Abstract:Miscarriage is the loss of pregnancy before the baby survives outside the womb. [ 1 ] The most common complication after miscarriage or abortion is a partial loss of products of uterine conception, referred to as incomplete abortion. [ 2 ] Patients may present with vaginal bleeding or with lower abdominal pain. Untreated leaves can lead to bleeding, infections, shock, and death. [ 2 , 3 ] Complications of incomplete abortion considerably affect a woman physical and mental health, including sleep disturbances, social withdrawal, anger, and marital disturbance. [ 4 ] Fathers can also be affected emotionally by short-term anxiety and depression. [ 5 ] Therefore, patients with incomplete abortions should be effectively managed with safe and appropriate treatment.

Abstract:To analyze the efficacy and influencing factors of Mifepristone combined with estrogen-progesterone sequential therapy (Femoston) in the treatment of incomplete abortion. This retrospective cohort study included 93 patients with incomplete abortion. All patients took 50 mg of Mifepristone 2 times a day for 5 days and then took Femoston once a day (starting with estradiol tablets/2 mg) for 28 days. Without any indication of intrauterine residue by ultrasonic examination was judged to be effective. According to statistical analysis, this study calculated the effective rate and analyzed its influencing factors. A 2-sided value of P < .05 was considered statistically significant. The total response rate of the treatment regimen was 86.67%. body mass index was a significant influencing factor for treatment outcome (OR 0.818, 95% confidence interval 0.668-0.991, P = .041). For patients with incomplete abortion, Mifepristone combined with estrogen-progesterone sequential therapy has a remarkable therapeutic effect. Patients with a lower body mass index may respond much more significantly to this treatment regimen.

Abstract:OBJECTIVE:To explore the application of Femoston in hormone replacement treatment-frozen embryo transfer (HRT-FET) versus endometrial preparation with Progynova and examine the effects of different endometrial thickness and estradiol levels on clinical outcomes. METHODS:Retrospective analysis was conducted for a total of 1072 HRT-FET cycles at our center during January-June 2012. According to the type of medication, the patients were divided into 2 groups: Group A: Progynova only (798 cycles). Group B: non-satisfactory endometrial thickness or E2 level only by Progynova or repeated implantation failure, Progynova plus Femoston for preparing endometrium (274 cycles). Then the inter-group differences of basic profiles and clinical outcomes were compared. Comparison of clinical outcomes was performed based on different estradiol (E2) levels and endometrial thickness on the day of using progesterone. And then, the patients whose endometrial double thickness was thinner than 7 mm in Progynova + Femoston group were compared with the group of similar endometrial thickness in 2011. The data were analyzed with SPSS 16.0. RESULTS:In Group A, the percentage of first transfer, average endometrial thickness on the day of using progesterone in FET cycles were all significantly higher than Group B. And the ratio of < 7 mm was significantly higher in Group B (8.8% vs 2.4%). However, no significant differences existed in clinical pregnancy rate (54.8% vs 52.9%) or embryo implantation rate (38.1% vs 35.8%). With endometrial thickening, both clinical pregnancy and embryo implantation rates increased. There was no significant difference in maximum E2 level. Clinical pregnancy rate, embryo implantation rate and live birth rate of the patients whose endometrial double thickness was thinner than 7 mm in Group B were all higher than those of similar endometrial thickness (all Progynova only) in 2011. CONCLUSION:For patients with thin endometrium, vaginal Femoston may be added if ideal endometrial thickness or E2 level is not achieved by Progynova alone. It improves endometrial receptivity and clinical outcomes are more satisfactory.

Abstract:OBJECTIVE To observe the effctiveness and clinic accept ability of medical abortion of early pregnancy induced by miferprestone accompanied with misoprostol.METHODS 102 women,who had early pregnancies after cesarean section, were compared with 78 women with early pregnancy after natural delivery and 129 women having no birth. Miferprestone and misoprostol were given in the routine way. Several items were recorded: drugs acting time, gestation sac passing out time, vaginal bleeding, side effects of the drugs, remainder in uterus or not and menstruation recovery days. RESULTS Comparison of several key items such as gestation sac passing out time, complete abortion rate, vaginal bleeding and menstruation recovery days, no obvious differences were observed in these three groups.CONCLUSION It is secure to process medical abortion of early pregnancy induced by miferprestone accompanied with misoprostol.

Abstract:Abstract Objective: To explore the application of mifepristone in the stream of Residual tissue after abortion . Methods: A retrospective study of 1067 women who selected induced abortion in first-trimester pregnancy(gestational age ≤ 70 days) between January 1st，2018 and May 31st，2019 in Chongqing Health Center For Women and Children . Inclusion criteria: the first pregnancy patients, aged 18-30 years, gestational age ≤ 70 days. Test group (mifepristone + misoprostol) 467 cases. Control group(misoprostol) 600 cases. Analyzing the incidence of residual tissue after induced abortion. Evaluating the preventive effect of preoperative oral mifepristone on abortion incompleteness. Results: a total of 9 in test group occur residual tissue in uterine cavity，the incidence is 1.93%. In the controlled group, the number of residual tissue is 25,the incidence is 4.16%(P<0.05)，there was statistic difference between two groups. The uterine residue diameter in the test group(1.1±0.31cm) was lower than that in the control group(1.24±0.64cm)，but there was no statistical difference between the two groups of residue diameter (P>0.05)Conclusion: Oral mifepristone before operation can significantly reduce the incidence of tissue residual after induced abortion.
Comparison of GnRH-a Prolonged Protocol and Short GnRH-a Long Protocol in Patients with Thin Endometrium for Assisted Reproduction: A Retrospective Cohort Study

Abstract:Purpose Gonadotrophin releasing hormone agonist (GnRH-a) is widely used for pituitary down-regulation and recruiting more follicles in assisted reproduction. However, no information is available on its value for patients with thin endometrial thickness. Patients and Methods This was a retrospective cohort study of 302 patients with endometrium <8 mm undergoing fresh embryo transfer at a fertility center of a university hospital from January 2016 and December 2018. In 148 cycles of the GnRH-a prolonged protocol, one depot of 3.75 mg GnRH-a was injected on day 2 of the menstrual cycle, while in 154 cycles of the short GnRH-a long protocol, 0.1 mg of GnRH-a was injected daily from the mid-luteal phase. The live birth rate and clinical pregnancy rate were compared between the two groups. Other outcome measures included the implantation rate, miscarriage rate, and characteristics of stimulation procedures. Results Live birth rates and clinical pregnancy rates were significantly higher in the GnRH-a prolonged protocol group than in the other group (36.5% vs 20.8%, P=0.002; 43.9% vs 28.2%, P=0.006, respectively). The live birth rate was significantly increased in the prolonged protocol group (crude OR: 2.190, 95% CI: 1.311, 3.660; adjusted OR: 2.458, 95% CI: 1.430, 4.224) compared with that in the reference group. The implantation rate of the former group was also significantly higher than that of the latter group (35.4% vs 15.9%, P=0.000). There was no significant difference in miscarriage rates between the two protocols. In terms of stimulation procedures, the GnRH-a prolonged protocol group required significantly higher Gn time (10.9 vs 9.5 days, P=0.000) and Gn consumption (2625.0 vs 2047.5 IU, P=0.000) than the short GnRH-a long protocol group. Conclusion The GnRH-a prolonged protocol in fresh embryo transfer cycles yielded better clinical outcomes of patients with thin endometrium than the short GnRH-a long protocol.
Dydrogesterone for Threatened Miscarriage:A Systematic Review

Abstract:OBJECTIVE:To evaluate the efficacy and safety of dydrogesterone in the treatment of threatened miscarriage.METHODS:Randomized controlled trials(RCT) involving dydrogesterone in the treatment of threatened miscarriage were identified from PubMed,Embase,Cochrane Central,CBM,CNKI,VIP,ClinicalTrials.gov and WHO ICTRP.Published and unpublished RCTs were obtained on dydrogesterone in the treatment of threatened miscarriage.Included literatures were extracted and bias risks were evaluated independently by two reviewers.RevMan 5.0 software was used for Meta-analysis.RESULTS:A total of 8 RCT involving 1 185 participants were included.Dydrogesterone vs.conservative treatment:4 RCT were included,the results of Meta-analysis showed that participants treated with dydrogesterone had a lower risk of miscarriage,compared with conservative treatment [RR=0.46,95%CI(0.32,0.65),P0.000 1];no statistical significance was found in the incidence of premature birth(P=0.77),antepartum haemorrhage(P=0.57) and congenital malformation(P=0.71).Dydrogesterone vs.progesterone:2 RCT were included.We did not conduct Meta-analysis as included studies were in high risk of bias.No statistical significance was reported between two groups in the incidence of miscarriage(P=1.000 and P=0.630).Dydrogesterone vs.HCG:1 RCT was included.No statistical significance was reported between two groups regarding to the incidence of miscarriage(P=0.711),premature birth(P=0.727) and congenital malformation(P=0.461).Dydrogesterone combined with TCM treatment:2 RCT were included,the incidences of miscarriage in dydrogesterone combined with Zishen yutai pills group and dydrogesterone alone group were 8% and 13%,there was no statistical significance(P=0.655);miscarriage occurred in 7.5% of women treated with dydrogesterone combined with Bushen huoxue antai prescription,and in 15% of that treated with progesterone,without statistical significance,neither(P=0.390).CONCLUSION:Dydrogesterone group has a lower incidence of miscarriage than conservative treatment only,but further studies with good methodological design are still needed to investigate its efficacy.There is no sufficient evidence to show any negative outcomes occurred in mothers or children related to dydrogesterone.Current studies with poor methodological quality show no difference in the efficacy between dydrogesterone and progesterone or HCG.There is insufficient evidence to show the benefit of dydrogesterone combined with TCM.
Effects of dydrogesterone using for luteal phase support on IVF obstetric outcomes

Abstract:Objectives:To discuss the effects of dydrogesterone using for luteal phase support,instead of intramuscular progesterone,on IVF obstetric outcomes.Methods:The data of patients treated with IVF/ICSI in our department from July 1 ,201 0 to December 31 ,201 1 were reviewed.The clinical pregnancy rate and live birth rate between 1 28 patients who were treated with intramuscular injection of 40mg progesterone combined with 20mg oral dydrogesterone for luteal support and patients who were treated with intramuscular injection of 60mg progesterone were compared.Results:In dydrogesterone group,the clinical pregnancy rate was 47. 3% and the live birth rate was 40. 7%.Both the two rates had no difference with the patients treated with progesterone in the same period. When only selecting patients treated with short-acting GnRHa long protocol for down-regulation and patients trea-ted with ICSI,the clinical pregnancy and live birth rate of dydrogesterone group did not differ from these of the other two groups.Conclusions:Dydrogesterone,used for luteal phase support,did not affect the obstetric outcomes of the patients who received IVF treatment.

Abstract:Purpose Endometriosis is a common, chronic gynecological disease that affects women’s fertility potential. Dydrogesterone is an effective and safe drug that is under-utilized due to limited clinical research. The purpose of this evidence mapping is to identify, describe, and analyze the current available evidence regarding dydrogesterone for the treatment of endometriosis. Materials and methods We performed a search in electronic databases: Medline, The Cochrane Library, EMBASE, PubMed, CNKI, Wanfang, VIP, and CBM. We also hand-searched google for relevant studies. Our primary outcomes included changes in pain relief including pelvic pain, dysmenorrhea, and dyspareunia. Secondary outcomes included pregnancy rate, frequency of analgesic use, and other reported outcomes according to specific settings in the studies. Results Of 377 references screened, 19 studies were included in the data synthesis involving 1709 female participants. Nearly three-quarters were either randomized control trials or clinical control trials. Compared with gestrinone, dydrogesterone relieved dysmenorrhea, increased the pregnancy rate, and reduced the risk of certain adverse events. Compared with GnRH-a, dydrogesterone also lowered the risk of endometriosis recurrence and elevated transaminase levels. Whether there was any difference in efficacy between dydrogesterone and leuprolide acetate, letrozole or traditional Chinese medicine remains unclear due to insufficient data. Conclusions The amount and quality of evidence evaluating the effects of dydrogesterone for the treatment of endometriosis is generally very low. Limited evidence suggests that dydrogesterone may have some advantages over gestrinone, GnRH agonists, and other therapeutic interventions in treating endometriosis. However, this conclusion should be interpreted with caution.

Abstract:Objective To explore the effect of cyclogest and femotroneinthreatened abortion treatment and summerize key points of nursing. Methods Forty-two cases of first trimester threatened abortion women were selected from department of gynecology and obstetrics, which were randomly divided into the control group (n=21) and experiment group (n=21)using random digit table. The control group was given femotrone 40mg intramuscular injection each day while the experiment group was given 90 mg of vaginal progesterone suppository (Cyclogest) each day. Compare the improvement of symptoms, the success rate of pregnancy, changes in serum human chorionic gonadotropin (HCG) and progesterone. Results The improvement of symptoms, curative effects and change of HCG of both groups were no significantly difference. But elevatorconcentration of serum progesterone in femotrone group was higher than that of the cyclogest groupwith statistically difference. Conclusions Although cyclogest cannot appear significantly in increasing serum progesterone levels, it has the same effect as femotrone. Recommendations and notes about the use and side effects of cyclogest and femotrone should be provided for education to alleviate concerns of patients.
Preliminary study on the effect of mifepristone followed by diffferent regimens of misoprostol for termination of pregnancy between 8 and 16 weeks of gestation

Abstract:Objective:To explore the efficacy and safety of mifepristone combined with different regimens of misoprostol to terminate pregnancy at 8-16 weeks of gestation.Methods:120 women were recruited and randomized to receive 200mg mifepristone orally (100 mg of mifepristone at the first and the second day,respectively),then they were given six different regimens of misoprostol after taken the first mifepristone 36 hours later.The outcome measures were the rate of successful abortion,induction to abortion interval,the amount of vaginal bleeding and the adverse events.Results:There were no significant differences in the rate of successful abortion (from 90％ to 100％,P=0.465) and induction to abortion intervals (from 4.2h to 9.5h,P=0.253) among the groups.The rate of successful abortion after first dose of misoprostol of women in vaginal groups was significantly higher than that of women in oral groups.The average amount of bleeding after placenta expulsion was 67.4ml within 2h and 37.7ml between 2h and 24h.The emergency curettage rate was 11.8％.There were no serious adverse events(SAE),the adverse events(AE)related to drugs were mild or moderate,and there were no statistical significant different (P =0.686) in the study.The overall satisfaction rate was 84.6％.Conclusion:This preliminary result of the study may suggest different regimens of misoprostol combined with mifepristone are all effective and safe to terminate pregnancy at 8-16 weeks of gestation.Yet,the sample size should be expanded to explore the optimal regimen.
A cohort study of the efficacy of the dienogest and the gonadotropin-releasing hormone agonist in women with adenomyosis and dysmenorrhea

Abstract:Purpose: To study the efficacy and safety of the dienogest and the gonadotropin-releasing hormone agonist (GnRH-a) in symptomatic females with uterine adenomyosis. Methods: A total of 127 patients with adenomyosis with a chief complaint of dysmenorrhea were recruited. The first group received 2 mg of dienogest (DNG) daily, whereas the second group received goserelin acetate (GS) (3.6 mg/4 weeks) for 12 weeks. Outpatient follow-up was undertaken after 12 weeks. Results: Among 127 women, 56/63 (88.9%) patients completed the treatment in the DNG group, whereas 62/64 (96.9%) patients completed the treatment in the GS group. A significant decrease in dysmenorrhea symptoms as measured by the visual analog scale (VAS) and Carcinoma antigen125 (CA125) after 12 weeks of treatment was observed in both groups (p < .001). The hemoglobin of anemic patients did not significantly improve after 12 weeks of treatment (p＝0.21) and the uterine volume slightly increased without statistical significance (p＝0.10) in the DNG group. Simultaneously, The hemoglobin of anemic patients significantly improved (p < .001) and the uterine volume significantly decreased (p < .001) in the GS group. Conclusions: Dienogest effectively alleviates the symptoms of dysmenorrhea in patients with adenomyosis, but it cannot improve the anemia or reduce the size of the uterus. GnRH-a is more effective in improving anemia and reducing the uterine volume in patients with adenomyosis. TRIAL REGISTRATION: ChiCTR1900024958.

[Impact of Different Luteal Support Methods on Clinical Outcomes of Frozen-Thawed Embryo Transfer Cycles].

Abstract:OBJECTIVE:To determine whether luteal support of dydrogesterone may enhance the outcome of frozen-thawed embryo transfer (FET) in natural cycles by comparing the FET outcomes of different luteal support methods.METHODS:A total of 2 248 natural FET cycles with an endometrial thickness of 8-12 mm and type A on ovulation day, from January 2011 to March 2013 were chosen. Oral dydrogesterone (n = 1 967) or intramuscular progesterone (n = 281) was used for luteal support. The rates of clinical pregnancy, implantation, ectopic pregnancy, miscarriage and live birth were compared among these groups.RESULTS:The patients receiving oral dydrogesterone or intramuscular progesterone had similar profiles of age, type of infertility, endometrial thickness and average numbers of embryo transfer. The dydrogesterone group had higher rates of clinical pregnancy (43.26% vs 38.79%) and implantation (25.76% vs 21.13%). However a lower miscarriage rate (17.27% vs 22.02%) was insignificant. The rates of ectopic pregnancy, premature birth and live birth were also similar. When infertile factors were considered, the dydrogesterone group had significantly higher clinical pregnancy rates (43.78% vs 34.38%, P < 0.05), lowered miscarriage rates (16.54% vs 29.55%, P < 0.05) and increased live birth rates (34.16% vs 23.44%, P < 0.05) among simple tubal factor patients.CONCLUSION:Oral administration of dydrogesterone for luteal support offers more advantages in natural cycle FET than intramuscular progesterone, especially for simple tubal factor infertility.
Effectiveness Of Dehydroepiandrosterone In Poor Ovarian Responders Undergoing In Vitro Fertilization: A Meta-Analysis Of Randomized Controlled Trials

Abstract:This meta-analysis aimed to evaluate whether DHEA could improve the probability of pregnancy in poor ovarian responders undergoing IVF/ICSI cycles. Electronic literature searches were carried out in MEDLINE, Scopus, EMBASE, Cochrane, CNKI, Wanfang, and VIP databases up to April 2016. All randomized controlled trials in which pretreatment with DHEA was compared with placebo or blank control were identified. Standard meta-analytic methodology was used for the combination of results and the exploration of bias. The primary outcome was clinical pregnancy rate, the secondary outcomes were the number of retrieved oocytes, cancellation rate, E-2 level on the day of hCG administration, miscarriage rate, and total dose of gonadotropin units. A total of nine studies (1072 cases) met the inclusion criteria. The clinical pregnancy rate (OR: 1.64, 95% CI: 1.20-2.24; P = 0.002), the number of retrieved oocytes (MD 1.27, 95% CI: 0.60-1.94; P = 0.0002) were significantly higher and the cancellation rate (OR 0.54, 95% CI: 0.33-0.87; P = 0.01) was significantly lower in the DHEA group. Miscarriage rate (OR: 0.54, 95% CI: 0.24-1.22; P = 0.14), E-2 level on the day of hCG administration (MD: -13.62, 95% CI: -300.33-273.10; P = 0.93), and total dose of gonadotropin (MD -257.40; 95% CI: -696.45-181.66; P = 0.25) did not significantly differ between two groups. Based on the present meta-analysis, pre-IVF DHEA treatment may improve the clinical pregnancy rate, increase the number of retrieved oocytes and lower the cancellation rate. The miscarriage rate, total dose of gonadotropin, and E-2 level on the day of hCG administration were not affected.

Abstract:Ethnopharmacological relevance: Dan'e fukang decoction (DFD) is a traditional Chinese medicine formula. DFD obtains 10 herbs, including Salvia yunnanensis C.H.Wright (Zidanshen), Curcuma zedoaria (Christm.) Roscoe (Ezhu), Angelica sinensis (Oliv.) Diels (Danggui), Cyperus rotundus L. (Xiangfuzi), Corydalis yanhusuo (Y.H.Chou & Chun C. Hsu) W.T.Wang ex Z.Y.Su & C.Y.Wu, Bupleurum marginatum Wall. ex DC. (Yanhusuo), Sparganium stoloniferum (Buch.-Ham. ex Graebn.) Buch.-Ham. ex Juz. (Sanleng), Panax notoginseng (Burkill) F.H.Chen (Sanqi), Paeonia lactiflora Pall. (Shaoyao) and Glycyrrhiza uralensis Fisch. (Gancao). DFD is now clinically used for the treatment of menstrual irregularities, dysmenorrhea and menstrual discomfort caused by blood stasis and easing of endometriosis. Based on this, it is reasonable to presume that DFD may be effective in treating incomplete abortion and reducing postpartum bleeding, but no specific studies have been reported so far. Aim of the study: To investigate the efficacy of Dan'e fukang decoction (DFD) in reducing prolonged vaginal bleeding followed by mifepristone induced incomplete abortion and explore the mechanisms of action of DFD in treating incomplete abortion. Methods: An incomplete abortion model of rat was established by single intragastrically administered 8.5 mg/kg mifepristone on the 7th day of pregnancy. From the 8th day of pregnancy, the abortive rats were administered solvent, a positive control drug, or different doses of DFD, respectively for seven consecutive days. The efficacy of DFD was assessed by measuring the vaginal bleeding volume of the rats. Four coagulation parameters and platelet counts were measured. Hematoxylin and eosin (HE) staining was performed to evaluate pathological changes in the uterine embryos. Serum levels of progesterone and estrogen were measured using ELISA. Network pharmacology and transcriptomics were used to predict potential targets and pathways for DFD to reduce hemorrhage. The levels of mRNA related to cell adhesion molecules (CAMs) were detected by RT-qPCR. The levels of progesterone and estrogen receptors and the proteins associated with CAMs pathway in uterine tissues were detected by Western Blot. Results: DFD significantly reduced the volume of vaginal bleeding of the abortive rats and significantly downgraded the pathological scores of uterine embryos. DFD significantly increased serum levels of E2, and had no impact on serum levels of P4 and the protein expression of ER and PR in the uteri of the abortive rats. Pathways in cancer, lipid, focal adhesion and immune-related signaling were predicted to be influenced by DFD via the analysis of network pharmacology. The CAMs signaling was found the most critical pathway regulated by both mifepristone and DFD via RNA-seq assay, followed by axon guidance, basal cell carcinoma, hippo signaling pathway and neuroactive ligand-receptor interaction. Combining the two analytical methods, ICAM-1 was predicted likely the key targeted gene by DFD. Finally, DFD was validated to decrease the protein expression of ICAM-1, ITGB2, ITGB7 and RASSF5 in the uterine tissues, which correlated to suppress the CAMs signaling pathway. Conclusion: DFD significantly reduced hemorrhage. DFD significantly increased the serum levels of E2 and inhibited CAMs signaling pathway, which was likely to be involved in the mechanism of action of DFD facilitating residual uterine embryo expulsion in the rat model of incomplete abortion.

Effect of conservative treatments for incomplete induced abortion and the factors related to successful removal of the remnants

Abstract:Objective:To study the effect of conservative treatments for abortive remnants of induced abortion,and probe into the influencing factors related to the successful removal of the remnants.Methods:A total of 101 patients with abortive remnants after induced abortion were selected by simple random sampling,and randomly divided into 4 groups,with 3 groups treated with different dosages of mifepristone(50 mg/d×28d,100 mg/d×14d,and 200mg/d×7d)and 1 control group.Based on the treatment outcomes,the patients were subgrouped into cured group and uncured group.The parameters of age,weight,gravidity,parity,bleeding time after treatment,baseline β-hCG(human chorionic gonadotrophin) levels,and the size of abortive remnants measured by B-mode ultrasonic diagnostic equipment were analyzed.Rank sum test and logistic regression method were used to analyze the correlated factors with successful treatment of abortive remnants of induced abortion with mifepristone.Results:The group with mifepristone 200mg/d×7d showed the most effective results(P<0.05).Compared with the uncured group,cured group had lower levels of serum β-hCG,smaller abortive remnants,shorter bleeding time after treatment,greater resistance index of blood flow(P<0.05 for all above factors).Logistic regression analysis showed that dosage of mifepristone,baseline β-hCG,the average size of the abortive remnants,and resistance index of blood flow status of the remnant tissue were closely correlated to the treatment outcomes(P<0.05).Conclusion:Mifepristone was effective in the treatment of incomplete induced abortion,and it was most effective when used at a short-term high dose.The baseline β-hCG,size of the abortive remnants and resistance index of bleed flow of the abortive remnants could predict the treatment outcomes.

Abstract:Objective: To compare the pregnancy outcome of the fresh cycle or freeze-thaw cycle embryo transfer of patients treated with a long follicular phase regimen and antagonist regimen, and explore the clinical therapeutic effect of the two regimens. Methods: This was a retrospective study. The data of a total of 543 patients who underwent in vitro fertilization/intracytoplasmic sperm injection and embryo transfer (IVF/ICSI-ET) or frozen-thawed embryo transfer (FET) in Baoding Maternal and Child Health Care Hospital from January 2020 to December 2022 were retrospectively analyzed in this study and were divided into four group to analyze the basic conditions, medication, laboratory indicators and clinical outcomes after embryo transfer of patients in each group. Results: The pregnancy rate and implantation rate in Groups A, B and C were higher than those in Group-D, and the differences were statistically significant (p 0.05). The influencing factors of clinical pregnancy rate were identified by binary Logistic regression analysis. Advanced age was found to be a risk factor for improving the pregnancy rate, while the increase in the number of oocytes retrieved is a protective factor for improving the pregnancy rate. The differences were statistically significant (p<0.05). Effect of different endometrial preparation protocols on pregnancy outcomes in frozen- embryo transfer cycles: a retrospective cohort study

Abstract:Abstract Background Owing to the crucial role the endometrium plays in embryo implantation, the four main endometrial preparation protocols have become important factors in the study of pregnancy outcomes in the FET cycles. Previous studies have shown that the best of these four protocols remains controversial for women undergoing FET. Methods A total of 10333 FET cycles from January 2018 to December 2018 were analyzed in this study. They were categorized into four groups according to endometrial preparation regimen: natural cycles (Group NC,n = 815), hormone replacement therapy cycles (Group HRT ,n = 6434), GnRH agonist artificial cycles (Group GAC,n = 1392) and ovarian stimulation cycles (Group OC, n = 1692). All patients were followed up for at least 1 year. Pregnancy outcomes were compared between the four groups and multiple logistic regression models were used to adjust for the effects of confounding factors. Results The ectopic pregnancy rate (P = 0.627) and miscarriage rate (P = 0.164) were not statistically significant between the four groups. Moreover, biochemical pregnancy rate, clinical pregnancy rate and live birth rate in the NC group were not statistically significant compared to the other three groups. After adjusting for covariates, multiple logistic regression analysis showed no statistical significance in pregnancy outcomes in the HRT, GAC and OC groups compared to the NC group.And the adjusted OR for live births was 0.988 (95 % CI0.847-1.152) for the HRT group, 0.955 (95 % CI0.795-1.146) for the GAC group,0.898 (95 % CI0.754-1.070) for the OC group. Conclusions Our study showed that natural cycles have similar pregnancy outcomes in terms of clinical pregnancy and live birth to the other three endometrial preparation options. As it has other advantages, the natural cycle protocol can therefore be the recommended option for endometrial preparation in the FET population.
