Abstract:The search for molecules that may contribute to better identify patients at risk for cardiovascular diseases (CVD) represents today an active field in clinical research. Few biomarkers have already been identified as reliable and useful tools in medical decision making, such as cardiac troponin (cTn) and NT-proBNP. At the same time, evidence regarding the possible role of other molecules is piling up. Every new putative biomarker has demonstrated effectiveness in at least one clinical application: cardiovascular risk assessment, diagnosis or outcome prediction. On the other hand, combination of preventive, diagnostic and prognostic implications for the same molecule is expected to improve enormously the usefulness of a biomarker in medicine. We performed a search of the literature looking for circulating molecules found to exert discriminating abilities in all three mentioned clinical applications. The purpose of the present review is to bring to the attention of medical and research communities those biomarkers for which a relevant amount of evidence has been accumulated regarding their potential application in all clinical steps of the cardiovascular continuum. Furthermore, since simultaneous testing of different plasmatic molecules has been proposed as a suitable tool to improve medical decision making, we also discuss feasible associations of biomarkers that promise to be the most effective for cardiovascular risk assessment in the general population and for outcome prediction in patients affected by acute coronary syndrome (ACS) and by heart failure (HF).

Abstract:The use of a large number of cardiovascular biomarkers, meant to complement the use of the electrocardiogram, echocardiography cardiac imaging, and clinical symptom assessment, has become a routine in clinical diagnosis, differential diagnosis, risk stratification, and prognosis and guides the management of patients with suspected cardiovascular diseases. There is a broad consensus that cardiac troponin and natriuretic peptides are the preferred biomarkers in clinical practice for the diagnosis of the acute coronary syndrome and heart failure, respectively, while the roles and possible clinical applications of several other potential biomarkers are still under study. This review mainly focuses on the recent studies of the roles and clinical applications of troponin and natriuretic peptides, which seem to be the best-validated markers in distinguishing and predicting the future cardiac events of patients with suspected cardiovascular diseases. Additionally, the review briefly discusses some of the large number of potential markers that may play a more prominent role in the future.
Imaging biomarkers in cardiac CT: moving beyond simple coronary anatomical assessment

Abstract:Cardiac computed tomography angiography (CCTA) is considered the standard non-invasive tool to rule-out obstructive coronary artery disease (CAD). Moreover, several imaging biomarkers have been developed on cardiac-CT imaging to assess global CAD severity and atherosclerotic burden, including coronary calcium scoring, the segment involvement score, segment stenosis score and the Leaman-score. Myocardial perfusion imaging enables the diagnosis of myocardial ischemia and microvascular damage, and the CT-based fractional flow reserve quantification allows to evaluate non-invasively hemodynamic impact of the coronary stenosis. The texture and density of the epicardial and perivascular adipose tissue, the hypodense plaque burden, the radiomic phenotyping of coronary plaques or the fat radiomic profile are novel CT imaging features emerging as biomarkers of inflammation and plaque instability, which may implement the risk stratification strategies. The ability to perform myocardial tissue characterization by extracellular volume fraction and radiomic features appears promising in predicting arrhythmogenic risk and cardiovascular events. New imaging biomarkers are expanding the potential of cardiac CT for phenotyping the individual profile of CAD involvement and opening new frontiers for the practice of more personalized medicine.

Abstract:Cardiovascular diseases (CVDs) are recognized as a significant threat to global health. The rapid, sensitive, and precise measurement of relevant biomarkers is essential for the timely diagnosis of CVDs. Molecularly imprinted polymers (MIPs), which act as artificial receptor recognition materials, have been extensively utilized in the detection of CVD biomarkers. Their widespread application is due to their cost-effectiveness, physical and chemical stability, straightforward preparation processes, and excellent compatibility with various sensor types. This review introduces the principles of MIP sensors in combination with electrochemical, optical, thermal transfer, and acoustic detection techniques for detecting CVD-related biomarkers. It then discusses methods developed over the past decade for detecting biomarkers of three major CVDs— coronary artery disease (CAD), acute myocardial infarction (AMI), and heart failure (HF)—using MIP sensors. Finally, the review summarizes the potential of MIP sensors in CVD biomarker detection and provides an outlook on future research directions.

Precision Medicine in Cardiology: An Evolving Understanding of Biomarkers in Coronary Artery Disease Prevention a 10-year Thematic Review

Abstract:Abstract Background: Coronary artery disease (CAD) is a leading cause of morbidity and mortality worldwide. Precision medicine, utilizing biomarkers for early detection and prevention of CAD, has emerged as a promising approach to improve patient outcomes. This systematic review aims to summarize the current state of knowledge regarding biomarkers in CAD prevention, focusing on the most promising and well-studied biomarkers over the past decade. Methods: Following PRISMA guidelines, a systematic review was conducted. PubMed, Embase, and Cochrane Library databases were searched for relevant studies published between 2013 and 2023. Inclusion criteria were original research articles, systematic reviews, or meta-analyses focusing on biomarkers in CAD prevention, with sufficient data on biomarker performance. Exclusion criteria were case reports, editorials, conference abstracts, and studies on biomarkers in established CAD. The STARD Standards for Reporting Diagnostic accuracy studies 2015 guideline criteria for assessing diagnostic tools was also utilized to ensure the precision of the methodology and help readers to appraise the applicability of the study findings and the validity of conclusions and recommendations. The main outcome assessed was the association between biomarkers and CAD risk, using various statistical methods. Findings: The search identified 2,345 articles, of which 40 met the inclusion criteria, including 32 original research articles, 6 systematic reviews, and 2 meta-analyses. The biomarkers studied included traditional risk factors (lipid profiles and blood pressure), novel biomarkers (high-sensitivity C-reactive protein, homocysteine, and adipokines), and imaging biomarkers (coronary artery calcium scores and carotid intima-media thickness). Several studies demonstrated the association between these biomarkers and increased CAD risk, independent of traditional risk factors. Multi-marker approaches combining various biomarkers showed improved accuracy in CAD risk assessment compared to traditional risk factors alone. The risk of bias and variability between studies were assessed and reported. Interpretation: This systematic review provides a comprehensive overview of the current landscape of biomarkers in CAD prevention. While traditional risk factors remain important, novel and imaging biomarkers have shown promise in improving risk stratification and guiding personalized prevention strategies. However, challenges remain in translating biomarker research into clinical practice, including the need for standardized guidelines, cost-effectiveness analyses, and further research on multi-marker approaches and personalized prevention strategies. Addressing these challenges and developing evidence-based guidelines for biomarker use in CAD prevention can improve risk assessment accuracy, tailor prevention strategies, and ultimately reduce the global burden of CAD. Abstract:Here we review the highlights of cardiovascular basic science in published in 2021 and early 2022 on behalf of the European Society of Cardiology Council for Basic Cardiovascular Science. We begin with non-coding RNAs which have emerged as central regulators cardiovascular biology, and then discuss how technological developments in single-cell 'omics are providing new insights in cardiovascular development, inflammation and disease. We also review recent discoveries on the biology of extracellular vesicles in driving either protective or pathogenic responses. The Nobel Prize in Physiology or Medicine 2021 recognised the importance of the molecular basis of mechanosensing and here we review breakthroughs in cardiovascular sensing of mechanical force. We also summarise discoveries in the field of atherosclerosis including the role of clonal haematopoiesis of indeterminate potential, and new mechanisms of cross-talk between hyperglycemia, lipid mediators and inflammation. The past 12 months also witnessed major advances in the field of cardiac arrhythmia including new mechanisms of fibrillation. We also focus on inducible pluripotent stem cell (iPSC) technology which has demonstrated disease causality for several genetic polymorphisms in long QT syndrome and aortic valve disease, paving the way for personalized medicine approaches. Finally, the cardiovascular community has continued to better understand COVID-19 with significant advancement in our knowledge of cardiovascular tropism, molecular markers, the mechanism of vaccine-induced thrombotic complications and new anti-viral therapies that protect the cardiovascular system.

Abstract:Myocardial remodelling, entailing cellular and molecular changes in the different components of the cardiac tissue in response to damage, underlies the morphological and structural changes leading to cardiac remodelling, which in turn contributes to cardiac dysfunction and disease progression. Since cardiac tissue is not available for histomolecular diagnosis, surrogate markers are needed for evaluating myocardial remodelling as part of the clinical management of patients with cardiac disease. In this setting, circulating biomarkers, a component of the liquid biopsy, provide a promising approach for the fast, affordable and scalable screening of large numbers of patients, allowing the detection of different pathological features related to myocardial remodelling, aiding in risk stratification and therapy monitoring. However, despite the advances in the field and the identification of numerous potential candidates, their implementation in clinical practice beyond natriuretic peptides and troponins is mostly lacking. In this review, we will discuss some biomarkers related to alterations in the main cardiac tissue compartments (cardiomyocytes, extracellular matrix, endothelium and immune cells) which have shown potential for the assessment of cardiovascular risk, cardiac remodelling and therapy effects. The hurdles and challenges for their translation into clinical practice will also be addressed.

Abstract:The atherosclerotic vascular disease is a cardiovascular continuum in which the main role is attributed to atherosclerosis, from its appearance to its associated complications. The increasing prevalence of cardiovascular risk factors, population ageing, and burden on both the economy and the healthcare system have led to the development of new diagnostic and therapeutic strategies in the field. The better understanding or discovery of new pathophysiological mechanisms and molecules modulating various signaling pathways involved in atherosclerosis have led to the development of potential new biomarkers, with key role in early, subclinical diagnosis. The evolution of technological processes in medicine has shifted the attention of researchers from the profiling of classical risk factors to the identification of new biomarkers such as midregional pro-adrenomedullin, midkine, stromelysin-2, pentraxin 3, inflammasomes, or endothelial cell-derived extracellular vesicles. These molecules are seen as future therapeutic targets associated with decreased morbidity and mortality through early diagnosis of atherosclerotic lesions and future research directions.

Abstract:This extensive study explores the field of novel biomarkers for the early detection of cardiovascular disease (CVD), looking at a variety of indicators, including circulating endothelial cells, growth differentiation factor-15 (GDF-15), high-sensitivity C-reactive protein (hs-CRP), microRNAs, and myeloperoxidase (MPO). The review emphasizes how important these indicators are to improving CVD risk assessment, prognosis, and treatment plans. While each biomarker offers distinct insights into particular facets of cardiovascular health, multi-biomarker panel integration is suggested for a more thorough knowledge. The paper also discusses the difficulties with standardization, moral issues, and legal restrictions related to using these biomarkers in therapeutic settings. In addition, the revolutionary potential of these new biomarkers in conjunction with state-of-the-art technologies is emphasized, providing a promising path towards precision cardiovascular care and customized medicine

Abstract:While the received traditional predictors are still the mainstay in the diagnosis and prognosis of CVD events, increasing studies have focused on exploring the ancillary effect of biomarkers for the aspiring of precision. Under which circumstances, soluble ST2 (sST2), lipoprotein-associated phospholipase A2 (Lp-PLA2), myeloperoxidase (MPO), and procalcitonin (PCT) have recently emerged as promising markers in the field of both acute and chronic cardiovascular diseases. Existent clinical studies have demonstrated the significant associations between these markers with various CVD outcomes, which further verified the potentiality of markers in helping risk stratification and diagnostic and therapeutic work-up of patients. The current review article is aimed at illuminating the applicability of these four novels and often neglected cardiac biomarkers in common clinical scenarios, including acute myocardial infarction, acute heart failure, and chronic heart failure, especially in the emergency department. By thorough classification, combination, and discussion of biomarkers with clinical and instrumental evaluation, we hope the current study can provide insights into biomarkers and draw more attention to their importance.

Understanding cardiac cell biology, its intricate microenvironments, and the mechanisms that govern their function in health and disease are crucial to designing novel therapeutical and behavioral interventions. Recent advances in single-cell and spatial omics technologies have significantly propelled this understanding, offering novel insights into the cellular diversity and function and the complex interactions of cardiac tissue. This review provides a comprehensive overview of the cellular landscape of the heart, bridging the gap between suspension-based and emerging in situ approaches, focusing on the experimental and computational challenges, comparative analyses of mouse and human cardiac systems, and the rising contextualization of cardiac cells within their niches.
