Abstract:Objective To discuss the clinical characteristics, fungal species, therapy and prevention of candidemia.Methods A retrospective analysis was performed on the predisposing factors, distribution of fungi,treatment and outcomes of 14 cases of candidemia from August 2011 to July 2016.Results The majority of the cases had three or more predisposing factors.The most frequently found predisposing factors were elderly male, hypoalbuminemia, use of broad spectrum antibiotics, and deep venous catheterization.The duration of hospitalization from admission to positive result of fungal culturing was (30.3 ± 28.4) days on average.Candida albicans were the most frequently isolated pathogens (7, 46.7％).Caspofungin and fluconazole were the main therapeutic methods to treat candidemia.The overall mortality of candidemia was 28.6％.Conclusion Candidemia usually occurred in patients with multiple predisposing factors, especially those with hypoalbuminemia and deep venous catheterization.Correct choice of antifungal agents is critical for the management of high risk patients with candidemia.
Incidence, Clinical Characteristics, Risk Factors and Outcomes of Patients with Mixed Candida/bacterial Bloodstream Infections: a Retrospective Study

Abstract:Abstract Purpose The mixed Candida/bacterial bloodstream infections (mixed C/B-BSIs) is worthy of particular attention recently, and we analyzed the incidence, co-pathogens, clinical characteristics, risk factors, and outcomes of mixed C/B-BSIs compared with monomicrobial candidemia (mono-candidemia) in adult patients in China. Methods All hospitalized adults with candidemia were recruited for this retrospective observational study from January 1, 2013, to December 31, 2019. Results Of the 296 patients with candidemia, 78 cases (26.3%) were mixed C/B-BSIs. Candida albicans (C. albicans) was the most common Candida species among all candidemia, and Klebsiella pneumoniae (K. pneumoniae) was the most concomitant bacteria (30.6%), followed by Acinetobacter baumannii (A. baumannii) (12.9%) and Enterococcus faecium (E. faecium) (11.8%) in mixed C/B-BSIs. In the multivariable analysis, prior β-lactams exposure [adjusted odds ratio (aOR), 1.97; 95% confidence interval (CI), 1.01–3.87], burn injury (aOR, 6.35; 95% CI 1.82–22.21) and continuous renal replacement therapy (CRRT) (aOR, 3.00; 95% CI 1.46–6.17) were independent risk factors for mixed C/B-BSIs. Compared with mono-candidemia, patients with mixed C/B-BSIs developed with more proportion of septic shock (55.1% vs. 39.9%, P < 0.05), prolonged stay in ICU [22.0(12.0–57.0) vs. 9.5(0.0–37.0) days, P < 0.001] and longer mechanical ventilation time [19.0(4.5–40.8) vs. 6.0(0.0–24.8) days, P < 0.001]. The in-hospital mortality in patients with mixed C/B-BSIs was higher than those with mono-candidemia (59.0% vs. 34.9%, P < 0.001). Survival analysis revealed that 28-day and 60-day mortality were significantly higher in patients with mixed C/B-BSI than in those with mono-candidemia (57.7% vs. 31.7%, P < 0.001; 59.0% vs. 34.9%, P < 0.001; respectively). Conclusions There is a high rate of mixed C/B-BSIs cases among candidemia, and K. pneumoniae is the predominant coexisting species. Prior β-lactams exposure, burn injury, and CRRT are independent risk factors for mixed C/B-BSIs. The mortality of patients with mixed C/B-BSIs is significantly higher than those with mono-candidemia, this deserves further attention for clinicians.

Abstract:Objective To investigate the clinical characteristics and associated risk factors for patients with mixed Candida/bacterial bloodstream infections (BSIs).Methods A retrospective study was conducted in the Second Affiliated hHospital of Zhejiang University School of Medicine from February 2012 to June 2015.The clinical data of cases was collected,and the clinical characteristics,the microbiology data and outcomes in patients with mixed Candida/bacterial BSIs confirmed by blood culture were compared with those with candidaemia.A Logistic regression analysis was performed to investigate the independent risk factors.Results A total of 136 candidaemia cases were analyzed including 40 cases (29.4％) of mixed Candida/boacterial BSIs and 96 cases of candidaemia.Among the 136 candidas strains,the proportion of non-albicans exceeded the albicans (50.7％ vs 49.3％),although the later was still the predominant one.There was no significant difference in the distribution of candidas strains between patients with mixed Candida/bacterial BSIs and patients with candidaemia.In patients with mixed Candida/bacterial BSIs,25 strains (61.0％) of gram-positive cocci and 16 strains (39.0％) of gram-negative bacilli were isolated.Compared with patients with candidaemia,patients with mixed Candida/bacterial BSIs needed longer period of antifungal therapy [12.0 (4.0-25.0)days vs 7.0 (3.0-13.5) days,P=0.027],but the crude 30-day and 90-day mortality did not differ between the two groups (40.0％ vs 32.3％;45.0％ vs 36.5％;both P＞0.05).Univariate analysis revealed that the prior hospital stay,ICU admission at the onset of candidaemia,blood transfusion,human albumin infusion,mechanical ventilation,linezolid use and high SOFA score were related with the occurrence of mixed Candida/bacterial BSIs (all P＜0.05).Multivariate analysis showed that only high SOFA score was the independent risk factor (P=0.003).Conclusions Gram-positive cocci were the predominant species in mixed Candida/bacterial BSIs.Compared with candidaemia,mixed Candida/bacterial BSIs needs a longer ICU stay,a longer hospital stay,and a prolonged antifungal therapy.High SOFA score is the independent risk factor for mixed Candida/ bacterial BSIs.

Abstract:Abstract Background Antifungal prophylaxis may result in breakthrough infections in hematology patients with severe agranulocytosis, with few studies assessing risk factors and clinical outcomes of breakthrough candidemia. We described the distribution of Candida species, assessed risk factors for mortality in such patients, and determined differences in the incidence and mortality of breakthrough candidemia between patients who did or did not receive an allogeneic hematopoietic stem cell transplant. Methods We collected clinical and microbiological data of patients with hematologic malignancies and breakthrough candidemia from a single center. Seven-day and 30-day follow-up outcomes were recorded; the incidence and mortality of breakthrough candidemia between patients who did or did not undergo an allogeneic transplant were compared. Kaplan-Meier survival estimates were used to generate survival curves, and predictors were identified using Cox regression analyses. Results Of 71 enrolled patients, 17 received allogeneic transplants. Incidences of breakthrough candidemia were 17 of 2924 (0.58%) and 54 of 12 015 (0.45%) in the transplant and nontransplant groups, respectively (P = .35). The most common isolate was Candida tropicalis, and antifungal agent combinations were the most common first-line treatment. Cumulative mortality rates of patients were 21.1% and 31.0% at days 7 and 30, respectively, and they significantly differed between both groups. Septic shock, central venous catheter removal, and granulocyte recovery were significantly associated with 7-day mortality; the latter 2 remained independent predictors of 30-day mortality. Conclusions Breakthrough candidemia-related mortality was higher in the allogeneic transplant group, although the incidence was not significantly different between the groups. Prompt and adequate antifungal treatment with catheter removal may reduce mortality.

Clinical Features and Prognostic Risk Factors of Candidemia in Non-Intensive Care Unit Wards

Abstract:Objective To investigate clinical features,pathogen distribution and prognostic risk factors of candidemia in patients from non-intensive care unit (ICU) wards.Methods Seventy two patients with blood culture-confirmed candidemia were admitted in non-ICU wards in the First Affiliated Hospital of Zhejiang University during January 1 st 2011 and January 31 th 2014.The clinical data of patients,including clinical features,pathogen distribution,treatment and prognosis were retrospectively analyzed.The risk factors for the infection and the fatality of patients were assessed by chi-square test,Fisher exact probability test and multivariate Logistic regression analysis.Results Among 72 patients with candidemia,29 (40.28％) were related to Candida albicans and 43 (59.72％) were related to non-Candida albicans.Univariate analysis showed that elderly people (x2 =6.109,P ＜ 0.05),male (x2 =5.258,P ＜ 0.05),solid tumor (x2 =7.186,P ＜ 0.01),parenteral nutrition (x2 =6.512,P ＜ 0.05),surgery within 30 d (x2 =4.284,P ＜ 0.05),indwelling gastric tube (x2 =4.164,P ＜ 0.05),and indwelling urethral catheter (x2 =3.952,P ＜ 0.05) were risk factors of Candida albicans-related candidemia;while patients with hematologic malignancies (P ＜ 0.0l) and neutropenia (P ＜ 0.05) were prone to non-Candida albicansrelated candidemia.Fourteen cases died within 28 days from the diagnosis of candidemia with an overall 28-d fatality rate of 19.44％.Multivariate Logistic regression analysis indicated that septic shock(OR:84.818,95％ confidential interval:3.560-2020.567,P ＜ 0.01)and acute physiology and chronic health evaluation Ⅱ(APACHE Ⅱ) score (OR:1.252,95％ confidential interval:1.035-1.515,P ＜ 0.05) were independent risk factors of death,while removal or replacement of central venous catheter (OR:0.002,95 ％ confidential interval:0-0.135,P ＜ 0.01) was a protective factor.Conclusions There are more non-Candida albicans-related candidemia than Candida albicans-related candidemia in non-ICU wards.Patients with high APACHE Ⅱ score and septic shock usually have poor prognosis,while removal or replacement of central venous catheter may improve the prognosis.
Elevated serum level of progranulin is associated with increased mortality in critically ill patients with candidemia

Abstract:Candidemia is a severe disease with high mortality in both intensive care unit (ICU) and non-ICU settings. Considering that progranulin (PGRN) is a potential therapeutic target for the candidemia caused by Candida albicans, we determined the serum level of PGRN after candidemia and evaluated its association with mortality. A retrospective discovery cohort (62 patients) and a validation cohort (70 patients) were enrolled. Blood was collected on day of first blood culture positivity for C. albicans, and serum PGRN levels were then measured. In the discovery cohort, all serum PGRN studied were expressed at higher levels in candidemia patients than in bacteremia patients and healthy volunteers, non-survivors presented with significantly higher serum PGRN concentrations when compared with survivors. Serum PGRN concentration was associated with 30-day mortality and patients at a higher risk of death showed higher serum PGRN levels. These results were confirmed in the independent validation cohort. Interestingly, in vitro study demonstrated that macrophages, neutrophils and lymphocytes may be the major source of PGRN production after C. albicans infection instead of epithelial cells. Our findings highlight that serum PGRN appears as a biomarker in candidemia patients and as a promising tool for mortality risk stratification in managing candidemia.
Use of Granulocyte Transfusions in the Management of Severe Infections Among Children with Neutropenia

Abstract:Background/Objectives: Infections remain the leading cause of mortality among neutropenic patients with haematologic malignancies, making effective infection management crucial. Achieving a sufficient neutrophil count is essential for the elimination of pathogens. Granulocyte concentrate (GC) can be a treatment option for neutropenic patients with severe infections. This study aimed to evaluate the efficacy, safety, and impact on survival of GC transfusions in neutropenic children with severe infections treated over the past 13 years in a single centre. Methods: The retrospective study analysed clinical data from 60 children (median age 9.5 years) who received GC transfusions at our centre. Granulocytes were collected by apheresis from donors stimulated with granulocyte colony-stimulating factor. The majority of the patients (70%) were diagnosed with acute leukaemia. The main indications for GC were severe pneumonia (45%) and bacterial sepsis (38.33%). Results: The patients received 1 to 29 GC transfusions for 1 to 70 days, with a median time of administration of 3 days. Neutrophil counts increased to >1000/μL within a median of 5 days. GCs were well tolerated by most patients. One patient presented symptoms of anaphylaxis, the other acute lung injury related to transfusions, and alloimmunisation was reported in one patient. Of the patients analysed, 78.33% survived the infection that justified GC administration. We did not observe significant differences in survival depending on the aetiology of the infection. Conclusions: Based on our research, GC appears to be a beneficiary for neutropenic children with severe infections and reduces infection mortality rates. However, further well-designed randomised trials are needed to define its role in this setting.

Abstract:Fifteen patients with prolonged neutropenia (a median of 23 days with granulocyte [PMN] < or = 500/microliters) and established fungal infections that had not responded to adequate antifungal therapy were transfused with PMN concentrates collected from 35 cytokine-primed granulocyte colony-stimulating factor (GCSF) donors. Patients received a median of six transfusions. Leukocytosis and granulocytosis were observed within 24 hours of the first GCSF injection, which yielded concentrates averaging 55 x 10(9) white blood cells and 41 x 10(9) PMN. Data analysis suggested that response might be related to the duration of neutropenia and known infection, as patients given PMN tx earlier in the infectious course tended to have a better response. No significant toxicity was observed in donors.

Abstract:OBJECTIVE:To investigate thee status of Candida colonization and the risk factors of invasive fungal infection (IFI) in hospitalized patients with hematological malignancies.METHODS:114 patients with hematological malignancies admitted to the hematology wards from May 2004 to April 2005 underwent fungal culture of their samples of urine, feces, and saliva once a week until the end of the sixth week of hospitalization or they were discharged. Culture of blood, sputum, or sterile body fluids were carried out when the patients were suspected to have IFI.RESULTS:165 strains of Candida spp. were isolated from 46 of the 114 patients, C. albicans accounting for 78.8% and non-albicans Candida for 21.2% respectively. Candida was found in 38 patients (33.3%) were found to have colonization of Candida, chiefly C. albicans (76.3%). The risk factors of Candida colonization included long duration of leucopenia (WBC < 0.5 x 10(9)/L for more than 10 days) and broad-spectrum antibiotic administration. Univariate analysis showed that the risk factors of IFI included Candida colonization, long duration of leucopenia, and administration of carbapenem antibiotics, and multivariate analysis showed that long duration of leucopenia (WBC < 0.5 x 10(9)/L for more than 10 days) was the only risk factor of IFI (P = 0.015).CONCLUSION:C. albicans is still the major organism isolated from the patients with hematological malignancies. Patients with hematological malignancies have a high incidence of Candida colonization. Long duration of leucopenia is the only risk factor for IFI.

Abstract:IntroductionBloodstream infections caused by Candida species, known as candidemia are on the rise due to increasing complexity of surgical procedures, patient's underlying co‐morbidities and shift in patient's demographics. This study was conducted to evaluate the epidemiology, risk factors, co‐morbidities, antifungal treatment, and outcomes of candidemia in Candida albicans (C. albicans) and Candida non‐albicans (C. non‐albicans) in East Sussex Healthcare Trust (ESHT), England. Material and methodsThis retrospective and prospective study was performed during January 2006 to June 2017. ResultA total of 102 episodes of candidemia on 100 patients (55 males) were identified. C. non‐albicans were predominant (55%). All isolates were sensitive to amphotericin B, caspofungin and voriconazole while One C. albicans and 5 C. non‐albican isolates were resistant to fluconazole. The risk factors in C. albicans and C. non‐albicans groups were comparable which included intensive care unit (ICU) stay (15% vs 10%), the presence of intravascular line (35% vs 42%), previous antibiotic exposure (39 vs 49%), surgical intervention (19% vs 19%), mechanical ventilation (5% vs 8%), total parenteral nutrition (30% vs 27%) and urinary catheters (33 vs 38). The comorbidities in both groups (C. albicans and C. non‐albicans) were solid organ cancer (15&14), haematology malignancy (1&3), steroid use (14&13), diabetes (9&7), and chemotherapy (2&4). Main sources of candidemia in C. albicans were line (12), respiratory (10), and urinary tracts (6) while line (26) and urinary tract (9) were predominant in C. non‐albicans group. Only a small number of patients underwent echocardiography (30%) and ophthalmology reviews (20%). A total of 45 fatal cases were recorded (C. albicans 23). The highest mortality was seen in patients with C. albicans and among them the risk factors were elderly age group >65 years (17/23), surgical intervention (9/23) and the history of ICU stay (7/23). ConclusionC. albicans candidemia, >65 years of age and surgical procedure is associated with significant mortality, however the use of fluconazole has shown the increased survival rate. This study suggests the surveillance of candidemia, and antifungal susceptibility pattern in current practice and the treatment.

Abstract:OBJECTIVE To investigate the effect of recombinant human granulocyte colony-stimulating factor(rhG-CSF) combined with antifungal drugs in the treatment of malignant hematologial diseases with fungus infection.METHODS Malignant hematologial patients with fungus infection were randomized to receive fluconazole with or without rhG-CSF.(RESULTS) The response rate in patients who received fluconazole combined with rhG-CSF was 89.1% and in(control) patients was 62.8%(P<0.05).The absolute neutrophil count(ANC) in patients who(received)(fluconazole) combined with rhG-CSF recovered faster than that of without rhG-CSF.CONCLUSIONS rhG-CSF can accelerate the recovery of ANC,and enhance the curative effect when combined with antifungal drugs.

Abstract:Objective To analyze the effects of high-dose (> 5 μg/kg) and standard-dose (5 μg/kg) granulocyte colony-stimulating factor (G-CSF) on infection and blood transfusion in elderly patients with acute myeloid leukemia (AML) during chemotherapy. Methods During the 273 cycles of chemotherapy, 82 elderly patients with AML were treated with two doses of G-CSF. The use of antibiotics and antifungal agents, fever days, the degree of fever, neutropenia time and the blood transfusion were analyzed retrospectively. Results In 273 cycles of chemotherapy, there were 223(81.7%) times of concurrent infection, 11(13.4%) patients were discontinued due to the infection; In standard treatment group 2(2.3%) patients died during induction treatment. Subgroup analysis showed the high-dose group significantly shortened the neutropenia and fever time and decreased the use of antifungal agents. Conclusion High-dose G-CSF could improve the safety and tolerability of chemotherapy in elderly patients with AML.
Effectiveness analysis of fluconazole prophylaxis of fungus infection in patients with hematological diseases

Abstract:Objective To evaluate the effectiveness of fluconazole 200-400 mg by oral and intravenous administration for antifungal prophylaxis in patients with high-dose chemotherapy or autologous hematopoietic stem cell transplantation(HSCT).Methods From Ruijin hospital during 2008-2009,35 patients with hematological malignancy received 21 cycles high-dose chemotherapy or 21 cycles of autologous HSCT.All these patients received fluconazole 200-400 mg by oral and intravenous administration as antifungal prophylaxis throughout the neutropenia phase.The patients with neutropenic fever were received broad spectrum antibiotics and computed tomography(CT) scan,which was galactomannan(GM) antigen/1-3-beta-D glucosans(BDG) antigen assay monitoring to guide the antifungal treatment.Results After high-dose chemotherapy or autologous HSCT,neutropenia occurred in all patients with median nadir white blood cell(WBC) level at 0.1×109/L [(0.1-0.5)×109/L] for a median of 9 d(2-21 d).There was no significant difference between high-dose chemotherapy and autologous HSCT(P=0.36).6 cases(14.3%) were not found neutropenic fever.A total of 36 cases(85.7%) had neutropenic fever with a median of 4.5 d(1-13 d).The incidence and duration of neutropenia and neutropenic fever had no statistical significance in the high-dose chemotherapy and autologous HSCT(P=0.38).Though a total of 7 patients failed to respond to broad spectrum antibiotics with persistent fever over 7 d,but no one was in line with the diagnosis standard of invasive fungal infection(IFI).Only 3 cases(8.3% in neutropenic fever patients) received broad spectrum antifungal treatment.Conclusions In patients with hematological malignancy who receive cytarabine-based high-dose chemotherapy and autologous HSCT,fluconazole prophylaxis is effective.Unlike those patients who receive allogeneic HSCT,no broad spectrum antibiotics are necessary as prophylaxis due to the relative low risk of Aspergillus infection.

Abstract:Abstract Background: The standard (150mg/day) micafungin treatment regimen, is an effective and well tolerated empirical therapy for invasive fungal disease (IFD) among hematologic patients; however, experience with high dosage (200-300mg/day) micafungin treatment is limited. We retrospectively evaluated both safety and effectiveness of micafungin as an empirical antifungal therapy via standard and high dosage treatments for IFD.Methods: Patients were considered to be qualified for this analysis if they were proven, probable, or possible diagnosed as IFD. Patients without any previous exposure to an echinocandin were included; for those who changed treatment dosages and had previously received another systemic therapy except fluconazole for prevention were excluded. Safety and effectiveness were evaluated in all participated patients. IFD was diagnosed and classified according to standards defined by both European Organization for Research and Treatment of Cancer / Invasive Fungal Infections Cooperative Group, and the National Institute of Allergy and Infectious Diseases Mycoses Study Group Consensus Group.Results: Totally 72 hematologic patients participated this study. 45 patients were received a standard (150mg/day)micafungin treatment, and 27 patients were received a high dosage (200-300mg/day)micafungin treatment. Neutropenia status were similar between two groups (37.78% and 33.33%), so were other underlying risk factors. The mean duration of therapy were 15.27 days (Range: 7-47 days) in standard micafungin group, and 14.96 days (Range: 7-51 days) in high dosagemicafungin group. Drug related adverse events (AEs) occurred in 11.11% of patients received the standard regimen, and 14.81% of patients received the high dosage regimen (difference, 3.70%; 95% confidence interval [CI], -5.75% to 10.89%; statistically insignificant). The most common drug related AEs in the standard and high dosage treatment groups were liver dysfunction. Only 1 patient discontinued standard micafungin treatment, because liver function test value is 5 times greater than the upper limit of normal value. 86.67% of patients from standard treatment group, and 88.89% of patients from high dosage treatment group had favorable overall responses (difference, 2.22%; 95% CI, -4.94% to 8.64%; statistically insignificant).Conclusion: Both micafungin dosing treatments were effective, and well tolerated among hematologic patients with invasive fungal infection. Therefore, there hasn’t arisen any safety concern of high dosage(200-300mg/day) micafungin treatment.

Abstract:Objective To explore the clinical characteristics of the hematological malignancies which combined with invasive fungal infections(IFI).Methods Forty-six patients of IFI during Jan 1998 to Jan 2005 were enrolled in this investigation and clinical manifestation,fungal spectrum,treatment and outcome were observed retrospectively.Results Total of 642 cases of hematological malignancies were diagnosed and 46 cases combined with IFI(7.16%).Lung was the frequently fungal infection site(29 cases,63.1%).The hospitalized day longer than 14 days,chemotherapy approaches combined with glucocorticoids,broad-spectrum antibiotics using>7 days and agranulocytosis persistence>5 days,associated with general conditions such as diabetes mellitus were all the high risk factors for IFI in hematological malignancies.Clinical manifestations of IFI varied with different infection sites and different fungal species.The most common fungal subtypes were Candida albicans(representing 41.8%,13 strains)and aspergillus(representing 19.4%,6 strains).Despite amphotericin B and itraconozole adopted to treat IFI,but there are still 15 cases of patients died because of uncontrolled respiratory failure.Conclusion Because of various risk factors existence,IFI,which with a marked increasing mortality,was prone to occur combined with hematological malignancies.Early prophylaxis or empiric treatment could reduce the mortality of IFI,and made a better prognosis.
Candidemia combined with bacterial bloodstream infection: analysis of clinical features and associated risk factors

Abstract:Objective To investigate the clinical characteristics of and risk factors for candidemia combined with bacterial bloodstream infection (BSI) by retrospective analysis of cases.Method The clinical data of cases diagnosed as candidemia combined with BSI confirmed by blood culture were compared with those of cases with mono-candidemia in Beiing Tongren Hospital from January 2009 to December 2011.A logistic regression analysis was performed to investigate the independent risk factors.Results Forty-two cases diagnosed as candidemia were analyzed including 14 cases of candidemia combined with BSI and 28 cases of mono-candidemia.Ten strains of gram-positive cocci and 4 strains of gram-negative bacilli were isolated from candidemia combined with BSI group.Six strains of C.albicans,4 strains of C.glabrata,3 strains of C.tropicalis and 1 strain of C.krosei were isolated.There was no C.parapsilosis isolated from candidemia combined with BSI group but 9 strains in the mono-candidemia group.The septic shock rate of the candidemia combined with BSI group was higher than that of the mono-candidemia group (12/14 vs 7/28,P =0.000).The mortality rate of the candidemia combined with BSI group was higher than that of the mono-candidemia group (10/14 vs 15/28),but the difference did not reach statistical significance (P =0.266).Four factors were found statistically different by univariate analysis,including hospitalization more than 4 weeks (P =0.001),bacteremia before candidemia(P =0.005),hematological tumor (P =0.01) and abdominal infection (P =0.001).Multivariate analysis showed that hospitalization more than 4 weeks was the independent risk factor.Conclusion Gram-positive cocci were the predominant species and septic shock was more common in candidemia combined with BSI.Hospitalization more than 4 weeks was the independent risk factor for candidemia combined with BSI.

Abstract:BackgroundCancer patients are at risk for candidemia. Resistance and breakthrough Candida represent an emerging threat. The study aims to analyze the clinical features, resistance patterns and breakthrough candidemia among pediatric cancer patients.MethodsThe retrospective study was conducted from 2014 - 2020 in children admitted to Children Cancer Hospital, Egypt. Clinical, microbiological, and outcomes were analyzed. Mortality was estimated within 30 days from Candida spp. Isolation from blood.ResultsThree hundred fifty (350) pediatric cancer patients were diagnosed with candidemia. The median age was four years (4 months- 18 years). Most of the patients had hematological malignancies (54%) while solid tumors represent (46%). Clinical features of the patients included neutropenia (80%), the presence of central venous line (57%), ICU admission (44%), and steroid use (43%). Non-Albicans candida was predominant (71%) compared to Candida Albicans (29%). The isolated species were: C. Albicans (29%), C.Tropicalis (29%), C. parapsilosis (21%), C. krusei (7%), C.glabrata(3%), C. guilliermondii (1.5%)and C. famata (2%). Chronic disseminated candidiasis was reported in 30 (9%) patients, with the liver being the most affected organ. Breakthrough candidemia on top of echinocandin prophylaxis was reported among 62(18%) patients with C. parapsilosis is the most common 31(50%) isolated pathogen. From 96 patients with breakthrough candidemia, all-cause mortality was reported in 32 patient (33%) while candida attributable cause of death in elven (11%) patients. Antifungal resistance was reported in 74(21%) patients: fluconazole (15%), liposomal ampho-B (6%), echinocandin (5%) and voriconazole (3%). Among 74 patients with resistant candidemia, death was reported in 24%. Echinocandin Resistant candida was a direct cause of mortality in 3(17%) patients.The overall day- 30 mortality rate was 24% (85/350 patients), while candida attributable of mortality was (10%). Patients with septic shock, ICU admission, and resistant candida had a higher mortality risk.ConclusionEchinocandin-resistant and breakthrough Candida are significant threat with a high mortality rate among pediatric cancer patients. Abstract:Objective:To study the risk factors and treatment of IFI with hematological malignancy.Methods:Retrospectively analyzed clinical data of 453 patients with hematological malignancies,counted IFI incidence,analyzed risk factors for IFI;IFI paitients were randomly divided into A, B group, given fluconazole、itraconazole for treatment.Results:Logistic regression analysis showed that agranulocytosis,deep venous catheter,mucosal injury,use of immunosuppressive agents were the risk factors of IFI (P<0.05);total efficiency and mycological of B group were higher than that in A group (P<0.05).Conclusion:For the low immunity,agranulocytosis,muco sal injury hematological malignancies should increase attention,and actively prevent fungal infections;itraconazole can effectively remove fungi,improve treatment success rates.

Abstract:Background: This study was performed to assess the incidence of and risk factors for Candida infection in the first 100 days after allogeneic hematopoietic stem cell transplantation (HSCT) and the impact on long-term survival.Methods: We performed an outcome analysis of 28542 acute leukemia patients who underwent HSCT from 2000 to 2012. There were 347 patients with candidemia by day 100 and 28195 without candidemia or any other type of Candida infection.Results: The incidence of candidemia by day 100 was 1.2% and occurred at a median of 22 days after HSCT. Higher 100-day nonrelapse mortality (NRM; hazards ratio [HR], 3.0, P < .0001) and lower 100-day overall survival (OS; HR, 2.5, P < .0001) were observed in patients with candidemia. The case fatality rate by day 100 in patients with candidemia was 22% (76/347). Factors associated with candidemia occurrence were female gender, bone marrow or cord blood stem cell source, T-cell depletion, use of total body irradiation, and acute graft vs host disease. Among the patients alive at day 100, the 5-year NRM and OS after a median follow-up of 5.6 years (95% confidence interval, 5.5 - 5.7) for patients with and without candidemia were 22.5% vs 13.5%, P < .0001 and 45.6% vs. 53.4%, P = .0003, respectively. In multivariate analysis, the occurrence of a candidemia episode by day 100 was an independent risk factor for higher NRM (HR, 1.7, P = .001) and lower OS (HR, 1.4, P = .001).Conclusions: The early occurrence of candidemia after HSCT is still associated with higher NRM and lower short- and-long-term OS.

Abstract:The study of candidemia in Chinese leukemia patients has been limited. This retrospective study aims to investigate the characteristics and prognostic factors of candidemia among leukemia patients in a Chinese chemotherapy center. From 2009 to 2015, 30 isolates of candidemia were detected in 19 patients with acute leukemia after chemotherapy. The overall incidence of candidemia was 2.12 episode per 1000 admissions. Candida tropicalis was the most common Candida species (n = 17; 89.5%), followed by Candida albicans (n = 2; 10.5%). The most common underlying disease was acute myeloid leukemia (94.7%) and induction chemotherapy phase was the most susceptible stage. The vast majority of candidal infections are endogenous rather than central venous catheter-related. The overall 30-day crude mortality rate was 31.6%. Neutrophil recovery (P = 0.000) and initiation of empiric antifungal treatment before first positive blood culture (P = 0.041) were associated with a significant improvement in overall survival. Although the incidence of candidaemia appears to be quite low in patients with leukemia submitted to intensive chemotherapy, its high mortality rate continues to be a crucial problem despite the availability of new effective antifungal drugs. Early diagnosis followed by rapid antifungal treatment remains the cornerstone of successful management. Catheter removal should be considered on an individual basis. The widespread use of newer antifungal agents as prophylaxis among patients with acute leukemia may result in a decreased candidemia incidence.
