They performed a case‒control study to investigate the influence of surgical procedures on the transmission of viral hepatitis. The study enrolled 8,176 patients with HBV infection and 2,179 with HCV infection from a national surveillance system in Italy. They found that invasive procedures were a significant risk factor for acquiring parenterally transmitted hepatitis viruses. The study underlined the importance of observing universal precautions to control the iatrogenic transmission of hepatitis viruses. We believe that these meaningful findings will provide guidance for patients with surgical interventions in the future. Here, we want to bring up some possible limitations of the study. The whole study could be designed more convincingly. First, to enhance the efficacy of the models, univariate and multivariate analyses could be performed to explore the clinical factors that correlated with HBV and HCV hepatitis. It is necessary to conduct a stratified analysis for those factors, such as invasive procedures. Table 1 shows five risk factors that were significantly correlated with HBV and HCV infections. These factors and invasive procedures, which are more important in the transmission of HBV and HCV, need to be further analysed and considered. Likewise, we recommend that some of the groupings for uncommon interventions may need to be collapsed to allow for adequate statistical power of this kind of analysis. Third, the study excluded groups with fewer than 15 participants, but an “other intervention” group with 7 cases is shown in Table 3. The design may influence the stability and validity of their multiple logistic regression models. Fourth, coinfection with HBV-HCV was not emphasized. The coinfection of HBV-HCV is a complex clinical entity that has an estimated worldwide prevalence of 1-15%.[4]Mavilia M.G. Wu G.Y. Hepatitis B virus: advances in prevention, diagnosis, and therapy.Clin Microbiol Rev. 2020; 33Google Scholar Hence, it is necessary to clarify those cases of coinfection with HBV and HCV. Clearly, Caminada et al. have performed an innovative and systematic study exploring the correlation between invasive procedures and hepatitis. They found that gynaecological surgery and neurosurgery had the strongest association with HBV and HCV infection, respectively. These results explain the still numerous and unexpected hepatitis cases diagnosed in the elderly population. Their meaningful results will provide guidance for the treatment of patients who require surgical intervention.
