Abstract:Objective To evaluate the value of CT enterography (CTE)imaging scores in assessing the clinical activity of Crohn's disease (CD).Methods Forty-eight patients with pathologically or clinically confirmed CD were collected retrospectively.The CTE findings associated with disease activity were analyzed and scored,and the well-known Crohn's disease activity index (CDAI)was also performed.The correlation between CTE score and CDAI score was analyzed by Pearson correlation method.The diagnostic efficacy and optimal diagnostic threshold of CTE score for clinical activity of CD were evaluated by ROC curve analysis.Results There was a good correlation between the CTE score and CDAI (r =0.790,P <0.001).ROC curve analysis showed the area under the curve (AUC)was 0.913 (P <0.001).The optimal diagnostic threshold was 5.5.A CTE score of 5.5 or above indicated the disease was active,with a sensitivity of 84.2％,specificity of 80％,positive predictive value of 94.1％,negative predictive value of 57.1％ and the Youden index of 0.642.Conclusion The CTE comprehensive score has high sensitivity and specificity for estimating clinical activity of CD.
A Novel Computed Tomography Enterography Radiomics Combining Intestinal and Creeping Fat Features Could Predict Surgery Risk in Patients with Crohn’s Disease

Abstract:OBJECTIVE:The objective of this study is to segment creeping fat and intestinal wall on computed tomography enterography (CTE) and develop a radiomic model to predict 1-year surgery risk in patients with Crohn's disease. METHODS:This retrospective study included 135 Crohn's disease patients who underwent CTE between January and December 2021 (training cohort) and 69 patients between January and June 2022 (test cohort). A total of 1874 radiomic features were extracted from the intestinal wall and creeping fat respectively on the venous phase CTE images, and radiomic models were constructed based on the selected features using the Boruta and extreme gradient boosting algorithms. The combined models were established by integrating clinical predictors and radiomic models. The receiver operating characteristic curve, calibration curve, and decision curve analyses were used to compare the predictive performance of models. RESULTS:In the training and test cohorts, the area under the curve (AUC) values of the creeping fat radiomic model for surgery risk stratification were 0.916 and 0.822, respectively, similar to the intestinal model with AUC values of 0.889 and 0.822. Moreover, the combined radiomic model was superior to the single models, showing good discrimination with the highest AUC values (training cohort: 0.963; test cohort: 0.882). Addition of clinical predictors to the radiomic models failed to significantly improve the diagnostic ability. CONCLUSION:The CTE-based creeping fat radiomic model provided additional information to the intestinal radiomic model, and their combined radiomic model enables accurate surgery risk prediction of Crohn's disease patients within 1 year of CTE.

Abstract:Background: Assessment of Crohn's disease (CD) activity is important to identify patients with active inflammation for therapy management. Quantitative analysis can provide objective measurement of disease presence. Purpose: To evaluate the feasibility of quantitative analysis of contrast-enhanced dual-energy computed tomography (DECT) data in detection of small bowel inflammation in patients with CD with an emphasis on iodine quantification. Material and methods: DECT enterography was prospectively performed in 20 patients with active CD and in 20 healthy individuals, as the control group. Iodine overlay images were created. Wall thickness, attenuation, absolute iodine density, relative iodine density, and fat fraction were measured in the terminal ileum of all patients by two radiologists. Intraclass correlation coefficients were calculated to assess inter-rater agreement. Parameters were compared between patient groups using mixed model analysis. Receiver operating characteristic (ROC) analysis was performed. Results: Both absolute and relative iodine density were significantly higher in active disease than in normal small bowel (all P < 0.001). In contrast, measurement of fat fraction was not significantly different in affected terminal ileal loops compared to normal terminal ileum ( P = 0.075). ROC analysis demonstrated a similar excellent diagnostic accuracy of wall thickness, attenuation, and absolute and relative iodine density with area under the ROC curve (AUC) values in the range of 0.96 for attenuation to 1 for relative iodine density. Conclusion: DECT with iodine quantification can be used in distinguishing normal small bowel from active inflammatory CD. Further research should investigate the value of iodine quantification in grading CD activity and in monitoring therapeutic response.
Study of Spectral CT Enterography in the Evaluation of Crohn's Disease Activity by Regression Mathematical Model

Abstract:Purpose: To investigate value of spectral CT enterography in the evaluation of Crohn's disease activity by logistic regression method. Methods: The spectral CT enterography data from a cohort of 140 Crohn's disease patients from May 2010 to September 2013, who were confirmed by endoscopy and histopathology were analyzed retrospectively. Eighty-three cases were diagnosed as active and 57 cases were inactive. The parameters for differentiation between them were screened: 1. bowel thickness of the terminal ileum; 2. CT value of the bowel wall in AP and PVP; 3. relative contrast enhancement; 4. comb sign score; 5. type of enhancement pattern; 6. morphology of the bowel wall; 7. extra-luminal manifestations; 8. iodine concentration. SPSS for Windows version 16.0 software was used to analyze the data and screen for parameters which were valuable for the differential diagnosis. Then, parameters with statistical significance were further analyzed by multivariable logistic regression. ROC analysis was performed to assess the diagnostic efficacy of regression mathematical model. The area under the ROC curve, sensitivity, specificity, accuracy, positive predictive value and negative predictive value were evaluated. Results:Parameters which were helpful in differentiating active CD from inactive CD included bowel thickness, mural homogenous enhancement and the iodine concentration in PVP by multivariable logistic regression analysis. The area under the ROC curve were 0.961, sensitivity, specificity, accuracy, positive predictive value and negative predictive were 95.8％, 98.2％, 96.4％, 97.5％, 93.3％, respectively and a predictive boundary value of 0.753 was obtained.Conclusion: The accuracy of the regression mathematical model established by using the spectral CT enterographic parameters was 96.4％, which suggests that spectral CT enterography is helpful for improving the accuracy in assessment of CD activity.

Abstract:Objective To evaluate the application of multislice CT enterography(MSCTE) in diagnosis of Crohn's diease.Methods MSCTE were performed in 48 patients with suspected Crohn's disease(CD).Using enteroscopy and pathological findings as gold standard,the sensitivity and specificity of MSCTE imaging in diagnosis of Crohn's disease were analyzed.Results The diagnostic sensitivity and specificity of MSCTE were 89.2% and 36.4% respectively.The terminal ileum was the most frequently involved in CD(81.1%).The most common signs of MSCTE were annular thickness of intestinal wall(89.2%),mucosal hyperenhancement(97.3%),mucosal surface coarse,mural stratification enhancement(88.9%),moderate to severe segmental luminal narrowing(86.7%).The common complications were ileus,mesenteric lymph nodes and the comb sign(prominent vasa recta) and intestinal fistula.Conclusion MSCTE is of value in diagnosis of CD and its complications.MSCTE combined with enteroscopy,pathology and clinical manifestation can improve the diagnosis rate.

Abstract:Background Distinguishing Crohn's disease (CD) and intestinal Behçet's disease (BD) is difficult in clinical practice. Aim To evaluate the ability of CT enterography (CTE) to enhance the diagnostic value of endoscopy in differentiating CD from intestinal BD and to establish differential diagnosis models. Methods A total of 113 patients with CD and 70 patients with intestinal BD from seven tertiary inflammatory bowel disease centers were enrolled. The univariate and multivariate analyses were used by SAS software version 9.2. Three differential scoring models based on the multivariate analysis of endoscopic features alone (model 1), endoscopic features combined with clinical symptoms (model 2), and endoscopic features combined with clinical symptoms and CTE (model 3) were established. Results The results showed that model 2 increased the efficacy of model 1 in differential diagnosis and model 3 had the highest accuracy of 84.15% at a cutoff value of two points. The scoring of model 3 was as follows: genital ulcer (−3 points), skin lesions (−3 points), oval ulcer (-2 points), longitudinal ulcer (1 point), number of ulcers > 5 (3 points), inflammatory polyps (2 points), mucosal severe enhancement (2 points), and fibrofatty proliferation (1 point). Conclusion Clinical symptoms and CTE increased the ability of endoscopy to differentiate CD from intestinal BD.

Virtual monoenergetic dual-layer, dual-energy CT enterography: optimization of keV settings and its added value for Crohn's disease

Abstract:Objectives: To determine the optimal keV on dual-layer, dual-energy CT enterography (dlDE-CTE) and to investigate the added value of virtual monoenergetic images (VMIs) for the diagnosis of active Crohn's disease (CD). Methods: We collected 76 patients (including 45 CD patients) who underwent dlDE-CTE. CD was diagnosed using ileocolonoscopy. Conventional polychromatic images (PCI) were reconstructed using an iterative reconstruction algorithm at 120 kVp, and VMI at 40 keV (VMI40), 55 keV (VMI55), and 70 keV (VMI70). Contrast-to-noise ratio (CNR) was compared using Kruskal-Wallis test. Three radiologists independently reviewed PCI and subsequently combined PCI and the optimized VMI for the diagnosis of active CD using a 5-point scale. Multi-reader multi-case receiver operating characteristic analysis was performed. Results: Mean ± standard deviation of CNRs for both normal (13.6±6.5, 6.1±3.2, 2.0±2.1, 1.9±1.6; P<0.001) and abnormal (9.4±7.3, 6.5±4.8, 4.9±3.1, 3.7±2.3; P<0.001) bowels were significantly greatest on VMI40, followed by VMI55, VMI70, and PCI. When VMI40 were added to PCI, overall area-under-the-curve of the three radiologists was significantly improved from 0.891 to 0.951 for diagnosing active CD (P=0.009). Conclusions: The lowest monoenergetic images (VMI40) provided the best CNR on dlDE-CTE. Furthermore, the diagnostic performance for diagnosing active CD can be significantly improved with the addition of VMI40. Key points: • CNR for both normal and abnormal bowel walls is greatest on VMI 40 . • Subjective image quality on VMI 40 is better than those on PCI. • When VMI 40 images are added to PCI, radiologists' diagnostic performance can be improved.

Abstract:Rationale and Objectives: This study evaluates the accuracy of dual-energy spectral computed tomography (DEsCT) imaging with the aid of computer-aided diagnosis (CAD) system in assessing serosal invasion in patients with gastric cancer.Materials and Methods: Thirty patients with gastric cancer were enrolled in this study. Two types of features (information) were collected with the use of DEsCT imaging: conventional features including patient's clinical information (eg, age, gender) and descriptive characteristics on the CT images (eg, location of the lesion, wall thickness at the gastric cardia) and additional spectral CT features extracted from monochromatic images (eg, 60 keV) and material-decomposition images (eg, iodine- and water-density images). The classification results of the CAD system were compared to pathologic findings. Important features can be found out using support vector machine classification method in combination with feature-selection technique thereby helping the radiologists diagnose better.Results: Statistical analysis showed that for the collected cases, the feature "long axis" was significantly different between group A (serosa negative) and group B (serosa positive) (P <.05). By adding quantitative spectral features from several regions of interest (ROls), the total classification accuracy was improved from 83.33% to 90.00%. Two feature ranking algorithms were used in the CAD scheme to derive the top-ranked features. The results demonstrated that low single-energy (approximately 60 key) CT values, tumor size (long axis and short axis), iodine (water) density, and Effective-Z values of ROls were important for classification. These findings concurred with the experience of the radiologist.Conclusions: The CAD system designed using machine-learning algorithms may be used to improve the identification accuracy in the assessment of serosal invasion in patients of gastric cancer with DEsCT imaging and provide some indicators which may be useful in predicting prognosis.

Abstract:Crohn’s disease (CD) is a chronic inflammatory disease of the digestive tract with unknown etiology. It follows a relapse-remission pattern, making disease activity assessment crucial for treatment. Our study aims to evaluate the diagnostic accuracy of various imaging modalities and to validate and compare the International Bowel Ultrasound Segmental Activity Score (IBUS-SAS), the multidetector computed tomography enterography score (MDCTEs), and the simplified endoscopic activity score for Crohn’s disease (SES-CD). We assessed diagnostic performance using the CD Activity Index (CDAI). We first categorized patients into remission and active groups. For those in the active stage, we further categorized them into mild/moderate and severe activity groups. We used Spearman rank correlation to evaluate the relationships among IBUS-SAS, bowel wall thickness (BWT), Color Doppler imaging signal (CDS), inflammatory fat (i-fat), bowel wall stratification (BWS), and clinical inflammatory indicators. A total of 103 CD patients were evaluated. The IBUS-SAS cut-off for remission and activity was 23.8, with an AUC of 0.923, sensitivity of 91.4

Abstract:To build computed tomography enterography (CTE)-based multiregional radiomics model for distinguishing Crohn's disease (CD) from intestinal tuberculosis (ITB). A total of 105 patients with CD and ITB who underwent CTE were retrospectively enrolled. Volume of interest segmentation were performed on CTE and radiomic features were obtained separately from the intestinal wall of lesion, the largest lymph node (LN), and region surrounding the lesion in the ileocecal region. The most valuable radiomic features was selected by the selection operator and least absolute shrinkage. We established nomogram combining clinical factors, endoscopy results, CTE features, and radiomic score through multivariate logistic regression analysis. Receiver operating characteristic (ROC) curves and decision curve analysis (DCA) were used to evaluate the prediction performance. DeLong test was applied to compare the performance of the models. Abstract:Objectives: To evaluate the efficacy of quantitative parameters from dual-energy CT (DECT) and basic CT features in predicting the postoperative early recurrence (ER) of pancreatic ductal adenocarcinoma (PDAC). Methods: In this study, patients with PDAC who underwent radical resection and DECT from 2018 to 2022 were enrolled and categorised into ER and non-ER groups. The clinical data, basic CT features and DECT parameters of all patients were analyzed. Independent predictors of ER were identified with Logistic regression analyses. Three models (model A: basic CT features; model B: DECT parameters; model C: basic CT features + DECT parameters) were established. Receiver operating characteristic curve analysis was utilized to evaluate predictive performance. Results: A total of 150 patients were enrolled (ER group: n = 63; non-ER group: n = 87). Rim enhancement (odds ratio [OR], 3.32), peripancreatic strands appearance (OR, 2.68), electron density in the pancreatic parenchymal phase (P-Rho; OR, 0.90), arterial enhancement fraction (AEF; OR, 0.05) and pancreatic parenchyma fat fraction in the delayed phase (OR, 1.25) were identified as independent predictors of ER. Model C showed the highest area under the curve of 0.898. In addition, the corresponding ER risk factors were identified separately for resectable and borderline resectable PDAC subgroups. Conclusions: DECT quantitative parameters allow for the noninvasive prediction of postoperative ER in patients with PDAC, and the combination of DECT parameters and basic CT features shows a high prediction efficiency. Our model can help to identify patients with high-risk factors to guide preoperative decision making.

Abstract:Background:: Preoperative T-staging is essential for planning optimal treatment and care for colorectal cancer (CRC). Objective:: To evaluate the accuracy of Dual-energy CT (DECT) in preoperative T-staging of CRC. Methods:: The clinical data and DECT images of 37 patients with 39 CRC lesions were retrospectively analyzed. The performance of the DECT quantitative parameters in CRC T-staging was evaluated. Postoperative pathologic results were used as a gold standard. Receiver operating characteristic curves were used to assess the diagnostic efficacy of DECT parameters. P & 0.05 was deemed significant. Results:: The overall accuracy of T-staging by DECT was 76.9%. The DECT parameters were significantly different between the T3 pericolic fat stranding, T4a pericolic fat stranding, and normal pericolic fat stranding. Arterial phase λHU had the best diagnostic performance with a cut-off value of ≥0.967, resulting in a 70.6% sensitivity and a 100% specificity in differentiating between T3 and T4a stages of CRC. Conclusion:: DECT has high accuracy in the T-staging of CRC. Arterial phase λHU has the best diagnostic performance in differentiating between T3 and T4a stages of CRC.

Abstract:Background: Mucosal healing has become the primary treatment target for patients with Crohn's disease (CD). We aimed to develop a noninvasive and convenient tool to evaluate the endoscopic activity in patients with ileocolic CD. Methods: A retrospective multicenter study including 300 CD patients (training, 210 patients; test, 90 patients) was conducted at two tertiary referral centers. Independent risk factors associated with endoscopic activity were explored, which were then combined into a comprehensive index. The predictive performance was evaluated with the area under receiver operating characteristic curve (ROC). Cohen's Kappa was adopted to examine the consistency between each indicator and endoscopic activity. Results: A total of 210 CD patients were recruited in the training cohort. We found that Crohn's Disease Activity Index (CDAI), C-reactive protein (CRP) and platelet-to-lymphocyte percentage ratio (PLpR) were independently associated with endoscopic activity. Additionally, the comprehensive index generated from the above three indices achieved good discrimination and performed better than CDAI in AUC (0.849 vs. 0.769, P < 0.05). This was further well demonstrated by the external test cohort, which showed good discrimination (AUC: 0.84, 95% CI: 0.744-0.936). Intra-individual comparison revealed the comprehensive index to be superior in the prediction of endoscopic activity. In the subgroup analysis, the AUC of comprehensive index was significantly higher than CDAI especially in inflammatory phenotype (0.824 vs. 0.751, P < 0.05). Conclusion: Combining CDAI, CRP and PLpR significantly improved the accuracy for predicting endoscopic activity in ileocolic CD, which can help better monitor an endoscopic flare.

Abstract:Background: Crohn's disease (CD) is a chronic inflammatory disease of the digestive tract with unknown etiology. It follows a relapse-remission pattern, making disease activity assessment crucial for treatment. Our study aims to evaluate the diagnostic accuracy of various imaging modalities and to validate and compare the International Bowel Ultrasound Segmental Activity Score (IBUS-SAS), the multidetector computed tomography enterography score (MDCTEs), and the simplified endoscopic activity score for Crohn's disease (SES-CD). Methods: We assessed diagnostic performance using the CD Activity Index (CDAI). We first categorized patients into remission and active groups. For those in the active stage, we further categorized them into mild/moderate and severe activity groups. We used Spearman rank correlation to evaluate the relationships among IBUS-SAS, bowel wall thickness (BWT), Color Doppler imaging signal (CDS), inflammatory fat (i-fat), bowel wall stratification (BWS), and clinical inflammatory indicators. Results: A total of 103 CD patients were evaluated. The IBUS-SAS cut-off for remission and activity was 23.8, with an AUC of 0.923, sensitivity of 91.4%, and specificity of 84.8%. The SES-CD had an AUC of 0.801, sensitivity of 62.9%, and specificity of 84.4% at a cut-off of 4.5. The MDCTEs showed an AUC of 0.855, sensitivity of 77.1%, and specificity of 75.8% for a cut-off of 6.5. The Delong test revealed significant differences in diagnostic efficacy when comparing IBUS-SAS to SES-CD and IBUS-SAS to MDCTEs. In the group of mild or moderate-to-severe active, the IBUS-SAS had an AUC of 0.925, sensitivity of 83.7%, and specificity of 88.9% at a cut-off of 40. The SES-CD exhibited an AUC of 0.850, sensitivity of 90.7%, and specificity of 70.4% at a cut-off of 8.5. MDCTEs showed an AUC of 0.909, sensitivity of 83.7%, and specificity of 85.2% at a cut-off of 8.5. During Delong test, the IBUS-SAS, MDCTEs, and SES-CD showed no significant differences in assessing moderate-to-severe activity. Both IBUS-SAS and ultrasound parameters correlated with certain serum indicators (p < 0.05), although only weakly to moderately (all r < 0.5). Conclusion: The IBUS-SAS, MDCTEs and SES-CD can evaluate disease remission/active and mild/moderate-to-severe active in CD, and IBUS-SAS offers the potential to precisely define CD activity.
Computed Tomography Enterography Versus Balloon‐assisted Enteroscopy for Evaluation of Small Bowel Lesions in Crohn's Disease

Abstract:Background and Aim We aimed to assess the correlation between computed tomography enterography (CTE) and balloon-assisted enteroscopy on severity of small bowel lesions, and evaluated the accuracy of CTE parameters in assessing small intestine lesions in patients with Crohn's disease (CD). Methods We performed an observational study of a single-center cohort. Data were retrieved from our inpatient databases starting from October 2007. Correlations between computed tomography parameters (bowel wall thickness, mural enhancement, comb sign, extramural findings, and stricture), endoscopic and histological severity scores, CD Activity Index [CDAI], and C-reactive protein were assessed using Spearman's rank correlation. Results Seventy patients were included in this study. One hundred fifty-seven segments were examined. Bowel wall thickness (r=0.6334, P<0.0001), mural enhancement (r=0.5477, P<0.0001), comb sign (r=0.5898, P<0.0001), and extramural findings (r=0.4754, P<0.0001) were moderately correlated with the segmental Capsule Endoscopy CDAI. The segmental CTE score also moderately correlated with the segmental Capsule Endoscopy CDAI (r=0.6714, P<0.0001), while the total CTE score strongly correlated with the total Capsule Endoscopy CDAI (r=0.7252, P<0.0001). Both total CTE score (r=0.5937, P<0.0001) and total Capsule Endoscopy CDAI (r=0.6364, P<0.0001) correlated significantly with the Harvey-Bradshaw Index. Of five computed tomography parameters, bowel wall thickness have the best accuracy to detect small intestine lesions with an area under the receiver operating characteristic curve of 0.811 (P<0.0001), with a sensitivity and specificity of 81.82% and 74.14%, respectively. Conclusion CTE is a reliable technique for detecting small intestine lesions in patients with CD, also provides accurate information on small bowel CD severity and activity, with close agreement to inflammatory markers, CDAI, and histopathology.
Differentiation of Crohn's disease, ulcerative colitis, and intestinal tuberculosis by dual-layer spectral detector CT enterography

Abstract:Aim: To investigate the value of radiological features and energy spectrum quantitative parameters in the differential diagnosis of Crohn's disease (CD), ulcerative colitis (UC), and intestinal tuberculosis (ITB) by dual-layer spectral detector computed tomography (CT) enterography (CTE). Materials and methods: Clinical and CTE data were collected from 182 patients with CD, 29 with UC, and 51 with ITB. CT images were obtained at the enteric phases and portal phases. The quantitative energy spectrum parameters were iodine density (ID), normalised ID (NID), virtual non-contrast (VNC) value, and effective atomic number (Z-eff). The area under curve (AUC) of the receiver operating characteristic curve (ROC) was calculated. Results: The vascular comb sign (p=0.009) and enlarged lymph nodes (p=0.001) were more common in patients with CD than UC or ITB. In the differentiation of moderate-severe active CD from UC, enteric phase NID (AUC, 0.938; p<0.001) and portal phase Z-eff (AUC, 0.925; p<0.001) had the highest accuracy, which were compared separately. In the differentiation of moderate-severe active CD from ITB, enteric phase NID (AUC, 0.906; p<0.001) and portal phase Z-eff (AUC, 0.947; p<0.001) had the highest accuracy; however, the AUC value was highest when the four parameters are combined (AUC, 0.989; p<0.001; AUC, 0.986; p<0.001; AUC, 0.936; p<0.001; and AUC, 0.986; p<0.001). Conclusion: The present study shows that the combined strategies of four parameters have higher sensitivity and specificity in differentiating CD, UC, and ITB, and may play a key role in guiding treatment.
