Abstract:Climate change poses a significant occupational health hazard. Rising temperatures and more frequent heat waves are expected to cause increasing heat-related morbidity and mortality for workers across the globe. Agricultural, construction, military, firefighting, mining, and manufacturing workers are at particularly high risk for heat-related illness (HRI). Various factors, including ambient temperatures, personal protective equipment, work arrangements, physical exertion, and work with heavy equipment may put workers at higher risk for HRI. While extreme heat will impact workers across the world, workers in low- and middle-income countries will be disproportionately affected. Tracking occupational HRI will be critical to informing prevention and mitigation strategies. Renewed investment in these strategies, including workplace heat prevention programs and regulatory standards for indoor and outdoor workers, will be needed. Additional research is needed to evaluate the effectiveness of interventions in order to successfully reduce the risk of HRI in the workplace. Abstract:Background: Extreme heat can trigger or exacerbate cardiovascular disease (CVD) through ischemia or thrombosis, and its deleterious impacts are inclined to worsen as increased heat exposure and heightened vulnerability from climate change interacted with natural and social environment. Research Questions: Previous studies have primarily focused on quantifying the increased risk of CVD due to heat exposure, while lacking a comprehensive understanding of the association, underlying mechanisms, and vulnerability in CVD patients. Consequently, there is a scarcity of clinical and public health interventions aimed at preventing the convergence of extreme heat exposure and the risk of CVD within the context of climate change. Methods: We conducted a systematic search in PubMed, Embase, and Web of Science from inception to December, 2023, and we retrieved articles which assessed the effects of extreme heat or climate change on CVD. Results: In this review, we outline the existing evidence on the epidemiology and pathophysiological mechanisms that elucidate how extreme heat affects the risk of CVD. Extreme heat is associated with increased risks for CVD morbidity and mortality, where older persons, low socioeconomic status, pre-existing medical conditions, and unfavorable living environments confer greater susceptibility to heat. The adverse effects of extreme heat on cardiovascular system involved a wide range of complex mechanisms between different organs and regulatory pathways (Figure 1). Additionally, we identify the factors that contribute to the vulnerability of CVD patients to heat exposure. The discrepancy in heat-related risk can be attributed to individual susceptibility, sociocultural factors, population temperature adaptations, and environmental factors. Future climate change is projected to exacerbate the burden of extreme heat, particularly due to the interaction of air pollution, population aging, and urbanization (Figure 2). Conclusions: Addressing extreme heat should be within the framework of the prevention of CVD. Health professionals and scientists are responsible for mitigating the cardiovascular effects mediated by extreme heat, with an emphasis on engaging in risk management tactics and health research to implement preventive strategies. As CVD is a climate sensitive disease, evidence-based interventions during heat extremes are urgently needed to cope with the cardiovascular risks associated with the inevitable trajectory of climate change.

Abstract:Background: The accelerating accumulation of greenhouse gases in the Earth's atmosphere is changing global environmental conditions in unprecedented and potentially irreversible ways. Climate change poses a host of challenges to the health of populations through complex direct and indirect mechanisms. The direct effects include an increased frequency of heat waves, rising sea levels that threaten low-lying communities, anticipated extremes in the global hydrologic cycle (droughts, floods, and intense storms), and adverse effects on agricultural production and fisheries due to environmental stressors and changes in land use. Indirectly, climate change is anticipated to threaten health by worsening urban air pollution and increasing rates of infectious (particularly waterborne and vector-borne) disease transmission. Objective: To provide a state-of-the-science review on the health consequences of a changing climate. Findings: Environmental public health researchers have concluded that, on balance, adverse health outcomes will dominate under these changed climatic conditions. The number of pathways through which climate change can affect the health of populations makes this environmental health threat one of the largest and most formidable of the new century. Geographic location plays an influential role the potential for adverse health effects caused by climate change, and certain regions and populations are more vulnerable than others to expected health effects. Two kinds of strategies are available for responding to climate change: mitigation policies (which aim to reduce greenhouse gas emissions) and adaptation measures (relating to preparedness for anticipated impacts). Conclusions: To better understand and address the complex nature of health risks posed by climate change, interdisciplinary collaboration is critical. Efforts to move beyond our current reliance on fossil fuels to cleaner, more sustainable energy sources may offer some of the greatest health opportunities in more than a century and cobenefits beyond the health sector. Because the nations least responsible for climate change are most vulnerable to its effects, the challenge to reduce greenhouse gas emissions is not merely technical, but also moral.
Climate Change-Related Environmental Exposures and Perinatal and Maternal Health Outcomes in the U.S

Abstract:Purpose: Climate change poses one of the greatest risks to human health as air pollution increases, surface temperatures rise, and extreme weather events become more frequent. Environmental exposures related to climate change have a disproportionate effect on pregnant women through influencing food and water security, civil conflicts, extreme weather events, and the spread of disease. Our research team sought to identify the current peer-reviewed research on the effects of climate change-related environmental exposures on perinatal and maternal health in the United States. Design and methods: A systematic literature review of publications identified through a comprehensive search of the PubMed and Web of Science databases was conducted using a modified Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) approach. The initial search across both databases identified a combined total of 768 publications. We removed 126 duplicates and 1 quadruplet, and the remaining 639 publications were subjected to our pre-set inclusion and exclusion criteria. We excluded studies outside of the United States. A total of 39 studies met our inclusion criteria and were retained for thematic analysis. Findings: A total of 19 studies investigated the effect of either hot or cold temperature exposure on perinatal and maternal health outcomes. The effect of air pollution on perinatal outcomes was examined in five studies. A total of 19 studies evaluated the association between natural disasters (hurricanes, flash floods, and tropical cyclones) and perinatal and maternal health outcomes. High and low temperature extremes were found to negatively influence neonate and maternal health. Significant associations were found between air pollutant exposure and adverse pregnancy outcomes. Adverse pregnancy outcomes were linked to hurricanes, tropical cyclones, and flash floods. Conclusions: This systematic review suggests that climate change-related environmental exposures, including extreme temperatures, air pollution, and natural disasters, are significantly associated with adverse perinatal and maternal health outcomes across the United States.

Abstract:Climate change is widely recognized as one of the most significant challenges facing our planet and human civilization. Human activities such as the burning of fossil fuels, deforestation, and industrial processes release greenhouse gases into the atmosphere, leading to a warming of the Earth's climate. The relationship between climate change and cardiovascular (CV) health, mediated by air pollution and increased ambient temperatures, is complex and very heterogeneous. The main mechanisms underlying the pathogenesis of CV disease at extreme temperatures involve several regulatory pathways, including temperature-sympathetic reactivity, the cold-activated renin-angiotensin system, dehydration, extreme temperature-induced electrolyte imbalances, and heat stroke-induced systemic inflammatory responses. The interplay of these mechanisms may vary based on individual factors, environmental conditions, and an overall health background. The net outcome is a significant increase in CV mortality and a higher incidence of hypertension, type II diabetes mellitus, acute myocardial infarction (AMI), heart failure, and cardiac arrhythmias. Patients with pre-existing CV disorders may be more vulnerable to the effects of global warming and extreme temperatures. There is an urgent need for a comprehensive intervention that spans from the individual level to a systemic or global approach to effectively address this existential problem. Future programs aimed at reducing CV and environmental burdens should require cross-disciplinary collaboration involving physicians, researchers, public health workers, political scientists, legislators, and national leaders to mitigate the effects of climate change.

Building Vulnerability in a Changing Climate: Indoor Temperature Exposures and Health Outcomes in Older Adults Living in Public Housing during an Extreme Heat Event in Cambridge, MA

Abstract:In the Northeastern U.S., future heatwaves will increase in frequency, duration, and intensity due to climate change. A great deal of the research about the health impacts from extreme heat has used ambient meteorological measurements, which can result in exposure misclassification because buildings alter indoor temperatures and ambient temperatures are not uniform across cities. To characterize indoor temperature exposures during an extreme heat event in buildings with and without central air conditioning (AC), personal monitoring was conducted with 51 (central AC, n = 24; non-central AC, n = 27) low-income senior residents of public housing in Cambridge, Massachusetts in 2015, to comprehensively assess indoor temperatures, sleep, and physiological outcomes of galvanic skin response (GSR) and heart rate (HR), along with daily surveys of adaptive behaviors and health symptoms. As expected, non-central AC units (Tmean = 25.6 °C) were significantly warmer than those with central AC (Tmean = 23.2 °C, p < 0.001). With higher indoor temperatures, sleep was more disrupted and GSR and HR both increased (p < 0.001). However, there were no changes in hydration behaviors between residents of different buildings over time and few moderate/several health symptoms were reported. This suggests both a lack of behavioral adaptation and thermal decompensation beginning, highlighting the need to improve building cooling strategies and heat education to low-income senior residents, especially in historically cooler climates.

Abstract:Climate change is increasing both seasonal temperatures and the frequency and severity of heat extremes. As the endocrine system facilitates physiological adaptations to temperature changes, diseases with an endocrinological basis have the potential to affect thermoregulation and increase the risk of heat injury. The effect of climate change and associated high temperature exposure on endocrine axis development and function, and on the prevalence and severity of diseases associated with hormone deficiency or excess, is unclear. This Perspective summarizes current knowledge relating to the hormonal effects of heat exposure in species ranging from rodents to humans. We also describe the potential effect of high temperature exposures on patients with endocrine diseases. Finally, we highlight the need for more basic science, clinical and epidemiological research into the effects of heat on endocrine function and health; this research could enable the development of interventions for people most at risk, in the context of rising environmental temperatures.

Abstract:There is increasing understanding, globally, that climate change and increased pollution will have a profound and mostly harmful effect on human health. This review brings together international experts to describe both the direct (such as heat waves) and indirect (such as vector-borne disease incidence) health impacts of climate change. These impacts vary depending on vulnerability (i.e., existing diseases) and the international, economic, political, and environmental context. This unique review also expands on these issues to address a third category of potential longer-term impacts on global health: famine, population dislocation, and environmental justice and education. This scholarly resource explores these issues fully, linking them to global health in urban and rural settings in developed and developing countries. The review finishes with a practical discussion of action that health professionals around the world in our field can yet take.

Abstract:There is scientific consensus that the global climate is changing, with rising surface temperatures, melting ice and snow, rising sea levels, and increasing climate variability. These changes are expected to have substantial impacts on human health. There are known, effective public health responses for many of these impacts, but the scope, timeline, and complexity of climate change are unprecedented. We propose a public health approach to climate change, based on the essential public health services, that extends to both clinical and population health services and emphasizes the coordination of government agencies (federal, state, and local), academia, the private sector, and nongovernmental organizations.

Abstract:Increased fossil fuel use has increased carbon dioxide concentrations leading to global warming and climate change with increased frequency and intensity of extreme weather events such as thunderstorms, wildfires, droughts, and heat waves. These changes increase the risk of adverse health effects for all human beings. However, these experiences do not affect everyone equally. Underserved communities, including people of color, the elderly, people living with chronic conditions, and socioeconomically disadvantaged groups, have greater vulnerability to the impacts of climate change. These vulnerabilities are a result of multiple factors such as disparities in health care, lower educational status, and systemic racism. These social inequities are exacerbated by extreme weather events, which act as threat multipliers increasing disparities in health outcomes. It is clear that without human action, these global temperatures will continue to increase to unbearable levels creating an existential crisis. There is now global consensus that climate change is caused by anthropogenic activity and that actions to mitigate and adapt to climate change are urgently needed. The 2015 Paris Accord was the first truly global commitment that set goals to limit further warming. It also aimed to implement equity in action, founded on the principle of common but differentiated responsibilities. Meeting these goals requires individual, community, organizational, national, and global cooperation. Health care professionals, often in the frontline with firsthand knowledge of the health impacts of climate change, can play a key role in advocating for just and equitable climate change adaptation and mitigation policies.

Abstract:Climate instability, extreme weather, and natural disasters have surged over the past 50 years, resulting in substantial and increasing harms to human health. Almost daily, evidence of the rapidly escalating climate crisis grows. As we write, wildfires fueled by record high temperatures rage on the West Coast. In July, extreme rainfall—600% higher than usual—caused historic flooding and 39 deaths in eastern Kentucky (1). In June 2021, 157 people died when record-breaking temperatures topped 110 °F in Washington, making it the deadliest weather event in the state's history (2). A driver of these and other disasters are planet-warming greenhouse gas emissions from fossil fuel combustion. However, climate-mediated disasters are only one of the myriad health hazards from fossil fuels and industrialization. Pollution and environmental degradation from the same sources that cause climate instability compound harms. Although isolated exposure to air pollution or extreme heat can be lethal, mortality due to the combination is 3 times greater than the sum of the individual effects (3). As the climate crisis worsens, such coexposures will be the norm. Health care providers must prepare to meet these converging and exponential environmental health threats.

Projected Change in the Burden of Excess Cardiovascular Deaths Associated With Extreme Heat by Midcentury (2036-2065) in the Contiguous United States

Abstract:Background: Climate change is causing an increase in extreme heat. Individuals with cardiovascular disease are at high risk of heat-related adverse health effects. How the burden of extreme heat-associated cardiovascular deaths in the United States will change with the projected rise in extreme heat is unknown. Methods: We obtained data on cardiovascular deaths among adults and the number of extreme heat days (maximum heat index ≥90 °F [32.2 °C]) in each county in the contiguous United States from 2008 to 2019. Based on representative concentration pathway trajectories that model greenhouse gas emissions and shared socioeconomic pathways (SSP) that model future socioeconomic scenarios and demographic projections, we obtained county-level projected numbers of extreme heat days and populations under 2 scenarios for the midcentury period 2036 to 2065: SSP2-4.5 (representing demographic projections from a "middle-of-the-road" socioeconomic scenario and an intermediate increase in emissions) and SSP5-8.5 (demographic projections in an economy based on "fossil-fueled development" and a large increase in emissions). The association of cardiovascular mortality with extreme heat was estimated with a Poisson fixed-effects model. Using estimates from this model, the projected number of excess cardiovascular deaths associated with extreme heat was calculated. Results: Extreme heat was associated with 1651 (95% CI, 921-2381) excess cardiovascular deaths per year from 2008 to 2019. By midcentury, extreme heat is projected to be associated with 4320 (95% CI, 2369-6272) excess deaths annually, which is an increase of 162% (95% CI, 142-182) under SSP2-4.5, and 5491 (95% CI, 3011-7972) annual excess deaths, which is an increase of 233% (95% CI, 206-259) under SSP5-8.5. Elderly adults are projected to have a 3.5 (95% CI, 3.2-3.8) times greater increase in deaths in the SSP2-4.5 scenario compared with nonelderly adults. Non-Hispanic Black adults are projected to have a 4.6 (95% CI, 2.8-6.4) times greater increase compared with non-Hispanic White adults. The projected change in deaths was not statistically significantly different for other race and ethnicity groups or between men and women. Conclusions: By midcentury, extreme heat is projected to be associated with a significantly greater burden of excess cardiovascular deaths in the contiguous United States.
Extreme Heat and Air Quality: Community Leader Perspectives on Information Barriers and Opportunities in Two Environmental Justice Communities

Abstract:Extreme heat and air pollution exposure are leading causes of adverse cardiorespiratory health outcomes. Exposure reduction strategies are often focused at the local level. This study examined critical challenges community leaders face in understanding and sharing environmental exposure and health information. We conducted interviews with 19 community leaders of two urban environmental justice communities in Massachusetts, United States. Using directed content analysis, we examined air quality and heat perceptions, information and data resources, and barriers to understanding and communicating relevant local information. Participants shared concerns about both poor air quality and extreme heat. They also expressed the opinion that exposure risk information about these topics is siloed; heat and air quality data can be hard to access, interpret, and effectively communicate with community members. Solutions recommended by participants included community engagement, open-data portals, and creative science communication. Increasing sustainable collaborations among academic, government, healthcare, and nonprofit sectors is recommended.

In this science and society article we summarize how heat affects our body and discuss the associated health threats, but also the potential health benefits of heat exposure. Moreover, we provide practical suggestions for sustainable and health-oriented strategies to cope with heat.

The Heat Index as a Measure of Future Heat Stress with Climate Change

Abstract:The wet-bulb temperature and wet-bulb globe temperature (WBGT) have been widely used to forecast the physiological consequences of extreme heat associated with climate change. Unfortunately, those metrics are not based in any physiological model of thermoregulation, which means their values do not map one-to-one onto physiological states. In contrast, the widely used heat index is based on a model of human thermoregulation: each value of the heat index maps onto a unique state of the human body. This mapping is obscured in practical applications of the heat index, which use the National Weather Service's empirical fit to values tabulated in Robert Steadman's seminal 1979 paper. In addition, that empirical fit is used, without scientific justification, to extrapolate the heat index to high heat and humidity, where Steadman's paper gave no values because of an apparent failure in the underlying physiological model. In recent work, Steadman's model has been extended, allowing the heat index to be calculated for extreme heat and humidity while retaining full backward compatibility. This reveals that the extrapolation used by the National Weather Service underestimates the heat index during severe US heat waves by as much as 10 degrees Celsius. For example, the peak heat index at Chicago's Midway International Airport was underreported by 9 degrees Celsius during the July 1995 heat wave, which killed hundreds of people. This correction has important implications for physiology: these corrected values reveal that even walking in the shade would have brought a healthy person close to the point of hyperthermia. This highlights how close global warming is to making the most severe US heat waves a fatality risk for even young and healthy adults. Climate change and quality of health care: evidence from extreme heat

Abstract:BackgroundExtreme temperatures, one of the most notable consequences of climate change, can reduce worker productivity in physically demanding occupations and even inhibit cognitive skill development. We aimed to examine whether heat waves can affect access to and quality of health care provided by physicians.MethodsWe investigated the effect of heat waves on physicians using unique data on the quality of health care provided at 63 community health centres in urban Xi'an, China. We used standardised patients recruited from the local community and trained them to present consistent cases of illness to providers. Adherence to two clinical checklists, thoroughness of physical examination, and accuracy of diagnoses were used to assess quality of primary care through 492 interactions between physicians and standardised patients in Aug 17–28, 2017, and July 30 to Aug 10, 2018. Exploiting random fluctuations in temperature and other weather patterns (air pollution, wind speed, rainfall, sunlight hours, barometric pressure, and humidity) according to the exact time and geographical locations of the interviews, we identified the effect of transitory exposures to heat waves on quality of health service delivery in models with day-specific and community health centre-specific fixed effects.FindingsExtremely hot days are associated with improved quality of care. One Fahrenheit temperature increase improves the adherence to clinical checklists by 1·51 units, representing an increase of 36·92% over the mean number of checklist items completed by physicians (4·09 items; p<0·001). This improvement could be explained by fewer patients waiting for services during hot days. However, the improved adherence to the clinical checklists was not accompanied with a larger chance of physicians offering correct diagnoses or appropriate treatments.
