with asthma during their hospitalization at the Children's Hospital, Zhejiang University School of Medicine were selected. The data on lung ventilation function testing and bronchodilator testing were collected. The difference in abnormal rates of measured/predicted values of FEVt in children in lung ventilation function testing was compared. Diagnostic test four grid table was used to analyze the sensitivity and specificity of different cut-off points for the improvement rates of FEV0.5 and FEV0.75 in the diagnosis of asthma before and after bronchodilation testing.Results·A total of 148 children were included. All enrolled cases completed lung ventilation function testing, with 51 cases completing bronchodilation testing simultaneously. The results of lung ventilation function testing showed that 21 patients had obstructive ventilation dysfunction, 30 patients had small airway dysfunction, and the remaining patients had normal, restrictive or mixed ventilation dysfunction. The results of bronchodilation testing showed that in the 51 asthmatic children with obstructive ventilation dysfunction and small airway dysfunction, 22 of them were positive for bronchodilation testing. The comparison of the abnormal rates of FEVt measured/predicted values showed that the abnormal rates of FEV0.75 measured/predicted values were higher than those of FEV1 and FEV0.5 (χ2=10.18, P=0.000; χ2=10.95, P=0.000). The sensitivity and specificity analysis results of different cut-off points for the improvement rates of EFV0.75 and FEV0.5 showed that the cut-off point for FEV0.5 improvement rate at 16% had very high sensitivity and specificity, with the highest Jordan index; the cut-off points for FEV0.75 improvement rate were 12% and 14%, respectively, with sensitivity and specificity reaching 100%.Conclusion·The measured/predicted value of FEV0.75 in lung ventilation function testing can be considered as an alternative indicator for the measured/predicted value of FEV1 to further evaluate the presence of obstructive ventilation dysfunction. On the basis of the results of bronchodilation testing, it can be considered to analyze the airway reversibility by combining the best cut-off points of FEV0.75 and FEV0.5 improvement rates, in order to enhance the clinical value of bronchodilation testing in the diagnosis of asthma.

Abstract:Lung function testing is an essential modality of investigation in children as it provides objective evidence of lung disease/health. With advances in technology, various tests are available that can aid in the diagnosis of lung disease, assess the progression and response to therapy and document the lung development and evolving lung diseases in infants. This narrative review discusses lung function tests in infants and children. Currently, lung function tests can be performed in every age group, from neonates to the elderly. Spirometry and peak expiratory flow rate (PEFR) are the most employed tests in children more than six years of age. Spirometry helps diagnose and monitoring of both obstructive and restrictive diseases. There is a need for expertise to perform and interpret spirometry correctly. The forced oscillation technique (FOT) or impulse oscillometry (IOS) is done with tidal volume breathing and is feasible even in preschool children. Their utility is mainly restricted to asthma in children at present. Lung function tests can be performed in neonates, infants and children using infant pulmonary function test (PFT) equipment, although their availability is limited. Diffusion capacity for carbon monoxide (DLCO) is a valuable tool in restrictive lung diseases. Lung volumes can be assessed by body plethysmography and multiple washout technique. The latter can also assess lung clearance index. It is essential to perform and interpret the lung function test results correctly and correlate them with the clinical condition for optimum treatment and outcome.

Abstract:Pulmonary function tests(PFT)in childhood are very important in both research and clinical practice.Major techniques and methods of PFT include tidal breathing pattern,airway resistance,body plethsmography and rapid thoracoabdominal compression.Tidal breathing,as the most frequently used test,its most important parameters are TPTEF/TE(time to peak expiratory flow versus the time of expiratory)and VPEF/VE(volume to peak expiratory flow versus the volume of expiratory)that reflect the obstruction degree and clinical conditions.Bronchial challenge(BC)tests(or broncho-provocation tests,BPT),as an assistant diagnosis tool of asthma,typically is performed in adults and children older than 7 years in which FEV1(forced expiratory volume in 1 second)(or peak expiratory flow,PEF)is the parameter related to diagnosis.In recent 15 years,based on mechanism of IOS(impulse oscillation system)the new bronchial provocation test has been developed to measure the changes of airway resistance during tidal breathing in which Dmin(dose minimal)is recorded as a judge index.This technique can be adapted to children elder than 4 years old to expand its use and facilitate clinical diagnosis of asthma.
Fiberoptic Bronchoscopy in Neonatal and Pediatric Intensive Care Units: A 5-Year Experience

Abstract:Objectives: To determine the value and safety of fiberoptic bronchoscopy in neonatal and pediatric intensive care units (NICUs, PICUs). Subject and Methods: A total of 53 fiberoptic bronchoscopy procedures on 47 patients were reviewed. Bronchoalveolar lavage (BAL) was performed in 23 patients. Results: The primary diseases were pneumonia (n = 16), foreign body aspiration (n = 14), congenital airway abnormality (n = 12), trauma and/or following operations (n = 4) and Guillain-Barré syndrome (n = 1). The major bronchoscopic findings included inflammation in 26 patients, foreign body in 14, congenital airway abnormality in 12 and blood clotting in 3. Microbiology on BAL fluid was positive in 19 of 23 patients. In 23 patients with atelectasis, full and partial re-expansion was obtained in 14 and 6 patients, respectively, at 24 h after the procedures. The clinical features of 9 patients with sputum retention or blood clotting improved significantly after BAL. Positive or negative microbiologic BAL fluid results changed treatment in 11 patients, leading to marked clinical improvement in 9 patients. Moreover, 13 patients were extubated within 24 h of bronchoscopy. Conclusion: These data show that fiberoptic bronchoscopy is safe and effective in the diagnosis and therapy of pulmonary disorders in NICUs and PICUs.
Bronchoscopy findings in children with congenital lung and lower airway abnormalities

Abstract:Congenital lung and lower airway abnormalities are rare, but they are an important differential diagnosis in children with respiratory diseases, especially if the disease is recurrent or does not resolve. The factors determining the time of presentation of congenital airway pathologies include the severity of narrowing, association with other lesions and the presence or absence of congenital heart disease (CHD). Bronchoscopy is required in these cases to assess the airway early after birth or when intubation and ventilation are difficult or not possible. Many of these conditions have associated abnormalities that must be diagnosed early, as this determines surgical interventions. It may be necessary to combine imaging and bronchoscopy findings in these children to determine the correct diagnosis as well as in operative management. Endoscopic interventional procedures may be needed in many of these conditions, ranging from intubation to balloon dilatations and aortopexy. This review will describe the bronchoscopic findings in children with congenital lung and lower airway abnormalities, illustrate how bronchoscopy can be used for diagnosis and highlight the role of interventional bronchoscopy in the management of these conditions.

Abstract:Objective To explore the instructive role of pulmonary function monitoring in diagnosis and treatment of infants with asthma.Methods Spirometry was used to determine respiratory physiologic parameters respectively both in period of acute episode and remission.Results 45 cases were found to be with obstructive ventilation dysfunction out of 62 cases with mild to moderate acute episode,while 10 cases out of 15 with severe acute episode were found to be with mixed ventilation dysfunction,15 cases in period of remission showed mild to moderate obstructive ventilation dysunction.Conclusion Tidal breath pulmonary function monitoring may play a very important role in judging the state of asthma,deciding curative effects and following up.

Abstract:The flow-volume loop (FV-loop) provides a graphical representation of the inspiratory and expiratory flow of both mechanically provided breaths and patient-triggered breaths during invasive mechanical ventilation. The FV-loop on the ventilator-delivered breath displays the active inspiratory flow reflective of lung compliance and the passive expiratory flow reflective of airway resistance. Our case report highlights the importance of the FV-loop in determining a fixed airway obstruction. A five-month-old male presented to the emergency department with worsening respiratory distress in the setting of rhino-enterovirus. He was admitted to the pediatric intensive care unit (PICU) and intubated for acute hypoxic respiratory failure. The findings on his ventilator FV-loop graphics denoted a fixed airway obstruction, as seen by the truncation of inspiratory and expiratory flow. The patient was subsequently found to have a left pulmonary artery (LPA) sling with a vascular ring and several complete tracheal rings. He was transferred to a referral institution for operative management, returned to our PICU, and discharged home after 47 days of hospital management. During mechanical ventilation, FV-loops can be effectively utilized to assist in the diagnosis of fixed intra- or extra-thoracic airway obstructions.
Study on the reference values of tidal flow-volume loop in 1002 normal Chinese children under 4 years old

Abstract:Objective To establish the predicted value of pulmonary function for normal Chinese children under 4 years old.Methods The tidal flow-volume loop (TFV) and other parameters (the volume,flow rate and resistance) were measured in 1002 normal children,aged between 0～47 months.They were devided into 8 groups.Results TFV in healthy baby appeared to be atypical ellipse,it changed to be tactful as the child grow up.Most of the parameters had close relationship with height,weight and age,especially for the first two ones.Relative parameters showed that the expansion of volume was quicker than the increment of the flow.The development of small airways was quicker than the large airways.The metabolic rate decreased with the age increasing.The change of TPTEF/TE,VPEF/VE could reflect the degree of the obstruction in the small airway.Conclusion Tidal breathing testing is the first choice in testing the pulmonary function for children under 4 years with more accurate,simplicity and reproducibility.
Diagnostic utility of spirometry for children with induced laryngeal obstruction or chronic non-specific cough

Abstract:Purpose: To determine the diagnostic utility of spirometry in distinguishing children with Induced Laryngeal Obstruction (ILO) or chronic non-specific cough (a.k.a. tic cough) from those with mild or moderate to severe asthma. Methods: Retrospective cross sectional design. Children diagnosed with ILO (N = 70), chronic non-specific cough (N = 70), mild asthma (N = 60), or moderate to severe asthma (N = 60) were identified from the electronic medical record of a large children's hospital. Spirometry was completed before ILO, non-specific cough, or asthma diagnoses were made by pediatric laryngologists or pulmonologists. Spirometry was performed following American Thoracic Society guidelines and was interpreted by a pediatric pulmonologist. Forced Vital Capacity (FVC), Forced Expiratory Volume in 1 Second (FEV1), FEV1/FVC Ratio (FEV1/FVC), Forced Mid-Expiratory Flow 25--75 % (FEF25-75%), pulmonologist interpretation of flow volume loops, and overall exam findings were extracted from the medical record. Results: Ninety seven percent of children with ILO or chronic non-specific cough presented with spirometry values within normative range. Patients with ILO, non-specific cough, and mild asthma presented with FVC, FEV1, FEV1/FVC, and FEF25-75% values in statistically similar range. Children with moderate to severe asthma presented with significantly reduced FVC (p < .001), FEV1 (p < .001), FEV1/FVC (p < .001), and FEF25-75% (p < .001) values when compared with patients in the other groups. Flow volume loops were predominantly normal for children with ILO and non-specific cough. Conclusions: Findings indicate that ILO and chronic non-specific cough can neither be diagnosed nor differentiated from mild asthma using spirometry alone. Spirometry should therefore be used judiciously with this population, bearing in mind the limitations of the procedure. Future research should determine the most effective and efficient ways of delineating ILO and non-specific cough from other respiratory conditions in children.

Abstract:Rationale Lung T1 MRI is a potential method to assess cystic fibrosis (CF) lung disease that is safe, quick, and widely available, but there are no data in children with mild CF lung disease. Objective Assess the ability of lung T1 MRI to detect abnormalities in children with mild CF lung disease. Methods We performed T1 MRI, multiple breath washout (MBW), chest computed tomography (CT), and spirometry in a cohort of 45 children with mild CF lung disease (6–11 years of age). Main Results Despite mean normal ppFEV1 values, the majority of children with CF in this study exhibited mild lung disease evident in lung clearance index (LCI) measured by MBW, chest CT Brody scores, and percent normal lung perfusion (%NLP) measured by T1 MRI. The %NLP correlated with chest CT Brody scores, as did LCI, but %NLP and LCI did not correlate with each other. Analysis of the Brody subscores showed that %NLP and LCI largely correlated with different Brody subscores. Conclusions T1 MRI can detect mild CF lung disease in children and correlates with chest CT findings. The %NLP from T1 MRI and LCI correlate with different chest CT Brody subscores, suggesting they provide complementary information about CF lung disease.

Pulmonary function of children with tracheomalacia and associated clinical factors

Abstract:Objectives: Spirometry is easily accessible yet there is limited data in children with tracheomalacia. Availability of such data may inform clinical practice. We aimed to describe spirometry indices of children with tracheomalacia, including Empey index and flow-volume curve pattern, and determine whether these indices relate with bronchoscopic features. Methods: From the database of children with tracheomalacia diagnosed during 2016-2019, we reviewed their flexible bronchoscopy and spirometry data in a blinded manner. We specially evaluated several spirometry indices and tracheomalacia features (cross-sectional lumen reduction, malacic length, and presence of bronchomalacia) and determined their association using multivariable regression. Results: Of 53 children with tracheomalacia, the mean (SD) peak expiratory flow (PEF) was below the normal range [68.9 percent of predicted value (23.08)]. However, all other spirometry parameters were within normal range [Z-score forced expired volume in 1 s (FEV1 ) = -1.18 (1.39), forced vital capacity (FVC) = -0.61 (1.46), forced expiratory flow between 25% and 75% of vital capacity (FEF25%-75% ) = -1.43 (1.10), FEV1 /FVC = -1.04 (1.08)], Empey Index = 8.21 (1.59). The most common flow-volume curve pattern was the "knee" pattern (n = 39, 73.6%). Multivariable linear regression identified the presence of bronchomalacia was significantly associated with lower flows: FEV1 [coefficient (95% CI) -0.78 (-1.54, -0.02)], FEF25%-75% [-0.61 (-1.22, 0)], and PEF [-12.69 (-21.13, -4.25)], all p ≤ 0.05. Other bronchoscopic-defined tracheomalacia features examined (cross-sectional lumen reduction, malacic length) were not significantly associated with spirometry indices. Conclusion: The "knee" pattern in spirometry flow-volume curve is common in children with tracheomalacia but other indices, including Empey index, cannot be used to characterize tracheomalacia. Spirometry indices were not significantly associated with bronchoscopic tracheomalacia features but children with tracheobronchomalacia have significantly lower flow than those with tracheomalacia alone.

Abstract:Background: Little is known about the short-term evolution of lung function in preterm infants. Aim: Compare multiple-breath washout (MBW) and tidal breathing parameters in preterm infants at 36 and 44 postconceptional weeks. Methods: Preterm infants born between 2003 and 2006 had a lung function at 36 and 44 postconceptional weeks. We compared lung clearance index (LCI), FRC per bodyweight (FRC/kg), respiratory rate (RR), tidal volume per bodyweight (TV/kg), minute ventilation per bodyweight (MV/kg) and ratio time to peak tidal expiratory flow to expiratory time (tPTEF/tE)) by paired t-test as well as by mixed model regression analyses, adjusting for length at study visit for values not per bodyweight and CO 2 -sensor for LCI and FRC. Results: We included 44 preterm infants with a mean±SD gestational age of 29±3 weeks, 66% were male. We found no difference for LCI and FRC/kg between 36 and 44 weeks. MV/kg and tPTEF/tE were higher at 36 compared to 44 weeks: mean(SD) MV/kg 474.2 (78.7) ml/kg versus 372.8 (78.9) ml/kg with a mean difference (95% CI) of 109.9 ml/kg (69.1; 150.7 ml/kg, p-value < 0.001), and for tPTEF/tE 0.40 (0.14) versus 0.28 (0.09) with a mean difference of 0.12 (0.09; 0.16 ml/kg, p-value <0.001). The other tidal breathing parameters showed no difference. In the adjusted mixed model, differences in tPTEF/tE and MV/kg remained significant. Conclusion: In our preterm cohort, tPTEF/tE and MV/kg were consistently higher at 36 compared to 44 weeks postconceptional age indicating early adaptive changes of breathing pattern, while the heterogeneous change of MBW outcomes points out the complexity of the underlying pathophysiology.

[Lung Function Measurements Using Body Plethysmography in Young Children with Acute Lower Respiratory Tract Infection].

Abstract:OBJECTIVE:Body plethysmography is a typical method to measure functional residual capacity (FRC) and airway resistance (Raw). The aim of the study was to test the feasibility of measuring lung function with the body plethysmography in young children with acute lower respiratory tract infection (ALRI) by evaluating changes and prognosis of lung function for infants with ALRI with or without wheezing via body plethysmograph.METHOD:Pulmonary function tests (PFTs) were performed by using body plethysmography in 444 children with ALRI, aged 1-36 months, to assess their tidal breathing parameters such as ratio of time to peak tidal expiratory flow to total expiratory time (TPTEF/TE), ratio of volume to peak tidal expiratory flow to total expiratory volume (VPTEF/VE), plethysmographic functional residual capacity (FRCP), FRCP per kilogram (FRCP/kg), specific effective airway resistance (sReff), effective airway resistance (Reff), Reff per kilogram (Reff/kg), etc. According to whether there was wheezing or not, children who had ALRI with wheezing were classified as Group-W, or without wheezing as Group-N. Changes or correlations of tidal breathing parameters and plethysmographic parameters were compared.One hundred and three contemporaneous healthy controls aged 1-36 months underwent the same tests for comparison. And 36 wheezing children accepted PFTs at follow-up in recovery phase.RESULT:Mean values of TPTEF/TE in Group-W,Group-N and the Control respectively were (20.5 ± 6.7)%,(22.8 ± 6.5)%,(34.6 ± 5.0)% (F = 110.500, P < 0.001), while VPTEF/VE respectively were (23.0 ± 6.3)%,(25.2 ± 6.8)%,(34.5 ± 4.2)% (F = 107.800, P < 0.001). Compared to the Control,Group-W and Group-N had significantly higher values of FRCP (226 vs. 176 vs. 172 ml, χ(2) = 64.870, P < 0.001), FRCP/kg(24.40 vs.17.80 vs.17.60 ml/kg,χ(2) = 68.890, P < 0.001), sReff(1.00 vs. 0.52 vs. 0.46 kPa·s,χ(2) = 75.240, P < 0.001), Reff (3.90 vs.2.74 vs.2.20 kPa·s/L, χ(2) = 36.480, P < 0.001) and Reff/kg [0.42 vs. 0.29 vs.0.22 kPa·s/(L·kg), χ(2) = 29.460, P < 0.001]. Although 25 (12.8%) wheezing children with ALRI had normal values of tidal breathing parameters, they already had increased FRCP, FRCP /kg, sReff, Reff and Reff/kg (t = 2.221, 1.997, 2.502, 2.587, 2.539, all P < 0.05). Values of FRCP and Reff in infants caught ALRI were inversely correlated to that of TPTEF/TE and VPTEF/VE (P < 0.05); 36 children with wheezing who accepted PFTs at follow-up had shown significant decline in the specific parameters of plethysmography such as FRCP, FRCP/kg, sReff, Reff and Reff/kg (Z = -1.999, -2.195, -2.038, -1.823, -2.054, all P < 0.05), while no improvement in the main parameters of tidal breathing such as TPTEF/TE.CONCLUSION:Measuring lung function with the body plethysmography in young children with ALRI is feasible. FRC and Raw, as special lung function testing parameters of body plethysmography, were sensitive indicators reflecting impairment of lung function in infants with ALRI (especially for children caught ALRI with wheezing) and shows significant correlation with parameters from lung function testing via tidal breathing. Therefore plethysmography is worthy of clinical promotion.
Assessment of airway reversibility in asthmatic children using forced oscillation technique - A single-center experience from North India

Abstract:Background: Lung function testing is recommended for the management of asthma. Due to certain limitations of conventional spirometry in vulnerable patients, forced oscillation technique (FOT) has been studied with promising results. As there is a paucity of data from developing world, we planned to conduct this study in children using FOT. To assess airway reversibility after inhaled salbutamol in asymptomatic children with suspected asthma. Settings: This study was conducted at pediatric asthma clinic of a tertiary care referral hospital in North India. Design: This was a prospective interventional study over 1-year period. Subjects and methods: Asymptomatic children between 2 and 18 years of age, with history suggestive of asthma, were eligible for participation. Baseline and postbronchodilator pulmonary functions were assessed using FOT. Airway resistance and reactance were monitored at various frequencies. SPSS version 17 was used for statistical analysis. Results: Among the 345 enrolled children, baseline mean ± standard deviation total airway resistance (R5), central airway resistance (R19), peripheral airway resistance (R5-R19), reactance (X5), and resonant frequency (Fres) were 6.85 ± 2.60, 5.23 ± 1.93, 1.6 ± 1.16, and - 2.54 ± 1.36 cmH2O/L/s and 17.28 ± 3.06 Hz. The median (interquartile range) percentage change after inhaled salbutamol was 19.9 (11.40, 29.12), 22.86 (6.88, 38.76), 14.08 (3.40, 22.62), 39.20 (8.20, 62.39), and 15.79 (8.33, 27.27) in respective parameters. All changes were statistically significant. The studied respiratory variables were in maximum negative correlation with height, followed by body mass index. Conclusions: FOT is a simple technique for monitoring lung functions in children during asthma management.

Abstract:Background: Recently, the development of advanced, noninvasive methods has allowed the study of respiratory function even in uncooperative infants. To date, there is still little data on the application of this technique in infants with suspected airway obstruction. The aims of our study were: - To evaluate the role of respiratory function testing (PFR) in the diagnosis and follow-up of infants with stridor - To evaluate the differences between patients with inspiratory stridor and expiratory stridor. - To evaluate the concordance between PFR and endoscopy. Methods: We enrolled infants aged < 1 year with a diagnosis of inspiratory and/or expiratory chronic stridor and a group of healthy controls. For each patient we performed PFR at diagnosis (T0) and for cases at follow-up, at 3 months (T1), 6 months (T2), 12 months (T3). At T0, all patients were classified according to a clinical score, and at follow-up, stature-ponderal growth was assessed. When clinically indicated, patients underwent bronchoscopy. Results: We enrolled 48 cases (42 diagnosed with inspiratory stridor and 6 expiratory stridor) and 26 healthy controls. At T0, patients with stridor had increased inspiratory time (p < 0.0001) and expiratory time (p < 0.001) than healthy controls and abnormal curve morphology depending on the type of stridor. At T0, patients with expiratory stridor had a reduced Peak expiratory flow (p < 0.023) and a longer expiratory time (p < 0.004) than patients with inspiratory stridor. We showed an excellent concordance between PFR and endoscopic examination (k = 0.885, p < 0.0001). At follow-up, we showed a progressive increase of the respiratory parameters in line with the growth. Conclusions: PFR could help improve the management of these patients through rapid and noninvasive diagnosis, careful monitoring, and early detection of those most at risk.

Abstract:Introduction Bronchiolitis is the most common viral lower respiratory tract infection in children under 2 years of age. Respiratory support with high-flow nasal cannula (HFNC) is increasingly used in this patient population with limited understanding of the patients most likely to benefit and considerable practice variability of use. This study aims to understand the factors associated with failure of HFNC support among patients with bronchiolitis and to describe the current practice variations of HFNC use in patients with bronchiolitis in Canadian hospitals including fluid management and parameters to initiate, escalate and discontinue HFNC support. Methods and analysis This is a multicentre retrospective cohort study including hospitalised patients aged 0–24 months with bronchiolitis requiring support with HFNC between January 2017 and December 2021. Clinical data will be collected from patient medical records from Canadian hospitals (n=12), including academic and community centres. HFNC failure will be defined as the need for escalation to non-invasive or invasive mechanical ventilation. Factors associated with HFNC failure will be analysed using logistic regression. Descriptive statistics will be used to describe practice variations of HFNC utilisation and management. Ethics and dissemination Approval from the Research Ethics Boards (REBs) has been obtained for each participating study site prior to onset of data collection including Clinical Trials Ontario for all Ontario hospital sites and REBs from British Columbia Children's Hospital, Stollery Children's Hospital, Montreal Children's Hospital and CHU Sainte-Justine. Study results will be disseminated through presentation at national/international conferences and publication in high-impact, peer-reviewed journals.
