Abstract:Background In patients with secondary tricuspid regurgitation (STR), right atrial remodeling (RAR) is a proven marker of disease progression. However, the prognostic value of RAR, assessed by indexed RA volume (RAVi) and reservoir strain (RAS) remains to be clarified. Accordingly, the aim of our study is to investigate the association with outcome of RAR in patients with STR. Methods We enrolled 397 patients (44% men, 72.7±13 years) with mild to severe STR. Complete two-dimensional and speckle-tracking echocardiography analysis of RA and right ventricular (RV) size and function were obtained in all patients. The primary endpoint was the composite of death from any cause and heart failure (HF) hospitalization. Results After a median follow-up of 15 months (IQR: 6-23), the endpoint was reached by 158 patients (39%). Patients with RAS 48 mL/m 2 had significantly lower survival rates compared to patients with RAS ≥13% and RAVi ≤ 48 mL/m 2 (log rank p<0.001). On multivariable analysis, RAS <13% (HR 2.11; 95% CI, 1.43-3.11; p 48mL/m 2 (HR 1.49; 95% CI, 1.01-2.18; p=0.04) remained associated with the combined endpoint, even after adjusting for RV free-wall longitudinal strain (RV-FWLS), significant chronic kidney disease, and NYHA class. STR excess mortality increased exponentially with values of 18.2% and 51.3 mL/m 2 for RAS and RAVi, respectively. In nested models, the addition of RAS and RAVi provided incremental prognostic value over clinical, conventional echocardiographic parameters of RV size and function, and RV-FWLS. Conclusions In patients with STR, RAR was independently associated with mortality and HF hospitalization. Assessment of RAR could improve risk stratification of patients with STR, potentially identifying those who may benefit from optimization of medical therapy and a closer follow up.

Abstract:Objective This study attempted to quantitatively assess left and right ventricular function by serially measuring Tei indices in patients with atrial septal defects before and after Amplatzer closure.Methods Fifteen patients with secondary atrial septal defect undergoing transcatheter Amplatzer closure were prospectively investigated.Serial Tei indices were obtained by measuring the ratio of isovolumic time (isovolumic contraction time IVCT and isovolumic relaxation time IVRT) to the ejection time of the left ventricle (LV) and right ventricle (RV) before,during and 30 days after Amplatzer device implantation.Results The LV Tei index was significantly decreased in the patients of secondary atrial septal defects between 30 days after and before Amplatzer device implantation (0.30±0.07 vs 0.22±0.09,P<0.05).The RV Tei index has no obvious change in the patients of secondary atrial septal defects before,during and 30 days after Amplatzer device implantation.Conclusions The LV function has a tendency of improvement after Amplatzer device implantation in the patients with secondary atrial septal defects.The changes of volume load has no significant influence to the RV Tei index.

Abstract:Abstract Funding Acknowledgements Type of funding sources: None. OnBehalf TRAP Introduction. Secondary Tricuspid Regurgitation (TR) is a turning point in cardiac diseases. Symptoms occurred too late according to the right heart reverse remodelling capabilities. The ideal timing for proposing a valve repair remains a challenge. We sought to analyse characteristics of patients with significant secondary TR, seeking for changes in parameters over time that could be related to the risk of clinical event. Material and method. We designed a prospective observational French multicentre study of 160 patients with significant secondary TR (effective regurgitant orifice area > 30 mm2) and LVEF > 40%; we collected clinical, echocardiographic, and EKG data at baseline, 1- and 2-year follow-up. All echocardiographies were centrally analyzed. The primary outcome was death or hospitalization for heart failure Results. At 2-year 95 patients (59.3%) presented the primary outcome. They had significant morphological and functional alteration of the right heart parameters. Right Atrial Volume Index (RAVI) and right ventricular free wall Strain were respectively 73mL/m2 and -19.8% versus 64.7mL/m2 and 22.3% in the event-free group. None of them had significant group*time interaction. In the multivariable analysis, TAPSE/sPAP ratio > 0.4 (OR = 0.41 95%CL 0.2 to 0.82) associated to RAVI > 60mL/m2 (OR = 2.13 95%CL 0.96 to 4.75) were the two independent parameters that enable the fairest and parsimonious evaluation. Conclusion. We demonstrated that short analysis using RAVI and TAPSE/sPAP almost reached us to an easier patient assessment by limiting the number of involved parameters. Abstract:Clinically significant tricuspid regurgitation (TR) is common and associated with excess mortality. At the same time right ventricular (RV) failure is a complex clinical syndrome that results from many causes, but is often associated with long-term prognosis. Whilst results of isolated tricuspid valve (TV) surgery are often unsatisfactory and limited by the prohibitive risk of most patients, the recent development of percutaneous recovery techniques has opened new scenarios. In consideration of the complexity of the mechanisms that lead to right heart failure and RV dysfunction it is important to understand the real advantages that percutaneous TV treatment can offer, more specifically the effect of TR reduction on RV remodeling in the setting of functional tricuspid regurgitation (fTR).

Prognostic Implications of Right Ventricular Remodeling and Function in Patients With Significant Secondary Tricuspid Regurgitation

Abstract:Background: In patients with significant (moderate and severe) tricuspid regurgitation (TR), the decision to intervene is influenced by right ventricular (RV) size and function. RV remodeling in significant secondary TR has been underexplored. The aim of this study was to characterize RV remodeling in patients with significant secondary TR and to investigate its prognostic implications. Methods: RV remodeling was characterized by transthoracic echocardiography in 1292 patients with significant secondary TR (median age 71 [62-78]; 50% male). Four patterns of RV remodeling were defined according to the presence of RV dilation (tricuspid annulus≥40 mm) and RV systolic dysfunction (tricuspid annulus systolic excursion plane<17 mm): pattern 1, normal RV size and systolic function; pattern 2, dilated RV with preserved systolic function; pattern 3, normal RV size with systolic dysfunction; and pattern 4, dilated RV systolic dysfunction. The primary end point was all-cause mortality and the event rates were compared across the 4 patterns of RV remodeling. Results: A total of 183 (14%) patients showed pattern 1 RV remodeling; 256 (20%) showed pattern 2; 304 (24%) presented with pattern 3; and 549 (43%) had pattern 4 RV remodeling. Patients with pattern 4 RV remodeling were more frequently male; more often had coronary artery disease, worse renal function, and impaired left ventricular ejection fraction; and were more often symptomatic. Only 98 (8%) patients underwent tricuspid valve annuloplasty during follow-up. During a median follow-up of 34 (interquartile range, 0-60) months, 510 (40%) patients died. The 5-year survival rate was significantly worse in patients presenting with patterns 3 and 4 RV remodeling in comparison with pattern 1 (52% and 49% versus 70%; P=0.002 and P<0.001, respectively), and were independently associated with poor outcome on multivariable analysis. Conclusions: In patients with significant secondary TR, patients with RV systolic dysfunction have worse clinical outcome regardless of the presence of RV dilation.

Abstract:The efficacy of transcatheter edge-to-edge tricuspid valve repair (TTVR) has been recently demonstrated not only for the reduction of tricuspid regurgitation (TR) but also in terms of right ventricular remodeling. We report the case of a 78-year-old female patient with a history of atrial fibrillation and severe isolated TR in New York Heart Association functional classification III despite optimal medical therapy. These case images help to illustrate immediate and considerable right ventricular reshaping due to successful TTVR with a sustained result at 6 months in isolated primary TR.

Abstract:BACKGROUND: Functional tricuspid regurgitation (TR) can develop either because of right ventricular (RV) remodeling (ventricular functional TR) and/or right atrial dilation (atrial functional TR).OBJECTIVES: This meta-analysis aimed to investigate the association between right heart remodeling and long-term (>1 year) all-cause mortality in patients with significant TR (at least moderate, ≥2+).METHODS: MEDLINE, ISI Web of Science, and SCOPUS databases were searched. Studies reporting data on at least 1 RV functional parameter and long-term all-cause mortality in patients with significant TR were included. This study was designed according to PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) requirements.RESULTS: Out of 8,902 studies, a total of 14 were included, enrolling 4,394 subjects. The duration of follow-up across the studies varied, ranging from a minimum of 15.5 months to a maximum of 73.2 months. Overall, long-term all-cause mortality was 31% (95% CI: 20%-41%; P ≤ 0.001). By means of meta-regression analyses, an inverse relation was found between tricuspid annular plane systolic excursion (11 studies enrolling 3,551 subjects, -6.3% [95% CI: -11.1% to -1.4%]; P = 0.011), RV fractional area change (9 studies, 2,975 subjects, -4.4% [95% CI: -5.9% to -2.9%]; P < 0.001), tricuspid annular dimension (7 studies, 2,986 subjects, -4.1% [95% CI: -7.6% to -0.5%]; P = 0.026), right atrial area (6 studies, 1,920 subjects, -1.9% [95% CI: -2.5% to -1.3%]; P < 0.001) and mortality.CONCLUSIONS: RV dysfunction parameters are associated to worse clinical outcomes in patients with TR, whereas right atrial dilatation is linked to a better prognostic outcome. Further studies are needed to unravel the pathophysiological differences within the functional TR spectrum. Abstract:Objectives: Repair outcomes of tricuspid regurgitation (TR) associated with ischaemic mitral regurgitation (IMR) are inferior to functional TR in terms of TR recurrence and right ventricular (RV) reverse remodelling. Our objective is to analyse right versus left heart reverse remodelling after surgery for IMR-associated TR. Methods: From 2001 to 2011, 568 patients with severe IMR underwent mitral valve surgery (repair 87%, replacement 13%), and 131 had concomitant tricuspid valve repair. Median follow-up was 3.0 years; 25% of living patients were followed up for 6.3 years. Longitudinal analysis of 1527 follow-up echocardiograms was performed to assess ventricular reverse remodelling and function. Results: Unlike the left heart, the right heart failed to reverse remodel (failed to recover ventricular function or halt dilatation). During follow-up after surgery, the right ventricle continued to dilate while the left ventricle regressed in size. RV ejection fraction decreased (46% at 1 month and 44% at 5 years), while left ventricular ejection fraction increased (33% and 37%, respectively). RV strain showed early (-11% at 1 month) and late (-12% at 5 years) dysfunction. Patients who underwent tricuspid valve repair had worse RV function. Mitral regurgitation remained stable after surgical intervention, and TR gradually recurred (37% moderate, 20% severe at 7 years). Conclusions: Surgical treatment of IMR and TR along with revascularization failed to induce reverse remodelling of the right heart. These findings warrant further investigations to identify optimal timing and approach of intervention for IMR-associated TR with respect to RV remodelling.

Abstract:Characterizing flow within the right heart (RH) is particularly challenging due to its complex geometries. However, gaining insight into RH fluid dynamics is of extreme diagnostic importance given the high prevalence of acquired and congenital heart diseases with impaired RH functionality. In this study, we present a comprehensive, patient-specific, image-based computational analysis of the hemodynamics of the RH in healthy and repaired-ToF (ToF) cases. From multi-series cine-MTRI, we reconstruct the geometry and motion of the patient’s right atrium, ventricle, and pulmonary and tricuspid valves. For this purpose, we develop a novel and flexible reconstruction procedure that enables us, for the first time, to integrate fully patient-specific tricuspid valve dynamics into a computational model, enhancing the accuracy of our RH blood flow simulations. This work provides novel insight into the altered hemodynamics of repaired-ToF RH with severe pulmonary regurgitation and into the hemodynamics changes induced by the pulmonary valve replacement intervention. Modelling the whole RH enables us to understand the contribution of the superior and inferior vena cava inflows to the ventricular filling, as well as the impact of the impaired right atrial function on the ventricular diastole. We analyze the turbulent and transitional behaviour by including the Large-Eddies Simulation sigma model in our computational framework. This highlights how the contribution of the smallest scales in the dissipation of the turbulent energy changes among health and disease.

Abstract:Abstract Aims Characterize the impact of residual tricuspid regurgitation (TR) on right ventricle (RV) remodeling and clinical outcomes after transcatheter tricuspid valve intervention. Methods We performed a single‐center retrospective analysis of transcatheter tricuspid valve repair (TTVr) or replacement (TTVR) patients. The primary outcomes were longitudinal tricuspid annular plane systolic excursion (TAPSE), fractional area change (FAC), pulmonary artery systolic pressure (PASP), and RV dimensions (RVd). We used multivariable linear mixed models to evaluate association with replacement versus repair and degree of TR reduction with changes in these echo measures over time. Multivariable Cox regression was used to identify associations between changes in these echo measures and a composite clinical outcome of death, heart failure hospitalization, or re‐do tricuspid valve intervention. Results We included a total of 61 patients; mean age was 77.5 ± 11.7 and 62% were female. TTVR was performed in 25 (41%) and TTVr in 36 (59%). Initially, 72% ( n = 44) had ≤ severe TR and 28% ( n = 17) had massive or torrential TR. The median number of follow up echos was 2: time to 1st follow‐up was 50 days (interquartile range [IQR]: 20, 91) and last follow‐up was 147 (IQR: 90, 327). Median TR reduction was 1 (IQR: 0, 2) versus 4 (IQR: 3, 6) grades in TTVr versus TTVR ( p < 0.0001). In linear mixed modeling, TTVR was associated with decline in TAPSE and PASP, and TR reduction was associated with decreased RVd. In multivariable Cox regression, greater RVd was associated with the clinical outcome (hazard ratio: 9.27, 95% confidence interval: 1.23–69.88, p = 0.03). Conclusion Greater TR reduction is achieved by TTVR versus TTVr, which is in turn associated with RV reverse remodeling. RV dimension in follow‐up is associated with increased risk of a composite outcome of death, heart failure hospitalization, or re‐do tricuspid valve intervention.

Early Right Heart Remodeling in Patients With Mitral Valve Prolapse

Abstract:Patients with MVP and less than moderate mitral or tricuspid regurgitation showed tricuspid annulus disjunction, increased thickness of the tricuspid leaflets, and dilation of the tricuspid annulus and right atrium. Patients with MVP and coexistent TVP also showed greater TAPSE, which was independently associated and correlated with end‐systolic tricuspid diameter. MVP: mitral valve prolapse; RA: right atrium; TAPSE: tricuspid annular plane systolic excursion; TVP: tricuspid valve prolapse. Purpose Mitral valve prolapse (MVP) has been associated with left heart remodeling. This study explored cardiac remodeling in patients with MVP without significant regurgitation, focusing on the right heart. Methods This single‐center study enrolled consecutive patients referred to trans‐thoracic‐echocardiography (TTE) with MVP, excluding those with significant regurgitation or known cardiovascular or pulmonary diseases. A group of healthy controls was included. Results Forty‐nine patients with MVP and 54 controls were finally selected (mean age of 62, 52–71; 52% males), and echocardiographic parameters were compared among groups. Twenty‐nine (41%) patients with MVP showed tricuspid valve prolapse (TVP). Patients with MVP, irrespective of TVP, showed greater tricuspid annulus (systolic annulus: 31±6 vs. 32±5 vs. 27±3 mm for MVP+/TVP−, MVP+/TVP+, and controls, respectively; all p < 0.01) and greater minimum right atrial volume indexed (13, 12–15 mL/m2 vs. 15, 12–20 mL/m2 vs. 11, 10–14 mL/m2; all p < 0.05). Right ventricular dimensions and systolic indexes did not differ among groups, except TAPSE, which was significantly greater in MVP+/TVP+ patients compared to controls (25±4 vs. 22±3 mm, p = 0.004). A significant correlation (ρ = 0.43; p < 0.001) and an independent association at multivariate analysis (ß = 0.28; 95% CI 0.09–0.47; p = 0.004) were observed between end‐systolic tricuspid diameter and TAPSE. Conclusion In patients with MVP with less‐than‐moderate mitral or tricuspid regurgitation, dilation of the right atrium and tricuspid annulus was found. The latter finding was associated with increased values of TAPSE, which should then be used with caution, while other indexes may be preferred when assessing the systolic function of these patients.

Abstract:(1) Introduction and Aims: Right ventricular (RV) remodeling significantly impacts the prognosis of dilated cardiomyopathy (DCM) patients, and right atrial (RA) size and function are still often neglected in DCM patients. Accordingly, our aims were to (i) evaluate right heart subclinical changes and (ii) the prognostic value of RA compared to left atrial (LA) size and function in patients with DCM by advanced echocardiography. (2) Materials and Methods: Sixty-eight patients with DCM (with a mean age of 60 years; 35 men) were evaluated by comprehensive transthoracic echocardiography, compared to 62 age- and sex-matched healthy controls (with a mean age of 61 years; 32 men), and followed up for 12.4 ± 5 months. (3) Results: DCM patients have RV and RA global longitudinal dysfunction by 2DSTE, higher RA minimum volumes and tricuspid annulus areas despite having normal RV volumes, ejection fractions, and RA maximum volumes by 3DE compared to the controls. The RA strain and RV strain are correlated with each other. The RA reservoir strain (with an AUC = 0.769) has an increased value for outcome prediction compared to that of the LA strain. (4) Conclusion: Patients with DCM have RV longitudinal dysfunction and decreased RA function, in the absence of clinical RV involvement or atrial arrhythmias, and the RA strain is associated with an increased risk of hospitalization and cardiac death.

This knowledge is needed to guide patient selection for the evolving therapeutic options. We sought to explore the prevalence and predictors of secondary TR in the elderly, as well as the associated pattern of right-heart chamber and TV remodelling.

Abstract:This editorial refers to ‘Impact of reduction in early- and late-systolic functional mitral regurgitation on reverse remodelling after cardiac resynchronization therapy, by Y. Liang et al. , doi:10.1093/eurheartj/ehq134 Functional mitral regurgitation (MR) is a common finding in patients with global left ventricular (LV) dilatation and dysfunction.1 In these patients, the mitral leaflets are structurally normal and leaflet coaptation failure results from an imbalance between valve closing forces and valve tethering forces.2,3 Impaired LV contractility, global LV dyssynchrony, and reduced mitral annulus systolic contraction lead to a significant reduction in valve closing forces. In turn, valve tethering forces are increased due to mitral annulus dilatation, LV remodelling, and papillary muscle displacement.4 Furthermore, MR per se induces additional adverse ventricular remodelling through volume overload, and plays an important role in the progression of the cardiomyopathy. The presence of significant functional MR also has important prognostic implications,5,6 and is associated with a relatively high morbidity and mortality. Surgery is the preferred therapeutic option, but several recent studies demonstrated that cardiac resynchronization therapy (CRT) might also be able to reduce functional MR.7–10 Reduction of MR following CRT was shown to be related to various issues. Abstract:Cardiac resynchronization therapy (CRT) restores ventricular dyssynchrony, improving left ventricle (LV) systolic function, symptoms, and outcome in patients with heart failure, systolic dysfunction, and prolonged QRS interval. The left atrium (LA) plays tremendous roles in maintaining cardiac function, being often inflicted in various cardiovascular diseases. LA remodeling implies structural—dilation, functional—altered phasic functions, and strain and electrical—atrial fibrillation remodeling. Until now, several important studies have approached the relationship between LA and CRT. LA volumes can predict responsiveness to CRT, being also associated with improved outcome in these patients. LA function and strain parameters have been shown to improve after CRT, especially in those who were positive responders to it. Further studies still need to be conducted to comprehensively characterize the impact of CRT on LA phasic function and strain, and, also, in conjunction with its impact on functional mitral regurgitation and LV diastolic dysfunction. The aim of this review was to provide an overview of current available data regarding the relation between CRT and LA remodeling.

Abstract:BackgroundRight ventricular dilation and dysfunction is a common long‐term complication in patients with repaired pulmonary stenosis (rPS). Additionally, abnormal right and left ventricular interactions have been reported in right‐sided heart defect after intervention, including in pulmonary stenosis.PurposeTo analyze ventricular strain, remodeling, and left and right ventricular interactions in rPS patients with preserved right ventricular ejection fraction (RVEF) compared with healthy children using cardiac magnetic resonance.Study TypeA cross‐sectional study.PopulationIn all, 34 rPS patients and 10 healthy children volunteers (controls).Field Strength/Sequence3.0T/2D balanced steady‐state free precession (2D b‐SSFP) cine, late gadolinium enhancement (LGE), and 2D phase contrast (2D‐PC).AssessmentPulmonary regurgitation (PR) fractions of the main pulmonary artery, biventricular volumes, masses, function, and cardiac strain.Statistical TestsMann–Whitney U‐test, t‐test, Pearson correlation coefficients, Spearman's correlation coefficients, and intraclass correlation coefficients analysis were performed.ResultsFor group analysis, the right ventricular (RV) global circumferential strain and radial strain were significantly increased in patients when compared with controls (−13.57 ± 2.69 vs. −5.91 ± 3.16, P < 0.001; 25.31 ± 8.12 vs. 9.87 ± 5.32, P < 0.001, respectively). The fraction of PR displayed moderate correlation with right ventricular end‐diastolic volume index (RVEDVi) (r = 0.452, P = 0.022). RVEDVi and mass index were larger in patients vs. control (104.92 ± 27.46 vs. 85.15 ± 11.98, P = 0.016; 18.28 ± 4.95g/m2 vs. 11.67 ± 2.14 g/m2, P < 0.001, respectively). Patients presented with preserved left ventricular ejection function, but was lower than healthy controls (60.89% ± 4.89% vs. 65.95% ± 4.56%, P = 0.006). Regional circumferential strain of segment 3 of left ventricle (LV) were significantly decreased in patients (−7.79 ± 6.52 vs. −13.56 ± 3.22, P = 0.003).Data ConclusionCompensated increased RV strain, myocardial remodeling of RV, and adverse right and left ventricular interactions occur in rPS patients with preserved RVEF. The decreased interventricular septum strain may lead to impaired LV function due to RV dilation as a result of PR.Level of Evidence: 3Technical Efficacy Stage: 3J. Magn. Reson.
