Abstract:Intramural pregnancy, an extremely rare form of ectopic pregnancy, refers to a pregnancy located within the uterine wall, completely surrounded by myometrium and separate from the uterine cavity and fallopian tubes. It may lead to an undesirable hysterectomy because of uterine rupture and life-threatening hemorrhage. Thus, early diagnosis and treatment of intramural pregnancy can avoid serious complications. Here, we report the case of a woman with intramural pregnancy diagnosed early by transvaginal 2dimensional (2D) and 3-dimensional (3D) sonography. To our knowledge, it was the first case treated with gestational sac excision under hysteroscopy combined with the systemic use of methotrexate (MTX). A 20-year-old woman, gravida 2, para 0010, was admitted to our hospital with intermittent vaginal bleeding for 10 days. She underwent dilation and curettage 1 year previously. Her last menstrual period was 50 days previously. Her serum β-human chorionic gonadotropin (β-hCG) level was 36,268 IU/L. Two-dimensional sonography revealed an empty endometrial cavity and a gestational sac measuring 1.8 cm in the left posterior myo metrium (Figure 1A), with a sinus connecting the sac with the endometrial cavity (Figure 1B). Color Doppler imaging showed abundant vascularity, with a resistive index of 0.55 (Figure 1C). It was not certain whether the gestational sac was situated at the interstitial part of the fallopian tube. Three-dimensional sonography was performed, and it showed that the gestational sac was located in the left posterior side of the myometrium, about 1.4 cm above the internal os, clearly outside the endometrium. It also showed the sinus clearly (Figure 1D). Combining the symptoms, β-hCG level, and 3D images, the diagnosis was considered intramural pregnancy. Because it was thought that a sinus was connecting the sac with the endometrial cavity, hysteroscopy was performed, and it showed normal bilateral tubal ostia without dilatation, no septum in the uterine cavity, and a dimple in the left internal uterine wall, just as on the 3D images. The uterine endometrium and myometrium were cut with a ring electrode until the villi were exposed, and then dilation and curettage was performed; the tissue obtained by curettage measured 1 × 1 × 0.5 cm. Cytoxin at a dose of 20 IU was given intravenously, and gauze was inserted into the fornix of the vagina after gestational sac had been excised. Methotrexate at a dose of 50 mg was used systemically 1 day after surgery. Another sonographic examination on the seventh postoperative day showed the intrauterine gestational sac to be absent but still showed abundant vascularity in the myometrium. The serum βhCG level was 5106 IU/L. Another 50 mg of MTX was administered systemically. The β-hCG level declined to normal 22 days later. Histologic examination confirmed the villi and decidual tissue. Intramural pregnancy is extremely rare, constituting less than 1% of the total number of ectopic pregnancies. Its clinical characteristics have been described mainly in case reports. Intramural pregnancy may result in serous morbidity secondary to hemorrhage from rupture. Thus, early diagnosis is necessary. Intramural pregnancy usually presents as a missed period, vaginal bleeding, abdominal pain, and a positive pregnancy test result, but all of these manifestations are also typical of ectopic pregnancy and natural abortion. Early diagnosis of intramural pregnancy has become possible with increased understanding of the disease and progress in sonographic techniques. Now, sonography is the most common tool for diagnosing the disease. The 2D sonographic findings, including a gestational sac inside and surrounded completely by the myometrium and no gestational sac in the endometrial cavity, are regarded as the typical sonographic appearance of this condition.1 Color Doppler imaging shows increased peritrophoblastic blood flow.2 In our case, transvaginal 2D sonography showed a gestational sac in the myometrium, and color Doppler imaging showed abundant flow around the sac; thus, the diagnosis of intramural pregnancy was suspected. Previous reports also showed that almost all intramural pregnancies could be correctly diagnosed before surgery as long as sonography showed a gestational sac in the myometrium.2–6 However, 2D sonography cannot always clearly distinguish intramural pregnancy from interstitial or corneal ectopic pregnancy. Instead, with 3D sonography, more accurate localization of the gestational sac can be obtained at a distance from the uterine cornu or interstitial tube and from the endometrial cavity. The availability of multiple scan planes on 3D sonography can help define the relationship of the gestational sac with the endometrial cavity.3,6 In our case, two successive 2D sonographic examinations showed two different sites of the uterus, and on one of the examinations, we even suspected that the sac was situated at the interstitial part of the fallopian tube. On 3D sonography, however, the sac site was exactly located in the left posterior side of the myometrium, about 1.4 cm above the internal os, far from the interstitial part of the fallopian tube. Thus, the diagnosis of intramural pregnancy was basically confirmed by combining the sonographic findings with the clinical manifestations and elevated hCG level.
