Abstract:Obstructive sleep apnea hypopnea syndrome (OSA) in pregnant women is an under-diagnosed but common condition, due to the numerous physiological changes favoring upper airway collapse. Risk factors such as significant weight gain during the 1st trimester, maternal age and parity should be systematically investigated. Diagnosis is made by sleep recording. OSA can lead to maternal and fetal complications (gestational diabetes, eclampsia, intrauterine growth restriction, prematurity…) during pregnancy, delivery and the post-partum period. It is therefore essential to treat apneic patients as early as possible in pregnancy. Treatment includes hygienic and dietary measures, as well as continuous positive airway pressure (CPAP). Systematic post-partum follow-up with polygraphic monitoring should be proposed.

Abstract:Objectives Obstructive sleep apnea (OSA) is associated with numerous medical comorbidities. Our objective is to establish whether OSA increases adverse maternal and neonatal outcomes. Methods We performed a retrospective cohort study using the Healthcare Cost and Utilization Project – National Inpatient Sample from the United States. All pregnant patients with a diagnosis of OSA were identified using International Classification of Disease-9 coding from 2006 to 2015. Trends of OSA over time were calculated, baseline characteristics were compared between pregnancies with and without OSA, and multivariate logistic regression analysis was used to estimate the effect of OSA on maternal and neonatal outcomes. Results Among 7,907,139 births included in our study, 3,115 were to women with OSA, for an overall incidence of 39 per 100,000 births. Rates increased over the 10-year study period, from 10.14 per 100,000 births in 2006 and 78.12 per 100,000 births in 2015(p<0.05). Women with OSA were more likely to have pregnancies complicated by preeclampsia 2.2 (2.0–2.4), eclampsia 4.1 (2.4–7.0), postpartum hemorrhage 1.4 (1.2–1.7), need of blood transfusion 2.2 (1.9–2.7), disseminated intravascular coagulation, 2.4 (1.0–5.4), and venous thromboembolisms, 2.7 (2.1–3.4). Women with OSA were also more likely to develop cardiovascular and respiratory complications. Neonates born to mothers with OSA were more likely to be premature, 1.3 (1.2–1.5), and have congenital malformations, 2.3 (1.7–3.0). Conclusions OSA in pregnancy is associated with and increased risk of adverse maternal and neonatal outcomes. Women with OSA may benefit from management in Tertiary care centres and closer follow-up during pregnancy.

Abstract:Background: Obstructive Sleep Apnea (OSA) is a common sleep-related breathing disorder, is characterized by recurrent collapse or blockage of the pharynx during sleep that causes intermittent cessation of airflow and a hallmark snoring gasping pattern. When OSA occurs in pregnancy, it is independently associated with increased risk of gestational hypertension, preeclampsia, gestational diabetes and possibly fetal growth restriction and other adverse neonatal outcomes. In particular, OSA affects sleep quality and duration of sleep in pregnant women. The aim was to study the risk of Obstructive Sleep Apnea in pregnant females in different trimester. Methods: The study consists of a non-experimental, quantitative and cross sectional research design. A total of 60 pregnant females were included in the study and conducted at maternity clinic. Results: By analyzing data we found that females in 3rd trimester shows symptoms of compromised breathing and thus are at high risk of developing OSA followed by 2nd trimester. Conclusions: Our study shows that breathing limitation starts from 2nd trimester onwards, so, we conclude that starting breathing exercises early in antenatal period would be beneficial for the expecting mothers. The study concludes that there is high risk of obstructive sleep apnea in 3rd trimester of the pregnancy.
Trends in and outcomes associated with obstructive sleep apnea during deliveries in the United States, 2000-2019

Abstract:Background: Population-level data on obstructive sleep apnea among pregnant women in the United States and associated risk for adverse outcomes during delivery may be of clinical importance and public health significance. Objective: This study aimed to assess trends in and outcomes associated with obstructive sleep apnea during delivery hospitalizations. Study design: This repeated cross-sectional study analyzed delivery hospitalizations using the National Inpatient Sample. Temporal trends in obstructive sleep apnea were analyzed using joinpoint regression to estimate the average annual percentage change with 95% confidence intervals. Survey-adjusted logistic regression models were fit to assess the association between obstructive sleep apnea and mechanical ventilation or tracheostomy, acute respiratory distress syndrome, hypertensive disorders of pregnancy, peripartum hysterectomy, pulmonary edema/heart failure, stillbirth, and preterm birth. Results: From 2000 to 2019, an estimated 76,753,013 delivery hospitalizations were identified, of which 54,238 (0.07%) had a diagnosis of obstructive sleep apnea. During the study period, the presence of obstructive sleep apnea during delivery hospitalizations increased from 0.4 to 20.5 cases per 10,000 delivery hospitalizations (average annual percentage change, 20.6%; 95% confidence interval, 19.1-22.2). Clinical factors associated with obstructive sleep apnea included obesity (4.3% of women without and 57.7% with obstructive sleep apnea), asthma (3.2% of women without and 25.3% with obstructive sleep apnea), chronic hypertension (2.0% of women without and 24.5% with obstructive sleep apnea), and pregestational diabetes mellitus (0.9% of women without and 10.9% with obstructive sleep apnea). In adjusted analyses accounting for obesity, other clinical factors, demographics, and hospital characteristics, obstructive sleep apnea was associated with increased odds of mechanical ventilation or tracheostomy (adjusted odds ratio, 21.9; 95% confidence interval, 18.0-26.7), acute respiratory distress syndrome (adjusted odds ratio, 5.9; 95% confidence interval, 5.4-6.5), hypertensive disorders of pregnancy (adjusted odds ratio, 1.6; 95% confidence interval, 1.6-1.7), stillbirth (adjusted odds ratio, 1.2; 95% confidence interval, 1.0-1.4), pulmonary edema/heart failure (adjusted odds ratio, 3.7; 95% confidence interval, 2.9-4.7), peripartum hysterectomy (adjusted odds ratio, 1.66; 95% confidence interval, 1.23-2.23), and preterm birth (adjusted odds ratio, 1.2; 95% confidence interval, 1.1-1.2). Conclusion: Obstructive sleep apnea diagnoses are increasingly common in the obstetrical population and are associated with a range of adverse obstetrical outcomes during delivery hospitalizations.

Abstract:Abstract Introduction Obstructive sleep apnea (OSA) has traditionally been underdiagnosed and undertreated in pregnancy. Untreated OSA has been associated with adverse peripartum outcomes, such as hypertensive disorders of pregnancy, diabetes, cesarean delivery and NICU admissions. In August 2023, a taskforce from the Society of Anesthesia and Sleep Medicine and the Society for Obstetric Anesthesia and Perinatology published recommendations that pregnant patients with obesity, gestational or pre-existing diabetes, or hypertensive disorders of pregnancy be screened for OSA. Currently, our Maternal-Fetal Medicine (MFM) group only screens for OSA in the obese pregnant population on a conditional basis. It is unknown how many of these patients are screened for OSA and/or referred to sleep medicine for follow-up. Methods This is a cross-sectional retrospective study to characterize the 19,439 pregnancies from January 1, 2012-December 31, 2022 at a rural health system with five labor and delivery units that would meet criteria for screening for OSA and a sleep medicine referral. Patients were included if they were over 18, pregnant and had at least a hypertensive disorder of pregnancy, diabetes (pre-existing or gestational) or were obese. The Cochrane-Armitage trend test was used to evaluate the rates over the time-period. Results In the initial data review, we found that between 2012 and 2022, the practice grew considerably; there was an increase in the proportion of women with gestational diabetes (16.9-31.2%, p< 0.01), obesity (75.0-75.2%, p< 0.01) and a hypertensive disorder of pregnancy (22.9-25.8%, p< 0.01). Despite the increase in sleep medicine referrals during pregnancy (0.4-1.1%, p< 0.01) over the years, the overall referral rate to sleep medicine regardless of the selected peripartum co-morbidity remained low at 0.5-0.9%. Conclusion Less than 1% of patients at high-risk for OSA in pregnancy are being screened or tested for OSA. Based on the guidelines published in August 2023, these results establish a wide practice gap in our center. It is likely that this knowledge is generalizable to centers around the country. We conclude that there is a large opportunity to improve our maternal outcomes by connecting these at-risk patients with sleep medicine referrals to be screened and treated for OSA. Support (if any)

Abstract:Objective To explore the clinical characteristics of obstructive sleep apnea-hypopnea syndrome (OSAHS) diagnosed during pregnancy. Methods Data of 12 patients with OSAHS diagnosed by nocturnal blood oxygen saturation (SaO2) monitoring and polysomnography (PSG) during pregnancy from May 2008 to Oct 2011 at Peking University People's Hospital were analyzed retrospectively.Results All the 12 patients were snorers, one with OSAHS family history, one with hypothyroidism, the other 10 with hypertensive disorders complicating pregnancy (HDCP),8 cases with BMI≥25 kg/m2 before pregnancy. Ten patients received continuous positive airway pressure (CPAP ) treatment with good compliance under informed consent. The mean SaO2 (MSaO2) and the lowest SaO2 (LSaO2) during sleep after treatment had significantly increased than that of before treatment (96.29±0.63) % vs (94.32±1.14 )%( P<0.001), (89.5 ±5.28 )% vs( 80.85±9.42 )% (P<0.05);while the number of oxygen desaturation ≥4% per hour (DI4) significantly decreased (0.96±1.00 vs 11.16±12.06, P<0.05).Two pregnency ended before 28 gestational weeks due to exacerbated HDCP. The case number of preterm labor, concurrent gestational diabetes and postpartum hemorrhage were 5, 6 and 2,respectively. The average weight of 10 live births were (3 231.7±702.5)g (1 500～4 070)g, nine patients had taken CPAP therapy after thediagnosis of OSAHS, there were no fetal growth retardation nor neonatal asphyxia, 5 premature newborns and one term newborn with hypoglycemia were transferred to NICU. Conclusions Pregnant women who have OSAHS risk factors should be paid attention to during the perinatal care and nocturnal SaO2 monitoring or PSG should be performed in the suspected OSAHS patients. The incidence of HDCP, gestational diabetes mellitus and preterm labor were relatively high in OSAHS patients. CPAP was a safe and effective treatment to OSAHS patients in pregnancy.
Sleep-disordered breathing in pregnancy: a review of the physiology and potential role for positional therapy

Abstract:Background: Sleep disordered breathing (SDB) has been shown to be associated with negative clinical sequelae such as systemic hypertension and cardiovascular disease. It has been reported in the literature that approximately 53-56 % of patients with obstructive sleep apnea (OSA) have positional OSA, known as POSA. Potential treatment for sdb in pregnancy: These findings have led to a resurgence of positional therapy as an effective treatment for select patients with SDB. Pregnant patients can also be diagnosed with SDB, the negative consequences of which not only pertain to the patient but to the unborn fetus as well. Despite this, however, SDB is under-diagnosed in pregnant patients and research looking at SDB in pregnant patients and potential therapies are scarce. Conclusions: This article reviews the literature regarding the physiologic respiratory changes that occur during pregnancy, SDB in pregnancy, supine hypotensive syndrome (SHS), complications and current treatments for these events and potential roles for positional therapy in pregnant women whose problems may be specifically position dependent.

Screening Tools for Obstructive Sleep Apnea in Pregnant Women: An Extended and Updated Systematic Review and Meta-analysis

Abstract:The prevalence of obstructive sleep apnea syndrome (OSA) increases in women during pregnancy and negatively affects maternal and fetal outcomes. The updated systematic review and meta-analysis aimed to evaluate the validity of the Berlin, STOP-Bang, and Epworth sleepiness scale (ESS) questionnaires in detecting OSA in pregnant women. PubMed, Embase, and Web of Science were searched systematically up to March 2022. After eligible studies inclusion, two independent reviewers extracted demographic and clinical data. Bivariate random effects models were used to estimate the pooled accuracy measures including sensitivity and specificity, positive (PPV) and negative predictive values (NPVs), diagnostic odds ratio (DOR), and receiver operating characteristic curve (ROC) curve. We included 8 studies including 710 pregnant women with suspected OSA. The performance values of Berlin, STOP-Bang, and ESS questionnaires were as follows: the pooled sensitivity were 61% (95% confidence interval (CI): 40%-80%), 59% (95% CI: 49%-69%), and 29%, (95% CI: 10%-60%); pooled specificity were 61% (95% CI: 42%-78%), 80% (95% CI: 55%-93%), and 80% (95% CI: 50%-94%); pooled PPVs were 60% (95% CI: 0.49-0.72), 73% (95% CI: 61%-85%), and 59% (95% CI: 31%-87%); pooled NPVs were 60% (95% CI: 0.49-0.71), 65% (95% CI: 54%-76%), and 53% (95% CI: 41%-64%); and pooled DORs were 3 (95% CI: 1-5), 6 (95% CI: 2-19), and 2 (95% CI: 1-3), respectively. It seems that the Berlin, STOP-Bang, and ESS questionnaires had poor to moderate sensitivity and specificity in pregnancy, with the ESS showing the worst characteristics. Further studies are required to evaluate the performance of alternative screening methods for OSA in pregnancy.

Abstract:PurposeSleep disturbances such as insomnia, nocturnal awakenings, restless legs syndrome, habitual snoring, and excessive daytime sleepiness are frequent during pregnancy, and these have been linked to adverse maternal and fetal outcomes.MethodsA prospective observational study was performed in high-risk Indian pregnant women. We used modified Berlin questionnaire (MBQ), Pittsburgh sleep quality index (PSQI), International Restless Legs Syndrome Study Group 2011 criteria, and Epworth sleepiness scale to diagnose various sleep disorders, such as symptomatic OSA, poor sleep quality and insomnia, RLS, and excessive daytime sleepiness, respectively, in successive trimesters of pregnancy. Outcome variables of interest were development of gestational hypertension (GH), gestational diabetes mellitus (GDM), and cesarean delivery (CS); the Apgar scores; and low birth weight (LBW). The relationship between sleep disorders and outcomes was explored using logistic regression analysis.ResultsOutcome data were obtained in 209 deliveries. As compared to nonsnorers, women who reported snoring once, twice, and thrice or more had odds ratios for developing GH—4.0 (95 % CI 1.3–11.9), 1.5 (95 % CI 0.5–4.5), and 2.9 (95 % CI 1.0–8.2) and for undergoing CS—5.3 (95 % CI 1.7–16.3), 4.9 (95 % CI 1.8–13.1), and 5.1 (95 % CI 1.9–14.9), respectively. Pregnant women who were persistently positive on MBQ had increased odds for GH and CS.ConclusionsSnoring and high-risk MBQ in pregnant women are strong risk factors for GH and CS. In view of the significant morbidity and health care costs, simple screening of pregnant women with questionnaires such as MBQ may have clinical utility.

Continuous Positive Airway Pressure Treatment and Hypertensive Adverse Outcomes in Pregnancy: A Systematic Review and Meta-Analysis

Abstract:Importance: Pregnancy may contribute to the development or exacerbation of obstructive sleep apnea (OSA) and increase the risk of gestational complications. Continuous positive airway pressure (CPAP) is the first-line and criterion standard treatment for OSA and is regarded as the most feasible choice during pregnancy. However, the association between CPAP therapy in pregnant women with OSA and reduced gestational complications remains inconclusive. Objective: To investigate the association between CPAP therapy in pregnant women with OSA and the reduction of adverse hypertensive outcomes during gestation. Data sources: Keyword searches of PubMed, Embase, and the Cochrane Database of Systematic Reviews and Clinical Trials were conducted from inception to November 5, 2023. Study selection: Original studies reporting the treatment effect of CPAP use on lowering hypertension and preeclampsia risks in pregnant women with OSA were selected. Data extraction and synthesis: The Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guideline was followed in the reporting of reviews. Data were independently extracted by 2 authors. Random-effects model meta-analyses were performed and risk ratios (RRs) reported. Subgroup analysis, meta-regression based on age and body mass index (BMI; calculated as weight in kilograms divided by height in meters squared), and publication bias assessment were also conducted. Main outcome and measures: The primary outcome was the RR of gestational hypertension and preeclampsia between pregnant women with OSA receiving CPAP treatment and those who did not receive CPAP treatment. Results: Six original studies in 809 participants (mean age, 31.4 years; mean BMI, 34.0) were identified and systematically reviewed for meta-analysis. The pooled results showed significant differences between the intervention (CPAP use) and the control (non-CPAP use) groups in reducing the risk of gestational hypertension (RR, 0.65; 95% CI, 0.47-0.89; P = .008) and preeclampsia (RR, 0.70; 95% CI, 0.50-0.98; P = .04). Meta-regression revealed that patients' age (coefficient, -0.0190; P = .83) and BMI (coefficient, -0.0042; P = .87) were not correlated with reduction of risk of hypertension and preeclampsia. Conclusions and relevance: These findings suggest that implementing CPAP treatment in pregnant women with OSA may reduce the risk of gestational hypertension and preeclampsia.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3><p>Obstructive sleep apnoea (OSA) appears common in pregnancy. Complications of pregnancy such as gestational diabetes and hypertension predispose women to cardiometabolic disease in later life. It is unknown if snoring during pregnancy is a risk marker for later-life OSA.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>We analysed data from N=897 women in the Sleep Apnea Global Interdisciplinary Consortium (SAGIC), which recruited patients attending sleep clinics at 11 sites. There were 577 cases with current OSA and 320 controls. Cases were further categorised into mild, moderate, and severe OSA based on apnoea-hypopnoea index. Retrospective self-report of snoring during pregnancy was the exposure of interest and was reported by 2.9% of cases and 3.4% of controls.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>Multinomial regression demonstrated that snoring during a previous pregnancy was not significantly associated with mild (OR 0.34, 95% CI 0.09–1.25, p=0.10), moderate (OR 0.69, 95% CI 0.21-2.19, p=0.52), or severe OSA (OR 1.86, 95% CI 0.77-4.48, p=0.17) compared to no snoring during pregnancy. Results were unchanged after adjustment for age, body mass index, and ethnicity. 79% of women reported current snoring but all who snored during pregnancy reported current snoring.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>Women who snore during pregnancy continue snoring in later-life but do not appear more likely to develop OSA. These findings are limited by self-reported data, recall bias, and small numbers of women who reported snoring during pregnancy. A prospective study with objective measurement of sleep and snoring during pregnancy is needed to examine the links between sleep disorders in pregnancy with health in later life.</p>

Abstract:Extract Sleep disordered breathing (SDB) during pregnancy has been associated with various adverse pregnancy outcomes and is increasingly gaining recognition as a significant yet often overlooked aspect of maternal health. Notably, accumulating evidence from prospective cohort and clinical studies has shown robust associations between SDB during pregnancy and hypertensive disorders of pregnancy, pre-eclampsia and gestational diabetes [1–3]. Studies using polysomnography indicate that ~30% of individuals develop SDB as they advance from early to late pregnancy [4, 5], and that this prevalence is much greater than in non-pregnant women aged 30–49 years with (13.5%) and without obesity (4.2%) [6].
