Aseptic encephalitis and blindness complicating OKT3 therapy

Abstract:We report 2 cases of aseptic encephalitis associated with OKT3 therapy occurring within 1 week of each other. One case was further complicated by the acute onset of bilateral optic nerve blindness. While aseptic meningitis and seizures are both described complications of OKT3 therapy, neither aseptic encephalitis nor isolated cranial nerve involvement has been previously reported. In a recent article (Clin Transplantation 1991: 5: 60‐62) reference was made to “cortical” blindness that followed OKT3 treatment for steroid‐resistant rejection, but no details were forthcoming. Review of the Orthoclone® OKT3 Postmarketing Adverse Experience database revealed 2 reports of temporary sixth cranial nerve palsies and 2 cases associated with visual problems. Encephalitis with or without cranial nerve involvement following OKT3 therapy may result in significant morbidity for a small subset of patients. Although not common, these experiences emphasize the importance of continued close follow‐up of all patients who receive OKT3.
