Abstract:Objective: To elucidate the 18F-fluorodeoxyglucose (FDG) PET/CT characteristics and its prognostic value in the patients with anti-melanoma differentiation associated protein 5 antibody positive (anti-MDA5+) dermatomyositis (DM). Methods: This retrospective cross-sectional study included 26 patients with anti-MDA5+ DM and 43 patients with anti-MDA5 negative (anti-MDA5–) idiopathic inflammatory myopathy (IIM) who were examined by 18F-FDG PET/CT from January 1, 2017 to December 31, 2020. The maximum standardized uptake value (SUVmax) of multiple organs and other clinical characteristics of the patients were measured and analyzed. Results: Compared with the anti-MDA5– group, the patients in the anti-MDA5+ group showed higher bilateral lung SUVmax (p = 0.029), higher SUVmax of spleen (p = 0.011), and bone marrow (p = 0.048). Significant correlations between the spleen SUVmax and serum ferritin levels (r = 0.398, p < 0.001), erythrocyte sedimentation rate (ESR) (r = 0.274, p = 0.023), platelet count (r = −0.265, p= 0.028), myositis disease activity assessment score (r = 0.332, p = 0.005), bone marrow SUVmax (r = 0.564, p < 0.001), and bilateral lung SUVmax (r = 0.393, p < 0.001) were observed. Conclusion: 18F-FDG PET/CT was found valuable in quantifying the pulmonary focal inflammation and potentially unveil the distinctive characteristics and pathophysiological mechanisms in the patients with anti-MDA5+ DM.

Abstract:Adult-onset Still disease (AOSD) is a rare autoinflammatory condition. The presence of an evanescent, salmon-pink, nonpruritic rash is one of the major diagnostic criteria for the disease. The rash occurs with fever and subsides with defervescence. The presence of dyskeratotic keratinocytes in the upper one-third layer of the epidermis is a distinctive histopathological feature of persistent pruritic lesions associated with AOSD. Here, we report 2 cases of AOSD characterized by persistent pruritic lesions resembling those observed in dermatomyositis. Identifying the clinical and histopathological manifestation of the cutaneous lesions is essential for the early diagnosis of AOSD and for differentiating this condition from those presenting with dyskeratotic cells in the epidermis.

Abstract:Two of the happiest days of my life have been ruined by inappropriate footwear. The first time was on my honeymoon. Not in bed, I hasten to add. We Northerners admit to being sewn into our woolly vests from November to March, come what may, but when it comes to amorous dalliance we always remove our boots, and often our socks, weather permitting. The honeymoon in question was spent in the Austrian Alps, and it was on an alp that the footwear problem first arose. In those days I was a keen rock-climber and on my last holiday as a bachelor I had triumphantly led Tower Ridge on Ben Nevis. Actually I should have been second on that occasion, but my male climbing partner had what doctors call a temporary disturbance of the bowel habit as a result of over-indulgence in the Glencoe rough cider on the previous evening, and it seemed decidedly less risky to lead, even if I did have to climb above my usual standard. It scarcely seemed necessary therefore to take my climbing boots for a gentle wander around an alp or two, especially as I was anticipating frequent pauses for refreshment, flower-picking, photography, amorous dal-liance etc. in no particular order (my wife's enthusiasm for 'proper' climbing had never been marked, partly because she seems to be congenitally incapable of bringing her foot up to the level of her ear, and her enthusiasm had dwindled to zero as soon as the engagement ring was safely in place). Thus it was that on about the fourth day of our honeymoon , we thought we'd try another sport, and decided, boots or no boots, to conquer a peak or so. Abstract:This case report highlights dermatomyositis (DM) characterised by the concurrent presence of anti-melanoma differentiation-associated protein 5 (anti-MDA5) and anti-Ro52 antibodies. A 64-year-old woman initially presented with erythema on the palms, which later spread to the dorsum of the hands, followed by involvement of the face, forehead, and upper eyelids. The patient reported joint pain, fatigue, and dyspnea. Physical examination revealed characteristic cutaneous manifestations, including heliotrope rash and Gottron's sign, accompanied by skin ulceration and muscle weakness. Blood tests showed elevated levels of creatine phosphokinase and C-reactive protein. A high-resolution computed tomography (HRCT) scan revealed interstitial lung disease (ILD) with an organising pneumonia (OP) pattern. Magnetic resonance imaging (MRI) confirmed the presence of myositis. Autoantibody analysis revealed concurrent positivity for both anti-MDA5 and anti-Ro52 antibodies. At the time of diagnosis, she had no respiratory impairment, but had an elevated C-reactive protein and high levels of anti-MDA5 antibody. She was started on triple combination therapy with glucocorticoids, cyclophosphamide, and tacrolimus. She had worsening oxygenation and elevated ferritin during the first weeks of treatment, but then her symptoms improved. Early detection of a co-positive anti-Ro52 antibody led to early initiation of triple combination therapy and a good prognosis.
MDA5 Expression is Associated with TGF-β-induced Fibrosis: Potential Mechanism of Interstitial Lung Disease in Anti-Mda5 Dermatomyositis.

Abstract:Objectives This study aimed to investigate the high-resolution CT (HRCT) characteristics of anti-melanoma differentiation-associated gene 5 (MDA5) antibody positive dermatomyositis-associated interstitial lung disease (anti-MDA5 DM-ILD), and to clarify the underlying mechanisms of the clinical phenomenon. Methods Clinical data and HRCT patterns were compared between anti-MDA5 DM-ILD (n = 32) and antisynthetase syndrome-associated ILD (ASS-ILD) (n = 29). RNA sequencing of whole-blood samples from the two groups, and in vitro experiments using human embryonic lung fibroblasts (HELFs) were conducted to explore the potential mechanisms of the clinical findings. Results The anti-MDA5 DM-ILD subset had a significantly higher incidence of rapidly progressive ILD (RPILD) than ASS-ILD (65.6% vs 37.9%; P = 0.031). The relative percentage of the lung fibrosis HRCT pattern was significantly lower in the anti-MDA5 DM-ILD group, especially the RPILD subgroup (P = 0.013 and 0.003, respectively). RNA sequencing detected the upregulated genes including interferon-induced helicase C domain 1 (encoding MDA5), and a trend towards downregulated expression of TGF-beta signalling components in anti-MDA5 DM-ILD. In vitro culture of HELFs revealed that upregulated expression of MDA5 in HELFs was correlated with the downregulated expression of alpha smooth muscle actin, connective tissue growth factor, collagen I and collagen III by suppressing the TGF-beta signalling pathway. Conclusions Anti-MDA5 DM-ILD patients have significantly less lung fibrosis and elevated MDA5 expression. The upregulated expression of MDA5 has relations with the suppression of the pro-fibrotic function of fibroblasts via the TGF-beta signalling pathway, which may partially explain the mechanism of the clinical phenomenon.

Abstract:Introduction: Anti-SAE1 antibodies have a low prevalence in dermatomyositis patients. Case Description. A 61-year-old woman presented with progressive shortness of breath, arthralgia, heliotrope rash, Gottron's papules, and erythematous rash. She had an interstitial lung disease (ILD) with a significant decrease in lung function. There was no muscle involvement. Immunological laboratory test results showed strongly positive anti-SAE1 antibodies. Glucocorticoid treatment resulted in remission of dermatomyositis. Conclusion: Anti-SAE antibodies in dermatomyositis patients are closely linked to unique clinical features.

Abstract:Panniculitis, a rare cutaneous manifestation in patients with dermatomyositis (DM), usually presents as a painful erythematous lesion. We herein report a 32-year-old woman with panniculitis that appeared as an indurated plaque without pain or redness after a 4-month episode of clinically amyopathic DM during treatment with prednisolone and tacrolimus. She experienced no pain; however, the firmness and extent gradually worsened. Based on our findings, including the histopathological results, DM panniculitis was diagnosed. Azathioprine was additionally administered, leading to remission. DM panniculitis can develop as a painless induration during immunosuppressive treatment, and azathioprine may be a useful treatment.

Abstract:Background: Dermatomyositis with positive anti-melanoma differentiation-associated gene 5 (anti-MDA5) antibody has a distinct phenotype associated with small hand joint arthritis, mucocutaneous ulceration, palmar papules and less muscle involvement. It is also associated with increased risk of rapidly progressive interstitial lung disease (RP-ILD) and has a high mortality rate in adults. There is evidence that cases complicated with spontaneous pneumomediastinum (PNM) have an increase in mortality. While most of the evidence for this rare disease is derived from the adult literature, we report a case diagnosed in an adolescent complicated with both RP-ILD and PNM with a good outcome after aggressive immunosuppressive therapy. Our case also illustrates the potential challenges in diagnosis of this condition in the setting of non-specific clinical manifestations, the need for a high index of suspicion, and the importance of testing for myositis-specific antibodies (MSA) early to aid in diagnosis given the risk of rapid progression in these patients. Case presentation: A 16-year-old Chinese female presented with fever and cough for 1 day, and finger swelling for 3 weeks. Physical examination revealed arthritis of fingers and wrists, ulcers and palmar papules over fingers, hyperpigmentation of interphalangeal joints, and rash over the neck. The diagnosis of dermatomyositis was made 1 month later with the onset of malar rash, Gottron's papules, calcinosis and myalgia. The diagnosis was supported by the presence of anti-MDA5 antibody and evidence of inflammatory myopathy on magnetic resonance imaging. In retrospect, she already had interstitial lung disease at first presentation manifested as cough and opacity on chest radiograph, which was later confirmed with chest computed tomography. She was treated according to adult guidelines with steroid and calcineurin inhibitor. Her disease was resistant to initial therapy and was complicated by RP-ILD and spontaneous PNM. Intensive immunosuppressive therapy including cyclophosphamide and rituximab were required to induce remission. Conclusions: Recognition of distinct clinical features of anti-MDA5 antibody-positive dermatomyositis and testing for MSA is crucial in patients with skin ulceration and abnormal pulmonary findings of unknown etiology, as prompt diagnosis with early aggressive treatment and anticipation of complications could make a difference in the outcome of this disease with high mortality.

Abstract:Idiopathic inflammatory myopathies (IIM) are a heterogeneous group of autoimmune disorders classically characterized by proximal skeletal muscle inflammation leading to weakness, but they often possess additional systemic manifestations such as cutaneous, pulmonary, and articular disease.1 Although originally dichotomized as either dermatomyositis (DM) or polymyositis, the discovery of new myositis-specific antibodies (MSA) and myositis-associated antibodies has led to the delineation of more refined IIM patient subgroups. The antimelanoma differentiation-associated gene 5 (MDA5) MSA is an example, found to define a clinically distinct DM subgroup characterized by hypomyopathic or amyopathic muscle disease, distinct cutaneous features, and increased risk for rapidly progressive interstitial lung disease (RP-ILD).1,2 Many of the cutaneous manifestations of anti-MDA5 antibody–positive DM (anti-MDA5-DM) are unique or atypical when compared to other IIM subtypes. This can have implications for diagnosis and treatment delays. Atypical features include cutaneous ulceration (with a predilection for the nailfolds and overlying Gottron papules/sign), palmar papules (occurring on the palms and palmar aspect of the fingers, particularly within the joint creases), and mechanic hands (fissured, hyperkeratotic plaques along the thenar, radial, and ulnar aspects of the fingers, classically associated with the antisynthetase syndrome), among other more classic DM cutaneous findings.2 The unique clinical features of anti-MDA5-DM and its implications with regard to prognosis, given the association with highly morbid RP-ILD, further stresses the necessity for increased clinician awareness of this rare clinical syndrome so that earlier diagnosis and treatment can be initiated. One such strategy to increase overall clinical awareness of rare disorders is through patient support groups and organizations. Abstract:A 53-year-old female developed erythema spreading from her right eyelid to her face over 3 months despite topical steroids. Ten days before admission, she visited the dermatology department of a general hospital, where she exhibited Gottron's sign, the inverse Gottron's sign, the shawl sign, and the V sign. She reported no muscle weakness, and CK levels were normal, leading to a strong suspicion of clinical amyopathic dermatomyositis (CADM). Computed tomography of the chest revealed interstitial lung disease (ILD), prompting her emergency admission to our department of rheumatology.

Abstract:A 44-year-old woman with autism spectrum disorder developed bulbar symptoms and generalized muscle weakness 7 months before referral. Six months before, she was administered glucocorticoid for liver involvement. During the course, while she presented alopecia, skin ulcers, and poikiloderma, hyperCKemia was observed only twice. Due to complications including cardiac involvement and hearing loss as well, we suspected mitochondrial disease and performed a muscle biopsy. The muscle pathology showed sarcoplasmic myxovirus resistance A (MxA) expression with scattered pattern. Since anti-melanoma differentiation-associated gene 5 (MDA5) antibody was detected, we diagnosed the patient with anti-MDA5 antibody-positive dermatomyositis (DM). We reinforced immunosuppressive therapy, and her clinical symptoms and liver involvement were improved. When we diagnose a case of anti-MDA5 antibody-positive DM who is difficult to make clinical diagnosis, it may be valuable to evaluate sarcoplasmic MxA expression on muscle pathology.

Abstract:Abstract Objectives: Anti-melanoma differentiation associated gene 5 antibody (anti-MDA5 Ab)-positive dermatomyositis (DM) is representative of rapidly progressive interstitial pneumonia. However, its association with thrombotic microangiopathy (TMA), characterized by thrombocytopenia, hemolytic anemia, and organ dysfunction, has not been defined. This study aimed to elucidate the characteristics of anti-MDA5 Ab-positive DM accompanied by TMA. Methods: We reviewed our hospital records from November 2009 to September 2022. We included patients in accordance with the 2017 European League Against Rheumatism/American College of Rheumatology classification criteria and with the criteria of Bohan and Peter. TMA was diagnosed according to the criteria for transplantation-associated TMA proposed by the International Working Group. Results: This study enrolled a total of 26 anti-MDA5 Ab-positive DM patients, four of whom developed TMA. The patients with TMA had an increased urine protein/creatinine ratio (UPCR). In addition, these four patients showed significantly elevated levels of ferritin and anti-MDA5 Ab titers and were considered to have high disease activity; yet, all of them survived. Conclusions: Out study indicated that anti-MDA5 Ab-positive DM patients with hyperferritinemia, a high anti-MDA5 Ab titer, and an increased UPCR should be carefully managed, bearing in mind a complication of TMA.

Abstract:BackgroundAnti-melanoma differentiation-associated gene 5 (MDA5) antibody has been recognized to be significantly associated with a subset of dermatomyositis patients with rapidly progressive interstitial lung disease (RP-ILD).ObjectivesTo elucidate the clinical characteristics and poor prognostic factors in Japanese dermatomyositis patients with anti-MDA5 antibody.Materials and methodsClinical features of anti-MDA5 antibody-positive dermatomyositis patients and risk factors, potentially associated with a poor prognosis, were retrospectively analysed.ResultsA total of 37.3% (28/75) dermatomyositis patients were positive for anti-MDA5 antibody. The frequency of Gottron’s papules, palmar violaceous macules, antihelix/helix violaceous macules, and clinically amyopathic dermatomyositis (CADM) was significantly higher in patients with anti-MDA5 antibody. Of anti-MDA5 antibody-positive dermatomyositis patients, 57.1% developed RP-ILD, and in those with RP-ILD, serum ferritin level was markedly high and partial pressure of arterial oxygen (PaO2) was significantly low at first visit. Among patients with anti-MDA5 antibody with RP-ILD, non-survivors were older and revealed lower PaO2 at first visit relative to survivors. Furthermore, patients who did not take triple therapy (prednisolone, calcineurin inhibitor and cyclophosphamide) as initial treatment resulted in poor outcome.ConclusionAnti-MDA5 antibody may be associated with CADM, the progression to RP-ILD, high serum ferritin level, and characteristic skin manifestations. High serum ferritin level in patients with anti-MDA5 antibody may be associated with the development of RP-ILD and poor prognosis. Early treatment with triple therapy, including intravenous cyclophosphamide, may improve the prognosis of RP-ILD.

Abstract:Abstract Idiopathic inflammatory myopathies (IIM) are a group of highly heterogeneous systemic autoimmune diseases, of which clinically amyopathic dermatomyositis (CADM) is a distinct sub‐type. Pulmonary hypertension (PH) is a life‐threatening medical condition that can occur as a complication of connective tissue diseases. Herein, our report first suggests that PH can develop in CADM. A 48‐year‐old woman came to our hospital due to a 3 months history of facial edema and shortness of breath. Relevant examinations revealed Gottron's sign, normal creatinine kinase levels, elevated levels of mean pulmonary artery pressure, double‐positive anti‐MDA5 and anti‐Ro52 antibodies, and typical pathological changes associated with myositis. The diagnosis of CADM combined with PH was considered. The patient responded well to the immunosuppression therapy and PH‐related drug therapy. We provide further insights that patients with IIM need to undergo regular assessment of PH.

Abstract:Background Interstitial lung disease (ILD) is a common complication of polymyositis/dermatomyositis (PM/DM). It is often lethal if it occurs as a rapidly progressive pattern especially in patients present as clinically amyopathic dermatomyositis (CADM), a unique subtype of DM frequently seen in East Asia populations. Anti-MDA5 antibody (also known as anti-CADM140 antibody) has been associated with this severe clinical entity in Japanese population [1], while the clinical utility of this autoantibody remains unclear in Chinese patients. Objectives The purpose of this study was to investigate the diagnostic and prognostic significance of anti-MDA5 antibody in Chinese PM/DM patients. Methods A total of 64 PM/DM patients (including 9 CADM patients, 34 classic DM patients, and 21 polymyositis patients), 16 idiopathic pulmonary fibrosis (IPF) patients, and 10 normal controls were recruited from the affiliated Drum Tower Hospital, Nanjing University Medical School, China. The diagnosis was based on the Bohan and Peter criteria for PM/DM [2]. Information on all hospitalizations was obtained from electronic medical records. Serum anti-MDA5 antibody titer was measured using an ELISA method described previously [1]. The clinical features, laboratory findings, and prognosis were compared. Results A total of 9 (100%) CADM patients, 17 (50%) classic DM patients and 1 (6.3%) IPF patient were identified as anti-MDA5 antibody positive. Anti-MDA5 antibody was more prevalence in PM/DM with ILD patients than in PM/DM without ILD patients (54.2% vs 0%, P P P P =0.0008), but lower levels for creatine phosphokinase (CK) and CK-MB ( P =0.003 and P =0.008, respectively), lower positive rate for anti-nuclear antibody (37.5% vs 80%, P =0.002) in patients with anti-MDA5 antibody. 7 of 26 patients positive for anti-MDA5 antibody, but no patient negative for anti-MDA5 antibody, died from respiratory failure within 12 months after the disease onset. The mortality rate for patients with anti-MDA5 antibody was significantly higher than that for patients without anti-MDA5 antibody ( P =0.026). Especially, anti-MDA5 antibody was associated with DM complicated ILD but not with other PM/DM patients in Chinese, with a sensitivity of 72.2% (26/36) and a specificity of 100% (28/28). Abstract:ObjectiveTo investigate the impact of sex differences on the clinical characteristics and prognosis of patients with anti-melanoma differentiation-associated gene 5-positive dermatomyositis (MDA5+ DM).MethodsWe retrospectively analyzed a cohort of 251 patients with MDA5+ DM, including 71 in the male group and 180 in the female group. A multivariate logistic regression model was built to analyze independent risk factors for RPILD in each group. An ROC curve was drawn to evaluate the predictive value of independent risk factors. Kaplan‒Meier analysis was used to compare the cumulative survival rates, while the log-rank test was used to test for significant differences between the two groups.ResultsPatients in the male group had a significantly higher prevalence of heliotrope rash, V sign, severe interstitial lung disease (ILD), and rapidly progressive interstitial lung disease (RPILD) than those in the female group. Anti-Ro52 positivity, high CRP level and short disease were identified as independent risk factors for RPILD in both male and female groups by multivariate logistic regression analysis. The mortality rates of males and females were 33.8% and 22.0%, respectively, and the survival time of patients in the male group was shorter than that in the female group.ConclusionMale patients with MDA5+ DM exhibit an increased risk of RPILD, elevated mortality rates and reduced overall survival time compared to their female counterparts, and anti-Ro52 positivity may be an unfavorable prognostic factor for these patients.Key Points• The prevalence of solar rash, V sign, severe interstitial lung disease (ILD) and rapidly progressive interstitial lung disease (RPILD) in anti-MDA5-positive female patients was significantly lower than that in male patients.• Positive Anti-Ro52, high CRP level, and short course of disease were independent risk factors for RPILD in both men and women.• Female patients exhibited a lower mortality rate than male patients (22.0% vs 33.8%) and demonstrated longer survival time.
