Does realizing strengths, insight, and behavioral practice through a psychological intervention promote personality change? An intensive longitudinal study

Abstract:The mechanisms of change underlying the effectiveness of personality change interventions are largely unclear. In this study, we used data from a three-month digital intervention with an intensive longitudinal design to test whether a greater realization of general change factors is partly responsible for personality change. Participants ( N = 679, 53.0% female; age: M = 25.3 years, SD = 7.1) seeking to increase either Emotional Stability, Conscientiousness, or Extraversion provided self-ratings on their weekly personality states and the three generic change factors of strengths, insights, and behavioral practice. We found a single-factor structure of change factors within and between individuals. Results showed within-person increases in Emotional Stability, Extraversion, and Conscientiousness states as well as increases in change factors across the intervention. Changes in personality states were coupled with changes in generic change factors. Finally, the results provide initial support for the hypothesis that the realization of general change factors is partly responsible for the effects of the intervention. Within-person increases in the change factors were associated with subsequent increases in Extraversion and Emotional Stability states during the following week. The present findings highlight the need to better understand how and why people change in personality as a result of interventions.

Abstract:OBJECTIVE: The present paper elaborates a process perspective of change in psychotherapy for personality disorders (PDs). Firstly, the paper reviews the literature of mechanisms of change in treatments of PD, with the main focus on emotional processing and socio-cognitive processing. Secondly, it proposes an illustrative case-series analysis of eight cases, drawn from a mediation analysis conducted within the context of a randomized controlled trial for borderline personality disorder (BPD).METHOD: As such, cases with good and poor outcomes are compared, as are cases with poor and good intake features and cases with poor and good process markers across treatment.RESULTS: The results illustrate possible pathways to healthy change over the course of four months of treatment, and possible pathways of the absence of change.CONCLUSIONS: These results are discussed with regard to three main research perspectives: the combination of qualitative and quantitative methodology in psychotherapy research may be applied to case study research, a neurobehavioral perspective on change may incorporate the individualized experience in the laboratory and therapist responsiveness to patient characteristics may be a core feature of fostering change. Clinical or methodological significance of this article: The present paper illustrates individual pathways to change in personality disorders. It illustrates how coping capacities influence the process of psychotherapy and outcome in personality disorders. It demonstrates the relevance of individualizing treatments for personality disorders. It demonstrates several integrative features of psychotherapy research, in particular the use of neurobehavioral paradigms and the integration of single-case research within randomized controlled trials.

A Systematic Review of Personality Trait Change Through Intervention

Abstract:The current meta-analysis investigated the extent to which personality traits changed as a result of intervention, with the primary focus on clinical interventions. We identified 207 studies that had tracked changes in measures of personality traits during interventions, including true experiments and prepost change designs. Interventions were associated with marked changes in personality trait measures over an average time of 24 weeks (e.g., d = .37). Additional analyses showed that the increases replicated across experimental and nonexperimental designs, for nonclinical interventions, and persisted in longitudinal follow-ups of samples beyond the course of intervention. Emotional stability was the primary trait domain showing changes as a result of therapy, followed by extraversion. The type of therapy employed was not strongly associated with the amount of change in personality traits. Patients presenting with anxiety disorders changed the most, and patients being treated for substance use changed the least. The relevance of the results for theory and social policy are discussed.

Abstract:With the dimensional shift, personality pathology is now commonly conceptualized using a combination of personality functioning and (pathological) personality traits. Personality functioning has been deemed more sensitive to treatment than the specific trait combination of personality problems. To empirically examine just that, the goal of this pilot study was to simultaneously compare changes in personality functioning (LPFS-BF 2.0), pathological traits (PID-5-BF), and normal-range traits (BFI-2) among individuals receiving integrative, dynamic-relational psychotherapy (baseline n = 52, follow-up n = 31) and a matched control group ( n = 31). The results showed that clients had stronger changes in personality functioning than in traits when compared to the control group. In addition, clients lower on personality functioning were more inclined to drop-out of therapy. This study points to the unique clinical utility of personality functioning and provides a foundation for future research focusing on the sensitivity of personality functioning and personality traits to changes within the context of psychotherapy.

Abstract:Significance Personality traits have consequences and are malleable throughout the lifespan. However, it is unclear if and how personality traits can be changed in desired directions. A 3-mo digital personality change intervention was deployed, and a large-scale randomized controlled trial ( n = 1,523) was conducted to examine the effects of intended personality change in a nonclinical sample. The intervention group showed greater changes than the control group, and changes aligned with intended goals for change. Observers also perceived personality changes, but reported changes were less pronounced. Moreover, self- and observer-reported changes persisted until 3 mo after the end of the intervention. These findings provide the strongest evidence to date that normal personality traits can be changed through intervention in nonclinical samples.

Abstract:Recent intervention research has shown that personality traits can be modified through psychological interventions. However, it is unclear whether reported effects represent changes in the trait domain or only some facets or items. Using data ( N = 552) from a recent intervention trial, the present study examined the effects of a digital-coaching intervention on self- and observer-reported personality facets and items. We focused on participants who wanted to decrease in Negative Emotionality, increase in Conscientiousness or increase in Extraversion. We used measurement invariance testing to examine which level of the trait domain hierarchy changed during the intervention. For the self-reports, we found some heterogeneity in the effects on all three trait domains, but most notably Extraversion and Conscientiousness. Specifically, participants reported to increase strongly on sociability (Extraversion), and moderately on productiveness and organization (Conscientiousness), but not on the other facets of these trait domains. Observers generally reported small but non-significant changes, with no scalar invariance violations except for Extraversion. Overall, this suggests considerable heterogeneity in intervention-related personality change that can be overlooked if only focusing on the trait domain level. We discuss the relevance of measurement invariance testing and measurement approaches for personality development and intervention research.

Abstract:Outcome research in counselling and psychotherapy has come to rely heavily on the use of self‐report questionnaire measures of anxiety, depression and general symptomatology. The validity of these measures in the context of psychotherapy research has, in general, been taken for granted. This paper proposes that, despite the success of self‐report questionnaires in personality research and personnel selection, there are significant problems involved in the use of these methods in research into therapeutic change. Specifically, three areas of difficulty are reviewed. First, questionnaires are completed in a social setting that itself changes as a result of therapy. Second, the experience of psychotherapy changes the way that clients make sense of questionnaire items (the ‘response shift’ phenomenon). Third, the conceptualisation of the person implicit in questionnaire design is not compatible with the conceptualisation of persons espoused by most contemporary therapeutic approaches. It is argued that the continuing popularity of self‐report questionnaires can be understood in terms of Kurt Danziger's model of the social construction of psychological knowledge.
The effects of a personality intervention on satisfaction in 10 domains of life: Evidence for increases and correlated change with personality traits.

Abstract:The desire to change one's personality traits has been shown to be stronger if people are dissatisfied with associated aspects of their life. While evidence for the effects of interventions on personality trait change is increasing, it is unclear whether these lead to subsequent improvements in the satisfaction with various domains of life. In this study, we examined the effects of a 3-month digital-coaching personality change intervention study on 10 domains of satisfaction. We focused on the three largest intervention groups of the study (<i>N</i> = 418), which included participants who wanted to increase their Emotional Stability, Conscientiousness, or Extraversion. Bivariate latent change score models were used to examine correlated change between the targeted personality traits and satisfaction domains. We found that global life satisfaction and satisfaction with oneself as a person increased in all three intervention groups. In addition, increases in specific satisfaction domains were reported for the Conscientiousness (e.g., work/school, health, friendships) and Emotional Stability (e.g., family, sexual relationships, emotions) group. Increases were stable up to the 3-month follow-up. In contrast, the waitlist control group did not report any changes in global or domain-specific life satisfaction. Changes in the satisfaction domains were positively correlated with self-reported personality trait change to a similar degree as the cross-sectional associations, but not to observer-reported personality trait change. The personality intervention thus seemed to have a positive effect on satisfaction with various domains of life, which was associated with the degree of self-reported personality trait change. Abstract:Measurement in personality development faces many psychometric problems. First, theory–based measurement models do not fit the empirical data in terms of traditional confirmatory factor analysis. Second, measurement invariance across age, which is necessary for a meaningful interpretation of age–associated personality differences, is rarely accomplished. Finally, continuous moderator variables, such as age, are often artificially categorized. This categorization leads to bias when interpreting differences in personality across age. In this tutorial, we introduce methods to remedy these problems. We illustrate how Ant Colony Optimization can be used to sample indicators that meet prespecified demands such as model fit. Further, we use Local Structural Equation Modeling to resample and weight subjects to study differences in the measurement model across age as a continuous moderator variable. We also provide a detailed illustration for both tools with the Neuroticism scale of the openly available International Personality Item Pool – NEO inventory using data from the UK sample ( N = 15 827). Combined, both tools can remedy persistent problems in research on personality and its development. In addition to a step–by–step illustration, we provide commented syntax for both tools. © 2019 European Association of Personality Psychology

Predictors for improvement in personality functioning during outpatient psychotherapy: A machine learning approach within a psychodynamic psychotherapy sample

Abstract:Background Since its introduction in the diagnostic manuals DSM-5 and ICD-11, the construct of personality functioning has gained increasing attention. However, it remains unclear which factors might predict improvement in personality functioning. Methods We examined a sample of 648 completed psychodynamic psychotherapies conducted by 172 therapists at the Heidelberg Institute for Psychotherapy. A machine learning approach was used to filter for variables that are relevant for the prediction of the improvement of personality functioning from a broad data set of variables collected at the beginning of each psychodynamic psychotherapy. Results On average, we found an improvement of 0.24 (SD = 0.48) in the OPD-SQ. This corresponds to a medium effect in the improvement of personality functioning. Patients with initially high impairment experienced particularly large improvements. Overall, we found a large number of variables that proved to be predictive for the improvement of personality functioning. Limitations in social activity due to physical and emotional problems proved to be one of the most important predictors of improvement. Most of the effect sizes were small. Conclusions Overall, the improvement in personality functioning during psychotherapy is determined more by the sum of a large number of small effects than by individual variables. In particular, variables that capture social areas of life proved to be robust predictors.

Who in the world is trying to change their personality traits? Volitional personality change among college students in six continents.

Abstract:Recent research conducted largely in the United States suggests that most people would like to change one or more of their personality traits. Yet almost no research has investigated the degree to which and in what ways volitional personality change (VPC), or individuals' active efforts toward personality change, might be common around the world. Through a custom-built website, 13,278 college student participants from 55 countries and one of a larger country (Hong Kong, S.A.R.) using 42 different languages reported whether they were currently trying to change their personality and, if so, what they were trying to change. Around the world, 60.40% of participants reported that they are currently trying to change their personalities, with the highest percentage in Thailand (81.91%) and the lowest in Kenya (21.41%). Among those who provide open-ended responses to the aspect of personality they are trying to change, the most common goals were to increase emotional stability (29.73%), conscientiousness (19.71%), extraversion (15.94%), and agreeableness (13.53%). In line with previous research, students who are trying to change any personality trait tend to have relatively low levels of emotional stability and happiness. Moreover, those with relatively low levels of socially desirable traits reported attempting to increase what they lacked. These principal findings were generalizable around the world. Abstract:Recent research has found that a vast majority of people want to change their personality traits—and they may be able to find some degree of success in doing so. However, desires for self-change have been theoretically and empirically linked to reduced well-being. The present study utilized a longitudinal design to better understand the associations between people’s desires and attempts to change their personality traits and their psychological well-being. Results indicated that possessing change goals did not necessarily predict growing deficits in well-being over time. In contrast, people who were able to change their personality traits in ways that aligned with their desires tended to experience increases in well-being over time. These findings are consistent with theory that dissatisfaction can precipitate change goals, and successful change can ameliorate dissatisfaction.

Abstract:Introduction: While ample data demonstrate the effectiveness of inpatient psychosomatic treatment, clinical observation and empirical evidence demonstrate that not all patients benefit equally from established therapeutic methods. Especially patients with a comorbid personality disorder often show reduced therapeutic success compared to other patient groups. Due to the heterogeneous and categorical personality assessment, previous studies indicated no uniform direction of this influence. This complicates the derivation of therapeutic recommendations for mental disorders with comorbid personality pathology. Methods: Analyzing n = 2094 patients from German university hospitals enrolled in the prospective "MEPP" study, we tested the dynamic interaction between dimensionally assessed personality functioning and psychopathology of anxiety and depression. Results: Longitudinal structural equation modelling replicated the finding that the severity of symptoms at admission predicts symptom improvement within the same symptom domain. In addition, we here report a significant coupling parameter between the baseline level of personality function and the change in general psychopathology - and vice versa. Discussion and conclusion: These results imply that personality pathology at admission hinders the therapeutic improvement in anxiety and depression, and that improvement of personality pathology is hindered by general psychopathology. Furthermore, the covariance between both domains supports the assumption that personality functioning and general psychopathology cannot be clearly distinguished and adversely influence each other. A dimensional assessment of the personality pathology is therefore recommendable for psychotherapy research and targeted therapeutic treatment.
Four Types of Change and Self‐Other Agreement on Change in Personality Traits During College Years: A Multi‐Informant Longitudinal Study

Abstract:Research in personality trait change has largely relied on mean‐level and rank‐order change across the lifespan. The current research expanded the literature in several ways: analyzing four types of change and correlated change patterns, obtaining multi‐informant reports, including lower‐order personality traits, and collecting multiple assessments during a short yet important time for college‐attending emerging adults (baseline N = 259, Mage = 18.79). There was little evidence for mean‐level change, yet participants showed significant individual differences such that rank‐ordering and ipsative profiles were much more dynamic than mean score patterns. Informant‐reports from close others demonstrated largely similar patterns: little to no mean‐level change, significant increase in rank‐ordering, and about half of participants reporting configural change mostly in elevation and scatter rather than in profile shapes. Interestingly, there was no correlated change between self and other‐reports. This indicated that close others do not share individuals' perception of their own personality trait change, at least not in the demographic group studied. By examining individual‐level, sample‐level, and multi‐informant perspectives, our thorough investigation provided useful benchmarks for future research to examine the source of variability in change trajectories.

Personality and self-reported treatment effectiveness in depression

Abstract:Objective: The aim of the present study was to examine the associations between eight personality styles and retrospective self-reported response to a wide range of treatments for depression, including pharmacological, psychological and alternative strategies. Method: An online survey posted on the Black Dog Institute website was completed by 3486 respondents reporting a history of treatment for depression. Inclusion criteria resulted in a sample of 2692 respondents. Participants completed the Temperament and Personality Questionnaire, which assesses eight personality dimensions: anxious worrying, perfectionism, personal reserve, irritability, social avoidance, rejection sensitivity, self-criticism and self-focus. Results: A series of linear regressions quantified that the personality dimensions--as well as age and gender--accounted for very little of the variance in self-reported treatment effectiveness (1-7%). Self-criticism was associated with a poorer response to most treatments, and social avoidance was associated with a poorer response to psychological treatments (e.g. cognitive behaviour therapy) and behavioural strategies (e.g. exercise). Several other weak associations are reported. Conclusion: The association between personality style and retrospective self-reported treatment response was overall weak at most. The association between a personality style of self-criticism and poor treatment response warrants clarification.
Becoming More Conscientious or More Open to Experience? Effects of a Two–Week Smartphone–Based Intervention for Personality Change

Abstract:Research indicates that it might be possible to change personality traits through intervention, but this clinical research has primarily focused on changing neuroticism. To date, there are no established, proven techniques for changing other domains of personality, such as conscientiousness and openness. This research examined the effects of a two–week smartphone–based intervention to either change one facet of conscientiousness (i.e. self–discipline) or one facet of openness to experience (i.e. openness to action). Two intervention studies (total N = 255) with two active intervention groups for mutual comparisons were conducted. Results of self–reports and observer reports showed that people who wanted to become more self–disciplined were less self–disciplined at pretest. Similarly, people who wanted to become more open to action were less open to action at pretest. The results showed that people who chose the self–discipline intervention showed greater increases in self–discipline, and people who chose the openness to action intervention showed greater increases in openness to action compared with the other group. Changes were maintained until follow–up two and six weeks after the end of the intervention. Future work is needed to examine whether these personality changes are enduring or reflect temporary accentuation as a result of participation in the intervention. © 2020 European Association of Personality Psychology

Current Directions in Psychological Science

Abstract:A highly relevant but provocative research question is whether and how one can intentionally change personality traits through psychological interventions, given that traits are relatively stable by definition. Recently, research has begun to investigate personality change through intervention in nonclinical populations. One attractive and innovative interventional avenue may lie in using digital applications to guide and support people in their desire to change their personality and trigger change processes. This article provides a rationale for nonclinical personality-change interventions and discusses motivations to change, the potential of using digital applications for intervention efforts, key studies that illustrate this emerging field of research, and future directions.

Abstract:In a cross-cultural study using samples from China and Germany(n = 419), short forms of established mental health psychometric instruments were evaluated for their psychometric properties. These instruments were selected because they operationalize the different phases of Howard's phase model of psychotherapy: remoralization, remediation and rehabilitation. The main goal of the study was to determine if the short forms of these instruments could serve as means for quality management in a cross-cultural training project for cognitive behavioural therapy (CBT). Sufficient reliability and validity (convergent validity and sensitivity to pathology) were found in both cultures. At first glance, high intercorrelations of the tests suggested a one-dimensional model of mental health. However, an exploratory factor analysis and theoretical considerations favoured the alternative of differentiating mental health into eight sub-dimensions, whereby each of the sub-dimensions could be linked to basic or social emotions. These findings and their implications for quality management are discussed in some detail.
The Perceived Problems of People With Subclinical Personality Disorders: A Mental Health Literacy Study

Abstract:This study looked at lay theories of how people with personality disorders (PDs) are perceived to cope with their interpersonal relationships. In all, 213 participants read 14 vignettes derived from Oldham's and Morris's (2000) book describing DSM III personality disorders for a popular audience. Participants were invited to do six ratings, including how happy each person in each vignette appeared to be and how successful at establishing long-term relationships. Effect sizes for each question across the 14 vignettes were small to medium. The six ratings factored into a single social adjustment scale, and there were many differences across the PDs on this measure. Those with dependent PD were judged as most successful in their social relationships while those who were schizoid PD were judged as least successful. A similar analysis using the three higher order clusters showed significant differences: Cluster C disordered people were judged as better adjusted than Cluster A people. Limitations of the methodology and implications are discussed.
