Abstract:Background: Multiple primary malignant tumors (MPMTs) are defined as two or more histologically distinct malignancies in one individual, standard treatments for MPMTs are not well established, we aimed to clinical analyze the factors influence the treatment efficacy of MPMTs. Methods: This study retrospectively analyzed 15,321 malignant tumor patients at the Sir Run Run Shaw Hospital, College of Medicine, Zhejiang University, China, between March 2006 and June 2016. The survival analysis was performed with SPSS version 22.0 (SPSS Inc., Chicago, IL, USA) with Kaplan-Meier methodology. Results: The prevalence of MPMTs in our study was 1.09% (167/15321), with a male to female ratio of 2.34:1. Specifically, 98 patients harbored synchronous MPMTs, and 69 patients harbored metachronous MPMTs. The most common cancer pairs were digestive-digestive tumor (43 patients, 25.75%), digestive-lung cancer (32 patients, 19.16%), and head & neck-digestive tumor (11 patients, 6.59%). Among patients with synchronous and metachronous first primary cancers, 65.86% received surgery. 33.33% (27/81) of the patients with synchronous MPMTs received simultaneous resection. Of the 69 patients with metachronous MPMTs, 31.88% (22/69) were treated with surgery alone, 62.32% (43/69) received chemotherapy and/or radiotherapy for the first primary tumor, and 44.93% (31/69) received surgery for the other primary tumor. 98.20% (164/167) of patients with MPMTs were effectively followed up, the overall 2- and 5-year survival rates were 54.3% and 31.4%, respectively, with a median survival time of 28.0 months. Conclusions: The early diagnosis of rare MPMTs should not be neglected in patients not only when treated for a primary malignancy but also during long-term follow-up. Effective treatment for MPMTs may yield promising curative effect and warrants further investigation.
Surgical outcome of a resectable primary cancer of pancreas associated with another synchronous/metachronous resectable primary malignant tumor: a report of 13 patients and review of the literature

Abstract:Objective The incidence of a resectable primary cancer of pancreas associated with another synchronous/metachronous resectable primary malignant tumor is low.The aim of this study was to investigate the clinical features and to review our experience and outcomes with surgical treatment for these patients.Methods Between January 2000 and October 2012,13 patients (nine females and four males,median age:70 years,range:56-79 years) were admitted into our hospital with a resectable primary cancer of pancreas which was associated with another synchronous/metachronous resectable primary malignant tumor.The clinical features,treatment and results were analysed.Results The other primary tumors were non-small-cell lung carcinoma (n 2),gastric carcinoma (n=2),lobular carcinoma of breast (n=3),colonic carcinoma (n=2),esophageal carcinoma (n=1),renal cell carcinoma (n=1),malignant thymoma (n=1),and endometrialcarcinoma (n=1).Among these patients,eleven were metachronous pancreatic cancer which occurred after resection of the other organ malignancies.Two patients were synchronous pancreatic cancer.All patients underwent radical operation that included eleven pancreaticoduodenectomy and two distal pancreatectomies with splenectomy.There were no patients who died in the pert operative period,but three patients developed a pancreatic fistula.The morbidity rate was 20％.The median survival of the radically operated patients was 15months (range:4 35 months).Nine patients died of disease,while 4 are still alive at the time of this report.Conclusions Surgical resection could be performed safely in selected patients with a resectable primary carcinoma of pancreas which was associated with another synchronous/metachronous resectable malignant tumor with good long term survival outcomes.The final survival would depend on the primary cancer of pancreas.
Analysis of clinical epidemiology and pathological characteristics of multiple primary malignant tumors accompanied with colorectal malignant tumors

Abstract:Objective:To explore the epidemiological and pathological characteristics of multiple primary malignant tumors (MPMTs) accompanied with colorectal malignancies.Methods:The MPMTs patients with colorectal malignant tumors admitted to the Second Affiliated Hospital of Zhejiang University School of Medicine from January 2013 to December 2019 were screened, and the data of gender, age, onset interval, lesion distribution, TNM stage, and pathological type were collected. Then we analyzed the epidemiological characteristics and clinicopathological characteristics.Results:A total of 235 MPMTs patients with colorectal malignancies were treated, accounting for 4.00% of the 5878 patients with colorectal malignancies during the same period. There were 154 males and 81 females, with a male-to-female ratio of 1.90∶1. The median age at diagnosis of the first primary cancer was 64 years. There were 90 synchronous multiple primary carcinomas (SC) patients (38.30%) and 145 metachronous multiple primary carcinomas (MC) patients (61.70%), and the age of first primary cancer diagnosis in SC patients was older than that in MC patients (65 and 62 years, respectively, P=0.022). The median onset interval was 13.25 months (95% CI: 19.61, 27.23 months), and the age at diagnosis of the first primary cancer was negatively correlated with the onset interval ( r=-0.224, P<0.001). There were 257 colorectal malignant tumor lesions in 235 patients, which were distributed in all intestinal segments. The TNM stages were mainly Ⅱ and Ⅲ, and the pathological type was mainly adenocarcinoma. There were no significant differences in the distribution of intestinal segments, TNM stages and pathological types of colorectal malignant tumors between the SC group and the MC group (all P>0.05). There were 234 extraintestinal lesions, mainly distributed in the lung (48), stomach (45), and liver and gallbladder (25). The proportion of extraintestinal lesions located in the stomach of SC patients was 32.18% (28/87), which was higher than that of MC patients [11.56% (17/147), P<0.001]; the proportion of extraintestinal lesions located in the thyroid of SC patients was 2.30% (2/87), lower than that of MC patients [10.20% (15/147), P=0.024]. Conclusions:Approximately 4% of patients with colorectal malignant tumors suffer from two or more malignant tumors, slightly higher in male than in female, and slightly higher in MC patients than in SC patients. The diagnosis age of the first and second primary malignant tumors is mainly concentrated among 55-75 years old, and the interval is inversely related to the diagnosis age of the first primary malignant tumor. The colorectal malignant tumors of SC and MC patients are distributed in all intestinal segments, and the pathological stages are mainly stage Ⅱ and Ⅲ, while the pathological types are mainly adenocarcinoma. Extraintestinal lesions are mostly located in the lungs and stomach.
Do Patients Diagnosed with Metastatic Pancreatic Cancer Benefit from Primary Tumor Surgery? A Propensity-Adjusted, Population-Based Surveillance, Epidemiology and End Results (SEER) Analysis

Abstract:BACKGROUND With the progress in surgical techniques and management of complications, pancreatic resection can be safely performed in experienced hospitals. Pancreatic resection enables surgeons to assess the effect of surgery for metastatic cases, even when there is limited information. In the present study we evaluated the role of primary tumor resection for metastatic pancreatic cancer (mPC) by using the Surveillance, Epidemiology and End Results (SEER) database. MATERIAL AND METHODS Metastatic pancreatic cancer patients treated at our hospital from 2004 to 2015 were identified. The effect of surgery on cancer-specific survival was assessed by restricted mean survival time (RMST) and stabilized inverse probability of treatment weight-adjusted analysis after propensity score matching (PSM). RESULTS A total of 2694 mPC patients were included. Of this population, 365 adults underwent primary tumor resection. After propensity matching, postsurgical patients had longer RMST than non-surgery patients (1: 1 PSM 11.60 months vs. 8.98 months; 1: 2 PSM 11.61 months vs. 9.10 months; p<0.01). Stabilized inverse probability of treatment weight-adjusted analysis yielded similar results (p<0.01). CONCLUSIONS Our study supports the hypothesis that patients with mPC can benefit from primary tumor surgery. However, the surgical inclusion criteria and the appropriate role of surgery, such as its effect on symptom control, quality of life, and the extent to which it prolongs survival for metastatic pancreatic cancer, remain to be completely assessed by well-designed, prospective, randomized clinical trials.
The Impact of a Prior Malignancy on Outcomes in Gastric Cancer Patients

Abstract:Abstract Background The number of cancer survivors has increased rapidly, and there is a higher risk of developing a second cancer. Whether a prior malignancy could affect survival outcomes is unknown. We aimed to investigate the clinical characteristics and prognostic outcomes of prior malignancies in patients with gastric cancer. Methods Patient data were extracted from the Surveillance, Epidemiology, and End Results (SEER) database. We used the Kaplan–Meier method, competing risk models, and Cox regression models to evaluate the impact of prior malignancy on survival outcomes. Results Among 71,809 patients with primary gastric cancer, 6667 (9.3%) patients had a pre‐existing cancer. Prostate (31.86%), breast (14.34%), and colon and rectum (10.32%) cancer were the most common types. A significant difference was observed in the overall survival rates between patients with and without prior cancer (log‐rank=139.73, p < 0.001). In the subgroup analysis, patients with prostate, uterine corpus, lung and bronchus, colon and rectum, esophagus, urinary bladder, leukemia, brain and other nervous system, oral cavity and pharynx, and breast cancer faced inferior survival than those without prior cancer. Conclusions A history of prior cancer was associated with worse overall survival in patients with gastric cancer, and the effects varied by different initial cancer types. The exclusion and inclusion of patients who had previous malignancies should be reconsidered according to the specific malignancy types.
The Effects of a Prior Malignancy on the Survival of Patients with Ovarian Cancer: a Population-Based Study

Abstract:Background: With the improvement in the prognostic outcomes of multiple malignancies, the population of cancer survivors is growing rapidly and is at higher risk of developing secondary ovarian cancer. However, the prevalence and clinical outcomes of prior cancer among newly diagnosed ovarian cancer patients remain unknown. Methods: Patients diagnosed with ovarian cancer between 2004 and 2015 were identified using the Surveillance, Epidemiology, and End Results database. Patients were divided into two groups based on whether there was a prior malignancy. A multivariate Cox regression analysis was used to calculate all-cause and ovarian-specific survival. Furthermore, we conducted subgroup survival analyses of patients stratified by previous cancer site to explore the associations between prior cancer site and survival outcomes. Results: A total of 52,182 patients with primary ovarian cancer were identified, and 3.6% (n=1,860) had a documented prior malignancy. In multivariate analyses, patients with prior malignancies had a worse all-cause and ovarian cancer-specific prognosis than those without. In subset analyses, patients with a history of thyroid cancer had a better all-cause and ovarian cancer-specific prognosis, and patients with prior colorectal, urinary system, skin, lung, haematologic and stomach cancers were at risk of decreased survival compared to that of patients without a prior cancer. Conclusions: Prior malignancy has an adverse impact on the survival of patients with ovarian cancer, and the impact on prognostic outcomes varies by different prior cancer sites. The inconsistent survival effects of previous malignancies should be considered in clinical trial design and recruitment.

Abstract:Multiple primary cancer (MPC) denotes individuals with two or more malignant tumors occurring simultaneously or successively. Herein, a total of 11,000 pancancer patients in TCGA database (1993–2013) were divided into MPC or non-MPC groups based on their history of other malignant tumors. The incidence of MPC has risen to 8.5–13.1% since 2000. Elderly individuals, males, early-stage cancer patients, and African Americans and Caucasians are identified as independent risk factors (p < 0.0001). Non-MPC patients exhibit significantly longer overall survival (OS) and disease-free survival (DFS) (p = 0.0038 and p = 0.0014). Age (p < 0.001) and tumor staging at initial diagnosis (p < 0.001) contribute to this difference. In our center, MPC was identified in 380 out of 801 tumor events based on SEER criteria. The peak occurrence of secondary primary was about 1–5 years after the first primary tumor, with a second small peak around 10–15 years. Multiple tumors commonly occur in the same organ (e.g., breast and lung), constituting 12.6%. Certain cancer types, notably skin cutaneous melanoma (SKCM), exhibit significantly higher tumor mutational burden (TMB) in the MPC group (17.31 vs. 6.55 mutations/MB, p < 0.001), with high TMB associated with improved survival (p < 0.001). High TMB in MPC may serve as a predictor for potential immunotherapy application.

Abstract:PURPOSE:In the past 20 years, surgical resection has been a secure and applicable procedure for pancreatic cancer (PC), but it remains controversial for stage III PC with data evaluating its efficacy mostly derived from small randomized trials. Hence, we designed this study to further evaluate its benefit using surveillance, epidemiology, and end results dataset.PATIENTS AND METHODS:Patients with stage III PC were identified in the surveillance, epidemiology, and end results registries from 2004 to 2014. The effect of surgery on cancer-specific survival was assessed by risk-adjusted Cox proportional hazard regression modeling and propensity score matching.RESULTS:Overall, 6,138 patients with stage III PC were included. Of these, 608 patients underwent primary tumor surgery. On multivariable analyses, surgery was independently associated with improved cancer-specific survival (HR=0.580; 95% CI=0.523-0.643, p<0.001). The survival benefit with surgery was also observed in the propensity score-matched cohort (HR=0.501; 95% CI=0.438-0.573, p<0.001).CONCLUSION:Primary tumor surgery is associated with improved survival in stage III PC. Prospective randomized trials are needed to confirm these results, and further efforts are required to address patient selection.

Abstract:Aim: To investigate the nature of multiple primary cancers initiated by esophageal cancer-multiple primary cancers (EC-MPC). Patients & methods: SEER data about patients’/tumor characteristics, and survival were analyzed and compared. Results & conclusion: 1727 of 29,733 registered EC patients have EC-MPC. Individuals diagnosed at 60–79 years old, earlier stage and/or moderately differentiated EC were more likely to get EC-MPC. Fewer patients in the EC-MPC group suffered from metastases. Patients in the EC-MPC group showed a longer survival rate and lower EC-specific deaths. Other factors like age, sex, race, tumor differentiation and Tumor, Node, Metastasis stage also affected survival. Radiation can improve survival. EC-MPC patients have some distinct features compared with solitary EC.

Abstract:INTRODUCTION:Pancreatic cancer is a lethal disease with a very poor prognosis. This study investigates survival of patients diagnosed with metastatic pancreatic cancer (mPC) based on local treatment of the primary tumor. METHODS:Patients diagnosed with stage IV mPC between 2004 and 2013 were identified from the Surveillance, Epidemiology and End Results (SEER) database. Cancer-specific survival (CSS) and overall survival (OS) were examined. CSS and OS were examined by using the Kaplan-Meier method with the log-rank test. Multivariable survival analyses of CSS and OS were conducted using the Cox proportional hazard model. RESULTS:A total of 28918 patients with mPC were included in this analysis. There were 467 patients who received surgical resection (1.6%) and 28451 patients who did not (98.4%). Patients who were younger than 70 years (odds ratio [OR]=1.45, 95% CI=1.04-2.03, p=0.03), diagnosed from 2004 to 2008 (OR=1.49, 95% CI=1.25-1.80, p<0.001), female (OR=1.31, 95% CI=1.08-1.58, p<0.001), married (OR=1.56, 95% CI=1.27-1.90, p<0.001), at T3 stage (OR=3.53, 95% CI=1.10-11.37, p=0.035), at N1 stage (OR=2.05, 95% CI=1.68-2.50, p<0.001), presenting histological types other than adenocarcinoma (OR=2.04, 95% CI=1.43-2.94, p<0.001), and with tumor of the pancreatic head (OR=1.90, 95% CI=1.27-2.82, p=0.002) were more likely to be treated with surgical resection. The results of multivariate analysis showed that surgical resection of the primary tumor was associated with CSS (hazard ratio [HR]=0.58, 95% CI=0.52-0.64, p<0.001) and OS (HR=0.59, 95% CI=0.53-0.65, p<0.001) benefits. In addition, not receiving chemotherapy (HR=2.33, 95% CI=2.27-2.39, p<0.001), age >50 years (HR=1.25, 95% CI=1.09-1.42, p=0.001), male (HR=1.121, 95% CI=1.09-1.15, p<0.001), black ethnicity (HR=1.11, 95% CI=1.1-1.15, p<0.001), unmarried (HR=1.20, 95% CI=1.17-1.23, p<0.001), histological type of adenocarcinoma (HR=1.18, 95% CI=1.14-1.22, p<0.001), and primary site other than the pancreatic head (HR=1.08, 95% CI=1.05-1.11, p<0.001) are factors associated with poor survival. CONCLUSION:This study reveals that local treatment has the primary benefit of both CSS and OS in patients with mPC. These results may guide the management of this patient population.
Rare Heterochronous Liver and Pancreatic Multiple Primary Cancers: a Case Report and Literature Review

Abstract:In recent years, with the continuous improvement of medical diagnostic techniques, the incidence of multiple primary carcinoma (MPC) has increased gradually. Elderly patients are at high risk of MPC. However, the risk of developing a second primary malignancy is 1 % for primary liver malignancy 1. The risk of pancreatic cancers as secondary malignancies with primary liver malignancy is very rare. Here, we report a rare case of heterochronous liver and pancreatic MPCs and review the related literature. We report this case to raise the clinician’s attention to the disease because the long-term survival rate can be effectively improved by timely diagnosis and appropriate treatment. For patients with HCV-positive primary cancers, active antiviral therapy should be simultaneously provided with antitumor therapy, thereby effectively reducing the incidence of MPC.

Abstract:Objective To investigate the incidence and clinical features of multiple primary malignant tumors related to renal cell carcinoma. Methods The patient history, treatment, and prognosis in 21 cases of multiple primary malignant tumors associated with renal cell carcinoma were analyzed retrospectively. Results From December 2004 to December 2007, the incidence rate of multiple primary malignant tumors associated with renal cell carcinoma in our hospital was 6.23% (21/337). In 12 male cases, prostate cancer, colon cancer, esophageal cancer, and non-Hodgkin’s lymphoma were the most common. Occurrence of other primary malignancies before renal cell carcinoma was found in 4 cases, co-occurrence in 3 cases, and post-occurrence in 5 cases. Nine female patients were mainly characterized by breast cancer. Occurrence of other primary malignancies was found in 7 cases, but co-occurrence in 2 cases. The sub-type of renal cell carcinoma belonged to clear cell carcinoma in all patients. Conclusion Multiple primary malignant tumors and concurrent renal cell carcinoma are not rare. The treatment of multiple primary malignant tumors related to renal cell carcinoma should be conducted in accordance with the principle of different strategies for different malignancies.
Clinicopathological features and prognosis of patients with esophageal cancer as the second primary cancer: a large population-based analysis using the SEER program [2000-2015]

Abstract:Background: This study used the Surveillance, Epidemiology, and End Results (SEER) data to investigate clinicopathological characteristics and prognoses of patients with esophageal cancer as a second primary cancer from 2000 to 2015. Methods: Esophageal cancer as the second primary cancer (SEPC) patient information collected from the SEER database were extracted, evaluated, and compared with only primary esophageal cancer (OPEC) patient data. Overall survival (OS) was evaluated by the Kaplan-Meier method. Univariate analysis (UVA) and multivariate analysis (MVA) by the Cox proportional hazards regression identified risk factors that predicted OS and the results were used to construct a nomogram to predict OS in SEPC patients. Results: A total of 7,161 esophageal cancer patients were identified from the SEER registry, which included 813 patients (11.4%) with SEPC in the patient history. The most common cancer for the first primary cancer prior to esophageal cancer (FPC) was genital system cancer (43.5%). The median OS was 7 (95% CI: 6.69-11.31) months and was not significantly different from that of patients with OPEC. The patients with SEPC appeared with unique clinical pathological features compared with OPEC, including older age, more squamous cell carcinoma, and early stage esophageal cancer, and less lower third of esophageal cancer (P<0.001). UVA and MVA identified M stage, surgery, and chemotherapy as independent risk factors. The nomogram had a calibration index of 0.682. Conclusions: SEPC had unique clinicopathological characteristics. M stage, surgery, and chemotherapy were independently associated with OS. The nomogram accurately predicted OS.
Clinicopathological Characteristics and Prognosis Analysis of Colorectal Synchronous Multiple Primary Cancer

Abstract:OBJECTIVE:To investigate the clinicopathological features and prognosis of colorectal synchronous multiple primary cancer(SMPC).METHODS:From January 2008 to June 2011, 51 patients diagnosed with colorectal SMPC underwent surgery at Department of General Surgery of Peking University First Hospital. Their clinicopathological features, diagnosis, treatment and prognosis were summarized and analyzed. SMPC was diagnosed according to the following criteria: each tumor must have a definite pathologic picture of malignancy; metastasis or recurrence from another colorectal cancer was excluded; tumors must be distinctly separated by at least 5 cm of all intact bowel wall from each other; SMPC has abnormal cells between tumor and normal mucosa and abnormal gland of transitional zone; each cancer is infiltrating carcinoma except the carcinoma in situ; all the cancers are detected at the same time or within 6 months. Multiple primary colorectal cancer originated from familial colonic polyposis or ulcerative colitis was excluded.RESULTS:These 51 colorectal SMPC patients accounted for 3.5% of 1 452 colorectal cancer patients in the same period at our hospital, with 32 males and 19 females, and mean age of (63±13)(29 to 82) years. Of 51 cases, 46(90.2%) had 2 original carcinoma, 3(5.9%) had 3 original carcinoma and 2(3.9%) had 4 carcinoma; 23(45.1%) complicated with colon polyps, 4(7.8%) complicated with malignancy outside the colorectum. In TNM staging, 7(13.7%), 15(29.4%), 24(47.1%) and 5(9.8%) patients were stage I(, II(, III( and IIII( respectively. Among 51 patients undergoing surgery by different procedures, 16 were subtotal colon resection, 8 were extended right colon resection, 5 were extended left hemicolon resection, 8 were right hemicolon resection plus Dixon procedure, 10 were Dixon, and 4 were right hemicolon resection plus sigmoid colon resection. Adjuvant chemotherapy and support treatment were given according to the condition after operation. A total of 105 tumors were found, including 25(23.8%) tumors in sigmoid colon, 24(22.9%) in rectum, 22(21.0%) in ascending colon and 4 in organs outside the colorectum. Tubular adenocarcinoma (86/105, 81.9%) was the main pathological type in these colorectal SMPC patients. During the follow-up of median 43.5 months, 10 cases presented local recurrence and 6 cases had liver metastasis. Multivariable analysis showed that ≤65 years old (OR=22.757, 95%CI: 1.562-331.543, P=0.002),undifferentiated carcinoma or mucous adenocarcinoma (OR=27.174, 95%CI: 2.834-260.512, P=0.004), stage III(-IIII( (OR=29.626, 95%CI: 3.216-272.884, P=0.003) were independent risk factors of postoperative 5-year recurrence and metastasis, but the number of SMPC lesions and the surgical method were not associated with postoperative 5-year recurrence and metastasis (P=0.564, P=0.513). The 3-year and 5-year survival rates of colorectal SMPC patients were 76.5% and 64.7%.CONCLUSION:Two-original carcinoma is the most common in colorectal SMPC patients, which mainly distributes in sigmoid colon and rectum. Postoperative monitoring should be strengthened for those patients with younger age, poor pathological types and advanced staging to prevent recurrence and metastasis.
Impact of tumor multiplicity on the prognosis of patients with primary renal cell carcinoma: a SEER database analysis

Abstract:To compare clinical characteristics and survival outcomes of patients with multiple renal cell carcinoma versus single renal cell carcinoma. Develop a prognostic model for predicting prognosis in patients with multiple tumors and analyze prognostic factors. Patients with primary multiple renal cell carcinoma were selected from the Surveillance, Epidemiology, and End Results database (2004–2015). They were divided into single-tumor and multiple-tumor groups. Survival analysis was conducted using the Kaplan–Meier method and log-rank test. A Cox regression model was used to identify potential prognostic factors. A total of 19,489 renal cell carcinoma cases were included, with 947 in the multiple-tumor group and 18,542 in the single-tumor group. The multiple-tumor group had lower cancer-specific survival ( P = 0.03, HR = 1.431). Cox regression identified risk factors for the multiple-tumor group including number of tumors, gender, combined summary stage, T stage, N stage, tumor size, and type of surgery. The predicted probabilities showed acceptable agreement with the actual observations at 3-, 5-, and 8-years area under the curve values in both the training and validation cohorts (0.831 vs. 0.605; 0.775 vs. 0.672; and 0.797 vs. 0.699, respectively). Compared with single renal cell carcinoma, multiple renal cell carcinoma is associated with decreased cancer-specific survival. Additionally, we identified several prognostic factors including the number of tumors, T stage, tumor size, and type of surgery. These findings offer valuable insights for selecting appropriate treatment strategies for patients diagnosed with multiple renal cell carcinomas.

Abstract:BACKGROUND Multiple primary malignant tumors (MPMTs) was first described by Billroth as early as 1889, with the first report published by Warren and Gates in 1932. Since then, numerous cases have been reported. A literature review of 1104269 patients with cancer revealed that the incidence of MPMTs ranged from 0.73 to 11.7%. In recent years, however, there has been a significant upward trend in the incidence of this phenomenon, which may be associated with many different factors, including the advancement of modern diagnostic procedures facilitating the examination and diagnosis of more MPMTs, increased exposure to chemotherapy and radiotherapy that exacerbate the risk of new malignant tumors in patients with cancer, and prolonged survival of patients with cancer allowing sufficient time for the development of new primary cancers. AIM To analyze the incidence, clinical features, treatment factors, prevalence, and prognosis of patients with MPMTs in the gastrointestinal tract treated in a single center. Additionally, we analyzed the different tumor combinations, time interval between the occurrence of tumors, and staging. METHODS This retrospective cohort study analyzed 8059 patients with pathologically confirmed gastrointestinal malignant tumors treated at the Gansu Province Hospital in Lanzhou, Gansu, China between June 2011 and June 2020. Of these, 85 patients had MPMTs. The clinical features, treatment factors, prevalence, and prognosis of this latter cohort were analyzed. RESULTS The incidence of MPMTs in patients with gastrointestinal malignant tumors was 1.05% (85/8059), including 83 double primary malignant tumors and two triple primary malignant tumors of which 57 (67.06%) were synchronous MPMTs (SMPMTs) and 28 (32.94%) were metachronous MPMTs (MMPMTs). The most frequent associations were found between the rectum colon cancers within the SMPMT category and the gastric-colon cancers within the MMPMT category. For the MMPMTs, the median interval was 53 months. The overall 1-, 3- and 5-year survival rates from diagnosis of the first primary cancer were 91.36%, 65.41%, and 45.97%, respectively; those from diagnosis of the second primary cancer were 67.90%, 29.90%, and 17.37%, respectively. CONCLUSION MPMTs in the gastrointestinal tract have a high incidence and poor prognosis. Thus, it is necessary to perform both gastroscopy and colonoscopy in patients with gastrointestinal tumors. Multidisciplinary comprehensive diagnosis and treatment may improve the diagnosis rate and treatment efficiency of MPMTs.

The Incidence of Second Primary Cancer in Male and Female Patients with Initial Colorectal Cancer: a SEER Population-Based Study.

Abstract:Background Second primary cancer (SPC) after primary colorectal cancer (CRC), emerges as a novel challenge for cancer prevention with pronounced differences between female and male patients. Methods This was a retrospective study of 140 907 CRC survivors from the surveillance, epidemiology, and end results program database. Competing risk models and nomograms were constructed to predict the risk of SPCs, which were assessed with the C-Index, calibration and decision curve analysis. Results The 10-year cumulative incidence of SPC was higher in male than in female CRC survivors. The top five common SPCs in female CRC survivors were colorectal, breast, lung and bronchus, corpus and uterus and pancreatic cancers, while in male were prostate, colorectal, lung and bronchus, urinary cancer and melanoma of the skin. Breast and prostate were the most common sites for the development of SPCs after CRC. Older age, stage I and surgery were common risk factors for SPCs in both female and male. The nomogram for predicting the risk of developing SPC-breast cancer in female patients included age, race, site, histology grade, surgery, chemotherapy and stage. However, the model of predicting SPC-prostate cancer in male patients included age, race, site, size, surgery, chemotherapy, radiation and stage. Notably, the nomograms were validated to have a precise discriminative ability, accuracy and clinical effectiveness. Conclusions The study surveyed the characteristics of CRC survivors with a particular focus on the incidence of SPC. The models could help supervise the development of a second breast or prostate cancer in female or male CRC survivors.
Metachronous Second Primary Cancers: Clinical Analyses of 506 Cases in a Single Institution

Abstract:Objective: To elucidate the clinical features and prognosis of multiple primary cancers, in order to make improvement of diagnosis and treatment. Methods: A total of 506 patients with two primary cancers admitted from 1973 to 2004 were analyzed retrospectively. Results: These cases accounted for 0.9% of all the hospitalized cases in the same period among which 126 were males, with the ratio of male to female 1:3. The median age at the onset of the first disease was 48 y (ranged from 24 to 77). The interval between the two cancers was longer in patients under 50 y and in males, but without statistical significance. The onset age of the two primary cancers was mainly centered around 40 to 60 y, while 70% of the second cancer occurred within 80 m after the first cancer but half of them occurred within five years. The interval between the two cancers played crucial role in affecting the prognosis ( P <0.005). Conclusion: Fewer lethal cancers are involved in either the primary or the secondary malignancies. The interval between the two primaries contributes most to the prognoses.

Abstract:Purpose:To probe into the multiple primary malignancies in urogenital system.Methods:Fifteen urogenital cancer patients with multiple primary malignancies were reviewed. The diagnosis, incidence, etiology and prognosis are discussed in the light of literature.Results: Survival time was 1.5 years in 1 case, 2 years in 1, 3 years in 2, 5 years in 2 after the second cancer were operated on. Conclusions:It was emphasized that multiple primary carcinoma was different from late metastatic cancer, so the significance of the differential diagnosis between multiple primary malignance and metastatic cancer is important for patients with multiple malignancies.
