Abstract:Prof. Lanjuan Li Editor-in-ChiefThe 20th century has witnessed a steep decline of the global burden of infectious diseases due to improved hygiene conditions in large parts of the world and the introduction of vaccines and antimicrobials. However, in today's global landscape infectious diseases still remain a major concern. According to the 2017 Global Burden of Disease Study, communicable diseases still accounted for approximately 8.14 million deaths, which was 14.6% of all age deaths for that year.1 Furthermore, it accounted for 24% of all age years of life lost. Infectious disease outbreaks have been rising since the 1980s2 and pathogenic microbes are now spreading faster geographically due to international traveling. Simultaneously, emerging and re-emerging infectious diseases are being identified faster than ever before. The resurgence of these emerging and re-emerging diseases might be caused by climate change, changing international food chains, and a rise in antimicrobial resistance. According to a recent report on antimicrobial resistance issued by the UK government,3 unless proper action is taken, drug-resistant diseases might cause 10 million deaths per year by 2050, costing the global economy up to 100 trillion dollar. This is a daunting future scenario that we should be able to avert with joined global efforts and extensive surveillance, reporting and intensified fundamental and applied research on infectious microbes and diseases. Over the past decades, China has transformed itself into an international powerhouse with an economic aggregate that has become the second-largest in the world. As a result, China's science and technology have entered a stage of great outbreak, and the growth rate of its scientific research has attracted worldwide attention. However, according to Web of Science, the number of English journals in China is only just over 300. Despite its rapid growth, the number of English journals still lags behind the overall scientific research output of China. Therefore, there is a serious imbalance between the scientific research level and the number of international journals in China. One of the fields that have shown rapid growth in China is in the field of infectious microbes and diseases. China has gradually moved to the forefront in our global efforts to combat infectious diseases. Of the 32 new infectious diseases discovered in the world over the past 20 years, about half have emerged in China. Furthermore, China has been at the center of efforts to contain pandemics of Severe Acute Respiratory Syndrome in 2003,4 H1N1 influenza in 20095 and H7N9 influenza in 2013.6 Many of the Chinese scientists responsible for pushing the field teamed-up to jointly establish the Collaborative Innovation Centre for Diagnosis and Treatment of Infectious Diseases (CCID) in 2012. The CCID was housed at the State Key Laboratory for Diagnosis and Treatment of Infectious Diseases in China and has attracted a large number of domestic and foreign outstanding talents. Since its establishment, authors affiliated with CCID have published over 2500 high-quality international research papers, which are estimated to be approximately 1/25th of all high-level publications in the field of infectious diseases containing Chinese authors. Now, CCID decided to launch a new journal in the field of infectious microbes and diseases to help Chinese international publishing grow up to its potential. The new journal “Infectious Microbes & Diseases” will take the open-access model, and will be co-published by Lippincott Williams & Wilkins and Zhejiang University Press on behalf of the CCID and the State Key Laboratory for Diagnosis and Treatment of Infectious Diseases in China. CCID hopes to run a world-class academic journal that promotes the timely and widespread dissemination of research on infectious microbes and diseases, improves the level of diagnosis, treatment, prevention, and control of infectious microbes and diseases, and safeguard human health. The research field of infectious microbes and diseases deserves an excellent platform based in China to publish and rapidly disseminate excellent research, and which is committed to solving the public health and safety problems caused by infectious microbes and diseases globally.

Abstract:The emergence of the COVID-19 pandemic has starkly exposed our significantly limited ability to promptly identify and respond to emergent biological threats. Consequently, there is an urgent need to advance biotechnological methods for addressing both known and unforeseen biological hazards. Recently, the CRISPR/Cas system has revolutionized genetic engineering, enabling precise and efficient synthetic biology applications. Therefore, this review aims to provide a comprehensive introduction to the fundamental principles underlying the CRISPR/Cas system and assess the advantages and limitations of various CRISPR/Cas-based techniques applicable to the detection of, defense against, and treatment of viral infections. These techniques include viral diagnostics, the development of antiviral vaccines, B cell engineering for antibody production, viral activation/interference, and epigenetic modifications. Furthermore, this review delves into the challenges and bioethical considerations associated with use of the CRISPR/Cas system. With the continuous evolution of technology, the CRISPR/Cas system holds considerable promise for addressing both existing and unforeseen biological threats. Abstract:The complexity of the antimicrobial resistance (AMR) crisis and its global impact on healthcare invokes an urgent need to understand the underlying forces and to conceive and implement innovative solutions. Beyond focusing on a traditional pathogen-centric approach to antibiotic discovery yielding diminishing returns, future therapeutic interventions can expand to focus more comprehensively on host-pathogen interactions. In this manner, increasing the resiliency of our innate immune system or attenuating the virulence mechanisms of the pathogens can be explored to improve therapeutic outcomes. Key pathogen survival strategies such as tolerance, persistence, aggregation, and biofilm formation can be considered and interrupted to sensitize pathogens for more efficient immune clearance. Understanding the evolution and emergence of so-called 'super clones' that drive AMR spread with rapid clonotyping assays may guide more precise antibiotic regimens. Innovative alternatives to classical antibiotics such as bacteriophage therapy, novel engineered peptide antibiotics, ionophores, nanomedicines, and repurposing drugs from other domains of medicine to boost innate immunity are beginning to be successfully implemented to combat AMR. Policy changes supporting shorter durations of antibiotic treatment, greater antibiotic stewardship, and increased surveillance measures can enhance patient safety and enable implementation of the next generation of targeted prevention and control programmes at a global level.

Abstract:Abstract The Mycoses Study Group Education and Research Consortium is a collective of clinicians, researchers, and educators with the common goal to advance awareness, diagnosis, and management of invasive fungal diseases. Clinical Mycology Today, the Mycoses Study Group Education and Research Consortium's biennial meeting, is dedicated to discussing the most pressing contemporary issues facing the field of clinical mycology, promoting clinical, translational, and basic science collaborations, and mentoring the next generation of clinical mycologists. Here, we review the current opportunities and challenges facing the field of mycology that arose from discussions at the 2022 meeting, with emphasis on novel host risk factors, emerging resistant fungal pathogens, the evolving antifungal pipeline, and critical issues affecting the advancement of mycology research.

Abstract:INTRODUCTION: Community-associated MRSA (CA-MRSA) represents a global epidemic which beautifully encapsulates the fascinating ability of bacterial organisms to adapt quickly on an evolutionary basis to the extreme selective pressure of antibiotic exposure. In stark contrast to Healthcare-associated MRSA (HA-MRSA), it has become apparent that CA-MRSA is less straight forward of a challenge in terms of controlling its transmission, and has forced clinicians to adjust empiric management of clinical syndromes such as skin and soft tissue infection (SSTI) as well as pneumonia.AREAS COVERED: This review details the history and epidemiology of CA-MRSA, while covering both current and future treatment options that are and may be available to clinicians. The authors reviewed both historic and more recent literature on this ever-evolving topic.EXPERT OPINION: While development of new anti-MRSA agents should be encouraged, the importance of antimicrobial stewardship in the battle to stay ahead of the curve with regards to the ongoing control of the MRSA epidemic should be emphasised.

Abstract:Poster session 3, September 23, 2022, 12:30 PM - 1:30 PMAs the world enters the third decade of the 21st century, infectious diseases continue to pose major challenges to the survival and well-being of human kind. The COVID-19 pandemic demonstrated how indispensable technological advances and innovations are in our fight against the vast legion of bugs; also, it a fascinating living proof of the adage 'Necessity is the mother of invention' to witness at what stupendous pace not only diagnostics but therapies and even vaccines were developed, approved, validated, distributed and utilized. All fueled by the overwhelming and unprecedented health emergency that was the severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) pandemic. Many more infectious pathogens, presumably viral, are believed to be waiting in the wings; Monkeypox did not waste much time in proving us right!In our country, India, the foresight of our best scientific brains and scientific policymakers led to the establishment of a network of Viral Research and Diagnostic Laboratories (VRDL) at the medical college, state, and regional levels with the premier institutes like National Institute of Virology (NIV), Pune, acting as an able mentor, under the aegis of the Indian Council of Medical Research (ICMR). The VRDL Network proved its worth and played an extremely important role in monitoring and managing the pandemic as, wave after wave, swept across our nation. Moreover, our indigenous drug makers jumped into the fray and excelled on par with our western counterparts in the timely development of effective vaccines.In a country like India, we are faced with additional challenges in the form of certain infectious diseases whose burden is borne mainly by the socio-economically disadvantaged, mainly in the tropics. These diseases and strategies for their mitigation have been taken up by the World Health Organization (WHO) in a big way by means of the Initiative for Control of Neglected Tropical Diseases (NTDs). Besides bacterial and viral diseases, Eukaryotic organisms are also important entities as etiological agents of NTDs.Eukaryotic medical microbiology (EMM) comprising medical mycology, medical protistology, medical helminthology, and medical entomology needs greater attention in India. Except malaria which has been at the forefront owing to the huge disease burden and associated mortality, several other parasitic infestations have indeed been neglected—an issue which the WHO rightly highlighted as part of its NTD elimination initiative.Though Mycetoma, chromoblastomycosis, and other deep mycoses are the only fungal diseases to find a place on the list of NTDs, many other fungal diseases continue to plague our communities. Mucocutaneous candidiasis and dermatophytosis may neither be commoner in the tropics nor are they neglected (rather, dermatophytosis tends to be overtreated in many cases!) but they merit attention simply because of the discomfort and disruption to daily life that they cause.We propose that a network of Medical eukaryotic microbiology (MEM) Laboratories be set up with special emphasis on the NTDs on a regional and national scale in our country to effectively deal with these infectious diseases prevalent in our communities. The diagnostic services offered by these 'MEM-NTDLs' can comprise of:Microscopy, including fluorescent microscopy and special stainsMicroarraysMALDI-TOFMultiplex PCRNGS, including handy DNA sequencing technologies exemplified by the Nanopore MinIONFor dealing with these eukaryotic diseases, we believe that we need to adopt advanced techniques like MALDI and NGS in a big way.Culture, which continues to be the gold standard for bacteriological diagnoses, may now be termed to have historical significance at best in the case of fungal diseases because of the slow turnaround time and by the ubiquitous presence of fungal spores behaving as lab contaminants. Similarly, serological techniques may have a very limited role in the workup of eukaryotic infections owing to the complex antigenic profile of these organisms and the chronicity of the disease conditions. Microscopy can be retained for the relative procedural simplicity and rapidity of results.The 'advanced' diagnostic techniques mentioned above have been around for quite some time now and have been widely applied in the microbiology diagnostic laboratory as well. With requisite training and provision of equipment and appropriately trained technical staff, a MEM-NTDL can be operated successfully at the district level in a planned manner. This would greatly enhance the quality of health services available to our population and enable our country to reach the goal of Health for All in the near future.Following identification, comes the very important aspect of antimicrobial chemotherapy susceptibility testing. Abstract:Respiratory tract infection remains a hot clinical topic.Disappointingly, routine microbiology tests do not meet the needs of target antimicrobial therapy.Widespread practice of antibiotic empiricism has lead to dramatic use of broad-spectrum antibiotics and antimicrobial resistance.The application of new clinical microbiology tests to identify the offending pathogen may allow administration of narrow spectrum antibiotic therapy.To do so,the full clinical microbiology armamentarium with newer point-of-care tests and polymerase chain reaction tests must be exploited.The return of the sputum gram stain and introduction of newer molecular tests would provide information for judicious selection of antimicrobials.

Abstract:Clinical microbiology has possessed a marvellous past, an important present and a bright future. Western medicine modernization started with the discovery of bacterial pathogens, and from then, clinical bacteriology became a cornerstone of diagnostics. Today, clinical microbiology uses standard techniques including Gram stain morphology, in vitro culture, antigen and antibody assays, and molecular biology both to establish a diagnosis and monitor the progression of microbial infections. Clinical microbiology has played a critical role in pathogen detection and characterization for emerging infectious diseases as evidenced by the ongoing COVID-19 pandemic. Revolutionary changes are on the way in clinical microbiology with the application of "-omic" techniques, including transcriptomics and metabolomics, and optimization of clinical practice configurations to improve outcomes of patients with infectious diseases.

Abstract:In 1925, Sir Thomas Horder, a leading physician of his day, gave a lecture, published in this journal, entitled 'Some cases of pyrexia without physical signs'. The paper highlighted what was already a familiar clinical presentation "which taxes our resources to the utmost". Fast-forward through 90 years of careful clinical description, technological innovation in diagnosis and treatment, emergent infections, novel diagnoses, demographic shifts, and radical changes in the health economy. Sir Thomas would find certain aspects familiar, and others revolutionary, in the differential diagnosis and management of the 21st century patient with pyrexia of unknown origin (PUO). Within high-income settings, the proportion of cases due to infection has declined, albeit unevenly. The era of untreated HIV, and the consequences of iatrogenic intervention and immunosuppression, led to Durack and Street's subclassification of the condition in the early 1990 s into classic, nosocomial, neutropenic and HIV-associated PUO. Shifts towards ambulatory care have driven a change in the definition of many diseases. An era of observant clinicians, who lent their names to eponymous syndromes, followed by meticulous serological, genetic and clinicopathological correlation, generated a battery of diagnoses that, along with malignancy, form a large proportion of diagnoses in more recent clinical care. In the current era, universal access to cross-sectional imaging and an infinite array of laboratory tests has undermined the attention paid to history and examination. In some areas of the clinical assessment, such as assessing the fever pattern, this shift is supported by research evidence. The issues that need to be addressed in the next 90 years of technological innovation, information sharing and health service transformation are likely to include: transcriptomic approaches to diagnosis; the place of positron emission tomography (PET) in the diagnostic pathway; the optimal management of high ferritin states; and the most cost-effective diagnostic environment, in the face of this era of specialisation and fragmentation of care. In the meantime, this review covers some important early 21st century lessons to be shared in avoiding diagnostic pitfalls and choosing empirical therapy.

Abstract:Purpose of the review This review aims to evaluate the incorporation of rapid molecular diagnostics (RMD) in antimicrobial stewardship programs (ASPs) in the management of patients in the emergency department (ED) and intensive care unit (ICU), highlighting a shift from conventional microbiological diagnostic tests to RMD strategies to optimize antimicrobial use and improve patient outcomes. Recent findings Recent advances in RMD have demonstrated the superior accuracy of RMD in identifying pathogens, combined with shorter turnaround times. RMD allows speeding up of antimicrobial decision making in the ED and facilitates faster escalation when empirical therapy was inappropriate, as well as more efficient de-escalation of empirical therapy later in the course of the treatment. Implementation of RMD however may be challenging. Summary RMD hold great value in simplifying patient management and mitigating antimicrobial exposure, particularly in settings with high levels of antimicrobial resistance where the use of broad-spectrum antimicrobials is high. While the impact on the use of antimicrobials is significant, the impact on patient outcomes is not yet clear. Successful integration of RMD in clinical decision making in the ED and ICU requires a team approach and continued education, and its use should be adapted to the local epidemiology and infrastructure.

Advances in Microbial Physiology

Abstract:Over the last two centuries, great advances have been made in microbiology as a discipline. Much of this progress has come about as a consequence of studying the growth and physiology of individual microbial species in well-defined laboratory media; so-called "axenic growth". However, in the real world, microbes rarely live in such "splendid isolation" (to paraphrase Foster) and more often-than-not, share the niche with a plethora of co-habitants. The resulting interactions between species (and even between kingdoms) are only very poorly understood, both on a theoretical and experimental level. Nevertheless, the last few years have seen significant progress, and in this review, we assess the importance of polymicrobial infections, and show how improved experimental traction is advancing our understanding of these. A particular focus is on developments that are allowing us to capture the key features of polymicrobial infection scenarios, especially as those associated with the human airways (both healthy and diseased).
