Youth Perspectives on the Flint Water Crisis and Its Health Effects

Abstract:Background: While the youth of Flint are at risk from the effects of the water crisis, little is known about their perspective of the impact of the water on their health. Objective: To explore adolescents' perceptions of living in Flint during the water crisis, its impact on their health, and ways to address the crisis and rebuild trust. Methods: In summer 2018, four focus groups were conducted with adolescents in Flint (ages 13 to 17 years). Group sessions were conducted by trained facilitators who were Flint residents along with members of the community-academic research team. Group sessions were audio-recorded and transcribed. Transcripts were reviewed and themes identified by the research team and youth from the focus groups. Results: Of the 53 participants, most identified as Black (66%) and male (64%). Participants expressed concern about the impact of the water on their health. They knew people who developed rashes, experienced cognitive changes, and who died, they believed, owing to the water. The crisis also led to psychological consequences for youth. They expressed ongoing mistrust of the water and of institutions that should protect them, as well as concern that exposure may have long-term effects on their health and their children's health. Despite these concerns, participants expressed resilience and a desire to share that they had the potential to live successful lives. Conclusion: Youth expressed concern about current and long-term health sequelae of the water crisis. Further work is needed to monitor the consequences of the crisis and to identify resident-informed approaches to mitigate its effects and rebuild trust.
Water Is K'é: Learning from the Navajo Community to Promote Early Child Health

Abstract:Background: Drinking water instead of sugary drinks is key to reducing health disparities. Since beverage habits are shaped by complex personal, community, and environmental factors, community input is critical to design any intervention promoting water. Objectives: We worked with community partners to design a program to promote healthy beverage habits among young Navajo children. Methods: The socioecological model, community-based participatory methods, and strengths-based principles shaped our process. In Phase 1, multigenerational feedback taught us about the cultural importance of water and how water quality concerns influence beverage choices. In Phase 2, our Water is K'é Community Advisory Group played a leading role to design the intervention centered around cultural connection, health literacy, and water access. Lessons learned: Water is K'é was created through community partnership. Community listening and mini-pilots take time but allows the program to meet community's needs and interests. Conclusions: The solutions to health disparities lie within the community itself.

Abstract:Youth &Society, Ahead of Print. This paper proposes a new conceptual approach to the development and utilization of youth-led environmental assessments to inform the planning of youth-enabling environments. Interdisciplinary research has established the influential effects of the physical environment on children and adolescents' well-being and development, yet there is a gap in our understanding of how to create everyday environments for youth that speak to their needs and interests and provide opportunities for them to flourish. Engaging youth through participatory action research to both develop and conduct environmental assessments can have positive implications for youth empowerment and well-being while also altering research and planning practices to effectively integrate youth voice. The proposed approach integrates elements from affordance theory, the Capability Approach, and positive youth development within a youth participatory action research framework to create a process that encourages capability formation, fosters positive development, and improves our understanding of what constitutes a youth-enabling environment.

Abstract:The prevalence and pervasive nature of emerging chemicals of concern have created widespread environmental injustice apprehensions in vulnerable communities. To alleviate and address these concerns, identifying, engaging, and training a diverse environmental health research workforce will be critical and necessary steps to combat and prevent the consequences of environmental injustice. While there is an obvious need to enhance diversity in environmental health research, this process is hampered by facets of systemic racism that reduce access to educational resources needed to build interest and knowledge in students and teachers. We present here a historical perspective to offer a guide for building programs and relationships with underserved schools to help overcome limiting factors that have plagued certain public school systems. With the proper training and mentorship, the untapped workforce present within these schools will be empowered to understand and address current and emerging environmental health and safety threats. Through this transformative 8-week high school research program, we will develop well-prepared, ethical researchers committed to scientific inquiry, intensive fieldwork, and collaborative problem solving to address environmental health challenges. Following the four-step risk assessment process, students, teachers, and faculty mentors will work collaboratively to identify toxicants, potential hazards and risks, and environmental disparities in urban neighborhoods, which provides the necessary training to formulate critical thinking skills for use in academic or nonacademic careers.NEW & NOTEWORTHY The Engaging Multidisciplinary Professional Opportunities for Women in Environmental Research (EMPOWER) program is a one-of-a-kind research summer experience for minority female high school students in the state of Georgia. In addition, this program provides high school teachers with hands-on experiences that can be adapted to use in the classrooms. This combination of lab and field research immerses participants in understanding urban environmental exposures and their health effects. The EMPOWER program was established to meet the critical need for increasing diversity in science, technology, engineering, and mathematics.
Teaching Environmental Health Science for Informed Citizenship in the Science Classroom and Afterschool Clubs

Abstract:In the era of growing concerns about human-induced climate change and sustainable development, it is important for the schools to prepare students for meaningful engagement with environmental policies that will determine the future of our society. To do this, educators need to face a number of challenges. These include deciding on the science knowledge and skills needed for informed citizenship, identifying teaching practices for fostering such knowledge and skills, and finding ways to implement new practices into the tightly packed existing curriculum. This paper describes two collaborative efforts between the U.S. National Library of Medicine (NLM) and University of Maryland College of Education that attempt to meet these challenges. The focus of both projects is on helping students develop information seeking and evaluation and argumentation skills, and applying them to complex socio-scientific issues that have bearing on students' daily lives. The first effort involves co-designing an afterschool environmental health club curriculum with an interdisciplinary team of middle school teachers. The second effort is the development and implementation of a week-long school drinking water quality debate activity in a high school environmental science classroom. Both projects center on Tox Town, an NLM web resource that introduces students to environmental health issues in everyday environments. The paper describes successes and challenges of environmental health curriculum development, including teachers' and researchers' perception of contextual constraints in the club and classroom setting, tensions inherent in co-design, and students' experience with socio-scientific argumentation.

Abstract:Arsenic (As) is a naturally occurring chemical element widely distributed in the Earth's crust. Human activities have significantly altered As presence in the environment, posing significant threats to the biota as well as human health. The environmental fates and adverse outcomes of As of various species have been extensively studied in the past few decades. It is imperative to summarize these advances as a whole to provide more profound insights into the As cycle for sustainable development. Embracing the One Health concept, we systematically reviewed previous studies in this work and explored the following three fundamental questions, i.e., what the trends and associated changes are in As contamination, how living organisms interact and cope with As contamination, and most importantly what to do to achieve a sustainable future with As. By focusing on one critical question in each section, this review aims to provide a full picture of the complexity of environmental As. To tackle the significant research challenges and gaps in As pollution and mitigation, we further proposed a One Health framework with potential coping strategies, guiding a coordinated agenda on dealing with legacy As in the environment and ensuring a sustainable As future.
Engaging Youth in Food Activism in New York City: Lessons Learned from a Youth Organization, Health Department, and University Partnership

Abstract:Research indicates that insufficient emphasis on community collaboration and partnership can thwart innovative community-driven work on the social determinants of health by local health departments. Appreciating the importance of enhancing community participation, the New York City Department of Health and Mental Hygiene (DOHMH) helped lead the development of the Health Equity Project (HEP), an intervention aimed at increasing the capacity of urban youth to identify and take action to reduce food-related health disparities. DOHMH partnered with the City University of New York School of Public Health and several local youth organizations to design and implement the intervention. HEP was conducted with 373 young people in 17 cohorts at 14 unique sites: six in Brooklyn, six in the Bronx, and two in Harlem. Partnered youth organizations hosted three stages of work: interactive workshops on neighborhood health disparities, food environments, and health outcomes; food-focused research projects conducted by youth; and small-scale action projects designed to change local food environments. Through these activities, HEP appears to have been successful in introducing youth to the social, economic, and political factors that shape food environments and to the influence of food on health outcomes. The intervention was also somewhat successful in providing youth with community-based participatory research skills and engaging them in documenting and then acting to change their neighborhood food environments. In the short term, we are unable to assess how successful HEP has been in building young leaders who will continue to engage in this kind of activism, but we suspect that more extended interactions would be needed to achieve this more ambitious goal. Experiences at these sites suggest that youth organizations with a demonstrated capacity to engage youth in community service or activism and a commitment to improving food or other health-promoting community resources make the most suitable and successful partners for this kind of effort.

Abstract:Incorporating stakeholder engagement into environmental management may help in the pursuit of novel approaches for addressing complex water resource problems. However, evidence about how and under what circumstances stakeholder engagement enables desirable changes remains elusive. In this paper, we develop a conceptual framework for studying social and environmental changes possible through stakeholder engagement in water resource management, from inception to outcomes. We synthesize concepts from multiple literatures to provide a framework for tracing linkages from contextual conditions, through engagement process design features, to social learning, community capacity building, and behavioral change at individual, group, and group network levels, and ultimately to environmental change. We discuss opportunities to enhance the framework including through empirical applications to delineate scalar and temporal dimensions of social, behavioral, and environmental changes resulting from stakeholder engagement, and the potential for negative outcomes thus far glossed over in research on change through engagement.

Abstract:Growing socioeconomic and structural disparities within and between nations have created unprecedented health inequities that have been felt most keenly among the world’s youth. While policy approaches can help to mitigate such inequities, they are often challenging to enact in under-resourced and marginalized communities. Community-engaged participatory action research provides an alternative or complementary means for addressing the physical and social environmental contexts that can impact health inequities. The purpose of this article is to describe the application of a particular form of technology-enabled participatory action research, called the Our Voice citizen science research model, with youth. An overview of 20 Our Voice studies occurring across five continents indicates that youth and young adults from varied backgrounds and with interests in diverse issues affecting their communities can participate successfully in multiple contributory research processes, including those representing the full scientific endeavor. These activities can, in turn, lead to changes in physical and social environments of relevance to health, wellbeing, and, at times, climate stabilization. The article ends with future directions for the advancement of this type of community-engaged citizen science among young people across the socioeconomic spectrum.

Abstract:With the proliferation of participatory approaches to promoting youth development such as Youth-Led Participatory Action Research (YPAR), it is more important than ever that researchers and practitioners understand how to create curriculum that will authentically engage youth of color. To that end, there are important lessons public health researchers can learn from the fields of community psychology, education, and social work, among others. We explore the benefits of a youth-led research and action curriculum steeped in critical pedagogy and critical race theory focused on public health. Youth of color ( N = 35) between the ages of 13 and 34 years from five U.S. cities were hired as researchers. The majority (57%) of youth were 16 or 17 years, and the gender split was female (57%) and male (43%). Key informant interviews with adults and qualitative data analysis techniques were employed to examine program products and outcomes. Materials were deductively coded for content related to critical pedagogy and race theory (e.g., empowerment, capacity building and community engagement, skill development) as well as conceptualizations of health. Findings indicate young people (a) began to think more consciously about health, what it means to be healthy, and the factors that influence health; (b) developed critical skills; and (c) described feeling empowered. When given resources and space to lead, we found youth to be insightful, resourceful, compassionate, and deeply committed to improving the lives of their peers, family members, and communities.

Abstract:The World Health Organization has labeled the problem of arsenic contamination of groundwater in South Asia as “the largest mass poisoning in human history.” Various technical solutions to the problem fall into one of two broad categories: (i) cleaning contaminated water before human consumption and (ii) encouraging people to switch to less contaminated water sources. In this paper, we review research on the behavioral, social, political, and economic factors that determine the field-level effectiveness of the suite of technical solutions and the complexities that arise when scaling such solutions to reach large numbers of people. We highlight the conceptual links between arsenic-mitigation policy interventions and other development projects in Bangladesh and elsewhere, as analyzed by development economists, that can shed light on the key social and behavioral mechanisms at play. We conclude by identifying the most promising policy interventions to counter the arsenic crisis in Bangladesh. We support a national well-testing program combined with interventions that address the key market failures (affordability, coordination failures, and elite and political capture of public funds) that currently prevent more deep-well construction in Bangladesh.
Applying Social Science to Bring Resident Stakeholders into Pollution Governance: A Rural Environmental Justice Public Health Case Study

Abstract:The purpose of this engaged public sociology study was to use social science to bring resident stakeholders into the process of governing pollution production in a rural community. The community has cancer clusters. Residents have concerns about direct exposure to pollution production in their neighborhood by a steel recycling plant that has been cited numerous times for environmental violations. The facility has been under voluntary remediation since 2009, but neighborhood residents were marginalized from the governance process. This case study details how social science was used to bring neighborhood residents’ concerns about direct exposure to toxic air pollution into remediation governance. A curricula-as-research model was developed to provide an engagement framework that guided the case study as it progressed through a series of six stages over five years. The principal investigator maintained this collaboration by integrating the project into courses, securing small grants, developing an affordable air pollution monitoring method, and convening multiple community meetings. The air monitoring results are analyzed and discussed. Finally, the impact of the case study on the company, the state environmental management agency, local government, the nonprofit partner, and residents’ sense of human agency is evaluated.
West Oakland's Experience in Building Community Power to Confront Environmental Injustice Through California's Assembly Bill 617

Abstract:We explored how air quality management processes associated with Assembly Bill 617 (AB 617) in West Oakland, California, represent a shift in power relationships between government agencies and communities toward the goal of addressing legacies of environmental injustice. We drew from a statewide assessment of community engagement in AB 617's first year, and an analysis of the West Oakland AB 617 process. The first comprised 2 statewide surveys (n = 102 and n = 106), 70 key informant interviews, observation of all AB 617 first-year sites, and analysis of related planning documents. The second comprised 2 rounds of interviews (n = 22 and n = 23, with a total of 19 individuals) and extensive participant observation. Several factors are necessary for pursuing environmental justice: (1) invest in community partnerships and collaborations, (2) honor community knowledge and data, (3) ensure that community constituents share power in environmental governance, and (4) adopt explicit racial justice frameworks. Although still a work in progress, AB 617 offers important lessons for community and policy organizations nationwide engaged in environmental justice.
