Abstract:de Winter syndrome, also termed anterior ST-segment elevation myocardial infarction (STEMI) equivalent, is estimated to be present in approximately 2% of patients with acute myocardial infarction, but is often under-recognized by clinicians. Therefore, de Winter syndrome is associated with increased morbidity and mortality. We report a 51-year-old man with typical chest tightness and a characteristic electrocardiographic pattern without classic ST-segment elevation, but with acute nearly total occlusion of the left anterior descending coronary artery. Although the patient presented as a non-STEMI case, the definite diagnosis of de Winter syndrome was made on the basis of clinical and electrocardiographic findings. The patient's symptom of chest tightness was relieved immediately after acute percutaneous coronary intervention and the left ventricular ejection fraction had not deteriorated at 1 month of follow-up.
A Case of Acute Non-ST Elevation Myocardial Infarction Later Revealed by Contrast-Enhanced Computed Tomography

Abstract:BACKGROUND Acute coronary syndrome is life-threatening. The diagnosis can be confirmed by electrocardiography (ECG) and serum cardiac biomarkers. Early diagnosis and treatment of non-ST segment elevation myocardial infarction (NSTEMI) is important because delayed treatment is associated with poor prognosis, especially in older adults. CASE REPORT An 82-year-old woman presented to the Emergency Department (ED) with epigastric and back pain. Despite the symptoms, the electrocardiogram revealed no abnormality, and the high-sensitivity cardiac troponin (Hs-cTn) value was below the detection limit. Chest contrast-enhanced computed tomography (CT) performed to exclude fatal diseases such as aortic dissection revealed no obvious abnormalities. The patient's symptoms improved and she was discharged. On the following day, the radiologist reviewed the CT and noted reduced cardiac uptake of contrast medium, and so suspected a subendocardial infarction. The patient was immediately recalled to the ED. She had no symptoms, but her Hs-cTn level was markedly elevated and asynergy was found on echocardiography. Emergency coronary angiography revealed complete obstruction of the left anterior descending coronary artery. CONCLUSIONS Similar to patients with acute ST segment elevation myocardial infarction, those with unstable angina or NSTEMI should be treated early. Delayed diagnosis and treatment of acute coronary syndrome is associated with poor prognosis, especially in older adults. Therefore, in patients presenting to the ED with chest pain, careful attention should be paid to myocardial staining in addition to the aorta, pulmonary arteries, and abdominal organs, when performing contrast-enhanced CT.
Approach to Imaging of Patients Presenting With Acute Coronary Syndrome With No Culprit Lesion Identified at Angiography

Abstract:An 81-year-old male presents to the local emergency department (ED) with chest pain that had been present for 2 weeks. He describes the pain as retrosternal in location and occurs in a "coming and going" pattern. After climbing 10 flights of stairs, he feels "out of breath." His medical history is significant for nonobstructive hypertrophic cardiomyopathy, with no previous treatment with septal alcohol ablations or surgical interventions and no history of arrhythmias. Initial ECG in the ED revealed ST elevations in V2-5 with poor R wave progression in the anterior leads, with initial cardiac troponin (cTn) at the 99th percentile and falling on the reassessment 2 hours later. Coronary computed tomography angiography (CCTA) subsequently demonstrated a ruptured plaque in the left anterior descending coronary artery, consistent with a diagnosis of type I myocardial infarction (MI).

Abstract:OBJECTIVE: To study various characteristics of chest pain in acute myocardial infarction patients.METHODOLOGY: A total of 331 patients of AMI admitted at Cardiology unit Nishtar Hospital Multan and Chaudhry Pervez Elahi Institute of Cardiology Multan, irrespective of the age and gender, were included in this study. The study duration was one year starting from June 2011 to June 2012. Non-probability purposive sampling technique was used in this descriptive study. Informed consent to participate in this study was taken. Data were entered and analyzed using SPSS-11.RESULTS: A total number of 331 patients with AMI were included in the study. Mean age was 54.99±11.25 years with minimum age 20 years and maximum age 90 years. It included 264(79.8%) male and 67(20.2%) female patients with male to female ratio of 3.9:1. Out of these 331 patients 308 (93.1%) patients reported chest pain as the presenting complaint. Remaining 23(6.9%) presented with clinical features other than chest pain. There were 127(38.4%) patients with pre-cordial chest pain, 115(34.7%) had retrosternal chest pain, 58(17.5%) were having epigastric pain. Severe chest pain was seen in 281(84.9%) patients while 26(7.9%) had only mild chest discomfort. Radiation of the pain to shoulder, neck and jaw was seen in 75 (22.7%) patients. In 42(12.7%) patients, pain radiated to both sides of chest. Another 55(16.6%) patients had pain radiation to chest, shoulder, upper arm and ulnar side of left forearm. Chest pain radiation to interscapular region along with both sides of chest was present in 10(3.0%) patients. In 11(3.3%) patients' pain radiated only to left side of chest. Pain persisting for >20 minutes was reported by 298 (90%) patients while only 10(3.1%) had pain persisting for <20 minutes.CONCLUSION: There is considerable overlap in chest pain of cardiac as well as non cardiac causes. However, vigilant evaluation of characteristics of chest pain in history taking may help to overcome this dilemma. Severe and prolonged precordial chest pain in a male patient between the age of 41-70 years, with pain radiation to left shoulder, neck and jaw is highly suggestive of AMI.

Abstract:Clinical introductionVignette A man in his 40s presented to our emergency department with sudden onset of severe central chest pain radiating to his left arm. He was sweaty, clammy and had accompanying breathlessness. There was no antecedent angina. His cardiovascular risk profile was low except for being a smoker (15 cigarettes per day). There was no medical history and he was not on any medical treatment. The 12-lead ECG showed sinus rhythm with hyperacute T waves in leads V3 and V4. Coronary angiography was performed using right radial access. The right coronary artery was small and free of disease. The left system showed no disease in the left main stem, but mild plaque disease in the dominant left circumflex artery. Angiographic findings of the left anterior descending artery are shown in figure 1A. Intravascular ultrasound was also performed (figure 1B).

Abstract:Background: Both acute pancreatitis and acute myocardial infarction (AMI) are rapidly progressive and frequently fatal diseases that can be interrelated and lead to a vicious cycle for further problems. The concomitant occurrence of AMI and acute pancreatitis is rare but critical, and efficient diagnosis and treatment of such patients are challenging. Case Summary: We reported an uncommon case of abnormal ECG findings in a 63-year-old woman with acute pancreatitis. The patient exhibited increased biomarkers of myocardial injury, such as creatine kinase-MB (CK-MB) and troponin T, as well as ST segment elevation in inferior leads II, III, and aVF. Both of these have been previously observed in patients with acute abdomen in the absence of ST-segment elevation myocardial infarction (STEMI), including pancreatitis. In addition, lacking complaints of chest pain or tightness was also supportive of this idea. Echocardiography indicated abnormalities in the functioning of the left inferior posterior wall segments and decreased overall systolic function of the left ventricle with a 51% ejection fraction. Eventually, AMI was diagnosed after coronary computed tomography angiography (CCTA) showing critical stenosis of the right coronary artery and left anterior descending artery segments. The patient was urgently transferred to intensive care unit and was treated with anticoagulation, antiplatelet aggregation, lipid-lowering and other palliative drugs. Conclusion: Concomitant acute pancreatitis and AMI are often considered to be critical conditions with a poor prognosis. Therefore, it is important to rapidly identify this condition and consider transferring patients for multidisciplinary supportive care.

Abstract:Management of patients with acute chest pain poses a significant challenge in identifying those requiring urgent coronary reperfusion. Electrocardiogram (ECG) constitutes the cornerstone in making prompt clinical decisions by identifying ST‐segment elevation, commonly associated with ST‐segment elevation myocardial infarction. It is important to note that ST‐segment elevation can also be a manifestation of various cardiac and non‐cardiac conditions, from acute myocarditis, early repolarization syndrome, acute pericarditis, and left bundle branch block to unknown origins. The similarity of ECG changes among these conditions complicates clinical differential diagnosis, necessitating a detailed medical history and thorough examinations. Here, we presented a case of a 52‐year‐old female with chest pain and unidentified convex ST‐segment elevation. Considering the negative emergent coronary angiography results, normal echocardiography, and long‐lasting ST‐segment elevation for the following 1 year, the final diagnosis was non‐myocardial infarction, probably related to a prior cerebral haemorrhage.

Abstract:Acute coronary syndrome is an umbrella term encompassing three types of coronary artery disease that affect millions worldwide annually. Despite the availability of diagnostic tests (blood analysis, imaging, electrocardiogram, and screening tools), the diagnosis of myocardial infarction (MI) is still sometimes missed. According to the Centers for Disease Control and Prevention, the reported prevalence of heart disease is higher among males than females, with adults over the age of 75 having the highest prevalence. Typical "heart attack" features include chest pain that feels like pressure or squeezing, pain or discomfort in one or both arms that can radiate to the neck or jaw, shortness of breath, diaphoresis, nausea, vomiting, and lightheadedness. However, there are three subgroups where the typical warning signs do not always present: the elderly, individuals with diabetes, and females. The following is an atypical case presentation of unstable angina and non-ST-elevation MI.

Abstract:Background: Chest pain/discomfort is the hallmark symptom of Acute Myocardial Infarction (AMI). An important percentage of AMI patients may not present with chest pain/discomfort. We hypothesized that the presence or absence of chest pain/discomfort at the time of admission may be influenced by the AMI classification type: ST segment elevation MI (STEMI) or Non STEMI (NSTEMI). Methods: We analyzed data from 1,143,513 AMI patients (565,195 with STEMI; 578,318 with NSTEMI) in the National Registry of Myocardial Infarction (NRMI) during the 1994-2006 period. Multivariable models were developed to examine the predictors of AMI presentation without chest discomfort by AMI type, adjusting for age, gender, ethnicity, payer status, cardiac risk factors, past medical history and past cardiac history. Results: The proportion of AMI patients who presented with chest pain was significantly higher in patients with STEMI (72.9%) than NSTEMI (56.4%) (p <0.001). Conversely, percentage of AMI patients who presented without chest pain was higher in NSTEMI (43.6%) and much lower in STEMI (27.1%) p <0.001. Abstract:Background: Pulmonary embolism (PE) typically presents with chest pain, tachypnea, hemoptysis, syncope, and increased markers of myocardial injury. On an electrocardiogram (ECG), sinus tachycardia, right bundle branch block (RBBB), S1Q3T3 pattern, and/or precordial T-wave inversion may be observed. Despite being one of the common causes of chest pain, a third-degree atrioventricular block (III° AVB) is rare in cases of PE, which can lead to difficulties in diagnosis or even overlooking this condition. Case summary: In this case report, we present a patient who was transferred to our hospital with suspected acute myocardial infarction (AMI). The patient's ECG showed ST-segment elevation in the inferior wall and a III° AVB, combined with significantly increased markers of myocardial injury. Interestingly, the patient also had a history of cerebral hemorrhage (ICH) for 7 days prior to being transferred to our hospital. After undergoing a systematic examination and evaluation, the final diagnosis for the patient was PE. Conclusions: In addition to considering common symptoms, it is important not to overlook rare symptoms when diagnosing a disease. This case serves as an example of how the misdiagnosis rate for PE can be reduced by conducting a comprehensive clinical evaluation and paying attention to all clinical clues and examination results.

Abstract:Background: Percutaneous coronary intervention (PCI) is extensively used to treat acute coronary syndromes (ACS). Acute mesenteric ischemia is a life-threatening disease if untreated. Case summary: An 81-year-old female presented with 3 d of lethargy and 1 d of dyspnea. On November 16, 2021, the patient developed a coma. Her oxygen saturation dropped to 70%-80%, the patient was admitted to the intensive care unit for further treatment. Chest computed tomography (CT) showed chronic bronchitis, emphysema, and multiple lung infections. Abdominal CT scan showed no obvious abnormalities, but have severely calcified abdominal vessels. The patient received assisted ventilation, and vasoactive, and anti-infection drugs. Troponin level was elevated. Since the patient was in a coma, it could not be determined whether she had chest pain. The cardiologist assumed that the patient had developed ACS; therefore, the patient underwent PCI via the left femoral artery approach, and no obvious abnormalities were found in the left and right coronary arteries. On the second postoperative day, the patient presented with abdominal distension and decreased bowel sounds; constipation was considered and a glycerin enema was administered. On day 4, the patient suddenly lost consciousness, and had decreased blood pressure, abdominal wall swelling with increased tension, and absence of bowel sounds. An urgent abdominal CT scan revealed gas in her hepatic portal system with extensive bowel wall necrosis. The patient died on day 5 due to intractable shock. Conclusion: The potential serious complications in patients undergoing PCI, especially the patients who are hemodynamically unstable and have severely calcified abdominal vessels, should all be considered.
Mimicking descending necrotizing mediastinitis as acute myocardial infarction in a patient with severe coronary artery disease: A case report

Abstract:It is known that the descending necrotizing mediastinitis (DNM) is a potentially lethal disease of the mediastinum with a mortality rate as high as 40%. [ 1 ] Differential diagnosis for chest pain is often complicated by the presence of DNM, leading to potential misdiagnosis as acute myocardial infarction (AMI). [ 2–4 ] In this case, the patient presented with chest pain and an elevation of troponin I (TnI), which raised suspicions of AMI. Additionally, the patient's medical history, laboratory results, the presence of a severe coronary stenosis observed on coronary angiography. Here, we present a rare case that the diagnosis of chest pain was challenging due to the presence of severe coronary artery stenosis leading to the initial diagnosis of AMI, accompanied by concurrent DNM.

Abstract:Background: Acute pericarditis is a rapid inflammatory condition of the pericardium with both infectious and non-infectious etiology. Most acute pericarditis is self-limited, with a small portion evolving rapidly. The definitive diagnosis of acute pericarditis often requires detailed physical examination, ECG, echocardiography, blood analysis and chest X-ray. It's usually challenging to distinguish acute pericarditis from ST-elevated myocardial infarction (STEMI) due to the similar ECG characteristics (ST segment change). Here we present a case of purulent pericarditis probably caused by esophageal perforation. Case: A 52 year-old male presented with chest pain and dyspnea for 16 h. ST-segment elevation and positive cardiac markers lead to the initial diagnosis of ST-elevated myocardial infarction. Coronary angiography demonstrated normal coronary artery, while transthoracic echocardiography (TTE) showed massive pericardial effusion. Then, pericardiocentesis was performed with 250 ml of yellowish-green pus-like fluid extracted. A detailed history examination revealed a week history of possible esophageal perforation caused by a fishbone. And a further computed tomography (CT) demonstrated the presence of pneumomediastinum, and effusions in mediastinum, which lead to the diagnosis of purulent pericarditis. However, the patient's family refused further treatment and the patient died soon after discharge. Conclusion: The differential diagnosis of chest pain should include acute pericarditis, which can be equally critical and fatal. And it's important to note the peculiar characteristics of acute pericarditis, which include concave and diffused ST-segment elevation, PR segment depression, and the ratio of ST-segment elevation to T wave >0.24 in lead V6. Moreover, comprehensive medical history and physical examination are crucial to the differential diagnosis of chest pain patients.

Abstract:A 57-year-old man with a history of hypertrophic cardiomyopathy diagnosed by echocardiography experienced atypical chest pain and dyspnea for 6 months. A rest Tc-MIBI myocardial SPECT imaging and a F-FDG myocardial PET/CT imaging were performed, which showed multiple matched myocardial perfusion/metabolism defects in the left ventricle, indicating scar tissue. Surprisingly, subsequent contrast coronary angiography revealed no significant coronary artery stenosis.

Abstract:A 66-year-old woman with a history of hypertension complained about sudden short-term memory loss. On arrival to our outpatient clinic, she was alert and oriented and did not have chest pain or shortness of breath. Neurological and neuropsychological examinations were within normal limits. In light of a transient anterograde amnestic attack and no neurological focal deficit, we clinically diagnosed transient global amnesia (TGA). To confirm whether there was an intracranial lesion or not, diffusion-weighted MRI of the brain was performed, and revealed hyper-intense lesions in the left hippocampus and right corpus callosum. Consequently, the patient was admitted to our hospital on follow-up for suspected cerebral infarction. On day 1, laboratory tests indicated an elevated troponin I level, and electrocardiogram revealed an inverted T wave in the inferior leads. Coronary angiography on day 9 of admission demonstrated severe stenosis of the right coronary artery, leading to a diagnosis of non-ST elevation myocardial infarction. Although TGA itself typically has a favorable prognosis, clinicians should consider potential concurrent painless myocardial infarction in patients with TGA.

Abstract:Acute coronary syndrome (ACS) is associated with high mortality rates. Although the goal was to achieve a missed diagnosis rate of < 1%, the actual data showed a rate of > 2%. Chest pain diagnosis has remained unchanged over the years and is based on medical interviews and electrocardiograms (ECG), with biomarkers playing complementary roles. We aimed to summarize the key points of medical interviews, ECG clinics, use of biomarkers, and clinical scores, identify problems, and provide directions for future research. Medical interviews should focus on the character and location of chest pain (is it accompanied by radiating pain?) and the duration, induction, and ameliorating factors. An ECG should be recorded within 10 minutes of the presentation. The serial performance of an ECG is recommended for emergency department (ED) evaluation of suspected ACS. Characteristic ECG traces, such as Wellens syndrome and De Winter T-waves, should be understood. Therefore, troponin levels in all patients with suspected ischemic heart disease should be examined using a highly sensitive assay system. Depending on the ED facility, the patient should be risk stratified by serial measurements of cardiac troponin levels (re-testing at one hour would be preferred) to determine the appropriate time to perform an invasive strategy for a definitive diagnosis. The diagnostics should be based on Bayes' theorem; however, care should be taken to avoid the influence of heuristic bias.
A Case Report on Unstable Angina Pectoris Manifesting as Orofacial Pain

Abstract:Cardiac ischemia, such as angina pectoris or myocardial infarction, is associated with pain in the oral cavity, lower jaw, head, or neck, or spanning from the left upper arm to the shoulder. When presenting to a dentist, however, appropriate treatment for such patients is often delayed, as dental problems are usually the first to be suspected when the chief complaint is orofacial pain. This report describes a case of a 70-year-old woman who was aware of pain and a burning sensation in the oral cavity upon exertion for a year prior to presenting at our clinic. She had been examined by her family physician, an otolaryngologist, and another dentist, none of whom found any abnormalities other than suspected periodontal disease and caries, for which she was treated. An examination at our clinic revealed no abnormal dental findings that would have been consistent with the mandibular pain, however. Although no chest symptoms were reported, pain was elicited on exertion, suggesting cardiogenic toothache. An immediate referral to a cardiologist was therefore made on the same day. The patient visited the cardiology department of the University Hospital of Tokyo Dental College 6 days later. The increased frequency of symptoms on exertion suggested unstable angina, and the patient was admitted to the emergency department on the same day. Emergency coronary angiography showed that right coronary artery #1 was 99% stenosed proximally (highly calcified plaque). The diagnosis was unstable angina pectoris, with the right coronary artery #1 as the responsible lesion, and percutaneous coronary angioplasty was performed on the same day. Subsequently, all the orofacial pain disappeared, confirming unstable angina as the cause. The pain characteristics in this case were consistent with pain associated with cardiac ischemia, which led to the immediate referral to the cardiology department. In cases of toothache associated with cardia ischemia, it is essential to seek cardiological care as soon as possible.
