Abstract:To the editor: A 20-year-old Chinese female patient presented with several gradually enlarging, slightly pruritic plaques and nodules on her body over 3 years. Initially the patient had been diagnosed as dermatitis, cutaneous tuberculosis, and granuloma successively in local hospitals, while irresponsive to oral antihistamines, antitubercular agents, or topical corticosteroids. The patient had no fever, weight loss, or any remarkable past medical history. On physical examination, no lymphadenopathy was found. Several sporadic reddish plaques and nodules on the neck and shoulders, behind the ears were observed (Figure 1A). The largest lesion was on the right shoulder about 6 cm × 7 cm in size. They were a little swelling with poor activity. Laboratory examination revealed haematological tests, C-reactive protein, C3, C4, and antinuclear antibody levels were all within normal limits. Histological examination of the largest lesion revealed a diffuse infiltration by histiocytes, plasma cells, neutrophils, lymphocytes, and multinucleated giant cells in the dermis, and some histiocytes displayed emperipolesis (inflammatory cells within the cytoplasm of the histiocytes, with peripheral halos). The principal histiocytes were positive for S100 and CD68, of those sparse positive for CD1a (Figure 1 B-E), periodic acid-schiff staining (PAS, -), and acid-fast bacilli staining (-). Based on the clinicopathological findings, cutaneous Rosai-Dorfman disease (CRDD) accompanied by Langerhans cell hyperplasia was diagnosed. The progressive lesions over 3 years, the ineffectiveness of previous treatment, and the patient's complain of cosmetic appearance might represent a therapeutical challenge. We performed combined treatment, including oral prednisone (30 mg/d initially), thalidomide (75 mg/d), and intralesional injection of corticosteroid. The combined approaches started to work after 3 weeks, but the two larger nodules were irresponsive, prompting local electron beam irradiation therapy (30 Gy, 14 fractions). Following 2 weeks' treatment, it turned out to be effective, then prednisone tapered over the ensuing weeks. After 6 weeks of treatment, all the lesions were markedly improved (Figure 1F).Figure 1.: Clinical and histopathological manifestations of the patient. Immunohistological findings: CD1a sparse (+) among those histiocytes (original magnification ×200); F: Skin lesions relieved markedly after therapy.CRDD, as an extranodal Rosai-Dorfman disease, is generally a benign idiopathic histiocytic disorder with distinctive clinicopathological appearance.1 Histopathologically the entity is characterized by emperipolesis of histiocytes with immunohistochemical staining positive for S100 and CD68 while negative for CD1a.2 In the current case, sparse CD1a (+) histiocytes presented in CRDD were unusual. Considering the diversified morphology of histiocytes, we postulated the movements of histiocytes in an active stage, which contains two possibilities, one might be a kind of reactivity, while the alternative might mean a forewarning for further progression. Distinct movements of histiocytes might lead to different prognosis.3 The etiology of RDD remains uncertain, despite documented associations with disorders of immune regulation4 (possibly following infection or an immune challenge, including herpesvirus 6 and Klebsiella spp.). A recent study defined CRDD as a histiocytic-mesenchymal transition by molecular, immunophenotypic, and sequencing analyses,5 which has thrown a new light on the investigation of its pathogenesis. Combined treatment might be helpful in patients with such entities.
