Fluctuation in body size and glymphatic system derangement in obesity

Abstract:Reduction of obesity is recommended as a specific action throughout the life course to prevent/limit cognitive decline [1]. Assessment of the association between long-term variation in obesity measures and brain health was very recently attempted by Sun et al. [2] in a multicenter, community-based cohort study in China. The authors demonstrated that rising levels of obesity measures are associated with magnetic resonance imaging findings of brain macro/micro structural alterations and features of small vessels disease [2]. Conversely, a decrease in obesity measures was associated with neuroimaging metrics of improved brain health [2]. These results support reduction of obesity as an effective action against dementia. In our opinion, the study by Sun et al. [2] presents some limitations that affect their conclusions, with the risk of undermining the substantiation of the above-mentioned recommendation [1]. Indeed, assessment of body size was a time-dependent continuous variable, while neuroimaging was performed once, at an undefined time point during the patient observation period.
