Abstract:Climate change is no longer a problem for future generations and the impact is already taking a toll in many parts of the world. Climate change has already caused substantial, and increasingly irreversible, damage to ecosystems. All these issues combined will inevitably lead to an increase in human suffering and forced displacement. This has significant ramifications for health care systems. In this editorial we outline how climate change is already impacting both physical and mental health. Health professionals have a role to play in addressing this great challenge of our time. Health professionals should reflect on how to promote means of climate change mitigation and adaptation within their spheres of influence - clinical, education, advocacy, administration, and research.

Abstract:There is scientific consensus that the global climate is changing, with rising surface temperatures, melting ice and snow, rising sea levels, and increasing climate variability. These changes are expected to have substantial impacts on human health. There are known, effective public health responses for many of these impacts, but the scope, timeline, and complexity of climate change are unprecedented. We propose a public health approach to climate change, based on the essential public health services, that extends to both clinical and population health services and emphasizes the coordination of government agencies (federal, state, and local), academia, the private sector, and nongovernmental organizations.

Abstract:Urgent warnings about the existential threat of climate change are coming from leaders in nearly every sector of society, including virtually all climate scientists, notable heads of civil governments around the globe, the world's top religious leaders, prestigious medical journals, as well as principals of the largest financial firms. Surveys show that the majority of U.S. physicians in several specialties are caring for patients who are experiencing direct health harms due to climate change. In public platforms, physicians are expressing their awareness that this public health crisis places everyone at risk, but many people are at greater risk, including children, pregnant women, people with chronic health conditions, elders, and those who experience environmental injustice or live in harm's way. Physicians should respond to this crisis with meaningful activities performed within the context of their current roles. The role of medical care provider is the best known role. But, throughout their careers, physicians have ongoing responsibilities as educators of colleagues, trainees, and patients. They are influential employees of medical institutions, trusted experts who exercise civic responsibility, and sources of guidance for public policymakers. Physicians and other health professionals, individually and through their organizations, also work to influence our societal response to the challenge of climate change. The first annual Lancet Journal Countdown Report in 2016 tracking health indicators of climate change stated that climate change had the potential to wipe out all public health gains of the last half century, but it also presents a tremendous opportunity to save lives and improve health. All physicians should work toward the latter outcome.

Abstract:Health professionals from every corner of the health sector—from allergy to vascular surgery, from epidemiology to environmental health, from nursing to hospital administration—have recognized the magnitude and urgency of the climate crisis. A growing literature provides guidance on how to conceptualize and meet the vast challenges we face and on how to keep our spirits up as we do so. Sorensen et al 1 propose roles and responsibilities for health professionals in the context of the climate crisis. They outline a far-reaching job description—from primary prevention to disaster preparedness and response to attribution science, from clinical care to patient education to policy advocacy. They point out that neither our current professional training nor the configuration of our health sector is fit for purpose: the climate crisis calls for substantial enhancement of health curricula and for systemic change across the health sector and beyond. Several aspects of the article by Sorensen et al 1 are noteworthy. First, they apply the traditional prevention hierarchy to the climate crisis. In this model, primary prevention consists of efforts to prevent the onset of disease, such as through immunization or smoking cessation; secondary prevention consists of early detection of disease such as through Papanicolaou tests, followed by interventions to improve outcomes; and tertiary prevention involves treatment or rehabilitation of people already affected, such as diabetes management or stroke rehabilitation. 2 However, these categories are grounded in clinical thinking; they do not always lead us upstream, to consider what have been called the causes of the causes . 3 Climate change, as the ultimate system problem, demands upstream thinking, far upstream of the health sector. Accordingly, as Sorensen et al 1 point out, primary prevention in this context means tackling the root causes of climate change: reducing carbon emissions, reversing destructive land use changes, and promoting carbon drawdown. In the parlance of climate action, this is mitigation (ie, avoiding the unmanageable). Since climate change is already amplifying such outcomes as malaria and malnutrition, measures that would conventionally be considered primary prevention (eg, mosquito eradication programs and nutritional supplements) are best viewed as secondary prevention, or in the parlance of climate action, adaptation (ie, managing the unavoidable). Finally, tertiary prevention in the climate context consists of efforts to reduce suffering among those already affected by climate change, including care for climate refugees displaced from their homes, those affected by weather disasters, and those with climate anxiety. These tertiary prevention actions correspond to disaster response. This reframing of prevention categories with the shift from clinical care to climate action is not just an academic exercise. It has real implications. Few physicians are transportation planners, few nurses are agronomists, and few public health professionals are electrical engineers, but the pathways to preventing climate-related suffering run directly through low-carbon transport, regenerative agriculture, and renewable energy. As Sorensen et al 1 point out, a transdisciplinary approach is necessary, because climate change is so far reaching in its causes and effects. Health professionals need not become experts in these domains, but they need to understand them well enough to help integrate health into the work of other sectors, to endorse them as health strategies, and to advocate for them. A second key point Sorensen et al 1 make is the need for coordination between clinical care and public health. The disconnect between these domains has long been recognized and was highlighted during the COVID-19 pandemic. 4 For example, clinical insights and clinical data need to flow into public health surveillance systems, which need to transform the data into actionable information, which needs to flow back to clinicians (and to the public and policymakers). However, the integration of clinical care and public health is only a beginning. We also need collaboration between the health sector and other sectors, including meteorology, which provides invaluable climate models and weather forecasts; emergency management, which responds to climate disasters; parks and recreation, which manages nature-based solutions in cities; and agriculture, which produces food in the context of changing climate constraints. Should required premedicine courses include not only biology and chemistry, but also earth sciences and ecology? Should medical students rotate not only through pediatrics and surgery wards, but also through transportation and emergency management agencies? Abstract:The health care system contributes substantially to global greenhouse gas emissions, a driver of climate change. At the same time, climate change has caused disruptions in health care delivery. In this article, the authors describe both how the health care industry contributes to climate change and how climate change affects patient care. The authors also provide clinical recommendations for health care practitioners to counsel patients on health effects of climate change and underscore the need for developing the workforce needed to respond to unique health care delivery challenges resulting from climate-related factors.

Abstract:The 2015 Lancet Commission on Health and Climate Change has been formed to map out the impacts of climate change, and the necessary policy responses, in order to ensure the highest attainable standards of health for populations worldwide. This Commission is multidisciplinary and international in nature, with strong collaboration between academic centres in Europe and China.The central finding from the Commission's work is that tackling climate change could be the greatest global health opportunity of the 21st century. The key messages from the Commission are summarised below, accompanied by ten underlying recommendations to accelerate action in the next 5 years.

Abstract:Background The climate and ecological emergency is the single biggest health threat facing humanity, yet it is not clear to what extent the public health workforce have been involved in work on this topic. This research aimed to establish what public health consultants working in local authorities in England perceive their role to be, whether the climate crisis is seen as a core component of public health and to identify barriers to action. Methods Semi-structured interviews were undertaken with a purposive sample (n=11) of local authority public health consultants in England. Participants were recruited via public health organisations, social media or snowballing. Thematic content analysis was used to identify codes and themes. Results Public health professionals have started work on climate change but reported being unclear about their role and feeling isolated working on this topic. Barriers to action included shortage of financial resources, communication tools and capacity, limited sharing of best practice, lack of local expertise and conflict between the need for action on climate change and other urgent issues. Conclusion We highlight the need to urgently address existing barriers to enable this important part of the public health workforce to play their role in tackling the climate and ecological crisis.

Abstract:Background As global CO2 emissions continue to rise and the 'era of global boiling' takes hold, the health workforce must cope with the challenge of providing care to increasing numbers of patients affected by climate change‐related events (e.g., hurricanes, wildfires, floods). In this review, we describe the impacts of these events on the health workforce, and strategies responding to these challenges. Methods This rapid systematic review was guided by the Preferred Reporting Items for Systematic reviews and Meta‐Analyses and a registered protocol (PROSPERO CRD42023433610). Eight databases were searched in May 2022 and again in June 2023. Empirical studies discussing climate change and workforce policy, planning, preparedness, and capacity were included. Inductive thematic analysis of extracted data was conducted. Results From the 60 included studies, two categories emerged: the impacts of climate events on the health workforce (n = 39), and workforce responses to and preparations for climate events (n = 58). Thirty‐seven studies reported on both categories. Four impact themes were identified: absenteeism, psychological impacts, system breakdown, and unsafe working conditions; and six responses and preparations themes: training/skill development, workforce capacity planning, interdisciplinary collaboration, role flexibility, role incentivisation, and psychological support. Conclusion This review provides an overview of some of the deleterious impacts of climate events on the health workforce, as well as potential strategies for the health workforce to prepare or respond to climate events. Future studies should assess the implementation and effectiveness of these strategies to ensure a continuously improving healthcare system, and a well‐supported health workforce.

Abstract:Climate change poses significant public health and health system challenges including increased demand for health services due to chronic and acute health impacts from vector-borne diseases, heat-related illness, and injury from severe weather. As climate change worsens, so do its effects on health systems such as increasing severity of weather extremes causing damage to healthcare infrastructure and interference with supply chains. Ironically, health sectors globally are significant contributors to climate change, generating an estimated 5% of global emissions. Achieving "net zero" health systems require large-scale change with shared decision-making to coordinate a pan-Canadian approach to creating climate-resilient and low-carbon healthcare. In this article, we discuss healthcare professionals' and health leaders' perceptions of responsibility for practicing and advocating for climate-resilient and low-carbon healthcare in Canada.

Abstract:Climate change is an ongoing and escalating health emergency. It threatens the health and wellbeing of billions of people, through extreme weather events, displacement, food insecurity, pathogenic diseases, societal destabilisation, and armed conflict. Climate change dwarfs all other challenges studied by health psychologists. The greenhouse gas emissions driving climate change disproportionately originate from the actions of wealthy populations in the Global North and are tied to excessive energy use and overconsumption driven by the pursuit of economic growth. Addressing this crisis requires significant societal transformations and individual behaviour change. Most of these changes will benefit not only the stability of the climate but will yield significant public health co-benefits. Because of their unique expertise and skills, health psychologists are urgently needed in crafting climate change mitigation responses. We propose specific ways in which health psychologists at all career stages can contribute, within the spheres of research, teaching, and policy making, and within organisations and as private citizens. As health psychologists, we cannot sit back and leave climate change to climate scientists. Climate change is a health emergency that results from human behaviour; hence it is in our power and responsibility to address it.

Abstract:Policy Points The US health care system faces mounting pressure to reduce greenhouse gas emissions and adapt to the impacts of climate change; motivated institutions and an engaged health care workforce are essential to the development, implementation, and maintenance of a climate‐ready US health care system. Health care workers have numerous profession‐specific and role‐specific opportunities to address the causes and impacts of climate change. Policies must address institutional barriers to change and create incentives aligned with climate readiness goals. Institutions and individuals can support climate readiness by integrating content on the health care implications of climate change into educational curricula.

Abstract:More than 230 medical journals have called for worldwide action against global warming and for nature restoration. They call on health professionals to actively contribute to increasing support for the radical measures necessary to maintain the health and well-being of life on our planet. This call concerns not only making one's own living and working environment greener, but above all the use of medical knowledge, expertise and authority for measures that matter worldwide. This starts with concrete advice to patients to prevent damage from climate change at an individual level. Health workers must stand up for prevention and care outside the consultation room and across national borders. After all, medical doctors have historically been in a position to speak out about these threats and to remind political leaders of their responsibility.Health professionals can lead by example by realizing greener and more sustainable care themselves.

Examining the role of community health workers amid extreme weather events in low- and middle-income countries: a scoping review

Abstract:Objectives: The increased frequency and severity of extreme weather events (EWEs) have underscored the need to strengthen climate-resilient health systems and capacity. Community health workers (CHWs) are integral health systems actors with the potential to protect and improve population health in a changing climate. The aim of this review was to synthesize the literature on the roles of CHWs amid EWEs in low- and middle-income countries, the barriers and facilitators to implement these roles, and program supports to strengthen CHW capacity and health system functions. Study design: Scoping review. Methods: Four academic databases and gray literature published between January 2000 and June 2023 were searched. Data were thematically analyzed using a deductive-inductive approach guided by the World Health Organization's (WHO's) Operational framework for building climate-resilient health systems. Results: Thirty sources were included. Amid EWEs, CHW roles included: 1) delivery of diagnostic, treatment, and other clinical services; 2) support with access, utilization, or navigation of health services and/or referrals; 3) community education and health promotion; 4) data collection and health surveillance; 5) psychosocial supports; and 6) weather-related health emergency response. Facilitators and barriers to the provision of CHW supports amid EWEs were categorized within WHO's building blocks of health systems. Considerations for strengthening CHW programs to enhance climate-resilient health systems are also discussed. Conclusions: CHWs are uniquely positioned to provide health-related supports amid EWEs that extend to emergency preparedness and response to climate-health challenges. These efforts can contribute to the community and health systems resilience to climate change.

Abstract:Climate change has led to a multitude of ecological disruptions and downstream reproductive health consequences that impair our reproductive capacity and, in turn, harm the health and survival of future generations. Atmospheric changes, driven by anthropogenic emissions, expose global populations to droughts, heat waves, rising sea levels, and extreme weather events-posing major threats to public health and exacerbating environmental health disparities. Existing evidence demonstrates the potential for climate-driven events to impact reproductive health outcomes, yet very few studies have explored this relationship. Recently, the American Society for Reproductive Medicine, the American College of Obstetricians and Gynecologists, and the International Federation of Gynecology and Obstetrics released position statements regarding reproductive health and environmental exposures. Unfortunately, such initiatives have yielded little action within the health care system. To address this stagnation, health care workers must meld research findings into actionable preventive medicine strategies and transition to a more action-oriented approach to address the climate crisis. The objective of this article is to elucidate the urgency of the climate crisis in relation to reproductive health and push the health care workers to recognize their intrinsic opportunity as leaders in climate action at local, state, national, and international levels. We call on health care organizations and health care workers to leverage their inherent positions as climate action leaders to increase climate resilience and mitigate climate-related adverse reproductive health outcomes.

Abstract:During the past several decades, unprecedented global changes in climate have given rise to an increase in extreme weather and other climate events and their consequences such as heavy rainfall, hurricanes, flooding, heat waves, wildfires, and air pollution. These climate effects have direct impacts on human health such as premature death, injuries, exacerbation of health conditions, disruption of mental well-being, as well as indirect impacts through food- and water-related infections and illnesses. While all populations are at risk for these adverse health outcomes, some populations are at greater risk because of multiple vulnerabilities resulting from increased exposure to risk-prone areas, increased sensitivity due to underlying health conditions, and limited adaptive capacity primarily because of a lack of economic resources to respond adequately. We discuss current governmental public health responses and their future opportunities to improve resilience of special populations at greatest risk for adverse health outcomes. Vulnerability assessment, adaptation plans, public health emergency response, and public health agency accreditation are all current governmental public health actions. Governmental public health opportunities include integration of these current responses with health equity initiatives and programs in communities.

Abstract:Abstract The climate crisis calls for urgent action from every level of the US healthcare sector, starting with an acknowledgment of our own outsized contribution to greenhouse gas emissions (at least 8.5% of carbon emissions). As the climate continues to become warmer and wetter, the medical establishment must deal with increasing rates of pulmonary and cardiovascular diseases, heat-related illness, and emerging infectious diseases among many other health harms. Additionally, extreme weather events are causing healthcare delivery breakdown due to physical infrastructure damage, slowed supply chains, and workforce burden. Pathways for healthcare systems to meet these challenges are emerging. They entail significant measures to mitigate our carbon footprint, embrace shared and equity-driven governance, develop new metrics of accountability, and build more resilience into our care delivery processes. We call upon SHEA to play a unique leadership role in the fight for sustainable, equitable, and efficient health care in a rapidly changing climate that immediately threatens human well-being.

Abstract:Climate change may be the greatest health threat of the twenty-first century, impacting lives both directly and indirectly, through undermining the environmental and social determinants of health. Rapid action to decarbonize economies and build resilience is justified on health, human rights, environmental and economic grounds. While the necessary health response is wide ranging, it can largely be encapsulated within three grand challenges: (i) promote actions that both reduce carbon emissions and improve health; (ii) build better, more climate-resilient and low-carbon health systems; and (iii) implement public health measures to protect from the range of climate risks to health. The health community can make a unique and powerful contribution, applying its trusted voice to climate leadership and advocacy, providing evidence for action, taking responsibility for climate resilience and decarbonization of healthcare systems, and guiding other sectors whose actions impact substantially on health, carbon emissions and climate resilience.
