Abstract:Nepal, with its diverse and geographically challenging landscape, faces a significant burden of respiratory diseases like tuberculosis, chronic obstructive pulmonary disease (COPD), and pneumonia. While research plays a crucial role in understanding and addressing these diseases, conducting ethical research in this context presents unique challenges. This paper examines the ethical concerns surrounding respiratory disease research in Nepal, exploring vulnerabilities, potential solutions, and recommendations for promoting ethical practice. The Nepal Health Research Council is an apex body to regulate health research in Nepal. Every year, around 1000 proposals are reviewed by the Ethical Review Board of the Nepal Health Research Council, of which 100 are related to respiratory disease. Strengthening ethical review capacities, promoting transparency, and ensuring community representation in review processes are crucial to ensuring ethical conduct and participant well-being. In the other part, there is an urgent requirement to build the research capacity of researchers, healthcare professionals, and communities on national ethical guidelines, regulations, codes of conduct, ethical principles, culturally appropriate research methods, and informed consent procedures. Conducting ethical respiratory disease research in Nepal requires a shift towards cultural sensitive and context-specific approaches. Researchers must prioritize participant well-being, engage with communities and embrace culturally appropriate frameworks for informed consent and during and after research benefit sharing. Such efforts can- contribute to the continuous improvement of health outcomes and address the unique needs of respiratory disease research.
